GnIH/RFRP-3 Inhibits PI3K/Akt/MTOR Pathway Through GPR147 and Induces Apoptosis of Estrogen Receptor Positive Human Breast Cancer MCF-7 Cells

Objective:
To investigate the effects of gonadotropin inhibitory hormone (GnIH) on proliferation, apoptosis and PI3K/Akt/mTOR signaling pathway of human breast cancer MCF-7 cells based on GPR147 receptor.
Methods:
① Human breast cancer cell line MCF-7 was cultured in vitro and divided into blank control group, control group and GnIH experimental group (0.1ng/mL, 1ng/mL, 10ng/mL, 100ng/mL, 1000ng/mL, 10000ng/mL). CCK-8 method was used to detect the cell proliferation; flow cytometry was used to detect the apoptosis rate; real time fluorescent quantitative PCR (QRT-PCR) and Western blot wereused to detect the effect of GnIH on apoptosis related proteins (Bcl-2, Bax, cytochrome C) at mRNA and protein levels . The expression of PI3K, Akt/p-Akt and mTOR proteins in Caspase-3, and p53 and PI3K/Akt/mTOR signaling pathways were detected. Western blot was used to detect the expression of GPR147 in MCF-7 cell line.② The cells were divided into PBS control group, DMSO control group, GnIH group and GnIH+RF9 inhibitor group for 24 hours. PBS 15 μL, DMSO 3μL, GnIH 10000 ng/mL and GnIH 10000 ng/mL+RF9 10μmol were added to the cells for 24 hours. The expression of Akt/p-Akt protein was detected by Western blot.
Results:
CCK-8 results showed that the GnIH experimental group 0.1ng/mL, 1ng/mL, 10ng/mL, 100ng/mL, 1000ng/mL cells compared with the control group, the difference was not statistically significant (P>0.05); Compared with the control group, GnIH10000ng/mL in the experimental group has a statistically significant difference (P<0.05). The apoptotic rates of control group, 1000ng/mL and 10000ng/mL were (7.76±1.57)% and (16.14±3.001)%, respectively, with significant difference (F=6.164, P=0.0351). The expressions of Bax, cytochrome c, caspase-3 and p53 were significantly up-regulated (P<0.05), while the expressions of Bcl-2 were significantly down regulated (P<0.05). In PI3K/Akt/mTOR signaling pathway, the protein expression of PI3K, Akt/pAkt and mTOR was significantly down regulated (P<0.01). GPR147 was found in MCF-7 cell line. When the drug concentration was 10000ng/mL, the receptor was activated compared with the control group (P<0.01). The expression of GPR147 in the experimental group was significantly higher than that in the control group (P<0.01). After the intervention, the expression of Akt/p-Akt protein was significantly higher in the group of GnIH +RF9 than in the group of GnIH (P<0.05).
Conclusion:
GnIH receptor GPR147 exists in breast cancer cell line MCF-7. GnIH may inhibit the proliferation and induce apoptosis of MCF-7 cells by binding to gpr147 receptor, and the occurrence of apoptosis may be related to the inhibition of PI3K/Akt/mTOR signaling pathway.

MicroRNA-27b-Mediated Silencing of Homeobox Protein HOXB8 Modulates Human Gastric Cancer Stem Cell-Like Characteristics

Objective:
To explore the expression levels of miR-27b and homeobox protein HOXB8 in gastric cancer tissues, and the mechanisms by which they regulate human gastric cancer stem cell-like characteristics.
Methods:
Collected 30 cases of gastric cancer tissues and corresponding normal tissues adjacent to the cancer in our hospital. The differences in the expression of miR-27b and HOXB8 in tissues was detected with real-time fluorescence quantitative PCR. Bioinformatics tool TargetScan7.2 and dual luciferase reporter gene technology were adopted to predict and verify the targeting relationship between miR-27b and HOXB8. The immunomagnetic bead sorting method was used to separate gastric cancer stem cells from the gastric cancer cell line SGC-7901. MiR-27b mimic and negative control (miR-27b NC) were transfected into gastric cancer stem cells by liposome-mediated method. The expression of miR-27b in gastric cancer stem cells after transfection was detected with real-time fluorescence quantitative PCR, Western blot was adopted to detect HOXB8 protein expression in gastric cancer stem cells after transfection, CCK-8 method was used to detect the activity of gastric cancer stem cells, EdU test was applied to detect the proliferation of gastric cancer stem cells, Cell spheroidization experiment and cell clone formation experiment were conducted to detect the self-renewal ability of gastric cancer stem cells.
Results:
The expression level of miR-27b in gastric cancer tissues was higher than that of normal tissues adjacent to cancer, while HOXB8 was lower than that of normal tissues adjacent to cancer (P<0.01); miR-27b and HOXB8 3'-UTR have base complementation in specific regions. The relative fluorescence of cells co-transfected with miR-27b mimic and WT-HOXB8 3’-UTR recombinant plasmid was lower than that of cells co-transfected with miR-27b NC and WT-HOXB8 3’-UTR recombinant plasmid (P<0.01). After sorting, high-purity CD44+ labeled gastric cancer stem cells were obtained, and miR-27b mimic was successfully transfected into gastric cancer stem cells. Compared with the blank control group, miR-27b expression in gastric cancer stem cells of miR-27b mimic group increased (P<0.01), HOXB8 protein expression decreased (P<0.01), cell viability decreased (P<0.01), EdU-labeled positive cells decreased (P<0.01), the number of cells into spheres is less, the size of spheres becomes smaller, and the number of cell clone formation decreased (P<0.01).
Conclusion:
The expression of miR-27b is decreased and the expression of HOXB8 is increased in gastric cancer tissues. MiR-27b regulates the proliferation and self-renewal ability of gastric cancer stem-like cells by targeting to silence the expression of HOXB8.

Correlation Between Levels of Serum HbA HbF and G-6-PD and Neonatal Hyperbilirubinemia

Objective:
To analyze the correlation between levels of serum hemoglobin A (HbA), hemoglobin F (HbF) and glucose-6-phosphate dehydrogenase (G-6-PD) and neonatal hyperbilirubinemia.
Methods:
A total of 106 neonates with neonatal hyperbilirubinemia admitted to the pediatric department of the hospital from February 2017 to February 2020 were selected as study group, and 106 normal neonates in the hospital at the same period were selected as control group. The neonates in study group were divided into mild group (n=55), moderate group (n=31) and severe group (n=20) according to the disease conditions of children patients with hyperbilirubinemia. The levels of serum HbA, HbF and G-6-PD were compared between study group and control group, and the correlation between levels of serum HbA, HbF and G-6-PD and neonatal hyperbilirubinemia was analyzed.
Results:
The levels of serum HbA and TBil of hyperbilirubinemia children patients in study group were significantly higher than those in control group (P<0.05), and the levels of serum HbF and G-6-PD were significantly lower than those in control group (P<0.05). The level of serum HbA of severe group was higher than that of mild group and moderate group (P<0.05), and the level of moderate group was higher than that of mild group (P<0.05), and the HbF level of severe group was lower than that of mild group and moderate group (P<0.05), and there was no significant difference in the G-6-PD level in each group (P>0.05). Serum HbA level of children patients with hyperbilirubinemia was positively correlated with TBil level (P<0.05), and HbF level was negatively correlated with TBil level (P<0.05), and the G-6-PD level was not obviously correlated with TBil level (P>0.05).
Conclusion:
The level of serum HbA is higher and the levels of HbF and G-6-PD are lower among neonates with hyperbilirubinemia, and the disease severity of children patients is related to the levels of HbA and HbF. Detection of serum HbA and HbF levels is helpful to evaluate the disease severity of neonates with hyperbilirubinemia.

