1937 The Experimental Research of Berberine Intervening Diabetic Atherosclerosis by Regulating PI3K/AKT Signal Pathway

Objective: To investigate the expression level of PI3K/AKT signal pathway in the abdominal aorta of diabetic rats and the intervention mechanism of berberine. Methods: The SD rats were randomly divided into 3 groups, the normal control group, diabetic model group and berberine intervention group, 20 rats in each group. The diabetic rat model was established by intraabdominal injection of streptozotocin. The rats in berberine intervention group received berberine gastric perfusion with the dosage of 100mg·kg-1·d-1 while other 2 groups with the same dosage of normal saline after the model were established successfully. After 6 weeks, the blood glucose and lipid level were compared before and after intervention. The abdominal aorta were taken out and the PI3K, AKT, p21 expression level were detected by immunohistochemical technique. Results: The blood glucose level of diabetic model rats increased after modeling. The blood glucose level of the berberine intervention group decreased after pharmacological intervention and lower than that of the diabetic model group. There was no statistically significant difference of TC and TG between 3 groups before and after intervention. The expression level of PI3K, AKT in abdominal aorta were significant high while the level of p21 was low in the diabetic model group. BBR could decrease the level of blood glucose and inhibit the expression of PI3K, AKT while upregulating the level of p21 in abdominal aorta of diabetic rats. Conclusion: The activation of PI3K/AKT signal pathway may be one of the pathogenesis of DA. BBR may intervene the pathogenesis of DA by inhibiting cell proliferation by regulating the PI3K/AKT signal pathway.

1943 Study on the Effect of ANP on the Proliferation and Migration of Gastric Cancer MGC-803 Cells and Its Mechanism

Objective: To investigate the in vitro effect of atrial natriuretic peptide (ANP) on human gastric cancer cell line MGC-803 and the combined effect of PI3K/Akt signaling pathway inhibitor (LY294002) on human gastric cancer cell line MGC-803 and the possible mechanism. Methods: The gastric cancer MGC-803 cells were treated with different concentrations of ANP and different concentrations of LY294002 .The cell proliferation inhibition rate was detected by MTT assay. MGC-803 cells were randomly divided into four groups: control group, ANP group , LY294002 group and ANP combined LY294002 group. CCK8 was used to detect cell proliferation, scratch test was used to detect cell migration, cell morphology was observed under an inverted microscope and Western blot was used to detect the expression of key signal molecules total Akt (t-Akt) and p-Akt protein of PI3K/Akt signaling pathway. Results: CCK8 results showed that compared with control group, ANP group and LY294002 group, the combination of ANP and LY294002 can inhibit the proliferation of MGC-803 cells, with statistically significant differences(P<0.05); the inhibition of ANP alone and LY294002 alone on the proliferation of MGC-803 cells was time-dependent and dose-dependent. The 24-hour scratch area repair rate of the control group, ANP group, LY294002 group and ANP combined LY294002 group were (50.99±1.80)%, (43.33±2.22)%,(35.81±4.09)%,(26.67±1.69)% and the difference was statistically significant (P<0.05). Four groups of MGC-803 cells showed different cell morphology changes. Both ANP and LY294002 decreased the expression level of p-Akt protein in MGC-803 cells, and ANP combined with LY294002 had a more significant inhibitory effect on the expression of p-Akt protein than they were used alone (P<0.05). The expression level of t-Akt had no significant change in the four groups of cells (P>0.05). Conclusion: ANP can inhibit the proliferation and migration of human gastric cancer cell MGC-803. The combination of ANP and LY294002 can increase the inhibitory effect on gastric cancer cell MGC-803. The mechanism may be related to the inhibition of PI3K/Akt signaling pathway.

1947 Expressions and Significance of Serum MiR-34a NO and TNF-α in Children with Craniocerebral Injury

Objective: To study the expression levels of microRNA-34a (miR-34a), nitric oxide (NO) and tumor necrosis factor-α (TNF-α) in serum of children with craniocerebral injury and their significance. Methods: 78 children with craniocerebral injury admitted to Qinghai Women and Children's Hospital from September 2017 to August 2019 were divided into mild group (23 cases), moderate group (30 cases) and severe group (25 cases) according to Glasgow Coma score(GCS); at the same time, 80 healthy controls in the same period were selected. Real-time quantitative PCR (qRT-PCR) was used to detect the level of miR-34a in serum; the level of NO in serum was detected by chemical colorimetry; and enzyme-linked immunosorbent assay (ELISA) was used to detect the level of TNF-α in serum. Pearson method was used to analyze the correlation between serum miR-34a, NO, TNF-α and GCS score. Receiver operating characteristic curve (ROC) was applied to analyze the predictive value of serum miR-34a, NO, TNF-α levels for TBI. Results: ①The levels of serum miR-34a, NO and TNF-α in children with craniocerebral injury were higher than those in the control group, and the differences were significant (P<0.05); ②The levels of serum miR-34a, NO and TNF-α in severe group were higher than those in moderate group and mild group, the levels of serum miR-34a, NO and TNF-α in moderate group were higher than those in mild group, and the differences were significant (P<0.05); ③Pearson analysis showed that levels of serum miR-34a, NO and TNF-α were negatively correlated with GCS scores in children with craniocerebral injury (P<0.05); ④The area under the curve of TBI predicted by serum miR-34a, NO and TNF-α levels is 0.821(95%CI:0.675~0.967), 0.902(95%CI:0.816~0.989), 0.851(95%CI:0.741~0.961); cutoff values are 0.65, 38.45 μmol/L, 153.49 pg/mL; sensitivity is 88.9%,94.4%,72.2%; specificity is 72.0%, 70.0%, 98.0%; The area under the curve predicted by the joint detection is 0.998 (95%CI: 0.992~1.000); the sensitivity is 100.0%; and the specificity is 98.0%; ⑤The levels of serum miR-34a, NO and TNF-α in the death group were significantly higher than those in the survival group, and the differences were significant (P<0.05). Conclusions: miR-34a, NO, and TNF-α are all highly expressed in the serum of children with craniocerebral injury, which is closely related to the degree of craniocerebral injury. Joint detection has certain predictive value, and it has a certain degree of assessment on the grim prognosis of children with craniocerebral injury.

1952 The Correlations Between the Expressions of ApoB IL-18 andPostpartum Depression in Elderly Pregnant Women

Objective: To explore the correlation between the expression levels of apolipoprotein B (ApoB) and interleukin-18 (IL-18) in the serum of elderly pregnant women and postpartum depression (PPD). Methods: From January 2016 to June 2019, 85 women who met the diagnostic criteria for depression were selected and followed up as required for 42 days after delivery, which was called PPD group; and 87 cases of pregnant women who did not meet the diagnostic criteria for depression during the same period of follow-up were selected for a controlled study, which was made the normal group. The general data of the two groups were compared and analyzed. The level of serum ApoB of all pregnant women was measured by immunoturbidimetry. The level of IL-18 was detected by enzyme-linked immunosorbent assay (ELISA). The Edinburgh Postpartum Depression Scale (EPDS) was used to evaluate the depression status of all pregnant women. Pearson's method was used to analyze the relationships between the levels of serum ApoB, IL-18, EPDS score and total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), and the correlations between the levels of serum ApoB, IL-18 and EPDS score. And the influencing factors of PPD were analyzed by Logistic regression. Results: The levels of serum TC and HDL-C in PPD group were significantly lower than those in normal group (P<0.05), and the levels of ApoB and IL-18, and EPDS score in PPD group were significantly higher than those in normal group (P<0.05). The levels of serum ApoB and IL-18, EPDS score in PPD patients were negatively correlated with TC and HDL-C (P<0.05), and the levels of serum ApoB and IL-18 were positively correlated with EPDS score (P<0.05). TC,HDL-C levels were protective factors of PPD (P<0.05), and high levels of ApoB and IL-18 were risk factors of PPD (P<0.05). Conclusion: The expression levels of ApoB and IL-18 in serum of women with postpartum depression are up-regulated, which may be related to the degree of depressive symptoms. They may play an important role in the occurrence and development of PPD.

