1 Mechanism of MicroRNA-193a Suppressing the Tumorigenesis of Hepatocellular Carcinoma Through Targeting WT1

Objective: To elucidate the function of miR-193a in HCC. Methods: The expression levels of miR-193a and Wilms' tumor-1 (WT1) were detected by quantitative real Time-polymerase chain Reaction (qRT-PCR) assay. In addition, miR-193a expression was also investigated in MHCC97H,HCCLM3,Huh7 and LO2 cell lines. Moreover, the HCC proliferation was examined through cell counting Kit-8 (CCK-8) assay after miR-193a overexpression or inhibition. Tumor growth of HCC was observed via xenograft tumor formation assay. The relationship between miR-193a and WT1 was verified by the dual-Luciferase assay. Results: Downregulation of miR-193a whereas upregulation of WT1 were identified in HCC, which predicted a worse prognosis in HCC patients. Meanwhile, miR-193a was significantly decreased in LO2 cell lines compared with MHCC97H,HCCLM3,Huh7 cell lines. Low miR-193a expression HCC patients showed severer TMN stage and presence of lymph node metastasis. Furthermore, it was found that miR-193a mimic suppressed cell proliferation by inhibiting WT1. miR-193a also inhibited tumor growth of HCC. Conclusion: miR-193a suppressed tumorigenesis of HCC by targeting WT1.

5 Effects of Isoflurane on the Formation of Collateral Circulation and Notch Pathway in Rats with Cerebral Ischemia-reperfusion

Objective: To investigate the effects of isoflurane on the formation of collateral circulation and Notch pathway in rats with cerebral ischemia-reperfusion. Methods: 90 clean grade SD rats were random divided into control group, model group, edaravone group (3 mg/kg), isoflurane low-dose group (50 μg/kg), isoflurane high-dose group (100 μg/kg), 18 in each group. The rats in the model group, edaravone group, isoflurane low-dose group and high-dose group were established cerebral ischemia-reperfusion model. After successful modeling, the rats in the edaravone group, isoflurane low-dose group and high-dose group on the first day after operation were intraperitoneally administrated corresponding drugs, once a day, for 1 week. The rats in the control group and the model group were given the same volume of saline. After the experiment, the microvessel density and neuromotor function scores in the reimplantation area of the rat brain were measured, and the results of the bilateral sticker removal and balance beam crossing experiment were obtained. The neuron structure in the reimplantation area of the rat brain injury was observed. And real-time fluorescence quantitative reversal Record-polymerase chain reaction (RT-PCR) and Western-blot (Western-blot) method to measure Notch1, vascular metabolic growth factor (VEGF) mRNA and protein levels. Results: The neuron cells in the control group were intact; a large number of necrotic neurons were seen in the hippocampus of the model group, the nerve cell nucleus was contracted, and there was significant inflammatory cell infiltration; the neuronal cell structure in the edaravone group and the isoflurane high-dose group was relatively complete, and the inflammatory cell infiltration Reduced; significant infiltration of inflammatory cells is still visible in the low-dose isoflurane group. Compared with the control group, the microvascular density in cerebral ischemia-reperfusion area, Notch1, VEGF mRNA and protein levels in the model group were decreased, and the neuromotor function score, the removal time of bilateral stickers and the time of passing the balance beam increased (P<0.05). Compared with the model group, microvascular density in cerebral ischemia-reperfusion area, Notch1, VEGF mRNA and protein levels in the edaravone group, the low-dose isoflurane group and the high-dose isoflurane group were increased, and the neuromotor function score, the removal time of bilateral stickers and the time of passing the balance beamdecreased (P<0.05). The microvascular density in cerebral ischemia-reperfusion area, Notch1, VEGF mRNA and protein levels of high dose isoflurane group were higher than those of low dose isoflurane group, and the neuromotor function score, the removal time of bilateral stickers and the time of passing the balance beam lower than those of low dose isoflurane group (P<0.05). There were no significant changes in microvascular density in cerebral ischemia-reperfusion area, Notch1, VEGF mRNA and protein levels, neuromotor function score, bilateral Decal removal time and balance beam passing time of rats between the edaravone group and high dose isoflurane group (P>0.05). Conclusion: Isoflurane can promote the formation of collateral circulation in rats with cerebral ischemia-reperfusion, and its mechanism may be related to that isoflurane can promote the expression of Notch1, VEGF mRNA and protein in rats with cerebral ischemia-reperfusion and then activate Notch signaling pathway.

11 Effects of Statins on Blood Lipid IL-6 TNF- α and Blood Viscosity After Emergency Percutaneous Coronary Intervention in Patients with AMI

Objective: To investigate the effects of statins on blood lipid, IL-6,TNF- α and blood viscosity after emergency percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (AMI). Methods: A total of 100 patients with AMI treated in our hospital from December 2017 to August 2018 were randomly divided into two groups: control group (n = 50) and trial group (n = 50).The patients in both groups were treated with PCI and corresponding drug therapy. The blood lipid, IL-6,TNF- α and blood viscosity were measured and compared before and after treatment with rishovastatin in the groups. Results: After treatment, the level of IL-6,TNF- α in the two groups was lower than that before treatment, and that in the trial group was more obvious than that in the control group(P<0.05).The levels of blood lipid (HDL-C,LDL-C,TC,TG) were significantly improved in the treatment group than in the control group (P<0.05).After treatment, NBH and NBL in the two groups were lower than those before treatment, especially in the test group(P<0.05). Conclusion: After emergency PCI in patients with AMI, risuvastatin could significantly inhibit the level of IL-6,TNF- α and blood viscosity, and effectively improve the level of blood lipid.

15 Effects of Diosgenin on Treg Cell in Spleen of CIA Mice

Objective: To evaluate the effect of diosgenin on regulatory Treg cells in spleen of collagen induced arthritis (CIA) mice. Methods: The CIA mice model was established by immunizing DBA1/J mice with chicken type II collagen and Freund’s completed adjuvant. And CD4+CD25+Treg cells in spleen of CIA model mice were isolated by MACS. Then, to detect the purity of Treg cells by flow cytometry and culture them in vitro. 48 hours after treatment, the proportion of Treg cells was detected by flow cytometry, the expression of transcription factor Foxp3 in Treg cells was detected by real-time fluorescence quantitative PCR, and the levels of IL-10 were detected by ELISA. Results: CIA model mice were established successfully. The purity of Treg cells isolated by MACS was more than 90%. Compared with the control group, the percentage of Treg cells, the Foxp3 mRNA level, and the expression of IL-10 in cell supernatant were increased significantly (P<0.05) 48 hours after treatment.Conclusion: Diosgenin can significantly increase the proportion of Treg cells, the levels of transcription factor Foxp3 and IL-10 in CIA mice.

18 Effect of Dexmedetomidine on Stress Response Hemodynamics and Immune Function in Patients Undergoing Spinal Anesthesia

Objective: To ivestigate the effect of dexmedetomidine on stress response in patients undergoing spinal anaesthesia. Methods: A total of 192 patients who underwent spinal anesthesia in our hospital from November 2015 to September 2018 were selected retrospectively as the study subjects. According to the random number table method, they were simply randomly divided into two groups, 96 patients in each group. Control group was treated with propofol, and research group was treated with dexmedetomidine. Stress response indexes (NE, E, Cor) were compared before and after surgery. The two groups were observed of postoperative hemodynamic indexes (MAP, HR, SpO2) change. The levels of T cell subsets (CD3+, CD4+, CD8+, CD4+ / CD8+) was compared, and adverse reactions occurred in both groups were recorded. Results: Before surgery, there was no significant difference in NE, E, Cor levels between the two groups (P>0.05). After operation, the levels of NE, E and Cor in the study group were lower than those in the control group (P<0.05).MAP and HR levels in the study group were lower than those in the control group, and SpO2 levels were higher than those in the control group (P<0.05).There was no significant difference in CD3+, CD4+, CD8+ and CD4+/CD8+(P>0.05). After operation, CD3+, CD4+, CD8+ and CD4+/CD8+ were significantly higher in the study group than in the control group(P<0.05).The incidence of adverse reactions was 7.29% in the study group and 21.88% in the control group, with statistically significant differences (χ2=12.335, P=0.002). Conclusion: Dexmedetomidine, applied in spinal anesthesia, can effectively reduce the stress response of patients, and plays an important role in improving the immune function and reducing the adverse reactions of patients.

