KANG Lili, CHEN Zhaowei, SONG Jian
DOI: 10.3969/j.issn.1006-6233.2023.09.01
Objective: To explore the effects of miR-891a-5p on the proliferation, invasion, migration, apoptosis and angiogenesis of colon cancer cells and its mechanism. Methods: The cell with the highest expression of miR-891a-5p in each cell line of FHC, SW480, HCT116, HCT8, and SW620 was screened by PCR for SW480, and miR-891a-5p mimic (miR-891a-5p mimic), miR-891a-5p inhibitor (miR-891a-5p inhibitor) and their corresponding controls were transfected into SW480 cells. SW480 were divided into OE-NC group and OE-NLRP7, and the proliferative ability of SW480 was determined by CCK-8 method, the invasive ability of SW480 was determined by Transwell test, and the migratory ability of SW480 was determined by scratch test; the protein expression of VEGF and MMP-9 was analyzed by Western blot; and the angiogenic ability was examined by HUVEC angiogenesis assay. The targeting relationship between miR-891a-5p and NLRP7 was validated with luciferase reporter gene assay; the interaction between miR-891a-5p and NLRP7 was analyzed with RNA immunoprecipitation. Results: The expression of miR-891a-5p in colon cancer cells was significantly higher than that in human normal colon epithelial cells, and the expression of NLRP7 in colon cancer cells was significantly lower than that in normal stem cells. Inhibition of miR-891a-5p significantly decreased the proliferation, invasion, migration and angiogenesis of SW480 cells (P<0.01). The protein expression levels of VEGF and MMP-9 were significantly decreased (P<0.01). The results of targeting relationship showed that there was a targeting relationship between miR-891a-5p and NLRP7. NLRP7 overexpression could reduce the proliferation, migration, and angiogenesis of SW480 cells (P<0.01). Conclusion: miR-891a-5p can regulate NLRP7 to promote the proliferation, invasion, migration and angiogenesis of colon cancer cells to accelerate the development of colon cancer.

CHEN Yanzhi
DOI: 10.3969/j.issn.1006-6233.2023.09.03
Objective: To investigate the exact function and mechanism of CDC25B in TNBC angiogenesis. Methods: Bioinformatics database was used to analyze the expressions of CDC25B and its upstream regulatory molecule miR-141-3p in TNBC tumor tissues, and qRT-PCR was used to analyze the expressions of CDC25B and miR-141-3p in TNBC cell lines. The proliferation and angiogenesis capacity of HUVEC cells were analyzed by CCK-8 and angiogenesis assay, and the expressions of VEGFA, VEGFR-2 and VEGFR-3 proteins were detected by Western blot. Dual luciferase reporter assay was used to explore the specific interaction between CDC25B and miR-141-3p.Results: It was found that CDC25B was up-regulated in TNBC, and its high expression could activate the VEGF signaling pathway, and the silencing of CDC25B significantly inhibited the proliferation and angiogenesis of HUVEC cells, and decreased the expression of VEGFA, VEGFR-2 and VEGFR-3 proteins. In addition, the expression of miR-141-3p was down-regulated in TNBC, which could target the inhibition of CDC25B expression. Overexpression of CDC25B reversed the inhibitory effect of miR-141-3p overexpression on proliferation and angiogenesis of HUVEC cells. Conclusion: miR-141-3p targets CDC25B to inhibit VEGF pathway and inhibit TNBC angiogenesis, and we provide theoretical basis for the possibility of miR-141-3p/CDC25B/VEGF pathway as a new choice for TNBC anti-angiogenesis therapy.

