1585 Effect of Midazolam Regulation of miR-875-5p/STAG2 Axis on Proliferation and Apoptosis of Esophageal Cancer Cells

Objective: To investigate the effects of midazolam (MID) on the proliferation and apoptosis of esophageal cancer cells by regulating the miR-875-5p/stromal antigen 2 (STAG2) axis. Methods: Esophageal cancer Eca109 cells were treated with different concentrations of MID (0,10,20,30,40 μmoL/L) for 24h,and the appropriate MID concentration was screened by CCK-8 method; high-throughput miRNA transcriptomics was used to screen the differentially expressed miRNAs in 30 μmoL/L-treated Eca109 cells; miR-875-5p and STAG2 mRNA expression levels were examined by qRT-PCR; the relationship between miR-875-5p and STAG2 was verified by dual luciferase assay; miR-875-5p mimics (miR-875-5p mimics) were transfected in Eca109 cells and intervened with 30 μmoL/L MID to obeserve survival rate,apoptosis rate and cell cycle changes;Western blot was used to detect the expression level of STAG2 protein;in vivo xenograft tumor experiments were conducted to detect the effects of MID and miR-875-5p on the growth of xenograft tumors in nude mice. Results: 30μmoL/L MID was used for follow-up research; miR-875-5p was screened as significantly down-regulated miRNAs in Eca109 cells treated with 30μmoL/L; 30μmoL/L MID intervention or down-regulation of miR-875-5p expression could reduce the proliferation ability,miR-875-5p expression,and the proportion of cells in S phase of Eca109 cells,while the apoptotic rate and the proportion of cells in G0/G1 phase increased (P<0.05 ); there is a negative regulatory relationship between miR-875-5p and STAG2; up-regulating the expression of miR-875-5p can reverse the effects of MID on the proliferation,apoptosis and cell cycle of Eca109 cells; at the same time,it can also reverse the inhibitory effect of MID on the growth of transplanted tumors in nude mice. Conclusion: MID targets and down-regulates the level of miR-875-5p to promote the expression of STAG2,thereby inhibiting the proliferation and promoting cell apoptosis of Eca109 cells.

1591 LncRNA-NKILA Targeting miR-3195 Mechanism on Oral Cell Cell Activity and Vascular Generation

Objective: To investigate the effect of lncrNA-NKILA targeting miR-3195 on the activity and angiogenesis of oral cancer cells. Methods: The expression levels of LncRNA-NKILA in human normal oral epithelial cells and human oral squamous carcinoma cells were measured by qRT-PCR.According to the random number table method,they were divided into empty plasmid group (SCC25 cell line + pcDNA),NEAT1 group (SCC25 cell line + pcDNA-NKILA),NC-group (SCC25 cell line + miR-3195-NC-mimics); miR-3195 group (SCC25 cell line + miR-3195-mimics),and Combination 1 group (SCC25 cell line + pcDNA-NKILA + miR-3195-NC-mimics) and Combination 2 group (SCC25 cell line + pcDNA-NKILA + miR-3195-mimics).Cell proliferation was detected by CCK-8 method; apoptosis rate was detected by flow cytometry,VEGF and VEGF-C proteins were detected by immunoblotting; and miR-3195 regulation by LncRNA-NKILA was detected by luciferase method. Results: SCC25 cell lines transfected with pcDNA-NKILA showed decreased cell proliferation,increased apoptosis,and decreased VEGF and VEGF-C proteins,compared with the empty plasmid group (P<0.05); compared with the NC group,SCC25 cell lines in the miR-3195 group showed decreased proliferation,increased apoptosis,and decreased VEGF and VEGF-C proteins (P<0.05).The proliferation of SCC25 cell lines was reduced,apoptosis rate was increased,and VEGF and VEGF-C proteins were reduced in the combination 2 group compared with the combination 1 group (P<0.05); luciferase assay showed that miR-3195 was a LncRNA-NKILA target gene. Conclusion: LncrNA-NKILa can inhibit the proliferation of oral cancer cells and promote their apoptosis by regulating the expression of miR-3195,and the study mechanism may be related to the inhibition of VEGF expression.

1597 Naringin Promotes Osteogenic Differentiation of Human Adipose Mesenchymal Stem Cells via the cAMP/PKA/CREB Pathway

Objective: To investigate the role of naringin in promoting osteogenic differentiation of human adipose derived mesenchymal stem cells through the cyclic adenosine phosphate (cAMP)/protein kinase A (PKA)/cAMP response element binding protein (CREB) pathway. Methods: The cells were divided into control group,naringin low,medium and high dose group,cAMP inhibitor (SQ22536) group and osteogenic differentiation induction group (induction group).12.5 μmoL/L,25 μmoL/L and 50 μmoL/L naringin were added to the naringin low,medium and high dose groups,respectively,and 100 μmoL/L SQ22536 was added to the SQ22536 group.L SQ22536,10~8 moL/L dexamethasone,10 mmoL/L sodium β-glycerophosphate and 50mg/L ascorbic acid were added to the induction group; only equal volumes of culture solution were added to the control group,and six replicate wells were set up for each group.After 48h of incubation,ALP activity was measured by alkaline phosphatase (ALP) staining and p-nitrophenyl phosphate (PNPP); calcium deposition was observed by Alizarin Red S (ARS) staining; cAMP level was measured by ELISA; PKA,CREB,RUNX2,Osteocalcin (OCN),OPN messenger ribonucleic acid (mRNA) expression was measured by RT-qPCR.The expression of PKA,CREB,RUNX2,osteocalcin (OCN) and osteopontin (OPN) messenger ribonucleic acid (mRNA) was measured by RT-qPCR,and the expression of PKA,CREB,RUNX2,OCN,OPN and p-PKA and p-CREB were measured by WB. Results: ALP staining showed a small amount of blue precipitation in the control group,with the least amount of blue precipitation in the SQ22536 group and gradually increasing in the induction and naringin 3 kinds of dose groups.ALP activity was significantly decreased in the SQ22536 group compared to the control group (P<0.01); ALP activity was significantly increased in the induction and naringin 3 dose groups compared to the control and SQ22536 groups (P<0.01).A small amount of calcium deposition was present in the control group,significantly reduced in the SQ22536 group and gradually increased in the induction and naringin 3 dose groups; cellular cAMP levels,PKA,CREB,RUNX2,OCN,OPN mRNA and protein expression and p-PKA and p-CREB levels were significantly reduced in the SQ22536 group compared to the control group (P<0.01).Compared with the control and SQ22536 groups,the cAMP level,PKA,CREB,RUNX2,OCN,OPN mRNA and protein expression and p-PKA and p-CREB levels increased in the induction and naringin 3 dose groups,and the differences were statistically significant (P<0.01); the differences between the above indices in the induction and naringin low dose groups were not statistically significant ( P>0.05); the above indicators naringin showed a dose dependence. Conclusion: Naringin can promote osteogenic differentiation of human adipose derived mesenchymal stem cells,which may be related to the activation of cAMP/PKA/CREB signaling pathway,promotion the expressions of RUNX2,OCN and OPN,and up-regulation of ALP activity.