23 Effects of Propofol on AMPK Autophagy Pathway and Cognitive Function in Alzheimer's Rats

Objective: To investigate the effect of propofol on adenosine monophosphate-activated protein kinase (AMPK) autophagy pathway and cognitive function in Alzheimer's rats. Methods: 100 clean grade SD rats were random divided into 5 groups: control group, model group, minocycline hydrochloride group (50 mg/kg), propofol low and high dose group (50, 100mg/kg). Model group, minocycline hydrochloride group, propofol low and high dose group were hippocampus injected into Aβ1-42 to establish Alzheimer's disease model. After the model was established successfully, the rats in the minocycline hydrochloride group, propofol low and high dose group were given corresponding drugs by gavage for 4 weeks, 1 times a day. The control group and the model group were gavaged the same volume of normal saline. At the end of the experiment, Morris water maze test was used to measure the rats nerve function, HE staining to observe the pathological structure of hippocampal neurons in rats, and acridine orange staining was used to measure the rats autophagy of hippocampal neurons, and RT-PCR and Western blot were used to measure the AMPK, N-methyl-D-aspartate receptor subunit 2B(NR2B) mRNA and protein levels in hippocampal tissue of rats. Results: In the control group, there were more neuron cells in the hippocampus and they were arranged neatly; in the model group, the number of neurons in the hippocampal area was decreased and the pyknosis was obvious; the number of necrotic neurons decreased in low dose propofol group; in minocycline hydrochloride group and propofol high dose group, a small number of necrotic neurons were found in the hippocampus, and the structure of neurons was complete. Compared with the control group, the escape latency time, AMPK, NR2B mRNA and protein expression levels in the model group were significantly increased, and the number of times of passing through the original platform position, the time of staying in the original platform quadrant, and the level of autophagy bodies were significantly decreased(P<0.05). Compared with the model group, the escape latency time, AMPK, NR2B mRNA and protein expression levels in the minocycline hydrochloride group and the propofol low and high dose group were significantly decreased, and the number of times of passing through the original platform position, the time of staying in the original platform quadrant, and the level of autophagy bodies were significantly increased (P<0.05). Compared with minocycline hydrochloride group, the escape latency time, AMPK, NR2B mRNA and protein expression level of propofol low-dose group were significantly increased, and the number of times of passing through the original platform position, the time of staying in the original platform quadrant, the level of autophagy bodies of neurons were significantly decreased (P<0.05). The escape latency time, AMPK, NR2B mRNA and protein expression, the number of times of passing through the original platform position, the time of staying in the original platform quadrant, and the level of autophagy bodies were similar between the minocycline hydrochloride group and propofol high dose group (P>0.05). Conclusion: Propofol can increase the autophagy level of neurons in Alzheimer's rats and then significantly reduce the degree of Hippocampus neuron injury. The mechanism is related to the inhibition of AMPK signaling pathway by propofol.

28 Effects of Dexmedetomidine on Recovery Quality and Serum IL-1β and TNF- α Levels in Resuscitation of Sevoflurane Inhalation Anesthesia

Objective: To investigate the effect of dexmedetomidine on recovery quality and serum IL-1β and TNF-α levels in resuscitation stage of sevoflurane inhalation anesthesia . Methods: A total of 108 patients underwent laparoscopic cholecystectomy in our hospital from March 2016 to March 2020 were selected and randomly divided into two groups based on the random number table method, with 54 patients in each group. In the study group, dexmedetomidine was continuously pumped after sevoflurane inhalation, while in the control group, constant dose of normal saline was continuously pumped. Perioperative indicators, incidence of agitation and serum inflammatory factors (IL-1β, TNF-α) were compared. Results: There were no statistically significant differences in intraoperative infusion volume, recovery time of spontaneous breathing, tracheal extubation time or awakening time between the two groups (P>0.05).The generalized equation analysis showed that the incidence of agitation in waking period in the study group was lower than that in the control group at different time points (Waldχ2group=4.097, Waldχ2time point=30.388, Pgroup=0.043, Ptime point<0.001). Results of repeated measurement variance showed that VAS score, serum TNF- and IL-1 levels were significantly different between the two groups at different time points (P<0.05); The VAS score, serum TNF-α and IL-1β of the study group were significantly lower than those of the control group (P<0.05). Conclusion: Dexmedetomidine in sevoflurane inhalation anesthesia can effectively improve the postoperative recovery quality of patients, reduce restlessness of patients during convalescence, and has an important role in the reduction of inflammatory response in patients.

33 Observation of Different Target Concentration Propofol Combined with Remifentanil and Sevoflurane Combined with Remifentanil for LC

Objective: To investigate the effects of propofol of different target concentrations combined with remifentanil, and sevoflurane combined with remifentanil inhalation anesthesia in patients undergoing laparoscopic cholecystectomy. Methods: A total of 100 patients with laparoscopic resection admitted to our hospital from March 2019 to September 2019 were selected and divided into observation group and control group with random number table, with 50 cases in each group. The control group received propofol combined with remifentanil anesthesia, and the observation group received sevoflurane combined with remifentanil inhalation anesthesia. The effects of anesthesia and hemodynamics parameters and adverse reactions were compared between the two groups. Results: In the observation group, MAP and HR levels in T1 and T2 stagesn were significantly higher than those of the control group (all P<0.05), and MAP and HR levels in T4 and T5 stages were significantly lower than those in the control group (all P<0.05). While at stages T2 and T5, SpO2 of the observation group was significantly lower than that of the control group (all P<0.05). The recovery time of spontaneous breathing, eye opening time, speech response time and directional power recovery time of patients in the observation group were significantly shorter than those in the control group (all P<0.05). Conclusion: Clinical use of sevoflurane remifentanil intravenous anesthesia in LC surgery can significantly maintain hemodynamic stability, shorten the time of spontaneous breathing recovery, eye-opening, speech response and orientation recovery. It has good clinical application value.

37 Analysis of the Autoregulation of Cerebral Blood Flow in Sleep Disorders Accompanied by Anxiety and Depression

Objective: To explore the automatic regulation of cerebral blood flow in patients with sleep disorders accompanied by anxiety and depression. Methods: A total of 65 patients with sleep disorders accompanied by anxiety and depression admitted to our hospital from 2017 to December 2019 were selected as the observation group, and 70 healthy controls were made the control group. The cerebral blood flow autoregulation function, anxiety and depression scores of the two groups were compared and analyzed, and the correlation between the Pearman Hamilton Rating Scale for Anxiety (HAMA) score, Hamilton Depression Rating Scale(HAMD) score and the cerebral blood flow autoregulation function parameters was used. Results: The HAMA score and HAMD score of the observation group were significantly higher than those of the control group, and there was a statistical difference between the two (P<0.05). Calculated by the blood flow analyzer curve, the left and right hemisphere phases of the observation group were significantly lower than those of the control group, and there was a statistical difference between the two (P<0.05). Using the correlation between Pearman's HAMA score and the parameters of cerebral blood flow autoregulation, it was found that there was no significant correlation between the HAMA score and the HAMD score of the observation group's bilateral cerebral hemisphere phase difference and gain (P>0.05). Conclusion: The cerebral blood flow autoregulation function of patients with sleep disorders accompanied by anxiety and depression is significantly reduced, but the degree of impairment of its regulatory function is not significantly related to the patient's anxiety and depression. The HAMA scale and HAMD scale cannot be used to evaluate the brain in clinical practice. The degree of damage to the automatic regulation of blood flow.

