The Impact of Nobiletin on the Inflammatory Injury in Rats with Periodontitis by Regulating the CCL2-CCR2 Signaling Axis

Objective:
To investigate the effects of Nobiletin on inflammatory injury in periodontal disease rats and its mechanism by constructing a model of periodontitis rats.
Methods:
The rat model of periodontal disease was established by silk thread ligation and 10% sucrose drinking water; periodontal disease rats were separated into a model group, a low-dose Nobiletin group, a medium dose Nobiletin group, and a high-dose Nobiletin group, with 10 rats in each group. Another 10 rats were selected as the control group. The low, medium and high dose Nobiletin groups were administrated with 25mg/kg, 50mg/kg and 100 mg/kg, respectively, while the control group and model group were orally administrated with the same amount of normal saline. HE staining was performed to detect pathological changes in the periodontal tissue of rats in each group; qPCR was applied to measure the levels of C-C chemokine ligand 2 (CCL2), C-C chemokine receptor 2 (CCR2), tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and IL-6 mRNA in periodontal tissues of rats in each group. Immunohistochemical staining was applied to detect the expression of CCL2 in the periodontal tissue of rats in each group; Western blot was applied to detect the expression of CCL2, CCR2, TNF-α, IL-1β, and IL-6 proteins in the periodontal tissue of rats in each group.
Results:
Four weeks after the establishment of the periodontal disease rat model, compared with the control group, the model rats showed alveolar bone resorption, tooth root loosening and bleeding, indicated that the establishment of the periodontal disease rat model was successful; compared with the control group, the periodontal tissue of rats in the model group showed alveolar bone resorption, with a large number of inflammatory cell infiltration, the number of CCL2 positive cells increased, the mRNA and protein levels of CCL2, CCR2, TNF-α, IL-1β, IL-6 in periodontal tissue were obviously increased (P<0.05); compared with the model group, the inflammatory cell infiltration in the periodontal tissue of rats in the low, middle and high dose Nobiletin groups gradually reduced, the number of fibroblasts gradually increased, the number of CCL2 positive cells decreased, the levels of CCL2, CCR2, TNF-α, IL-1β, and IL-6 in periodontal tissue were obviously reduced, were dose-dependent (P<0.05).
Conclusion:
Nobiletin may alleviate the inflammatory injury of periodontal disease rats by inhibiting the CCL2-CCR2 signal axis.

Mechanistic of Gas6/MerTK Pathway Induces Mitochondrial Autophagy in the Progression of Lung Cancer

Objective:
To investigate the possible mechanism of mitochondrial autophagy in the progression of lung cancer by downregulating the Gas6/MerTK signaling pathway.
Methods:
Calu-1 and A-427 cell lines were cultured and divided into (1) control group, Gas6 intervention (downregulation of Gas6) group, (2) control group, rapamycin (autophagy inducer, RAPA) group. Western blotting was used to detect the protein expression levels of Gas6, MerTK, LC3Ⅱ, and Beclin1. Transmission electron microscopy was employed to observe autophagic vesicles in cells. Colony formation assay was conducted to assess the cloning ability of Calu-1 and A-427 cells. CCK-8 assay was performed to measure cell viability. Scratch assay was used to evaluate the migration ability of Calu-1 and A-427 cells. Immunofluorescence was utilized to detect the expression levels of LC3 in Calu-1 and A-427 cells.
Results:
Compared with the control group, the Gas6 intervention group showed a significant decrease in Gas6 and MerTK protein levels in Calu-1 and A-427 cells (P<0.05), a significant increase in LC3Ⅱ and Beclin1 protein levels (P<0.05), and the formation of autophagic vesicles with abundant autophagic vacuole accumulation. The cloning ability, cell viability, and migration distance of Calu-1 and A-427 cells were significantly reduced (P<0.05). Compared with the control group, the RAPA group exhibited an increase in LC3 protein expression in Calu-1 and A-427 cells (P<0.05). The cloning ability, cell viability, and migration distance of Calu-1 and A-427 cells were significantly reduced (P<0.05).
Conclusion:
Downregulating the Gas6/MerTK signaling pathway can induce mitochondrial autophagy in Calu-1 and A-427 cells and inhibit their proliferation, activity, and migration abilities.

Evaluation and Correlation Analysis of the Therapeutic Value of miR-190 and ANXA11 in Neoadjuvant Chemoradiotherapy for Rectal Cancer

Objective:
To analyze the roles and relationship of microRNA 190 (miR-190) and membrane-associated protein A11 (ANXA11) in the evaluation of neoadjuvant radiotherapy (NAC) for rectal cancer.
Methods:
A total of 148 patients who were diagnosed with rectal cancer and received treatment in our hospital from June 2019 through August 2022 were selected as the study subjects and included in the rectal cancer group, and 150 healthy people who underwent general physical examination in our hospital during the same period were selected as the control group, and real-time fluorescence quantitative detection (RT-PCR) detected the expression of serum miR-190 and ANXA11 mRNA in the two groups. All rectal cancer patients were treated with the same neoadjuvant radiotherapy and chemotherapy (NAC) before surgery, and were divided into the excellent efficacy group (n=89) and the poor efficacy group (n=59) according to the efficacy assessment criteria, and the serum miR-190 and ANXA11 mRNA expression of the patients in the two groups were compared. Spearman's correlation coefficient was used to analyze the relationship between serum miR-190, ANXA11 mRNA expression and clinical efficacy; Kaplan-meier was plotted to observe the effect of high and low miR-190, ANXA11 mRNA expression on the clinical efficacy of NAC; Subject operating characteristic curves (ROCs) were plotted to analyze the value of miR-190, ANXA11 single and combined assays for clinical utility assessment of NAC.
Results:
Compared with the control group, miR-190 expression was significantly lower, while ANXA11 mRNA expression was significantly higher in rectal cancer patients (0.87±0.18 vs 1.53±0.30, 2.00±0.68 vs 0.95±0.09, t=-22.719, 18.731, P<0.05). Compared with the poor-efficacy group, miR-190 expression was significantly higher in patients in the excellent-efficacy group, while ANXA11 mRNA expression was significantly lower (0.96±0.15vs0.73±0.15, 1.81±0.47vs2.28±0.83, t=8.932, -3.958, all P<0.05); Spearman correlation coefficient analysis showed that miR-190 was positively correlated with NAC efficacy, while ANXA11 mRNA expression was negatively correlated with NAC efficacy (P<0.05); Kaplan-meier curves showed that the NAC efficacy excellence rate of the patients in the miR-190 high-expression group was significantly higher than that of the patients in the miR-190 low-expression group (χ

=8.772, P=0.003); the ROC showed that the area under the curve (AUC) of the combined assay of miR-190 and ANXA11 was significantly higher than that of the single assay (P<0.05), and the sensitivity was 84.75%, the specificity of 87.64%.
Conclusion:
miR-190 and ANXA11 have specific expression levels in patients with different NAC efficacy, and there is a close association between them and NAC efficacy. miR-190 and ANXA11 detection can effectively predict the clinical utility of NAC by combining miR-190 and ANXA11 assays, which can provide a new idea for the clinical assessment of rectal cancer efficacy.

Screening Study on Serum Exosomal LncRNA as Early Diagnostic Molecular Markers for Ankylosing Spondylitis

Objective:
To screen serum exosomal long non-coding RNA (LncRNA) as potential molecular markers for the early diagnosis of ankylosing spondylitis (AS).
Methods:
Whole blood samples of AS patients in our hospital from January 2021 to December 2022 were collected. Exosomes were isolated from the plasma of AS patients (n=50) and healthy individuals (n=50) by ultrafast centrifuge. Total exosomal RNA was extracted using TRIzol reagent. LncRNAs were analyzed by microarray analysis in the two groups of patients, and the expression levels of LncRNAs in all patients and healthy subjects were measured by real-time quantitative reverse transcription-polymerase chain reaction (qRT-PCR).
Results:
The results of this study showed that six lncrnas (LNCV6_116109、LNCV6_98390、LNCV6_38772、LNCV6_108266、LNCV6_84003和LNCV6_98602) were significantly up-regulated in AS patients by qRT-PCR. Not a healthy individual. Receiver operating characteristic curve (ROC) was used to verify its diagnostic accuracy. The area values under the curve of lncRNA were 0.8052(LNCV6_116109)、0.7088(LNCV6_98390)、0.7460(LNCV6_38772)、0.7292(LNCV6_108266)、0.7356(LNCV6_84003)and 0.6800(LNCV6_98602.
Conclusion:
This study showed that the expression of exosome LncRNAs (LNCV6_84003, LNCV6_98602, LNCV6_108266, LNCV6_116109, LNCV6_98390 and LNCV6_38772) was significantly up-regulated in plasma of patients with AS. They may serve AS potential non-invasive biomarkers for early diagnosis of AS.