1604 Effect of miR-218 Targeting PI3K/Akt/mTOR Signaling Pathway on Thyroid Cancer Cells

Objective: To investigate the effects of microRNA-218 (miR-218) on the proliferation,migration and invasion of thyroid carcinoma cells through the phosphatidylinositol 3-kinase (PI3K)/protein kinase B (AKT)/mammalian target of rapamycin (mTOR) signaling pathway. Methods: 8505C cells in logarithmic growth phase were divided into:control group (untreated 8505C cells),miR-218 NC group (transfected with negative mimics),miR-218 mimics group (transfected with miR-218 mimics),pathway activator group (100 ng/mL PI3K/AKT/mTOR pathway activator IGF-1),and miR-218 mimics+pathway activator group (miR-218 mimics+100 ng/mL IGF-1).The expression of proliferation protein (ki-67),migration and invasion proteins [matrix metalloproteinase 2 (MMP-2),matrix metalloproteinase 9 (MMP-9)] and PI3K/AKT/mTOR signaling pathway related proteins in 8505C cells were detected. Results: Compared with the miR-218 NC group,the expression level of miR-218 in 8505C cells in the miR-218 mimics group [(1.01±0.17) vs (2.31±0.39)] was significantly up-regulated (P<0.05),the OD450 value [(2.21±0.07) vs (0.81±0.02)],cell scratch healing rate [(34.08±3.17) vs (11.36±0.21)]%,number of invasion [(81.23±5.68) vs (42.84±2.25)],and the protein expression levels of ki-67 [(1.68±0.15) vs (0.73±0.02)],MMP-2 [(0.88±0.07) vs (0.39±0.02)],MMP-9 [(2.68±0.27) vs (0.67±0.02)],p-PI3K [(2.87±0.24) vs (0.36±0.03)],p-AKT [(0.93±0.05) vs (0.55±0.01)],and p-mTOR [(1.64±0.16) vs (0.47±0.02)] were significantly down-regulated (P<0.05). Conclusion: MiR-218 may inhibit the proliferation,migration and invasion of 8505C cells by inhibiting the activation of PI3K/Akt/mTOR signaling pathway.

1609 Transformation of Subcellular Structure and Expression Profile in Distant Cancerous Tissues of Liver Metastases from Colorectal Cancer

Objective: To investigate the changes of subcellular structure and expression profile of distal tissue in colorectal cancer liver metastasis. Methods: The structural changes of human liver were observed by electron microscope,and confirmed in a colorectal cancer liver metastasis mice model.Transcriptome profiling was used to explore the differential expressed genes in distal tissues in the mice model. Results: The subcellular structure of liver cells in distant tissue in both human and mouse liver metastases were obviously different from that of healthy liver cells.Transcriptome analysis showed that differentiated genes in distal tissues were enriched to pathways related to tumor. Conclusion: The morphological structure and gene expression of distal tissues were also affected by the liver metastasis.

1614 Effect of LncRNA LUCAT1 on Proliferation Apoptosis and Invasion of Non-Small Cell Lung Cancer Cells by Regulating miR-493-5p/RAC1 Axis

Objective: To elucidate the underlying mechanism of the long non-coding RNA (lncRNA) lung cancer associated transcript 1 (LUCAT1) on non-small cell lung cancer (NSCLC). Methods: RT-qPCR and Western blot were used to determine gene and protein expression levels.Dual-luciferase reporter gene (DLRG) was used to verify the relationship between miR-493-5p and LUCAT1 or RAC1.CCK-8,colony formation,flow cytometry and Transwell were applied to evaluate the cell biological behaviors.In addition,the biological function of LUCAT1 in NSCLC was investigated by tumor xenograft experiments. Results: The expression of LUCAT1 and RAC1 mRNA was up-regulated in NSCLC tissues and cell lines,and the expression of miR-493-5p was down-regulated (P<0.05); moreover,in NSCLC,it was observed that LUCAT1 was negatively correlated with miR-493-5p and positively correlated with RAC1 mRNA,and miR-493-5p was negatively correlated with RAC1 mRNA (P<0.05).DLRG confirmed the interaction between miR-493-5p and LUCAT1 or RAC1 (P<0.05).Silencing LUCAT1 could significantly up-regulate the level of miR-493-5p and down-regulate the level of RAC1 (P<0.05).Silencing LUCAT1 significantly attenuated cell proliferative viability,decreased invasion abilities,and promoted cell apoptosis (P<0.05).In addition,inhibition of miR-493-5p or overexpression of RAC1 could partially reverse the effects of LUCAT1 silencing on NSCLC cells (P<0.05).In vivo xenograft tumor experiments confirmed that silencing LUCAT1 significantly inhibited the growth of NSCLC tumors in vivo (P<0.05). Conclusion: LUCAT1 silencing inhibits NSCLC tumorigenesis by regulating the miR-493-5p/RAC1 axis.

1620 The Mechanism Research of Survivin Regulating Migration and Invasion in Esophageal Cancer via Wnt/β-catenin Signaling Pathway

Objective: To investigate the expression of Survivin in esophageal cancer and its effect on Wnt/β-catenin signaling pathway,invasion and migration in esophageal cancer cells. Methods: The expression of Survivin protein in human esophageal cancer tissues and EC109 cells was examined and its correlation with clinical case characteristics was analyzed.Survivin small interfering RNA was transfected into oesophageal cancer EC109 cells by liposome mediated method.Relative expression of gene mRNA was detected by RT-qPCR.Protein expression was detected by immunoprotein blotting (Western blot).Cell viability,migration and invasion ability were measured by MTT,scratch healing and Transwell assays. Results: The expression of Survivin was increased in esophageal cancer tissues and EC109 cells.The TNM stage,differentiation and lymph node metastasis of esophageal cancer tissues correlated with positive Survivin expression; down-regulation of Survivin by RNA interference significantly inhibited Survivin mRNA and protein expression in esophageal cancer cells.The expression of wnt3α,β-catenin,cyclinD1 and c-myc in esophageal cancer cells decreased after down-regulation of Survivin,and the Wnt/β-catenin signaling pathway was inhibited,and the survival,migration and invasion ability of esophageal cancer cells were suppressed. Conclusion: Down-regulation of Survivin gene can inhibit the survival,migration and invasion of esophageal cancer cells by affecting the Wnt/β-catenin signaling pathway.

1625 Correlation and Clinicopathological Significance of lncRNA TUSC7 and miR-1270 Expression in Colorectal Cancer Tissues

Objective: To investigate the correlation between long non-coding RNA (lncRNA) tumor suppressor candidate 7 (TUSC7) and microRNA-1270 (miR-1270) in colorectal cancer (CRC) tissues and their relationship with the prognosis of CRC patients. Methods: The lnCAR database was used to search the expression of lncRNA TUSC7 in CRC and normal colorectal tissues.A total of 130 CRC patients accepted by our hospital from June 2018 to January 2019 were regarded as the research subjects.The surgically resected CRC and paracancerous tissues were collected,and the expression levels of lncRNA TUSC7 and miR-1270 in CRC and paracancerous tissues were detected; the correlation between the expression level of lncRNA TUSC7 and miR-1270 in CRC tissues was analyzed; the relationship between the expression levels of lncRNA TUSC7 and miR-1270 in CRC tissues and the clinicopathological characteristics and prognosis of CRC were analyzed,Bioinformatics website was used to predict the relationship between lncRNA TUSC7 and miR-1270. Results: The lnCAR database analysis showed that the expression level of lncRNA TUSC7 in CRC tissue was lower than that in normal colorectal tissue (P<0.05); qRT-PCR results showed that the expression level of lncRNA TUSC7 in CRC tissue was lower than that in adjacent tissue (P<0.05),and the expression level of miR-1270 was higher than that in adjacent tissue (P<0.05); the expression levels of lncRNA TUSC7 and miR-1270 in CRC tissue were correlated with the degree of differentiation,lymph node metastasis,depth of invasion and TNM stage (P<0.05); bioinformatics analysis showed that lncRNA TUSC7 had a targeted binding site for miR-1270; the expression level of lncRNA TUSC7 in CRC tissue was negatively correlated with miR-1270 (r=-0.578,P<0.05); the cumulative survival rate of CRC patients in lncRNA TUSC7 low expression group and miR-1270 high expression group was lower than that in lncRNA TUSC7 high expression group and miR-1270 low expression group (P<0.05). Conclusion: The expression of lncRNA TUSC7 is down-regulated and the expression of miR-1270 is up-regulated in CRC tissue,and the two are negatively correlated,which provides a new marker for clinical diagnosis and treatment of CRC and evaluation of the prognosis of CRC patients.

