1761 Effect of Pristimerin on Cognitive Function and Neuroinflammation in Ischemic Stroke Rats by Regulating the cGAS/STING Signaling Pathway

Objective: To investigate the effects of pristimerin (PT) on cognitive function and neuroinflammation in ischemic stroke rats by modulating the cyclic GMP-AMP synthase-stimulator of interferon genes (cGAS/STING) signaling pathway. Methods: The middle cerebral artery occlusion (MCAO) model was established by using the filament occlusion method.Rats were randomly assigned to the control group,MCAO group,low,medium,and high-dose PT groups (PT-L group,PT-M group,PT-H group),and PT-H+DMXAA group (PT-H+DMXAA group).Zea-Longa score was employed to evaluate the neurological behavior of rats.The Morris water maze was used to assess the cognitive ability of the rats.Cerebral infarction area was calculated by using the 2,3,5-chlorophenyltetrazolium (TTC) staining method.Pathological changes in rat brain tissue were observed via hematoxylin and eosin (HE) staining.Enzyme-linked immunosorbent assay (ELISA) was performed to measure serum levels of interleukin-6 (IL-6) and interleukin-1β (IL-1β).Western blot analysis was utilized to determine the expression levels of M1 marker (iNOS),M2 marker (Arg-1),cGAS,and STING proteins in microglia of rat brain tissue. Results: When compared to the control group,rats in the MCAO group exhibited significantly increased neurological deficit scores,prolonged escape latencies,larger cerebral infarction areas,elevated IL-6 and IL-1β levels,and higher expression of iNOS,cGAS,and STING.The expression of Arg-1 decreased significantly (P<0.05).In comparison to the MCAO group,rats in the PT groups showed significantly reduced neurological deficit scores,escape latencies,cerebral infarction areas,IL-6 and IL-1β levels,as well as the expression of iNOS,cGAS,and STING.The expression of Arg-1 increased significantly (P<0.05) in a dose-dependent manner.In comparison to the PT-H group,rats in the PT-H+DMXAA group had significantly higher neurological deficit scores,prolonged escape latencies,larger cerebral infarction areas,increased IL-6 and IL-1β levels,and higher expression of iNOS,cGAS,and STING.The expression of Arg-1 decreased significantly (P<0.05). Conclusion: Pristimerin can mitigate cognitive impairment and neuroinflammatory damage in ischemic stroke rats.Its mechanism of action may be associated with the inhibition of the cGAS/STING signaling pathway.

1767 Impact of Geniposide on Angiogenesis in Steroid-Induced Femoral Head Necrosis Rats by Regulating the HIF-1α/VEGF Signaling Pathway

Objective: To investigate the impact of geniposide (GEN) on angiogenesis in rats with steroid-induced femoral head necrosis via the regulation of hypoxia inducible factor-1α (HIF-1α)/vascular endothelial growth factor (VEGF) signaling pathway. Methods: 18 rats were randomly marked a blank control group (NC group),while the remaining rats were used to create a rat model of glucocorticoid-induced osteonecrosis of the femoral head (GIONFH).The rats in the GIONFH group were further randomized into the following groups:GIONFH group,GEN group (gavaged with 50mg/kg of GEN),IDF 11774 group (gavaged with 30mg/kg of HIF-1α/VEGF signaling pathway inhibitor IDF 11774),and GEN+IDF 11774 group (gavaged with 50mg/kg GEN and 30mg/kg IDF 11774).GEN and IDF 11774 were administered by gavage daily for four months,with 18 rats in each group.The GIONFH and NC groups were given an equal volume of physiological saline by gavage.Angiography and micro CT scanning were performed on the femoral heads.Serum bone metabolism indicators were measured using the ELISA method.Histological changes in the femoral tissue were assessed through HE staining.Immunohistochemical staining was conducted to evaluate the expression of CD31 and vWF in the femoral tissue.Western blot analysis was performed to examine the protein expression of the HIF-1α/VEGF signaling pathway in femoral tissue. Results: The GIONFH group exhibited severe damage to femoral head blood vessels,disrupted bone surface structures,extensive cavities,thinning of the femoral head cartilage layer,disordered bone arrangement,and increased cavities.Compared to the NC group,the GIONFH group showed significantly reduced vascular volume,BV/TV,BALP,BGP content,CD31,vWF positive area percentage,HIF-1α,and VEGF protein levels (P<0.05),while CTX-I content significantly increased (P<0.05).In contrast,the GEN group provided substantial protection to the femoral head blood vessels,resulting in smoother femoral head surfaces with intact structures and no observable internal abnormalities.The vascular volume,BV/TV,BALP,BGP content,CD31,vWF positive area percentage,HIF-1α,and VEGF protein levels were significantly elevated (P<0.05),and the CTX-I content markedly decreased (P<0.05) compared to the GIONFH group.These effects were reversed in the IDF 11774 group. Conclusion: Geniposide (GEN) may improve angiogenesis in GIONFH rats by activating the HIF-1α/VEGF signaling pathway.

1772 Fuling Sini Decoction Inhibits TLR4/NF-κB Pathway and Improves Cardiac Function in Rats with Chronic Heart Failure

Objective: To investigate the influences of Fuling Sini Decoction on cardiac function in rats with chronic heart failure and the possible molecular mechanism. Methods: The rat model of chronic heart failure was established using intraperitoneal injection of doxorubicin.Rats that were successfully modeled were randomly divided into the model group,low,medium,and high-dose Fuling Sini Decoction groups (6 g/kg,12 g/kg,18 g/kg),and a captopril group (2.25mg/kg),with 12 rats in each group.An additional 12 rats were intraperitoneally injected with normal saline as the control group.Cardiac function indicators were assessed using echocardiography; serum levels of tumor necrosis factor-α (TNF-α),interleukin (IL)-1β,and IL-6 were measured using ELISA; myocardial tissue histopathological changes were observed using HE staining; cardiomyocyte apoptosis rates were determined through TUNEL staining; Western blot was employed to measure the expression levels of Toll-like receptor 4 (TLR4),nuclear factor-κB (NF-κB),p-NF-κB,nuclear factor κB inhibitory protein α (IκBα),and p-IκBα proteins in myocardial tissue. Results: Compared to the control group,rats in the model group exhibited increased left ventricular ejection fraction (LVEF) and left ventricular short-axis shortening rate (LVFS) (P<0.05),as well as increased left ventricular end-systolic diameter (LVESD),left ventricular end-diastolic diameter (LVEDD),serum levels of TNF-α,IL-1β,and IL-6.The degree of myocardial tissue pathological injury worsened,and there was an increase in cardiomyocyte apoptosis rates and the expression levels of TLR4,p-NF-κB/NF-κB,and p-IκBα/IκBα proteins in myocardial tissue (P<0.05).Compared to the model group,rats in the low,medium,and high-dose Fuling Sini Decoction groups and the captopril group exhibited increased LVEF and LVFS (P<0.05),decreased LVESD,LVEDD,serum TNF-α,IL-1β,and IL-6 levels,and reduced pathological damage in myocardial tissue.There was also a decrease in cardiomyocyte apoptosis rates and the expression levels of TLR4,p-NF-κB/NF-κB,and p-IκBα/IκBα proteins in myocardial tissue (P<0.05). Conclusion: Fuling Sini Decoction can inhibit the activation of the TLR4/NF-κB pathway,reduce myocardial damage,and improve cardiac function in rats with chronic heart failure.

1777 Impact of LncRNA MAFG-AS1 on the Malignant Biological Behavior of Gastric Cancer Cells by Regulating the miR-331-3p/SERPINE1 Axis