1956 Correlation Analysis of Hcy Level with Cognitive Dysfunction and Disease Progression in Patients with CSVD

Objective: To explore the correlation between homocysteine (Hcy) level and cognitive dysfunction and disease progression in patients with small cerebral vascular disease (CSVD). Methods: A total of 78 cases of CSVD patients diagnosed and treated in our hospital from December 2016 to December 2018 were retrospectively selected as the study objects. With the Montreal cognitive assessment scale (MoCA) score, they were divided into the cognitively normal group (32 cases) and the cognitively impaired group (46 cases), and another 52 cases of healthy physical examination patients were selected as the healthy control group. The differences in clinical data and Hcy level among the three groups were compared, and the correlation between Hcy level and cognitive dysfunction and disease progression was analyzed. Results: The control group of Hcy level was (10.12±3.45) umol/L, the cognitive normal group was (18.18±3.86) umol/L, and the cognitive impairment group was (23.35±4.42) umol/L, with statistically significant differences (P<0.05). MoCA score of cognitive normal group was higher than that of cognitive impairment group, and the difference was statistically significant (P<0.05). Pearson correlation showed that Hcy level was negatively correlated with MoCA total score, visual spatial executive function, memory and attention, abstract thinking, and directional force (r=-0.438, -0.264, -0.239, -0.333, P<0.05), but not with naming, language, and delayed recall (r=-0.005, -0.001, -0.083, P>, 0.05).After a one-year follow-up, 62.82% (49/78) in the CSVD progression group and 37.18% (29/78) in the non-progression group, and Pearson correlation showed that Hcy level was significantly negatively correlated with imaging outcome scores of CSVD patients (r=-0.351,P<0.01).According to ROC curve analysis, the AUC of Hcy in predicting the course of CSVD was 0.938, the sensitivity and specificity were 87.8% and 93.1%, respectively, showing high accuracy. Conclusion: The increased level of Hcy can be involved in the cognitive impairment of CSVD, and is negatively correlated with the progression of CSVD.

1961 Protective Effect of Sulforaphane on Lipopolysaccharide-Induced Acute Lung Injury via Nrf2/ARE Signaling Pathway

Objective: To study whether sulforaphane (SF) can reduce the acute lung injury (ALI) by up-regulating the transcription factor NF-E2 related factor 2 (Nrf2) to play an antioxidant and anti-inflammatory role. Methods: Thirty-two healthy mice were randomly divided into acute lung injury group (ALI group), control group (control group), SF group, and SF solvent group, 8 mice in each group. The control group was injected intraperitoneally with 10 mg/kg saline. SF group was intraperitoneally injected with 20 mg/kg (SF), SF solvent group was intraperitoneally injected with 20 mg/kg (SF carrier) solvent, 2 twice a day, continuous injection was performed for 3 days, and ALI group, SF group, and SF solvent group were injected with lipopolysaccharide at the 4th day. (LPS) 10 mg/kg, and an endotoxin-induced ALI model was prepared. Six hours after injection of LPS or normal saline, vena cava blood was removed and serum IL-6 and TNF-ɑ levels were measured by ELISA. Pulmonary tissue pathological results were scored for lung injury after observation with light microscope. Nrf2 nucleus in mouse lung tissue Protein expression was measured by Western blotting, and lung dry weight/wet weight ratio was calculated simultaneously. Colorimetric method was used to detect SOD, MPO and iNOS content in lung tissue homogenate. Results: The lung injury score, iNOS, MPO, IL-6, and TNF-ɑ levels in the ALI group and the SF solvent group were significantly higher than those in the control group, and the expression of Nrf2 protein was up-regulated (P<0.05). SOD was lower than that in the control group (P<0.05). There was no significant difference in the above indicators between the ALI group and the SF solvent group (P>0.05). The IL-6, TNF-ɑ, lung injury score, and iNOS content in the SF group were significant. Compared with the ALI group, the lung dry weight/wet weight ratio and SOD content increased, and the expression of Nrf2 protein was up-regulated (P<0.05). Conclusion: SF can reduce the severity of LPS-induced acute lung injury by inducing the expression of Nrf2 protein through SF, and it can also reduce the levels of IL-6, TNF-ɑ and MPO, confirming that Nrf2 transcription factor can effectively protect ALI.

1965 Imatinib Mesylate Enhances the Killing Effect of ALA-PDT on Skin Basal Cell Carcinoma Cells

Objective: To investigate the effects of 5-aminolevulinic acid (ALA) and/or imatinib mesylate on the content of porphyrin and the expression of ABCG2 protein in HSC-2 of keratinocytes and basal cell carcinoma of HaCaT, and to provide theoretical basis for further improving the clinical efficacy of PDT. Methods: HaCaT keratinocytes and HSC-2 basal cell carcinoma cell lines were cultured with or without imatinib mesylate. The content of ABCG2 protein was determined by Western blotting, the content of porphyrin in cells was detected by ELISA, and the location of porphyrin in cells was observed by confocal laser scanning microscope. Results: In HaCaT cells and HSC-2 cells, the combination of ALA and imatinib mesylate could improve the accumulation of Protoporphyrins in HSC-2 cells, and enhance the cell death induced by PDT. At the same time, compared with the combination of ALA and imatinib mesylate, the combination of ALA and imatinib mesylate could significantly reduce the expression of ABCG2 protein To enhance the killing effect of PDT on HSC-2 cells. Conclusion: This study reveals the possible reasons for the decreased sensitivity of ALA-PDT in the treatment of BCC, and provides a theoretical basis for the further improvement of the efficacy of ALA-PDT combined with imatinib mesylate in the treatment of BCC.

1969 Diagnostic Value of Chemical Staining Combined with Blue Laser Magnifying Endoscopy on Early Esophageal Carcinoma and Precancerous Lesions

Objective: To investigate the value of chemical staining combined with blue laser magnifying endoscopy in the diagnosis of early esophageal carcinoma and precancerous lesions. Methods: From May 2017 to May 2019,100 patients with 138 suspicious esophageal lesions were examined by endoscopy in our hospital. The 100 patients were examined by blue laser magnifying endoscopy, iodine staining and blue laser magnifying endoscopy combined with iodine staining, and the pathological diagnosis was taken as the gold index. Kappa consistency test was performed for the microscopic and pathological results of the three methods, and the reliability of the McNemar chi-squartest was evaluated by sensitivity, specificity, positive predictive value, negative predictive value and Youden Index. Results: The combination of blue laser amplification and iodine staining showed good consistency, high sensitivity, specificity, positive predictive value, negative predictive value and Youden index. Conclusion: The combination of blue laser amplification and iodine staining can improve the diagnosis rate of early esophageal carcinoma.

1973 Effect of Propofol Combined with Remifentanil or Sufentanil on the Dosage and Cognitive Level of Propofol in Resection of Cerebral Cortex Tumor

Objective: To investigate the effect of propofol combined with remifentanil or sufentanil on propofol dosage and cognitive level in remifentanil. Methods: From January 2016 to November 2019, 70 patients with cerebral cortical functional area tumors underwent craniotomy in our hospital were randomly divided into 35 cases of remifentanil group (group RF) and sufentanil group (group SF). Group RF was anesthetized intravenously with propofol combined with remifentanil. Group SF was anesthetized intravenously with propofol combined with sufentanil. The pre-induction (T0), during intubation (T1), 1 min after intubation (T2), 5 min after intubation (T3), After extubation (T4) propofol plasma target concentration, systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) were observed and compared between the two groups. The cognitive changes of the two groups were observed and compared. Results: Compared with the target concentration of propofol at different time points in the two groups, group RF was significantly lower than group SF (P<0.05). After extubation, the plasma concentration of propofol in group RF was lower than that in group SF (P<0.05). The MMSE scores in group RF were higher than those in group SF (P<0.05) after 5 days after operation (P>0.05). The adverse reaction rate of group RF (17.14%) was significantly lower than that of SF group (40%) (P<0.05). Conclusion: Compared with propofol combined with sufentanil, propofol combined with remifentanil can reduce the plasma target concentration of propofol in anesthesia, reduce the dosage of propofol appropriately, and effectively reduce the degree of cognitive impairment after operation.