DOI: 10.3969/j.issn.1006-6233.2023.09.08
Objective: To investigate the changes of serum squamous cell carcinoma antigen (SCC), macrophage colony stimulating factor (M-CSF), and chemokine (Chemrin) in patients with cervical epithelial neoplasia and their diagnostic value. Methods: A total of 101 patients with cervical epithelial neoplasia (CIN) confirmed by pathological examination in our hospital from April 2019 to April 2022 were selected as the CIN group, and 50 patients with early cervical cancer confirmed by pathological examination in the same period were selected as the cervical cancer group. The measured values of serum SCC, M-CSF, and Chemerin before treatment were compared between the two groups, and the hierarchical comparative analysis was carried out according to the CIN stage and the results of high-risk human papillomavirus (HR-HPV) viral load test. The levels of serum SCC, M-CSF and Chemerin in CIN patients before and 3 months after operation were compared. The application value of SCC, M-CSF and Chemerin in the differential diagnosis of CIN and early cervical cancer was analyzed by the subject working characteristic curve (ROC). Results: The measured values of serum SCC, M-CSF and Chemerin in patients with cervical cancer were significantly higher than those in patients with CIN (P<0.05). The areas under ROC curve of SCC, M-CSF and Chemerin were 0.906, 0.846 and 0.791, respectively. Among the 101 patients with CIN, there were 21 patients with stage I, 46 patients with stage II, and 34 patients with stage III. With the increase of CIN stage, the levels of SCC, M-CSF, and Chemerin in the patients increased gradually, and the difference between the groups was statistically significant (P<0.05). The measured values of SCC and M-CSF in patients with HR-HPV DNA positive CIN were higher than those in patients with negative CIN, and the difference between the groups was statistically significant (P<0.05). Three months after the operation, the measured values of SCC, M-CSF and Chemerin in CIN patients showed a significant downward trend, and the difference was statistically significant (P<0.05). Conclusion: Compared with cervical cancer patients, the determination values of SCC, M-CSF and Chemerin in CIN patients were lower, but with the increase of CIN grading and the higher levels of SCC, M-CSF and Chemerin in patients with positive expression of HR-HPV DNA, Detection of SCC, M-CSF and Chemerin levels can differentiate CIN from cervical cancer.

BAI Yanyan, LI Ruibo, ZHAO Rui, et al
DOI: 10.3969/j.issn.1006-6233.2023.09.014
Objective: To investigate the value of cysteine aspartic protease-3 (caspase-3), neuron-specific enolase (NSE) and early cerebral oxygen utilization (O2UCc) in early prediction of delayed encephalopathy caused by acute carbon monoxide poisoning. Methods: A total of 125 patients with acute carbon monoxide poisoning admitted to our hospital from January 2019 to December 2020 were selected. According to whether delayed encephalopathy of carbon monoxide poisoning occurred within 60 days after poisoning, the patients were divided into encephalopathy group (34 cases) and control group (91 cases). Serum caspase-3, NSE and O2UCc levels of the two groups were compared within 24h after admission, and the value of each index in predicting delayed encephalopathy in patients with acute carbon monoxide poisoning was analyzed by receiver operating curve (ROC). Logistic multivariate model was used to analyze the relationship between caspase-3, NSE, O2UCc and delayed encephalopathy caused by carbon monoxide poisoning. Results: The measured values of caspase-3 and NSE in encephalopathy group were significantly higher than those in control group, while the measured values of O2UCc in encephalopathy group were significantly lower than those in control group, with statistical significance (P<0.05). The sensitivity of caspase-3, NSE, O2UCc, caspase-3+NSE+O2UCc to predict delayed encephalopathy of acute carbon monoxide poisoning were 63.41%, 57.52%, 62.77% and 92.33%, respectively. The specificity was 85.28%, 80.66%, 82.94%, 78.58%, and the area AUC under ROC curve was 0.802, 0.797, 0.798, 0.936, respectively. Logistic regression model showed that aging, prolonged CO exposure time, consciousness disturbance ≥10h, severe poisoning, increased caspase-3, increased NSE and decreased O2UCc were independent risk factors for delayed encephalopathy of acute carbon monoxide poisoning (P<0.05). Conclusion: Early caspase-3 and NSE levels were significantly increased and O2UCc levels were significantly decreased in patients with acute carbon monoxide poisoning delayed encephalopathy, and there was a significant relationship with the occurrence of delayed encephalopathy in patients with acute carbon monoxide poisoning.