1631 Clinical Outcomes of DLSS Combined with LS in Patients Treated by Posterior Open-Loop Decompression Orthopedic Fusion Internal Fixation

Objective: To investigate the clinical efficacy of posterior fenestration, decompression, orthopedic fusion and internal fixation (PLIF) in the treatment of DLSS combined with LS. Methods: 109 patients with DLSS and LS were randomly divided into observation group (n=55) and control group (n=54). The control group was treated with total laminectomy (LAM) and intervertebral fusion IF, and the observation group was treated with PLIF. The effects of different surgical methods on lumbar stability, sagittal spine morphology lumbar dysfunction were compared. And complications were recorded. Results: There was a significant difference between the two groups in terms of vertebral fusion one year after operation (P<0.05). The intraoperative blood loss in the observation group was significantly lower than that in the control group, the operation, bed rest and hospitalization time were lower than that in the control group, and the incision length was shorter than that in the control group (P<0.05). After surgery, lumbar lordosis angle (LL), shapeless vertical axis (SVA), pelvic incidence (PI), thoracic kyphosis angle (TK) and pelvic tilt angle (PT) in both groups were significantly decreased, but there was no significant difference between the two groups in the above indexes before and after operation (P>0.05). After operation, the spinal dysfunction score (ODI) and pain score (VAS) of the two groups were significantly decreased, and the human dysfunction score (JOA), intervertebral space height, intervertebral foramen height, and lumbar lordosis angle were increased. The difference of the above indexes before and after operation between the two groups was statistically significant (P<0.05) Conclusion: In the clinical treatment of patients with DLSS and LS, PLIF and LAM interbody fusion IF have the same effect in spinal canal fusion and spinal morphology improvement, but the former has less trauma, low pain and less blood loss, and can effectively promote the recovery of limb function, which is worthy of clinical promotion.

1636 Efficacy of Ceftriaxone Sodium Combined with Dexamethasone in the Treatment of Children with Purulent Meningitis and Its Effects on Serum IFN-γ and IL-4 Levels

Objective: To observe the efficacy of ceftriaxone sodium combined with dexamethasone in the treatment of purulent meningitis (PM) in children and the effects on levels of serum interferon (IFN-γ) and interleukin 4 (IL-4). Methods: Using the random remainder grouping method of random number table method, 96 children with PM who were admitted to the hospital from March 2019 to March 2022 were randomly divided into the observation group and the control group, with 48 cases in each group. Both groups were given basic symptomatic treatment, and the control group underwent ceftriaxone sodium therapy additionally while the observation group underwent ceftriaxone sodium + dexamethasone therapy additionally, and both group were treated for 2 weeks. After 2 weeks of treatment, the clinical efficacy and recovery status of symptoms and signs of the two groups were compared, and the levels of inflammatory factors and serum related factors were observed in the two groups before and after treatment. Results: After 2 weeks of treatment, the total clinical effective rate of the observation group was higher than that of the control group (97.92% VS 87.50%) (P<0.05). The fever abatement time, cerebrospinal fluid recovery time and recovery time of peripheral blood leukocyte of (2.34±0.76) d, (7.65±0.83) d and (5.24±1.27) d in the observation group were shorter than (3.62±0.81) d, (9.88±1.12) d and (7.18±2.03) d in the control group (P<0.05). After 2 weeks of treatment, serum levels of INF-γ, IL-4, TNF-α, PCT, MMP-9 and sB7-H3 in the two groups were lower than those before treatment, and the differences before and after treatment were statistically significant between the observation group [(4.31±1.08) pg/mL, (53.54±4.17) pg/mL, (91.85±19.90) ng/L, (12.51±2.68) ng/mL, (251.09±24.07) μg/L, (1850.66±269.54) ng/L] and the control group [(2.97±1.38) pg/mL, (45.83±0.38) pg/mL, (75.98±20.76) ng/L, (8.76±2.79) ng/mL, (204.51±18.88) μg/L, (1320.23±240.71) ng/L] (P<0.05). Conclusion: Ceftriaxone sodium+dexamethasone can shorten the recovery times of symptoms and signs, reduce the levels of inflammatory factors, and promote the rehabilitation of children with PM.

1640 Risk Factors for Gestational Diabetes Mellitus Combined with Hypothyroidism and the Impact on Pregnancy Outcome

Objective: To analyze the risk factors for gestational diabetes mellitus combined with hypothyroidism and its impact on maternal pregnancy outcome. Methods: A total of 114 parturients with gestatiional diabetes mellitus combined with hypothyroidism admitted from January 2019 to December 2021 were retropectively selected as the clinical observation group, and 114 parturients with gestational diabetes mellitus alone admitted during the same period were selected as the control group. The influencing factors were explored and the impact on pregnancy outcomes was analyzed. Results: Univariate analysis showed that gestational hypertension, previous history of thyroid disease, family history of thyroid disease, parity, PT,TT,APTT,FDP,PTR,INR,ALT and UA were risk factors for gestational diabetes combined with hypothyroidism (P<0.05). Multivariate Logistic regression analysis showed that previous gestational hypertension, previous history of thyroid disease, combined family history of thyroid disease, PT, TT were independent influencing factors for gestational diabetes complicated with hypothyroidism (P<0.050). The incidence of premature rupture of membranes (16.67% vs 6.14%), premature delivery (12.28% vs 4.38%), neonatal hypoglycemia (6.14% vs 1.75%) and fetal distress (7.89% vs 1.75%) in the observation group were significantly higher than those in the control group (P<0.05). Conclusion: Pregnancy diabetes combined with hypothyroidism will increase the incidence of adverse pregnancy outcomes, and the history of pregnancy induced hypertension, thyroid disease, family history of thyroid disease and abnormal Pt and TT are the key risk factors.

Objective: To observe the changes in levels of postoperative serum neuropeptide Y (NPY), substance P (SP) and prostaglandin E2 (PGE2) in patients with spontaneous pneumothorax, and to explore their relationship with postoperative recurrence. Methods: A total of 80 patients with spontaneous pneumothorax who were admitted between May 2017 and December 2020 were selected as the case group, and 50 healthy subjects undergoing physical examination during the same period were included in the control group. All patients underwent single-port thoracoscopic pulmonary bulla resection. The changes in serum NPY, SP and PGE2 levels were detected in the case group before surgery and at 30 d after surgery and were compared with those in the control group. At 1 year of follow-up, the postoperative recurrence was statistically analyzed, and univariate analysis and multivariate Logistic regression analysis were used to explore the influencing factors of postoperative recurrence. Receiver operating characteristic curve (ROC curve) was plotted to explore the predictive value of postoperative serum NPY, SP and PGE2 levels on recurrence in patients. Results: The levels of serum NPY, SP and PGE2 after surgery in the case group were reduced compared with those before surgery (P<0.05), and the levels of preoperative serum NPY, SP and PGE2 in the case group were higher than those in the control group (P<0.05), and the levels of serum SP and PGE2 after surgery were higher than those in the control group (P<0.05). There were 11 cases of recurrence among 80 patients, with the recurrence rate of 13.75%. Univariate analysis showed that there were statistical differences between the recurrence group and the non-recurrence group in terms of body mass index, postoperative smoking, presence or absence of postoperative pulmonary bulla omission, presence or absence of pleurodesis, presence or absence of postoperative persistent air leakage, postoperative serum SP and postoperative serum PGE2 (P<0.05). Multivariate Logistic regression analysis showed that postoperative smoking, pulmonary bulla omission, persistent air leakage, serum SP and serum PGE2 were the influencing factors of postoperative recurrence of spontaneous pneumothorax (P<0.05). The areas under the ROC curves of serum SP and PGE2 on predicting recurrence were 0.921 and 0.872 respectively (P<0.05), and the cut-off value, sensitivity and specificity were 1.85 μg/mL, 90.91% and 85.51% of SP, and were 172.7 pg/mL, 72.73% and 92.75% of PGE2 respectively. Conclusion: The expressions of serum NPY, SP and PGE2 in patients with spontaneous pneumothorax are significantly increased, while the levels can be significantly decreased after surgical treatment. The expressions of serum SP and PGE2 in patients with recurrent pneumothorax are significantly higher than those in patients without recurrence, and the two levels may be used as predictors of postoperative recurrence of spontaneous pneumothorax.