Clinical Significance of Differential Expression of Serum Peripheral miRNAs in Spinal Tuberculosis

Objective:
To explore the clinical significance of differential expression of serum peripheral micrornas(miRNAs)in spinal tuberculosis.
Methods:
From May 2020 to May 2023, 50 patients with spinal tuberculosis (experimental group) and 30 healthy people (control group) admitted to the Department of Spinal Orthopaedics of our hospital were taken as the research objects. MiRCURYTM LNA array (v.18.0) was used to screen the expression profile of miRNAs in 10 experimental patients and 10 healthy controls. The expression levels of 10 selected miRNAs were verified by RT-qCPR in the research cohort. Hsa-miR-4708-3p, hsa-miR-5100 and hsa-miR-493-3p were selected to construct the spinal tuberculosis diagnosis model (STB model) based on miRNAs. Diagnostic value of STB model in spinal tuberculosis by analyzing receiver operating characteristic (ROC) curve.
Results:
Compared with healthy people, 288 kinds of miRNAs were found in the serum of patients with spinal tuberculosis(123 kinds were up-regulated and 165 kinds were down-regulated) (multiple change>2, P<0.05). RT-qPCR analysis confirmed that compared with the control group, the levels of hsa-miR-4708-3p and hsa-miR-5100 in serum samples of the experimental group increased significantly(P<0.05), and the level of hsa-miR-493-3p decreased significantly(P<0.05). Based on the expression levels of hsa-miR-4708-3p, hsa-miR-5100 and hsa-miR-493-3p as variables, a multivariate logistic regression model was established as follows: 0.582+1.055×(hsa-miR-4708-3p)+0.811× (hsa-miR-5100)-0.970×(hsa-miR-493-3p). Compared with the control group, the STB model score in the experimental group was significantly higher (0.32±0.04 vs 0.65±0.08, t=6.09, P<0.001). When the critical value was set to 0.46, the area under ROC curve (AUC) of STB model in identifying spinal tuberculosis was 0.911, with sensitivity of 88.9% and specificity of 77.0%. The STB model scores of patients with spinal tuberculosis with abnormal CRP and erythrocyte sedimentation rate were significantly higher than those with normal CRP and erythrocyte sedimentation rate (P<0.05). The score of STB model was positively correlated with CRP and erythrocyte sedimentation rate (r=0.633, 0.499, all P<0.001).
Conclusion:
Hsa-miR-4708-3p, hsa-miR-5100 and hsa-miR-493-3p have stable differential expressions in peripheral blood of patients with spinal tuberculosis, and the diagnostic model containing these three miRNAs has good diagnostic performance for spinal tuberculosis.

The Role of NLRC3 in the Tumor Immune Mechanism Involved in the Pathogenesis of Endometriosis

Objective:
To explore the expression of Nucleotide-binding Oligomerization Domain (NOD)-like receptor family CARD domain-containing protein 3 (NLRC3) in ovarian ectopic endometrial tissues of stage III and IV endometriosis (EMT) patients and its relationship with clinical pathological features.
Methods:
Immunohistochemistry, Western Blot, and qRT-PCR were used to detect the expression of NLRC3 in 17 cases of stage III, 17 cases of stage IV ovarian ectopic endometrial tissues, and 17 cases of normal endometrial tissues. The relationship between NLRC3 and clinical pathological features of EMT patients was analyzed.
Results:
Compared with normal endometrial tissues, the expression level of NLRC3 was decreased in stage III and stage IV ectopic endometrial tissues (P<0.05), while there was no statistically significant difference in NLRC3 expression between stage III and stage IV endometriosis patients (P>0.05). The positive rate of NLRC3 protein expression was lower in patients with higher CA125 levels (P<0.05). There was no statistically significant difference in NLRC3 expression among EMT patients of different ages, lesion sizes, and degrees of dysmenorrhea (P>0.05).
Conclusion:
The pathogenesis of EMT is associated with immune imbalance in the peritoneal microenvironment, and the decreased or lost immune surveillance function of NLRC3 may be involved in the occurrence and development of EMT. The combined detection of CA125 and NLRC3 expression may play a significant role in predicting or diagnosing EMT.

Construction of a Predictive Model and Efficacy Evaluation of Paraspinal Multifidus Fat Infiltration in Patients with Non-specific Low Back Pain Based on MRS and IDEAL Sequence

Objective:
To assess the relative content of paravertebral multifidus fat in patients with nonspecific low back pain, and construct a predictive nomogram model utilizing magnetic resonance spectroscopy imaging (MRS) and iterative decomposition of water and fat with echo asymmetry and least squares estimation (IDEAL) quantitative analysis.
Methods:
The clinical data of 88 patients with nonspecific low back pain who were treated in our hospital from July 2021 to July 2023 were included. All patients were divided into small group and large group according to Goutallier grading standard. MRS and IDEAL sequences were used to detect fat and water content in both groups. The correlation between fat content and the degree of paravertebral multifidus muscle infiltration was analyzed. ROC curves were plotted to analyze the predictive value of fat and water content for the degree of paravertebral multifidus fat infiltration.Fat and water content detected by MRS and IDEAL sequences were included in the nomogram model. R software was used to evaluate the efficacy of the nomogram model using the C-index, ROC curves, and calibration curves.
Results:
There were no significant differences in gender, age, and duration between the two groups (P>0.05). Fat (%) detected by MRS and IDEAL sequences in the many group was significantly higher, while Water (%) in the few group was significantly higher, with statistical significance (P<0.05). Correlation analysis showed a positive correlation between Fat (%) and the degree of paravertebral multifidus fat infiltration, with significant values for MRS (r=0.420, P<0.05) and IDEAL sequences (r=0.477, P<0.05). Water (%) showed a negative correlation, with significant values for MRS (r=-0.469, P<0.05) and IDEAL sequences (r=-0.496, P<0.05). ROC curve results showed that the AUC values for evaluating the degree of paravertebral multifidus fat infiltration in patients with nonspecific low back pain were 0.800 and 0.841 for MRS and IDEAL sequences, respectively.The nomogram model based on Fat (%) and Water (%) detected by MRS and IDEAL sequences had good discrimination, with a C-index of 0.978. ROC curve analysis showed an AUC of 0.978 (95% CI: 0.948-1.000) for the modeling group and 0.935 (95% CI: 0.838-1.000) for the validation group, indicating good predictive efficacy.
Conclusion:
The nomogram model based on Fat (%) and Water (%) detected by MRS and IDEAL sequences provides an intuitive prediction of the degree of paravertebral multifidus fat infiltration in patients with nonspecific low back pain.

Ras and ERK1/2 Protein Expression in Cancer Tissues of Patients with Type 2 Diabetes Mellitus and Colorectal Cancer: Significance and Correlation with Lymph Node Metastasis and Survival

Objective:
To investigate the expression levels of Ras-ERK1/2 signaling in patients with type 2 diabetes mellitus (T2DM) and colorectal cancer (CRC), analyze their relationship with lymph node metastasis, and explore their correlation with survival status.
Methods:
A total of 59 patients who underwent colorectal cancer surgery at People's Hospital of Shijiazhuang from March 2019 to January 2021 were selected. Patients were divided into the diabetes group (n=29) and non-diabetes group (n=30) based on the presence of concurrent diabetes. Immunohistochemical methods were used to detect the expression levels of Ras and ERK1/2 proteins in surgically resected cancer tissues. Differences in lymph node metastasis were analyzed using the Log-rank test. All patients were followed up for 36 months, and survival conditions were statistically compared. The Spearman correlation analysis was used to analyze the correlation between Ras and ERK1/2 expression and survival period.
Results:
Ras and ERK1/2 were expressed in both CRC and CRC combined with T2DM, mainly concentrated in the cytoplasm of cancer cells. Compared with the non-diabetes group, the positive expression rates of Ras and ERK1/2 in the CRC combined with T2DM group were significantly increased (P<0.05). The lymph node metastasis rate in the CRC combined with T2DM group was significantly higher than that in the pure CRC group (P<0.05). Compared with the Ras and ERK1/2 negative expression group, the lymph node metastasis rate was significantly higher in the Ras and ERK1/2 positive expression group (P<0.05). The Spearman correlation analysis showed that the protein expression of Ras and ERK1/2 in colorectal cancer patients was negatively correlated with median survival time (P<0.05).
Conclusion:
Positive expression of Ras-ERK1/2 proteins is positively correlated with lymph node metastasis and negatively correlated with the survival period of colorectal cancer patients, making it an important target for early diagnosis and prognosis assessment.

