1409 Correlation Between Plasma Cor NSE Insulin Resistance and Prognosis of Patients with Hypertensive Cerebral Hemorrhage

Objective: To explore the correlation between plasma Cortisol (Cor), neuron-specific enolase (NSE), insulin resistance (IR) and prognosis of patients with hypertensive cerebral hemorrhage (HCH).Methods: A total of 106 HCH patients were enrolled and divided into survival group (88 cases) and death group (18 cases) according to their prognosis.According to neurological deficit degree, they were divided into mild group (70 cases), moderate group (26 cases) and severe group (10 cases).Fifty middle-aged and elderly health controls in the same period were enrolled as control group.The levels of plasma Cor, NSE, fasting blood glucose (FBG) and fasting insulin were detected within 24h after confirming.IR index was calculated.Results: The levels of Cor and NSE, and IR index in cerebral hemorrhage group were higher than those in control group (P<0.05).Cor level in moderate group and severe group was higher than that in mild group (P<0.05).NSE level in severe group was higher than that in mild group and moderate group (P<0.05).IR index in moderate group and severe group was higher than that in mild group (P<0.05).The levels of Cor and NSE, and IR index were positively correlated with NIHSS score (r=0.325, r=0.530, r=0.336, P<0.05).The levels of Cor and NSE, and IR index in death group were higher than those in survival group (P<0.05).The high-level Cor (β=0.853, OR=2.347, 95%CI = 1.286-4.283), high-level NSE (β=0.469, OR=1.598, 95%CI=1.258-2.030) and increased IR index (β=0.543, OR=1.721, 95%CI=1.112-2.665) were positively correlated with unfavorable prognosis.Conclusion: The levels of plasma Cor and NSE, and IR are closely related to disease severity and prognosis of HCH patients.

1413 Analysis of Pathogen Distribution and the Correlation Between Serum Inflammatory Factors and Disease Condition and Prognosis in Diabetic Foot Patients

Objective: To analyze the distribution of pathogenic bacteria in diabetic foot patients and the correlation between serum inflammatory factors and disease condition and prognosis.Methods: The clinical data of 86 diabetic foot patients admitted to Jinan Central Hospital from January 2018 to May 2019 were selected and divided into mild infection group (30 cases), moderate infection group (33 cases) and severe infection group (23 cases) according to the infection degree of diabetic foot.As to the outcome of treatment, 28 cases were cured, 44 cases were improved and 14 cases were ineffective.At the same time, 78 health controls were made the control group.The distribution of pathogenic bacteria in diabetic foot patients was analyzed.The levels of serum procalcitonin (PCT), high sensitivity C-reactive protein (hs-CRP) and interleukin -6(IL-6) in each group were compared.Results: A total of 102 strains of pathogenic bacteria were isolated from 86 patients with diabetic foot.Gram-negative bacteria were dominant, mainly Pseudomonas aeruginosa and Acinetobacter baumannii, followed by Gram-positive bacteria, mainly Staphylococcus aureus and Staphylococcus epidermidis.The levels of serum inflammatory factors in diabetic foot infection groups with different degrees were higher than those in the control group, and the difference was statistically significant (P<0.05).The level of serum inflammatory factors in diabetic foot severe infection group was significantly higher than that in moderate infection group and mild infection group (P<0.05).The levels of serum inflammatory factors in patients with diabetic foot failure were significantly higher than those in the cured and improved groups, and the difference was statistically significant (P<0.05).Conclusion: The distribution of pathogenic bacteria in diabetic foot patients is dominated by gram-negative bacteria, and the level of serum inflammatory factors increases significantly with the aggravation of infection degree.In addition, the level of serum inflammatory factors can reflect the clinical effectiveness of diabetic foot patients.