1978 Expression of SLC7A11 and Ki-67 in Laryngeal Carcinoma and the Clinical Significance

Objective: To observe the expression of SLC7A11 and Ki-67 in laryngeal carcinoma, and to explore its role in the occurrence and progression of laryngeal carcinoma. Methods: The relative expression of SLC7A11 and Ki-67 in 117 cases of laryngeal carcinomas tissues ( laryngeal carcinoma group) and 65 cases of vocal polyps ( control group) was detected by immunohistochemical SP method. The relationship between the expression and the clinicopathological features of laryngeal carcinoma was analyzed. Chi-square test was used to analyze the relationship between SLC7A11 and Ki-67 expression and prognosis and recurrence of patients with laryngeal carcinoma. Results: The positive expression levels of and SLC7A11 and Ki-67 were higher in the laryngeal carcinoma group than in the control group ( χ2 = 9.630、8.006,both P<0.05). High expression of SLC7A11 and Ki-67 was associated with high clinical stage of laryngeal cancer(P<0.05).And it was not related to the survival of patients with laryngeal cancer. Conclusion: The high expression of SLC7A11 and Ki-67 in the laryngeal carcinoma are related to the disease progression and postoperative recurrence,and it is important for the prognosis of patients.

1982 The Mechanism of Wound Healing and Anti-Inflammatory Effect of Red Ointment Combined with Photon Therapy Instrument in Patients with Anal Fistula After Operation

Objective: To explore the mechanism of red ointment combined with photon therapy on wound healing and anti-inflammatory in patients with anal fistula. Methods: From March 2018 to April 2019, 93 patients with anal fistula who were operated on in our hospital were randomly divided into observation group (47 cases) and control group (46 cases). On the first day after operation, the patients in the control group were treated with red ointment after routine dressing change, and the observation group was treated with photon therapy instrument on the basis of the control group. The clinical effect, VAS score, edema, wound healing time, serum levels of IL-4, IL-17 and TNF-α were compared between the two groups. Results: The total effective rate of the observation group was significantly higher than that of the control group (P<0.05). There was no significant difference in VAS score between the two groups on the first day after operation (P>0.05), but the VAS score of the observation group was significantly lower than that of the control group on the third day and the seventh day after operation (P<0.05). There was no significant difference in the area of wound between the two groups (P>0.05), and the area of wound in the observation group was significantly lower than that in the control group (P<0.05). The wound healing time of the observation group was significantly shorter than that of the control group (P<0.05). The levels of IL-4, IL-7 and TNF - α in the two groups were significantly lower than those in the first day after operation (P<0.05), and the levels of IL-4, IL-7 and TNF - α in the observation group were significantly lower than those in the control group (P<0.05). Conclusion: The combination of red oil ointment and photon therapy instrument can effectively relieve the postoperative pain of patients with anal fistula and promote the wound healing. Its effect may be related to the inhibition of inflammatory response of the body, which is worthy of clinical promotion.

1986 Changes of Serum Vaspin and MCP-1 Levels in Patients with Coronary Heart Disease and Their Correlation with Myocardial Injury and Cardiac Function

Objective: To analyze the changes of serum Vaspin and MCP-1 levels in patients with coronary heart disease and their correlation with the degree of myocardial injury and cardiac function. Methods: 120 patients with coronary heart disease admitted to our hospital from July 2017 to September 2019 were studied. According to the types of disease, they were divided into unstable angina group (71 cases) and stable angina group (49 cases). According to plaque stability, they were divided into unstable plaque group (53 cases) and stable plaque group (67 cases). According to the number of pathological changes, they were divided into single-vessel disease group (57 cases), double-vessel disease group (38 cases), and triple-vessel disease group (25 cases). In addition, 94 healthy controls in the same period were included. The serum Vaspin and MCP-1 levels were detected by enzyme linked immunosorbent assay. The differences of serum Vaspin and MCP-1 levels, myocardial injury markers and cardiac function parameters between the two groups were compared. The differences of serum Vaspin and MCP-1 levels were compared. The correlation of serum Vaspin and MCP-1 levels with myocardial injury index and cardiac function parameters in patients with coronary heart disease was analyzed. Results: The serum Vaspin level of coronary heart disease group was significantly lower than that of the control group (P<0.05), and the MCP-1 level was significantly higher than that of the control group (P<0.05). The levels of CK-MB, CK, H-FABP and cTnI in the coronary heart disease group were significantly higher than those in the control group (P<0.05). LVEF and FS in coronary heart disease group were significantly lower than those in control group (P<0.05), LVEDD and LVESD were significantly higher than those in control group (P<0.05). The serum Vaspin level in the unstable angina group was significantly lower than that in the stable angina group (P<0.05), and the MCP-1 level was significantly higher than that in the stable angina group (P<0.05). The serum Vaspin level in the unstable plaque group was significantly lower than that in the stable plaque group (P<0.05), and the MCP-1 level was significantly higher than that in the stable plaque group (P<0.05). The levels of serum Vaspin and MCP-1 were significantly higher in ripplee- vessel disease group and double-vessel disease group than in single-vessel disease group (P<0.05). The serum Vaspin level in the triple-vessel disease group was significantly lower than that in the double-vessel disease group (P<0.05), and the MCP-1 level was significantly higher than that in the double-vessel disease group (P<0.05). Serum Vaspin level was positively correlated with LVEF and FS (P<0.05), and negatively correlated with CK-MB, CK, H-FABP, cTnI, LVEDD and LVESD (P<0.05). Serum MCP-1 level was positively correlated with CK-MB, CK, H-FABP, cTnI, LVEDD and LVESD (P<0.05), and negatively correlated with LVEF and FS (P<0.05). Conclusion: The levels of serum Vaspin and MCP-1 in patients with coronary heart disease are closely related with the degree of myocardial injury and cardiac function.

Objective: To compare the effects of immuno-histochemistry (IHC), dual-color silver-enhanced insitu hybridization (DISH) and fluorescence insitu hybridization (FISH) on the status of breast cancer HER-2 gene. Methods: Retrospective study was conducted on 180 cases of unilateral breast cancer patients admitted to our hospital from January 2017 to December 2019, all of whom were female . Tissue specimens were collected and examined for HER-2 by IHC, DISH and FISH. Results: Results of HER-2 gene in 180 patients: IHC test showed negative expression of HER-2 in 118 patients and positive expression of HER-2in 49 patients. DISH detection of HER-2 gene amplification was performed in 56 cases, 107 cases without amplification and 17 cases with uncertainty. FISH detected 54 cases of HER-2 amplification, 107 cases without amplification, and 19 cases with uncertainty. Comparison of different detection Methods: the Kappa value of HER-2 gene detected by DISH and IHC was 0.788, showing good consistency; The Kappa value of HER-2 gene examined by FISH and IHC was 0.784, wIHCh was highly consistent; The Kappa value of HER-2 gene detected by DISH and FISH was 0.939, indicating a high consistency. Conclusion: IHC, DISH and FISH can evaluate the her-2 gene status of breast cancer patients, and IHC can be used as a screening method. If medical resources permit, treatment plans should be made based on DISH test results.

1995 Efficacy of Nasal Endoscopic Maxillary Sinus Natural Ostium Enlargement Combined with Anterior Lacrimal Recess Approach Resection in the Treatment of Non-Invasive Fungal Rhino-Sinusitis

Objective: To explore the efficacy of nasal endoscopic maxillary sinus natural ostium enlargement combined with anterior lacrimal recess approach resection on non-invasive fungal rhino-sinusitis. Methods: 148 patients with non-invasive fungal rhino-sinusitis admitted to our hospital were randomly divided into control group and observation group by simple random grouping, with 74 cases in each group. Control group was given nasal endoscopic maxillary sinus natural ostium enlargement, and observation group was given nasal endoscopic maxillary sinus natural ostium enlargement combined with anterior lacrimal recess approach resection. The clinical efficacy, surgery, pain, nasal endoscopic examination score (Lund-Kennedy), postoperative complications and recurrence were compared between the two groups. Results: The total effective rate of clinical treatment in observation group was significantly higher than that in control group (P<0.05). The surgical time and postoperative hospital stay in observation group were significantly shorter than those in control group (P<0.05), and the intraoperative blood loss was significantly less than that in control group (P<0.05). At 6 months after surgery, the Visual Analogue Scale (VAS) pain score, nasal endoscopic score and nasal sinus CT score in the two groups were significantly lower than those before surgery (P<0.05), and the differences of the above scores before and after surgery in study group were greater than those in control group (P<0.05). The incidence rate of postoperative complications in observation group was lower than that in control group [5.41% (4/74) vs 16.22% (12/74)](P<0.05). At 6 months of follow-up, the recurrence rates in observation group and control group were 2.70% (2/74) and 8.11% (7/74) respectively (P>0.05). Conclusion: Nasal endoscopic maxillary sinus natural ostium enlargement combined with anterior lacrimal recess approach resection has a significant efficacy in the treatment of non-invasive fungal rhino-sinusitis. It can completely clear the lesions, improve drainage after maxillary sinus surgery, reduce the incidence rates of postoperative complications, and help reduce the risk of recurrence.

