Correlation Analysis of Neonatal VAP Mice and Copeptin IL-32 IL-17 Levels and Different Degrees of Pneumonia

Objective:
To explore the symptoms and influencing factors of ventilator-associated pneumonia (VAP) to provide a basis for clinical diagnosis and treatment.
Methods:
54 Balb/c mice were randomly divided into a control group, mild group (1×10

cfu/L), severe group (6×10
6
cfu/L) (n=18). Except for the control group, mice were mechanically ventilated for 24 hours and tracheal instillation of Pseudomonas aeruginosa to construct a VAP model. Forced vital capacity (FVC) and forced expiratory volume in the first second (FEV1) were measured at 1, 3, and 7 days after the model was constructed, respectively, and the value of FEV1/FVC% was calculated. The contents of Copeptin, IL-32 and IL-17 in serum were determined by ELISA. The staining was used to observe the histopathological changes of lung after 7 days. The protein expression levels of NF-κB, TLR2 and TLR4 in lung tissues were determined by immunohistochemical after 7 days. The lung function indexes were correlated with the levels of Copeptin, IL-32 and IL-17. The results of Copeptin, IL-32, IL-17 and the incidence of VAP were analyzed by ROC to evaluate the risk factors of VAP.
Results:
Within 7 days after the occurrence of VAP, compared with the control group, the FVC and FEV1 levels of the mild and severe groups were decreased, while the serum levels of Copeptin, IL-32 and IL-17 were increased (P<0.05); The number of inflammatory cells, neutrophils and the expression of TRL2, TRL4 and NF-κB protein in lung increased with the aggravation of VAP symptoms; Correlation analysis and ROC analysis showed that lung function indexes were closely correlated with the levels of Copeptin, IL-32, and IL-17, with a negative correlation trend.
Conclusion:
The incidence of ventilator-associated pneumonia is closely related to Copeptin, IL-32 and IL-17, and its mechanism may be related to the activation of NF-κB pathway to mediate inflammatory response, thereby reducing lung function.

Effects of Panax Notoginseng Saponins Combined with Aminoguanidine Treatment on Renal Function Blood CRP TNF-α MDA and SOD Levels in Diabetic Nephropathy Rats

Objective:
To explore the effects of panax notoginseng saponins combined with aminoguanidine treatment on renal function, blood C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), malondialdehyde (MDA) and superoxide dismutase in diabetic nephropathy rats ( SOD) level.
Methods:
75 clean-grade male SD rats were randomly divided into normal control group (15 rats) and experimental group (60 rats). The experimental group was intraperitoneally injected with streptozotocin to establish a rat model of diabetic nephropathy. The rats in the experimental group were randomly divided into model group (13 rats), PNS group (13 rats), AG group (13 rats), and combination group (13 rats). The rats were given corresponding drugs by gavage for 4 weeks. Observe the pathological changes of rat kidney tissues in each group, and compare the levels of glomerular volume, blood glucose, blood creatinine, 24h urine protein, plasma CRP, TNF-α, serum MDA and SOD in each group.
Results:
The glomerulus in the normal control group had a regular and complete appearance, with few mesangial cells and thin basement membrane; the shape of the renal tubules was clear, and the basement membrane was unchanged. The glomerulus of the experimental group was significantly enlarged, mesangial cells increase, basement membrane thickens, and renal tubules. There was no significant change; compared with the model group, the glomerular pathology of the PNS group, AG group, and combination group was significantly improved. The glomerular volume, blood glucose, blood creatinine, 24h urine protein, plasma CRP, and TNF-α level of the model group, PNS group, AG group, and combination group were higher than those of the normal control group, PNS group, AG group, and combination group. Blood glucose, blood creatinine, 24h urine protein, plasma CRP, and TNF-α level were lower than the model group. The combined group had higher glomerular volume, blood glucose, blood creatinine, 24h urine protein, plasma CRP, and TNF-α level than the PNS group. AG group: The differences were statistically significant (P<0.05). The MDA level of the model group, PNS group, AG group, and the combination group was higher than the normal control group, and the SOD level was lower than the normal control group; the MDA level of the PNS group, the AG group, and the combination group was lower than the model group, and the SOD level was higher than the model group; The SOD level of the group was higher than that of the AG group: the differences were statistically significant (P<0.05).
Conclusion:
PNS combined with AG treatment can significantly improve glomerular pathology and glomerular function in diabetic nephropathy, reduce plasma inflammatory factors CRP and TNF-α level, and reduce oxidative stress level.

miR-145-5p Influences the Proliferation and Migration of Non-Small Cell Lung Cancer Cells by Regulating PHB2

Objective:
To explore the role and mechanism of miR-145-5p in regulating the proliferation and migration of non-small cell lung cancer cells by targeting PHB2.
Methods:
The expression of miR-145-5p in NSCLC tissues and adjacent tissues, normal human bronchial epithelial cell line (BEAS-2B), and non-small cell lung cancer cell line (A549) was assessed by RT-PCR. The effect of miR-145-5p overexpression on NSCLC activity was determined by MTT and EDU. The effect of miR-145-5p overexpression on migration and invasion of NSCLC was evaluated by scratch test and Transwell. The luciferase reporter assay confirmed the role of miR-145-5p in the 3' -untranslated region (3'-UTR) of the PHB2 protein coding gene PHB2 mRNA. The expression of PHB2 protein in A549 cells was detected by western blot.A549 cells were transfected with pcDNA-phB2 and the levels of cell proliferation, migration and invasion were assessed.
Results:
The expression of miR-145-5p in NSCLC was significantly lower than that in paracancerous tissues (P<0.01), which was about 47.54±10.12% of paracancerous tissues. After transfection with miR-145-5p mimic, the proliferation, migration and invasion of tumor cells were significantly inhibited in vitro (P<0.001), which were about 28.31±13.97%, 30.57±5.84% and 32.56±6.99% of those in the control group, respectively. miR-145-5p negatively regulates the expression of PHB2 by directly binding the 3'-UTR of PHB2 mRNA. After co-transfection with miR-145-5p and PHB2, the proliferation, migration and invasion ability of A549 cells were significantly enhanced compared with that of miR-145-5p group alone.
Conclusion:
MiR-506-3p inhibitions the proliferation, migration and invasion of non-small cell lung cancer cells by targeting PHB2 mRNA, and may be a promising therapeutic direction in anti-cancer therapy.

Expression of ACE2 and Collectrin in the Kidney of Spontaneously Hypertensive Rats

Objective:
To investigate the role of ACE2 and Collectrin in the development of hypertension by studying the expression of ACE2 and Collectrin in spontaneously hypertensive rats (SHR).
Methods:
Rats were randomly divided into 2 groups: WKY group and SHR group. RT-PCR was used to detect the expression levels of ACE2 and Collectrin mRNA in rat kidney tissues, and Western blot was used to detect the expression levels of ACE2 and Collectrin protein in rat kidney tissues.
Results:
Compared with the WKY group of the same age, the blood pressure in the SHR group was significantly increased (P<0.01), and the expression of ACE2 mRNA and protein in the kidney tissue was significantly reduced (P<0.01); compared with the 24-week-old WKY group, Collectrin mRNA and Protein expression and significantly increased (P<0.01); Compared with the 12-week-old SHR group, the 24-week-old SHR group had a significant decrease in ACE2 mRNA and protein expression (P<0.01), and Collectrin mRNA and protein expression was significantly increased (P<0.01).
Conclusion:
The expression of ACE2 and Collectrin mRNA and protein in kidney tissue is related to the increase of blood pressure.