1417 Comparison of the Effects of Solid Ankle-Foot Orthosis and Tuned Solid Ankle-Foot Orthosis on Abnormal Gait and Serum VEGF and CK-BB Levels in Children with Cerebral Palsy

Objective: To investigate the effects of solid ankle-foot orthosis and tuned solid ankle-foot orthosis on abnormal gait in children with cerebral palsy.Methods: 80 children with cerebral palsy treated in our hospital from June 2017 to June 2019 were selected and randomly divided into observation group (n = 40) and control group (n = 40) with simple random number table method.The control group was equipped with solid ankle-foot orthosis and the observation group was equipped with tuned solid ankle-foot orthosis.The spatial parameters, limb kinematic parameters, limb kinetic parameters and serum levels of vascular endothelial growth factor (VEGF) and creatine kinase brain isoenzyme (CK-BB) were observed and compared between the two groups.Results: Compared with the control group, the observation group had higher walking frequency and speed, higher range of motion of knee joint, lower flexion angle of knee joint touching the ground for the first time, larger maximum extension angle of standing knee joint and maximum dorsiflexion angle of ankle joint during swing, larger maximum flexion moment of knee joint, smaller extension moment, larger maximum metatarsal flexion moment of ankle joint, smaller dorsal flexion moment, lower serum VEGF and CK-BB levels.The difference was statistically significant (P<0.05).Conclusion: Wearing tuned solid ankle-foot orthosis is better than solid ankle-foot orthosis in improving abnormal gait (more stable and coherent) with lower levels of serum VEGF and CK-BB in children with cerebral palsy, which is worth promotion in clinical practice.

1421 Curative Effect of Norepinephrine Combined with Milrinonein Treating Septic Shock of Children and the Effects on Serum Levels of α-HBDH PCT Hs-CRP

Objective: To study the curative effect of norepinephrine combined with milrinone in treating children with septic shock and the effects on serum levels of α-hydroxybutyrate dehydrogenase (HBDH), procalcitonin (PCT), hs-CRP.Methods: 100 children with septic shock admitted to our hospital from May 2017 to May 2019 were selected for the study, and were divided into the observation group (n=51) and the control group (n=49) by the random number table method.The control group was treated with norepinephrine, and the observation group was treated with norepinephrine and milrinone.The clinical effectiveness, serum α - HBDH, PCT, hs CRP, PaCO2, pH, acute physiological and chronic health status scoring system II (APACHE II), sequential organ failure assessment (SOFA), admission time to intensive care unit (ICU) and 28-day mortality were compared between the two groups.Results: After treatment, the total effective rate of the observation group was 96.08%, significantly higher than that of the control group (79.59%) (P<0.05).After treatment, the serum levels of α- HBDH, PCT and hs-CRP in both groups were improved, and the serum levels of α- HBDH, PCT and hs-crp in the observation group were significantly lower than those in the control group (P<0.05).Before treatment, there was no significant difference in blood gas level between the two groups.After treatment, the blood gas indexes of the two groups were improved, and the PaCO2 and pH levels of the observation group were significantly lower than those of the control group (P<0.05).After treatment, APACHE Ⅱ, SOFA score level of the two groups are improved, and in the observation group, APACHE Ⅱ, SOFA scores were significantly lower than control group (P<0.05).The ICU admission time, 28d mortality and total mortality of the observation group were significantly lower than those of the control group, with significant differences (P<0.05).Conclusion: Norepinephrine combined with milinone has a significant effect on the serum levels of α- HBDH, PCT and hs-CRP in children with septic shock.