Changes in Peripheral Blood NRG-1 NT proBNP CRP Levels and Their Relationship with Prognosis in Elderly Patients with Severe Decompensated Acute Heart Failure

Objective:
To investigate the changes of peripheral blood Neuromodulation protein 1 (NRG-1), N-terminal pro B-type natriuretic peptide (NT proBNP), C-reactive protein (CRP) levels in elderly patients with severe decompensated acute heart failure and their relationship with prognosis.
Methods:
A total of 128 elderly patients with severe decompensated acute heart failure who were treated in our hospital from January 2021 to February 2022 were selected as the observation group, while 100 patients with chronic heart failure were selected as the control group, the levels of peripheral blood NRG-1, NT-proBNP, and CRP were compared between the two groups. The differences in peripheral blood NRG-1, NT-proBNP, and CRP levels among patients with different clinical characteristics in the observation group were analyzed, as well as the differences in clinical characteristics and peripheral blood NRG-1, NT-proBNP levels and CRP levels of dead and surviving patients.
Results:
The peripheral blood NRG-1 of the observation group was (936.61±204.40) pg/ml, which was significantly lower than that of the control group (P<0.05), while NT proBNP and CRP were (3611.88±505.52) pg/ml and (28.77±8.05) mg/L, respectively, which was significantly higher than that of the control group (P<0.05). The peripheral blood NRG-1 of NYHA grade IV patients in the observation group was (1106.65±210.41) pg/ml, which was significantly lower than the control group (P<0.05), while NT proBNP and CRP were (3410.45±541.15) pg/ml and (24.40±9.94) mg/L, respectively, which was significantly higher than the control group (P<0.05). The peripheral blood NRG-1 of patients with LVEF<40% in the observation group was (1014.46±201.45) pg/ml, which was significantly lower than that of patients with LVEF ≥ 40% (P<0.05), while NT proBNP and CRP were (3521.08±500.78) pg/ml and (25.71±8.38) mg/L, respectively, which was significantly higher than those of patients with LVEF>40% (P<0.05). Peripheral blood NRG-1 was positively correlated with LVEF (r=0.477, P<0.05), while NT proBNP and CRP levels were negatively correlated with LVEF (r=-0.398 and -0.466, P<0.05). The proportion of dead patients aged ≥ 80 years, the proportion of NYHA grade IV patients, and the proportion of LVEF<40% in the observation group were 70.00%, 65.00%, and 85.00%, respectively, which were significantly higher than those of surviving patients (P<0.05). The peripheral blood NRG-1 of the observation group's deceased patients was (924.54±194.65) pg/ml, which was significantly lower than that of the surviving patients (P<0.05), while the levels of NT-proBNP and CRP were (3681.54±445.54) pg/ml and (27.84±6.15) mg/L, respectively, which was significantly higher than those of the surviving patients (P<0.05). Logistic regression analysis showed that NYHA grading, LVEF, NRG-1, NT proBNP, and CRP were influencing factors for patient mortality (P<0.05).
Conclusion:
The peripheral blood NRG-1 levels in elderly patients with severe decompensated acute heart failure decrease, while the levels of NT-proBNP and CRP increase, are related to the patient's cardiac function grading and LVEF, and are also influencing factors for patient prognosis.

Mechanism of miR-21 Targeting PTEN to Regulate AKT/FoxO1 Signaling Pathway on Apoptosis in Gastric Cancer Cells

Objective:
To investigate the effect of miR-21 targeting PTEN to regulate AKT/FoxO1 signaling pathway on apoptosis in gastric cancer cells.
Methods:
Gastric cancer SGC-7901 cells were divided into miR-NC inhibitors group (SGC-7901 cells transfected with miR-NC inhibitors plasmid), miR-21 inhibitors group (SGC-7901 cells transfected with miR-21 inhibitors plasmid), miR-21 inhibitors+sh-PTEN group (SGC-7901 cells transfected with miR-21 inhibitors and sh-PTEN plasmids); and control group (SGC-7901 cells not transfected with any plasmids). qRT-PCR was performed to detect miR-21 mRNA expression in cells; CCK-8 was used to detect cell proliferation ability; Transwell assay for cell invasion shift ability; flow cytometry for apoptosis rate; protein blotting for PTEN, AKT, p-AKT, PI3K, p-PI3K, FoxO1 protein expression in cells; dual luciferase reporter for targeting relationship between miR-21 and PTEN.
Results:
Compared with normal human gastric mucosal epithelial cell GES-1 (1.00±0.10), the expression of miR-21 (1.89 ± 0.17) in gastric cancer cell SGC-7901 was significantly increased (P<0.05). Compared with the control group, the expression of miR-21 (0.83 ± 0.10), proliferation rate (45.31 ± 4.92)%, number of invasions (62.34 ± 5.83), and p-PI3K/PI3K (0.42 ± 0.05), p-AKT/AKT (0.51 ± 0.05) ratios were significantly reduced in the miR-21 inhibitors group, while the apoptosis rate (23.48 ± 3.51)%, PTEN (0.98 ± 0.10), and FoxO1 (0.76 ± 0.08) protein expression were significantly increased (P<0.001). Compared with the pcDNA-NC group, the cell proliferation rate (48.26 ± 5.01), number of invasive cells (65.37 ± 6.02), and p-PI3K/PI3K (0.46 ± 0.05), p-AKT/AKT (0.55 ± 0.06) ratios in the pcDNA-PTEN group were significantly reduced, while the cell apoptosis rate (25.61 ± 3.27)% and the expression of PTEN (0.91 ± 0.09) and FoxO1 (0.70 ± 0.08) proteins in the cells were increased (P<0.05). It was predicted that the 3'UTR end of PTEN had complementary binding sites with miR-21. Compared with the miR-NC group, the luciferase activity of the miR-21 group (0.32 ± 0.03) was significantly reduced (P<0.05) when transfected with wild-type PTEN (PTEN-WT). Compared with the miR-21 inhibitors group, the miR-21 inhibitors+sh-PTEN group showed a significant increase in cell proliferation rate (90.25 ± 9.14)%, invasion cell count (125.69 ± 10.31), and p-PI3K/PI3K (0.80 ± 0.08), p-AKT/AKT (0.76 ± 0.08) ratios. The apoptosis rate (6.24 ± 1.32) and the expression of PTEN (0.30 ± 0.04) and FoxO1 (0.38 ± 0.05) proteins in the cells were also decreased (P<0.001).
Conclusion:
Knockdown of miR-21 can target negative regulation of PTEN expression, modulate AKT/FoxO1 signaling pathway, inhibit proliferation and invasion of gastric cancer cells, and promote apoptosis.

Lnc-FOXD3-AS1 Promotes the Expression of Ferrimodulin in Rats with Iron Deficiency Anemia by Activating SMAD1/5/8

Objective:
To investigate the regulatory effect of long-chain noncoding RNA FOXD3-AS1 (Lnc-FoxD3-AS1) on the expression of hepcidin in rats with iron deficiency anemia (IDA).
Methods:
The IDA model was induced by repeated bloodletting and feeding low iron diet to SD rats without specific pathogen grade. The rats were divided into control group, IDA group, IDA rats treated with LCN-FoxD3-AS1 overexpression group (pcDNA-FOXD3-AS1+IDA group), IDA rats treated with empty vector expression group (pcDNA-null+IDA group), and IDA rats treated with pcDNA-FOXD3-AS1 combined with Sm ad1/5/8 activation inhibitor Compound C treatment group (pcDNA-FOXD3-AS1+Compound C+IDA group), n=8 for each group. The content of iron in blood of rats in each group was detected by kit method, the expression of Lnc-FOXD3-AS1 in liver tissue and serum was detected by qRT-PCR, and the expression and activation of ferrimodulin and SMAD1/5/8 protein in liver tissue and serum were detected by western blot and ELISA.
Results:
Iron deficiency anemia model was successfully induced in IDA group. Compared with the control group, iron content in liver tissue and serum of rats in IDA group remained in the state of iron deficiency (all P<0.05), and the expression levels of ferrimodulin, LCNC-FOXD3-AS1 and SMAD1/5/8 were significantly decreased (all P<0.05), and the phosphorylation level of SMAD1/5/8 was significantly decreased (all P<0.05). Compared with pcDNA-null+IDA group, iron content in liver tissue and serum in pcDNA-FOXD3-AS1+IDA group was significantly increased (all P<0.05), and the expression levels of ferrimodulin, LCNA-FOXD3-AS1 and SMAD1/5/8 were significantly increased (all P<0.05). Moreover, the phosphorylation level of SMAD1/5/8 was significantly increased (P<0.05). Compared with pcDNA-FOXD3-AS1+IDA group, the iron content in liver tissue and serum of pcDNA-FOXD3-AS1+Compound C+IDA group was significantly decreased (all P<0.05), and the expression levels of ferromodulin and SMAD1/5/8 were significantly decreased (all P<0.05).
Conclusion:
lnc-FOXD3-AS1 is downregulated in the IDA rat model, and it promotes the expression of Hepcidin by activating the SMAD1/5/8 pathway. This study is of great significance for a deeper understanding of the biological network mechanisms underlying IDA occurrence.