Expression and Correlation of CARD9 PKM2 and CMTM6 in Breast Cancer Tissues

Objective:
To investigate the expression and correlation of caspase recruitment domain protein 9 (CARD9), pyruvate kinase M2 (PKM2), and chemokine-like superfamily member 6 (CMTM6) in breast cancer tissues.
Methods:
A total of 79 patients with breast cancer admitted to our hospital from November 2019 to November 2020 were selected as research objects. The expressions of CARD9, PKM2 and CMTM6 in cancer tissues and paracancerous tissues were detected by immunohistochemistry, and the expressions of CARD9, PKM2 and CMTM6 in cancer tissues and paracancerous tissues were detected by real-time fluorescence quantitative (RT-PCR). To analyze the relationship between CARD9, PKM2, CMTM6 expression and breast cancer.
Results:
Compared with paracancerous tissues, CARD9, PKM2 and CMTM6 expressions were higher in cancer tissues (P<0.05). With high degree of differentiation, lymph node metastasis, Ⅰ Ⅱ period compared to patients with breast cancer, low degree of differentiation, lymph node metastasis, Ⅲperiod breast cancer patients CARD9 Ⅳ, PKM2, CMTM6 had higher expression among patients of low degree of differentiation, lymph node metastasis and Ⅲ-Ⅳ period than patients of mediun-high degree of differentiation, no lymph node metastasis and I-II period (P<0.05). Correlation analysis among CARD9, PKM2 and CMTM6 showed a positive correlation between CARD9 and PKM2 (r=0.257, P=0.022). Card9 and CMTM6 were positively correlated (r=0.260, P=0.021). PKM2 and CMTM6 were positively correlated (r=0.254, P=0.024). Compared with CARD9, PKM2 and CMTM6, the combination of these three items had higher predictive value for breast cancer, with statistical difference (P<0.05).
Conclusion:
The facts that CARD9, PKM2, CMTM6 have abnormally high expression in breast cancer tissue and CARD9, PKM2, CMTM6 are linearly positively correlated are related with the degree of differentiation, lymph node metastasis, clinical stages, and participation in the occurrence and development of breast cancer. The three combined tests have high predictive value for breast cancer.

Changes of Serum Total Bile Acids and Hs-CRP BNP Levels in Patients with Atrial Fibrillation and Their Correlation with Heart Rate Variability

Objective:
To investigate the changes of serum total bile acid (total bile acid,TBA), hypersensitive C reactive protein (hypersensitive C reactive protein,hs-CRP), plasma brain natriuretic peptide (Brain Natriuretic Peptide,BNP) levels and their correlation with heart rate variability (heart rate variability,HRV) in patients with atrial fibrillation.
Methods:
To select and analyze 217 patients with atrial fibrillation admitted to our department from March 2017 to June 2019, and another 100 healthy controls were selected as control group. Both groups underwent 24 h ECG monitoring. The serum TBA, hs-CRP, BNP levels and the standard deviation (NN standard deviation,) HRV [24h all sinus rhythm intervals], RMSSD were measured and compared. NN standard deviation of mean of interval of mean per 5 mins and PNN50 were recorded.
Results:
The comparison shows that TBA, hs-CRP, BNP in the study group were higher than in the control group, SDNN, RMSSD, SDANN, PNN50 lower than the control group; the difference was statistically significant (P<0.05). Correlation analysis showed that TBA and SDNN, RMSSD, SDANN, PNN50 are positive (r=0.469,0.474,0.447,0.418, P<0.05); hs-CRP is in positive phase with SDNN, RMSSD, SDANN, PNN50 (r=0.560,0.595,0.600,0.582, P<0.05); BNP and SDNN, RMSSD, SDANN, PNN50 were positive (r=0.566,0.634,0.624,0.598, P<0.05).
Conclusion:
Serum TBA, hs-CRP, BNP levels increased in patients with atrial fibrillation, and serum TBA, hs-CRP, BNP levels were positively correlated with HRV.

DcR3 and ALB Levels and Prognosis of Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease Complicated with Respiratory Failure

Objective:
To analyze the relationship between the levels of DcR3 and ALB and the prognosis of AECOPD patients with respiratory failure.
Methods:
From May 2017 to May 2020,93 patients with acute exacerbation of chronic obstructive pulmonary disease in hospital were selected as observation objects, which were divided into observation group (combined respiratory failure ,41 cases) and control group (no symptoms of respiratory failure ,52 cases) according to whether the patients had combined respiratory failure. Differences in serum biochemical and inflammatory factors, arterial blood gas and pulmonary function were observed in the two groups. The survival rate of the two groups was observed at 1 year follow-up, and the independent risk factors affecting the prognosis of AECOPD patients with respiratory failure were analyzed by multivariate analysis.
Results:
DcR3,CRP,TNF-α in observation group was significantly higher than that in control group(P<0.05). ALB in observation group was significantly lower than that in control group(P<0.05). PO

An Analysis of the Correlation Between Serum 25-Hydroxyvitamin D and Hcy Levels and Left Ventricular Function in Patients with Hypothyroidism

Objective:
To analyze the correlation between serum 25-hydroxyvitamin D[25(OH)D]and homocysteine(Hcy) levels and left ventricular function in patients with hypothyroidism.
Methods:
From January 2017 to January 2021, 121 patients with hypothyroidism were selected as the observation group. Meanwhile, 80 healthy individuals were selected as the control group. Serum 25(OH)D and Hcy levels, and left ventricular function indicators [left ventricular end diastolic diameter (LVEDD), left ventricular end systolic diameter (LVESD), left ventricular posterior wall thickness (LVPW), interventricular septal thickness (IVS), left ventricular ejection fraction (LVEF)] were tested. Pearson correlation analysis was performed to analyze the correlation of serum 25(OH)D and Hcy levels with LVEDD, LVESD, LVPW, IVS, and LVEF.
Results:
Compared with the control group, the observation group had lower serum 25(OH)D level, higher Hcy level, larger IVS and LVPW, and lower LVEF (P<0.05). There were no significant differences in LVEDD and LVESD between the two groups (P>0.05). Pearson correlation analysis showed that serum 25(OH)D level was negatively correlated with LVPW and IVS, and was positively correlated with LVEF (P<0.05). Serum Hcy level was positively correlated with LVPW and IVS, and was negatively correlated with LVEF (P<0.05).
Conclusion:
In patients with hypothyroidism, 25(OH)D level decreases and Hcy level increases, which aggravates myocardial damage and impaired left ventricular function.

The Relationship Between the Volume of Edema MMP-9 NSE and the Surgical Effect of Lateral Fissure Approach of Basal Ganglia Intracerebral Hemorrhage and Its Prognostic Value

Objective:
To explore the relationship between edema volume, matrix metalloproteinase-9 (MMP-9), neuron-specific enolase (NSE) and the curative effect of the lateral fissure approach of basal ganglia intracerebral hemorrhage, and analyze the prognostic value of each index.
Methods:
150 patients with basal ganglia cerebral hemorrhage admitted to our hospital from May 2019 to August 2020 were selected for surgical treatment of lateral fissure approach. According to the effect of surgical treatment, they were divided into effective group and ineffective group. The clinical data, relative perihematomal edema (rPHE) and serum MMP-9 and NSE levels before and after treatment were compared between the two groups. The Pearson correlation coefficient model was used to analyze the correlation between rPHE, serum MMP-9, NSE and the patient's neurological function (NHISS score), and the logistic regression model was used to analyze the factors affecting the surgical efficacy. Follow-up for 3 months after treatment, the prognosis of the patients was counted, and the levels of rPHE and serum MMP-9 and NSE in patients with different prognosis were compared after surgical treatment. The receiver operating characteristic (ROC) curve was used to analyze the prognostic value of rPHE, serum MMP-9, and NSE after surgical treatment.
Results:
The NIHSS score, hematoma volume, serum MMP-9 and NSE levels before and after treatment in the effective group were lower than those in the ineffective group, and rPHE before and after treatment was higher than that in the ineffective group (P<0.05). Before treatment, NHISS score of patients with basal ganglia intracerebral hemorrhage was negatively correlated with rPHE, and positively correlated with serum MMP-9 and NSE levels (P<0.05). Before treatment, NIHSS score, hematoma volume, rPHE, serum MMP-9, and NSE levels were all influencing factors for the curative effect of lateral fissure approach surgery in patients with basal ganglia intracerebral hemorrhage (P<0.05). The rPHE of patients with good prognosis after surgical treatment was higher than that of patients with poor prognosis, and the levels of serum MMP-9 and NSE were lower than those with poor prognosis (P<0.05). After surgical treatment, the area under the curve (AUC) of rPHE, serum MMP-9 and NSE predicting the prognosis of patients were all>0.7. Among them, the AUC predicted by serum NSE was the largest, which was 0.822. When the cut-off value was 20.38 μg/L, the prediction sensitivity and specificity were 76.67% and 83.33%, respectively.
Conclusion:
RPHE, serum MMP-9, NSE are significantly related to the curative effect of basal ganglia intracerebral hemorrhage with lateral fissure approach, and early postoperative indicators have good application value in predicting the prognosis of patients.