41 Effect of Early Stage Rehabilitation Exercise on PRWE Score Swelling Score and Wrist Function After Plaster Fixation of Distal Radius Fractures

Objective: To study the effect of early stage rehabilitation exercise on PRWE score, swelling score and wrist function after plaster fixation of distal radius fractures. Methods: A total of 426 patients with distal radius fracture hospitalized from August 2016 to February 2019 were selected and divided into control group and study group with the simple random number table, 213 cases in each group. Patients in the control group had regular exercise after plaster fixation, and patients in the study group had early stage rehabilitation exercise. After 7 days, the difference of swelling score between the two groups was observed. After 3 months, the difference of PRWE score and wrist function between the two groups was observed. Results: Analysis of variance of repeated measurements showed that there was significant difference between the two groups in the interaction effect of swelling score (P<0.05). There was no significant difference between swelling scores of the two groups before treatment (P>0.05); after 7 days of treatment, the swelling score of the study group was improved more significantly (P<0.05). There were significant differences in the interaction of time points, groups, time point and group in the two groups (P<0.05). There was no significant difference between the two groups in swelling score and PRWE score before treatment (P>0.05), and the PRWE score of the study group was higher 3 months after treatment (P<0.05). The results of rank sum test showed that there was no significant difference between the two groups in the four grades of subjective activity, kneading force and grip strength (P>0.05), but the total subjective activity, kneading force and grip strength of the patients in the study group were significantly higher than those in the control group (P<0.05). Conclusion: Early rehabilitation exercise has definite effect on distal radius fractures after plaster fixation, which is helpful to relieve postoperative swelling, improve PRWE score, and speed up the recovery of wrist function. However, attention should be paid to the control of training frequency and degree during exercise.

45 Expressions of Serum MiR-214 and Galectin-3 in Elderly Patients with Chronic Heart Failure of Coronary Heart Disease and Prognostic Correlation

Objective: To detect the levels of serum microRNA-214 (miR-214) and galectin-3 in elderly patients with coronary heart disease and chronic heart failure, and to explore the correlation between their levels and prognosis. Methods: From July 2017 to April 2019, 72 elderly patients with chronic heart failure of coronary heart disease in the Department of Cardiology of our hospital were selected as heart failure group; at the same time, 84 healthy people were selected as the control group; in addition, according to whether cardiovascular events occurred within 12 months after discharge, patients with chronic heart failure were divided into cardiovascular event group (23 cases) and non cardiovascular event group (49 cases). The level of serum miR-214 was detected by real-time fluorescent quantitative PCR (qRT-PCR); the levels of serum galectin-3 and N-terminal pro-B-type natriuretic peptide (NT-proBNP) were detected by enzyme linked immunosorbent assay (ELISA); left ventricular ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDD) and left ventricular end systolic diameter (LVESD) were measured by echocardiography; the correlation between serum miR-214, galectin-3 and cardiac function indexes in patients with chronic heart failure was analyzed; and the factors influencing the prognosis of patients with chronic heart failure were analyzed. Results: MiR-214, galectin-3, LVESD, LVEDD and NT-proBNP in HF group were higher than those in the control group, and LVEF was lower than that in the control group (P<0.05); the levels of serum miR-214 and galectin-3 were positively correlated with LVESD, LVEDD and NT-proBNP (P<0.05), and negatively correlated with LVEF (P<0.05); miR-214, galectin-3, LVESD, LVEDD and NT-proBNP in cardiovascular event group were higher than those in non cardiovascular event group, and LVEF was lower than those in non cardiovascular event group (P<0.05); miR-214 and galectin-3 were independent risk factors of adverse cardiovascular events in patients with chronic heart failure (P<0.05), and LVEF was the protective factor of adverse cardiovascular events in patients with chronic heart failure (P<0.05). Conclusion: The levels of serum miR-214 and galectin-3 may play an important role in predicting the prognosis of elderly patients with chronic heart failure of coronary heart disease.

50 Study on Effect of CIAPIN1 on Proliferation and Metastasis of Bladder Urothelial Carcinoma by Inhibiting ERK1/2 Pathway

Objective: To explore the role and possible mechanism of CIAPIN1 in bladder urothelial carcinoma. Methods: Bladder urothelial cancer cell lines J82, T24, UM-UC-3 and human bladder epithelial immortalized cells SV-HUC-1 were used as the research objects. qRT-PCR was used to detect the expression level of CIAPIN1 mRNA in the cells; Western blot was used to detect the expression levels of CIAPIN1 p-ERK1/2 and ERK1/2 protein in cells; 5-ethynyl-2'-deoxyuridine (EdU) labeling test was used to detect cell proliferation; cell scratch test was used to detect cell migration; Transwell test was used to detect cell invasion. Results: Compared with SV-HUC-1 group, the expression levels of CIAPIN1 mRNA and protein in cells of T24, UM-UC-3 and J82 groups were significantly increased (P<0.05). Cells in T24 group had the most obvious changes, so T24 cells were selected for subsequent experiments. Compared with the NC group, the number of EdU-positive cells, cell migration distance, and invasive cells in the si-CIAPIN1 group were significantly reduced (P<0.05), and protein expression level of p-ERK1/2/ERK1/2 was significantly increased (P<0.05) ; Compared with the si-CIAPIN1 group, the number of EdU positive cells, migration distance, and number of invasive cells in the si-CIAPIN1+ U0126 group were significantly increased (P<0.05), and the protein expression level of p-ERK1/2 / ERK1/2 was significantly reduced ( P<0.05). Conclusion: CIAPIN1 promotes the proliferation and metastasis of bladder urothelial cancer cells by inhibiting ERK1/2 pathway phosphorylation.

56 Correlation Analysis of Peripheral Blood Lymphocyte Count Monocyte Count Lymphocyte to Monocyte Ratio and Clinical Data and Prognosis of Patients with Multiple Myeloma

Objective: To explore the correlations among peripheral blood absolute lymphocyte count (ALC), absolute monocyte count (AMC), lymphocyte to monocyte ratio (LMR) and clinical data and prognosis in patients with multiple myeloma (MM). Methods: A total of 115 patients with newly diagnosed MM who were treated in the hospital from March 2016 to August 2019 were selected as the research subjects. Data such as ALC, AMC and LMR were collected, and the overall survival (OS) was statistically analyzed. Results: ① The peripheral blood ALC of the 115 patients with MM was (0.22~13.48)×109L-1, with an average of (1.77±0.45)×109L-1. The peripheral blood AMC was (0.01~0.98)×109L-1, with an average of (0.45±0.12) ×109L-1, and LMR was 1.18~12.74, with an average of 4.74±0.85. ② With median values of peripheral blood ALC, AMC and LMR as cut-off values, the patients were grouped. Differences between high ALC group and low ALC group, high AMC group and low AMC group in terms of hemoglobin (Hb), lactate dehydrogenase (LDH), β2-microspheres (β2-MG) were statistically significant (P<0.05). Differences in Hb and β2-MG between high LMR group and low LMR group were statistically significant (P<0.05). ③ Kaplan-Meier survival curves showed that the overall OS of high ALC group, low AMC group and high LMR group was significantly longer than that of low ALC group, high AMC group, and low LMR group (P<0.05). ④ Cox multivariate analysis showed that DS stage III, LDH>245U/L, Hb≤110g/L, ALC≤0.65×109L-1 and LMR≤3.85 were independent risk factors for poor prognosis of patients with initial treatment of MM (P<0.05). Conclusion: ALC, AMC and LMR of patients with MM are related to the clinical data and prognosis. Clinically, the results can be used to initially evaluate and predict the prognosis and guide clinical treatment.

61 Predictive Value of Fractional Flow Reserve Combined with Serum ESM-1 Level for Cardiovascular Events and In-Stent Restenosisin Patients with AMI After PCI

Objective: To explore predictive value of fractional flow reserve (FFR) combined with serum endothelial cell specific molecule 1 (ESM-1) level for cardiovascular events (CE) and in-stent restenosis (ISR) in patients with acute myocardial infarction (AMI) after percutaneous coronary intervention (PCI). Methods: The influencing factors of CE and ISR in the 182 AMI patients after PCI, and the predictive value of FFR combined with ESM-1 for CE and ISR were retrospectively analyzed. Results: ESM-1 in CE group and ISR group was higher than that in non-cardiovascular event (NCE) group and non-ISR (NISR) group, while FFR was lower than that in NCE group and NISR group, respectively (P<0.05). AUC values of FFR combined with ESM-1 for predicting CE and ISR were 0.874 and 0.884, which were higher than those of single index (P<0.05). ESM-1 was an independent risk factor of CE, and FFR was an independent protective factor of ISR within 1 year after PCI in AMI patients (P<0.05). Conclusion: ESM-1 was an independent risk factor of CE and ISR within 1 year after PCI in AMI patients, while FFR was an independent protective factor. The predictive value of their combination detection for CE and ISR is relatively higher.