1999 Establishment and Preliminary Evaluation of Tumor Marker TTF-1 for Predicting EGFR Mutation in Lung Adenocarcinoma

Objective: To explore the establishment and preliminary evaluation of a model for predicting epidermal growth factor receptor (EGFR) mutation in lung adenocarcinoma by thyroid transcription factor-1 (TTF-1), a tumor marker. Methods: 120 patients with primary lung adenocarcinoma admitted to our hospital from June 2018 to May 2019 were selected as subjects. According to the results of EGFR genotype detection, they were divided into wild (non-mutation) group of EGFR gene (72 cases) and mutation group of EGFR gene (48 cases). TTF-1 expression was detected by En Vision immunohistochemistry, mutations in exons 18, 19, 20 and 21 of EGFR gene were detected by amplification-blocked mutation system, and multivariate logistic regression was used to analyze the related factors affecting EGFR gene mutation in patients with lung adenocarcinoma. Results: The positive rate of TTF-1 expression in lung adenocarcinoma patients with EGFR gene mutation was significantly higher than that in wild EGFR gene group (P<0.05). The mutation of EGFR gene was not related to age, performance status (PS) score and location of lung adenocarcinoma (P>0.05), but related to sex, smoking history and TNM stage of lung adenocarcinoma (P<0.05). Multivariate logistic regression analysis showed that TNM stage and TTF-1 expression were independent risk factors for EGFR gene mutation in lung adenocarcinoma patients (P<0.05), while gender, smoking history and EGFR gene mutation in lung adenocarcinoma patients had no independent correlation (P>0.05). The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of TTF-1 expression alone and in combination with clinicopathological parameters in predicting EGFR gene mutation in lung adenocarcinoma patients were the combination prediction of TTF-1, sex, smoking and TNM stage, the combination prediction of TTF-1, sex and smoking, the combination prediction of TTF-1 and sex, and the separate prediction of TTF-1 expression. Conclusions: TTF-1 positive is associated with EGFR gene mutation. TTF-1 expression combined with other factors may be more helpful in predicting EGFR gene mutation in patients with lung adenocarcinoma.

2004 Ultrasonographic Features and Misdiagnosis Analysis of Nodular Goiter Cystic Degeneration

Objective: To explore the ultrasonographic features and misdiagnosis of nodular cystic goiter. Methods: A retrospective analysis was performed on 64 patients admitted to our hospital from January 2016 to June 2020 who were misdiagnosed by pre-operative ultrasound as thyroid adenoma and other thyroid sarcoidosis and diagnosed by post-operative pathological examination as nodular goiter cystic degeneration. Sixty-four patients with nodular cystic goiter were selected as the observation group, and 24 patients with thyroid adenoma diagnosed by ultrasound and pathology were selected as the control group. The ultrasonographic features of patients with nodular goiter, the comparison of ultrasonographic findings between nodular goiter cystic degeneration and thyroid adenoma, and the ultrasonographic features of patients with nodular goiter who were misdiagnosed as other thyroid nodular lesions were analyzed. Results: The ultrasonographic images of nodular goiter cystic variable nodules were mainly characterized by clear boundaries, complete nodules, mostly enveloped, and no halo. Compared with thyroid tumor, there was no significant difference between the two lesions (P>0.05).There were significant differences in envelop, echo and halo between the two groups (P<0.05).The ultrasonographic features are similar to those misdiagnosed as other thyroid diseases. Conclusion: The ultrasonographic images of nodular cystic goiter are complex and diverse, and the main reasons for misdiagnosis are the lack of understanding of the complex and diverse echogenicity of nodular cystic goiter and the unclear analysis of thyroid nodule capsule, echo, halo and other aspects. Moreover, they are often misdiagnosed as thyroid adenoma.

2008 Comparison of the Effects of Ureteroscopic Holmium Laser Lithotripsy and Partial Tubeless Percutaneous Nephrolithotripsy on Postoperative Recovery of Upper Urinary Calculi

Objective: To compare the effect of ureteroscopic holmium laser lithotripsy and partial tubular percutaneous nephrolithotripsy on patients with upper urinary calculi and their postoperative rehabilitation. Methods: 131 patients with upper urinary tract calculi admitted to our hospital from August 2016 to may 2019 were analyzed retrospectively. According to the operation methods, they were divided into group A (69 cases) with ureteroscopic holmium laser lithotripsy (holmium laser lithotripsy) and group B (62 cases) with tubeless holmium laser lithotripsy (holmium laser lithotripsy); the operation time and postoperative hemoglobin (hemoglobin) of the two groups were observed, HB, white blood cell (WBC), average hospitalization time, lithotripsy clearance rate at 2 days, 15 days and 1 month after operation, and postoperative complications were recorded and compared. Results: The operation time of group A was significantly longer than that of the group B, and the postoperative hospitalization time was significantly shorter than that of group B, and the difference was statistically significant (P<0.05). Levels of white blood cells and hemoglobin were compared, and the difference was not statistically significant (P<0.05). Lithotripsy clearance rates at 2 days, 15 days and 1 month after operation were compared, and the difference was statistically significant (P<0.05). There were no serious complications such as infection, septic shock and sepsis in both groups after operation. The incidence of fever in group B was significantly higher than that in group A, and the difference was statistically significant (P<0.05). Conclusion: Ureteroscopic holmium laser lithotripsy and partial tubeless percutaneous nephrolithotripsy have better effect on patients with upper urinary calculi, but the incidence of postoperative complications in the former patients is lower.

2012 Observation of the Effect of IROOT-BP Dental Pulp Revascularization in Young Patients with Periapical Lesions of Permanent Teeth

Objective: To explore the application value of iRoot-BP closed upper root canal in the treatment of endodontic revascularization of young permanent teeth. Methods: Forty young permanent teeth patients with periapical lesions who were treated in the Department of Stomatology of Xinjiang Uygur Autonomous Region People's Hospital from June 2018 to June 2019 were included as the research subjects and randomly divided into a treatment group and a control group with 20 cases in each. The control group was treated with apical induction angioplasty, and the treatment group was treated with IROOT-BP for endodontic revascularization. The two groups were compared on the clinical efficacy, apex index, root canal wall thickness, root crown length and tooth function of patients with diabetes. Results: The clinical effective rate of the treatment group was significantly higher than that of the control group (90.00% vs 60.00%), and the difference was statistically significant (P<0.05). According to the apex index evaluation criteria, the proportion of complete cure + improvement in the treatment group was higher than that in the control group, and the difference was statistically significant (P<0.05). The root canal wall thickness and root crown length of the treatment group were higher than those of the control group (P<0.05). The VAS score showed that the treatment group scored higher than the control group in terms of occlusion, tooth color, and chewing function, and the differences were statistically significant (P<0.05). Conclusion: Endothelium revascularization with iRoot-BP closed crown is effective in the treatment of periapical lesions of young permanent teeth, and can promote root canal wall thickness and root canal regeneration. It is worth promoting.