The Clinical Significance of Serum Carbohydrate Antigen 125 IL-6 and Squamous Cell Carcinoma Antigen Expression in Patients with Cervical Cancer

Objective:
To observe the expression levels of glucose antigen 125 (CA125), interleukin-6 (IL-6) and squamous cell carcinoma antigen (SCCA) in serum of patients with cervical cancer, uterine fibroids and healthy controls.
Methods:
Serum CA125, IL-6 and SCCA levels were detected in 96 patients with cervical cancer, 64 patients with uterine fibroids and 72 healthy controls, and the clinical significance of serum CA125, IL-6 and SCCA expression levels were analyzed.
Results:
There were significant differences in the expression levels of serum CA125, IL-6 and SCCA among the three groups (P<0.05). The serum levels of CA125, IL-6 and SCCA in cervical cancer patients were the highest (P<0.05). The serum levels of CA125 and IL were the highest in healthy control group (P<0.05). SCCA level was the lowest (P<0.05). The expression levels of CA125, IL-6 and SCCA in serum of highly differentiated, moderately differentiated and poorly differentiated cervical cancer patients were also different (P<0.05). The serum levels of CA125, IL-6 and SCCA in patients with low differentiation of cervical cancer were the highest (P<0.05), and the serum levels of CA125, IL-6 and SCCA in patients with high differentiation of cervical cancer were the lowest (P<0.05). There were significant differences in serum CA125, IL-6 and SCCA levels among cervical cancer patients with different pathological stages (P<0.05). The serum levels of CA125, IL-6 and SCCA in stage Ⅳ cervical cancer patients were the highest (P<0.05). There was no significant correlation between serum IL-6 concentration and age of patients with cervical cancer (r=0.096, P>0.05), but there was a positive correlation between tumor size and IL-6 concentration (r=0.697, P<0.05). Serum CA125 and SCCA were positively correlated with age and tumor size in patients with cervical cancer (P<0.05).
Conclusion:
CA125, IL-6 and SCCA play an important role in the induction, differentiation and development of cervical cancer. Detection of the expression levels of the above three genes in the serum of patients is helpful to judge and analyze the cervix in the process of monitoring the tumor characteristics and disease of patients.

Expressions and Clinical Significance of MiR-135b LZTS1 mRNA and HE4 mRNA in Ovarian Cancer

Objective:
To observe the expression level and clinical significance of microRNA-135b (miR-135b), leucine zipper tumor suppressor gene 1 (LZTS1) messenger RNA (mRNA), human epididymal protein 4 (HE4) mRNA in ovarian cancer.
Methods:
56 patients with ovarian cancer in our hospital from January 2018 to November 2020 were selected, and the ovarian cancer tissues and their corresponding adjacent tissues were collected. The expression levels of miR-135b, LZTS1 mRNA and HE4 mRNA in ovarian tissues of the two groups were detected by real time fluorescent quantitative PCR; the relationships between the expressions of miR-135b, LZTS1 mRNA, HE4 mRNA and the clinicopathological features of ovarian cancer were analyzed; and the diagnostic values of expression levels of miR-135b, LZTS1 mRNA and HE4 mRNA in ovarian cancer were analyzed.
Results:
The expression levels of miR-135b and HE4 mRNA in ovarian cancer group were significantly higher than those in adjacent group (P<0.05), and the expression level of LZTS1 mRNA was significantly lower than that in adjacent group (P<0.05); the expression levels of miR-135b, LZTS1 mRNA and HE4 mRNA in ovarian cancer were correlated with tumor differentiation, TNM stage and lymph node metastasis (P<0.05); the area under curve (AUC) of miR-135b, LZTS1 mRNA and HE4 mRNA in the diagnosis of ovarian cancer were 0.781, 0.755 and 0.773, respectively, the corresponding cutoff values were 1.25, 0.87 and 1.20, respectively, at this time, the corresponding sensitivities were 74.5%, 76.4% and 72.7%, and the specificities were 77.2%, 72.9% and 77.2%, respectively; the AUC of the combined diagnosis of ovarian cancer was 0.943, and the sensitivity and specificity were 90.9% and 84.2% respectively.
Conclusions:
MiR-135b and HE4 mRNA are highly expressed in ovarian cancer, LZTS1 mRNA is lowly expressed, miR-135b, LZTS1 mRNA and HE4 mRNA have certain diagnostic value for ovarian cancer, and the combination of them can effectively improve the diagnostic efficacy for ovarian cancer.

The Expression and Significance of TNF-α IL-6 and VEGF in Serum of Patients with Hypertensive Intracerebral Hemorrhage

Objective:
To observe and analyze the impact on the prognosis of the serum levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and vascular endothelial growth factor (VEGF) in patients with hypertensive cerebral hemorrhage (HICH).
Methods:
96 HICH patients admitted to Chengde Central Hospital from August 2018 to August 2020 were selected as the observation group, and 30 healthy controls during the same period were selected as the control group. At the same time, the observation group was subgrouped into 2 groups according to the amount of blood loss, 42 cases in the blood loss ≥30mL group, 54 cases in the <30mL group, and also subgrouped into 4 groups according to the prognosis status, 8 cases in the death group, 24 cases in the GCS score 3-8 group, 33 cases in the 9-12 group, 36 cases in GCS13-14 group. Enzyme-linked immunosorbent assay (ELISA) was used to detect serum TNF-α, IL-6 and VEGF levels in HICH patients and healthy subjects, and to analyze the changes in serum TNF-α, IL-6 and VEGF levels in HICH patients among different groups.
Results:
The levels of serum TNF-α, IL-6 and VEGF in the observation group were significantly higher than those in the control group (P<0.05). The levels of serum TNF-α, IL-6 and VEGF in the blood loss ≥30ml group were significantly higher than those in <30ml group (P<0.05), and the worse the prognosis, the higher the serum TNF-α, IL-6 and VEGF levels, and the changes in TNF-α, IL-6 and VEGF levels among the groups are statistically significant (P<0.05).
Conclusion:
The levels of TNF-α, IL-6 and VEGF in serum after hypertensive intracerebral hemorrhage may affect the prognosis of patients.

Clinical Diagnosis of Serum Amyloid A in Peritoneal Dialysis Associated Peritonitis

Objective:
To investigate the diagnostic values of serum amyloid A (SAA) in peritoneal dialysis (PD) related peritonitis, providing a theoretical basis for clinical diagnosis and treatment.
Methods:
In this study, 68 patients with PD related peritonitis were included. And 30 patients with maintenance PD without PD related peritonitis were selected as the control group. Serum was collected before and after diagnosis and treatment, respectively. Meanwhile, the curative effect was evaluated after treatment. The serum levels of SAA were measured by ELISA. Receiver operating characteristic curve (ROC) was used to evaluate diagnostic value of SAA for PD related infection.
Results:
There was no significant difference in clinical data between the two groups (all P>0.05). Compared with patients without PD related peritonitis, the serum SAA level in patients with PD related peritonitis was significantly increased (P<0.01). The sensitivity and specificity of SAA in the diagnosis of PD related peritonitis were 89.32% and 78.89%, respectively. For PD related peritonitis patients with atypical clinical symptoms, the serum SAA level was also significantly increased (P<0.01). Among 68 peritonitis patients included in this study, 54 cases were cured, 8 cases were persistent infection and 6 cases were failed. The serum SAA level in the cured group was significantly lower than that in the persistent infection group, but was higher than the normal level, while the SAA level in the failure group continued to be high. The difference was statistically significant (all P<0.05).
Conclusion:
Serum SAA level has very important value in the diagnosis and curative effect evaluation of PD related peritonitis.

Comparison of Postoperative Complications between Laparoscopic and Open Radical Gastrectomy Based on Propensity Score Matching Method

Objective:
To compare the postoperative complications between laparoscopic surgery (LG) and open surgery (OG) for gastric cancer and to evaluate the safety of laparoscopic surgery in patients with gastric cancer.
Methods:
From January 2019 to December 2019, 60 patients with gastric cancer underwent radical gastrectomy in Lu'an Hospital Affiliated to Anhui Medical University. Among them, 28 patients underwent laparoscopic subtotal gastrectomy or total gastrectomy, and 32 patients underwent open or laparoscopic subtotal gastrectomy or total gastrectomy, The baseline data were collected and the postoperative complications were analyzed by propensity score matching method.
Results:
The baseline data of the two groups were not statistically significant (P>0.05) after matching; The Clavien-Dindo grades before and after matching were statistically significant (P<0.05) between the two groups. The incidence of pleural infection and effusion before and after matching was greater than that of LG group (P<0.05) in the OG group; postoperative bleeding before and after matching showed that the incidence of OG group was greater than that of LG group (P<0.05); Visceral dysfunction before and after matching showed OG greater than LG group (P<0.05); There was no significant difference between the two groups before anastomotic fistula matching, LG group had a higher incidence than OG group after matching (P<0.05).
Conclusion:
LG has the advantage of less complication rate for radical gastrectomy than OG. It has good application prospect.