The Value of Alendronate after OVCF Vertebroplasty

Objective:
To investigate the role of alendronate anti-osteoporosis in fracture healing and prevention of re-fracture after vertebroplasty for osteoporotic vertebral compression fracture (OVCF).
Methods:
A total of 99 patients with OVCF who underwent PVP in our hospital from August 2021 to August 2022 were selected and randomly divided into control group (49 cases) and study group (50 cases). The control group was given basic anti-osteoporosis intervention after surgery, and the study group was given new generation bisphosphonate on the basis of the control group after surgery. The treatment continued 6 months. Postoperative rehabilitation, pain level (VAS score), lumbar spine function (ODI score, JOA score), bone mineral density, bone turnover indexes [N-terminal mid-portion osteocalcin (N-MID-OT), β-collagen special sequence (β-CTX), and total type 1 collagen amino-terminal prolongation peptide (T-P1NP)], vertebral stability indexes (height ratio of the injured vertebrae, Cobb's angle of the injured vertebrae), and incidence of re-fracture were compared in the two groups were compared.
Results:
The fracture healing time and recovery time of the study group were shorter than those of the control group (P<0.05). After 3 and 6 months of treatment, VAS, ODI scores and Cobb Angle of injured vertebra in the study group were lower than those in the control group, while JOA scores, height ratio of injured vertebra, Ward triangle and bone density of femoral neck in the study group were higher than those in the control group (P<0.05). The serum levels of N-MID-OT, β-CTX and T-P1NP in the study group were lower than those in the control group after 1 month, 3 months and 6 months of treatment (P<0.05). The incidence of refracture in the study group was lower than that in the control group (P<0.05).
Conclusion:
Alendronate combined with PVP in the treatment of OVCF improves bone metabolism, promotes fracture healing, reduces the incidence of postoperative re-fracture, and has significant advantages in the recovery of patients' lumbar spine function and pain relief.

Effects of LncRNA PINK1-AS on the Proliferation Apoptosis and Epithelial Mesenchymal Transformation of Gastric Cancer Cells by Regulating the miR-134-5p/SERPINE1 Axis

Objective:
To investigate the effects of LncRNA PINK1-AS on the proliferation, apoptosis and epithelial mesenchymal transformation (EMT) of gastric cancer cells by regulating the miR-134-5p/serine protease inhibitor clade E member 1 (SERPINE1) axis.
Methods:
QRT-PCR was applied to detect the expression levels of PINK1-AS, miR-134-5p, and SERPINE mRNA in 60 cases of gastric cancer tissues and 60 cases of adjacent tissues. Gastric cancer cells MKN28 were cultured in vitro and grouped into control group, si-NC group, si-PINK1-AS group, pc-NC group, pc-PINK1-AS group, miR-NC group, and miR-134-5p mimics group; QRT-PCR was applied to detect the expression levels of PINK1-AS, miR-134-5p, and SERPINE mRNA in cells of each transfection group, Cck-8 was applied to detect cell proliferation; Flow cytometry was applied to detect cell apoptosis rate; Transwell test was applied to detect cell migration and invasion; Western blot was applied to detect the expression levels of SERPINE1, Ki-67, cleaved caspase-3, E-cadherin, N-cadherin, and vimentin proteins in cells. Double Luciferase reporter gene experiment was applied to detect the targeting relationship between PINK1-AS and miR-134-5p, and between miR-134-5p and LncRNA SERPINE1.
Results:
The expression of PINK1-AS and SERPINE mRNA in gastric cancer tissue was obviously higher than that in adjacent tissues, while the expression of miR-134-5p was lower than that in adjacent tissues (P<0.05). Down-regulation the expression of PINK1-AS can reduce the PINK1-AS, SERPINE mRNA, OD450 values (24, 48h), cell migration and invasion, the expression of SERPINE1, Ki-67, N-cadherin, vimentin proteins in MKN28 cells ,increased the expression of miR-134-5p, cell apoptosis rate, the expression of cleaved caspase-3, and E-cadherin proteins (P<0.05). Overexpression of PINK1-AS can increase the PINK1-AS, SERPINE mRNA, OD450 values (24, 48h), cell migration and invasion, the expression of SERPINE1, Ki-67, N-cadherin, vimentin proteins in MKN28 cells, the expression of miR-134-5p, cell apoptosis rate, the expression of cleaved caspase-3, and E-cadherin proteins obviously decreased (P<0.05). Overexpression of miR-134-5p increased miR-134-5p expression, apoptosis rate, cleaved caspase-3, and E-cadherin protein expression, and decreased SERPINE mRNA, OD450 value (24, 48h), cell migration and invasion number, SERPINE1, Ki-67, N -cadherin, and vimentin protein expression (P<0.05).
Conclusion:
LncRNA PINK1-AS can regulate the miR-134-5p/SERPINE1 axis, thereby affecting proliferation, apoptosis and EMT of gastric cancer cells.

Impact of Hyperbaric Oxygen Combined with Butylphthalide on Neurological Function and Serum Levels of Ang-1 THBS-1 and Bcl-2 in Patients with Acute Ischemic Stroke

Objective:
To observe the effects of hyperbaric oxygen combined with butylphthalide on neurological function and serum levels of Ang-1, THBS-1, and Bcl-2 in patients with acute ischemic stroke.
Methods:
A total of 112 patients with acute ischemic stroke admitted from May 2021 to January 2023 were prospectively selected as the study subjects and divided into two groups using a completely random method. Both groups received conventional medication, with 56 patients in the Butylphthalide group receiving butylphthalide treatment and 56 patients in the Hyperbaric Oxygen group receiving hyperbaric oxygen combined with butylphthalide treatment. The differences in NIHSS score, Barthel Index (BI), and modified Rankin Scale (mRS) score between the two groups were compared. Additionally, brain blood flow perfusion, levels of Ang-1, THBS-1, and Bcl-2 were detected, and treatment efficacy and adverse reactions were statistically analyzed.
Results:
There was no statistically significant difference in the relevant scores before treatment in both groups (P>0.05). Compared with before treatment, the NIHSS and mRS scores decreased, while the BI index increased in both the Hyperbaric Oxygen and Butylphthalide groups, with greater reductions and increases observed in the Hyperbaric Oxygen group than in the Butylphthalide group (P<0.05). There was no statistically significant difference in brain blood flow perfusion before treatment between the two groups (P>0.05). Compared with before treatment, both the Hyperbaric Oxygen and Butylphthalide groups showed an increase in cerebral blood flow (CBF) and cerebral blood volume (CBV), with a greater increase observed in the Hyperbaric Oxygen group than in the Butylphthalide group (P<0.05). There was no statistically significant difference in Ang-1, THBS-1, and Bcl-2 levels before treatment between the two groups (P>0.05). Compared with before treatment, both the Hyperbaric Oxygen and Butylphthalide groups showed an increase in Ang-1 and Bcl-2 levels, and a decrease in THBS-1 levels, with a greater increase and decrease observed in the Hyperbaric Oxygen group than in the Butylphthalide group (P<0.05). The total effective rate in the Hyperbaric Oxygen group was 94.64% (53/56), higher than the Butylphthalide group's 82.14% (46/56), and there was no statistically significant difference in adverse reactions compared to the Butylphthalide group (P>0.05).
Conclusion:
Hyperbaric oxygen combined with butylphthalide can improve cerebral blood flow perfusion, regulate the expression of Ang-1, THBS-1, and Bcl-2, improve neurological function, and enhance patient independence.