66 Expression and Clinical Significance of CircHIPK3 in Non-Small Cell Lung Cancer

Objective: Circular RNA plays an important role in the occurrence and development of various human diseases. By detecting the expression of circHIPK3 in non-small cell lung cancer (NSCLC) and its correlation with clinicopathological characteristics and prognosis, the value of circHIPK3 as a biomarker of NSCLC for clinical diagnosis and decision-making was evaluated. Method: Using qRT-PCR to detect the expression level of circHIPK3 in 54 NSCLC cancer tissues and corresponding normal adjacent tissues to cancer, combined with clinical datas, the clinical value of circHIPK3 as a biomarker for diagnosis and prognosis of NSCLC was analyzed. Results: The expression level of circHIPK3 in cancer tissues of NSCLC patients was significantly increased compared with normal adjacent tissues adjacent to the cancer (P<0.001); the plasma circHIPK3 level of patients was significantly increased compared with the expression levels of healthy people (P<0.001). The expression of circHIPK3 in cancer tissues is related to tumor tissue differentiation, clinical stage, and lymph node metastasis. Receiver work curve analysis showed that circHIPK3 has relatively high diagnostic value.The area under the ROC curve of NSCLC cancer tissue was 0.890, and the area under the plasma ROC curve of NSCLC patients was 0.829. Kaplan-Meier analysis showed that circHIPK3 is closely related to the prognosis of patients, and patients with high expression have a lower survival rate(χ2=3.657,P=0.026). Cox multivariate regression analysis showed that high expression of circHIPK3 is an independent risk factor for poor prognosis of NSCLC. Conclusion: CircHIPK3 may be a potential molecular biomarker and therapeutic target for the diagnosis and prognosis of NSCLC.

71 Accurate Application of PDCA Management Tools to Reduce the Incidence of Major Nosocomial Infections in Abdominal Emergency Patients

Objective: To monitor the specific data and classification of acquired infections in abdominal emergency surgery, to evaluate the main risk factors of hospital infections of abdominal emergency surgery, to improve the quality of target monitoring, and to formulate reliable preventive measures targeted to effectively reduce the occurrence of hospital infections. Methods: Through effective monitoring and analysis of the clinical data collected in 2016, the main types of infections found were analyzed by single-factor and multi-factor analysis to find the risk factors of infection, and PDCA management was carried out.The task was expected to be completed in 2019. Statistics of relevant data were conducted through SPSS19.0 to confirm the effectiveness of PDCA. Results: In 2016, the incidence of general hospital infections was 7.91%, with surgical site infection (SSI) as the main type of infection, accounting for 3.6%. Through the collection and analysis of clinical data, it was found that the susceptibility factors of SSI are: The incidence of SSI with NNIS score ≥ 3 is higher than that with NNIS score <3; the difference was statistically significant (χ2=12.687, P<0.05). The incidence of SSI with irrational use of antimicrobials before surgery is higher than that of SSI with reasonable use of antimicrobials before surgery; the difference was statistically significant (χ2=9.092, P<0.05). The incidence of SSI with operation time ≥3h is higher than that with operation time <3h; the difference was statistically significant (χ2=9.087, P<0.05). Through the precise application of PDCA management, the incidence of SSI in 2019 dropped significantly from 3.6% to 1.33% (χ2=3.565, P<0.05).Conclusion: NNIS score ≥3, and irrational use of antimicrobials before surgery can be risk factors for infection of the surgical site. Effective PDCA management for the main risk factors mentioned above can effectively reduce the incidence of SSI.

75 Clinical Study on the Effect of Early Weight Bearing on the Rehabilitation of Achilles Tendon Rupture After Operation

Objective: To investigate the effect of early weight bearing on the recovery of acute closed Achilles tendon rupture. Methods: From January 2018 to March 2019, 40 patients with closed Achilles tendon rupture admitted to the First People's Hospital of Huainan City were randomly divided into two groups: 20 patients in early weight bearing group (18 males), age (41.20 ± 8.13); 20 patients in the control group (19 males), age (44.40 ± 10.03). Ankle extension and flexion exercise was started in the 3rd week in both groups. The two groups were operated by the same doctors. Both groups were followed up for 12 months. The postoperative complications, r-ATRA and ATRs were compared between the two groups. Results: Delayed healing: 1 case in control group and 2cases in the weight bearing group; and rerupture : 1 case in the control group. There was no significant difference between the two groups (P > 0.05). The Achilles tendon in both groups healed in 12 months. On the second day after operation, r-ATRA was as follows: weight bearing group[- 9.50 ± 6.51 (n = 20)], control group [- 11.6 ± 6.41 (n = 20)], there was no significant difference between the two groups (P > 0.05); on the second month after operation, r-ATRA was as follows: weight bearing group [9.28 ± 7.68 (n = 20)], control group [4.83 ± 11.58 (n = 19)], there was no significant difference between the two groups (P > 0.05); ATRs score of the second month after operation and one year after operation were as follows: weight bearing group [53.35 ± 9.94 (n = 20)], [78.84 ± 5.72 (n = 19)], control group [45.47 ± 10.57 (n = 19)], [72.83 ± 4.05 (n = 18)] . The difference is clear.(P < 0.05).Conclusion: Early weight bearing after Achilles tendon rupture does not increase the rate of Achilles tendon extension and postoperative complications, and it can improve the satisfaction of patients and is conducive to the recovery of limb function.

79 Adverse Effects of Different Methods of Cataract Surgery on Postoperative Endophthalmitis and Prognosis of Elderly Patients

Objective: To explore the adverse effects of different methods of cataract surgery on postoperative endophthalmitis and prognosis of elderly patients. Methods: From May 2017 to May 2019, 1636 patients (1645 eyes) with cataract treated in our hospital were selected as the research objects. According to the operation methods, they were divided into phacoemulsification group of 828 cases (843 eyes) and extracapsular enucleation group of 808 cases (802 eyes). Retrospective analysis was used to compare the clinical characteristics of cataract patients, and logistic regression was used to analyze the multi-factor influence of postoperative endophthalmitis in cataract patients. Results: There was no significant difference between the phacoemulsification group and the extracapsular enucleation group in the clinical indicators of gender, hypertension, diabetes, preoperative vision, and intraocular pressure (P>0.05). The age, diabetes, operation method, vitreous overflow, operation time, and incision length of patients with intraocular infection were significantly different (P<0.05). The age of cataract patients (≥65 years), diabetes, extracapsular resection, vitreous overflow, operation time (≥15min), incision length (≥5mm) are all risk factors for postoperative endophthalmitis in cataract patients (P<0.05 ). Conclusion: Both surgical methods can improve the intraocular pressure and visual acuity of cataract patients, but the overall effect of phacoemulsification is better than traditional surgical methods, which greatly improves the incidence of postoperative endophthalmitis.

84 Expression Relationship and Significance of CDKN3 and FOXA1 mRNA in Rectal Cancer

Objective: To investigate the expressions and clinical significance of cyclin dependent kinase inhibitor 3 (CDKN3) and forked head box A1 (FOXA1) mRNA in rectal cancer. Methods: 94 rectal cancer patients in Tangshan Union Hospital and the 82nd Army Hospital of the PLA from November 2017 to March 2020 were used as the observation group, and 95 healthy people who had routine physical examination in our hospital in the same period as the control group; the expressions of CDKN3 and FOXA1 mRNA in serum of patients with rectal cancer were detected by real-time fluorescent quantitative PCR (QRT PCR); the expression levels of CDKN3 and FOXA1 mRNA in serum of patients with high and low grade rectal cancer were analyzed; the relationships between the levels of CDKN3 and FOXA1 mRNA and the clinicopathological characteristics of rectal cancer patients were analyzed; and the diagnostic values of CDKN3 and FOXA1 mRNA levels in patients with high and low rectal cancer were predicted. Results: The expression levels of CDKN3 and FOXA1 mRNA in the observation group were significantly higher than those in the control group (P<0.05); the expressions of CDKN3 and FOXA1 mRNA in the serum of patients with high rectal cancer were significantly higher than those of patients with low rectal cancer (P<0.05); in the serum of patients with rectal cancer the expression levels of CDKN3 and FOXA1 mRNA were related to TNM stage, differentiation degree and lymph node metastasis (P<0.05); the area under ROC curve of CDKN3 mRNA in the diagnosis of high and low grade rectal cancer was 0.839, the cutoff value was 5.317, the sensitivity was 63.8%, and the specificity was 94.4%; the area under ROC curve of FOXA1 mRNA in the diagnosis of high and low grade rectal cancer was 0.853 , the cutoff value was 0.740, the sensitivity was 70.7%, and the specificity was 97.2%. Conclusions: The expressions of CDKN3 and FOXA1 mRNA in serum of patients with rectal cancer are increased, in addition, the expressions of them in patients with high-grade rectal cancer are significantly higher than those in patients with low-grade rectal cancer, CDKN3 and FOXA1 mRNA are involved in the process and development of rectal cancer, and have certain values in the diagnosis of high and low grade rectal cancer.