Effect of Preemptive Analgesia with Different Concentrations of Flurbiprofen Axetil on Cognitive Function and IL-6 Concentration in Elderly Patients Undergoing Hip Arthroplasty

Objective:
To explore the effect of different concentrations of flurbiprofen axetil on the cognitive function and the concentration of interleukin-6 (IL-6) in elderly patients undergoing hip arthroplasty.
Methods:
A total of 108 elderly patients undergoing hip arthroplasty in our hospital from March 2018 to March 2021 were selected and simply randomly divided into two groups, with 54 cases in each. The control group received 1.0 mg/kg flurbiprofen axetil as an intravenous bolus, and the study group received 5.0 mg/kg flurbiprofen axetil as an intravenous bolus. The cognitive function [Mini Mental State Scale (MMSE)], pain at 2h, 12h, 24h, 48h after operation (visual simulation Scale (VAS)], IL-6 levels and adverse reactions before operation, immediately after operation, and 24 hours after operation were observed and compared.
Results:
The two groups were compared in general data such as gender, age, and BMI (P>0.05). The cognitive function MMSE scores of the two groups was compared at different time points. Repeated measures analysis of variance showed that the differences in the time points of the cognitive function MMSE scores between the two groups and the interaction effects between the time points and the groups were statistically significant (P<0.05). The results of the pairwise comparison within the groups afterwards showed that the cognitive function MMSE scores of the two groups of patients 1d after the operation showed a significant downward trend compared with those before the treatment (P<0.05), and the cognitive function MMSE of the two groups of patients 3d after the operation showed a significant upward trend compared with 1 day after operation (P<0.05). The results of the paired comparison between the groups afterwards showed that there was no significant difference in the cognitive function MMSE scores between the two groups on the 1st day before the operation (P>0.05); the cognitive function MMSE scores of the study group were higher than those of the study group 1d after operation and 3d after operation. In the control group, the difference was statistically significant (P<0.05). The VAS pain scores between the two groups were compared at different time points. Repeated measures analysis of variance showed that there were statistically significant differences in the VAS pain scores between the two groups at time points and between groups (P<0.05), and there was no difference in the interaction effects between time points and groups. The difference was tatistically significant (P>0.05). The results of the pairwise comparison within the group showed that the VAS pain scores of the two groups showed a significant upward trend at 12h after surgery compared with 2h after surgery, and the VAS pain scores of the two groups showed a significant downward trend at 24h and 48h after surgery compared to 12h after surgery ( P<0.05). The results of the paired comparison between the groups afterwards showed that the VAS pain scores of the study group were lower than those of the control group at 2h, 12h, 24h, and 48h after the operation, and the difference was statistically significant (P<0.05). The IL-6 levels of the two groups at different time points. Repeated measures analysis of variance showed that the differences in IL-6 levels between the two groups were statistically significant (P<0.05), and the interaction between time points and groups was not statistically significant (P>0.05). The results of a pairwise comparison within the group after the event showed that the IL-6 levels of the two groups immediately after the operation and 24 hours after the operation showed a significant upward trend compared with the pre-operation (P<0.05). The results of the paired comparison between the groups afterwards showed that there was no significant difference in the levels of IL-6 between the two groups before the operation (P>0.05); immediately after the operation and 24 hours after the operation, the IL-6 levels of the study group were lower than those of the control group. The difference was statistically significant (P<0.05). In terms of safety evaluation, 2 cases (3.70%) had nausea in the study group and 1 case (1.85%) had a rash in the control group. There was no significant difference in adverse reactions between the two groups (χ

Conclusion:
Preemptive analgesia for elderly patients undergoing hip replacement surgery with 5.0 mg/kg flurbiprofen axetil is beneficial to improving the cognitive function of the patients, which can reduce the surgical pain of the patients, reduce the level of inflammatory factors, and is safe and reliable.

Correlation of Serum Tumor Markers CA153 CA125 Carcinoembryonic Antigen with Molecular Typing and Clinicopathological Characteristics in Patients with Stage I to III Invasive Breast Cancer before Treatment

Objective:
To analyze retrospectively the correlation between serum tumor markers CA153, CA125 and CEA levels and molecular typing and clinicopathological features before treatment in patients with invasive breast cancer from I to III.
Methods:
457 patients with stage I to III invasive breast cancer who visited the First Affiliated Hospital of Inner Mongolia Medical University from January 2016 to December 2020 were selected. The levels of serum CA153, CA125 and CEA before treatment were detected, and the clinical and pathological data were collected. (including the age of onset, tumor size, lymph node metastasis, clinical stage, histological grade, hormone receptor expression, HER-2 expression, Ki-67 expression, etc.), univariate analysis was used to analyze the correlation between the serological level of tumor markers and molecular subtypes and clinicopathological features, and multivariate analysis was used to analyze the independent risk factors related to each tumor marker.
Results:
Among 457 patients with stage I to III invasive breast cancer, 20 (4.38%), 34 (7.44%) and 25 (4.57%) of CEA, CA153 and CA125 serological levels were positive before treatment, respectively. Univariate analysis showed that there were significant differences in CEA and CA153 levels among different molecular subtypes (P <0.05). Serological levels of CEA, CA153 and CA125 were significantly increased in patients with tumor size ≥2cm with lymph node metastasis (N1) (P <0.05). The results of multivariate analysis showed that age (> 50 years old), tumor size (≥2cm), lymph node metastasis (N1) and HER-2(+) were all independent risk factors affecting the change of CEA level in breast cancer patients before treatment. Patients aged 50 years and older had a 4-fold increased risk of elevated CEA serological levels before treatment (OR=4.316, 95%CI: 2.262-9.443) compared with patients younger than 50 years old. Age (> 50 years old), tumor size (≥2cm) and lymph node metastasis (N1) were all independent risk factors affecting serum CA153 level changes in breast cancer patients before treatment (P <0.05), and the risk was about three times that of breast cancer patients under 50 years old (OR=3.685, 95%CI: 1.133-6.912,OR=3.331, 95%CI: 1.034-6.746). Lymph node metastasis (N1) and progesterone PR receptor (-) were independent risk factors for elevated CA125 serological levels in breast cancer patients before treatment (P <0.05).
Conclusion:
Age (> 50 years), tumor size (> 2cm) and lymph node metastasis (N1) are independent factors affecting the serological level of CEA and CA153 before treatment in patients with invasive breast cancer from I to III. Among the molecular typing results, HER-2 (+) independently can affecte the serum level of CEA, and progesterone PR receptor (-) independently affected the serum level of CA125.

Effect of Dahuang Mudan Decoction on Lipopolysaccharide-induced Pyrosis of Intestinal Epithelial Cells and NLRP3/caspase-1 Pathway

Objective:
To investigate the effect of Dahuang Mudan Decoction on lipopolysaccharide (LPS)-induced pyrosis of intestinal epithelial cells and the influence on nucleotide binding oligomerization domain-like receptor family pyrin domain containing 3/caspase-1 (NLRP3/caspase-1) pathway.
Methods:
Intestinal epithelial cells were randomly divided into control group, LPS group and rhubarb Mudan Decoction low, medium and high dose groups (2.52, 5.04 and 7.56 g / kg). Blank serum was added to the control group, LPS and blank serum were added to the LPS group, and LPS and medicated serum were added to the rhubarb Mudan Decoction group. The activity of intestinal epithelial cells was detected by thiazole blue (MTT), the apoptosis of intestinal epithelial cells was detected by flow cytometry, the expression levels of NLRP3, caspase-1 and gsdmd-c were detected by Western blot (WB), the expression of NLRP3 in cells was detected by immunofluorescence, and the pro-inflammatory factor interleukin (IL) - 1 was detected by enzyme-linked immunosorbent assay (ELISA) β And IL-18.
Results:
Compared with those in the control group, the cell activity of LPS group was significantly decreased, the apoptotic index increased significantly, the protein expression of NLRP3, caspase-1 and GSDMD-C, and the contents of IL-1β and IL-18 were significantly increased (all P<0.05); compared with those in LPS group, the cell activity of Dahuang Mudan Decoction group was significantly increased, the apoptotic index decreased significantly, the protein expression of NLRP3, caspase-1 and GSDMD-C, and the contents of IL-1β and IL-18 were significantly decreased (all P<0.05).
Conclusion:
Dahuang Mudan Decoction can inhibit NLRP3/caspase-1 pathway and reduce LPS-induced pyrosis of intestinal epithelial cells.