DOI: 10.3969/j.issn.1006-6233.2023.09.015
Objective: To explore the effects of minimally invasive surround-mammary areola incision surgery and open resection surgery in the treatment of gynecomastia (GYN). Methods: A total of 105 patients with GYN admitted to the hospital from August 2020 to August 2022 were selected and divided into two groups according to different surgical treatment regimens. Among the patients, 63 cases with GYN undergoing minimally invasive surround-mammary areola incision surgery were included in observation group, and 42 cases receiving open resection surgery were enrolled as control group. The surgery-related indicators, wound healing time, scar width, breast beauty, preoperative and postoperative traumatic stress indicators [norepinephrine (NE), epinephrine (E), cortisol (Cor)], occurrence of postoperative complications and postoperative effect satisfaction were compared between the two groups. Results: The surgical time, hospital stay and wound healing time in observation group were shorter than those in control group (P<0.05), and the intraoperative blood loss was less than that in control group (P<0.05), and the incision length and scar length were shorter compared with those in control group (P<0.05), and the breast beauty was higher compared to control group (P<0.05). The levels of serum NE, E and Cor in both groups were higher after surgery than those before surgery (P<0.05), but the decreases in observation group were lower than those in control group (P<0.05). The total incidence rate of postoperative complications was lower in observation group than that in control group (3.17% vs 21.43%, P<0.05). The satisfaction scores of postoperative chest appearance, wound scar and skin and areola nipple sensation and overall satisfaction score in observation group were higher compared with those in control group (P<0.05). Conclusion: Compared with open resection surgery, minimally invasive surround-mammary areola incision surgery for patients with GYN has shorter surgical time, wound healing time and hospital stay, less intraoperative blood loss and shorter postoperative incision length and scar length, and the latter one can better significantly relieve the postoperative traumatic stress response, reduce the postoperative complications, and enhance the patients’ satisfaction with postoperative effect.

DOI: 10.3969/j.issn.1006-6233.2023.09.016
Objective: To investigate the characteristics of changes in lipoprotein-related phospholipase A2 after intravenous thrombolysis with alteplase in patients with acute cerebral infarction and the effect on cognitive function recovery. Methods: One hundred and twenty-eight patients with acute cerebral infarction admitted to our hospital for intravenous thrombolysis with alteplase from January 2022 to February 2023 were selected. According to the evaluation results of the postoperative MoCA scale, the patients with MoCA score <26 were included in the poor recovery group (18 cases), and those with MoCA score ≥26 were included in the good recovery group (110 cases). The poor recovery group was randomly divided into a training set and a verification set. The change trend and clinical characteristics of Lp-PLA2 before and after treatment were compared between the two groups, and the relationship between the variables and outcome events (adverse recovery of cognitive function) was determined by multivariate Logistic regression analysis, and the prediction line graph and evaluation were drawn based on the risk factors. Then the model was stratified according to the risk score of the column graph by X-tile software, and the clinical application value of the model was further discussed. Results: The Lp-PLA2 levels in both groups were decreased after intravenous thrombolytic therapy with alteplase, but the decrease in the good recovery group (223.45±102.35 μg/L) was higher than that in the poor recovery group (289.36±110.23 μg/L), and the difference between the two groups was statistically significant (P<0.05). Age ≥65 years old, infarct site (frontal lobe), pre-thrombolysis hyperglycemia, fibrinogen, white blood cell count, Lp-PLA2, metabolic syndrome, APTT, and thrombolysis time in poor recovery group were all higher than those in good recovery group, while NIHSS score and MESSS score after treatment were lower than those in good recovery group (P<0.05). Infarct site (frontal cortex), pre-thrombolysis hyperglycemia, fibrinogen ≥2.89g/L, white blood cell count ≥8.00×109/L, Lp-PLA2≥291.36ng/mL, metabolic syndrome, APTT≥31.50s, thrombolysis time ≥4.50s, NIHSS score ≥15.00, and MESSS score ≥12.00 were the risk factor for poor cognitive recovery after intravenous thrombolysis of alteplase (P<0.05). The results of risk prediction model line graph evaluation showed that the C-index of training set and verification set were 0.862 and 0.826, respectively. The calibration curves of the two sets fit well with the ideal curves. The area under ROC curve (AUC) of the training and validation sets were 0.864 (95%CI: 0.789~0.898, P<0.001) and 0.834 (95%CI: 0.774~0.877, P<0.001), respectively. The decision curve showed that when the threshold probability was 0.01 ~ 0.91, the net benefit rate was > 0, which had a high net benefit value, safe and reliable, with strong practicability. The incidence of poor recovery in high risk group was significantly higher than that in medium risk group and low risk group (χ2=6.266, P=0.013). Conclusion: Based on this ideal and practical predictive model, we can identify and actively intervene early in patients with acute cerebral infarction after intravenous thrombolysis to improve their prognosis.