Objective: To investigate the impact of the long non-coding RNA (LncRNA) recombinant human v-maf musculoaponeurotic fibrosarcoma oncogene homolog G antisense RNA1 (MAFG-AS1) on the malignant biological behavior of gastric cancer cells by regulating the miR-331-3p/SERPINE1 axis. Methods: QRT-PCR was applied to detect the expression of MAFG-AS1,miR-331-3p,and SERPINE1 in gastric cancer tissues (n=30),paracancerous tissues (n=30),human gastric mucosa normal epithelial cells GES-1,human gastric cancer cells BGC-823,SGC-7901,and MGC-803; BGC-823 cells were randomly grouped into ctrl group,pcDNA3.1 group,pcLncRNA MAFG-AS1 group,pcLncRNA MAFG-AS1+miR-NC group,and pcLncRNA MAFG-AS1+miR-331-3p group; qRT-PCR was applied to detect the levels of MAGG-AS1,miR-331-3p,and SERPINE1 in BGC-823 cells in each group; CCK-8 method was applied to detect the activity of BGC-823 cells in each group; EDU method was applied to detect the proliferation of BGC-823 cells in each group; Transwell was applied to detect the migration and invasion of BGC-823 cells in each group; Western blot was applied to detect the expression of SERPINE1,PCNA,and MMP-2 in BGC-823 cells in each group; Double Luciferase reporter gene experiment was applied to verify the relationship between MAFG-AS1 and miR-331-3p,miR-331-3p and SERPINE1. Results: In comparison to adjacent tissues,gastric cancer tissues exhibited significantly increased expressions of MAFG-AS1 and SERPINE1 mRNA (3.01±0.37 vs 1.02±0.12 and 2.49±0.26 vs1.01±0.10; t=28.022 and 29.000; P<0.05) and markedly decreased miR-331-3p expression (0.32±0.03 vs 1.03±0.07; t=51.063; P<0.05).Furthermore,various gastric cancer cells displayed notably higher MAFG-AS1 and SERPINE1 mRNA expressions and lower miR-331-3p expression in comparison to GES-1 cells (P<0.05).Among the gastric cancer cell lines,BGC-823 cells exhibited the most significant changes,so we selected BGC-823 cells for subsequent experiments.Compared to the control group and pcDNA3.1 group,the pcLncRNA MAFG-AS1 group showed significantly higher levels of MAFG-AS1,SERPINE1 mRNA,cell viability (A value),EDU-positive cell rate,numbers of migration and invasion cells,as well as SERPINE1,PCNA,and MMP-2 protein levels,while miR-331-3p mRNA levels were significantly lower (P<0.05).In comparison to the pcLncRNA MAFG-AS1 group and the pcLncRNA MAFG-AS1+miR-NC group,the pcLncRNA MAFG-AS1+miR-331-3p group showed significantly lower SERPINE1 mRNA levels,cell viability (A value),EDU-positive cell rate,numbers of migration and invasion cells,as well as SERPINE1,PCNA,and MMP-2 protein levels,whereas miR-331-3p mRNA levels were significantly higher (P<0.05).MAFG-AS1 negatively regulated miR-331-3p expression,and miR-331-3p negatively regulated SERPINE1 expression. Conclusion: Overexpression of LncRNA MAFG-AS1 may upregulate SERPINE1 expression by negatively regulating miR-331-3p,thereby promoting proliferation,migration and invasion of BGC-823 cells.

1784 Mechanistic Study of miR-125a-5p Targeting EVA1A Induced by Helicobacter pylori Infection Affecting Proliferation and Invasion in Gastric Cancer

Objective: To identify miRNAs regulated by Helicobacter pylori infection and their mechanism of action in gastric cancer. Methods: Expression levels of miR-125a-5p and EVA1A were assessed using qRT-PCR in Hp-infected patient tissues.The biological functions of the miR-125a-5p/EVA1A axis in gastric cancer cell proliferation,migration and invasion were investigated using cell functional experiments after Hp infection.The binding relationship between miR-125a-5p and EVA1A was confirmed through a dual luciferase assay. Results: miR-125a-5p was down-regulated in Hp-infected patient tissues and Hp-infected gastric cancer cells.Functional experiments revealed that the overexpression of miR-125a-5p could inhibit gastric cancer cell proliferation and invasion.The targeting of miR-125a-5p down-regulated the expression of EVA1A,and overexpressing EVA1A could reverse the effects of miR-125a-5p overexpression on gastric cancer phenotype. Conclusion: Hp-induced miR-125a-5p,which targets EVA1A,inhibits the proliferation and invasion of gastric cancer.MiR-125a-5p and EVA1A may represent potential therapeutic targets for gastric cancer.

1789 The Metastasis Mechanism of Colon Cancer Cells Through the Regulation of HuR by MiR-155

Objective: To explore the mechanism of miR-155 involved in colon cancer cell metastasis through the regulation of HuR. Methods: HCT-116 colon cancer cell lines were used in the research.They were categorized into groups with miR-155 mimics,miR-155 inhibitors,miRNA mimics as a negative control,and miRNA inhibitors as a negative control.Cell viability,cell cycle,migration,and apoptosis were assessed using MTT,wound healing tests,colony formation assays,cell cycle analysis,and flow cytometry.The expression of miR-155 in cells was measured using qPCR and western blotting.The expression of HuR in cells was also measured using western blotting.Relevant targets were determined using a luciferase reporter gene. Results: Compared to the control group,the level of miR-155 increased after transfection with miR-155 mimics.After 72 hours of transfection with miR-155 mimics,cell activity and colony formation increased.The percentage of S-phase cells increased,and apoptosis was inhibited by miR-155 mimics in colon cancer cells.MiR-155 mimics also increased cell mobility,indicating they promoted the migration of colon cancer cells.These results were reversed after transfection with miR-155 inhibitors.The mRNA level of HuR decreased after transfection with miR-155 inhibitors and increased after transfection with miR-155 mimics.The luciferase reporter gene confirmed the binding of miR-155 to the 3'UTR of HuR,establishing HuR as a direct target of miR-155. Conclusion: The findings suggest that miR-155 promotes colon cancer cell proliferation,enhances colony formation,drives the cell cycle process,inhibits apoptosis,and increases cell migration.HuR is identified as a new target of miR-155.

1795 Mechanistic Study on the Regulation of Malignant Biological Activity of JAG1 Pathway in Hepatocellular Carcinoma Cells through miR-338-3p by Circ_0007841

Objective: To explore the regulatory mechanism underlying the malignant biological activity of hepatocellular carcinoma (HCC). Methods: Human-derived normal hepatic epithelial cells THLE-3 and HCC cell lines,including BEL-7405,SNU-387,MHCC-97H,HCCLM3,and Huh7,were used.Huh7 cells were transfected with si-0007841/NC or miR-338-3p inhibitor/NC.Expression levels of circ_0007841,miR-338-3p,and JAG1 were analyzed using qRT-PCR.Proliferation viability,migration,and invasion ability were assessed using CCK-8 and transwell assays.Predictions of the targeting relationships between circ_0007841 and miR-338-3p,as well as miR-338-3p and JAG1,were performed using Circular RNA Interactome and miRWalk datasets.Dual luciferase reporter assays confirmed the target binding. Results: Circ_0007841 was found to be highly expressed in HCC.Inhibiting circ_0007841 expression led to a decrease in proliferation viability and a reduction in migration and invasion abilities of HCC.Further inhibition of miR-338-3p expression partially reversed the inhibitory effects on malignant behavior by low expression of circ_0007841.In HCC,circ_0007841 could target and inhibit the expression of miR-338-3p,while miR-338-3p could target and inhibit the expression of JAG1. Conclusion: High expression of circ_0007841 in hepatocellular carcinoma enhances JAG1 transcription by competitively binding with miR-338-3p,ultimately promoting malignant biological activities in HCC cells.

1800 The Effect of YAP1 and RNA Molecular m6A Modification on Chemotherapy Sensitivity of Gastric Cancer

Objective: To investigate the effect of the interaction between Yes-associated protein 1 (YAP1) and RNA molecule N6-methyladenosine (m6A) modification on the chemosensitivity of gastric cancer (GC). Methods: Cisplatin-resistant AGS/DDP cells and their cisplatin-sensitive parental AGS cells were used as cell models.The expression of methyltransferase-like 3 (METTL3) and YAP1 proteins was analyzed using immunoblotting,and the extent of m6A modification was determined by Poly(A) RNA dot blotting.The influence of METTL3-induced m6A modification on YAP1 translation was assessed using a luciferase reporter gene.The impact of cisplatin treatment on apoptosis in METTL3- or YAP1-knockout AGS/DDP cells was examined by Tunel.A mouse xenograft model was established to study the effect of METTL3 knockout on cisplatin sensitivity in vivo. Results: The expression of m6A methyltransferase METTL3 was elevated in AGS/DDP cells,a cisplatin-resistant subline of GC cells,in comparison to the parental cells.METTL3 increased the m6A modification of YAP1 mRNA in AGS/DDP cells,enhancing its translation efficiency.Conversely,knockout of METTL3 in AGS/DDP cells reduced YAP1 expression and restored cisplatin sensitivity.Cisplatin treatment following METTL3 knockout induced apoptosis in AGS/DDP cells and reduced tumor growth derived from AGS/DDP cells in BALB/c nude mice. Conclusion: METTL3-induced m6A modification plays a significant role in acquired cisplatin resistance by promoting YAP1 upregulation,suggesting that METTL3 may be a potential candidate for targeted chemotherapy in GC.