Objective: To explore the effects of phacoemulsification (PEA) combined with trabeculectomy in the treatment of angle-closure glaucoma (ACG) complicated with cataract on postoperative visual acuity and changes in angle structure. Methods: A total of 94 patients with ACG and cataract treated in the Affiliated Hospital of Panzhihua College, Sichuan Province between January 2019 and January 2022 were enrolled and divided into control group (n=46, trabeculectomy) and combination group (n=48, PE and trabeculectomy). The changes of visual acuity, intraocular pressure and chamber angle structure parameters before and after surgery were compared between the two groups. All were followed up to assess visual quality at 1 month after surgery. And perioperative complications were recorded. Results: Before surgery, there were no significant differences in visual acuity, intraocular pressure, central anterior chamber depth (ACD), trabecular iris angle (TIA), angle opening distance AOD500 (AOD500), and trabecular ciliary process distance (TCPD) between the two groups (P>0.05). At 1 month after surgery, the visual acuity, ACD, TIA, and AOD500 of the two groups were significantly increased compared with those before surgery, and the intraocular pressure was significantly decreased, and the differences of the above indexes before and after operation in the combined group were higher than those in the control group (P<0.05). At 1 month after surgery, scores of visual quality of watching TV in combined group was (3.01±0.39), scores of visual quality of reading was (2.86±0.43), scores of visual quality of night vision was (2.72±0.35), scores of visual quality of fine operation was (2.82±0.39), which were higher than those in control group (2.23±0.47), (2.20±0.45), (2.38±0.40), (2.34±0.41) (all P<0.05), and total incidence of complications was lower than that in control group (14.58% vs 34.78%) (P<0.05). Conclusion: For patients with ACG and cataract, PE combined with trabeculectomy can significantly improve visual acuity and visual quality, reduce intraocular pressure and improve structural parameters of chamber angle, with fewer complications. It can be applied as an ideal treatment method for such patients.

1655 The Effect of GSTP1 Gene on Colorectal Cancer and Its Significance

Objective: To explore the glutathione S-transferase P-1 (GSTP1) genotype and its clinical significance in patients with colorectal cancer. Methods: The data of 80 patients with colorectal cancer diagnosed in Qinhuangdao First Hospital who agreed to undergo genetic testing were retrospectively selected. According to the GSTP1 gene situation, it is divided into gene mutation group (GSTP1 gene mutation) and gene wild type group (GSTP1 gene wild type). The blood samples and tissue samples of the patients were collected and sent to the Precision Medicine Department of our hospital for GSTP1 expression detection, and test results and the clinical and pathological characteristics of the patients were statistically analyzed. All patients were treated with chemotherapy, and the clinical efficacy, toxic and side effects, and progression-free survival were compared between the two groups. A sum of 18 tissue samples were selected to detect the expression of GSTP1 gene in colorectal cancer and adjacent tissues. Results: The GSTP1 gene detection of 80 patients showed that 35 cases were mutant (gene mutation group) and 45 cases were wild type (gene wild type group). There were no significant differences in the types, histological types, depth of invasion, nerve invasion, vascular tumor thrombus, and lymph node metastasis (P>0.05). The wild-type group (66.67%), with the difference being statistically significant (P<0.05); there was no significant difference in the occurrence of toxic and side effects between the two groups (P>0.05); the follow-up patients in the progression-free survival period gene mutation group [( 4.39±1.24) years] was significantly lower than that of the wild-type group [(6.49±2.15) years], with the difference being statistically significant (P<0.05); the relative expression of GSTP1 mRNA in colorectal cancer tissues was significantly higher than that in adjacent tissues, and the difference was statistically significant (P<0.05). Conclusion: The positive expression rate of GSTP1 gene in colorectal cancer tissues is high, and the chemotherapy effect and prognosis of patients with GSTP1 gene wild type are better than those of gene mutation patients. Clinically, prognostic evaluation and treatment plan selection should be combined with the patient's GSTP1 genotype.

1659 Effect of Adipose-Derived Stem Cells on Knee Osteoarthritis in Mice

Objective: To investigate the effects and possible mechanisms of suprapatellar-derived ASC autograft on knee inflammation and degenerative grade of cartilage in a mouse KOA model. Methods: ASC in the suprapatellar or infrapatellar region were isolated, ASC proliferation and fat-forming, osteogenic and cartilage-forming directed differentiation ability were measured respectively, the OA mouse model was replicated and cell transplantation was performed, and the GAG analysis and injury score of mouse knee tissue were examined. Results: In vitro experiments showed a significant increase in fat formation, osteogenesis and cartilage formation in the suprapatellar-derived ASC group compared to the infrapatellar-derived and infrapatellar-differentiated cell groups. In vivo experiments showed a significant increase in knee dimensions in all three groups after OA induction. hASC, SVF and PRP intervention groups all showed a significant decrease in increased knee volume compared to the control group, a significant increase in GAG content and a significant decrease in OA injury score in the hASC group. Conclusion: Human suprapatellar fat pad-derived ASC promotes cartilage regeneration by promoting effective endogenous cartilage formation in the knee joint in KOA mice.

1663 Association of Triglyceride to High-Density Lipoprotein Cholesterol Ratio with Metabolic Syndrome in High-risk Diabetic Population

Objective: To investigate the correlation between triglycerides to high-density lipoprotein cholesterol ratio (TG/HDL-C) and metabolic syndrome (Mets) in high-risk diabetic population. Methods: According to the definition of high-risk groups of type 2 diabetes in China's Guidelines for the Prevention and Treatment of Type 2 Diabetes (2017 Edition), patients were recruited from the outpatient of the Department of Endocrinology of Hebei General Hospital from April 2018 to April 2019. A total of 531 patients participated in the study. A multivariate Logistic regression model was used to assess their correlation between TG/HDL-C and Mets. Results: TG/HDL-C of the patients with Mets was significantly higher than those without Mets [1.73(1.03,2.55) vs 0.76(0.54,1.12), P<0.001]. With the increase of TG/HDL-C, the prevalence of Mets gradually increased (P<0.001). TG/HDL-C was an independent influencing factor of Mets. After correcting the relevant influencing factors, TG/HDL-C was positively correlated with Mets(Odds ratios=5.23, 95% Confidential interval: 3.30-8.29, P<0.001). Conclusion: TG/HDL-C is closely related to Mets in high-risk groups with type 2 diabetes mellitus, and with the increase of TG/HDL-C, the risk of Mets is significantly increased.

1667 Comparison of Primary Suture and T-Tube Drainage During Laparoscopic Common Bile Duct Exploration in Patients without Severe Acute Cholangitis

Objective: To investigate the safety and effectiveness of primary suture (PC) after laparoscopic common bile duct exploration (LCBDE) in patients with non-severe acute cholangitis (AC).Methods: The clinical and follow-up data of 50 patients with non-severe acute cholangitis who underwent LCBDE from March 2018 to March 2020 in the Department of Hepatobiliary Surgery, Hebei Provincial People's Hospital affiliated to North China University of Science and Technology were selected, and the clinical indexes of 25 cases with primary suture of common bile duct (PC groups) and 25 cases with T-tube drainage (T-tube group) were compared. Results: Of the 50 patients, 49 cases were successfully operated, and 1 case was converted to laparotomy. There was no significant difference in gender, age, body mass index (BMI), American Association of Anesthesiologists (ASA) grade, acute cholangitis (AC) grade, acute cholecystitis cases, common bile duct diameter, preoperative complications, clinical symptoms and signs between the two groups (P>0.05). There was no significant difference in serum total bilirubin (TB), direct bilirubin (DBIL), alanine aminotransferase (ALT), aspartate aminotransferase (AST) and glutamyl transpeptidase (GGT) between the two groups before operation (P>0.05). Compared with T-tube group, PC group had shorter operation time, less intraoperative blood loss, shorter abdominal drainage time and shorter postoperative hospital stay, and the differences were statistically significant (t/z/P =6.144/<0.001,2.143/0.032,2.504/0.016,2.561/0.014). There were no residual stones, injury of adjacent organs during operation and death in hospital in both groups. The total incidence of postoperative complications such as biliary fistula, abdominal hemorrhage, electrolyte disturbance, venous thrombosis of lower limbs, pleural effusion, nausea and vomiting in PC group was not significantly different from that in T-tube group (χ2=2.228, P=0.136). During the follow-up, no biliary stricture occurred in the two groups, and 2 patients in T-tube group had stone recurrence. Conclusion: PC after LCBDE in patients with non-severe AC choledocholithiasis has shown better clinical outcomes than the T-tube group in terms of operative time, intraoperative bleeding, abdominal drainage time and postoperative hospital stay, making it a safe and feasible treatment option.