Methods: A retrospective analysis was performed on the clinical data of 108 patients with early NSCLC admitted to the hospital between January 2020 and October 2022. According to different surgical methods, they were divided into single-port thoracoscopic precise segmentectomy group (group A, 58 cases) and lobectomy group (group B, 50 cases). All were followed up for 5 months after surgery. The differences in perioperative indexes (total number of lymph nodes dissection, intraoperative blood loss, operation time, indwelling time of thoracic catheter, thoracic drainage volume, hospitalization time), tumor markers [cytokeratin-19-fragment (CYFRA21-1), carcinoembryonic antigen (CEA), carbohydrate antigen 50 (CA50)], lung function indexes [peak expiratory flow (PEF), forced vital capacity (FVC), forced expiratory volume in one second (FEV1)], differences in the Quality of Survival in Lung Cancer Scale (EORTC QLQ-LC43) scores, and postoperative complication rates were compared. Results: The differences in the total number of lymph node dissection, intraoperative blood loss, and complication rates between the two groups were not statistically significant (P>0.05), and the operative time in Group A was higher than that in Group B. The time of chest tube retention, chest drainage, and hospitalization were lower than that in Group B (P<0.05); the levels of CYFRA21-1, CEA, and CA50 in both groups were lower than those in the preoperative period at 3 d postoperatively (P<0.05), whereas the difference between the two groups before and after surgery was not statistically significant in the same group (P>0.05); at 5 months postoperatively, the levels of PEF, FVC, and FEV1 were lower than those in the preoperative period in both groups, whereas the decrease in group A was smaller than that in group B, with a smaller difference between the pre- and post-surgical periods (P<0.05); after 5 postoperative months, all EORTC QLQ-LC43 scores in group A were higher than those in group B (P<0.05).Conclusion: The single-port thoracoscopic lobectomy and segmentectomy have comparable curative effect and safety. However, the latter is more beneficial to shorten the rehabilitation process, improve lung function and quality of life.

DOI: 10.3969/j.issn.1006-6233.2023.09.018
Objective: To investigate the application value of shear wave elastography (SWE) in ultrasound-guided fine needle aspiration (FNA) of papillary thyroid carcinoma. Methods: The relevant data of 90 patients suspected of papillary thyroid carcinoma who were admitted to the hospital from January 2019 to December 2021 were retrospectively analyzed. All patients needed to undergo ultrasound-guided FNA. According to the ultrasound technique used, the patients were divided into conventional group (n=42, two-dimensional ultrasonography) and SWE group (n=48). The sampling results, the incidence of puncture complications, the pathological classification of Bethesda system, and the value in diagnosing benign and malignant lesions were compared between the two methods. Results: The success rate of sampling and the detection rate of 2 needles in the SWE group were higher than those in the conventional group, but only the detection rate of 2 needles showed a statistical significance (P<0.05). The incidence of puncture complications in the SWE group was significantly lower than that in the routine group (4.16% vs 21.42%, P<0.05). The pathological results of Bethesda classification system showed no statistically significant difference between the two groups, but the diagnosis rate of class II~VI in the SWE group was higher than that in the conventional group (P<0.05). With pathological results as the golden standard, the diagnostic sensitivity and accuracy in the SWE group for malignant lesions were higher than those in the conventional group (P<0.05). Conclusion: SWE is helpful for determining the location of FNA in patients suspected of papillary thyroid carcinoma, improving the detection rate of 2 needles, reducing puncture complications, and improving the diagnosis of papillary thyroid carcinoma to a certain extent.