Evaluation of Clinical Efficacy of Solid Image Contrast Technique Combined with Cutting Prefabrication Technique in The Treatment of Special Ankle Fracture

Objective:
To evaluate the clinical efficacy of solid image contrast technique combined with cutting prefabrication technique in the treatment of special ankle fracture.
Methods:
A retrospective study was conducted on 63 cases of special type ankle fractures admitted to the Affiliated Hospital of Chengde Medical University from May 2015 to June 2022. According to the preoperative planning method, the patients were divided into the digital technology group (using solid image comparison technology combined with cutting prefabrication technique for preoperative planning, 31 cases) and the radiological technology group (using radiological data for preoperative planning, 32 cases). Ankle bone reconstruction surgery was performed at a scheduled time, and data of various indicators before, during, and after surgery were collected for comparative analysis.
Results:
The preoperative planning cost, preoperative planning time, and Burwell-Charnley anatomical reduction rate in the digital technology group were higher than those in the radiological technology group, and the differences between the two groups were statistically significant (P<0.05). The total intraoperative blood loss and intraoperative time in the digital technology group were lower than those in the radiological technology group, and the differences between the two groups were statistically significant (P<0.05). All patients were followed up for 9 to 13 months with an average of (11.23±2.62) months. The fracture healing time, full weight-bearing time, and complication rate in the digital technology group were lower than those in the radiological technology group, and the differences between the two groups were statistically significant (P<0.05). The AOFAS score in the digital technology group was higher than that in the radiological technology group, and the differences between the two groups were statistically significant (P<0.05). The main effects of time, group, and the interaction between time and group on the plantar pressure and ROM degrees on the affected side in both groups were statistically significant (P<0.001).
Conclusion:
The application of solid image comparison technology combined with cutting prefabrication technique in the treatment of special type ankle fractures has a definite clinical efficacy, with a low incidence of complications. However, it may increase patient medical expenses and preoperative planning time.

Relationship between Gingival Crevicular Fluid Levels of hBD-2 hBD-3 LPS and Periodontal Health Indicators in Patients with Type 2 Diabetes Mellitus and Chronic Periodontitis

Objective:
To explore changes in β-defensins (hBD-2, hBD-3), lipopolysaccharides (LPS) in gingival crevicular fluid (GCF), and their correlation with periodontal health indicators in patients with type 2 diabetes mellitus (T2DM) and chronic periodontitis.
Methods:
This study, approved by the Medical Ethics Committee, included 120 patients diagnosed with T2DM and chronic periodontitis (Group A), 120 chronic periodontitis patients without T2DM (Group B), and 120 periodontally healthy individuals without T2DM (Group C), between January 2020 and December 2022. GCF levels of hBD-2, hBD-3, LPS, oral and periodontal health-related indicators, and subgingival bacterial flora were measured. The correlation between hBD-2, hBD-3, LPS, and periodontal health indicators in T2DM patients with chronic periodontitis was analyzed.
Results:
The GCF levels of hBD-2, hBD-3, and LPS in Groups A, B, and C showed significant differences (P<0.05). In Group A, hBD-2, hBD-3 were lower than in Groups B and C, while LPS was higher than in Groups B and C. Group B had lower hBD-2, hBD-3 compared to Group C, and higher LPS than Group C (P<0.05). Periodontal parameters including PD, SBI, CAL, GCF in Groups A, B, and C showed significant differences (P<0.05). Group A had higher values than Groups B and C, and Group B had higher values than Group C (P<0.05). The levels of Porphyromonas gingivalis, Aggregatibacter actinomycetemcomitans, Prevotella intermedia, and Tannerella forsythia in Groups A, B, and C differed significantly (P<0.05). Group A had higher levels compared to Groups B and C, and Group B had higher levels compared to Group C (P<0.05). Significant negative correlations were found between GCF hBD-2, hBD-3, and PD, SBI, CAL, GCF, while LPS showed a significant positive correlation with these parameters (P<0.05).
Conclusion:
GCF levels of hBD-2, hBD-3 significantly decrease, and LPS significantly increase in T2DM patients with chronic periodontitis, showing a close relationship with the deterioration of periodontal health indicators.

Efficacy of Ganglioside Combined with Hypothermia in the Treatment of Neonatal Hypoxic-Ischemic Encephalopathy and Its Impact on Behavioral and Intellectual Development in Children

Objective:
To investigate the efficacy of ganglioside combined with hypothermia in the treatment of neonatal hypoxic-ischemic encephalopathy (HIE) and its impact on the behavior and intellectual development of affected children.
Methods:
A total of 148 patients with HIE admitted to the hospital were enrolled and randomly divided into experimental group and control group between January 2018 and December 2022, 74 cases in each group. The control group was treated with ganglioside, while experimental group was additionally treated with mild hypothermia. The curative effect after treatment, score of neonatal behavioral neurological assessment (NBNA), recovery time of clinical manifestations, score of intelligent development quotient (DQ), levels of nerve injury markers [S100β protein, neuron-specific enolase (NSE), myelin basic protein (MBP)] and serum inflammatory factors [interleukin-8 (IL-8), hypersensitive C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α)] in the two groups were observed before and after treatment.
Results:
After treatment, the total effective rate in the experimental group was higher than that in the control group (P<0.05). The NBNA scores at various time points in the experimental group were significantly higher than those in the control group (P<0.05). The clinical recovery times in the experimental group were significantly shorter than those in the control group (P<0.05). The DQ scores in the experimental group were higher than the pre-treatment values and greater than those in the control group (P<0.05). The levels of S100β protein, NSE, and MBP in the experimental group were lower than the pre-treatment values and lower than those in the control group (P<0.05). The levels of IL-8, hs-CRP, and TNF-α in the experimental group were lower than the pre-treatment values and lower than those in the control group (P<0.05).
Conclusion:
The combined application of ganglioside and hypothermia in the clinical treatment of HIE is effective, improving the behavioral and neural functions as well as the intellectual development of affected children. It shortens the recovery time of clinical manifestations such as primitive reflexes and sucking, and reduces the levels of inflammatory factors and neuroinjury markers in children.

Clinical Study on Main Ligament Injury Characteristics of Schatzker Type Ⅳ Tibial Plateau Fracture with Coronal Knee Dislocation

Objective:
To explore the main ligamentous injury characteristics in patients with Schatzker Type IV tibial plateau fracture accompanied by coronary knee dislocation.
Methods:
A total of 32 patients with Schatzker Type IV tibial plateau fracture accompanied by coronary knee dislocation, treated in our hospital from January 2019 to December 2022, were included in the study group. Additionally, 58 patients with isolated Schatzker Type IV tibial plateau fracture were selected as the control group. The two groups were analyzed and compared for cross ligament measurements, laxity coefficients, degree of knee cartilage injury, ligament function indicators, clinical indicators, knee HSS scores, and VAS scores.
Results:
There was no statistically significant difference in La, Da, and Dp between the two groups (P>0.05). However, the study group showed higher values for Lp, TCa, and TCp compared to the control group (P<0.05). Analysis of knee cartilage injury revealed no statistically significant difference between the two groups (P>0.05). Before treatment, there was no significant difference in the peak torque ratio of flexors/extensors and limb symmetry index between the two groups (P>0.05). After treatment, both groups showed improved ligament function, with the study group exhibiting a greater increase in peak torque ratio and limb symmetry index compared to the control group (P<0.05). Clinical indicators, including operation time, intraoperative blood loss, postoperative hospitalization time, fracture healing time, and postoperative complication rate, showed no significant difference between the two groups (P>0.05). There was no statistically significant difference in HSS scores before and 6 months after treatment between the two groups (P>0.05). However, compared to the control group, the study group had lower HSS scores at 7 days, 1 month, and 3 months postoperatively (P<0.05). Similarly, there was no significant difference in VAS scores before and 6 months after treatment between the two groups (P>0.05). Compared to the control group, the study group had higher VAS scores at 7 days, 1 month, and 3 months postoperatively (P<0.05).
Conclusion:
Compared to patients with isolated Schatzker Type IV tibial plateau fractures, those with Schatzker Type IV tibial plateau fractures accompanied by coronary knee dislocation exhibited higher postoperative Lp, TCa, and TCp values, worse ligament recovery, poorer knee joint function, and greater short-term pain. These patients require additional attention.