1672 To Investigate the Effect of SGLT2i on Cardiovascular Outcomes in Patients with Heart Failure with Mid-range Ejection Fraction Based on Propensity Score Matching

Objective: To observe the real-world effects of sodium-glucose cotransporter 2 inhibitors (SGLT2i) on cardiovascular outcomes in patients with chronic heart failure with mid-range ejection fraction (HFmrEF) with or without type 2 diabetes mellitus. Methods: This study is a single-center retrospective cohort study.A total of 728 HFmrEF patients with or without type 2 diabetes mellitus in Chengde Central Hospital from January 2019 to December 2021 were selected as research subjects and divided into exposure group and control group according to different treatment regimen.Patients in the control group were treated with guideline-oriented conventional heart failure medication,while patients in the exposure group were treated with dapagliflozin (at a dose of 10 mg once daily) in addition to conventional heart failure medication treatment.Both groups were treated with maintenance therapy for at least 6 months.SGLT2i was used as the grouping variable.Age,sex,smoking history,drinking history,hypertension history,diabetes history,hyperlipidemia history,coronary heart disease history,stroke history,percutaneous coronary intervention,History of PCI,history of coronary artery bypass grafting (CABG),history of permanent pacemaker implantation,and left ventricular ejection fraction (LVEF),estimated glomerular filtration rate (eGFR) and B-type natriuretic peptide (BNP) level were used as matching factors for propensity score.Propensity score matching was performed between the two groups.The Kaplan-Meier method was used to generate survival curves, and multivariate Cox proportional hazards regression was used to analyze end-point events.The primary endpoint event was a composite of hospitalization for heart failure,emergency admission for heart failure,and death from cardiovascular causes. Results: After propensity score matching,108 matching pairs were generated.The baseline characteristics of the two groups of patients were balanced,and there was no statistical difference.At a median follow-up of 19 months,the risk of the primary composite end point was significantly lower in the exposure group than in the control group(HR=0.51,95%CI:0.30~0.86,P=0.01).This effect was primarily associated with a lower risk of hospitalization for heart failure and emergency admission for heart failure in the exposed group.Compared with the control group,the risk of hospitalization for heart failure was significantly reduced in the exposure group (HR=0.53,95%CI:0.30~0.94,P=0.027).The risk of emergency admission for heart failure was significantly lower in the exposed group (HR=0.43,95%CI: 0.20~0.97, P=0.035).The risk of death from cardiovascular causes and death from any cause did not differ between the two groups. Conclusion: SGLT2i dapagliflozin significantly reduced the risk of hospitalization for heart failure and emergency admission for heart failure in patients with chronic HFmrEF with or without type 2 diabetes mellitus.

1678 Correlation between Plasma Mitochondrial DNA Level and Disease Severity and Prognosis in Children with Sepsis

Objective: To analyze the correlation between plasma mitochondrial DNA (mt-DNA) levels and the severity and prognosis of children with sepsis,and to find valuable biomarkers. Methods: A total of 103 children with sepsis in the pediatric intensive care unit (ICU) of our hospital from January 2021 to March 2022 were selected as the research subjects.According to the severity,they were divided into the sepsis group with 57 cases and the severe sepsis group with 46 cases.50 cases of ICU infected children matched by gender and age during the same period were set as the infection group,and healthy children matched by gender and age during the same period were used as the control group.Plasma mt-DNA,calcitoninogen (PCT),C-reactive protein (CRP) and lactate (Lac) levels were compared between the groups. The critical illness score (PCIS),the 2nd generation Paediatric Organ Dysfunction Score (PELOD-2) and the 3rd generation Paediatric Risk of Mortality Score (PRISM III) of 57 cases in the sepsis group were compared,and the correlation between plasma mt-DNA and PCT,CRP,Lac levels and PCIS scores was analysed. According to the outcome 28 days after admission,the children were divided into a survival group and a death group.The plasma mt-DNA,PCT,CRP and Lac levels were compared between the two groups,and ROC curves were established to analyse the predictive value of plasma mt-DNA,PCT,CRP and Lac on the prognosis of children with sepsis. Results: There were significant differences in the levels of mt-DNA,PCT,CRP and Lac among the groups (P<0.05).Pairwise comparison of mt-DNA levels between groups:the severe sepsis group was higher than the sepsis group,the infection group and the control group; the sepsis group was higher than the infection group and the control group; the infection group was higher than the control group,the differences were all Statistical significance (P<0.05).Pairwise comparison of PCT,CRP and Lac levels between groups:the severe sepsis group was higher than the sepsis group,the infection group and the control group; the sepsis group and the infection group were higher than the control group,and the Lac level in the sepsis group was higher than In the infection group,the difference was statistically significant (P<0.05).There was no significant difference in the levels of PCT and CRP between the sepsis group and the infection group (P>0.05).The PCIS score in the severe sepsis group was lower than that in the sepsis group,the PELOD-2 and PRISMIII scores were higher than those in the sepsis group,and the differences were statistically significant (P<0.05).Plasma mt-DNA was positively correlated with PCT,CRP,Lac,PELOD-2 and PRISMⅢ scores (r=0.877,0.904,0.875,0.923,0.870,all P=0.000).Plasma mt-DNA was negatively correlated with PCIS score (r=-0.841,P=0.000).The plasma levels of mt-DNA,PCT,CRP and Lac in the death group were higher than those in the survival group,and the differences were statistically significant (P<0.05).The ROC curve was established.When the cut-off of plasma mt-DNA,PCT,CRP,and Lac levels were 1124.28 pg/mL,18.49ng/mL,27.38mg/L,and 3.04 mmoL/L,Youden was the highest,with AUCs of 0.817 and 0.716,,0.607,0.759 respectively.The sensitivity of plasma mt-DNA in predicting the prognosis of sepsis was 80.95%,which was significantly higher than 71.43%,66.67% and 76.19% of PCT,CRP and Lac; the specificity of plasma mt-DNA in predicting the prognosis of sepsis was 93.90%.It was significantly higher than 81.71%,75.61% and 74.39% of PCT,CRP and Lac,and the differences were statistically significant (P<0.05). Conclusion: There are significant differences in mt-DNA in children with sepsis of different severity and prognosis.Plasma mt-DNA can be used as a biomarker for the early diagnosis of sepsis as it can help to assess the severity and prognosis of septic children.