89 The Effect of Interferon α-2b on the Treatment of Respiratory Syncytial Virus Bronchiolitis in Children and Its Effect on Serum SP-D TGF-β and IL-4 Levels

Objective: To analyze the effect of interferon α-2b on pediatric respiratory syncytial virus bronchiolitis and its effect on serum SP-D、TGF-β and IL-4 levels.Methods: 120 children with respiratory syncytial virus bronchiolitis admitted to our hospital (from January 2017 to October 2019) were selected and divided into group A (using conventional treatment methods) and group B (on the basis of the control group, interferon α-2b treatment method was used), 60 cases in each group; then 60 cases of healthy children were selected as the control group. The clinical symptoms and the disappearance time of basic vital signs (fever, wheezing, lung rales and cough, etc.) after treatment, and the total effective rate of clinical treatment were compared. The serum SP-D, TGF-β and IL-4 of the three groups were analyzed.Results: The disappearance time of fever, wheezing, lung rales and cough in group B was significantly shorter than that in group A, and the difference was statistically significant (P<0.05); the total clinical effective rate of group B was significantly higher than that in group A, and the difference was statistically significant (P<0.05).The results of single factor ANOVA variance analysis showed that the serum SP-D、TGF-β and IL-4 levels of the three groups were statistically significant (P<0.05). The results showed that the serum SP-D and IL-4 levels of bronchiolitis in group A and group B were significantly higher than those in the control group, and the serum TGF-β levels were significantly lower than those in the control group (P<0.05).The SP-D level of group B was significantly higher than that of the control group, lower than that of group A, the TGF-β level was significantly lower than that of the control group, and the IL-4 level was significantly higher than that of the control group (P<0.05).Conclusion: The effect of interferon α-2b on pediatric respiratory syncytial virus bronchiolitis is remarkable, at the same time, it can significantly reduce the serum SP-D、IL-4 level and improve the TGF-β level of children.

92 An Investigation of the Level of Serum 25-Hydroxyvitamin D in Early Type 2 Diabetic Nephropathy and Its Relationship with Advanced Glycation End Products

Objective: To investigate the relationship between 25-hydroxyvitamin D [25 (OH) D3] levels and advanced glycation end products (AGEs) in patients with type 2 diabetic nephropathy. Methods: A total of 131 T2DM patients admitted from Nov. 2016 to Nov. 2018 were selected and divided into normal albuminuria group (n=46), microalbuminuria group (n=43) , and macroalbuminuria group (n=42) according to the urine microalbumin/creatinine value (UACR). 46 healthy volunteers were selected as the control group. The general clinical data and level of fasting blood glucose (FBG), urea nitrogen (BUN), creatinine (Scr), calcium (Ca), phosphorus (P), glycosylated hemoglobin (HbA1c), UACR, 25 (OH) D3, AGEs, and the soluble isoform receptor of AGE (sRAGE) were measured and compared. Results: The pairwise comparison between the four groups had significant difference with the 25-(OH)D3 levels of (35.78±4.99ng/mL,26.81±1.99ng/mL,24.10±4.41ng/mL, 20.63±4.60ng/mL, F = 108.228,P<0.01), AGEs levels of (27.85±9.57ng/L, 37.68±9.80ng/L,55.75±7.69ng/L,92.60±5.88ng/L,F =294.843,P<0.01), and sRAGE levels of (46.11±7.64ng/mL,36.79±8.88ng/mL,29.02±4.41ng/mL, 20.75±4.51ng/mL,F = 114.109,P<0.01). Pearson correlation analysis showed that 25-(OH)D3 was negatively correlated with BMI, WC, FBG, HbA1c, UACR and AGEs (r =-0.168,r =-0.337,r=-0. 662,r=-0.682,r=-0.535,r=-0.659, P<0.05 or 0.01), and positively correlated with sRAGE (r =0.691, P<0.01). Multiple stepwise regression analysis showed that WC, UACR, AGEs and sRAGE were independent influencing factors of 25-(OH)D3.Conclusion: 25-(OH)D3 decreased in the early diabetic nephropathy, and the decreased level was independently correlated with AGEs, which may be involved in the occurrence and development of early diabetic nephropathy through the AGEs-RAGE signaling pathway.

97 Correlation Analysis between the Expression of Serum Stathmin VEGF-C and Recurrence Metastasis after Thoracoscopic Resection of Esophageal Cancer

Objective: To analyze the correlation between the expression of serum microtubule disaggregation protein (Stathmin), vascular endothelial growth factor (VEGF-C) and recurrence, metastasis after thoracoscopic resection of esophageal cancer. Methods: 105 patients undergoing thoracoscopic resection of esophageal cancer in the hospital were selected as the research subjects, and their clinical data were collected. They were divided into the recurrence and metastasis group and non-recurrence or metastasis group according to whether the patient had recurrence or metastasis after operation. The clinical data, postoperative serum Stathmin and VEGF-C levels were compared between the two groups. Multivariate Logistic regression analysis was performed to analyze the risk factors for recurrence and metastasis after thoracoscopic resection of esophageal cancer. Results: In this study, there were 40 cases with recurrence and metastasis after thoracoscopic resection of esophageal cancer, accounting for 38.10%, the median recurrence time was 18 months. Postoperative serum Stathmin and VEGF-C levels in the recurrence and metastasis group were significantly higher than those in the non-recurrence or metastasis group (P<0.05). There were significant differences in pathological staging, depth of tumor invasion, the maximum diameter of the lesion and postoperative adjuvant radiochemotherapy between the two groups (P<0.05). Multivariate Logistic regression analysis showed that pathological staging, the maximum diameter of the lesion, depth of tumor invasion, postoperative serum Stathmin and VEGF-C levels were closely related to tumor recurrence and metastasis after thoracoscopic resection of esophageal cancer (P<0.05). Conclusion: There is a correlation between the expression of serum Stathmin, VEGF-C and recurrence, metastasisin after thoracoscopic resection of esophageal cancer. The two have great predictive value for postoperative recurrence and metastasis.

101 Clinical Characteristics of Corona Virus Disease 2019 and Effectiveness Analysis of Convalescent Plasma

Objective: To investigate the clinical characteristics of Corona Virus Disease 2019 (COVID-19) efficacy of convalescent plasma therapy. Methods: A total of 6 cases COVID-19 patients admitted to Wuhan Jinyintan Hospital and received convalescent plasma therapy from January 23, 2020 to April 26, 2020 were reviewed retrospectively, and the clinical characteristics as well as efficacy of convalescent plasma therapy were summarized and analyzed respectively. Results: According to the guideline for Diagnosis and Treatment of Novel Coronavirus Infected Pneumonia, 6 cases were divided into severe group (n=4) and critical group (n=2). The clinical symptoms of 6 cases included fever (6 cases), cough (6 cases), fatigue (3 cases), diarrhea (1 case). Ratio of lymphocyte decreased in 3 patients. High sensitivity troponin and glutamic-pyruvic transaminase raised in one patient, respectively. The manifestations of early lung CT images in severe group were ground-glass-like high density shadows scattered outside the lungs. CT images in critical group showed that diffuse ground-glass-like high density shadows, or cotton ball-like and reticular nodules consolidation change were formed in both lungs. On the basis of antiviral therapy, 6 patients were administrated with convalescent plasma. All patients were successful in plasma transfusion except one, who had a skin allergy, and was relieved after symptomatic treatment. After active treatment, symptoms and various indicators of these patients were improved, and finally, all patients were cured and discharged. Conclusion: Fever and cough are the first symptoms of COVID-19, accompanied by different degrees of systemic symptoms, Plasma therapy in convalescent period has certain application value in treating COVID-19, but the potential side effects of blood transfusion should be paid attention to.