2016 Research on the Expression Relationship of MiR-23b and Notch1 in Keloid

Objective: To detect the expressions of microRNA-23b (miR-23b) and notch1 protein in keloid fibroblasts, and to explore the relationship between the expressions and their effects on keloid. Methods: From December 2017 to November 2019, 130 patients with keloid were collected from the 149 Hospital of the PLA. During the operation, the scar tissues and normal dermal tissues of each patient were taken at the same time. The scar size was recorded, According to the severity of keloid, it was divided into mild 65 cases, moderate 43 cases and severe 22 cases. Firstly, fibroblasts from keloid and normal dermis were extracted and cultured, the level of miR-23b in fibroblasts was detected by real-time fluorescent quantitative PCR (qRT-PCR), and the level of notch1 protein was detected by immunohistochemistry. Pearson method was used to evaluate the correlation between miR-23b and notch1 expressions in keloid. Results: There were 203 skin lesions in 130 patients with keloids, 78 on the chest (38.42%), 59 on the shoulder and back (29.07%), 40 on the limbs (19.71%), 13 on the ears (6.40%), and 8 on the abdomen (3.94%), 5 on the hip (2.46%). The cumulative area of skin lesions was 6 cases (4.61%) with large scars, 57 cases (43.85%) with medium scars, and 67 cases (51.54%) with small scars. Compared with normal dermal tissues, mir-23b mRNA expression level and Notch1 positive expression rate in fibroblasts in keloid tissues were significantly increased, with statistically significant differences (66.15% vs 40.00 %, P<0.05). The expressions of miR-23b mRNA and notch1 in different clinical grades of keloid were statistically significant (P<0.05), and with the aggravation of keloid, miR-23b mRNA level and notch1 positive expression rate increased; Pearson correlation analysis showed that miR-23b and Notch-1 were positively correlated in keloid tissue fibroblasts (r = 0.529, P<0.05). Conclusions: MiR-23b mRNA and Notch-1 protein are highly expressed in keloid tissue, and they are positively correlated, which may be involved in the formation and development of keloid at the same time.

2021 Efficacy and Safety Analysis of BiPAP Non-Invasive Ventilator in Patients with Acute Exacerbation of COPD

Objective: To study the application effect and safety of bi-level positive airway pressure (BiPAP) non-invasive ventilator in the treatment of chronic obstructive pulmonary diseases (COPD) patients in the phase of exacerbation. Methods: A retrospective analysis method was adopted. The study objects were 120 patients with COPD in the phase of acute exacerbation who admitted from June 2016 to June 2019. They were divided into control group and study group according to different treatment methods, 60 cases in each; the control group received conventional treatment, and the study group was administrated BiPAP non-invasive ventilator therapy based on conventional treatment. The blood gas analysis indicators (arterial oxygen pressure, carbon dioxide partial pressure), and D-dimer were compared between the two groups before and after treatment , E-selectin levels and the total effective rate after treatment, the incidence of adverse reactions (dizziness, nausea, vomiting, diarrhea). Results: After treatment, the arterial blood oxygen partial pressure, carbon dioxide partial pressure, D-dimer, hs-CRP, total effective rate and incidence of adverse reactions were (95.14±12.31) mmHg, (47.97±6.03) mmHg, (30.09±4.97) μg/L, (13.68±8.15) mg/L, 96.67%, 5.00%, the indexes of the control group after treatment are (86.43±10.08) mmHg, (60.15±6.81) mmHg, (41.85±5.22) μg/L, (19.25±10.25) mg/L, 85.00%, 6.67%. Compared with the control group, the study group had higher arterial blood oxygen partial pressure and total effective rate, and lower carbon dioxide partial pressure, D-dimer, and hs-CRP levels (P<0.05); the incidence of adverse reactions after treatment in both groups was not different (P>0.05). Conclusion: BiPAP non-invasive ventilator has a significant effect in the treatment of patients with acute exacerbation of COPD. It can effectively improve the blood gas index of patients, reduce the inflammatory response, and has fewer adverse reactions and high safety.

2024 Effects of Serum BAC on Inflammation and CT Imaging Characteristics in Patients with Craniocerebral Trauma

Objective: To explore the effect of serum alcohol concentration (BAC) on inflammation and CT imaging characteristics in patients with craniocerebral trauma. Methods: 106 patients with craniocerebral injury admitted in our hospital from October 2018 to October 2019 were selected as the research object. Gas chromatography was used to measure the BAC of the patients. According to the alcohol concentration, the patients were divided into alcohol-positive group (BAC>0.05%) and alcohol-negative group (BAC <0.05%), with 42 and 64 cases respectively. All patients underwent CT scan to compare the serum inflammatory response index level and CT image characteristics of the two groups of patients. Results: The systolic blood pressure, heart rate and body temperature of the alcohol-positive group were significantly lower than those of the alcohol-negative group, while the respiratory frequency BAC level was significantly higher than that of the alcohol-negative group (P<0.05). The results of rank sum test showed that there was no statistically significant difference in Rotterdam CT scores between the two groups (P>0.05), but the proportion of patients with Rotterdam CT scores of 4, 5 and 6 in the alcohol-positive group was significantly higher than that in the alcohol-negative group. The levels of serum IL-6, TNF-α, CRP and WBC in the alcohol positive group were significantly lower than those in the alcohol negative group (P<0.05). The BAC of patients with craniocerebral injury was positively correlated with serum IL-6, TNF-α, CRP and WBC counts, and the Pearson correlation coefficient r was 0.931, 0.467, 0.863, 0.935 respectively (P<0.05), which was not related to Rotterdam CT score (P>0.05). Conclusion: BAC can participate in the process of inflammation in patients with cranial head injury and can reduce the degree of inflammation in patients, but it has no obvious influence on Rotterdam CT score.

2028 Expression and Clinical Significance of EGFR Gene in Pleuroperitoneal Fluid Cells of Paraffin Blocks

Objective: To analyze the expression and clinical significance of epidermal growth factor receptor (Epidermal growth factor receptor , EGFR) gene in pleuroperitoneal fluid cells of paraffin blocks. Methods: A total of 72 pleuroperitoneal fluid cell paraffin blocks specimens which were archived in the hospital from June 2018 to June 2019 were collected. The staining situations of paraffin blocks specimens were observed by immunohistochemistry. The relative expression level of EGFR gene was detected by Real-Time PCR. The mutation of EGFR gene in pleuroperitoneal fluid cells of paraffin blocks was detected by Amplification Refractory Mutation System. The comparison situations between the detection results of EGFR gene mutation in malignant pleuroperitoneal fluid cells of paraffin blocks and the histopathological results were analyzed. Results: Immunohistochemical results showed that the EGFR gene was negative and weakly positive in benign pleuroperitoneal fluid cells of paraffin blocks, while it showed positive expression of different degrees in the malignant pleuroperitoneal fluid cells. The relative expression level of EGFR gene in malignant pleuroperitoneal fluid cells of paraffin blocks was significantly higher than that in benign paraffin blocks (P<0.05). Among the 53 malignant pleuroperitoneal fluid cells paraffin blocks, there was 1 case with EGFR gene mutation. There were deletion mutations at the 19th exon of EGFR gene, and mutation site was E746-A750 DEL. There were no mutation and SNP locus at 18th, 20th and 21th exon of EGFR gene. Taking histopathological results as the golden standard, Kappa value and detection accuracy rate of EGFR gene mutation in malignant pleuroperitoneal fluid cells of paraffin blocks were 0.658 and 98.08%, respectively. Conclusion: The high expression of EGFR gene in pleuroperitoneal fluid cells of paraffin blocks is an early warning of malignant pleuroperitoneal fluid. Mutation at E746-A750 DEL of EGFR gene 19th exon can improve the drug sensitivity of targeted therapy.

2032 Clinical Characteristics and Related Pathogenic Genes of Children with Epilepsy and Dysgnosia for Unknown Reason

Objective: To analyze the clinical characteristics and related pathogenic genes of children with epilepsy (Epilepsy in patients with unexplained mental disorders,ID-E) and dysgnosia for unknown reason. Methods: From January 1,2016 to June 31,2019,72 children with epilepsy and dysgnosia for unknown reason admitted to our hospital were selected as subjects. According to the detection rate of gene copy number (CNVs) variation, the positive and negative expression of children's clinical characteristics were observed, and the clinical treatment effect was observed. According to the mental disorder degree, children were divided into mild mental disorder group, moderate mental disorder group, and severe mental disorder group. Different types of CNVs were observed and recorded. Results: 72 cases of children with CNVs were divided into two groups, 15 cases Positive (20.83%), 57 cases negative (79.17%). The clinical characteristics of children with positive and negative CNVs were analyzed. It was found that the difference was statistically significant in the degree of mental retardation and seizure characteristics (P<0.05). According to the children's mental retardation, among 4 cases of moderate mental retardation positive CNVs, 3 cases were confirmed microdeletion and microduplication syndrome, 1 case was confirmed gene fragment mutation, the mutation chromosome involving chromosome 17, 21 and X; there were no children with uncertain pathogenicity. In 11 cases of severe mental retardation positive CNVs, 7 cases were microdeletion and microduplication syndrome; 3 cases were gene fragment mutation, involving chromosomes 17, 21 and X; one case of pathogenicity uncertainty was found, and Fisher's test showed that there was no significant difference in microdeletion and microduplication syndrome, gene fragment mutation and pathogenicity uncertainty (P>0.05). The difference was statistically significant (P<0.05); the total effective rate in the negative group was significantly higher than that in the positive group (P<0.05). Conclusion: Different CNVs expression patients have significant differences in the degree of intellectual impairment, seizure characteristics; this study through gene copy number variation analysis showed that 20.83% of children have pathogenic copy number variation.