1684 Correlation of MRI Parameters with Epidermal Growth Factor Receptor 2 and Proliferating Cell Nuclear Antigen in Breast Cancer

Objective: To explore the correlation between T2WI signal characteristics and apparent diffusion coefficient (ADC) of MRI and cerbB-2 and Ki-67 expression in breast cancer patients. Methods: One hundred and seventeen breast cancer patients admitted to our hospital from June 2017 to April 2021 were selected.All patients underwent routine MRI imaging and immunohistochemical staining of specimens,and the correlation between T2WI signal characteristics and ADC values obtained from MRI imaging and immunohistochemical detection of cerbB-2 and Ki-67 positivity was recorded and analyzed. Results: The ADC values were (0.81±0.11) and (0.90±0.15) for positive and negative expression of cerbB-2,respectively,and the T2WI signal and ADC values were significantly higher for positive expression of cerbB-2 (P<0.05).T2WI high signal and ADC values were significantly lower (P<0.05).T2WI high signal and ADC values were factors influencing cerbB-2 positivity,and both were positively correlated with it (r=0.372,r=0.310,P<0.001,P=0.001); T2WI high signal and ADC values were factors influencing Ki-67 positivity,T2WI signal was positively correlated with Ki-67 positively and ADC values were negatively correlated with them (r=0.274,r=-0.476,P=0.003,P<0.001). Conclusion: In this paper,we leave aside the commonly used ultrasound and mammography examinations for breast cancer and use MRI as the main breakthrough point to investigate the relationship between T2WI signals,ADC values and body biologic factors.Most studies have focused on the ADC values and the results of MRI examinations,ignoring the role of T2WI signals in assessing the relationship between breast cancer and body biologic factors.In this paper,the combination of ADC values,T2WI signals and biologic factors is used to support the search for new targets for breast cancer treatment.The T2WI signal and ADC value are associated with the expression of cerbB-2 and Ki-67,which can be used as a reference for subsequent breast cancer treatment.

1689 Association of Serum lncRNA DLX6-AS1 and Actinin-4 Expression with Recurrence and Prognosis of Cervical Cancer after Surgery

Objective: To study the relationship between the expressions of serum long non-coding RNA (lncRNA) DLX6-AS1 and actinin-4 in patients with cervical cancer and their relationship with recurrence and prognosis after radical resection of cervical cancer. Methods: A total of 120 cervical cancer patients who underwent radical resection of cervical cancer from February 2016 to February 2018 were selected as the research subjects (cervical cancer group),and 50 healthy individuals who underwent physical examination during the same period were selected as the control group.The cervical cancer group was divided into non-recurrence group (101 cases) and recurrence group (19 cases) according to the tumor recurrence and progression 3 years after operation.The expression of serum lncRNA DLX6-AS1 was detected by fluorescence real-time quantitative polymerase chain reaction,and the expression of serum actinin-4 was detected by enzyme-linked immunosorbent assay.Statistical analysis was performed to compare the relationship between the expression levels of serum lncRNA DLX6-AS1,actinin-4 and the clinicopathological characteristics of cervical cancer patients.At 3 years follow-up,Kaplan-Meier survival analysis was used to analyse the difference between serum lncRNA DLX6-AS1 and actinin-4 expression and postoperative prognosis of cervical cancer patients.The factors affecting postoperative recurrence of cervical cancer patients were analyzed. Results: Compared with the control group,the expression of serum lncRNA DLX6-AS1 and actinin-4 in the cervical cancer group was significantly higher (P<0.05).Pearson correlation analysis showed that the expression of serum lncRNA DLX6-AS1 and Actinin-4 in the cervical cancer group showed a significant positive correlation (r = 0.560,P = 0.000).There were significant differences in serum lncRNA DLX6-AS1 and Actinin-4 expression between patients with different FIGO stages,pathological grades,depth of interstitial invasion and lymph node metastasis(P<0.05).The 3-year progression-free survival rate and progression-free survival time were lower in the lncRNA DLX6-AS1 high expression group and Actinin-4 high expression group than in the low expression group ( All P<0.05).The pathological grade of low differentiation,FIGO stage IIA,high expression of lncRNA DLX6-AS1 and high expression of Actinin-4 were independent risk factors for recurrence of cervical cancer after radical cervical surgery (all P<0.05). Conclusion: Increased expression of serum lncRNA DLX6-AS1 and Actinin-4 in patients with cervical cancer is associated with postoperative recurrence of cervical cancer and may be a new serum marker for prognosis.

Methods: The 82 patients with complicated intrahepatic bile duct stones admitted to our hospital from January 2019 to December 2021 were retrospectively analyzed and divided into a study group (n=43,treated by laparoscopic left hepatectomy combined with choledochoscopic stone extraction) and a control group (n=39,treated by laparoscopic left hepatectomy) according to the different surgical methods.The clinical outcomes,changes in stress indicators cortisol (COR),epinephrine (EP) and C-reactive protein (CRP) before and 3d after surgery,and the levels of liver function ghrelin (ALT),serum total bilirubin (TBIL) and glutamyl transferase (GGT) before and 7d after surgery were compared between the two groups,as well as the incidence of postoperative complications such as bile leakage,infection,stone retention and pleural effusion. Results: The clinical effective rate of 93.02% in study group was higher than 76.92% in control group (P<0.05),the difference was statistically significant (P<0.05),and the efficacy grading was better than that in control group (P<0.05).The levels of COR,EP and CRP in the two groups after surgery were higher than those before surgery (P<0.05),and the differences of levels before and after surgery showed no statistical significance between the two groups (P>0.05).After surgery,the ALT,TBIL and GGT in the two groups were reduced compared with those before surgery,and the above levels were lower in study group than those in control group,and the differences of liver function indicators before and after surgery in study group were higher compared to control group (P<0.05).The total incidence rate of bile leakage,infection,residual stone and pleural effusion was 4.66% in study group,which was lower than 20.51% in control group (P<0.05). Conclusion: Laparoscopic left hemihepatectomy combined with choledochoscopy has a good clinical efficacy and has small impact on stress response in the treatment of complicated intrahepatic bile duct stones,and it can improve the liver function and reduce the complications,thus it is worthy of clinical application and promotion.

1701 Preventive Effect of Mecobalamin on Peripheral Neuropathy Induced by XELOX Regimen in Patients with Colorectal Cancer

Objective: To study the preventive effect of mecobalamin on peripheral neuropathy (CIPN) induced by oxaliplatin + capecitabine (XELOX) regimen in patients with colorectal cancer. Methods: A total of 76 patients with colorectal cancer who were admitted to the Fourth Affiliated Hospital of Guangzhou Medical University from January 2020 to January 2022 were selected and divided into observation group and control group according to the random number table method,with 38 cases in each group.The control group was treated with XELOX regimen,and the observation group was treated with Mecobalamin Combined with XELOX regimen.After six chemotherapy cycles,the incidence of CIPN,nerve conduction velocity,level of inflammatory factors and incidence of adverse reactions were evaluated and compared among the groups. Results: The incidence of CIPN of grade II,III and IV in the observation group was lower than that in the control group (P<0.05).After treatment,the SNCV,MNCV of the median nerve and sural nerve in the observation group were higher than those in the control group (P<0.05).The levels of Nod-like receptor protein 3 (NLRP3),hypoxia-inducible factor-1α (HIF-1α) and B cell activating factor receptor (BAFF-R) in the observation group were lower than those in the control group (P<0.05).The scores of European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30) in the observation group were higher than those in the control group (P<0.05).During the treatment,the incidence of adverse reactions was 15.79% (6/38) in the observation group and 10.53% (4/38) in the control group,with no significant difference (P>0.05). Conclusion: Mecobalamin has a significant preventive effect on CIPN of XELOX regimen in patients with colorectal cancer.It can reduce the incidence of CIPN,and it can improve peripheral nerve conduction velocity,and it can inhibit the level of related inflammatory factors,and it can improve the quality of life of patients,and it has high clinical value.