1805 Therapeutic Effects of American Ginseng Total Saponins on Stress Urinary Incontinence via Modulation of the Nrf2-ARE Pathway

Objective: To explore the therapeutic effect of total saponins of American ginseng on stress urinary incontinence by regulating Nrf2/ARE pathway. Methods: A stress urinary incontinence rat model was established.Sixty rats were randomly assigned to the following groups:Control group,Stress Urinary Incontinence (SUI) group,and SUI groups treated with low,medium,and high doses of American Ginseng Total Saponins (PQSL,PQSM,PQSH).Additionally,one SUI group received Nrf2-specific inhibitor ML385 (PQSH+ML385).Each group consisted of 10 rats.The positive sneezing rates were recorded.Urodynamic parameters,including MBC,LPP,ALPP,as well as intravesical pressure (voided volume,residual volume,voiding efficiency) were measured.Masson staining was used to assess collagen fiber levels in urethral tissues.Oxidative stress indicators (MDA,SOD,GSH-Px) were determined in urethral tissues.Western blot analysis was performed to assess the expression of Nrf2,HO-1,and NQO1 in brain tissue. Results: In the Control group,sneezing tests were consistently negative.In the SUI group,all 50 rats showed positive sneezing test results.The positive sneezing rates in PQSL,PQSM,and PQSH groups were 60% (6/10),40% (4/10),and 20% (2/10),respectively,whereas in the PQSH+ML385 group,the rate was 70% (7/10),and in the SUI group,it was 80% (8/10).Compared to the Control group,the SUI group exhibited reduced MBC,LPP,ALPP levels,urinary efficiency,GSH-Px content,and SOD activity,along with increased urine volume,residual volume,and MDA content (P<0.05).In comparison with the SUI group,the PQSL,PQSM,and PQSH groups demonstrated elevated MBC,LPP,ALPP levels,urinary efficiency,GSH-Px content,and SOD activity,and reduced urine volume,residual volume,and MDA content,with effects showing concentration-dependent patterns (P<0.05).When compared to the PQSH group,the PQSH+ML385 group displayed decreased MBC,LPP,ALPP levels,urinary efficiency,GSH-Px content,and SOD activity,alongside increased urine volume,residual volume,and MDA content (P<0.05).Compared to the Control group,the SUI group showed significantly decreased expression of Nrf2,HO-1,and NQO1 in urethral tissue (P<0.05).In contrast,the PQSL,PQSM,and PQSH groups exhibited significantly increased expression of Nrf2,HO-1,and NQO1 in urethral tissue,with effects being dose-dependent (P<0.05).When compared to the PQSH group,the PQSH+ML385 group showed significantly decreased expression of Nrf2,HO-1,and NQO1 in urethral tissue (P<0.05). Conclusion: American Ginseng Total Saponins may improve urinary function in stress urinary incontinence by reducing oxidative stress in the urethra.This effect appears to be associated with the activation of the Nrf2-ARE signaling pathway.

1811 Correlation between KRT17 and KLF4 Expression in Cervical Cancer Tissue and Patient Prognosis

Objective: To investigate the correlation between the expression of Keratin 17 (KRT17) and Kruppel-like factor 4 (KLF4) in cervical cancer (CC) tissue and the prognosis of patients. Methods: From April 2015 to April 2019,86 patients diagnosed with CC in the Department of Obstetrics and Gynecology at our hospital were included in this study.Tissue samples were collected from the cancerous areas (CC group) and adjacent non-cancerous areas (adjacent group).The expressions of KRT17 and KLF4 were assessed using immunohistochemistry.Kaplan-Meier analysis was employed to examine the relationship between KRT17,KLF4 expression,and the prognosis of CC patients.Additionally,COX regression analysis was conducted to identify risk factors influencing the prognosis of CC patients. Results: In comparison to the adjacent group,the CC group exhibited a significantly higher rate of positive KRT17 expression and a notably lower rate of positive KLF4 expression (P<0.05).The expression levels of KRT17 and KLF4 were found to be associated with the depth of myometrial invasion,lymph node metastasis,and the degree of differentiation (P<0.05).Kaplan-Meier analysis revealed that within the KRT17 high-expression group,patients had a lower 3-year cumulative survival rate compared to the low-expression group.Conversely,the 3-year cumulative survival rate in the KLF4 high-expression group was higher than in the low-expression group (P<0.05).Multivariate COX regression analysis demonstrated that myometrial invasion depth,lymph node metastasis,KRT17,and KLF4 expression were significant risk factors affecting the prognosis of CC patients (P<0.05). Conclusion: In the CC group,there was a significant increase in the rate of positive KRT17 expression and a significant decrease in the rate of positive KLF4 expression.These findings are closely related to clinicopathological characteristics and hold valuable implications for evaluating the prognosis of CC patients.

Objective: To analyze the variations in serum microRNA (miR-499) and miR-144-3p among patients with advanced non-small cell lung cancer (NSCLC) of epidermal growth factor receptor (EGFR) and anaplastic lymphoma kinase (ALK) wild-type,and evaluate their predictive value for prognosis. Methods: Clinical data were collected from 107 patients with EGFR/ALK wild-type advanced NSCLC who were admitted to the hospital between January 2018 and 2020 (NSCLC group).Additionally,107 healthy subjects who underwent physical examinations during the same period were selected as the control group.Real-time fluorescence RCR technology was employed to measure the expression of serum miR-499 and miR-144-3p.Differences between the two groups were compared,and changes in serum miR-499 and miR-144-3p expressions were assessed across patients with advanced NSCLC exhibiting various clinicopathological characteristics.Follow-up began at discharge and ended in January 2023,considering death as the endpoint event.The prognostic survival rate was compared between advanced NSCLC patients with high and low expressions of miR-499 and miR-144-3p,using an expression level greater than the average value as the criterion. Results: In the NSCLC group,relative expression levels of serum miR-499 [(0.74±0.18) vs (1.05±0.20)] and miR-144-3p [(0.82±0.19) vs (1.22±0.21)] were significantly lower compared to the control group (P<0.05).Among patients with advanced NSCLC,the relative expression levels of serum miR-499 [(0.64±0.17) vs (0.86±0.21)] and miR-144-3p [(0.71±0.16) vs (0.95±0.22)] were lower in stage IV patients compared to stage ⅢB and ⅢC patients (P<0.05).Furthermore,patients with lower differentiation exhibited lower expression levels compared to those with moderate-to-high differentiation [(0.63±0.16) vs (0.85±0.21),(0.70±0.15) vs (0.94±0.22),P<0.05].The follow-up duration for advanced NSCLC patients ranged from 4 to 56 months,with a median follow-up period of 34 months.Patients with high serum miR-499 expression had a significantly higher survival rate compared to those with low expression (P<0.05),as did patients with high serum miR-144-3p expression (P<0.05). Conclusion: Serum miR-499 and miR-144-3p exhibit low expression in patients with EGFR/ALK wild-type advanced NSCLC.Furthermore,their expression levels are even lower in stage IV patients and those with lower differentiation.Importantly,these expression levels hold predictive value for prognostic survival rates.

1821 Correlation between Peripheral Blood Levels of miR-30a miR-24 and Sestrin2 and the Extent of Myocardial Injury in Acute Myocardial Infarction Patients

Objective: To explore the correlation between the expression levels of miR-30a,miR-24 and Sestrin2 in peripheral blood and the degree of myocardial injury in AMI patients in order to evaluate its potential clinical value in myocardial infarction. Methods: The study included 180 patients who were diagnosed with acute ST-segment elevation myocardial infarction between January 2020 and January 2023.A control group consisting of 100 age-matched individuals without cardiac conditions was also established.The study compared the peripheral blood levels of miR-30a,miR-24,and Sestrin2 between the two groups.Coronary angiography was performed on the patients to assess the extent of myocardial injury.Patients in the study group were categorized into mild and severe groups based on the severity of myocardial injury.The study also analyzed the correlation between peripheral blood miR-30a,miR-24,and Sestrin2 expression and levels of cardiac markers (CK,CK-MB,LDH,and hs-cTnI). Results: Compared to the control group,patients in the study group showed increased relative expression levels of miR-30a and Sestrin2,and decreased relative expression of miR-24 (P<0.05).Within the study group,patients in the severe myocardial injury group exhibited higher relative expression levels of miR-30a and Sestrin2,and lower relative expression of miR-24 compared to the mild group (P<0.05).Additionally,CK,CK-MB,LDH,and hs-cTnI levels were elevated in the severe group when compared to the mild group (P<0.05).Peripheral blood miR-30a and Sestrin2 levels were positively correlated with levels of CK,CK-MB,LDH,and hs-cTnI,while miR-24 expression was negatively correlated with these cardiac markers (P<0.05).Using the degree of myocardial injury as a variable,the study analyzed the diagnostic value of peripheral blood levels of miR-30a and miR-24 as well as Sestrin2.The AUC values for each index were 0.942,0.989,and 0.862,respectively. Conclusion: In patients with acute myocardial infarction,those with severe myocardial injury had higher levels of miR-30a and Sestrin2 and lower levels of miR-24.The study also revealed a significant correlation between the expression of peripheral blood miR-30a,miR-24,and Sestrin2 and the extent of myocardial injury.These findings suggest that these markers could serve as potential biomarkers for assessing the degree of myocardial injury in AMI patients,providing a basis for risk stratification and guiding clinical treatment decisions in AMI patients.