2036 Clinical Application of 3D Printing in Paprosky Ⅲ Acetabular Revision

Objective: To explore the clinical application effect of 3D printing in Paprosky III acetabular revision. Methods: 80 patients with Paprosky III acetabular defects treated with acetabular revision in the hospital between March 2015 and March 2020 were enrolled in this study. They were divided into control group (n=40, even numbers) and observation group (n=40, odd numbers) according to the odd or even hospitalization number. The control group was treated with routine total hip revision, and the observation group was given repair with 3D printing. Hip recovery of the two groups was compared. Results: The excellent and good rate of fracture end reduction in the observation group was 82.50%, significantly higher than 62.50% in the control group (P<0.05). The operation time, intraoperative blood loss and blood transfusion volume of observation group were significantly shorter/less than those of control group (P<0.05). After surgery, the improvement of Harris scores and NRS scores was significantly better in the observation group than in the control group (P<0.05). After surgery, ranges of hip joint flexion and abduction were significantly increased in both groups. Besides, improvement of ranges of hip joint flexion and abduction and bone coverage rate of acetabular prosthesis were significantly higher in observation group than in control group (P<0.05). 3 months of follow-up found that the incidence rate of complications in observation group (2.5%) was significantly lower than that in control group (20.00%) (P<0.05). 2040 Comparison Between CVP、UVP and Traditional Bilateral PVP Surgery for Osteoporotic Vertebral Compression Fracture

Objective: To compare the effect of CVP, unilateral pedicle approach (UVP) and traditional bilateral percutaneous vertebroplasty (PVP) in the treatment of osteoporotic vertebral compression fracture. Methods: From January 2017 to December 2019, 150 patients with osteoporotic vertebral compression fracture were selected and divided into group A, group B and group C by random number table method. Group A was treated by CVP surgery, group B by UVP surgery, and group C by traditional bilateral surgery. To observe the difference of clinical efficacy in the three groups, and to compare the difference of operation time, intraoperative bleeding volume, hospitalization time and the incidence of postoperative complications in the three groups. Results: The clinical efficacy of three groups were 92.0%,90.0% and 88.0% respectively, with no significant difference(P>0.05). There were significant differences in operating time and intraoperative bleeding volume between the three groups(P<0.05). The operating time and blood loss in group A were significantly lower than those in groups B and C (P<0.05). Group A had significantly shorter hospital stays than group B (P<0.05). The operating time, blood loss and hospital stay in group B were significantly lower than those in group C (P<0.05). The incidence of postoperative bone cement leakage in group A, group B and group C was 0%, 2% and 4%, respectively, and there was no statistically significant difference in the incidence of postoperative complications among the three groups(P>0.05). Conclusion: CVP、UVP and PVP have similar efficacy in clinical treatment of osteoporotic vertebral compression fracture, but the safety of CVP is significantly higher than that of UVP and PVP surgery, which is worthy of popularization and use.

2044 An Analysis of Causes of Hinge Fracture During Laminoplasty and Its Effect on Clinical Efficacy

Objective: To observe the causes of hinge fracture during laminoplasty and its effect on clinical efficacy. Methods: A total of 214 patients with cervical spondylotic myelopathy who underwent laminoplasty in our hospital from May 2014 to October 2018 were enrolled, and they were divided into fracture group (58 cases) and intact group (156 cases) according to whether the hinge was fractured. The neurological function (JOA score), cervical curvature index (CCI), axial symptoms (VAS score) and factors associated with hinge fracture were observed and compared. Results: There was no significant difference in the hinge position between the 2 groups (P>0.05). The laminae opening angle of the fractured group were significantly greater than that of the intact group, while the bone mineral density was significantly lower than that of the intact group (P<0.05). The postoperative JOA score was significantly increased in both groups (P<0.05), and there was no statistical difference in neurological function recovery rate between the 2 groups (P>0.05). The CCI was well maintained in both groups (P>0.05), and there was no significant difference between groups (P>0.05). The VAS scores of the two groups showed a significant increase when compared with the preoperative level (P<0.05), and the scores in fractured group was higher than the intact group at different time points (P<0.05). Conclusions: The hinge fracture is associated with the decrease of bone mineral density and excessive opening angle. It will aggravate the axial symptoms, but will not affect the recovery of neurological function and the maintenance of cervical curvature.

Objective: To investigate the changes of the status of the elderly patients with osteoporosis vertebral fractures treated by percutaneous kyphoplasty. Methods: 114 elderly patients with osteoporotic vertebral fractures admitted to the second department of orthopedics, chengde central hospital from January 2009 to December 2019 were continuously collected. The patients were divided into operation group and control group according to whether they received surgical treatment. There were 70 patients in the operation group and 44 patients in the control group. Hamilton anxiety scale (HAMA-14), Hamilton depression scale (HAMD-24), sleep quality index scale (PSQI), mini-nutrition assessment brief form (MNA-SF) and ADL scale were used to assess the psychosomatic status of patients. Independent sample t test and chi-square test were used to compare the psychosomatic status of patients before and after treatment. Results: Compared with the control group, there were no significant differences in anxiety, depression, sleep, nutrition and self-care before treatment;After treatment, there were statistically significant differences in anxiety, depression, sleep and self-care, but no statistically significant differences in nutrition. Compared with the treatment group before and after treatment, there were statistically significant differences in anxiety, depression, sleep and self-care, but no statistically significant differences in nutrition. Compared with the control group before and after treatment, there were statistically significant differences in anxiety, but no statistically significant differences in depression, sleep, nutrition, and self-care. Conclusions: Anxiety, depression, sleep and self-care were significantly improved in patients receiving surgical treatment, and percutaneous kyphoplasty was beneficial to improve the psychosomatic status of elderly patients with osteoporosis vertebral fracture.

Objective: To analyze the effect of orthodontic micro-implant on MMP-2、MMP-9 and Bcl-2/Bax levels in patients with class II malocclusion. Methods: 110 patients admitted from January 2018 to December 2019 with class II malocclusion were randomly divided into observation group and control group. The control group had traditional orthodontic treatment, while the observation group had microimplant. The changes of MMP-2、MMP-9 and Bcl-2/Bax levels before and after treatment were observed, and the incidence of complications was different. Result: Repeated measurement analysis of variance showed that the differences of MMP-2 and MMP-9 between different groups and time points and the interaction between groups and time points were statistically significant (all P<0.05), and the increase of MMP-2 and MMP-9 in the control group was significantly higher than that in the observation group (P<0.05); the repeated measurement analysis of variance showed that the difference of Bcl-2/Bax value between different groups and time points, and the interaction between groups and time points were statistically significant (all P<0.05), and the Bcl-2/Bax value in the control group was significantly lower than that in the observation group (P<0.05). The incidence rates of complications during treatment in the observation group were 5.45% and 18.18%, and the difference between the two groups was statistically significant(P<0.05). Conclusion: The level of inflammatory factors caused by micro implants in orthodontic treatment of patients with class II malocclusion is lower than that of traditional orthodontic treatment, which is helpful to reduce the damage caused by orthodontic treatment to periodontal tissue, and the complications are less. So it is worth popularizing.