DOI: 10.3969/j.issn.1006-6233.2023.09.019
Objective: To investigate the effect of robot-assisted tension screw internal fixation surgery on Majeed score and gait parameters in patients with pelvic fracture (PF). Methods: Eighty-six patients with PF in our hospital from Februarry 2022 to Februarry 2023 were selected and divided into two groups based on the treatment plan, 43 patients each. The control group underwent manual incisional repositioning and fixation, and the observation group underwent robot-assisted tension screw internal fixation. The surgical indicators (surgical time, intraoperative bleeding, time to first weight-bearing, length of hospital stay) of the two groups were compared, as well as Majeed score, Matta score, and pain status. Enzyme-linked immunosorbent assay (ELISA) was used to detect the serum stress response indicators Norepinephrine (NE), tumor necrosis factor-α (TNF-α), angiotensin Ⅱ (Ang Ⅱ), C-reactive protein (CRP) before and after surgery. Fully automated chemiluminescence immunoassay analyzer was used to analyze serum fracture healing-related indexes alkaline phosphatase (ALP), osteocalcin (BGP), osteoprotegerin (OPG), postoperative gait parameters (stride speed, stride length, and percentage of support time), and adverse effects. Results: The operating time, intraoperative bleeding, time to first weight-bearing on the ground and hospital stay were less in the observation group than in the control group (P<0.05); the excellent rates of Majeed score and Matta score were higher in the observation group than in the control group at 3 months after surgery (P<0.05); the degree of postoperative pain was lower in the observation group than in the control group (P<0.05). The serum NE, TNF-α, Ang II and CRP levels in the observation group were lower than those in the control group at 24 h and 72 h after surgery, and the serum ALP, BGP and OPG levels were higher than those in the control group at 1 and 3 months after surgery (P<0.05); the gait speed, stride length and percentage of support time in the observation group were higher than those in the control group at 3 and 6 months after surgery (P<0.05); the incidence of adverse reactions in the observation group was lower than that in the control group (P<0.05). Conclusion: Robot-assisted tension screw internal fixation surgery for PF can optimize the surgery, reduce the stress response and relieve the patient's pain, while promoting fracture healing, improving the patient's pelvic function, restoring the body's coordination, and improving the safety of the surgery.

DOI: 10.3969/j.issn.1006-6233.2023.09.020
Objective: To explore the efficacy of ilioinguinal approach, modified Stoppa approach and parabdominis rectus approach in the treatment of pelvic and acetabular fractures. Methods: The data of patients with pelvic and acetabular fractures in the hospital from August 2020 to August 2022 were retrospectively analyzed, and the patients were classified into three groups according to the different regimens. Patients with ilioinguinal approach were selected as group A, patients with modified Stoppa approach were included in group B, and patients with parabdominis rectus approach were enrolled as group C. According to the propensity matching scoring method, 50 cases in group A, 50 cases in group B, and 52 cases in group C were finally obtained. There were no statistical differences among the three groups in terms of baseline data (P>0.05). The general surgery, postoperative hospital stay, quality of fracture reduction at 7 days after surgery, hip function (modified Merle D’ Aubignéand Postel scoring system) at different time points (before surgery, at discharge and at the last follow-up at 6 months after surgery) and occurrence of postoperative complications were compared among the three groups. Results: There were statistically significant differences in fracture end exposure time, intraoperative blood loss and postoperative Hb among the three groups (P<0.05). The exposure time in groups B and C was obviously shorter compared with that in group A (P<0.05), and the intraoperative blood loss was significantly less (P<0.05), and the postoperative Hb was significantly higher (P<0.05), but there were no statistical differences in the above surgical indicators between group B and group C (P>0.05). There was no statistically significant difference in the excellent and good rate of fracture reduction quality among the three groups at 7 days after surgery (P>0.05). Although the excellent and good rate in group C was higher than that in group A and group B, there was no statistical difference between the two groups (P>0.05). At discharge and at the last follow-up at 6 months after surgery, the Postel score of the three groups was gradually improved, and there was a statistical significance from the aspect of time-point effect among the three groups (P<0.05), but there was no statistical difference in the score from the aspects of between-group effect and interaction effect of between-group and time-point (P>0.05). The Postel score in the three groups at discharge and at the last follow-up at 6 months after surgery was higher than that before surgery (P<0.05). The total postoperative complication rate was significantly different in the three groups (P<0.05), and the rate in group C was significantly lower than that in group A (9.62% vs 36.00%, P<0.05). Conclusion: All three kinds of plate internal fixation in the treatment of pelvic and acetabular fractures can achieve good fracture reduction quality and restore hip function, but modified Stoppa approach and parabdominis rectus approach have better effects on exposure time, blood loss and postoperative Hb than ilioinguinal approach, and parabdominis rectus approach has a higher excellent and good rate of fracture reduction than the other two approaches and fewer complications.