1706 LASSO Regression Analysis of Treatment Failure in Refractory Periapical Periodontitis Based on Radiographic Characteristics

Objective: To investigate the least absolute shrinkage and selection operator (LASSO) regression for treatment failure in refractory periapical periodontitis based on radiographic characteristics. Methods: Prospectively,134 patients with refractory periapical periodontitis who had failed anterior root canal treatment and were admitted to our hospital from January 2017 to January 2021 were selected as study subjects,all of whom underwent micro-apical surgery to observe the effect of refractory periapical periodontitis treatment,and were divided into a success group and a failure group according to the result.The general information,excised apical segment specimens,apical X-ray films and CBCT data of the two groups of patients were collected.A LASSO-Logistic regression model was used to screen the influencing factors of treatment failure in refractory apical periodontitis,and a decision curve was plotted to evaluate the prediction of treatment failure in refractory apical periodontitis based on radiological indicators. Results: Among the 134 patients,2 were lost to follow-up,and 132 patients were finally included in the study.40 patients (30.30%) with refractory apical periodontitis who failed to be treated were classified as failure group,and the remaining 92 patients (69.70%) were classified as successful group after successful treatment.There were significant differences in Enterococcus faecalis infection,lateral wall perforation,tooth position,non-filled root canal volume,root canal filling density,and root apical filling volume between the two groups (P<0.05); the LASSO regression algorithm screened out four significant correlation indicators with non-zero coefficients,including enterococcus faecalis infection,tooth position,apical canal filling density,and apical filling volume.Further multivariate Logistic regression analysis showed that:enterococcus faecalis infection (OR=52.328,95%CI6.765~404.743),underfilling of the root apex (OR=6.242,95%CI1.738~22.423),and the affected teeth were mandibular teeth (OR=20.709,95%CI4.063~105.552) and uneven filling density of the apical canal (OR=11.374,95%CI1.933~66.913) were the risk factors for treatment failure of refractory apical periodontitis (all P<0.05) ; decision curve analysis showed that additional clinical benefit was provided when the radiographic features predicted treatment failure in refractory periapical periodontitis (0.1~0.53). Conclusion: Enterococcus faecalis infection,tooth location,apical filling density,and apical filling volume are risk factors for treatment failure in refractory periapical periodontitis.Inflammation treatment failure has high application value.

1712 A Meta-Analysis of the Effect of 10Hz-rTMS on Upper Limb Motor Function Rehabilitation in Stroke Patients

Objective: To systematically evaluate the effect of 10Hz repetitive transcranial magnetic stimulation(10Hz-rTMS) on upper limb motor function rehabilitation in stroke patients. Methods: The databases of CNKI,Wanfang,VIP,SinoMed,Cochrane Library,Embase,PubMed and Web of Science were searched by computer.The randomly controlled trials of 10Hz-rTMS on the rehabilitation of upper limb motor function in stroke patients were collected.A meta-analysis was performed using Revman5.4.1 software. Results: A total of 18 RCTs with a total of 1045 patients were included.The results showed that compared with the control group,the patients in the observation group has significantly lower FMA level[MD=8.56,95%CI(6.59,10.54),P<0.00001],MEP latency level[MD=-0.35,95%CI(-0.47,-0.23),P<0.00001],MEP level[MD=-0.35,95%CI(-0.47,-0.23),P<0.00001],CMCT level[MD=-1.06,95%CI (-1.63,-0.49),P=0.00003] and MBI level[MD=9.29,95%CI (7.42,11.16),P<0.00001]. Conclusion: 10Hz-rTMS can significantly improve the upper limb motor function of stroke patients.

1719 Changes of Ultrasound LCA/AoA and RCA/AoA Values in Self-Limited Recovery of Kawasaki Disease and Analysis of Their Predictive Value for Prognosis

Objective: To detect the left common coronary artery (LCA) and right common coronary artery (RCA) internal diameters and aortic annulus (AoA) internal diameters in children with Kawasaki disease (KD) using ultrasound,calculate LCA/AoA and RCA/AoA values,and analyse the changes in their self-limiting recovery and their predictive value for prognosis in children with KD. Methods: A total of 126 KD children who were treated in our hospital from January 2020 to December 2021 were selected as the observation group,and 63 healthy children who underwent physical examination during the same period were selected as the control group.The LCA/AoA and RCA/AoA values of the observation group in the acute phase,subacute phase and recovery phase were compared with those in the control group.The incidence of coronary artery lesion (CAL) in the acute phase and 3 months after the onset of the observation group was counted.The influencing factors of prognosis of children were analyzed,and the value of LCA/AoA and RCA/AoA values in predicting prognosis was evaluated. Results: The LCA/AoA and RCA/AoA values of the observation group in the acute stage,subacute stage and recovery stage were higher than those in the control group (P<0.05).The LCA/AoA and RCA/AoA values in the subacute phase of the observation group were higher than those in the acute phase,but were lower in the recovery phase than those in the acute and subacute phases (P<0.05).The incidence of CAL in the observation group was lower than that in the acute stage after 3 months of onset (P<0.05).After correction of platelet count and C-reactive protein level,LCA/AoA and RCA/AoA values in the recovery period were the prognostic factors for children with KD (P<0.05).The AUC value of LCA/AoA for predicting left coronary artery disease during the recovery period was 0.902,and the AUC value for RCA/AoA for predicting right coronary artery disease was 0.850. Conclusion: Ultrasound LCA/AoA and RCA/AoA values change significantly during the self-limited recovery of KD,especially in the recovery period,LCA/AoA and RCA/AoA values can effectively predict the prognosis of children and they provide a reference for the adjustment of clinical treatment plans.

1726 Diagnostic Value of Neutrophil/Lymphocyte Ratio and Mean Platelet Volume on Bacterial Infections During Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Objective: To explore the diagnostic value of neutrophil-to-lymphocyte ratio (NLR) and mean platelet volume (MPV) on bacterial infection during acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods: Clinical data of 101 elderly patients with acute exacerbation of COPD admitted between February 2019 and February 2022 were retrospectively analyzed.According to sputum bacteriological culture results,the patients were divided into infected group (n=62) and non-infected group (n=39).The white blood cell (WBC) count,platelet (PLT) count,NLR and MPV were compared between the two groups at admission. Receiver operating characteristic curve (ROC curve) was plotted to explore the diagnostic value of the aforementioned indicators on bacterial infection during acute exacerbation of COPD.After 1 week of treatment,patients were observed for changes in WBC,PLT,NLR and MPV. Results: WBC,NLR and MPV of (10.48±3.53) ×109 cell/L,(9.89±3.14) and (12.48±3.74) fl in infected group were significantly higher than (7.90±2.11) ×109 cell/L,(6.05±1.70) and (7.82±2.50) fl in non-infected group (t=4.131,6.998,6.866,P<0.001).However,there was no statistical significance in PLT level between the two groups [(256.50±96.06) ×109 cell/L vs (272.62±49.75) ×109 cell/L] (t=-0.968,P>0.05).The areas under ROC curves of WBC,NLR and MPV were 0.743,0.855 and 0.849 in the diagnosis of bacterial infection during acute exacerbation of COPD (P<0.001),and the critical values were 10.5×109 cell/L,8.5 and 9.5 fl respectively.The specificity of NLR combined with MPV in series was the highest,with the misdiagnosis rate of 2.6% (1/39).The sensitivity of NLR combined with MPV in parallel was the highest,with the missed diagnosis rate of 9.7% (6/62).After 1 week of treatment,WBC,NLR and MPV of (7.15±2.04) ×109 cell/L,(5.18±1.16) and (7.54±1.38) fl in patients with acute exacerbation of COPD were reduced compared with (9.49±3.30) ×109 cell/L,(8.41±3.26) and (10.68±4.02) fl before treatment (t=8.808,14.688,11.688,P<0.001),However,there was no statistical difference before and after PLT treatment [(262.72±81.43) ×109 cell /L vs (273.74±48.65) ×109 cell/L] (t=-1.703,P>0.05). Conclusion: NLR and MPV can be used as sensitive indicators to predict bacterial infection during COPD acute exacerbation,and have certain application value in the early diagnosis and treatment of patients.