2056 An Analysis of the Diagnostic Value of Lung Ultrasound Chest X-Ray and CPIS for Pulmonary Consolidation in ICU Patients with Severe Pneumonia

Objective: To explore the diagnostic value of bedside lung ultrasound, chest X-ray and clinical pulmonary infection score (CPIS) in pulmonary consolidation of patients with severe pneumonia. Methods: 31 patients with severe pneumonia were included in ICU of our hospital. Bedside lung ultrasound was used to scan the bilateral chest wall of the patients, and the specific signs of lung ultrasound were used as the diagnostic basis of pulmonary consolidation. And the chest X-ray, chest CT and CPIS are improved. To evaluate the diagnostic value of pulmonary ultrasound, chest X-ray and CPIS in pulmonary consolidation, the "gold standard" of CT characteristic imaging is applied. Results: 27 patients were diagnosed as pulmonary consolidation by CT. There were 26 cases of pulmonary consolidation in lung ultrasound, the sensitivity was 92.6%, the specificity was 75.0%, the positive predictive value was 96.2%, the negative predictive value was 60.0%,and the diagnostic accuracy was 90.3%; 19 cases were diagnosed in chest X-ray examination, the sensitivity was 66.7%, the specificity was 75.0%, the positive predictive value was 94.7%, the negative predictive value was 25.0%, and the diagnostic accuracy was 67.7%; 21 cases were diagnosed with CPIS≥6, with sensitivity of 70.4%, specificity of 50.0%, positive predictive value of 90.5%, negative predictive value of 20.0%, and diagnostic accuracy of 66.7%. The results of consistency test showed that there was a high consistency between pulmonary ultrasound and CT diagnosis (kappa =0.611). Conclusion: Lung ultrasound has a high diagnostic value for pulmonary consolidation. Ultrasound, chest X-ray and CPIS have certain diagnostic value for lung consolidation, and pulmonary ultrasound has better diagnostic value.

2061 Effects of Arthroscopic Microcreation Combined with Nephrotic Method on Serum Levels of UA ESR and COX-2 of Patients with Acute Knee Joint Gouty Arthritis

Objective: To research the effects of arthroscopic microcreation combined with nephrotic method on the levels of serum uric acid (UA), erythrocyte sedimentation rate (ESR) and cyclooxygenase-gene (COX-2) of patients with acute knee joint gouty arthritis. Methods: 114 patients with acute gouty arthritis of knee joint were divided into control group (57 cases) and study group (57 cases) according to random number table method. Patients in the control group were treated with arthroscopic minimally invasive surgery, while patients in the research group were treated with kidney-tonifying and dampness-removing therapy on the basis of the control group. The clinical efficacy, signs and symptom scores, UA, ESR, COX-2 level, Lysholm score and adverse reactions of the two groups were compared. Results: After treatment, the total effective rate of the research group was significantly higher than that of the control group (P<0.05). The levels of signs and symptoms scores, UA, ESR and COX-2 in the two groups were significantly lower than before treatment (P<0.05), and the differences of signs and symptoms scores, UA, ESR and COX-2 before and after treatment in the research group were significantly higher than those in the control group (P<0.05). The Lysholm scores of the two groups were significantly higher than those before treatment (P<0.05), and the difference of Lysholm scores of the research group before and after treatment was significantly higher than those of the control group (P<0.05). There was no statistical difference in adverse reactions between the two groups (P>0.05). Conclusion: Arthroscopic microtechnique combined with kidney-tonifying and dampnesss-removing therapy is more effective than the single use of arthroscopic minimally invasive surgery in the treatment of acute knee joint gouty arthritis, which may be associated to lower the serum UA, ESR, and COX-2 levels.

2065 Change and Significance of Lactate Dehydrogenase, Interleukin-8 and Thromboxane B2 Levels in Aseptic Pustulosis

Objective: To analyze the change of content and significance of lactate dehydrogenase (LDH), interleukin-8 (IL-8) and thromboxane B2 (TXB2) in aseptic pustulosis. Method: The clinical data of 112 patients with aseptic pustulosis admitted to our hospital's dermatology department from August 2017 to September 2019 were collected, and 98 healthy controls were selected as the normal group. The differences in the content levels of LDH, IL-8 and TXB2 in different people were compared, and the correlation between LDH, IL-8 and TXB2 and aseptic pustulosis was analyzed. Results: The content levels of LDH, IL-8 and TXB2 in the blood of the patients in aseptic pustulosis group were significantly higher than those in the normal group (P<0.05).After treatment, the levels of LDH, IL-8, and TXB2 all decreased significantly (P<0.05). After treatment in 112 patients, there were 87 cases with good prognosis and 25 cases with grim prognosis. The content levels of LDH, IL-8 and TXB2 in patients with the good prognosis were significantly lower than those in patients with grim prognosis (P<0.05). Logistic regression analysis showed that different types, LDH, IL-8, and TXB2 were independent risk factors that affected the grim prognosis of patients with aseptic pustulosis (P<0.05). Conclusion: LDH, IL-8, and TXB2 content levels all show high expression in patients with aseptic pustulosis, and the change of index can provide a reference for clinical diagnosis, monitoring of disease, and evaluation of prognosis.

2070 Multivariate Logistic Regression Analysis of Secondary Traumatic Arthritis After Acetabular Fracture and the Study on Its Prognosis

Objective: To study the influencing factors of the occurrence of secondary traumatic arthritis after surgery for fracture of acetabulum, and analyze the impact of total hip arthroplasty or total hip surface arthroplasty on the prognosis of patients with traumatic arthritis. Methods: The clinical data and follow-up data of 180 patients with acetabular fracture undergoing surgical treatment in our hospital from August 2016 to August 2019 were retrospectively analyzed. All the patients' sex, age, fracture cause, type, operation situation and the occurrence of traumatic arthritis were recorded. The treatment and prognosis of traumatic arthritis were analyzed, and the risk factors of secondary traumatic arthritis were analyzed by multivariate logistic regression analysis. Result: The incidence of postoperative traumatic arthritis in 180 patients with acetabular fracture surgery was 26.67%. Compound fracture,femoral head hose defects, surgery ≥10 days after injury, poor quality of postoperative reduction of fracture, and postoperative intense labor in patients with postoperative traumatic arthritis played a much greater role and the differences were statistically significant(P<0.05). Compound fractures, femoral head tube defects, surgery ≥10 days after injury, poor postoperative reduction quality, and excessive postoperative labor intensity are independent risk factors for traumatic arthritis after acetabular fracture (P<0.05).In the 48 patients with traumatic arthritis, 30 patients were treated with total hip replacement and 18 were treated with total hip surface replacement. All patients had significantly improved in hip joint function after operation, and no infection, loosening of prosthesis and joint dislocation. Conclusion: Traumatic arthritis after acetabular fracture should not be underestimated, especially for patients with compound fractures, femoral head tube defects, delayed surgery after injury, poor quality of postoperative reduction, and excessive postoperative labor intensity, targeted interventions should be given to reduce the incidence of traumatic arthritis. For patients with complicated traumatic arthritis, total hip replacement and total hip surface replacement have clinical efficacy and safety.

Objective: To explore the correlation between coagulation index and severity of patients withcraniocerebral injury. Methods: To collectmedical records of 87 patients with craniocerebral injuryfromOctober 2017 to October 2018,useglasgow coma scale to assess the severity of injury,make use ofbivariate regression analysis, multiple linear regression equation andarea under the receiver-operating characteristic curve(ROC) to analyze the correlation between coagulation index and severity. Results: Bivariate regression analysis results showed that GCS was correlated with prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FIB), d-dimer (D-D), albumin (ALB) and glucose (GLU), there were significant difference(P<0.05). By using stepwise fitting,three independent variables were selected intooptimal multiple regression analysis equation, they were D-D, PT and GLU, R=0.836,R2=0.615. The ROC curve analysis results showed that the area under the ROC curve of D-D, PT and GLU predicted the GCS value of patients with craniocerebral injury were respectively0.863, 0.736 and 0.727,indicating that when blood D-D value≥4.58ug/ml, blood PT value≥13.55s and blood GLU value≥8.29mmol/L, the patients was in critical condition. Conclusion: In the early judgment of patients with craniocerebral injury, the change of coagulation index has predictive significance for the patient's injury, which could be combined with the patient's post-injury blood D-D, PT, GLU value to help determine the patient's condition, andthentimely gave corresponding treatment.