DOI: 10.3969/j.issn.1006-6233.2023.09.022
Objective: To analyze the influencing factors of rehabilitation training effect in elderly post-stroke hemiplegia patients, and to construct a nomogram model to predict the effect of rehabilitation training. Methods: The clinical data of 286 elderly patients with hemiplegia after acute stroke who received rehabilitation training in Guiping People's Hospital from July 2017 to June 2021 were retrospectively collected and randomly divided into modeling group (n=200) and validation group (n=86) with the ratio of 7∶3. The data of the modeling group were used to construct a risk prediction nomogram model, and the data of the validation group were used to evaluate the performance of the model. The two groups were divided into a good effect group and a poor effect group according to the effect of rehabilitation training. Univariate and multivariate Logistic regression analyses were used to identify the risk factors for poor rehabilitation training effect, R3.6.3 software was applied to build a nomogram model for predicting poor rehabilitation effect, and the effect of the nomogram model in predicting poor rehabilitation effect was verified. Results: Among the 286 hemiplegic patients, 42.66% (122/286) had poor rehabilitation effect. The age, male proportion, alcohol consumption proportion, hypertension proportion, rehabilitation training start time >2 weeks, and rural living environment proportion of patients in the poor effect group were significantly higher than those in the good effect group, with significant differences (P<0.05). Through multivariate Logistic regression model, it was found that living environment, age, hypertension, rehabilitation training start time >2 weeks, and alcohol consumption were the risk factors for poor rehabilitation effect of hemiplegic patients (P<0.05). The area under the ROC of the modeling group was 0.801, and the area under the curve of the validation group was 0.851; the slopes of the calibration curves of both the modeling group and the validation group were close to 1. Conclusion: Based on age, drinking, high blood pressure, rehabilitation training start time, and living environment, the nomogram model for predicting poor rehabilitation training effect in elderly post-stroke hemiplegia patients can better predict poor rehabilitation training effect individually, and it has certain guiding significance for early intervention in rehabilitation training.

Methods: A total of 120 patients with early EC treated in the hospital were enrolled as the research objects between October 2021 and February 2022, while 120 patients undergoing hysteromyoma excision during the same period were enrolled as control group. All underwent transvaginal sonography contrast-enhanced ultrasonography (TVS-CEUS) examination. Taking pathological diagnosis as the golden standard, diagnostic efficiency of CEUS for EC was analyzed. The peripheral blood was collected to detect levels of carbohydrate antigen 125 (CA125) and vascular endothelial growth factor (VEGF). The relative expression levels of serum microRNA-125a (miR-125a) and miR-200a were detected by qRT-PCR. The correlation between CA125, VEGF and miR-125a, miR-200a was analyzed. The diagnostic efficiency of CEUS combined with CA125, VEGF, miR-125a and miR-200a for EC was evaluated by receiver operating characteristic (ROC) curves. Results: The sensitivity, specificity, accuracy, positive predictive value, negative predictive value and kappa value of TVS-CEUS in the early diagnosis of EC were 81.67%, 71.67%, 76.67%, 74.24%, 79.63%, and 0.533, respectively. The levels of serum CA125 and VEGF, and relative expression level of miR-200a in EC group were higher than those in control group, while relative expression level of miR-125a was lower than that in control group (P<0.05). The results of Pearson correlation analysis showed that levels of serum CA125 and VEGF were negatively correlated with relative expression level of miR-125a (P<0.05), while positively correlated with relative expression level of miR-200a (P<0.05). The result of ROC curve analysis showed that AUC of TVS-CEUS combined with CA125, VEGF, miR-125a, and miR-200a in the early diagnosis of EC was significantly greater than that of CA125, VEGF, miR-125a, and miR-200a alone (P<0.05). Conclusion: TVS-CEUS combined with serum tumor markers has high early diagnostic efficiency for EC, which can provide reference for condition evaluation of patients.