Meta Analysis of the Effect of Gestational Diabetes Mellitus on Neonatal Brain Injury

Objective:
To analyze the relationship between gestational diabetes mellitus and neonatal brain injury by meta analysis.
Methods:
The databases included CNKI, CBM, Wanfang, VIP, Cochrane Library, PubMed, web of science, EMBASE and other manual searches. The content is about the effect of gestational diabetes mellitus on neonatal brain injury. The date of literature search is March 12, 2021. The fixed (random) effect model was used to merge the results of the studies reported in the literature, and the odds ratio (or) and the corresponding 95% confidence interval (CI) were calculated. The causes of heterogeneity and analyze potential publication bias were explored.
Results:
A total of 421 articles were searched, and 13 articles were finally included in this analysis (2077 children), including 881 cases in the experimental group and 1196 cases in the control group. The included literatures are all case-control studies, and the NOS scale score results show that they are all high-quality literature. The Meta analysis showed heterogeneity across all studies (I

= 73%>50%,P<0.0001). The random effects model showed that the OR of influence of gestational diabetes mellitus on neonatal brain injury with an OR of 21.91(95%CI:8.98-53.50). The Begg test and Egger's test showed that there was no publication bias in the included literature (P>0.05) .
Conclusion:
Gestational diabetes mellitus increases the risk of brain ingury in newborns.

Correlation between Serum IGF-I and HGF Levels and Prognosis of Patients with Hypofractionated Radiotherapy after Gliomas Surgery

Objective:
To analyze the relationship between the expression of serum insulin-like growth factor 1 (IGF-1) and hepatocyte growth factor (HGF) and prognosis (mortality) of patients with hypofractionated radiotherapy after gliomas surgery.
Methods:
53 patients with gliomas who underwent low-dose radiotherapy after operation and received surgery in our hospital from January 2016 to January 2017 were selected. And all patients received low-dose radiotherapy 3~4 weeks after operation, 1.8~2.0 Gy/time, a total of 54~60 Gy. According to the prognosis (death from illness) within 3 years after radiotherapy, the patients were divided into a poor prognosis group and a good prognosis group, investigated the general data of the two groups, tested and compared laboratory indicators, the relationship between serum IGF-1 and HGF expression and prognosis of patients was mainly analyzed.
Results:
All 53 patients were followed up for 3 years after radiotherapy, 25 were died of illness, and the mortality rate was 47.17%; the serum VEGF, TGF-β, IGF-1 and HGF levels in the poor prognosis patients were higher than the good group (P<0.05); The results of regression analysis showed that VEGF, TGF-β, IGF-1 and HGF were all the risk factors of poor prognosis in patients with hypofractionated radiotherapy after gliomas (OR>1, P<0.05); the ROC curve was drawn,VEGF, TGF-β, IGF-1 and HGF levels alone and in combination predicted the AUC of poor prognosis in patients with low segment radiotherapy after operation were all >0.80, the best prediction value could be obtained when the cut-off value of each index were 221.885pg/ml, 42.205ng/l, 111.080ng/ml and 898.495 μg/l; the correlation analysis showed that there was a positive correlation between serum IGF-I and HGF level in patients with low segment radiotherapy after gliomas (r>0, P<0.05).
Conclusion:
The overexpression of serum IGF-I and HGF before hypofractionated radiotherapy is associated with poor prognosis after radiotherapy in patients with glioma after operation.

Effect of Bronchoalveolar Lavage with Fiberoptic Bronchoscopy on Cytomegalovirus Pneumonia in Children

Objective:
To investigate the effect of bronchoalveolar lavage with fiberoptic bronchoscope on cytomegalovirus (CMV) pneumonia.
Methods:
150 cases of CMV pneumonia in our hospital from January 2014 to December 2019 were divided into the control group and the observation group according to the order of admission. The control group was treated with conventional treatment, and the observation group was treated with bronchofiberscope alveolar lavage on the basis of the control group. The sputum bacterial clearance rate, inflammatory factors and curative effect were compared between the two groups before and after treatment.
Results:
The fever time and hospitalization time of the observation group were shorter than those of the control group, and the score of pulmonary infection after treatment was lower than that of the control group, the difference was statistically significant (P<0.05). The levels of interleukin-6 (IL-6), high-sensitivity C-reactive protein (hs-CRP) and tumor necrosis factor α (TNF-α) in the observation group after treatment were lower than those in the control group, the difference was statistically significan (P<0.05). There were no serious complications in the observation group.
Conclusion:
Bronchoalveolar lavage with fiberoptic bronchoscope can improve the positive rate of sputum bacterial culture in children with CMV pneumonia, guide early precise treatment, significantly reduce airway hyperresponsiveness, accelerate bacterial clearance, improve the condition and shorten the course of disease.

An Analysis of the Relationship Between the Concentration of Contrast Agent and the Quality of Portal Vein CT Images

Objective:
To explore the relationship between different contrast agent concentrations and the quality of CT images of the portal vein of patients.
Methods:
A total of 90 patients who underwent CTPV in our hospital from December 2019 to December 2020 were selected. According to the random number method, the patients were divided into an observation group and a control group, 45 cases in each. The observation group was a high-concentration contrast agent group, using 350 mg /ml concentration of iodine contrast agent; the control group was a low concentration contrast agent group, using 300mg/ml concentration of iodine contrast agent. The patients were subjected to portal vein CT imaging, and the CT value, signal-to-noise ratio, imaging quality and occurrence of adverse reactions were analyzed in the two groups .
Results:
The CT values of the main portal vein, the left portal vein and the right portal vein in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05); the SNR and CNR signal-to-noise ratios of the observation group were different from those in the control group. There was no significant difference (P>0.05); the observation group had more cases with higher image scores than the control group, and the difference between the two groups was statistically significant (P<0.05); the incidence of adverse reactions in the observation group was 11.1%, slightly higher, compared with 6.7% of the control group, the difference is not statistically significant (P>0.05).
Conclusion:
The use of high-concentration contrast agent for CTPV imaging can effectively improve the imaging quality of the image, and will not increase the incidence of adverse reactions, which is worthy of clinical promotion.

The Effect of Mannitol Application at Different Times on the Prognosis of Patients with Cerebral Hemorrhage and an Analysis of Related Factors

Objective:
To explore the influence of mannitol application at different times on the prognosis of patients with cerebral hemorrhage and to analyze the related factors.
Methods:
The clinical data of 80 patients with primary cerebral hemorrhage admitted between January 2017 and December 2020 were retrospectively analyzed. Among them, 40 patients were treated with mannitol 6 hours before admission and were included in group A, and 40 patients who were treated with mannitol 6 hours after admission were included to group B. The prognoses of the two groups of patients were compared before they were divided into a good prognosis group (58 cases) and a poor prognosis group (22 cases). Univariate analysis and multivariate logistic regression were used to analyze the prognostic factors of patients with cerebral hemorrhage.
Results:
The prognosis of patients in group B was better than that in group A, and the difference was statistically significant (P<0.05). Multivariate logistic regression analysis showed that advanced age, long time from onset to admission, high NIHSS score at admission, high GLU level, and bleeding volume were mostly independent risk factors for poor prognosis in patients with cerebral hemorrhage (P<0.05), while GCS score at admission was high. CT image showed that the midline did not shift, did not break into the ventricle, and the application of mannitol 6 hours after admission was an independent protective factor for the good prognosis of patients with cerebral hemorrhage (P<0.05).
Conclusion:
Patients with cerebral hemorrhage treated with mannitol 6 hours after admission can get a better prognosis, and age, onset to admission, GCS score, NIHSS score, blood sugar, bleeding volume, midline structure shift, break into the ventricle, and mannitol application timing are related to the prognosis of patients with cerebral hemorrhage. In clinical treatment, timely and effective treatment should be carried out according to the above related factors to improve the prognosis of patients.