Correlation Between Mir-208 Sst2 CLU and Left Ventricular Remodeling in Patients with Acute Myocardial Infarction

Objective:
To analyze the correlation between peripheral blood microrna-208 (mir-208), soluble growth stimulating gene 2 protein (sST2), clusterin (CLU) and left ventricular remodeling (LVR) in patients with acute myocardial infarction (AMI).
Methods:
62 patients with AMI diagnosed in our hospital from January 2019 to February 2020 were selected as AMI group. According to the LVR standard, AMI group was divided into left ventricular remodeling group (LVR group) and left ventricular non remodeling group (non-LVR group). At the same time, 35 outpatients who were tested negative by coronary angiography (stenosis < 50%) without any basic heart disease were selected as control group. The levels of high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), triglyceride (TG), total cholesterol (TC), sST2, CLU , mir-208, fasting blood glucose (FPG), white blood cell (WBC) and platelet (PLT) were measured, and echocardiography was performed. The correlation between the levels of mir-208, sST2 and CLU in peripheral blood of IMA patients and echocardiographic indexes was analyzed.
Results:
Compared with the control group, the levels of LDL-C, mir-208, sST2, CLU, LVESD and LVEF in non-LVR group and LVR group were significantly higher than those in non-LVR group (P<0.05); the levels of LVEDd, LVESV, LVEDV and LVMI in no LVR group and LVR group were significantly lower than those in non-LVR group (P<0.05). Pearson correlation analysis showed that LVESD and LVEF were positively correlated with mir-208, sST2 and CLU (P<0.05), while LVESV, LVEDd, LVEDV and LVMI were negatively correlated with mir-208, sST2 and CLU (P<0.05).
Conclusion:
The levels of sST2 and CLU in peripheral blood of IMA patients are closely related to LVR. Timely detection of these two factors is helpful to inhibit the occurrence and development of LVR after AMI.

An Analysis of Influencing Factors of Early Neurological Deterioration After Revascularization in Patients with Acute Large Artery Occlusive Cerebral Infarction

Objective:
To explore the factors affecting the early neurological deterioration(END) after vascular recanalization treatment in patients with acute aortic occlusion cerebral infarction, and to provide reference for the early identification of poor prognosis.
Methods:
A total of 120 patients with anterior circulation major artery occlusive cerebral infarction admitted to the Department of Neurology of our hospital from October 2017 to October 2020 were selected as the research subjects. According to whether the recanalization after treatment in patients with occurrence of END the patients were divided into the END group with 32 cases and the non-END group with 88 cases. The clinical data of two groups of patients were compared, and analysis of dangerous factors was conducted with Logistic regression. Variables of statistical significance were assigned values, and prediction model was made with R software (R3.3.2) and package RMS and verified with receiver operating characteristic curve (ROC), consistency index (C-index) and calibration diagram.
Results:
Logistic regression analysis showed that age ≥ 65 years, systolic blood pressure ≥ 140 mmHg, NIHSS score ≥20 points, large area of cerebral infarction and NLR ≥4.20 were independent risk factors for the occurrence of END after thrombolytic therapy in patients with cerebral infarction. The histogram model of risk prediction of END after thrombolytic therapy constructed according to the above risk factors has good differentiation and high consistency with the actual observed values.
Conclusion:
Age, systolic blood pressure, NIHSS score, large area of cerebral infarction and NLR before thrombolysis are independent risk factors for the occurrence of END in patients, which need to be vigilant about clinically.

Curative Effect of Holmium Laser Enucleation of the Prostate in the Treatment of Large-Volume Prostate and Its Influence on Urodynamics Serum T-PSA and F-PSA Levels

Objective:
To study the curative effect of holmium laser enucleation of the prostate (HOLEP) in the treatment of large-volume prostate, and its influence on urodynamics, serum total prostate specific antigen (T-PSA) and free prostate specific antigen (F-PSA) levels.
Methods:
A total of 107 male patients with benign prostate hyperplasia (BPH) treated in the hospital between January 2018 and December 2020 were enrolled in and randomly divided into control group (53 cases) and observation group (54 cases). The control group was treated with transurethral resection prostate (TURP), while observation group was treated with HOLEP. The surgical indexes, serum T-PSA and F-PSA levels, urodynamics, sexual function and the incidence of postoperative complications in the two groups were compared.
Results:
Compared with the control group, the observation group had significantly longer operation time, bladder irrigation time, catheter indwelling time and hospital stay, and significantly less blood loss (P<0.05). At 3 months after surgery, the observation group had significantly larger urine flow and bladder volume, and significantly less residual urine than the control group (P<0.05). At 1 month after surgery, the observation group had significantly lower serum T-PSA and F-PSA levels than the control group (P<0.05). At 3 months after surgery, the International Index of Erectile Function 5 (IIEF-5) scores and Chinese Index of Sexual Function for Premature Ejaculation (CIPE) scores of the observation group were significantly higher than those of the control group (P<0.05), and incidence of postoperative complications was significantly lower than the control group (P<0.05).
Conclusion:
HOLEP is safer in treating patients with large-volume prostate. It can improve urodynamics, reduce levels of T-PSA and F-PSA, and reduce postoperative complications.

Effects of Mouse Nerve Growth Factor Combined with Mecobalamin on Electromyography Serum Lipids and VitB12 Levels in Diabetic Peripheral Neuropathy

Objective:
To investigate the effect of rat nerve growth factor combined with methylcobalamin in the treatment of diabetic peripheral neuropathy on patients with electromyography, blood lipids and VitB12 levels.
Methods:
The clinical data of 87 patients with diabetic peripheral neuropathy were selected for retrospective analysis. The selection time was from February 2018 to June 2020. According to different treatment methods, they were divided into control groups (treated with mecobalamin, n=43) and Observation group (treated with mouse nerve growth factor combined with mecobalamin, n=44), compare the two groups of common peroneal nerve (SNCV, MNCV), median nerve (SNCV, MNCV), TC, TG, HLD-L, LDL-C, NSE, VitB12 and TSS scores.
Results:

The Value of Serum Levels of sICAM-1 and sVCAM-1 in the Early Diagnosis of Pregnancy-Induced Hypertension

Objective:
To research the value of serum sICAM-1 and sVCAM-1 levels in the early diagnosis of hypertension during pregnancy (HDCP).
Methods:
The blood pressure and serum levels of sICAM-1 and sVCAM-1 in 450 pregnant women were monitored dynamically. The patients were divided into normal group and HDCP group according to the rise of blood pressure. Analysising the relationship between the changes of serum levels of sICAM-1, sVCAM-1 and HDCP. The threshold value of serum sVCAM-1 level(±s)in normal group was used as the diagnostic threshold of HDCP. Verifying the accuracy of the method.
Results:
The elevation of blood pressure in HDCP group occurred at 20 weeks of gestation and later, 57 of 450 pregnant women had HDCP, the incidence was 12.67%;Serum sICAM-1 and sVCAM-1 in HDCP group increased significantly at 10 weeks of gestation compared with normal group (P<0.05).The level of systolic blood pressure after 20 weeks in HDCP group was positively correlated with the level of serum sVCAM-1 during pregnancy (r=0.754, P<0.05);The value of serum sVCAM-1 level(±s) at 10 weeks in normal group was (1136.8+253.8) ng/mL. The specificity based on this diagnosis is 83.2%, and the sensitivity is 90.6%.
Conclusion:
Monitoring serum sICAM-1 and sVCAM-1 can early diagnose HDCP. It has the characteristics of convenient sampling, accurate method and sensitivity.