Comparison of the Application of Double-Tube Negative Pressure Drainage and Traditional Chest Bandage Compression in Breast-Conserving Reconstruction Surgery with Subcutaneous Fat Fascial Flap

Objective:
To explore the application effects of double-tube negative pressure drainage and traditional chest bandage compression in breast-conserving reconstruction surgery with subcutaneous fat fascial flap.
Methods:
A total of 98 patients who underwent breast-conserving reconstruction surgery with subcutaneous fat fascial flap in our hospital from January 2020 to January 2023 were retrospectively selected. Patients were divided into two groups according to their treatment plans: the study group (n=49) received double-tube negative pressure drainage, and the control group (n=49) received traditional chest bandage compression drainage. Patients were followed up for 3 months, and postoperative chest tube removal time, axillary tube removal time, chest drainage volume, axillary drainage volume, FACT-B score, Brief Pain Inventory (BPI) score were recorded. Postoperative flap necrosis, occurrence of lymphedema, and comfort scores were observed.
Results:
There was no statistically significant difference in chest tube removal time and axillary tube removal time between the two groups (P>0.05). The chest drainage volume and axillary drainage volume in the study group were lower than those in the control group (P<0.05). There was no statistically significant difference in FACT-B and BPI scores before surgery between the two groups (P>0.05). The change in FACT-B and BPI scores before and after surgery in the study group was greater than that in the control group (P<0.05). Compared with the control group, the study group had a lower incidence of flap necrosis, and the incidence of high-grade flap necrosis was lower than that in the control group (P<0.05). There was no statistically significant difference in the incidence and severity of lymphedema between the two groups (P>0.05). There was no statistically significant difference in comfort scores between the two groups before surgery (P>0.05). Compared with the control group, the study group had higher comfort scores at 1 day, 1 month, and 3 months after surgery (P<0.05).
Conclusion:
Double-tube negative pressure drainage in breast-conserving reconstruction surgery with subcutaneous fat fascial flap is superior to traditional chest bandage compression in terms of drainage efficiency and pain relief. It improves comfort and quality of life, reduces postoperative flap necrosis, and minimizes complications such as lymphedema.

Clinical Efficacy Comparison of Laparoscopic Cholecystectomy Combined with Choledochotomy for Stone Extraction with One-Stage Closure versus T-Tube Drainage in Patients with Choledocholithiasis Complicated by Acute Cholangitis

Objective:
To explore the differences in the effectiveness of laparoscopic cholecystectomy (LC) combined with choledochotomy for stone extraction with one-stage closure and LC combined with choledochotomy for stone extraction with T-tube drainage in the treatment of patients with cholecystolithiasis and choledocholithiasis complicated by acute cholangitis.
Methods:
A total of 118 patients with gallbladder stones,common bile duct stones, and acute cholangitis diagnosed and requiring surgery at our hospital from January 2020 to December 2022 were selected as the research subjects. Randomized clinical trials were conducted to divide the patients into Group A and Group B, with 59 patients in each group. Group A received LC combined with choledochotomy for stone extraction with one-stage closure, while Group B received LC combined with choledochotomy for stone extraction with T-tube drainage. Surgical process indicators, changes in serum inflammatory factors, liver function indicators, electrolyte levels before and after surgery, and the incidence of surgical complications were compared between the two groups.
Results:
The operative process indicators and postoperative recovery of patients in the two groups were compared. The operative time in Group A was shorter than in Group B, and the difference was statistically significant (P<0.05). Statistical analysis of intraoperative blood loss, time to first anal exhaust after surgery, and length of hospital stay showed no significant differences between Group A and Group B (P>0.05). The inter-group effects, time effects, and interaction effects of IL-1β and TNF-α in patients of both groups were statistically significant (P<0.05). The time effect of CRP in both groups also showed statistical significance (P<0.05). Further pairwise comparisons of IL-1β and TNF-α between the two groups and inter-group comparisons of various inflammatory indicators revealed no significant differences in the levels of IL-1β and TNF-α between Group A and Group B 24 hours postoperatively (P>0.05). However, 48 hours postoperatively, the levels of IL-1β and TNF-α in Group A were lower than in Group B, and the difference between the two groups was statistically significant (P<0.05). The inter-group effects and interaction effects of ALT, TBIL, and AST in patients of both groups showed no statistical significance (P>0.05). Conclusion:
The efficacy difference between LC combined with choledochotomy for stone extraction with one-stage closure and LC combined with choledochotomy for stone extraction with T-tube drainage in patients with cholecystolithiasis and choledocholithiasis complicated by acute cholangitis is not significant. However, the former has the advantage of shorter operative time and lower postoperative inflammatory response.

Effectiveness and Safety Analysis of Drug-Eluting Microsphere Transarterial Chemoembolization Compared with Conventional Transarterial Chemoembolization in the Treatment of Intermediate to Advanced Hepatocellular Carcinoma

Objective:
To analyze the application value of drug-eluting microsphere transarterial chemoembolization (DEB-TACE) compared with conventional transarterial chemoembolization (cTACE) in intermediate to advanced hepatocellular carcinoma.
Methods:
A retrospective analysis was conducted on patients with intermediate to advanced hepatocellular carcinoma in our hospital from March 2018 to March 2021. The patients were grouped according to the treatment method, with 92 cases undergoing DEB-TACE (included in the DEB-TACE group) and 125 cases undergoing cTACE (included in the cTACE group). The efficacy and safety of the two groups were compared.
Results:
The disease remission rate in the DEB-TACE group was higher than that in the cTACE group at 1 month after surgery. The differences in TBIL, ALT, AST, and AFP before and after surgery were greater in the DEB-TACE group than in the cTACE group. The severity of postoperative vomiting was lower in the DEB-TACE group than in the cTACE group. The one-year survival rate was higher in the DEB-TACE group than in the cTACE group (P<0.05).
Conclusion:
DEB-TACE treatment is more effective for intermediate to advanced hepatocellular carcinoma, with fewer complications and higher safety.

Significance of YKL-40 Levels in Predicting the Risk of Acute Exacerbation in Chronic Obstructive Pulmonary Disease

Objective:
To explore the significance of chitin enzyme protein 40(YKL-40) and CXC chemokine ligand 9(CXCL9) in predicting the acute exacerbation risk of chronic obstructive pulmonary disease (COPD).
Methods:
From October 2019 to October 2021, 150 COPD patients (38 females and 112 males) and 150 normal people were selected as the research objects. Serum YKL-40 and CXCL9 levels were measured by ELISA, and the hospitalization time was calculated. Follow up the number of acute exacerbations of COPD within one year after this hospitalization.
Results:
Compared with the control group, the serum YKL-40 level in COPD patients increased significantly (22.23±15.35 ng/ml vs 63.96±13.66 ng/ml, t=24.79, P<0.0001), while the level of CXCL9 did not change significantly (17.92±9.01 ng/ml vs 19.29±9.84 ng/mL,t=1.26, P=0.210). Serum YKL-40 increased with the increase of GOLD grade in COPD patients (grade 1-2: 51.82 ±12.82 ng/ml; Grade 3: 63.26±4.59 ng/ml; Grade 4: 76.86±7.59 ng/mL, F=96.92, P<0.0001). Compared with COPD patients with mMRC score ≤ 2, the serum YKL-40 level of COPD patients with mMRC score of 2~3 or ≥3 was increased (58.35±15.37 ng/ml vs 64.55±10.60 ng/ml, 68.79±12.66 ng/mL, P<0.05). In COPD patients, serum YKL-40 was negative correlated with FEV1(r=-0.668, P<0.001) and FEV1/FVC(r=-0.167, P=0.041), and serum YKL-40 was positively correlated with IL-6(r=0.257, P=0.002), TNF-α(r=0.522, P<0.001) and CRP(r=0.231, P=0.006). Compared with COPD patients whose hospitalization time was ≤8 d, the serum YKL-40 level of COPD patients whose hospitalization time was 8~13 d and ≥13 d was increased (57.31±15.82 ng/ml vs 64.96±9.98 ng/ml, 69.79±11.58 ng/mL, P<0.05). The level of serum YKL-40 in patients with acute exacerbation ≤2 times was significantly lower than that in patients with acute exacerbation > 2 times (58.77±15.66 ng/ml vs 69.58±9.08 ng/ml, t=5.17, P<0.001). Multiple linear regression showed that serum YKL-40 was negatively correlated with FEV1% (OR=0.700, 95%CI=-0.958, -0.272) and positively correlated with hospitalization time ≥ 13 d (OR=1.125, 95%CI=1.007, 1.472).
Conclusion:
The level of serum YKL-40 at admission is positively correlated with the severity and poor prognosis of COPD patients.