1826 Value Analysis of Serum NSE and Urinary VMA Levels Combined with Blood Cell Analysis to Evaluate the Prognosis of Children with Neuroblastoma

Objective: To analyze the combined levels of serum Neuron-Specific Enolase (NSE),urinary Vanillylmandelic Acid (VMA),and blood cell parameters for evaluating the prognosis of children with neuroblastoma. Methods: 101 children with neuroblastoma treated at our hospital from January 2018 to March 2021 were divided into a survival group and a death group based on the 2-year follow-up outcomes,consisting of 69 cases in the survival group and 32 cases in the death group.General clinical data,laboratory index levels,platelet and red blood cell parameters were compared between the two groups.Additionally,the laboratory index levels were compared among children with different clinical stages. Results: The comparison of general clinical data between the two groups revealed no significant differences in age,gender,primary tumor site,pathological type,and mean platelet volume (P>0.05).In the survival group,there was a lower proportion of children in stages Ⅲ~Ⅳ,N-myc gene positivity,high-risk category,bone marrow metastasis,NSE,serum ferritin (SF),lactate dehydrogenase (LDH),C-reactive protein (CRP),24-hour urinary VMA levels,platelet distribution width,and red blood cell distribution width compared to the death group,with statistically significant differences (P<0.05).Children in stages Ⅰ~Ⅱ had significantly lower levels of NSE,SF,LDH,CRP,and 24-hour urinary VMA compared to children in stages Ⅲ~Ⅳ,with statistically significant differences (P<0.05).Platelet crit was higher in the survival group than in the death group with statistical significance (P<0.05).Logistic regression analysis indicated that the stage,NSE,24-hour urinary VMA,and platelet distribution width were influencing factors for the prognosis of children with neuroblastoma (P<0.05).NSE (ng/mL),platelet distribution width (%),and 24-hour urinary VMA measurements had AUC values for predicting the prognosis of children with neuroblastoma,which were 0.862 (0.791~0.932),0.838 (0.743~0.934),and 0.748 (0.641~0.856),respectively.The corresponding cutoff values were 45.565,10.375,and 51.405,with sensitivities of 0.823,0.781,and 0.711,specificities of 0.841,0.790,and 0.681,and Youden indices of 0.664,0.571,and 0.392,respectively. Conclusion: VMA,NSE,and platelet distribution width are associated with the prognosis of children with neuroblastoma.The combination of these parameters has good clinical value in predicting the prognosis of children with neuroblastoma.

1832 Combination of Hemodialysis and Hemoperfusion for the Treatment of End-Stage Chronic Kidney Disease Patients and Its Impact on Microinflammatory Response and Renal Function

Objective: To observe the effects of the combination of hemodialysis and hemoperfusion on micro inflammatory response and renal function in end-stage patients with chronic kidney disease (CKD). Methods: Ninety end-stage CKD patients admitted to our hospital from May 2020 to May 2022 were randomly assigned into an observation group and a control group (45 patients in each group).The control group received only hemodialysis treatment,while the observation group received combined hemoperfusion therapy in addition to hemodialysis.After the treatment,calcium and phosphorus metabolism indicators,renal function,microinflammatory response,and oxidative stress response were compared between the two groups. Results: After 6 months of treatment, the blood phosphorus and calcium phosphorus product levels in the observation group were lower than those in the control group [(1.24±0.29) mmoL/L ratio (1.85 ± 0.20) mmoL/L, (2.49±0.53) mmoL2/L2 ratio (3.75±0.59) mmoL2/L2], with a significant difference (P<0.05), while there was no significant difference (P>0.05) in blood calcium levels between the two groups [(2.01±0.41) mmoL/L ratio (2.03±0.47) mmoL/L]. The levels of UA, Scr, and BUN in the observation group were lower than those in the control group [(319.74±23.73)] μmoL/L ratio (376.25±25.86) μmoL/L, (244.06±16.35) μmoL/L ratio (286.59±20.82) μmoL/L and (15.87±4.26) mmoL/L was statistically significant (P<0.05) compared to (21.74±5.74) mmoL/L. Observation group micro inflammatory index TNF- α, The levels of IL-6 and hs CRP in the control group were lower than those in the control group [(75.15±6.02) ng/L ratio (80.82±6.51) ng/L, (25.48±4.65) ng/L ratio (31.29±5.28) ng/L, (6.24±2.05) mg/L ratio (9.87±2.24) mg/L], with statistically significant differences (P<0.05). The SOD level in the observation group was higher than that in the control group ](73.54±7.28) U/mL vs (66.29±6.87) U/mL], with a significant difference (P<0.05); The levels of MDA and MPO in the observation group were lower than those in the control group [(8.36±2.46) nmoL/mL vs (10.84±2.73) nmoL/mL, (56.29±5.29) U/L vs (61.74±6.73) U/L], with significant differences (P<0.05). Conclusion: The combination of hemodialysis and hemoperfusion for end-stage CKD patients is beneficial for regulating calcium and phosphorus metabolism,protecting renal function,and has a relatively small impact on microinflammatory status and oxidative stress response.This approach has significant clinical value.

1837 Correlation of Serum CTX-Ⅱ and HDAC3 Levels with Immune Function and Disease Activity in Patients with Rheumatoid Arthritis

Objective: To observe the changes in serum type Ⅱ collagen C-terminal peptide (CTX-Ⅱ) and histone deacetylase 3 (HDAC3) levels in patients with rheumatoid arthritis (RA),and to analyze their relationship with immune function and disease activity. Methods: A total of 84 RA patients (RA group) and 84 healthy people (NC group) admitted to our hospital were gathered.The changes in serum CTX-Ⅱ,HDAC3,immune function indexes [proportion of immunoglobulin (Ig) A,IgG,IgM,CD3+,CD4+,CD8+ T cells] and disease activity related indexes [C-reactive protein (CRP),erythrocyte sedimentation rate (ESR)] were analyzed.Pearson method was applied to analyze the correlation of serum CTX-Ⅱ and HDAC3 with immune indexes and disease activity-related indexes,respectively. Results: The levels of CTX-Ⅱ(9.26±1.84 vs 4.59±0.62) and HDAC3(83.72±13.51 vs 20.15±4.90) in the RA group were higher than those in the NC group (t=22.044,40.542,P<0.05).The levels of CTX-Ⅱ(7.37±1.71 vs 10.75±1.94) and HDAC3(72.97±12.10 vs 92.18±14.62) in the remission group were lower than those in the active group (t=8.347,7.940,P<0.05).The proportion of CD3+(64.42±6.91 vs 70.42±7.34) and CD4+(34.80±5.96 vs 39.74±5.80) T cells in the RA group was lower than that in the NC group,while the levels of CD8+(31.68±6.15 vs 29.62±7.15) T cells,IgM(2.06±0.39 vs 1.47±0.30),IgA(2.96±0.50 vs 1.51±0.36),IgG(15.49±3.05 vs 11.83±2.19),ESR(28.32±7.60 vs 13.75±2.18),CRP(10.94±2.02 vs 6.08±0.73) were obviously higher than those in the NC group (P<0.05).The CD3+(62.94±6.60 vs 66.30±7.30) and CD4+(33.57±5.74 vs 36.36±6.24) T cells in the active group were lower than those in the remission group,and the levels of IgM(2.19±0.41 vs 1.89±0.36),IgA(3.08±0.54 vs 2.81±0.45),IgG(16.16±3.27 vs 14.64±2.77),ESR(30.06±8.15 vs 26.11±6.90),CRP(12.37±2.24 vs 9.12±1.74) were higher than those in the remission group (P<0.05).Correlation analysis showed that serum CTX-Ⅱ was positively correlated with ESR,CRP and disease activity score in 28 joints (DAS28) (r=0.572,0.495,0.809,all P=0.000); serum HDAC3 was negatively correlated with the proportions of CD3+ and CD4+ T cells,respectively (r=-0.529,-0.464,P=0.000),and was positively correlated with IgM,IgA,IgG,ESR,CRP and DAS28 (r=0.460,0.491,0.445,0.540,0.519,0.864,all P<0.05). Conclusion: Serum levels of CTX-Ⅱ and HDAC3 are elevated in RA patients compared to healthy individuals.CTX-Ⅱ is associated with disease activity in RA,while HDAC3 is related to immune dysfunction and disease activity.

1842 Expression and Survival Analysis of C1q-Like Protein 4 in Breast Cancer

Objective: To investigate the expression of C1ql4 in breast cancer patients,and to analyze the relationship between C1ql4 and relevant clinicopathological features and prognosis of breast cancer. Methods: The study examined the expression of C1ql4 protein using immunohistochemistry in 143 breast cancer patients,with 20 breast fibroadenoma tissues serving as controls.Clinical and pathological data were collected,and the survival and prognosis of patients were monitored.The association between C1ql4 expression and prognosis was analyzed using Kaplan-Meier survival curves and multivariate Cox regression.Differences were considered statistically significant at P<0.05. Results: The positive expression rate of C1ql4 protein was 15% (3/20) in breast fibroadenoma and 98.6% (141/143) in breast cancer.In breast cancer tissues,C1ql4 low expression was observed in 47.6% (68/143) of cases,while high expression was found in 51.0% (73/143) of cases.C1ql4 expression was associated with various clinical factors,including tumor diameter,lymph node metastasis,TNM stage,and molecular subtypes (P<0.05).Patients in the high expression group of C1ql4 had a worse prognosis (P<0.05).Multivariate Cox regression analysis demonstrated that TNM stage,molecular subtyping,and high C1ql4 expression were independent risk factors for an unfavorable prognosis in breast cancer patients (P<0.05). Conclusion: C1ql4 is highly expressed in breast cancer tissues and is associated with prognosis and adverse pathological parameters.