Diagnostic Value of MRI and DCE-MRI in Ovarian Tumors and Tumor Like Lesions

Objective:
To explore the diagnostic value of magnetic resonance imaging (MRI) and dynamic contrast-enhanced MRI (DCE-MRI) in ovarian tumors and tumor-like lesions.
Methods:
81 patients with ovarian tumors (ovarian tumor group) and 132 patients with tumor-like lesions confirmed by surgical pathology (tumor-like lesion group) were enrolled in this study between December 2014-April 2021. The MRI and DCE-MRI data of the two groups of patients were collected and analyzed retrospectively.
Results:
Conventional MRI showed that the probabilities of irregular lesions, capsule wall>3.0mm, parenchyma>3.0mm, and massive pelvic effusion were greater in the ovarian tumor group than in the tumor-like lesion group (P<0.05). The largest diameter of the lesion in the ovarian tumor group was (12.30±3.68) cm, larger than (9.00±1.01) cm in the tumor-like lesion group (P<0.05). DCE-MRI showed that the volume transfer constant (Ktrans), extravascular extracellular volume fraction (Ve), and rate constant (Kep) of the region of interest (ROI) in the ovarian tumor group were greater than those in the tumor-like lesion group (P<0.05). The receiver operating characteristic (ROC) curve analysis showed that the sensitivity of MRI combined with DCE-MRI in differential diagnosis of ovarian tumors and tumor-like lesions was the highest, which was 93.83% (P<0.05). Youden index, area under the curve (AUC) and 95% CI were 0.757, 0.878, and 0.827 ~ 0.919.
Conclusion:
Both MRI and DCE-MRI have certain value in the diagnosis of ovarian tumors and tumor-like lesions, and the combination of the two has the highest value. Patients with ovarian tumors have more irregular lesions with thicker capsule walls, more parenchyma and larger diameters, more pelvic effusion, and larger Ktrans, Kep and Ve in DCE-MRI. In clinical practice, conventional MRI and functional MRI can be combined to differentiate benign and malignant ovarian lesions, thereby improving the accuracy and enable patients to receive treatment as soon as possible.

Short-term Effiaccy and Safety of Uniportal Thoracoscopic Lobectomy for Patients with Early-stage Lung Cancer

Objective:
To explore the short-term effect and safety of uniportal video-assisted thoracoscopic surgery (VATS) lobectomy for patients with early-stage lung cancer.
Methods:
The clinical data of 107 patients with early-stage lung cancer in our hospital undergoing VATS lobectomy during January 2017 and January 2020 were retrospectively reviewed. They were divided into uniportal group (n=55) and multi-port group (n=52) according to the surgical procedure. The patients underwent VATS lobectomy, and mediastinal lymph node dissection. The operation time, intraoperative blood loss, number of the harvested lymph node, the visual analogue scale (VAS), numerical rating scale (NRS), serum interleukin-6 (IL-6) and IL-8 after surgery of the patients in the two groups were recorded and compared. The complications within 6 months after surgery in both groups were also recorded.
Results:
The operation time in uniportal group was longer than the multi-port group; whereas the intraoperative blood loss was more than the multi-port group, without significant difference between the two groups (P>0.05). Furthermore, there was no significant difference in the number of lymph node dissection between the two groups (P>0.05, respectively). Moreover, the VAS, NRS, serum IL-6 and IL-8 of the paitents in uniportal group were significantly lower than those in the multi-port group (P<0.05, respectively). Additionally, the overall incidence of complications within 6 months after surgery in the uniportal group was similar to the multi-port group (P>0.05).
Conclusion:
Uniportal VATS lobectomy was feasible and safe in patients with early-stage lung cancer, and it can alleviate postoperative pain and inflammation stress.

Influence of Terminal Ileostomy and Transverse Colostomy on Perioperative Indicators Postoperative Complications and Prognosis of Patients with Low Rectal Cancer Undergoing Laparoscopic Anus-preserving Surgery

Objective:
To compare the influence of terminal ileostomy and transverse colostomy on perioperative indicators, postoperative complications and prognosis of patients with low rectal cancer undergoing laparoscopic anus-preserving surgery.
Methods:
The clinical data of 151 patients with low rectal cancer who received laparoscopic anus-preserving surgery in the hospital between January 2018 and April 2020were retrospectively analyzed. According to different stoma methods, they were divided into observation group (n=78, terminal ileostomy) and control group (n=73, transverse colostomy). The perioperative indicators, anal status at 1 month, 6 months and 12 months after surgery (Wexner anal function score scale), defecation status at 6 months after surgery (Xu Zhongfa's five-item ten-point scale) and prognosis at 1 month, 6 months and 12 months after surgery [European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30 (EOETC QLQ-C30)] were compared. The complications of the two groups were compared.
Results:
There was no statistical significance in the surgical time and intraoperative blood loss between the two groups (P>0.05). The postoperative first eating time, first defecation time and hospital stay were obviously shorter in observation group than in control group (P<0.05). There were statistical distinctions in the between-group effect, time-point effect and interaction effect of between-group and time-point of anal status score (P>0.05). The excellent and good rate of defecation status in observation group was observably higher than that in control group at 6 months after surgery (P<0.05). There were statistically significant differences in the between-group effect and time-point effect of EOETC QLQ-C30 evaluation (P<0.05), but the interaction effect of between-group and time-point was not statistically significant (P>0.05). There were no obvious differences in the incidence rates of complications between groups (P>0.05).
Conclusion:
Compared with transverse colostomy, terminal ileostomy has a better application effect on patients with low rectal cancer undergoing laparoscopic anus-preserving surgery. It can promote postoperative recovery and anal recovery, and improve the prognosis of patients.

Correlation Between Plasma Low Density Lipoprotein and Severity of Coronary Heart Disease in the Elderly

Objective:
To explore the relationship between the distribution of plasma low-density lipoprotein (LDL) subtypes and the severity, prognosis of senile coronary heart disease (SCHD) patients.
Methods:
A total of 192 SCHD patients who were hospitalized in Cardiology Department of our hospital from December 2018 to October 2019 were selected as the research objects (observation group), and 189 cases with normal coronary artery examination were selected as the control group. The levels of blood lipid were measured by automatic biochemical analyzer; the LDL subtypes were electrophoretic separated by linear polyacrylamide gel. The quantitative analysis of LDL subtypes was completed by high-precision scanning; the correlation between plasma LDL3, small and dense LDL (sdLDL) and Gensini score was analyzed by Pearson method.
Results:
The levels of plasma triglyceride (TG), total cholesterol (TC) and LDL cholesterol (LDL-C), LDL3, ldl4, ldl5, ldl6, ldl7 and sdLDL in the observation group were significantly higher than those in the control group, and the level of plasma high density lipoprotein cholesterol (HDL-C) was significantly lower than that in the control group (P<0.05). The levels of plasma LDL3 and sdLDL in single vessel group, double vessel group and three vessel group were significantly increased in turn (P<0.05). The levels of plasma LDL3 and sdLDL in the high Gensini score group were significantly higher than those in the low and middle score groups, the levels of plasma LDL3 and sdLDL in middle Gensini score group were significantly higher than those in low score group (P<0.05). The levels of plasma LDL3 and sdLDL in SCHD patients were positively correlated with Gensini score (r / P = 0.536/0.000, 0.521/0.000). The levels of plasma LDL3 and sdLDL in poor prognosis group were significantly higher than those in good prognosis group (P<0.05). Logistic regression analysis showed that LDL3, sdLDL and Cr were independent risk factors for the severity of SCHD patients. while age, sdLDL and Gensini score were independent risk factors for the prognosis of SCHD patients (P<0.05).
Conclusion:
The levels of LDL3 and sdLDL in different subtypes of plasma LDL in SCHD patients are significantly increased, which are related to the increase of the number of diseased branches, the increase of Gensini score and the occurrence of MACE.