1731 The Role of Multiparametric Magnetic Resonance in the Assessment of the Pathological Nature and Molecular Subtypes of Non-Mass-Enhancing Lesions in the Breast

Objective: To investigate the role of multiparametric MRI in the assessment of the pathological nature and molecular subtypes of non-mass-enhancing (NME) lesions of the breast. Methods: Twenty eight patients with NME admitted from January 2018 to December 2021 were selected and all underwent multiparametric MRI,including flat-scan 3D scrambled-phase fast gradient echo T1WI transaxial position,fat-suppressed T2WI transaxial position,dynamic enhanced magnetic resonance imaging (DCE-MRI),and diffusion-weighted imaging (DWI) scans.The patients were divided into malignant group (n=168) and benign group according to pathological findings.The baseline data and multiparametric MRI results of the two groups were compared,the value of multiparametric MRI in identifying malignant NME was analyzed using the receiver operating characteristic (ROC) curve,and the results of multiparametric MRI in patients with different molecular subtypes were compared. Results: In the malignant group,there were more patients with internal reinforcement clusters,segmental distribution and perineural oedema than in the benign group,and the ADC values were lower than in the benign group (P<0.05); the ROC curve showed that the combined diagnostic AUC of internal reinforcement clusters,segmental distribution,ADC values and perineural oedema was greater than that of any single parameter; the ADC values of HER-2 type were higher than those of Luminal A,Luminal B and TNBC types (P<0.05); the ADC values of HER-2 and TNBC types were higher than those of Luminal A and TNBC types (P<0.05).(P<0.05); more patients with HER-2 and TNBC types had peri-tumoural oedema than Luminal A and Luminal B types (P<0.05). Conclusion: Internal enhancement clusters,segmental distribution,ADC values and peri-tumoural oedema are of high diagnostic value for the pathological nature of NME lesions and can be used clinically to identify their benign and malignant nature and to assess their molecular subtypes,providing a reference basis for clinical treatment planning.

1738 AAA and AcurosXB Algorithms for Dosimetric Differences in SIB-VMAT Schemes for Locally Advanced Esophageal Squamous Cell Carcinoma

Objective: To investigate the difference of dosimetry between anisotropic analytical algorithm (AAA) and acuros external Beam algorithm (AXB) to the SIB-VMAT for locally advanced esophageal squamous cell carcinoma. Methods: Twenty four patients with locally advanced esophageal squamous cell carcinoma who received SIB-VMAT radiotherapy were randomly selected.Two algorithms modes of AAA and AXB were used to design SIB-VMAT plans with the same beam,MLC and optimized constraint function,to compare the differences of dosimetry parameters between the two groups. Results: In terms of Dmax,Dmean,D98%,and D50% of the target area,the AXB group were significantly higher than those in the AAA group,which was statistically significant (P<0.05).The conformity index (CI) and homogeneity index (HI) of the AXB group were better than those of the AAA group for PGTV,with statistical significance (P<0.05),but there was no significant difference in the PTV (P>0.05).About the dose of lung,AXB group could effectively reduce the dose of Dmin,Dmean,D50% and V5,but V20 and Dmax were slightly higher than AAA,which was statistically significant (P<0.05).About the dose of cardiac,except V30 was lower than AAA (1.29%),and Dmax was slightly higher than AAA (0.82%),which was statistically significant (P<0.05).About the dose of spinal cord protection area,the Dmax of AXB group was slightly higher than AAA (0.59%),which was statistically significant (P<0.05).The monitor unit(MU) of the AXB group was slightly higher than AAA (1.53%),which was statistically significant (P<0.05). Conclusion: The SIB-VMAT plan optimized by the AAA and AXB algorithms can meet the clinical requirements.Although the AXB algorithm and the AAA algorithm have certain differences in the dose results of the SIB-VMAT planning model,they both meet the clinical dosimetry requirements and can be used for the clinical treatment of locally advanced esophageal squamous cell carcinoma by the SIB-VMAT.

1743 Clinical Application of Fogarty Catheter Thrombectomy Combined with Catheter-Based Drug Thrombolysis in Arterial Embolization after Free Flap Surgery in the Foot and Ankle

Objective: To investigate the efficacy of Fogarty catheter thrombectomy combined with catheter-based drug thrombolysis in the treatment of arterial embolism after free skin flaps in the foot and ankle. Methods: A retrospective analysis was conducted of the clinical data of 16 patients admitted from April 2016 to February 2021 with soft tissue defects in the foot and ankle who underwent free flaps and who had arterial embolism in the flaps after surgery.All patients were treated with Fogarty catheter thrombectomy combined with catheter drug thrombolysis,and the operation time,intraoperative blood loss,arterial patency rate and related complications were recorded.During the follow-up period,the six indexes of the flap combined with the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot function score were used to evaluate the curative effect. Results: After surgery,12 flaps were completely viable,2 flaps were partially necrotic,and 2 cases had a recurrence of arterial crisis and were converted to an incisional exploration approach.Excluding 2 patients who were converted to incisional exploration,the 14 patients treated with this procedure had no major bleeding or deaths.The length of the thrombus removed was 0.2~1.8 cm,with an average length of (0.83±0.32) cm.The intraoperative arterial patency rate was 100% and the operative time ranged from 55 to 90 min,with a mean time of (65.12±12.72) min.One case (7.14%) had postoperative gastrointestinal bleeding,which was treated symptomatically.All patients were followed up for 8~21 months,with a median follow-up time of 14.5 (10.4,16.5) months.2 cases (14.3%) were affected by shoe wear,which improved after flap thinning,and 1 case (7.14%) developed a local ulcer,which healed after dressing change.At the last follow-up,the flap had a good texture and sensation was restored to S2 to S3 level,with protective sensation restored in all cases and only one point of discrimination in both cases.Foot and ankle function improved with an AOFAS ankle-hindfoot function score of 59 to 95,mean (83.72±6.42),with an excellent AOFAS ankle-hindfoot function rate of 71.42%. Conclusion: Fogarty catheter thrombectomy combined with drug thrombolysis in the treatment of arterial embolism after free skin flap in the foot and ankle is minimally invasive,fast,safe,and has definite clinical efficacy.

1748 Study on the Effects of Different Doses of Levothyroxine Dodium Tablet Treatment on Thyroid Hormones,Pregnancy Outcome and Offspring Psychological Developmental Functions in Pregnant Women with Hypothyroidism During Pregnancy

Objective: To study the effects of different doses of levothyroxine sodium tablets on thyroid hormones,pregnancy outcomes and offspring psychological development of women with hypothyroidism during pregnancy. Methods: A total of 100 pregnant women with hypothyroidism treated with levothyroxine sodium tablets in the hospital from August 2020 to August 2021 were selected.They were divided into group A (25ug/time,54 cases),group B (50ug/time,25 cases) and group C (75ug/time,21 cases) according to the dose.Relevant parameters in the three groups were comparatively analyzed. Results: Thyroid function indicators in the three groups showed no statistically significant difference before treatment (P>0.05).After treatment,the levels of thyroid stimulating hormone (TSH) and thyroid peroxidase antibody (TPO-Ab) were significantly decreased,and the levels of free triiodothyronine (FT3) and free thyroxine (FT4) were significantly increased in all groups.The increases or decreases were significantly greater in group C than in group A and group B (P<0.05).The glucose and lipid metabolism in the three groups showed no statistically significant difference before treatment (P>0.05).After treatment,fasting plasma glucose (FPG),fasting insulin (FINS),total cholesterol (TC),low-density lipoprotein cholesterol (HDL-C),and triglyceride (TG) were decreased,while high-density lipoprotein Cholesterol (HDL-C) was increased in the three groups.The changes in group C were greater than those in group A and group B (P<0.05).There were significant differences in the incidence rates of gestational diabetes mellitus and premature rupture of membranes among the 3 groups (P<0.05).The incidence of gestational diabetes mellitus in group C was significantly lower than that in group A (P<0.05).The other pregnancy outcomes of the 3 groups showed no statistically significant difference (P>0.05).The scores of offspring's gross motor,adaptability and social behavior in group C were significantly higher than those in group A and group B (P<0.05),but there was no statistically significant difference in language and fine motor (P>0.05). Conclusion: Compared with low-dose levothyroxine sodium tablets,high-dose levothyroxine sodium tablets can better maintain normal thyroid function in pregnant women with hypothyroidism,regulate their glucose and lipid metabolism,lower the risk of adverse pregnancy outcomes,and improve offspring's neuropsychological development.