1847 Clinical Efficacy of Arthroscopy Combined with High Tibial Osteotomy and Unicompartmental Knee Arthroplasty in the Treatment of Medial Compartmental Knee Osteoarthritis

Objective: To analyze the clinical efficacy of arthroscopy combined with high tibial osteotomy (HTO) and unicompartmental knee arthroplasty (UKA) in the treatment of medial compartmental knee osteoarthritis. Methods: Clinical data from 121 patients with medial compartmental knee osteoarthritis admitted to the First Affiliated Hospital of Xi'an Jiaotong University between January 2018 and January 2022 were retrospectively analyzed.Patients were divided into two groups:the HTO group (n=54) treated with arthroscopy combined with HTO,and the UKA group (n=67) treated with UKA.Clinical efficacy and perioperative factors were compared between the two groups.The incidence of postoperative complications was recorded,and knee function was assessed at 6 months and 1 year after surgery. Results: The UKA group showed significantly better clinical efficacy and a higher total effective rate (91.04% vs77.78%) compared to the HTO group (P<0.05).Patients in the UKA group had shorter postoperative ambulation times and hospital stays [(4.83±1.04) days vs (5.32±1.21) days,(10.76±1.55) days vs (11.34±1.18) days] than those in the HTO group (P<0.05).The UKA group also had a lower incidence of postoperative complications (7.46% vs 20.37%) compared to the HTO group (P<0.05).At 6 months and 1 year post-surgery,the UKA group had higher knee HSS scores [(80.27±5.86) points vs (76.42±7.89) points,(79.23±6.95) points vs (75.23±8.77) points] than the HTO group (P<0.05). Conclusion: Unicompartmental knee arthroplasty (UKA) demonstrates significant efficacy in the treatment of medial compartmental knee osteoarthritis.UKA is associated with fewer postoperative complications and better postoperative recovery.

1853 Effect of Three-Dimensional Visualization Reconstruction Technology Combined with Laparoscopic Surgery in the Treatment of Colorectal Cancer and Observation of Complication Incidence Rates

Objective: To investigate the impact of three-dimensional visualization reconstruction technology combined with laparoscopic surgery on the treatment of colorectal cancer and to observe the incidence rates of complications. Methods: Clinical data of 51 patients with colorectal cancer who underwent laparoscopic surgery at our hospital from March 2021 to March 2023 were retrospectively analyzed and included in the single group.Clinical data of 54 patients with colorectal cancer who underwent laparoscopic surgery combined with three-dimensional visualization reconstruction technology during the same period were analyzed and included in the combined group.Surgical indicators,short-term efficacy,gastrointestinal function,and stress responses (neutrophil (NE),white blood cell (WBC),cortisol (Cor)) were compared between the two groups before and 48 hours after surgery.The incidence of postoperative complications was compared between the two groups. Results: In comparison to the single group,the combined group exhibited shorter surgical and hospitalization times,lower intraoperative blood loss,reduced accidental vascular injuries,better short-term efficacy,and shorter recovery times for bowel sounds,first exhaust,first defecation,and first feeding (all P<0.05).At 48 hours after surgery,both groups showed a significant increase in NE,WBC,and Cor levels,but the combined group had lower levels compared to the single group (all P<0.05).The combined group had a lower incidence of complications compared to the single group (P<0.05). Conclusion: The combination of three-dimensional visualization reconstruction technology with laparoscopic surgery significantly benefits the treatment of colorectal cancer.It enhances gastrointestinal function,mitigates stress reactions,and reduces complications.

1857 Effects of Different Types of Dental Restorations on Masticatory Ability and Gingival Crevicular Fluid Inflammatory Markers in Patients with Posterior Dental Defects

Objective: To explore the effects of glass-based all-ceramic crown,zirconia all-ceramic crown and fiber post core zirconia crown restoration on masticatory ability and gingival crevicular fluid inflammatory injury indicators in patients with posterior dental defects. Methods: A total of 151 patients (151 teeth) with posterior dental defects who underwent selective post-core restoration after root canal therapy were included in this study.They were divided into three groups based on the material used for crown restoration:Group A received glass-based all-ceramic onlay restoration,Group B received zirconia all-ceramic crown restoration,and Group C received fiber post core zirconia crown restoration.Baseline data were analyzed using propensity score matching to obtain balanced and comparable baseline data in each group.The restoration outcomes were evaluated one year after the procedure,and masticatory function (occlusal force,masticatory efficiency),periodontal status (gingival bleeding index,periodontal pocket depth,attachment loss),and gingival crevicular fluid inflammatory markers (interleukin-1β,alkaline phosphatase,prostaglandin E2) were compared before and after restoration.Complications were also assessed. Results: Group A had significantly higher rates of restoration integrity,color matching,edge suitability,edge coloring,and gingival condition compared to Groups B and C (P<0.05).One year after restoration,all three groups showed significant improvements in occlusal force and masticatory efficiency compared to before restoration (P<0.05).Group A exhibited significantly greater improvements in occlusal force and masticatory efficiency compared to Groups B and C (P<0.05),and Group B had significantly greater improvements than Group C (P<0.05).Gingival crevicular fluid inflammatory markers (BI,PD,AL) increased significantly in all three groups after one year of restoration (P<0.05).Group A showed significantly smaller changes in BI,PD,and AL compared to Groups B and C (P<0.05),and Group B had significantly smaller changes than Group C (P<0.05).Similarly,inflammatory markers (IL-1β,ALP,PGE2) in gingival crevicular fluid increased significantly in all three groups after one year of restoration (P<0.05).Group A exhibited significantly greater changes in these markers compared to Groups B and C (P<0.05),and Group B had significantly greater changes than Group C (P<0.05).The total complication rate in Group A within one year after restoration was 1.96%,significantly lower than the 12.00% in Group B and 18.00% in Group C (P<0.05). Conclusion: Glass-based all-ceramic onlay restoration,when used for posterior dental defects after root canal therapy,leads to improved restoration outcomes,enhanced masticatory function,reduced inflammatory injury indicators in periodontal tissue and gingival crevicular fluid,and a lower incidence of complications compared to zirconia all-ceramic crown and fiber post core zirconia crown restoration.

1864 Correlation Analysis of RDW hs-CRP and Coagulation-Fibrinolysis Index with Cerebral Artery Stenosis in Ischemic Stroke Patients

Objective: To analyze the correlation between Red Blood Cell Distribution Width (RDW),high-sensitivity C-reactive protein (hs-CRP),and coagulation-fibrinolysis index with cerebral artery stenosis in patients suffering from ischemic stroke. Methods: A total of 126 patients with cerebral arterial thrombosis admitted to our hospital between January 2019 and May 2021 were included.They were categorized into two groups:the cerebral arterial stenosis group (n=57) and the cerebral arterial normal group (n=69) based on the presence of cerebral artery stenosis.Comparative analysis of RDW,high-sensitivity C-reactive protein (hs-CRP),and coagulation-fibrinolysis index was performed between the two groups,and their correlations were examined. Results: Significant differences were observed in the history of hypertension between the two groups of patients (P<0.05).In the cerebral arterial stenosis group,patients exhibited significantly larger red blood cell distribution width (RDW) values in the neck region compared to the cerebral arterial normal group (P<0.05).Additionally,hs-CRP levels in the cerebral arterial stenosis group were notably higher than those in the cerebral arterial normal group,with statistically significant differences (P<0.05).Furthermore,patients in the cerebral arterial stenosis group had significantly lower levels of activated partial thromboplastin time (APTT) compared to patients in the cerebral arterial normal group (P<0.05).In contrast,fibrinogen (FIB) and D-dimer (D-D) levels in the cerebral arterial stenosis group were significantly higher than those in the cerebral arterial normal group (P<0.05).Pearson or spearman correlation analysis results demonstrated a significant positive correlation between RDW,FIB,D-D,hs-CRP,and cerebral artery stenosis (P<0.05).However,APTT showed a significant negative correlation with cerebral artery stenosis (P<0.05). Conclusion: The findings suggest that RDW,hs-CRP,and coagulation-fibrinolysis index are indeed correlated with cerebral artery stenosis in patients with cerebral arterial thrombosis.