Comparative Study on the Effect of Different Approaches of Intramedullary Nail Internal Fixation in Tibial Fractures

Objective:
To explore the effects of different approaches of intramedullary nailing on complications, bone markers and prognosis of patients with tibial fracture, in order to find a safer and effective treatment.
Methods:
A retrospective analysis of the clinical data of 125 tibial fracture patients in our hospital from February 2018 to February 2020 was divided into suprapatellar group (n=63) and transpatellar ligament group (n=62) according to different treatment plans. The transpatellar ligament group was fixed with intramedullary nail via the transpatellar ligament approach, and the suprapatellar approach was adopted for the suprapatellar group. The perioperative indicators and complications of the two groups were compared with the inflammatory factors [Interleukin-1β (IL-1β) and C-reactive protein (C-reactive protein, CRP) before and 1 d and 3 d after the operation)], tumor necrosis factor-α (Tumor necrosis factor-α, TNF-α)], bone markers [osteocalcin (BGP), calcitonin (Calcitonin, CT), Total Nterminal propeptide of type I procollagen (Total-PINP), β-C-terminal telopeptide of type I collagen (β-CTX)] levels , Knee motion (maximum angle of knee flexion and extension) before operation, 3 months after operation, 6 months after operation, 12 months, American Special Surgery Hospital (HSS) scoring system, Johner-Wruhs scoring standard (Johner wruhs scoring system, Johner-Wruhs) scoring.
Results:
The fracture healing time of the suprapatellar group was shorter than that of the transpatellar ligament group, and the number of intraoperative fluoroscopy and the 3 d visual analogue scale (VAS) scores were lower than those of the transpatellar ligament group (P<0.05). The levels of serum IL-1β, CRP, TNF-α and Total-PINP and β-CTX levels in the suprapatellar group at 1 and 3 days after operation were lower than those in the transpatellar ligament group at 2 and 4 weeks after operation. The levels of serum BGP and CT at 2 and 4 weeks after operation were higher than those in the transpatellar ligament group (P<0.05). The postoperative complication rate in the suprapatellar group was 7.94%, which was lower than 22.58% in the transpatellar ligament group (P<0.05). The maximum angles of knee flexion and extension in the suprapatellar group were higher than those in the transpatellar ligament group at 3 months, 6 months, and 12 months after surgery. HSS and Johner-Wruhs scores were higher than those in the transpatellar ligament group (P<0.05).
Conclusion:
The suprapatella approach intramedullary nail internal fixation for tibial fractures can reduce postoperative pain, promote the improvement of bony markers and fracture healing, improve the range of motion, knee function and clinical function of the knee joint, and reduce complications. In addition, the body's inflammatory response is mild, and the overall effect is better than that of transpatellar ligament approach surgery.

The Effect of Percutaneous Transforaminal Endoscopic Technique on ODI Index and Pain Mediators in Elderly Patients with Free LDH

Objective:
To investigate the effect of percutaneous transforaminal endoscopic discectomy (PTED) on surgical efficacy, Oswestry dysfunction index (ODI), serum substance P (SP), prostaglandin E2 (PGE2), neuropeptide Y (NPY) and postoperative adverse events in elderly patients with free lumbar intervertebral disc herniation (M-LDH).
Methods:
The clinical data of 117 elderly M-LDH patients in our hospital from August 2018 to August 2020 were collected for retrospective analysis. According to the surgical plan, they were divided into a control group (n=58) and an observation group (n=59). The control group underwent mobile microendoscopic discectomy (MMED), and the observation group underwent PTED. The surgical efficacy, perioperative conditions, and postoperative adverse events of the two groups were observed, and the ODI index and pain degree (VAS score), serum SP, PGE2, NPY levels, and inflammatory stress index levels [tumor necrosis factor-α (TNF-α), matrix metalloproteinase-3 (MMP-3), malondialdehyde (MDA)] before and after the operation were compared.
Results:
The intraoperative blood loss of the observation group was less than that of the control group, and the operation time and the number of fluoroscopy were longer than those of the control group (P<0.05). Compared with the control group, the serum SP, PGE2 and NPY levels were lower in the observation group one month after the operation (P<0.05). Serum levels of TNF-α, MMP-3 and MDA in the observation group were lower than those in the control group on the 3rd day after operation (P<0.05). The VAS and ODI index scores of the observation group were lower than those of the control group at 1 month after operation (P<0.05). There was no significant difference in the incidence of adverse events and the excellent and good rate of surgery at 3 months after operation (P>0.05).
Conclusion:
PTED is a safe and effective measure for the treatment of elderly patients with M-LDH. It can further relieve inflammatory stress, reduce postoperative lumbar pain, and improve postoperative lumbar spine function. It is worthy of promotion.

Diagnostic Value of Ultrasonography, Serology and Their Combination in Trisomy 21 Syndrome

Objective:
To investigate the diagnostic value of ultrasonography combined with serology for trisomy 21 syndrome.
Methods:
1500 pregnant women for Production inspection from January 2017 to March 2021 in our hospital were selected as the research object, and were detected with ultrasound and serological. Those who were diagnosed with trisomy 21 syndrome were suggested to have amniotic fluid chromosome screening, and to be followed-up if not had amniotic fluid chromosome screening. The detection rates of trisomy 21 syndrome with ultrasonography and serology, alone or combined, were compared, the consistency of the three tests with the gold standard was analyzed by Kappa test, and the diagnostic value of the three tests for trisomy 21 syndrome was evaluated by ROC curve.
Results:
A total of 218 cases of trisomy 21-syndrome were diagnosed by amniotic fluid chromosome and follow-up results of 1500 pregnant women, among which 165 cases were detected as high risk and 53 cases as low risk by ultrasound. Serological tests showed high risk in 183 cases and low risk in 35 cases. Joint examination revealed 204 cases with high risk and 14 cases with low risk. Compared with the gold standard test results (Kappa=0.501, 0.647, 0.895, P< 0.05). ROC analysis showed that the AUC of ultrasonic serology and combined diagnosis of trisomy 21 syndrome were 0.809, 0.874 and 0.958, respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy were 75.69%, 86.19%, 48.24%, 95.42%, 84.67%, respectively. Serological diagnosis was 83.95%, 90.87%, 61.0%, 97.08%, 89.87%, respectively; the combined diagnosis was 93.58%, 97.97%, 88.70%, 98.90%, and 97.33%, respectively, both of which were of certain value.
Conclusion:
Both prenatal ultrasound and serology have higher value in prenatal screening for trisomy 21 syndrome, but the combined diagnosis value of the two is better.