Correlation Study between Vertebral CT Values and Bone Cement Leakage in Percutaneous Vertebral Augmentation for Osteoporotic Vertebral Compression Fractures

Objective:
To analyze the correlation between vertebral CT values and the occurrence of bone cement leakage in patients undergoing percutaneous vertebral augmentation (PVA) for osteoporotic vertebral compression fractures (OVCF).
Methods:
A total of 61 patients with osteoporotic vertebral compression fractures admitted and treated with PVA at the Affiliated Hospital of Chengde Medical College between September 2019 and October 2022 were included. The patients were divided into the leakage group and non-leakage group based on whether bone cement leakage occurred. Patient demographics, including gender, age, intraoperative bone cement injection volume, fracture location (thoracic or lumbar), fracture compression degree, and CT values of the L1 vertebra, were recorded for both groups.
Results:
There were no statistically significant differences in gender, age, fracture location, and bone cement injection volume between the two groups (P>0.05). However, differences in fracture compression degree and CT values of the L1 vertebra were statistically significant (P<0.05). Multivariate logistic analysis revealed a statistically significant association between changes in L1 vertebra CT values and bone cement leakage (P<0.05). Further ROC curve analysis showed an area under the curve (AUC) of 0.674 (95% CI: 0.524~0.824, P=0.025). The optimal cutoff value for L1 vertebra CT values was 62.1 HU, with a sensitivity of 79.5% and specificity of 63.6%.
Conclusion:
Vertebral CT values of the L1 vertebra are correlated with bone cement leakage in PVA treatment for osteoporotic vertebral compression fractures. Preoperative measurement of L1 vertebra CT values has a certain guiding significance in evaluating bone cement leakage.

Evaluation of Image Quality in Combined Low-Contrast Head and Neck CTA Using Different Triggering Methods with 256-layer iCT

Objective:
To observe the evaluation of the image quality of low-contrast head and neck CTA vascular scanning by two different triggering Methods: Test-Bolus and Bolus-Tracking.
Methods:
A total of 100 cases of head and neck CTA images completed in our department from November 2022 to March 2023 were randomly selected. Among them, Group A comprised 50 cases using the test-bolus method, and Group B comprised 50 cases using the bolus-tracking method. A retrospective analysis was performed to assess the impact of the two different triggering methods on image quality.
Results:
In Group A, images obtained using the test-bolus method had an average CT value of (338.06±73.54) HU for the carotid arteries and (323.72±73.13) HU for the cerebral middle arteries, with higher image quality compared to Group B. Group B, using the bolus-tracking method, had an average CT value of (276.82±66.72) HU for the carotid arteries and (270.32±65.39) HU for the cerebral middle arteries, with slightly more venous interference but without affecting diagnosis, and slightly inferior image quality. The objective evaluation of the CT values of the carotid and cerebral middle arteries in Groups A and B showed statistically significant differences (P<0.05). The subjective evaluation of the excellent and good rates between the two groups showed that Group A had a higher excellent rate (70%, 35/50) and a higher excellent and good rate (90%, 45/50) compared to Group B (excellent rate: 40%, 20/50; excellent and good rate: 86%, 43/50), with statistically significant differences (P<0.05) in image quality but not in the excellent and good rate (P>0.05).
Conclusion:
Both imaging methods can meet diagnostic requirements, but using the test-bolus method for scanning can improve image quality with less venous interference.

Analysis of the Relationship between the Changes of Coagulation Index and the Prognosis of Patients with Pulmonary Infection Caused by Multiple Injuries and Acute Respiratory Distress Syndrome

Objective:
To explore the relationship between the changes of coagulation index and the prognosis of patients with pulmonary infection caused by multiple injuries and acute respiratory distress syndrome.
Methods:
From August 2019 to December 2020, a total of 121 patients with multiple injuries were enrolled, and complete demographic characteristics and clinical laboratory baseline data were collected at the time of enrollment. According to the Simple Clinical Pulmonary Infection Score (sCPIS), the patients were divided into lung infection group (n=62) and non-pulmonary infection group (n=59). According to the 28-day mortality rate, patients with lung infection caused by multiple injuries and ARDS were divided into survival group (n=31) and death group (n=31). One-way analysis of variance assesses the presence or absence of lung infection and the difference between the demographic characteristics of the survival group and the death group and the clinical laboratory baseline. Logisitic regression and ROC curve analyze the SOFA score of patients with lung infection caused by multiple injuries and ARDS, The predictive value of PT, FDP, DD and ICU stay.
Results:
There were 121 patients with multiple injuries and 62 patients with pulmonary infection caused by multiple injuries and ARDS. The SOFA score, APTT, INR, AT-Ⅲ, ICU stay and 28-day mortality of patients with lung infection were higher than those of non-pulmonary infections, and SOFA scores, PT, FDP, PT, FDP, pulmonary infection caused by multiple injuries and ARDS death group, DD and higher than survival group (P<0.01). Logisitic regression and ROC curve analysis PT is a prognostic factor for patients with lung infection caused by multiple injuries and ARDS. The ROC curve area is 0.737, and the difference is statistically significant (P<0.05).
Conclusion:
This study showed that the coagulation index of the lung infection caused by multiple injuries and ARDS death group was higher than that of the survival group. Early warning values.

Analysis of Pathogen Distribution, Drug Sensitivity and Influencing Factors of Postoperative Respiratory Tract Infections in Patients with Gastrointestinal Surgery

Objective:
To investigate the distribution of pathogens, drug sensitivity, and influencing factors of postoperative respiratory tract infections in patients undergoing gastrointestinal surgery.
Methods:
A total of 105 patients with postoperative respiratory tract infections after gastrointestinal surgery from April 2019 to April 2023 were selected as the infection group. The distribution and drug sensitivity of pathogens in infected patients were analyzed. Another 105 patients who did not develop respiratory tract infections after gastrointestinal surgery during the same period were selected as the non-infection group. General information was collected for both groups, and multifactor logistic regression analysis was used to explore the risk factors for postoperative respiratory tract infections in patients undergoing gastrointestinal surgery.
Results:
In the 105 patients of the infection group, a total of 121 strains of pathogens were isolated from sputum samples. Among them, 78 strains (64.46%) were Gram-negative bacteria, mainly Pseudomonas aeruginosa; 37 strains (30.58%) were Gram-positive bacteria, mainly Staphylococcus aureus; and 6 strains (4.96%) were fungi, all Candida albicans. Pseudomonas aeruginosa, Klebsiella pneumoniae, Escherichia coli, and Acinetobacter baumannii had a 100% resistance rate to ampicillin. The resistance rates of Pseudomonas aeruginosa, Klebsiella pneumoniae, and Escherichia coli to piperacillin/tazobactam were 6.45%, 15.00%, and 7.69%, respectively. Staphylococcus aureus, hemolytic Staphylococcus, and Staphylococcus epidermidis had resistance rates of 100.00%, 90.00%, and 100.00% to penicillin, respectively. The resistance rates of Staphylococcus aureus, hemolytic Staphylococcus, and Staphylococcus epidermidis to quinupristin/dalfopristin were all 0.00%. There were statistically significant differences between the two groups in terms of age, smoking history, diabetes, history of lung disease, surgical method, surgical nature, surgical time, and gastric tube retention (P<0.05). Binary logistic regression analysis showed that smoking history (OR=1.772, p=0.002), diabetes (OR=2.212, P<0.001), history of lung disease (OR=1.950, P=0.001), open surgery (OR=2.522, P<0.001), emergency surgery (OR=2.389, P<0.001), surgery time ≥2h (OR=2.102, P=0.002), and gastric tube retention (OR=1.857, P<0.001) were all risk factors for postoperative respiratory tract infections in patients undergoing gastrointestinal surgery.
Conclusion:
Postoperative respiratory tract infections in patients undergoing gastrointestinal surgery are mainly caused by Gram-negative bacteria. Gram-negative bacteria have high resistance to ampicillin, and Gram-positive bacteria have high resistance to penicillin. The occurrence of postoperative respiratory tract infections is related to multiple risk factors.

Efficacy and Prognostic Analysis of Erlotinib in Combination with Anlotinib or Bevacizumab Monoclonal Antibody in the Treatment of EGFR-sensitive Mutant Advanced NSCLC

Objective:
To explore the efficacy and prognosis of erlotinib in combination with anlotinib or bevacizumab monoclonal antibody in the treatment of epidermal growth factor receptor (EGFR)-sensitive mutant advanced non-small cell lung cancer (NSCLC).
Methods:
A retrospective analysis was conducted on 107 patients with EGFR-sensitive mutant advanced NSCLC from September 2019 to February 2021, divided into Group A (55 cases) and Group B (52 cases). Group A received erlotinib + anlotinib, while Group B received erlotinib + bevacizumab. Short-term efficacy was assessed after one treatment cycle. Tumor markers, adverse reactions, and a 2-year follow-up were observed.
Results:
The objective response rate in Groups A and B was 58.18% and 63.46%, respectively, and the disease control rate was 89.09% and 90.38%, with no significant difference between the two groups (P>0.05). The differences in serum CA125, CEA, TPA, TK1, SCC-Ag, and CYFRA21-1 before and after treatment were not statistically significant (P>0.05). There was no significant difference in the incidence of adverse reactions between Groups A and B (P>0.05). The median progression-free survival in Groups A and B was 17.3 months and 18.1 months, respectively, with no statistically significant difference between the two groups (P>0.05).
Conclusion:
The efficacy and safety of erlotinib in combination with anlotinib or bevacizumab monoclonal antibody in the treatment of EGFR-sensitive mutant advanced NSCLC are comparable.