Effect of Prophylactic Ileostomy Distal Mucosal Closure on Anastomotic Fistula After Laparoscopic Radical Resection of Rectal Cancer and Its Related Factors

Objective:
To investigate the effect of prophylactic ileostomy distal mucosal closure on the occurrence of anastomotic fistula after laparoscopic radical resection of rectal cancer and its related factors.
Methods:
A retrospective analysis of the clinical data of 75 patients with rectal cancer admitted to our hospital from May 2019 to May 2020 was conducted. According to their surgical methods, they were divided into groups A (39 cases) and group B (36 cases). Group A was in laparoscopic rectal cancer. After radical cancer surgery, preventive ileostomy distal mucosal closure was performed. And group B received conventional laparoscopic radical rectal cancer surgery. The length of hospital stay, gastrointestinal function recovery time, anastomotic leakage healing time and postoperative occurrence were compared between the two groups, as well as the incision infection, intestinal adhesions and anastomotic leakage. According to whether an anastomotic leakage occurred after the operation, patients were subgrouped into anastomotic leakage group (10 cases) and non-anastomotic leakage group (65 cases). According to the results of univariate analysis, meaningful indicators were included into Logistic regression.
Result:
① The postoperative hospital stay, gastrointestinal function recovery time and anastomotic fistula healing time in group A were lower than those in group B, P<0.05; ② The incidence of postoperative incision infection (5.13%), intestinal adhesion (7.69%) and anastomotic fistula (5.13%) in group A were significantly lower than those in group B (19.44%, 25.00%, 22.22%),P<0.05;③ Logistic regression showed that distance from tumor to anal margin was an independent risk factor for anastomotic fistula, P<0.05.
Conclusion:
Prophylaxis of distal mucosal closure of ileostomy after laparoscopic radical resection of rectal cancer has significant clinical effect on patients with rectal cancer, which can effectively reduce the probability of anastomotic fistula. The distance between tumor and anal margin is an independent risk factor for anastomotic fistula, which requires more clinical attention and targeted nursing care for patients.

A Comparative Study on the Clinical Efficacy of Laparoscopic Hysterectomy in the Supine Position and the Head-Low-Feet-High Position

Objective:
To observe the influence of different postures (flat position and head-low-foot-high position) on the clinical efficacy of patients undergoing laparoscopic hysterectomy.
Methods:
Prospective analysis was conducted between April 2019 and March 2021 on 86 patients undergoing laparoscopic total hysterectomy, who were divided into research group and control group strictly with the random number table method. 43 cases in the control group undergoing laparoscopic total hysterectomy had traditional head-low-feet-high position, and 43 cases in the research group had the supine position. Postoperatively, the two groups were compared on the comfort level (comfort level was evaluated according to grade distribution), postoperative complications and patients' satisfaction with body position placement.
Results:
The total incidence of postoperative complications in the study group (4.65%) was significantly lower than that in the control group (23.26%), the difference was statistically significant (χ

=7.889, P<0.05).The distribution of postoperative comfort level in the study group was significantly better than that in the control group, the difference was statistically significant (Z=9.350, P<0.05). Relationship Between Serum NCAM TrkB BDNF Expression Levels and Clinical Symptoms in Patients with Schizophrenia

Objective:
To detect the expression of neural cell adhesion molecule (NCAM), tyrosine kinase receptor B (TrkB) and brain-derived neurotrophic factor (BDNF) in patients with schizophrenia, and to explore the correlation between their expression levels and clinical symptoms.
Methods:
Seventy schizophrenic patients from August 2018 to August 2020 were selected as the schizophrenia group, at the same time, 70 healthy people who had routine physical examination in the physical examination center of our hospital were selected as the control group. Enzyme linked immunosorbent assay (ELISA) was used to detect the expression levels of NCAM, TrkB and BDNF in serum. The positive and negative syndrome scale (PANSS) was used to evaluate the clinical symptoms. Pearson method was used to analyze the correlation between serum NCAM, TrkB, BDNF expression levels and PANSS score; and Logistic regression analysis was used to analyze the related factors of schizophrenia; receiver operating characteristic curve (ROC) was used to evaluate the diagnostic value of serum NCAM, TrkB and BDNF levels in patients with schizophrenia.
Results:
The serum levels of NCAM, TrkB and BDNF in schizophrenia group were significantly lower than those in control group (P<0.05). The total score of PANSS was (78.67 ± 7.51), and the positive symptom score was (29.38 ± 4.25), and the negative symptom score was (26.72 ± 5.96), and the general pathological symptom score was (22.57 ± 6.20). Serum NCAM, TrkB and BDNF levels were negatively correlated with total score and positive symptom score of PANSS (P<0.05). The low expression of NCAM, TrkB and BDNF was an independent risk factor of schizophrenia (P<0.05). The AUC of serum NCAM, TrkB and BDNF in the diagnosis of schizophrenia was 0.863, 0.788 and 0.885 respectively, and the cut-off value was 47.773 μg/mL, 42.727 ng/mL and 29.487 pg/mL, respectively, the corresponding sensitivity was 67.1%, 65.7%, 84.3%, and the specificity was 87.1%, 78.6% and 74.3%, respectively.
Conclusion:
The expression levels of NCAM, TrkB and BDNF in the serum of schizophrenics are significantly reduced, and are negatively related to the severity of clinical symptoms, and may participate in the occurrence and development of schizophrenia.

CT Perfusion Imaging Evaluates the Curative Effect of NP Chemotherapy Combined with Brucea Javanica Oil Emulsion in the Treatment of Non-Small Cell Lung Cancer

Objective:
To evaluate the clinical application value of NP chemotherapy combined with Brucea javanica oil emulsion in the treatment of non-small cell lung cancer using CT perfusion imaging technology.
Methods:
The imaging data of 40 patients with non-small cell lung cancer admitted to the First Affiliated Hospital of Hebei North University from January 2017 to December 2018 were retrospectively analyzed. According to the treatment plan, they were divided into observation group (n=20) and control group ( n=20). The control group was treated with conventional NP chemotherapy during 3 consecutive treatment cycles, 21 days in a cycle. The observation group was given intravenous drip of Brucea javanica oil emulsion 30ml and 0.9% sodium chloride injection 250ml in the same perion, 2 times/day, 28 days in a cycle, for 3 consecutive cycles of treatment. TOSHIBA Aquilion One 320-row CT scanner was used for conventional lung CT scan and Target lesion perfusion scan before and after 3 cycles of treatment, and data were recorded, including the changes in lesion size before and after treatment, perfusion parameter values: blood flow (BF), blood volume (BV), mean transit time (MTT), capillary permeability (PS) between groups.
Results:
After 3 cycles of treatment, the remission rate of the observation group was 60% (12/20), and the remission rate of the control group was 45% (9/20). The remission rate of the observation group was higher than that of the control group (60.0%∶45.0%). The difference was not statistically significant (χ
2
=0.902, P=0.342); after treatment, the BV and BF levels of the two groups decreased, and the observation group was lower than the control group (P<0.05), the MTT levels of the two groups increased (P<0.05); PS levels in the two groups decreased after treatment, and the observation group was lower than the control group, but the difference was not statistically significant (P>0.05).
Conclusion:
Brucea javanica oil emulsion is safe and effective in adjuvant treatment of lung cancer. It can inhibit the proliferation of tumor cells, improve the microcirculation of tumor tissues, and improve the immune function of the body. CT perfusion imaging can sensitively reflect the hemodynamic changes of lung cancer lesions, which is the curative effect of lung cancer. The evaluation provides objective functional imaging basis.