105 Effects of Different Blood Purification Methods on Mineral and Bone Metabolism in Patients with Maintenance Hemodialysis

Objective: To investigate the effect of hemoperfusion combined with hemodialysis and hemodiafiltration on mineral and bone metabolism in patients with maintenance hemodialysis. Methods: A total of 90 cases of MHD patients were selected from January 2017 to September 2019. Those patients were divided into two groups with 45 cases each according to the random number table method. The observation group was treated with hemoperfusion combined with hemodialysis, while the control group was treated with hemodiafiltration. The treatment lasted for 6 months. The renal function, mineral and bone metabolism of the two groups were compared before and 6 months after treatment, and the clinical symptoms of the two groups were observed. Results: After treatment, the levels of serum creatinine (Scr), blood urea nitrogen (BUN) and β2-MG were significantly reduced in both groups, and the levels of those indexes in the observation group were lower than those in the control group (P<0.05). After treatment, the incidences of skin pruritus, joint pain, muscle weakness and hypertension in the observation group were significantly lower than those in the control group (P<0.05). After treatment, there was no significant change in blood calcium level in the two groups compared with before treatment (P>0.05), while the blood phosphorus level in the observation group was significantly lower than that in the control group (P<0.05). The serum levels of osteoprotegerin (OPG), Bone morphogenetic protein-2 (BMP-2) and bone alkaline phosphatase (BALP) in the two groups were significantly reduced, and the observation group was lower than the control group after treatment (P<0.05). Conclusion: Hemoperfusion combined with hemodialysis can effectively improve the clinical symptoms of MHD patients and correct abnormal mineral and bone metabolism.

Objective: To investigate the role of AmpC and ompk35 in imipenem induced resistance of Klebsiella pneumoniae. Methods: A total of 200 strains of Klebsiella pneumoniae isolated and cultured in our hospital from January 2015 to January 2020 were collected, among which 50 strains were resistant to imipenem (group A), 150 strains were not resistant to imipenem (group B), the gene expression of AmpC and ompk35 was detected by PCR, and the protein expression of AmpC and ompk35 was detected by Western blot. Results: The relative expression of AmpC gene and protein in group A were (1.658±0.122) and (1.501±0.106), which were significantly higher than that in group B (P < 0.05); The relative expression of ompk35 gene and protein in group A were (0.712±0.110) and (0.611±0.107), which were significantly lower than that in group B (P < 0.05); The correlation between AmpC gene and ompk35 gene was negative (r=-0.329, P < 0.05); the correlation between AmpC protein and ompk35 protein was negative (r=-0.435,p<0.05). Conclusion: The expression of AmpC in imipenem resistant Klebsiella pneumoniae increase, and the expression of ompk35 decrease. They may play an important role in the mechanism of drug resistance.

112 Analysis of Risk Factors of Heart Complications after Operation of Osteoporotic Hip Fracture in the Elderly

Objective: To explore the risk factors of postoperative cardiac complications in elderly patients with osteoporotic hip fracture. Methods: From January 2017 to March 2019, 127 elderly patients with osteoporotic hip fracture were collected. Total hip arthroplasty was performed in selected time, and the population baseline characteristics and perioperative characteristics of patients were collected by Excel, The risk factors of cardiac complications after fracture operation were analyzed by logistic. Results: There were significant differences in age, BMI, smoking, history of hypertension, history of diabetes, history of hyperlipidemia, history of cerebral infarction and medication in the group without cardiac complications (P<0.05). There was no statistically significant difference in anesthesia mode, operation time, intraoperative blood loss, and postoperative complications between the non-cardiac complication group and the cardiac complication group (P>0.05). After logistic multivariate analysis, age and BMI, history of hypertension, history of diabetes, history of hyperlipidemia, anti-osteoporosis drugs were independent risk factors for postoperative cardiac complications (P<0.05). Conclusion: The incidence of postoperative cardiac complications in elderly patients with osteoporotic hip fracture is high. Patients with hypertension, diabetes, hyperlipidemia and oral anti osteoporosis drugs have a higher risk of heart complications.

116 Expression of IGF-I and EGFR in Uterine Tissues and Their Relationship with Recurrence of Leiomyoma

Objective: To investigate the expression of insulin-like growth factor-I (IGF-I) and epidermal growth factor receptor (EGFR) in uterine tissues and their relationship with myoma recurrence. Methods: The clinical data of 125 patients with hysteromyoma who underwent myomectomy from January 2018 to December 2018 were analyzed retrospectively. The expression of IGF-I and EGFR in myoma, Paratumor and myometrium were detected by immunohistochemistry. After 1 year follow-up, the patients were divided into recurrence group (37 cases) and nonrecurrence group (88 cases). The expression of IGF-I and EGFR in three kinds of uterine tissues of the two groups were analyzed and compared. Results: The expression of IGF-I and EGFR in different uterine tissues was statistically significant (P<0.05). The expression of IGF-I and EGFR in myoma, Paratumor and myometrium of patients with recurrence was higher than that of patients without recurrence (P<0.05). Conclusion: The expression level of IGF-I and EGFR in the myoma of patients with uterine leiomyoma is higher than that in the adjacent tissue and myometrium, and the increased expression intensity of IGF-I and EGFR may be one of the potential factors for the recurrence of myoma.

121 Clinical Evaluation of Ozone Intradermal Injection Combined with Low-temperature Plasma Nerve Ablation for Neuralgia in Herpes Zoster

Objective: To observe the clinical effect of intradermal ozone injection combined with low-temperature plasma nerve ablation on postherpetic neuralgia. Methods: 68 patients with postherpetic neuralgia on the chest and back were divided into two groups by random number table according to the order of visits. Each group had 34 cases. The control group was treated with intradermal injection of ozone, and the experimental group was treated with low-temperature plasma nerve ablation on the basis of the control group for 8 weeks of continuous treatment. The changes in VAS scores, sleep quality and quality of life were observed before and after treatment in the two groups. Results: The VAS scores of the experimental group were compared 4 weeks and 8 weeks after treatment with the before treatment, and the differences were statistically significant compared with the control group; The VAS scores in the group were significantly different after 8 weeks of treatment compared with 2 weeks after treatment. The quality of sleep and quality of life were significantly different in the scores of the two groups after treatment compared with those before treatment. The experimental group was compared with the control group after treatment. The differences were statistically significant. Conclusion: Ozone intradermal injection combined with low temperature plasma nerve ablation for the treatment of postherpetic neuralgia in the chest and back can better relieve pain, improve sleep, and improve the quality of life than using ozone intradermal injection alone.

125 The Clinical Effect of Benzbromarone Combined with Glucocorticoid Therapy on Patients with Gouty Arthritis and It's Influence on AST ALT UA Level

Objective: To investigate the effect of benzbromarone combined with glucocorticoid therapy on patients with gouty arthritis and its influence on the level of AST, ALT and UA. Methods: 300 patients with gouty arthritis who were treated in our hospital from February 2019 to February 2020 were selected as the research objects, and they were randomly divided into group A and group B with 150 cases each. Group A was treated with colchicine + Glucocorticoid therapy, group B was treated with benzbromarone combined with glucocorticoid, and the clinical efficacy and the changes of AST, ALT and UA levels were compared between the two groups. Results: In group A, 78 cases were markedly effective, 50 cases effective, 22 cases ineffective, and the total effective rate was 85.33%; Group B patients had 80 markedly effective cases, 60 cases effective , and 10 cases ineffective, with a total effective rate of 93.33%; The effective rate was analyzed by clinical statistics, and there was a difference (P<0.05); The two groups of patients had no statistical difference in age, gender, and BMI indicators (P>0.05); And among the ALT, AST, UA indicators, group B indicators were significantly better than group A, with significant statistical difference (P<0.05). Conclusion: Benzbromarone combined with glucocorticoid therapy in patients with gouty arthritis has a significant clinical effect on patients. At the same time, attention should be paid to monitoring changes in liver function.