Comparison of the Application of Protein Succinylate Iron Oral Solution Combined with Rosuvastatin or rhEPO in Non-Dialysis Renal Anemia Patients

Objective:
To compare the application effects of protein succinylate iron oral solution combined with rosuvastatin or combined with recombinant human erythropoietin (rhEPO) in non-dialysis renal anemia patients.
Methods:
A total of 120 inpatient and outpatient non-dialysis renal anemia patients admitted to our hospital from April 2020 to October 2022 were selected and divided into two groups using a simple randomization method. The control group was treated with protein succinylate iron oral solution combined with rhEPO, while the observation group was treated with protein succinylate iron oral solution combined with rosuvastatin. Differences in renal function indicators, anemia indicators, and iron metabolism indicators between the two groups were examined, and the incidence of adverse reactions was recorded.
Results:
There was no significant difference in renal function related factors between the two groups before and after treatment (P>0.05). There was no significant difference in renal function index between observation group and control group before and after treatment (P>0.05).There was no significant difference in anemia-related indicators between the two groups before treatment (P>0.05). Compared with before treatment, hemoglobin (Hb) and erythrocyte specific volume (HCT) of the two groups were increased after treatment, and the difference of Hb and HCT before and after treatment of the observation group was greater than that of the control group (P<0.05), and the immature reticulum ratio (IRF) between the two groups before and after treatment had no significant difference (P>0.05). There was no significant difference in iron metabolism between the two groups before treatment (P>0.05). Compared with the control group before treatment, the saturation of ferritin and transferrin in the two groups increased after treatment, and the total iron binding capacity decreased after treatment. The difference of iron metabolism indexes in the observation group before and after treatment was greater than that in the control group (P<0.05). The cumulative incidence of adverse reactions in the observation group was 11.67% (7/60) compared with 10.00% (6/60) in the control group, with no significant difference (P>0.05).
Conclusion:
The treatment of non-dialysis renal anemia with protein succinylate iron oral solution combined with rosuvastatin shows better improvement in anemia and iron metabolism indicators than the combination with rhEPO. At the same time, it ensures safety and is worthy of clinical recommendation.

Clinical Observation on the Efficacy of Metoprolol Sustained-Release Tablets Combined with Sacubitril Valsartan Tablets in the Treatment of Hypertension Complicated with Heart Failure

Objective:
To explore the clinical efficacy of metoprolol succinate sustained-release tablets combined with sacubitril valsartan tablets in the treatment of hypertension complicated with heart failure.
Methods:
A total of 135 patients with hypertension complicated with heart failure admitted to our hospital from January 2020 to January 2023 were selected. According to the order of admission, they were divided into the Bisoprolol Group (odd numbers, conventional treatment + metoprolol sustained-release tablets) with 68 cases and the Sacubitril Group (even numbers, based on the Bisoprolol Group + sacubitril valsartan tablets) with 67 cases, using single and double numbers. The treatment effects, blood pressure, heart rate, cardiac function indicators, vascular endothelial function, and levels of cardiac troponin I (cTnI) and N-terminal pro-brain natriuretic peptide (NT-BNP) before and after treatment were compared between the two groups. Adverse reactions during the treatment period were recorded.
Results:
The total effective rate in the Sacubitril Group was higher than that in the Bisoprolol Group (97.01% vs 86.76%) (P<0.05). After treatment, both groups showed a significant reduction in systolic and diastolic blood pressure, heart rate, left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), endothelin (ET), cTnI, and NT-BNP (P<0.05). Left ventricular ejection fraction (LVEF), nitric oxide (NO), and calcitonin gene-related peptide (CGRP) increased (P<0.05). The differences before and after treatment between the two groups were statistically significant (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05).
Conclusion:
The combination of metoprolol sustained-release tablets and sacubitril valsartan sodium is significantly effective in treating hypertension with heart failure. This treatment can effectively control blood pressure, improve cardiac function and vascular endothelial function, and reduce levels of cTnI and NT-BNP. It is worthy of promotion and application in clinical practice.

Effect of Low Molecular Weight Heparin Calcium on the Expression Levels of Serum sFlt-1 and PAPP-A in Early-Onset Severe Preeclampsia Patients

Objective:
To explore the therapeutic mechanism of Nicardipine combined with low molecular weight heparin calcium by detecting the expression levels of soluble fms-like tyrosine kinase 1 (sFlt-1) and pregnancy-associated plasma protein-A (PAPP-A) in patients with early-onset severe preeclampsia.
Methods:
Sixty-four pregnant women with early-onset severe preeclampsia, voluntarily opting for vaginal delivery, were selected from Chengde Maternal and Child Health Hospital between April 2021 and April 2022, with gestational age ranging from 26 to 34 weeks. The patients were randomly divided into a study group and a control group, with 32 cases in each group. The control group received Nicardipine treatment, while the study group received a combination of Nicardipine and low molecular weight heparin calcium. Peripheral blood samples were collected before treatment and at 1 week of treatment. Enzyme-linked immunosorbent assay (ELISA) was used to detect serum levels of sFlt-1, placental growth factor (PLGF), and PAPP-A. Peripheral blood routine and renal function were also examined. The average gestational age at delivery was recorded.
Results:
Before treatment, there were no significant differences in the levels of sFlt-1, PLGF, PAPP-A, packed cell volume (PCV), creatinine (Cr), and blood urea nitrogen (BUN) between the two groups (P>0.05). Compared with pre-treatment levels, after treatment, both groups showed a decrease in sFlt-1, PCV, Cr, and BUN levels, and an increase in PLGF and PAPP-A levels, with statistically significant differences (P<0.05). When comparing the study group with the control group after treatment, the study group exhibited a significant decrease in sFlt-1, PCV, Cr, and BUN levels, and a significant increase in PLGF and PAPP-A levels (P<0.05). The average gestational age at delivery in the study group was significantly higher than that in the control group, with a statistically significant difference (P<0.05).
Conclusion:
Nicardipine combined with low molecular weight heparin calcium contributes to the recovery of placental function, improves renal function, delays disease progression, optimizes pregnancy indicators, prolongs gestational weeks, and effectively treats patients with early-onset severe preeclampsia.

Analysis on the Epidemic Characteristics and Treatment of Chickenpox Public Health Emergencies in Chengde City from 2006 to 2021

Objective:
To analyze the epidemic characteristics and treatment effect of varicella public health emergencies in Chengde City in recent 15 years, and to provide scientific basis for improving the comprehensive ability to deal with varicella outbreaks.
Methods:
The public health emergencies of varicella reported in Chengde City from 2006 to 2021 were analyzed descriptively.
Results:
A total of 70 varicella public health emergencies were reported in Chengde from 2006 to 2021, accounting for 43.48% of the public health emergencies of infectious diseases in the same period. The number of reported cases was 2131, with an average incidence rate of 3.47% and a sex ratio of 1.34∶1. The age of onset was mainly from 7 to 12 years old, accounting for 85.83%. 28.53% of the cases received varicella vaccine; the occurrence was seasonal and bimodal, mainly concentrated in April-June (44.29%) and October-December (45.71%). There were significant differences in the reports of public health emergencies involving varicella in the 11 counties (cities and districts) of Chengde City. Chengde County had the largest number of reported incidents (17 cases, accounting for 24.29%), while Longhua County had no related incidents. All 70 incidents occurred in schools, mainly in rural primary schools (37 cases, 52.86%). The average reporting time was 6 days and the longest was 43 days. The average event duration was 20 days, and the reporting timeliness was positively correlated with the event duration (r=0.362, P<0.05).
Conclusion:
Most of the public health emergencies of chickenpox occurred in schools and rural primary schools in Chengde City in the past 15 years, and the number of people vaccinated with chickenpox vaccine was relatively low. It is suggested to strengthen the prevention and control of chickenpox epidemic in rural primary schools, improve the active and passive surveillance capabilities and the sensitivity of epidemic detection, improve the level of chickenpox vaccination among school-age children, and establish an effective immune barrier.