An Observation of the Therapeutic Effect of Repeated Transcranial MagneticStimulation Guided by Event-Related Potentials on Children with Autism Spectrum Disorder

Objective:
To analyze the clinical efficacy of high-low frequency repetitive transcranial magnetic stimulation (rTMS) under the guidance of event-related potential (ERP) combined with comprehensive rehabilitation training in the treatment of children with autism spectrum disorder (ASD).
Methods:
60 ASD children who visited the fifth affiliated hospital of Zhengzhou University from January 2018 to February 2019 were selected and randomly divided into the experimental group (n=30) and the control group (n=30). Both the two groups received comprehensive rehabilitation treatment. On this basis, the control group were treated with conventional rTMS treatment, while the experimental group were treated with high-frequency and low-frequency rTMS treatment in the abnormal brain areas according to the guidance of ERP. The children’s autism rating scale (CARS), autistic children’s behavior checklist (ABC), and autism treatment evaluation scale (ATEC) were used to compare the efficacy of the two groups after treatment.
Results:
Before treatment, there were no significant differences in CARS, ABC and ATEC scores between the two groups(P>0.05). After 3 months of treatment, the CARS score of the experimental group (34.63±1.94) was significantly lower than that of the control group (35.73±1.96, P<0.05), the total score of ABC (50.10±4.55) was significantly lower than that of control group(54.53±5.43, P<0.05), and the ATEC score (53.53±4.70) was significantly lower than that of the control group (59.07±5.95, P<0.05). After 6 months of treatment, the CARS score of the experimental group (33.87±2.49) was significantly lower than that of the control group (31.50±2.05, P<0.05), the total score of ABC (50.00±5.13) was significantly lower than that of control group (40.01±4.86, P<0.05), and the ATEC score (47.00±5.08) was significantly lower than that of the control group (54.93±6.70, P<0.05).
Conclusion:
The rTMS treatment plan under the guidance of ERP has a more significant improvement effect in the treatment of ASD than the conventional rTMS, which can provide a certain reference for the treatment of children with ASD by rTMS.

An Analysis of Allergen Spectrum and Expression of IL-25 IL-32 EOS in Adenoid Tissue in Children with Adenoid Hypertrophy and Allergic Rhinitis and the Diagnostic Value

Objective:
To analyze the allergen spectrum of children with adenoid hypertrophy and allergic rhinitis and the expression of IL-25, IL-3 and EOS in adenoid tissues and their diagnostic value.
Methods:
96 children with allergic rhinitis were selected as the clinical data of the study for retrospective analysis. The study period was from May 2017 to May 2020. Patients with adenoid hypertrophy and allergic rhinitis were selected according to the 1:1 individual matching principle. 48 Cases make up group A and 48 cases of patients with simple allergic rhinitis make up group B. In addition, 48 healthy subjects were selected as the control group, and the allergen spectrum of children with allergic rhinitis was observed, the IL-25, IL-32, and EOS of the three groups of subjects were evaluated, and the adenoid hypertrophy and degeneration were analyzed. The independent influencing factors of patients with atopic rhinitis, the correlation between IL25, IL32 and EOS, and the AUC value of IL25, IL32, EOS and the combination of the three.
Results:
96 children with allergic rhinitis were tested for serum allergen total IgE, 94 cases (97.92%) were positive; 42 cases (44.68%) were allergic to only one allergen, and were allergic to more than two kinds of allergens. There were 52 cases (55.32%) of primary allergies. Among the inhaled allergens, the positive rate of house dust mites was the highest, and the positive rate of dog dander was the lowest; among the inhaled allergens, the positive rate of chicken protein was the highest, and the positive rate of fish, crabs and shrimps was the lowest. The levels of IL-25, IL-32 and EOS in group A and B were higher than those in the control group (all P<0.05); the levels of IL-25, IL-32 and EOS in group A were higher than those in group B (all P<0.05). The binary logistic regression analysis showed that IL-25, IL-32 and EOS were all independent influencing factors in patients with adenoid hypertrophy and allergic rhinitis (P<0.05). Pearson correlation analysis showed that IL25, IL32 and EOS were positively correlated (r=0.742/0.871, P<0.05). ROC curve analysis showed that the AUC values of IL-25, IL-32, EOS, and the three items combined to predict adenoid hypertrophy with allergic rhinitis were (0.855, 0.977, 0.996, 0.994, P<0.05); the sensitivity was respectively Are (72.90%, 95.80%, 93.80%, 100.00%), specificity is (91.70%, 93.70%, 100.00%, 91.70%).
Conclusion:
IL-25, IL-32 and EOS are abnormally expressed in children with adenoid hypertrophy and allergic rhinitis, and they have a certain guiding role in clinical diagnosis.

The Effect of Abdominal Catheterization in the Treatment of Refractory Ascites and Analysis of Influencing Factors of Tube Falling

Objective:
To investigate the efficacy of abdominal indwelling central venous catheter in the treatment of refractory ascites, and to analyze the influencing factors of tube falling.
Methods:
A total of 96 cirrhotic with refractory ascites patients who received abdominal indwelling central venous catheter drainage ascites from January 2015 to January 2019 were retrospectively analyzed. The efficacy of central venous catheter in the treatment of refractory ascites were evaluated. Then, the patients were divided into a tube falling group and non- tube falling group, and influencing factors about tube fell out were analyzed.
Results:
The total effective rate of abdominal distension after indwelling central venous catheter was 90.63%. There were no significant differences in white blood cells, platelets, alanine aminotransferase, aspartate aminotransferase , total bilirubin, indirect bilirubin, creatinine, prothrombin time, and procalcitonin before and after tube placement (P>0.05). Albumin was higher than that before tube placement (P<0.05). The incidence of adverse events was 22.92%. Multivariate logistic regression analysis found that the catheter suture fixation as a protective factor for tube falling(OR=0.336).Ascites extravasation (OR=3.961), ascites volume (OR=1.732), catheter indwelling time (OR=3.219) were the risk factors of tube falling.
Conclusion:
Abdominal indwelling central venous catheter can effectively relieve the symptoms of abdominal distension in patients with refractory ascites due to cirrhosis, and does not affect the blood routine, liver and kidney function, and blood coagulation function. Catheter suture fixation, ascites extravasation, ascites volume, catheter indwelling time are influencing factors of tube fell out.

The Effect of Transparent Cap-Assisted Endoscopic Internal Hemorrhoid Sclerosis on Patients with II and III Degree Internal Hemorrhoids and Its Influence on Vascular Morphology

Objective:
To explore the effect of transparent cap-assisted endoscopic internal hemorrhoid sclerosis on patients with degree II and III internal hemorrhoids and its influence on blood vessel morphology.
Methods:
All 80 patients with grade II and III internal hemorrhoids were admitted between February 2020 and February 2021. They were randomly divided into 2 groups with 40 cases in each group. The control group was treated with closed hemorrhoidectomy, and the observation group was treated with transparent hemorrhoids. Cap-assisted endoscopic treatment of internal hemorrhoids sclerosing, and compare the visual analog pain (VAS) score, Likert score, anorectal dynamics index, perianal venous plexus area (S), diameter (D) and Various surgical indicators, postoperative complication rate.
Results:
The intraoperative blood loss and hospitalization expenses in the observation group were less than those in the control group (P<0.05), and the hospitalization time and operation time were shorter than those in the control group (P<0.05). The VAS score, Likert score, and postoperative 24h and 48h The incidence of complications was lower than that of the control group (P<0.05). The ARP and RRP at 24h and 48h after operation were higher than those of the control group (P<0.05). The postoperative S and D indexes were better than those of the control group (P<0.05).
Conclusion:
The transparent cap-assisted endoscopic internal hemorrhoid sclerotherapy has a significant effect on patients with II and III degree internal hemorrhoids, which can reduce hemorrhoids, improve anorectal dynamics, reduce body pain, and promote postoperative recovery.

Effect of Hemodialysis Combined with Hemoperfusion on Oxidative Stress and Islet Function in Patients with DN-ESRD

Objective:
To analyze the effects of hemodialysis combined with hemoperfusion on oxidative stress markers and islet function in patients with diabetic end-stage renal disease (DN-ESRD).
Methods:
Totally 80 patients with diabetic end-stage renal disease were randomly divided into two groups. 40 patients with HD were made the study group, and 40 patients with HD combined with HP were selected as the control group. The renal function markers [blood creatinine (SCr), urea nitrogen (BUN) and cystic filtration rate (GFR)], oxidative stress markers [reactive oxygen species (ROS), superoxide dismigase (SOD), propylene duedehyde (MDA)] were measured by ELISA Kit before and after treatment in the two groups. The islet function related indicators [insulin resistance index (HOMA - β), insulin resistance index (HOMA-IR) and insulin sensitivity index (ISI)] were calculated by homeostasis model.
Results:
Before treatment, there was no differences in renal function markers, oxidative stress markers, and islet function between groups (P>0.05). After treatment, the SCr, BUN, ROS, MDA and HOMA-IR levels were reduced in both groups, the GFR, SOD, HOMA-β and ISI levels were increased in both groups, and the change of the study group was higher than the control group, with statistically significant differences (P<0.05).
Conclusion:
Hemodialysis combined blood irrigation play a better role in reducing oxidative stress damage and improving islet function than hemodialysis alone, which is more effective in improving patients' kidney function.