DOI: 10.3969/j.issn.1006-6233.2023.09.026
Objective: To observe the effects of electronic bronchoscopy-guided stent placement on blood gas indexes, shortness of breath, and pulmonary function in patients with malignant airway stenosis. Methods: A total of 120 patients with malignant airway stenosis who were treated with electronic bronchoscopy-guided stent placement were included in this study. The success rate of one-time placement, complications and 6-month postoperative survival were observed. Blood gas indexes [partial pressure of oxygen (PaO2), partial pressure of carbon dioxide (PaCO2) and oxygen saturation (SaO2)], classification of shortness of breath, pulmonary function indicators [forced expiratory volume in 1s (FEV1), forced vital capacity (FVC) and peak expiratory flow (PEF)], and ECOG score were compared before and after treatment. Results: The success rate of one-time placement was 96.67%. After operation, there were 3 patients with obvious bloody phlegm, 9 patients with a little blood in sputum, and 8 patients with mild substernal pain, which all disappeared after symptomatic treatment. After treatment, dyspnea in all patients was relieved significantly. 21 (17.5%) patients died from the primary disease in 6 months after operation. After treatment, PaO2 and SaO2 increased significantly (P<0.05), and PaCO2 decreased significantly (P<0.05). Shortness of breath was significantly improved (P<0.05). FEV1, FVC and PEF increased significantly (P<0.05), and ECOG score decreased significantly (P<0.05). Conclusion: Electronic bronchoscopy-guided stent placement is safe and effective in the treatment of malignant airway stenosis. It can significantly improve blood gas indexes, shortness of breath and pulmonary function.

YANG Yi, LI Qun, HE Mengjun, et al
DOI: 10.3969/j.issn.1006-6233.2023.09.027
Objective: To investigate the relationship between the levels of C-reactive protein (CRP), suppression of tumorigenicity 2 (ST2), procalcitonin (PCT), lactic acid, and non-invasive ventilation in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) complicated with respiratory failure. Methods: A total of 146 patients with AECOPD and type II respiratory failure treated in our hospital from January 2021 to June 2022 were selected as the observation group, and 100 patients with simple AECOPD were selected as the control group. The levels of CRP, ST2, PCT and lactic acid in the observation group and the control group were compared. The observation group was given non-invasive ventilation treatment, and the clinical data, CRP, ST2, PCT and lactic acid levels of patients with treatment failure and success were compared. Results: The levels of CRP, ST2, PCT and lactic acid in the observation group were significantly higher than those in the control group (P<0.05). The course of COPD, APACHE Ⅱ score, fasting blood glucose, CRP, ST2, PCT and lactic acid levels of patients with treatment failure in the observation group were significantly higher than those of patients with treatment success (P<0.05). Logistic regression analysis showed that APACHE Ⅱ score, CRP, ST2, PCT and lactic acid were the influencing factors for the failure of noninvasive ventilation treatment (P<0.05). The area under the ROC curve of APACHE Ⅱ score, CRP, ST2, PCT, lactic acid and the combination of them to predict treatment failure were 0.662, 0.698, 0.717, 0.658, 0.759 and 0.806 respectively (P<0.05). Conclusion: The levels of CRP, ST2, PCT and lactic acid in AECOPD patients with type II respiratory failure increased significantly. The evaluation based on the patient's disease progress and APACHE Ⅱ score, CRP, ST2, PCT and LCR can provide a more comprehensive reference and basis for predicting the effect of ventilation treatment.