1868 Changes of Serum Small and Dense Low-Density Lipoprotein Cholesterol and Relationship with Insulin Resistance and Fat Attenuation Index in Patients with T2DM and NAFLD

Objective: To explore the changes of serum small and dense low-density lipoprotein cholesterol (sdLDL-C) in patients with type 2 diabetes mellitus (T2DM) complicated with non-alcoholic fatty liver disease (NAFLD) and the relationship with insulin resistance and fat controlled attenuation parameter (CAP). Methods: The clinical data of 61 patients with uncomplicated T2DM admitted to the hospital between March 2023 and April 2023 were retrospectively analyzed and included in the T2DM group.The clinical data of 48 patients with T2DM complicated by NAFLD,admitted to the hospital during the same period,were retrospectively analyzed and included in the T2DM/NAFLD group.General patient data,such as gender,age,waist-to-hip ratio,T2DM duration,body mass index (BMI),and blood pressure (diastolic and systolic) were analyzed in both groups.Lipid metabolism indicators (sdLDL-C,CAP,total cholesterol,triglycerides) and glucose metabolism indicators (fasting plasma glucose,glycosylated hemoglobin,homeostasis model assessment of insulin resistance) were compared between the two groups at admission.Pearson correlation analysis was used to evaluate the relationship between serum total cholesterol,triglycerides,sdLDL-C,and insulin resistance (HOMA-IR) and CAP in T2DM patients with NAFLD. Results: Patients in the T2DM/NAFLD group had higher levels of BMI,lipid metabolism indicators (total cholesterol,triglycerides,sdLDL-C,CAP),and glucose metabolism indicators (fasting plasma glucose,glycosylated hemoglobin,HOMA-IR) compared to those with uncomplicated T2DM (all P<0.05).Pearson correlation analysis showed that the sdLDL-C level in T2DM patients with NAFLD was positively correlated with HOMA-IR and CAP levels (all P<0.05). Conclusion: Serum sdLDL-C levels are elevated in T2DM patients with NAFLD and are closely associated with insulin resistance and CAP.

1872 A Study of Pulmonary Function Characteristics and Risk Factors in Elderly Patients with Combined Pulmonary Fibrosis and Emphysema

Objective: To investigate the characteristics of pulmonary function and risk factors of emphysema in elderly patients with pulmonary interstitial fibrosis complicated with emphysema syndrome (CPEF). Methods: The study included 144 eligible elderly patients with pulmonary interstitial fibrosis admitted to West China Hospital of Sichuan University from February 2019 to February 2022.Among these patients,90 were diagnosed with emphysema (CPEF group),and 54 had pulmonary interstitial fibrosis alone (control group).The study compared changes in pulmonary function parameters,blood routine,erythrocyte sedimentation rate,and other laboratory indicators between the two groups.Both univariate and multivariate methods were used to analyze the relationship between various factors and the development of pulmonary hypertension in patients with CPEF. Results: The study found no statistically significant differences in the FVC% pred and FEV1% pred measurements between the CPEF group and the control group (P>0.05).However,the FEV1/FVC and DLCO% pred measurements in the CPEF group were lower compared to the control group.Additionally,the CPEF group showed higher values for TLC% pred and RV% pred measurements compared to the control group,with these differences being statistically significant (P<0.05).The CPEF group had a higher proportion of male patients,a higher percentage of patients with a history of smoking,and a higher smoking index compared to the control group,and these differences were statistically significant (P<0.05).A logistic regression analysis revealed that male patients,smoking history,increased smoking index,decreased ALB (albumin),and decreased RDW (red blood cell distribution width) were independent risk factors for emphysema in patients with pulmonary interstitial fibrosis (P<0.05). Conclusion: Elderly patients with pulmonary interstitial fibrosis complicated by emphysema syndrome typically present with aggravated dyspnea and reduced pulmonary diffusion function.Risk factors for developing emphysema in these patients include male gender,smoking history,and decreased levels of ALB and RDW.

1876 Analysis of the Distribution of Digestive Tract Bleeding and Organ Damage Symptoms Associated with HP Infection in Children with Abdominal-Type Allergic Henoch-Schonlein Purpura and Their Related Influencing Factors

Objective: To analyze the distribution of Helicobacter pylori (Hp) infection,gastrointestinal bleeding and organ damage in children with abdominal allergic purpura (HSP). Methods: A total of 145 children diagnosed with abdominal HSP,admitted to our hospital from July 2018 to July 2021,were included in the study.They were divided into two groups:the gastrointestinal bleeding group (68 cases) and the non-gastrointestinal bleeding group (77 cases).Various parameters,including intestinal flora,inflammatory factors,auxiliary examination results,immune function,organ damage,and joint symptoms,were compared between the two groups.Logistic regression analysis was performed to assess whether abdominal HSP is an influencing factor for the occurrence of gastrointestinal bleeding. Results: Among the two groups of children,parameters such as the percentage of neutrophils (N%),hemoglobin (Hb),eosinophils (E%),platelet (PLT) count,PLT volume,erythrocyte sedimentation rate,D-dimer abnormality,chest and abdominal B-ultrasound and x-ray findings,intercellular adhesion molecule (ICAM1),and the proportion of cardiac lesions were similar and showed no statistically significant differences (P>0.05).Children in the gastrointestinal bleeding group had lower levels of Lactobacillus,lymphocytes (L%),immunoglobulin A (IgA),IgG,IgM,CD3+,and CD4+ compared to the non-gastrointestinal bleeding group,with statistically significant differences (P<0.05).Additionally,the gastrointestinal bleeding group had higher levels of Escherichia coli,serum interleukin-6 (IL-6),IL-8,tumor necrosis factor-α (TNF-α),white blood cell (WBC) count,CD8+,renal lesions,multiple organ involvement,and joint symptoms compared to the non-gastrointestinal bleeding group,with statistically significant differences (P<0.05).Logistic regression analysis revealed that TNF-α,IgA,IgM,and renal lesions were influencing factors for gastrointestinal bleeding in children with abdominal HSP (P<0.05). Conclusion: Children with gastrointestinal bleeding in abdominal HSP had lower levels of beneficial intestinal bacteria,higher levels of harmful bacteria,compromised immune function,greater inflammatory responses,and a higher proportion of renal lesions compared to those without gastrointestinal bleeding.The influencing factors for gastrointestinal bleeding in abdominal HSP included TNF-α,IgA,IgM,and renal lesions.

1882 Correlation of Parameters at Different Time Periods in Pediatric Holter Electrocardiography with the Diameter and Quantity of Infective Endocarditis Vegetations

Objective: To investigate the diagnostic value of different time parameters of Holter electrocardiography in children with infective endocarditis. Methods: A total of 90 children with suspected infective endocarditis who were treated at our hospital from January 2017 to June 2021 were selected.Among them,62 children with infective endocarditis confirmed by echocardiography and blood culture were assigned to the observation group,while the remaining 28 children were placed in the control group.Both groups underwent pediatric Holter electrocardiogram scanning.We compared various parameters at different time periods between the two groups.Enzyme-linked immunosorbent assay was used to measure serum IL-6 and cTnI levels,and serum CRP levels were determined using the immunoratio method.The diagnostic value of the diameter and number of endocardial vegetations in infective endocarditis was analyzed. Results: Compared to the control group,children in the observation group had lower values for Holter ECG parameters such as the standard deviation of normal R-R intervals (SDNN),root mean square difference of adjacent R-R intervals (rMSSD),and the percentage of R-R intervals with a standard deviation greater than 50ms (PNN50) in different time periods (P<0.05).Additionally,children with advanced infection (12h and 24h) had lower SDNN,rMSSD,and PNN50 levels than those with early infection (P<0.05).Furthermore,children with advanced infection exhibited higher serum levels of CRP,IL-6,and cTnI compared to children with early infection (P<0.05).The diameter and quantity of endocardial vegetations in children with advanced infection were lower than in those with early infection.The combined diagnosis of SDNN,rMSSD,CRP,IL-6,cTnI,and the size and quantity of vegetations demonstrated a high diagnostic value for infective endocarditis. Conclusion: Different time period parameters in pediatric Holter electrocardiography,as well as serum markers such as CRP,IL-6,and cTnI,along with the diameter and quantity of endocardial vegetations,have high diagnostic value in the context of infective endocarditis.This information provides valuable guidance for the early prevention and treatment of infective endocarditis.

1886 The Effect of Different Pancreaticoenteric Anastomotic Techniques on Postoperative Pancreatic Fistula Following Pancreaticoduodenectomy

Objective: To investigate the effect of two different pancreaticoenteric anastomotic techniques on postoperative pancreatic fistula (POPF) after pancreaticoduodenectomy. Methods: A retrospective analysis of clinical data from patients undergoing pancreaticoduodenectomy at the Affiliated Hospital of Chengde Medical College from January 2020 to December 2022 was conducted. Patients were divided into two groups based on different pancreaticojejunostomy techniques: the "Chen's" pancreaticojejunostomy group (Chen's group, n=21) and the "Hong's one-stitch method" pancreaticojejunostomy group (Hong's group, n=44). The preoperative disease status, POPF, intraoperative bleeding, length of hospital stay, and hospitalization cost were compared between the two groups. Multivariate Logistic regression analysis was used to explore the effect of various indicators on the risk of complications such as pancreatic fistula. Results: There was no statistical significant difference in the size of pancreatic duct between the two groups (P>0.05). The preoperative bilirubin level and biliary drainage in Hong's group were higher than those in Chen's group (P<0.05). Intraoperative blood loss, length of hospital stay and hospitalization cost in Chen's group were lower than those in Hong's group (P<0.05). The POPF incidence in Chen's group was significantly lower than in Hong's group (56.82% vs 14.29%;χ2=10.49, P=0.001). Multivariate Logistic regression showed that there was no statistical significance in preoperative bilirubin level, biliary drainage, intraoperative blood loss, length of hospital stay and hospitalization cost between the two groups (P>0.05). Compared with Hong's group, Chen's group decease the incidence of POPF (P<0.05). Conclusion: Chen's pancreaticojejunostomy technique is associated with a lower risk of pancreatic fistula after pancreaticoduodenectomy, and it is worthy of clinical promotion and application.