Diagnostic Value of Color Doppler Ultrasound Combined with Serum Creatine Kinase and HCG in Placental Implantation

Objective:
To analyze the diagnostic value of three-dimensional color Doppler flow imaging (CDFI) combined with serum creatine kinase (CK) and human chorionic gonadotropin (HCG) in prenatal placental implantation.
Methods:
195 pregnant women with high risk factors of placental implantation and underwent cesarean section in the Affiliated Hospital of Chengde Medical University from 2015 to 2017 were selected as the study objects. According to the pathological diagnosis results of placenta after delivery, pregnant women were divided into placental implantation group (n = 86) and non fetal disc implantation group (n = 109). CK and HCG in serum were detected by automatic biochemical analyzer; CDFI was used to analyze the placental implantation of the two groups; the gold standard was the pathological diagnosis of placenta after delivery, and the diagnostic values of serum CK, CHG and CDFI single and combined diagnoses in prenatal placental implantation were analyzed.
Results:
The levels of serum CK and HCG in placenta group were higher than those in non placenta group (P<0.05). The results of CDFI showed that the rates of characteristic appearance such as fuzzy boundary between placenta and myometrium, placental parenchymal lacunar blood flow signal, placenta highly vascularized, dark area of irregular fluid property in placenta, and bladder protuberance of pregnant women in placenta implantation group were significantly higher than those in non placenta implantation group (P<0.05). The sensitivities of serum CK, CHG and CDFI single diagnoses were 46.51%, 48.84% and 67.44% respectively, and the specificities were 82.41%, 80.66% and 83.07% respectively. The sensitivity and specificity of CDF1 combined with CK and CHG were 94.85% and 87.62% respectively.
Conclusions:
Detection of DFI combined with serum CK and HCG can improve the sensitivity and accuracy of prenatal placental implantation diagnosis, which has the potential to become the diagnostic standard of prenatal placental implantation.

Effect of Closing the Dura Mater after Standard Large Bone Flap Decompression on Serum S100β and NSE Levels and the Incidence of Postoperative Complications in Patients with Severe Head Injury

Objective:
To investigate the effect of closing the dura mater after standard large bone flap decompression on serum S100β, neuron specific enolase (NSE) levels and the incidence of postoperative complications in patients with severe head injury (SHI).
Methods:
A total of 80 SHI patients in our hospital from February 2019 to June 2020 were selected and divided into study group (n=41) and control group (n=39) according to different treatment plans. In study group, the dura mater was closed after standard large bone flap decompression, while in control group, it was not closed. The related indexes, complications, serum S100β and NSE levels before operation, 3 d and 7 d after operation were compared, as well as the neurological function score (NIHSS) and glasgow coma scaley score (GCS) before operation, 2 weeks and 1 month after operation, the cognitive function score (MMSE), ability of daily living score (BI), and quality of life score (GQOLI-74) before operation, 3 months and 6 months after operation between the two groups. The prognosis of 6 months after operation was calculated.
Results:
The length of stay in ICU of study group was shorter than that of control group (P<0.05). The serum S100β and NSE levels of study group were lower than those of control group at 3 and 7 days after operation (P<0.05). The NIHSS score of study group was lower than that of control group at 2 weeks and 1 month after operation, and the GCS score of study group was higher than that of control group (P<0.05). The scores of MMSE, BI and GQOLI-74 in study group were higher than those in control group at 3 months and 6 months after operation (P<0.05). The postoperative complication rate of study group was 12.20% lower than that of 30.77% in control group (P<0.05). The prognosis of study group was better than that of control group at 6 months (P<0.05).
Conclusion:
Closing the dura mater after decompressing the standard large bone flap in SHI patients can reduce serum S100β and NSE levels, reduce complications, improve cognitive function and daily activities, improve quality of life, and have important brain protection effects.

An Observation of the Clinical Effect of External Counterpulsation on Chest Pain in Menopausal Women with Positive Treadmill Exercise Test

Objective:
To observe the clinical effect of external counterpulsation on the chest pain of menopausal women with positive exercise test.
Methods:
Sixty-four patients with chest pain admitted from January 2020 to January 2021 were included in this study, all in menopause, and the results of treadmill exercise test were positive. They were divided into two groups by digital random method, with 32 patients in each group. The control group was treated with conventional drug therapy, and the observation group was treated with external counterpulsation.
Results:
The total effective rate of observation group was 96.9%, which was significantly higher than that of control group (78.1%, P<0.05). Exercising treadmill test data comparison before treatment has no statistical significance (P>0.05). Compared with before treatment, ST segment of the observation group has decreaseed amplitude, ST segment is prolonged and exercise tolerance is elevated (P<0.05), while decreaseed amplitude of ST segment in the control group is statistically significant compared with before treatment (P<0.05). After treatment, the above indexes in observation group were significantly better than those in control group (P<0.05). There were NO significant differences in NO, VEGF and SOD scores between the two groups before treatment (P>0.05). After treatment, NO and SOD levels in the two groups were higher and VEGF levels were lower than before treatment (P<0.05). After treatment, the level of VEGF in observation group was lower than that in control group, and the levels of NO and SOD were higher than those in control group (P<0.05).
Conclusion:
The clinical effect of ECP in the treatment of menopausal women with positive treadmill exercise test for chest pain is significant, which is worthy of promotion.

Curative Effect of Minimally Invasive Nasal Endoscopic Surgery on Sinusitis and Its Influences on Serum ECP HMGB1 IL-1β and IL-21 Levels

Objective:
To explore the curative effect of minimally invasive nasal endoscopic surgery on sinusitis and its influences on levels of serum eosinophile cationic protein (ECP), high mobility group protein B1 (HMGB1), interleukin-1β (IL-1β) and interleukin-21 (IL-21).
Methods:
A total of 163 sinusitis patients treated in the hospital from January 2019 to October 2020 were enrolled and divided into observation group (81 cases) and control group (82 cases) by random number table method. The control group was treated with microwave thermo-coagulation, while observation group was treated with minimally invasive nasal endoscopic surgery. Both groups were followed up for 3 months. The clinical curative effect after 3 months of treatment between the two groups was compared. Before and after treatment, subjective symptoms of sinusitis were evaluated by visual analogue scale (VAS). The morphology of nasal mucosa was evaluated by Lund-Kennedy endoscope scoring method. And their quality of life was assessed by sinonasal outcometest-20 (SNOT-20). The levels of ECP, HMGB1, IL-1β and IL-21 before and after treatment between the two groups were compared.
Results:
After treatment, the total response rate was higher in observation group than control group (92.59% vs 81.71%) (P<0.05). After treatment, scores of VAS (runny nose, nasal congestion, cheek pain, headache, hyposmia) in observation group were (2.03±0.52) points, (2.15±0.58) points, (1.24±0.37) points, (0.96±0.32) points and (1.25±0.31) points, lower than those in control group [(3.45±0.84) points, (3.21±0.74) points, (2.08±0.56) points, (1.36±0.41) points, (2.62±0.48) points] (P<0.05). After treatment, scores of Lund-Kennedy and SNOT-20 in observation group were (1.51±0.42) points and (2.08±0.62) points, lower than those in control group [(2.52±0.67) points, (3.61±0.86) points] (P<0.05). After treatment, levels of ECP, HMGB1, IL-1β and IL-21 in observation group were (4.23±1.08) ng/L, (64.25±6.13) μg/L, (30.25±4.36) pg/mL and (72.16±12.34) pg/mL, lower than those in control group [(5.46±1.31) ng/L, (72.18±7.29) μg/L, (38.47±5.19) pg/mL, (86.25±14.17) pg/mL] (P<0.05).
Conclusion:
Curative effect of minimally invasive nasal endoscopic surgery is good on sinusitis, which can effectively improve clinical symptoms of patients, reduce allergic reactions and inflammation level, and improve their quality of life.