128 Application of Ultrasound Elastography Strain Rate Ratio Method and MSCT in the Diagnosis of Thyroid Nodules

Objective: To explore the application value of ultrasound elastography strain rate ratio (SR) and multi-slice spiral computed tomography (MSCT) in the diagnosis of thyroid nodules. Methods: The clinical data of 208 patients with thyroid nodules in our hospital from January 2016 to June 2019 were retrospectively analyzed. The differences between MSCT and surgical pathology (gold standard) in the diagnosis of benign and malignant thyroid nodules were compared. Receiver operating characteristic curve (ROC curve) was used to obtain the optimal threshold of ultrasound elastography SR in the diagnosis of malignant thyroid nodules. 208 patients with thyroid nodules were given diagnosis. The diagnostic value of MSCT and ultrasound elastography SR on thyroid nodules was analyzed. Results: Among 292 nodules in 208 patients with thyroid nodules, there were 193 benign nodules and 99 malignant nodules. A total of 237 (81.16%) nodules were accurately diagnosed by MSCT, and the Kappa value was 0.599, with moderate consistency. The ultrasound elastography SR value of malignant thyroid nodules was significantly higher than that of benign thyroid nodules, the difference was statistically significant (P<0.05). After ROC curve analysis, ultrasound elastography SR had a high diagnostic value on malignant thyroid nodules, the difference was statistically significant (AUC=0.956, P<0.05), and its optimal threshold was 2.978 in the diagnosis of malignant thyroid nodules. 266 (91.10%) nodules were accurately diagnosed by the optimal threshold of ultrasound elastography SR, and Kappa value was 0.806, with extremely strong consistency. The sensitivity, specificity, positive predictive rate, negative predictive rate and accuracy rate of MSCT for thyroid nodules were lower than those of ultrasound elastography SR, the difference was statistically significant (P<0.05). Conclusion: Ultrasound elastography SR has higher accuracy rate than MSCT in the qualitative diagnosis of thyroid nodules, and has higher clinical application value.

131 Effects of Different Doses of Fluvastatin on Long-Term Blood Lipid MMP-9 Factor and Left Ventricular Remodeling in Patients with Acute Myocardial Infarction

Objective: To study the effects of different doses of fluvastatin on long-term blood lipid, MMP-9 factor and left ventricular remodeling in patients with acute myocardial infarction (AMI). Methods: A total of 192 AMI patients who were admitted to the hospital between January 2017 and March 2019 were randomly divided into high-dose group and low-dose group. And there were 93 cases and 94 cases in the above two groups completing the treatment, respectively. On basis of routine treatment, low-dose group had 40 mg/d fluvastatin, while high-dose group had 80 mg/d fluvastatin. All were treated for 6 months. The left ventricular ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDd), left ventricular end systolic diameter (LVESd), levels of serum total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), matrix metalloprotein-9 (MMP-9), N-terminal pro-brain natriuretic peptide (NT-proBNP) and cardiac troponin T (cTnT) in both groups before and after treatment were detected. The occurrence of adverse reactions during treatment in both groups was statistically analyzed. Results: After treatment, levels of TC, TG, LDL-C, MMP-9, NT-proBNP and cTnT were significantly decreased, while level of HDL-C was significantly increased in both groups (P<0.05). After treatment, decrease amplitude of TC, LDL-C, MMP-9, NT-proBNP and cTnT in high-dose group was significantly less than that in low-dose group (P<0.05). After treatment, LVEF was significantly increased, while LVEDd and LVESd were significantly decreased in both groups (P<0.05). After treatment, change amplitude of LVEF, LVEDd and LVESd in high-dose group was significantly greater than that in low-dose group (P<0.05). The adverse reactions in both groups were mainly on nausea, dizziness, abdominal pain and hypotension. There was no significant difference in the incidence of adverse reactions between high-dose group and low-dose group (21.51% vs 17.02%) (P>0.05). Conclusion: Fluvastatin can effectively interfere with blood lipid level in AMIpatients, reduce levels of MMP-9 factors, and inhibit left ventricular remodeling. The curative effect of high-dose (80 mg/d) fluvastatin is better.

136 An Analysis of Clinical Characteristics and Risk Factors of Gastroesophageal Reflux Disease Manifested by Extraesophageal Symptoms Among Elderly Patients

Obejective: To explore the clinical characteristics and risk factors of elderly patients with gastroesophageal reflux disease manifested by extraesophageal symptoms. Methods: The clinical data of elderly GERD patients in our hospital from January 2016 to January 2020 were retrospectively analyzed, and the patients were divided into extra-esophageal symptoms group and non-esophageal symptoms group according to whether extra-esophageal symptoms occurred, analyze the clinical characteristics and risk factors of patients with extraesophageal symptoms. Results: The incidence of extraesophageal symptoms in elderly GERD patients is about 52.59%, throat foreign body sensation, chronic cough, and reflux pharyngitis are most common, accounting for 31.97%, 24.59%, and 18.85%, respectively. There were significant differences between the two groups of patients in COPD, calcium channel antagonists, anxiety and depression, sleep disorders, constipation, gastroscopy reflux esophagitis, hiatal hernia, and BMI (P <0.05). Multiple logistic regression analysis revealed anxiety and depression (OR: 3.891, 95% CI: 1.254 to 5.169, P <0.001), hiatal hernia (OR: 4.114, 95% CI: 2.013 to 6.242, P <0.001), BMI (OR: 3.457, 95% CI: 2.287 ~ 4.728, P = 0.001), and smoking (OR: 2.782, 95% CI: 1.651 ~ 3.907, P = 0.009) were risk factors for extraesophageal symptoms in elderly GERD patients. Conclusion: The most common manifestations of extraesophageal symptoms in elderly GERD patients are foreign body sensation in the throat, chronic cough, and reflux pharyngitis. Anxiety and depression, hiatal hernia, BMI, and smoking are risk factors for extraesophageal symptoms in elderly GERD patients.

141 The Correlation Between Mammography and Contrast-Enhanced Ultrasonography and Immunohistochemical HER-2 and nm-23 Expression in Early Intraductal Breast Carcinoma

Objective: To explore the correlation between molybdenum target X-ray and CUES expression in early DCIS and immunohistochemical HER-2 and nm-23 expression. Methods: A total of 80 cases of DCIS patients admitted from January 2018 and January 2020 were retrospective selected as the research object. Expression levels of immunohistochemical HER-2 and nm-23 were detected and image data were collected, which were analyzed with Spearman. Results: Among the 80 patients, there were 36 cases with high expression of HER-2, 44 cases with low expression of HER-2, 38 cases with high expression of nm-23, and 42 cases with low expression of nm-23 by immunohistochemistry. Abnormal vascular distribution, non-centric enhancement, high enhancement, non-uniform enhancement, and PI in the high HER-2 expression group were significantly higher than those in the low HER-2 expression group (P<0.05), and TTP in the high HER-2 expression group was significantly lower than that in the low HER-2 expression group (P<0.05). The proportion of non-radioactive enhancement and TTP in the high expression group of nm-23 were significantly higher than those in the low expression group of nm-23 (P<0.05), while PI was significantly lower than that in the low expression group of nm-23 (P<0.05). High expression of HER-2 was positively correlated with TTP (P<0.05), and negatively correlated with PI (P<0.05). High expression of nm-23 was negatively correlated with TTP (P<0.05), and positively correlated with PI (P<0.05). Conclusion: The characteristics of moly target X-ray and CUES are correlated with the expression levels of immunohistochemical HER-2 and nm-23, and abnormal vascular distribution, enhanced centrality, high enhancement, radial enhancement, high TTP and low PI should be suspected to be high HER-2 expression, while low TTP and high PI should be suspected to be high nm-23 expression.

146 Effect of Zoledronic Acid Injection on Bone Pain Score and Bone Metabolism Index in Patients with Osteoporosis

Objective: To study the role of zoledronic acid in the treatment of osteoporosis and to explore its effect on bone metabolism. Methods: One hundred and sixty patients diagnosed as primary osteoporosis in our hospital from January 2017 to March 2019 were randomly divided into treatment group (80 cases) and control group (80 cases). Patients in the control group were giving oral Calcium D tablets daily for 12 months. Patients in the treatment group were given intravenous zoledronic acid (Zometa). ANOVA with repeated measurement data was used to compare the pain scores, bone mineral density and bone metabolism related indexes of the two groups of patients before and one year after treatment. Results: The differences in bone pain scores, bone mineral density, and other bone metabolism indexes between the two groups were statistically significant (P<0.001), and the differences before and after treatment were statistically significant (P<0.001). There is an interaction effect between the groups and time (P<0.001). The differences between the two groups before and after treatment were different, and the treatment group has a larger change (P<0.001). There was no statistically significant difference in blood calcium (Ca), blood phosphorus (P), and BGP levels between the two groups of patients, nor was the difference between before and after treatment, and there was no interaction; the difference in BALP between the two groups was statistically significant ( P<0.001), the differences in BALP levels before and after treatment were statistically significant (P<0.001), and there was an interaction between different groups and time (P<0.001), and the differences between the two groups before and after treatment were different. The group changed more significantly (P<0.001). The incidence of the control group was 7.50%, and the incidence of the treatment group was 8.75%. There was no significant difference in the incidence of adverse reactions between the two groups. Conclusion: Zoledronic acid can effectively improve bone pain and bone metabolism related indicators, reduce bone resorption, increase bone density, less adverse reactions, and better patient compliance.