LU Wen, GONG Chen, XIANG Wen, et al
DOI: 10.3969/j.issn.1006-6233.2023.09.028
Objective: To explore the changes in total cholesterol/high density lipoprotein (TC/HDL), apolipoprotein A1 (ApoA1) and lipoprotein (a) [Lp(a)] and their relationship with metabolic syndrome (MS) in patients with schizophrenia. Methods: A total of 93 patients with schizophrenia in the hospital were selected as the study subjects. General data of patients were counted. The levels of serum TC/HDL, ApoA1 and Lp(a) were detected by enzyme-linked immunosorbent assay, and the patients were divided into MS group (n=32) and non-MS group (n=61) according to whether they were complicated with MS. The general data and levels of serum TC/HDL, ApoA1 and Lp(a) were compared between the two groups. Multivariate Logistic regression analysis was used to analyze the risk factors of MS in patients with schizophrenia. Results: The BMI, SBP, DBP, FPG, serum TG and LDL-C in MS group were significantly higher than those in non-MS group (P<0.05) while the level of HDL-C was significantly lower than that in non-MS group (P<0.05). There were no statistically significant differences in gender, age, course of disease, smoking history, drinking history, antipsychotic drug use, serum TC level, and PANSS score between the two groups (P>0.05). TC/HDL and Lp(a) in MS group were significantly higher than those in non-MS group (P<0.05) while ApoA1 was significantly lower than that in non-MS group (P<0.05). Multivariate Logistic regression analysis showed that BMI, FPG, serum TG, LDL-C, Lp(a) and TC/HDL were risk factors affecting MS in patients with schizophrenia, and serum ApoA1 was a protective factor (P<0.05). Conclusion: Serum TC/HDL, ApoA1 and Lp (a) levels are the influencing factors of MS in patients with schizophrenia, and the increases of TC/HDL and Lp (a) and the decrease of ApoA1 can increase the risk of MS in patients.

DOI: 10.3969/j.issn.1006-6233.2023.09.029
Objective: To explore the application effect of Xueshuantong capsule combined with Conbercept in patients with central retinal vein occlusion (CRVO) and macular oedema. Methods: A total of 90 patients with ischemic CRVO and macular oedema were enrolled between October 2019 and September 2021. According to simple random grouping method, they were divided into group A (45 cases, 45 eyes, Conbercept combined with Xueshuantong capsules) and group B (45 cases, 45 eyes, Conbercept). All were treated for 3 months. The onset time of visual acuity, retinal circulation time, central macular thickness (CMT), fundus hemodynamics, best corrected visual acuity (BCVA), serum indexes and adverse reactions in the two groups were compared. Results: The response rate of treatment in the two groups was similar (χ2=0.179, P>0.05). Compared with group B, onset time of visual acuity was shorter in group A (t=2.237, P<0.05). After treatment, retinal circulation time was similar in the two groups (t=1.149, P>0.05). After 2 and 3 months of treatment, CMT was thinner in group A than group B (t=3.884, 2.104, P<0.05). Compared with group B after treatment, increments of peak systolic velocity and end-diastolic velocity were greater in group A, while decrements of resistance index and pulsatility index were greater in group A (t=7.240, 6.452, 6.202, 6.442, P<0.05). After 1 month of treatment, BCVA log MAR was lower in group A than group B (t=2.472, P<0.05). Compared with group B after treatment, increment of serum nitric oxide synthetase was greater in group A, while decrements of interleukin-6 and vascular endothelial growth factor were greater in group A (t=7.799, 15.034, 4.973, P<0.05). The incidence of adverse reactions was similar in the two groups (P>0.05). Conclusion: Compared with Conbercept alone, Conbercept combined with Xueshuantong capsule is conducive to the relief of macular edema, improvement of retinal microcirculation, and vision recovery in CRVO.