Effect of Intramuscular Binding Combined with Isokinetic Muscle Strength Training on Shoulder Joint and iEMG in Patients with Rotator Cuff Injury

Objective:
To explore the effect of intramuscular binding combined with isokinetic muscle strength training on shoulder joint and integrated electromyography (iEMG) in patients with rotator cuff injury.
Methods:
119 patients with rotator cuff injury in our hospital from October 2020 to April 2021 were randomly divided into control group (n = 59) and observation group (n = 60). The control group received routine rehabilitation training, and the observation group received isokinetic muscle strength training + intramuscular binding technology based on the control group for 4 consecutive courses of intervention. The degree of pain [visual analogue scale (VAS)], shoulder function [University of California shoulder scoring system (UCLA)], shoulder range of motion (flexion, internal rotation, external rotation and extension), rotator cuff muscle iEMG, degree of muscle atrophy and quality of life [concise health status scale (SF-36)] before and after intervention were counted.
Results:
The VAS scores (3.05 ± 0.61, 1.73 ± 0.53) in the observation group after 2 courses of intervention and 4 courses of treatment were lower than those in the control group (3.38 ± 0.65, 2.02 ± 0.62), the difference was statistically significant (T1 = 2.856, T2 = 2.744, P<0.05). After 4 courses of intervention, the UCLA score of the observation group was higher than that of the control group (P<0.05). After 4 courses of intervention, the activities of external rotation, internal rotation, flexion and extension of shoulder joint in the observation group (72.26 ± 5.08, 55.31 ± 4.44, 170.43 ± 13.38, 54.33 ± 4.12) ° were greater than those in the control group (57.73 ± 5.29, 46.62 ± 5.13, 145.56 ± 17.73, 36.61 ± 4.59) ° with significant difference (T1 = 15.284, T2 = 9.886, T3 = 8.646, T4 = 22.170, P<0.05). Analysis of Infection of Nosocomial Incision and Its Influencing Factors after Repair of Lower Extremity Wound Skin Flap

Objective:
To investigate the infection of the nosocomial incision and its influencing factors after the repair of the lower limb wound flap.
Methods:
282 patients who underwent lower extremity wound flap repair in our hospital from January 2017 to December 2020 were selected for retrospective analysis. The factors that may affect the nosocomial incision infection were collected. According to the presence or absence of nosocomial incision infection, the patients were divided into infected group and uninfected group. The medical records of the two groups were compared, and the factors with significant differences in univariate analysis were comprehensively analyzed by multivariate logistic regression analysis.
Results:
A total of 23 of the 282 patients undergoing skin flap repair of lower limb wounds included in this study had incision infection, and the infection rate was 8.16%. A total of 36 pathogenic bacteria were isolated and cultured in 23 patients, of which gram-positive bacteria accounted for 61.11%, gram-negative bacteria accounted for 33.33%, and fungi accounted for 5.56%.On the basis of univariate analysis and multivariate analysis, the results showed that the risk factors of nosocomial incision infection after lower limb skin flap repair were the prolongation of post injury visit time, operation time, hospital stay, glucocorticoid use and diabetes history, and the protective factors were the skin lesion area more than 10% of the wound area and the use of preventive antibiotics (P<0.05).
Conclusion:
The rate of nosocomial incision infection after lower limb skin flap repair is high, mainly Gram-positive infection, which is mainly affected by the time of treatment, operation time, hospital stay and other factors. In clinical practice, targeted intervention can be carried out for the above factors to reduce the incidence of nosocomial incision infection in such patients.

Outcome of 440 Chronic Total Occlusion and Left Ventricular Dysfunction Patients with or Without Enhanced External Counterpulsation

Objective:
To explore the effect of Enhanced External Counterpulsation(EECP) on the cardiac function and prognosis of chronic total occlusion (CTO) patients with reduced ejection fraction.
Methods:
A total of 220 CTO inpatients with EECP therapy and a control group of 220 cases without EECP, which were consecutively enrolled from January 2015 to January 2019 in the division of cardiology in our hospital. The clinical data of the patients were collected and analysis.
Results:
There were not significant difference in smoking, hypertension and type 2 diabetes between two groups (all P>0.05). Compared to control group, left ventricular ejection fraction,6-minute walk test(6MWT) and B-type natriuretic peptide(BNP) were significantly higher in EECP group after therapy (all P<0.05). In addition, Point-biserial correlation analysis found that EECP was associated with 6MWT and BNP, which correlation coefficient were 0.454 and -0.523. Besides, the multivariate COX regression model showed that EECP was an independent protect prognostic risk factors of CTO with reduced ejection fraction and HR was 0.357(P<0.05).
Conclusion:
EECP therapy can improve exercise tolerance and cardiac function, which was an independent protect prognostic risk factors of CTO with reduced ejection fraction undergone PCI.

Logistic Regression Equation Analysis of the Incidence of OSAHS and Influencing Factors in Patients with Acute Cerebral Infarction Evaluated by Polysomnography Monitor

Objective:
To investigate the polysomnography monitor to evaluate the incidence of obstructive sleep apnea hypopnea syndrome (OSAHS) in patients with acute cerebral infarction (ACI), and to further analyze its influencing factors by Logistic regression equation.
Methods:
A total of 192 ACI patients admitted to our hospital from May 2019 to September 2021 were selected as the research objects, and all of them were examined by a polysomnography monitor. The incidence of OSAHS was counted, and the logistic regression equation was used to analyze the influencing factors of OSAHS.
Results:
After 192 ACI patients were examined by a polysomnography device, the average AHI was (22.19±18.29) times/h, and the average minimum blood oxygen saturation was (80.78±9.67)%. The longest hypopnea time was (83.3±29.31) s, the longest apnea time was (49.96±5.13) s, and the total incidence of OSAHS was 77.08% (148/192). Logistic regression analysis showed that BMI >24 kg/m
2
, history of diabetes, longest apnea time >40 s, supine position, were risk factors for the occurrence of OSAHS (P<0.05). The AUC of the OSAHS efficacy predicted by the various parameters of the polysomnography monitor was 0.889 (95CI: 0.843~0.926), and the prediction sensitivity and specificity were 71.18% and 91.43%, respectively.
Conclusion:
Affected by many factors, patients with ACI are at risk of OSAHS, and polysomnography monitoring has good predictive value. In clinical work, prevention should be strengthened for high-risk groups with the above factors to further reduce the incidence of OSAHS.

An Analysis of the Relationship Between Serum Eotaxin Level and Lung Function Eos and SIgE in Children with Asthma

Objective:
To explore the relationship between serum Eotaxin level and lung function, Eos and sIgE in children with asthma.
Methods:
A retrospective analysis of the basic data of 65 healthy subjects (control group) and 67 children with asthma (observation group) collected from February 2018 to September 2020 were all tested for lung function, Eotaxin, Eos, and sIgE. The above indicators between the two groups of children were compared, and Pearson's method was conducted to analyze the correlation between Eotaxin and Eos, sIgE, and lung function.
Results:
PEF, FEV1 and FVC in the observation group were lower than those in the control group, while Eotaxin, Eos and sIgE were higher than those in the control group (P<0.05). According to Pearson method, Eotaxin is positively correlated with Eos and sIgE; it is negatively correlated with PEF, FEVI, and FVC. Multiple stepwise regression analysis showed that Eos, sIgE, and FEVI are all Eotaxin influencing factors, and Eotaxin=17.791+4.169×Eos+0.779×sIgE+(-5.711)×FEVI (R2=0.974).
Conclusion:
Eotaxin is closely related to Eos, sIgE and lung function, and participates in the pathophysiological process of children with asthma.