150 Effect of Butyphthal Combined with Alteplase on Neurological Function and Coagulation Function and Serological Indexes as TNF-α hs-CRP and Hcy in Patients with Acute Ischemic Stroke

Objective: To observe the clinical effect and safety of Ateplase combined with Agatroban on acute ischemic stroke (AIS) and its influence on neurological function and coagulation function and serological indexes such as high sensitive C reaction protein (hs-CRP)、tumornecrosis factor-α (TNF-α) and homocysteine (Hcy). Methods: A total of 134 patients with AIS were selected and divided into two groups by random number table method. The control group consisted of 67 patients who received basic treatment + alteplase and the combination group received basic treatment + alteplase + butylphthal. Both groups were treated for 2 weeks, and the clinical efficacy was compared. National Institutes of Health Stroke Scale (NIHSS), cerebral infarction volume, neuron-specific enolase (NSE), S100β protein, fibrinogen (FIB), D-dimer (D-D), prothrombin time (PT) , activated partial thromboplastin time (APTT), TNF-α, hs-CRP and Hcy was evaluated, detected, and compared. Results: The efficacy of the combination group was better than control group, respectively,with significant difference (P<0.05). Comparison before treatment revealed that the NIHSS score, cerebral infarction volume, NSE, S100β, FIB and D-D in the two groups were lower, and APTT and PT were increased after treatment. Compared with the control group, and NIHSS score, cerebral infarction volume, NSE, S100β, FIB and D-D in combination group was lower, while APTT and PT were increased after treatment, suggesting significant difference (P<0.05). After treatment, the TNF-α, hs-CRP, and Hcy in both groups were lower than those before treatment, and the TNF-α, hs-CRP, and Hcy in the combination group were lower than the control group, with statistical significance (P <0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Butylphthal combined with alteplase can improve the treatment effect, promote nerve function recovery, improve coagulation function, reduce the inflammatory reaction, and has good safety of patients with AIS, compared to the alteplase alone treatment.

156 Effects of Paroxetine and Olanzapine Respectively Combined with Flupentixol-Melitracen on HAMD, BI and NIHSS Scores in Patients with Post-Stroke Depression

Objective: To investigate the effect of paroxetine and olanzapine respectively combined with flupentixol-melitracen on the quality of life of post-stroke depression patients. Methods: From February 2017 to April 2019, 126 patients with stroke depression in our hospital were randomly divided into 3 groups by random number table method, 42 cases each in paroxetine combination group, olanzapine combination group and control group. Paroxetine combination group received paroxetine combined with flupentixol-melitracen, olanzapine combination group received olanzapine combined with flupentixol-melitracen, and the control group received flupentixol-melitracen alone treatment. The course of treatment is 8 weeks. The treatment efficacy, NIHSS, HAMD, BI score, and adverse reactions of the three groups were compared. Results: The therapeutic effect of the paroxetine combination group and olanzapine combination group was better than that of the control group (Z=3.063, 2.301, P<0.05); The total effective rates of the paroxetine combination group and olanzapine combination group were 95.24% and 90.48%, significantly higher than 73.81% of the control group (P<0.0167). After treatment, the NIHSS and HAMD scores of the three groups were significantly decreased (P<0.05), and the BI score was significantly increased (P<0.05); NIHSS and HAMD of the paroxetine combineation group and olanzapine combination group were significantly lower than that of the control group (P<0.05), the BI score was significantly higher than the control group (P<0.05). The incidences of total adverse reactions in the paroxetine combination group, olanzapine combination group and control group after treatment were 66.67%, 80.95% and 78.57% respectively. There was no statistically significant difference in the incidence of total adverse reactions among the three groups (P>0.05 ). Conclusion: Paroxetine combined with flupentixol-melitracen and olanzapine combined with flupentixol-melitracen in stroke patients with depression has a significant therapeutic effect compared with the use of flupentixol-melitracen alone. It can effectively reduce the degree of neurological deficit and depression, improve the ability of life, and will not increase adverse reactions.

160 Clinical Observation on Fire Needle Water Needle Combined with Buyang Huanwu Decoction in Treating Varicose Veins of Lower Limbs

Objective: To investigate the clinical efficacy of fire needle, water needle combined with Buyang Huanwu Decoction in the treatment of varicose veins of the lower extremities. Methods: From December 2018 to December 2019, 60 patients with varicose veins of the lower extremities who were admitted to the Department of Acupuncture and Moxibustion and Traditional Chinese Medicine of our hospital were selected into the study, and the random data table method was used to complete the grouping. The control group and the observation group include 30 samples respectively. The control group received acupuncture treatment, and the observation group received fire acupuncture and water acupuncture combined with Buyang Huanwu Decoction. The clinical benefit, VCSS score and long-term efficacy of the two groups were compared. Results: The clinical benefit of the observation group was 96.67%, which was significantly higher than 80% of the control group. After treatment, the VCSS score of the observation group was (2.01±0.98) points, which was significantly lower than the control group. After a 1-year follow-up, it was found that the recurrence rate of the control group was 20.00%, and the recurrence rate of the observation group was 0. With the testing result P<0.05, the difference was statistically significant. Conclusion: Fire needle, water needle combined with traditional Chinese medicine Buyang Huanwu Decoction has a definite clinical effect in the treatment of varicose veins of the lower extremities. It can significantly improve the patient's VCSS score. The long-term effect is stable and the recurrence rate is low. It is worthy of promotion.

163 Comparative Study of Two Methods of Sedation and Analgesia in Rescue Tracheal Intubation

Objective: To study the efficacy and safety of sufentanil combined with midazolam to induce sedation and analgesia to substitute fentanyl combined with midazolam. Methods: A total of 80 patients admitted to the Department of Intensive Medicine of our hospital for tracheal intubation due to various reasons were selected from January 1, 2019 to June 30, 2020. The grouping method was simple random grouping, and they were divided into sufentanil group and fentanyl group (40 cases in each). Sufentanil group used sufentanil 0.25-0.5μg / kg + midazolam 0.025-0.05mg / kg before intravenous intubation; Fentanyl group used before intubation Tani 0.5-1μg / kg + midazolam 0.025~0.05mg / kg; 4-5min after administration of the drug, tracheal intubation was conducted immediately. The blood pressure, heart rate, oxygen saturation, and respiratory rate of the two groups of patients were recorded before intubation, during intubation, and after intubation; at the same time, the reverse inhalation, apnea, arrhythmia, and intubation success during intubation were monitored. The required number of times, the success rate of one-time intubation, the incidence of choking cough during the induction period of general anesthesia and the serious situation were monitored. Results: In the fentanyl group and the sufentanil group, the differences in the mean arterial pressure, respiratory rate, blood oxygen saturation, and heart rate were statistically significant (all P<0.001), the patient's basic data, before and after intubation data showed statistically significant difference (all P<0.001), and there were interactions between different groups and time, the differences between the two groups at the same time point were statistically significant (all P<0.001). The same group at different time points within the period, the mean arterial pressure and blood oxygen saturation gradually increased, and the respiratory rate and heart rate gradually decreased (all P<0.001). Sufentanil group had lower incidence of reflux aspiration, apnea, and arrhythmia than fentanyl group (P <0.05). Sufentanil group required less successful intubation than the fentntanyl group (P <0.05). The degree of choking in sufentanil group was better than that in the fentanyl group (P <0.05).Conclusion: Sufentanil combined with midazolam is safer and more reliable than fentanyl combined with midazolam.