1891 Comparison of the Impact of Bedside Ultrasound-Guided Percutaneous Dilational Tracheostomy and Surgical Tracheostomy on Peri-Tracheal Tissue Damage in Critically Ill ICU Patients

Objective: To explore the effects of bedside ultrasound-guided percutaneous dilational tracheotomy (PDT) and surgery tracheostomy (ST) on tissue damage around the tracheal ring in critically ill patients in ICU. Methods: Clinical data from 48 critically ill ICU patients who underwent bedside ultrasound-guided PDT between January 2019 and August 2022 were retrospectively analyzed and categorized as the ultrasound group.Additionally,data from 46 ICU patients who received ST during the same period were analyzed and categorized as the traditional group.We compared perioperative factors (surgery duration,intraoperative blood loss,mechanical ventilation time),incision-related variables (incision length,one-time catheterization success rate,balloon rupture rate),and levels of inflammatory markers (erythrocyte sedimentation rate (ESR),C-reactive protein (CRP),procalcitonin (PCT)) before and one week after the surgery.Postoperative complications (pneumothorax,recurrent laryngeal nerve injury,esophageal injury,tracheal fistula,pulmonary infection,posterior tracheal wall injury) were assessed in both groups. Results: The ultrasound group exhibited significantly shorter surgery duration and less intraoperative blood loss compared to the traditional group (P<0.05).There was no significant difference in mechanical ventilation time between the two groups (P>0.05).Patients in the ultrasound group had significantly shorter incision lengths compared to the traditional group (P<0.05).The one-time catheterization success rate and balloon rupture rate showed no statistically significant differences between the two groups (P>0.05).One week after surgery,both groups showed significantly lower levels of ESR,CRP,and PCT compared to before the surgery,with the ultrasound group displaying significantly lower levels than the traditional group (P<0.05).The total incidence of postoperative complications was significantly lower in the ultrasound group compared to the traditional group (P<0.05). Conclusion: Bedside ultrasound-guided PDT,compared to ST,offers advantages such as shorter surgery duration,reduced patient trauma,decreased postoperative complications,and lower inflammatory response levels.This method provides better patient safety and preservation of peri-tracheal tissue integrity.

1896 Value Analysis of Thromboelastogram Parameters Combined with INR and D-D Levels in the Early Diagnosis of Sepsis with DIC

Objective: To investigate the value of thrombologram parameters combined with International normalized ratio (INR) and D-dimer (D-D) levels in the early diagnosis of sepsis with diffuse intravascular coagulation (DIC). Methods: A total of 179 sepsis patients admitted to our hospital from January 2021 to January 2023 were included in the study.They were divided into two groups:a non-DIC group (87 cases) and a DIC group (92 cases).All subjects underwent measurements of thromboelastogram parameters,INR,and D-D levels.Differences in thromboelastogram parameters,INR,and D-D levels between the two groups were compared,and their expression in different grades and stages of DIC patients was analyzed.Factors influencing sepsis with DIC,the correlation between thromboelastogram parameters and INR,D-D,and the diagnostic value for sepsis with DIC were analyzed. Results: In the DIC group,coagulation reaction time (R value),coagulation formation time (K value),INR,and D-D were higher than in the non-DIC group.Coagulation angle (α Angle),maximum amplitude (MA value),and comprehensive coagulation index (CI value) were lower in the DIC group compared to the non-DIC group.These differences were statistically significant (P<0.05).Dominant DIC patients had higher R value,K value,INR,and D-D levels compared to pre-DIC patients,while α Angle,MA value,and CI value were lower in the dominant DIC group compared to the pre-DIC group.These differences were statistically significant (P<0.05).Patients in the fibrinolysis stage had higher R value,K value,INR,and D-D levels compared to patients in the low-coagulation and high-coagulation stages.In contrast,α Angle,MA value,and CI value were lower in the fibrinolysis stage compared to the low-coagulation and high-coagulation stages.These differences were statistically significant (P<0.05).Multivariate Logistic regression analysis revealed that the number of infected sites,septic shock,organ failure,SOFA score,APACHE Ⅱ score,R value,K value,α Angle,MA value,CI value,INR,and D-D were all risk factors for sepsis with DIC (P<0.05).Pearson test showed that R value and K value were positively correlated with INR and D-D (P<0.05),while α Angle,MA value,and CI value were negatively correlated with INR and D-D levels (P<0.05).ROC curve analysis indicated that the diagnostic efficacy of thromboelastogram parameters combined with INR and D-D in sepsis with DIC was superior to single diagnosis.The diagnostic AUC was 0.943,with sensitivity and specificity at 92.30% and 87.93%,respectively.In a 28-day prognostic follow-up of 92 sepsis patients with DIC,36 patients died within the 28 days,resulting in a high mortality rate. Conclusion: The combination of thromboelastogram parameters with INR and D-D can accurately diagnose sepsis with DIC,providing support for accurate clinical diagnosis and treatment and reducing prognostic mortality.

1902 Evaluation of SI and NLR for Short-Term Prognosis in Acute ST-Segment Elevation Myocardial Infarction

Objective: To investigate the predictive value of neutrophil/lymphocyte ratio (NLR) and shock index (SI) in short-term prognosis of emergency St-segment elevation myocardial infarction (STEMI) patients. Methods: A total of 280 first-visit emergency department patients with STEMI were included in this study.Demographic and clinical data of all patients were recorded.Key vital signs including systolic blood pressure (SBP),diastolic blood pressure (DBP),and heart rate (HR) were monitored dynamically.Blood routine tests were conducted using the emergency green channel,and SI and NLR values were calculated.The occurrence of major adverse cardiac events (MACE) within 14 days was recorded,and the patients were categorized into the MACE and non-MACE groups.Receiver operating characteristic curve (ROC curve) analysis was employed to compare the ability of SI and NLR to assess adverse outcomes in STEMI patients. Results: Among the 280 STEMI patients,52 experienced adverse cardiovascular events,resulting in an incidence of 18.57%.These events included 24 cases of malignant arrhythmias,18 cases of cardiogenic shock,and 10 all-cause deaths,while the remaining 228 patients did not experience adverse cardiovascular events.In the MACE group,SI,white blood cell count,neutrophil count,NLR,and cardiac function grade Ⅲ and Ⅳ were significantly higher compared to the non-MACE group (P<0.05).Additionally,the MACE group exhibited significantly lower DBP,SBP,and lymphocyte counts compared to the non-MACE group,and these differences were statistically significant (P<0.05).The areas under the ROC curves for SI,NLR,and the combined SI and NLR were 0.684,0.769,and 0.801,respectively. Conclusion: Both NLR and SI demonstrate substantial value in evaluating short-term adverse events in emergency STEMI patients.When used in combination,they offer even higher predictive value.

1905 Clinical Efficacy of Shoulder Arthroscopy-Assisted TightRope Plate Fixation in the Treatment of Acromioclavicular Joint Dislocation and Its Impact on Postoperative Immune Stress Response in Patients

Objective: To investigate the clinical efficacy of shoulder arthroscopy-assisted TightRope plate fixation in the treatment of acromioclavicular joint dislocation and its effect on postoperative immune stress. Methods: A total of 100 patients with acromioclavicular joint dislocation who were admitted to our hospital from February 2021 to February 2022 were selected and divided into group A (n=52) and group B (n=48) according to the surgical plan.Group A was given shoulder arthroscopy The patients were treated with TightRope plate fixation assisted,and group B was treated with shoulder arthroscopy-assisted clavicle hook plate internal fixation.The clinical efficacy,shoulder joint function [European Shoulder Association scoring system Constant shoulder joint score],immune response (CD3+,CD4+,CD8+,CD4+/CD8+ ratio),stress response [cortisol (Cor),adrenocorticotropic cortex hormone (ACTH)] and postoperative complications] were compared between the two groups. Results: Compared with group B,the excellent and good rate of group A was significantly higher (P<0.05),6 months after treatment,the scores of each dimension of Constant shoulder joint were significantly higher (P<0.05),and the decreasing range of CD3+,CD4+,CD8+,CD4+/CD8+ ratio at 1 day after treatment was significantly lower (P<0.05).The increase rates of serum Cor and ACTH were significantly lower (P<0.05),and the total incidence of postoperative complications was significantly lower (P<0.05). Conclusion: For patients with acromioclavicular joint dislocation,shoulder arthroscopy-assisted TightRope plate fixation can improve the clinical excellent and good rate,promote the recovery of shoulder joint function,reduce the immune stress response,and reduce postoperative complications.