Clinical Study on Cardiac Toxicity of Anthracycline Chemotherapy Using Echocardiography Dot Tracking

Objective:
To study the application value of echocardiography dot tracking in the cardiotoxicity monitoring of anthracycline chemotherapy drugs.
Methods:
This is a retrospective analysis. The study subjects were 50 breast cancer patients admitted to the hospital from January 2018 to January 2020, who were treated with anthracycline chemotherapy drugs for 4 cycles before undergoing 12-lead ECG examination. According to the results, they were divided into a normal ECG group (25 cases) and an abnormal ECG group (25 cases). At the same time, 25 healthy women for physical examination during the same period were selected as the control group. All three groups had conventional echocardiographic examinations, related parameters were measured, two-dimensional speckle tracking imaging technology was selected to measure biomechanical parameters, and the three groups of conventional echocardiographic parameters and two-dimensional speckle tracking imaging technology related parameters were compared.
Results:
Compared with the normal ECG group and the control group, the E/A ratio of the abnormal ECG group was lower, and LVEDV was higher (P<0.05); compared with the control group, the PSD of the normal ECG group was higher, and the GLS, aLS, mLS, bLS, GRS, LV-Ptw, and AP-Prot were lower (P<0.05); compared with the normal ECG group and the control group, the PSD of the abnormal ECG group was higher, and GLS, aLS, mLS, bLS, GRS, GCS, LV-Ptw, AP-Prot were lower (P<0.05).
Conclusion:
The effect of echocardiographic spot tracking technology in the early monitoring of cardiotoxicity caused by anthracycline chemotherapeutics is better than conventional echocardiography. Observing the longitudinal myocardial activity of the left ventricle is helpful for evaluating early cardiac systolic dysfunction.

Analysis on Effect of Different Flux Smoothing Methods of Monaco Planning System on Postoperative Volume Rotary Intensity Modulated Radiotherapy for Rectal Cancer

Objective:
To investigate the dosimetric differences of volume rotary intensity modulated radiotherapy (VMAT) plan designed by three different flux smoothing methods in Monaco plan system.
Methods:
16 patients with rectal cancer were randomly selected to design VMAT plans (FSH, FSM, FSL) by three different fluence smoothing modes under the same optimized conditions and used the Monaco 5.11 TPS and the VersaHD accelerator model. The dose verification was measured by matrixx detector. The parameters mainly included the target volume and OAR dose-volume histogram, as well as conformity index (CI), homogeneity index (HI), dose gradient index (GI), monitor unit (MU), control point (CP), beam time (T) and gamma pass rate (GRP) were considered.

Conclusion:
In the postoperative VMAT plan for rectal cancer, the dosimetric parameters and GRP of FSH, FSM and FSL can meet the clinical requirements. FSL is superior to FSM and FSH in improving the quality of the plan, reducing the radiation dose and toxic and side effects of bladder and femoral head; FSH and FSM are better than FSL in improving the efficiency of plan implementation. Therefore, when designing the plan, we should comprehensively consider the clinical requirements and individual differences of patients, and give priority to FSM, which can not only meet the better plan quality, but also improve the plan implementation efficiency.

The Effect of Personal Rehabilitation Exercise on Cardiopulmonary Function of Patients with Chronic Heart Failure Under Precise Customization

Objective:
To investigate the effect of neomycin combined with accurately customize cardiac rehabilitation exercise on the heart failure patients' prognosis.
Methods:
108 heart failure patients were divided into control group(54 cases) and study group(54 cases) . The study group endured conventional treatment of chronic heart failure, intravenous infusion of neomycin combined with accurately customize cardiac rehabilitation exercise therapy. The control group endured routine treatment. The cardiac function, serum NT-proBNP levels and the total effective rate between the two groups were compared.
Results:
After treatment, the levels of LVEF, SV, CI and CO in the observation group were significantly higher than those in the control group (P<0.01), and the levels of NT-proBNP were significantly lower than those in the control group(t=11.47,P<0.01), the observation group's effective rate was 90.74%, which was significantly higher than the control group's 72.23% (χ

Conclusion:

The Effect of Dapagliflozin on Type 2 Diabetes Mellitus with Cardiac Insufficiency and Its Influence on Cardiac Function Platelet Lymphocyte Ratio and Plasma Hcy Level

Objective:
To explore the effect of Dapagliflozin on heart function, platelet lymphocyte ratio (PLR) and plasma homocysteine (hcy) in patients with type 2 diabetes mellitus.
Methods:
171 patients with type 2 diabetes mellitus complicated with cardiac insufficiency in our hospital from January 2019 to January 2020 were randomly divided into comparison group (n=85) and observation group (n=86). Clinical efficacy, blood glucose level [glycosylated hemoglobin (HbA1c), fasting blood glucose (FPG) and 2h postprandial blood glucose (2hPG)], cardiac function [left ventricular ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDD) and 6 min walking test (6MWT)], PLR, plasma hcy level and adverse reactions were compared between the two groups.
Results:
The total effective rate of the observation group was 94.19%, which was significantly higher than that of the control group (84.71%) (P<0.05); after treatment, compared with the control group, the observation group had significantly lower levels of HbA1c, FPG and 2hPG (P<0.05), LVEF, 6MWT levels were significantly higher (P<0.05), LVEDD, PLR, and plasma hcy levels were significantly lower (P<0.05); the incidence of adverse reactions in the observation group was 10.47%, slightly higher than 5.88% in the control group. (P>0.05).
Conclusion:
Dapagliflozin for patients with type 2 diabetes complicated with cardiac insufficiency has a good clinical effect, which can obviously improve the blood glucose level and cardiac function of patients, regulate PLR and plasma hcy levels, and has good safety.

Effect of Ergometrine Combined with Carboprost Aminobutyric Triol on Preventing Postpartum Hemorrhage in Patients with Scar Uterus after Cesarean Section and Its Effect on GSH Px SOD MDA and Postpartum Lactation

Objective:
To investigate the effect of the combination of Ergometrine and Carboprost Tromethamine Injection in preventing postpartum hemorrhage in patients with scarred uterus re-cesarean section and its influence on glutathione peroxidase (GSH-px), superoxide dismutase (SOD), malondialdehyde (MDA) and postpartum lactation.
Methods:
A total of 92 patients with scarred uterus re-cesarean section in our hospital from February 2018 to April 2020 were selected as the research objects. They were divided into observation group and control group by simple randomization method, with 46 cases in each group. The control group was treated with Carboprost Tromethamine Injection to prevent postpartum hemorrhage, and the observation group was treated with Ergometrine combined with Carboprost Tromethamine Injection to prevent postpartum hemorrhage. The efficacy, postpartum hemorrhage, recovery, lactation, and adverse reactions, coagulation function indexes [activated partial thromboplastin time (APTT), prothrombin time (PT), fibrinogen (FIB), D-dimer (DD)], serum GSH-px, SOD, MDA levels before and 24 hours after the operation of the two groups were compared.
Results:
After treatment, the total effective rate of the observation group was 93.48% higher than that of the control group 76.09% (P<0.05). The bleeding volume and rate of postpartum hemorrhage at 2 h and 24 h in the observation group were lower than those in the control group, the duration of lochia and hospital stay were shorter than those in the control group, and the uterine fundus declined faster than the control group (P<0.05). The levels of APTT, PT, FIB, DD, and serum MDA in the two groups at 24 h after surgery were lower than those before the operation, and the observation group was lower than the control group, and the serum GSH-px and SOD levels were higher than those before operation, and the observation group was higher than the control group (P<0.05). The lactation initiation time of the observation group was shorter than that of the control group, and the lactation volume scores of the 1 and 2 d postpartum were higher than those of the control group (P<0.05); there was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).
Conclusion:
The combined application of Ergometrine and Carboprost Tromethamine Injection to prevent postpartum hemorrhage in patients with scarred uterus re-cesarean section can significantly improve the preventive effect, reduce postpartum hemorrhage, and promote the recovery of patients, improve coagulation function and lactation, reduce serum GSH-px, SOD, MDA levels, reduce oxidative stress, and have good safety.