The Predictive Value of CK AFP and β-hCG in Maternal Serum for Placental Implantation

Objective:
Based on the levels of CK, AFP and β-hCG in the maternal serum, the value of predicting placenta accreta was explored.
Methods:
50 cases of puerpera in the obstetrics department of Chengde Central Hospital from 2017 to 2019 who were pathologically diagnosed as placenta accreta were selected as the experimental group. 52 cases of non-placental implant women were randomly selected as the control group. The maternal serum CK, AFP and β-hCG levels were measured in each group, and the datas were analyzed by spss23.0 statistical software.
Results:
The levels of the three types of indicators in the experimental group were significantly higher than those in the control group. The datas were compared and the differences were statistically significant; and the three types of indicators had a certain predictive value for placental accreta, and the combination of the three indicators has higher predictive value for placenta accreta, with sensitivity and specificity of 70% and 81% respectively.The cut off values of serum CK, AFP and β-hCG for placenta accreta is 153.14U/mL, 193.23U/mL and 21909U/L.
Conclusion:
The combination of CK,AFP and β-hCG in maternal serum can effectively predict the placental accreta of pregnant women before delivery, and the predictive value is relatively high.

Correlation Analysis between Levels of Homocysteine Apolipoprotein A/Apolipoprotein B and Coagulation Indexes in Patients with Chronic Obstructive Pulmonary Disease

Objective:
To analyze the correlation between level of serum homocysteine (Hcy), apolipoprotein A1 (Apo A1)/apolipoprotein B100 (Apo B100) and blood coagulation indexes in patients with chronic obstructive pulmonary disease (COPD).
Methods:
The 188 COPD patients admitted to our hospital from Jan 2017 to Dec 2019 were divided into venous thromboembolism (VTE) group (n=92) and non-VTE group (n=96), and the general information of the two groups were compared. Other items of the two groups were compared too, including serum Hcy, Apo A1, Apo B100, Apo A1/Apo B100 levels and coagulation indexes such as activated partial thromboplastin time (APTT), fibrinogen (FIB), prothrombin time (PT), plasma viscosity (PV), D-dimer (DD) and antithrombin III (AT-III) activity. Then correlation analysis between serum Hcy, Apo A1, Apo B100, Apo A1/Apo B100 levels and coagulation indexes of COPD patients were performed.
Results:
The serum levels of Hcy, Apo B100, FIB, PV and DD in the VTE group were significantly higher than those in the non-VTE group (P<0.05 or P<0.01). The serum levels of Apo A1, Apo A1/Apo B100 and AT-III in the VTE group was significantly lower than those in the non-VTE group (P<0.05 or P<0.01). The levels of Hcy and Apo B100 in the serum of patients with COPD are significantly positively correlated with FIB, PV, and DD and other coagulation indicators (P<0.001), and significantly negatively correlated with AT-III (P<0.001); while Apo A1 and Apo A1/Apo B100 level is significantly negatively correlated with FIB, PV, DD and other coagulation indicators (P<0.001), and there is a significant positive correlation with AT-III (P<0.001).
Conclusion:
Serum Hcy, Apo A1, Apo B100, Apo A1/Apo B100 levels in COPD patients are correlated with the occurrence of VTE. Clinical monitoring of serum Hcy, Apo A1 and Apo B100 levels is of great value in evaluating the occurrence of VTE in COPD patients.

Effect of Brucea Javanica Oil Emulsion Injection Combined with NP Chemotherapy on Immune Function Coagulation and Pulmonary Function in Patients with Lung Cancer

Objective:
To explore the effects of gemcitabine or docetaxel combined with cisplatin chemotherapy in the treatment of non-small cell lung cancer (NSCLC) on the blood coagulation function, soluble fragments of cytokeratin 19 (CYFRA21-1), and carbohydrate antigen 199 (CA199).
Methods:
We retrospectively analyzed the case data of 104 patients with advanced NSCLC who underwent radiotherapy in the oncology department of our hospital from January 2019 to January 2020. According to the chemotherapy regimen, NSCLC was divided into 2 groups, GP group (gemcitabine combined with cisplatin) of 54 cases, and DP group (docetaxel combined with cisplatin) of 50 cases. Comparison of the effects of chemotherapy was conducted between the two groups concerning the coagulation function before and after chemotherapy [thrombin time (TT), activated partial thromboplastin time (APTT), platelets (PLT), prothrombin time (PT), fibrinogen (FIB), plasma D -Dimer (DD)], CYFRA21-1, CA199 levels, and toxic side effects.
Results:
The tumor control rates of GP group and DP group were 83.33% and 78.00%, respectively, and the difference was not statistically significant (P>0.05). Repeated measures analysis of variance showed that the difference between FIB before and after treatment was statistically significant (P<0.05), but the difference between groups was not statistically significant (P>0.05); the interaction between time and treatment has an effect on TT and PLT Statistically significant (P<0.05), the difference between TT and PLT before and after treatment in the GP group was greater than that in the DP group. Repeated measures analysis of variance showed that CYFRA21-1 and CA199 were different before and after treatment (P<0.05), but there was no difference between different treatment methods, and the interaction between treatment methods and time was not statistically significant (P>0.05) ). The incidence of abnormal liver function and thrombocytopenia in the GP group was higher than that in the DP group, and the incidence of leukopenia and muscle soreness in the GP group was lower than that in the control group. The difference was statistically significant (P<0.05); The incidence of grade III-IV thrombocytopenia was higher than that of the DP group, and the incidence of grade I-IV leukopenia was lower than that of the control group, and the difference was statistically significant (P<0.05). In addition, most of the toxic and side effects are grade Ⅰ to II, which can basically be relieved by themselves, while grades III to IV are obviously relieved after symptomatic treatment.
Conclusion:
GP and DP chemotherapy regimens have similar effects in the treatment of NSCLC, and have little difference in the level of CYFRA21-1 and CA199. However, the effect of GP chemotherapy in improving blood hypercoagulability is more significant than that of DP chemotherapy, and the side effects of GP chemotherapy are thrombocytopenia Mainly, the DP chemotherapy regimen mainly focuses on leukopenia.

Effect of Jiawei Self-made Bushen Tiaojing Decoction on Polycystic Ovary Syndrome and Its Influence on Levels of Serum AMH LH and FSH

Objective:
To analyze the treatment effect of self-made Bushen Tiaojing decoction on polycystic ovary syndrome (PCOS).
Methods:
A total of 140 patients with PCOS (kidney deficiency and phlegm stasis syndrome) who came to our hospital between March 2018 and February 2020 were selected as the research subjects, and they were randomly divided into observation group and control group according to the random number table method, with 70 cases in each group. Control group was given ethinylestradiol and cyproterone acetate tablets and conventional western medicine treatment, and observation group was given Jiawei self-made Bushen Tiaojing decoction on the basis of control group. The TCM syndromes scores, uterine-related indexes [endometrial thickness, ovarian volume, cervical mucus score (Insler cervical scoring method)], serum related factors [serum anti-Mullerian hormone (AMH), luteinizing hormone (LH), follicle stimulating hormone (FSH)] and occurrence of adverse reactions were compared between the two groups of patients before treatment and at 6 months of treatment. The ovulation rate and natural pregnancy rate were recorded in the two groups of patients during 1-year follow-up.
Results:
The scores of symptoms of irregular menstruation, soreness and weakness of waist and knees, fatigue, abnormal leukorrhea and dysmenorrhea in the two groups of patients after 6 months of treatment were significantly lower than those before treatment, and the scores of observation group were significantly lower than those of control group (all P<0.05). And the differences of symptoms scores before and after treatment in observation group were significantly higher than those in control group (all P<0.05). The endometrial thickness and cervical mucus score of the two groups of patients at 6 months of treatment were significantly increased compared with those before treatment while the ovarian volume was significantly decreased compared with that before treatment, and there were statistically significant differences between observation group and control group (all P<0.05), and the differences of indicators changes before and after treatment were significantly different between the two groups (all P<0.05). At 6 months of treatment, the levels of serum AMH, LH and FSH of the two groups were significantly reduced compared with those before treatment, and the levels of AMH and LH in observation group were significantly lower than those in control group (all P<0.05). There was no significant difference in FSH level between the two groups (P>0.05), and there were statistical significance in the differences of indicators before and after treatment between the two groups (all P<0.05). There were no significant differences in the occurrence of adverse reactions such as dizziness, nausea and mild abdominal pain between the two groups of patients during treatment (P>0.05). At 1-year follow-up, the ovulation rate and natural pregnancy rate of observation group were significantly higher than those of control group (all P<0.05).
Conclusion:
Jiawei Bushen Tiaojing decoction in the treatment of PCOS (kidney deficiency and phlegm stasis syndrome) can effectively improve the symptoms of irregular menstruation and fatigue as well as ovarian function, and it is conducive to smooth ovulation and promotion of natural pregnancy. It has a definite efficacy and good safety.