1911 Effect of Dapagliflozin on Quality of Life and Prognosis in Patients with Heart Failure

Objective: To investigate the effect of dapagliflozin treatment on cardiac function,quality of life and prognosis of patients with heart failure. Methods: A total of 250 patients with heart failure were randomly divided into experimental group and control group,of which 157 patients in the control group were treated with cardiac rehabilitation exercise and conventional anti-heart failure therapy,and 93 patients in the experimental group were treated with dapagliflozin 10mg once a day on the basis of the control group.Two groups were compared before and after treatment with serum N-terminal precursor brain natriuretic peptide (NT-proBNP),percentage of neutrophils (N%),hemoglobin (Hb),creatinine (Cr),left ventricular ejection fraction (LVEF),left ventricular end-diastolic diameter (LVEDD),Left ventricular wall relative thickness (RWT),6-minute walk experiment (6MWD),Minnesota Chronic Heart Failure Quality of Life Questionnaire (MLHFQ),and follow-up readmission rate and mortality within 6 months,and the differences of various indicators were compared and analyzed. Results: There were no significant differences between in NT-proBNP,N%,Hb,Cr,LVEF,LVEDD,RWT,6MWD and MLHFQ in the pre-intervention experimental group and control group (P>0.05). After the intervention,the serum NT-proBNP level,serum Cr level,LVEDD and 6MWD scores decreased (P<0.05),LVEF value and MLHFQ increased (P<0.05),all indicators after intervention in the experimental group were better than those in the control group. All patients were followed up.The readmission rates within six months in the experimental group and the control group were 16.13% and 22.93%,respectively.The difference was statistically significant between the two groups (P<0.05).The cardiogenic mortality rates of the two groups were 2.2% and 8.2%,respectively,and the difference between the two groups was statistically significant (P<0.05).There was no significant difference in the incidence of myocardial infarction and renal dysfunction between the two groups (P>0.05). Conclusion: Cardiac rehabilitation and dapagliflozin combination therapy can significantly improve the cardiopulmonary function and quality of life of patients,and is of great help to improve the long-term prognosis of patients with heart failure.

1916 Efficacy of Trastuzumab Combined with Pertuzumab in the Treatment of HER2-Positive Breast Cancer and Its Impact on Serum miR-375 and miR-30d Expression

Objective: To observe the efficacy of combination of trastuzumab and pertuzumab in treating human epidermal growth factor receptor-2 (HER2)-positive breast cancer and the effects on changes of expressions of serum microRNA-375 (miR-375) and microRNA-30d (miR-30d). Methods: From February 2020 to February 2023,a total of 139 patients with HER2-positive breast cancer were randomly assigned to the combined group (n=70,receiving trastuzumab and pertuzumab) and the control group (n=69,receiving trastuzumab) using a simple random number table and random remainder grouping method.Each patient received one course of treatment every 21 days,and a total of four courses were administered.Clinical efficacy,tumor-related indicators (including carbohydrate antigens CA125 and CA153,carcinoembryonic antigen CEA,and circulating tumor cells CTC),T lymphocyte subsets (CD3+,CD4+,CD8+,CD4+/CD8+),serum microRNA expression (miR-375 and miR-30d),and changes in quality of life (assessed using the Quality of Life Questionnaire-Cancer QLQ-C30) were compared between the two groups before and after treatment.Adverse reactions were also documented. Results: No patient died during the course of treatment in both groups.The total clinical effective rate was significantly higher in the combined group compared to the control group (P<0.05).After four courses of treatment,both groups exhibited reductions in serum CA125,CA153,CEA,CTC positive rate,CD8+ levels,and miR-375 and miR-30d expression,as well as improvements in QLQ-C30 scores compared to baseline (P<0.05).Additionally,serum CD3+,CD4+,and CD4+/CD8+ levels were elevated in both groups post-treatment compared to baseline,with the combined group displaying higher levels compared to the control group (P<0.05).Notably,the difference values of various indicators were significantly higher in the combined group compared to the control group (P<0.05).Adverse reactions were effectively managed,with a higher incidence of palpitations observed in the combined group (P<0.05). Conclusion: Trastuzumab combined with pertuzumab effectively enhances immune function,reduces tumor marker levels,modulates the expression of miR-375 and miR-30d,and improves the quality of life for HER2-positive breast cancer patients.Moreover,this treatment approach demonstrates a high safety profile and significant clinical efficacy.

1922 Influence of Qizhen Capsule Adjuvant Therapy on T Lymphocyte Subsets Tumor Markers and Prognosis of Breast Cancer

Objective: To explore the effect of Qizhen capsule on T lymphocyte subsets,tumor markers and prognosis of breast cancer. Methods: Eighty patients with breast cancer who received treatment at our hospital from January 2019 to January 2021 were selected for this study.They were randomly divided into a control group and an observation group,with 40 patients in each group.Both groups received the EC chemotherapy intervention (cyclophosphamide+doxorubicin+fluorouracil).Additionally,the observation group received Qizhen capsule as oral treatment.Both groups were treated continuously for 6 weeks.The study compared the blood routine status,T lymphocyte subpopulations (CD3+,CD4+,CD8+),and tumor marker indicators (carcinoembryonic antigen (CEA),carbohydrate antigen 153 (CA153),and cancer antigen 125 (CA125) levels between the two groups.As of the follow-up on February 28,2023,the study analyzed the difference in median survival time between the two groups. Results: Before treatment,there were no significant differences in red blood cells,white blood cells,and platelets between the two groups (P>0.05).After treatment,the difference in white blood cell and platelet counts before and after treatment in the observation group was greater than in the control group (P<0.05).Before treatment,there were no significant differences in CD3+,CD4+,and CD8+ between the two groups (P>0.05).After treatment,the difference in CD3+ and CD4+ before and after treatment in the observation group was greater than in the control group (P<0.05).Before treatment,there were no significant differences in CEA,CA153,and CA125 between the two groups (P>0.05).After treatment,CEA,CA153,and CA125 were decreased in both groups,with the difference in CEA,CA153,and CA125 before and after treatment in the observation group being greater than in the control group (P<0.05).As of February 28,2023,4 patients in the observation group experienced disease recurrence and metastasis,while 11 patients in the control group experienced recurrence and metastasis.Compared with the control group,the observation group had a higher disease-free survival rate,and the survival advantage was more pronounced (χ2=7.195,P=0.007).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Adjuvant treatment with Qizhen capsule can improve the T lymphocyte subsets,tumor markers,and postoperative survival of breast cancer patients,and the treatment is safe.

1927 Impact of Camrelizumab Combined with Chemotherapy and Hyperthermic Perfusion on the PD-1/PD-L1 Signaling Pathway in Primary Liver Cancer

Objective: To explore the mechanism of action of camrelizumab combined with chemotherapy and hyperthermic perfusion in the treatment of primary liver cancer based on the PD-1/PD-L1 signaling pathway. Methods: Eighty patients with advanced primary hepatocellular carcinoma (PHC) treated in the hospital from February 2019 to March 2021 were randomly divided into two groups:a chemotherapy and hyperthermic perfusion group and an apatinib plus chemotherapy and hyperthermic perfusion group,each consisting of 40 patients.The chemotherapy and hyperthermic perfusion group received chemotherapy and hyperthermic perfusion treatment,while the apatinib plus chemotherapy and hyperthermic perfusion group received apatinib plus chemotherapy and hyperthermic perfusion treatment after the initial treatment.Short-term clinical efficacy was compared between the two groups.Flow cytometry was used to assess changes in tumor markers (CEA,AFP,AFP-L3) and T lymphocyte subsets.Liver function indexes (ALT,AST,TBIL,ALP) were compared.PD-1 and PD-L1 expression in peripheral blood were assessed,and the incidence of adverse reactions was compared.Kaplan-Meier survival analysis was conducted during patient follow-up to compare survival rates. Results: In the apatinib plus chemotherapy and hyperthermic perfusion group,short-term clinical efficacy and peripheral blood CD4+,CD8+,and CD4+/CD8+ ratio were higher compared to the chemotherapy and hyperthermic perfusion group.Levels of AFP,AFP-L3,CEA,ALT,AST,TBIL,ALP,PD-1,and PD-L1 expression in peripheral blood were lower in the apatinib combination group (P<0.05).The apatinib combination group had a lower incidence of pain and a higher survival rate compared to the chemotherapy and hyperthermic perfusion group (P<0.05). Conclusion: The combination of camrelizumab and chemotherapy and hyperthermic perfusion can activate T lymphocyte subsets by inhibiting the PD-1/PD-L1 signaling pathway,thereby improving the body's anti-tumor immune function,prolonging the survival period of patients,and reducing pain.This approach is worth promoting in clinical practice.