Short-Term Efficacy of Anlotinib in the Treatment of Non-Small Cell Lung Cancer and Its Influence on Serum Tumor Markers CTC VGEF and Side Effects

Objective:
To explore the short-term efficacy of anlotinib in the treatment of non-small cell lung cancer (NSCLC), and its influence on serum tumor markers, circulating tumor cells (CTC), vascular endothelial growth factor (VGEF) and side effects.
Methods:
Totally 94 patients with NSCLC treated in the hospital were selected between August 2019 and December 2020. They were divided into anlotinib group (n=49, anlotinib) and control group (n=45, conventional chemotherapy) according to random number table method. Relevant parameters were compared between the two groups.
Results:
The total response rate and Karnofsky Performance Scale (KPS) score of the observation group were higher than those of the control group (P<0.001). After treatment, there was no difference in the positive detection rate of CTC between the two groups (P>0.05), but the concentrations of VGEF, serum tumor markers carcino-embryonic antigen (CEA) and cytokeratin 19 fragment antigen 21-1 in the anlotinib group were lower than those in the control group (P<0.05). There were no differences in the total incidence and severity of adverse reactions between the two groups (P>0.05). Hypertension, thyroid dysfunction, and hand-foot syndrome were main adverse reactions in the anlotinib group, which were controlled after reduction and symptomatic treatment.
Conclusion:
Anlotinib can significantly improve the short-term efficacy in the treatment of NSCLC, effectively lower the levels of serum tumor markers and VGEF as well as the positive detection rate of CTC, and improve the patients’ quality of life. Therefore, it is worthy of clinical promotion and application.

Effect of Benbromarone Combined with Low-Dose Febuxostat on Scd163 and NLRP3 Levels in Patients with Hyperuricemia and Gout

Objective:
To observe the effect of benbromarone combined with low-dose febuxostat on soluble monocyte macrophage hemoglobin scavenger receptor (sCD163) and nucleotide binding oligomerization domain like receptor 3 (NLRP3) in patients with hyperuricemia (HUA) and gout.
Methods:
110 patients with HUA and gout in our hospital from September 2017 to December 2020 were selected, and they were randomly divided into control group (55 cases) and study group (55 cases). Patients in the control group were treated with benbromarone, while patients in the study group were treated with benbromarone combined with low-dose febuxostat for 2 months. The clinical total effective rate, serum sCD163, NLRP3 levels and renal function indexes of the two groups were observed after treatment, and the occurrence of adverse reactions in the two groups were recorded.
Results:
The clinical total effective rate of the study group was higher than that of the control group, the difference was statistically significant (P<0.05). After treatment, the level of sCD163 in the observation group was higher than that in the control group, and the levels of NLRP3, Serum creatinine (Scr), uric acid (BUA), blood urea nitrogen (BUN) in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).
Conclusion:
Benbromaton combined with low dose of fenbustam in the treatment of patients with gout can promote BUA excretion, effectively regulate the serum levels of scd163 and NLRP3, promote the recovery of renal function, and the effect is significant.

Effect of Protein Iron Succinate Combined with Vitamin C on Reticulocyte and Crythrocyte Parameters in Pregnant Women with Iron Deficiency Anemia

Objective:
To explore the curative effect of iron-protein succinylate combined with vitamin C in the treatment of pregnant women with pregnancy complicating iron deficiency anemia (IDA) and its influence on reticulocytes and red blood cell parameters.
Methods:
Totally 147 pregnant women with pregnancy complicating iron deficiency anaemia who were treated in our hospital were selected as the research subjects, and they were divided into combined group and control group according to the Simple random grouping. Control group (n=73) was treated with ferrous fumarate tablets, and combined group (n=74) was given iron-protein succinylate and vitamin C. The clinical curative effect, reticulocytes [absolute reticulocyte value (RET), immature reticulocyte fraction (IRF), corpuscular hemoglobin concentration mean of reticulocyte (CHCMr)], red blood cell parameters [red blood cell count (RBC), mean corpuscular volume (MCV), hemoglobin (Hb), mean corpuscular hemoglobin (MCH)], iron metabolism [serum iron (SI), serum ferritin (SF), transferrin receptor (sTFr), serum total iron binding capacity (TIBC)], adverse reactions and perinatal outcomes were compared between the two groups.
Results:
After 6 weeks of treatment, the total clinical effective rate was 95.95% in combined group and was 86.30% in control group (P<0.05). The levels of RET and CHCMr in the peripheral blood of the two groups were significantly increased (P<0.05) while the level of IRF was significantly reduced (P<0.05), and the difference in the amplitude of change between the two groups was statistically significant after treatment (P<0.05). The levels of peripheral blood RBC, MCV, Hb, and MCH in the two groups were significantly increased (P<0.05), and the levels of serum SI and SF were significantly increased (P<0.05) while the levels of sTFr and TIBC were significantly reduced, and the difference in the amplitude of change between the two groups was statistically significants after treatment (P<0.05). During treatment, the total incidence rate of adverse reactions was 5.41% in combined group and was 9.59% in control group (P>0.05). At follow-up after treatment, the premature birth rate and incidence rate of maternal-infant adverse events were 4.05% and 5.41% in combined group and were 6.85% and 10.96% in control group (P>0.05).
Conclusion:
Iron-protein succinylate combined withvitamin C in the treatment of pregnancy complicating iron deficiency anemia can significantly improve the therapeutic effect, improve the development of red blood cells, and promote iron metabolism, with fewer adverse drug reactions and perinatal adverse events.

Study on the Mechanism of Wenyang Huoxue Decoction in the Treatment of Bone Metastatic Carcinoma

Objective:
To study the mechanism of Wenyang Huoxue Decoction in the treatment of bone metastatic carcinoma by network pharmacology.
Methods:
TCMSP, TCMID and TCM Database@Taiwan databases and literature search were used to screen the main effective components of drugs. Swiss Target and TCMSP databases were used to search the potential therapeutic targets of Wenyang Huoxue Decoction. DisGeNet and Drugbank databases were used to search the genes of bone metastatic carcinoma. Drug action targets and disease genes were mapped. Cytoscape 3.6.0 software was used to visualize and screen out the core genes. GO and KEGG pathway analysis was performed on potential therapeutic targets.
Results:
There were 109 main active ingredients in Wenyang Huoxue Decoction, which contained 374 target proteins. There are 773 disease targets of bone metastatic carcinoma, of which 105 are potential targets of Wenyang Huoxue Decoction for the treatment of bone metastatic carcinoma. Through GO and KEGG pathway analysis, it was found that Wenyang Huoxue Decoction exerted anti-cancer, inhibiting bone metastasis and immune system regulation mechanism through HIF-1, TNF, VEGF and other signaling pathways in the treatment of bone metastatic carcinoma. The key genes of Wenyang Huoxue Decoction in treating bone metastatic carcinoma are VEGFA, AKT1, IL6, TP53, ALB, CXCL8, CASP3, TNF, EGFR, EGF and MAPK3 and so on.
Conclusion:
Through network pharmacology, we found that the mechanism of Wenyang Huoxue Decoction in treating bone metastatic carcinoma involves multiple targets and pathways, which may be related to anti-cancer, inhibition of bone metastasis, regulation of immune system, etc.