2080 Efficacy of Endo Combined with Liposome Paclitaxel and Cisplatin in the Treatment of Liver Metastases from Lung Cancer

Objective: To observe the clinical efficacy of Endo combined with liposomal paclitaxel and cisplatin in the treatment of liver metastases from lung cancer. Methods: A total of 125 patients with non-small cell lung cancer (NSCLC) who were admitted to our hospital from January 2018 to December 2019 and were treated with chemotherapy were randomly divided into observation group and control group based on the random number table. In the observation group, 67 cases were treated with "Endo + liposomal paclitaxel + cisplatin"; in the control group, 58 cases were treated with "liposomal paclitaxel + cisplatin". The clinical efficacy of the primary lesions and liver metastases were compared between the two groups, and the occurrence of adverse reactions in the two groups was recorded at the same time, and the two groups were followed up for more than 1 year to record the progression-free survival (PFS). Results: In terms of the efficacy of primary lung lesions, there was no significant difference in overall efficacy, ORR and DCR between the two groups (P>0.05). In terms of the efficacy of liver metastases, compared with the control group, the observation group has better overall clinical efficacy (P<0.05); the observation group has significantly higher ORR (38.81% vs 20.69%) and DCR (85.07% vs 68.97%) (P<0.05). Compared with the control group, the PFS of the observation group was significantly longer (P<0.05); there was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Compared with the "liposomal paclitaxel + cisplatin" treatment, Endo combined with liposomal paclitaxel and cisplatin in the clinical treatment of lung cancer liver metastasis is more effective, and effectively inhibits liver metastasis, and it also significantly improves patients progression-free survival, improvs the prognosis of patients, and has clinical application value.

2083 Analysis of Treatment Effect and Cost of Imipenem Cilastatin Sodium and Meropenem in Patients with Severe Pulmonary Infection

Objective: To analyze the clinical symptoms of patients with severe pulmonary infection, and to give imipenem cilastatin sodium and meropenem treatment according to the actual situation of the case, explore the final effect of the program and calculate the treatment cost. Methods: 80 patients with severe pulmonary infection admitted to our hospital from January 2017 to December 2019 were selected as the study subjects. They were divided into two groups by the red-and-blue grouping method. Patients in group A were treated with imipenem xastatin sodium, and patients in group B were treated with meropenem. The intervention effect, blood gas level, inflammatory index, bacteria culture result, drug susceptibility test result and treatment cost were compared. Results: There was no significant difference in intervention effect between group A and group B (80.0%VS85.0%) (P>, 0.05). Before intervention, there was no significant difference in blood gas index and inflammatory index between the two groups (P>0.05). After intervention, the blood gas levels of group A, PaO2, PCO2, PaO2/FiO2, PCT and CRP were all better than those of group B, with statistically significant difference (P<0.05). After treatment, the bacteria culture of group B was lower than that of group A (P<0.05). The cost-effectiveness of treatment in group B was significantly lower than that in group A (P<0.05). Conclusion: Both imipenem and cilastatin sodium and meropenem are of positive significance in the treatment of severe pulmonary infection. Meropenem is better than imipenem and cilastatin sodium in improving patients' blood gas level, immunity, and pathogen removal, reducing the treatment effect-cost, and its clinical application value is better than imipenem cilastatin sodium.

Objective: To study the curative efficacy of pegylated interferon alpha-2b combined with entecavirin in treating chronic hepatitis B and its effects onRetinal acid-related orphan receptor (ROR gamt), glomerular filtration rate (eGFR). Methods: 100 patients of chronic hepatitis B who received therapy from June 2017 - June 2018 were selected as research objects. By lottery, they were divided into observation group (n=52) and control group (n=48). The control group was treated with entecavir, while the observation group was treated with pegylated interferon alpha - 2B, Then efficacy, ROR T, eGFR, T cell subsets, interleukin-17 (IL-17), interleukin-4 (IL-4), interferon -α(INF-α) were compared. Results: The total effective rates of the two groups were 75.00% and 54.17%, respectively, with significant differences (P<0.05). There was no significant difference in ROR T and eGFR between the two groups before treatment. After treatment, the levels of ROR T and eGFR in both groups were improved, and the observation group was better than the control group (P<0.05). After treatment, T cell subsets in both groups were improved, and the improvement in the observation group was more significant than that in the control group (P<0.05). Before treatment, there was no significant difference in serum cytokine levels between the two groups. After treatment, serum cytokine levels in both groups were improved, and the levels of INF- and IL-17 and IL-4 in the observation group were higher than those in the control group (P<0.05). Conclusion: In chronic hepatitis B patients, the application of peg interferon alpha-2b combined with entecavir has a significant effect, which can effectively improve the ROR gamma t and eGFR levels of patients.

Objective: To explore the effect of Liangxuesanyu decoction combined with calcium dobesilate in the treatment of patients with diabetic retinopathy on clinical efficacy, symptom score and vascular endothelial growth factor (VEGF). Methods: 152 patients (152 eyes) admitted to our hospital from October 2017 to December 2019 were diagnosed with diabetic retinopathy as the research object, the clinical data of all patients were retrospectively analyzed. According to the different treatment plans, the patients were divided into a control group (75 cases) and a study group (77 cases). The control group was treated with calcium dobesilate, and the research group was supplemented with Liangxuesanyu decoction. During the treatment, the patient's condition was closely observed, and the clinical efficacy, symptom score, and VEGF effect between the two groups were compared. Results: The total clinical effective rate of the control group was significantly lower than that of the study group (88.00% vs 97.40%) (P <0.05). After treatment, the macular thickness, hemorrhagic spots, hemangioma, symptom score, and visual acuity of the two groups significantly improved, and the improvement in the study group was greater than that in the control group (P <0.05). The expressions of TNF-α and VEGF in the two groups significantly reduced, and the reduction in the study group was significantly better than that in the control group (P <0.05). Conclusion: Liangxuesanyu decoction combined with calcium dobesilate in the treatment of diabetic retinopathy has a good clinical effect, which can effectively alleviate the change of the disease and improve the severity of symptoms, and it is worthy of popularization and application in clinic.

Objective: To explore the relationship between gestational diabetes and serum iron, ferritin and transferrin. Methods: The data of pregnant women in the second trimester of pregnancy (24 weeks to 28 weeks of pregnancy) who were admitted to the Affiliated Hospital of Chengde Medical College from May 2018 to January 2020 were selected for retrospective analysis. According to the diagnosis results of all pregnant women, they were divided into GDM group (n=127) and normal group (n=130). Compare the general clinical data of the two groups of pregnant women. Compare the blood-related indicators [hemoglobin (Hb), mean red blood cell volume (MCV), mean red blood cell hemoglobin content (MCH), mean red blood cell hemoglobin concentration (MCHC)], blood glucose indicators [fasting blood sugar, blood sugar 1h blood sugar, serving Sugar 2h blood glucose], iron-related indicators [serum iron (SI), serum ferritin (SF), serum transferrin (TRF)]. Analysis of the correlation between iron related indicators and fasting blood glucose was conducted. Analysis of independent factors of GDM was conducted. Results: The data of sex, age, BMI and gestational age in GDM group and normal group were not statistically significant (P>0.05). The fasting blood glucose, blood glucose for 1h, glucose for 2h, SI, SF, TRF in the GDM group were higher than those in the normal group (P<0.05). The difference in Hb, MCV, MCH, MCHC data between the two groups was not statistically significant (P>0.05). There was a positive correlation between SI, SF, TRF and fasting blood glucose (P<0.05). SI, SF, TRF have become independent factors of GDM (P<0.05). Conclusion: Patients with GDM are prone to increased iron storage. The increase in SI, SF, and TRF has a certain correlation with the incidence of GDM. In actual work, more attention is needed.

2109 Investigation and Supervision of Clinical Departments of Traditional Chinese Medicine in Tertiary General Hospitals of Hebei Province

Objective: Understand and master the current situation of TCM clinical departments in tertiary general hospitals in Hebei Province, analyze the main problems and explore the corresponding countermeasures. Methods: The current situation of clinical departments of traditional Chinese medicine in 40 general hospitals in Hebei Province was investigated by joint verification and administrative permission of provincial registered tertiary general hospitals. Results: There are some problems in tertiary general hospitals in Hebei Province, such as insufficient beds in TCM departments, insufficient allocation of TCM professionals, single or even shrinking of appropriate TCM diagnosis and treatment technologies. Conclusion: In order to ensure the healthy development of Chinese medicine, it is necessary to further strengthen the construction of TCM professionals, increase the research and development, application and promotion of TCM clinical diagnosis and treatment technology, strengthen the publicity and Legal Popularization of TCM, and strengthen the supervision and law enforcement of TCM, so as to ensure the healthy development of Chinese medicine.