1425 The Correlation Between Serum APN Leptin 8-OHdG Levels and Pediatric Epilepsy Seizures Cognitive Impairment

Objective: To explore the connection between serum adiponectin (APN), leptin, 8-hydroxy-deoxyguanosine (8-OHdG) levels and pediatric epilepsy seizures, cognitive impairment.Methods: A total of 122 children with pediatric epilepsy who were admitted to the hospital from January 2017 to December 2019 were enrolled as study objects.According to children's cognitive function, they were divided into cognitive normal epilepsy group (82 cases) and cognitive impairment epilepsy group (40 cases).Sixty normal children who underwent physical examination in the same period were included in healthy control group.Cognitive function was evaluated by Wechsler Intelligence Scale for Children-IV(WISC-IV).The levels of serum APN, leptin and 8-OHdG were detected.The connection of serum APN, leptin and 8-OHdG levels with cognitive function in epilepsy children was analyzed.Logistic regression analysis was performed on cognitive impairment in epilepsy children.Results: In cognitive impairment epilepsy group, the level of serum APN was lower than that in cognitive normal epilepsy group and healthy control group (P<0.05), while level of leptin and 8-OHdG were higher (P<0.05).In cognitive normal epilepsy group, the level of serum APN was lower than that in healthy control group (P<0.05), while levels of leptin and 8-OHdG were higher (P<0.05).The level of serum APN was positively connected with the score of WISC-Ⅳ (r=0.385, P<0.05),while levels of leptin and 8-OHdG were negatively connected with the score of WISC-Ⅳ (r=-0.533, r=-0.428, P<0.05).The increase of APN (OR=0.583) is negatively connected with cognitive impairment.The increase of leptin (OR=1.826) and 8-OHdG (OR=2.849) is positively connected with cognitive impairment.Conclusion: The levels of serum APN, leptin and 8-OHdG are closely connected to seizures of pediatric epilepsy, which are also important factors that cause cognitive impairment.

1429 Lyophilized Recombinant Human Brain Natriuretic Peptide and Sodium Nitroprusside on Acute Decompensated Heart Failure and Influence on the CTnI NT-ProBNP and Hs-CRP Levels

Objective: To analyze and compare the clinical effect on acute decompensated heart failure (ADHF) between lyophilized recombinant human brain natriuretic peptide (Lrh-BNP) and sodium nitroprusside (SNP) and influence on the plasma cardiac troponin I (cTnI), N-terminal pro-brain natriuretic peptide (NT-proBNP) and hypersensitivity C reactive protein (hs-CRP) levels.Methods: 98 patients with ADHF admitted to our hospital from May 2015 to May 2018 were selected and evenly divided into two groups with simple random number table method.Lrh-BNP group was treated with Lrh-BNP while SNP group was treated with SNP.The clinical effect, changes of heart function indexes and plasma cTnI, NT-proBNP and hs-CRP levels before and after treatment as well as the incidence of adverse reactions were compared between the two groups.Results: The overall effective rate of Lrh-BNP group after treatment for 7 days was evidently higher than that of SNP group (P<0.05).The LVEF values of both groups 7d after treatment were significantly increased as compared with those before treatment (P<0.01), and the LVEDD, LVESD values, plasma cTnI, NT-proBNP and hs-CRP levels were significantly decreased as compared with those before treatment (P<0.05).The LVEF value of Lrh-BNP group was higher than that of the SNP group (P<0.05), while the LVEDD, LVESD values, plasma cTnI, NT-proBNP and hs-CRP levels of Lrh-BNP group were obviously lower than those of the control group(P<0.01).No difference was found between the two groups in the incidence of adverse reactions(P>0.05).Conclusion: Compared with SNP, Lrh-BNP could obviously enhance the clinical efficacy in the treatment of ADHF, improve the cardiac function, which might be related to decrease the plasma levels of cTnI, NT-proBNP and hs-CRP.

1434 Effects of High-Dose Ambroxol Hydrochloride Combined with Fiberbronchoscope Lavage on Oxygenation Index Serum CRP and PCT Levels in Elderly Patients with Severe Pneumonia

Objective: To explore the fiberbronchoscope value of high-dose ambroxol hydrochloride combined with fiberbronchoscope lavage in elderly patients with severe pneumonia (SP).Methods: 90 elderly patients with SP treated in the hospital between July 2016 and July 2019 were divided into the single medication group and the combined medication group with the computer-assisted random number method.They were treated with high-dose ambroxol hydrochloride and high-dose ambroxol hydrochloride combined with bronchoscopic lavage, respectively.The curative effect and related indicators in the two groups were evaluated.Results: The total effective rate of treatment in the combined medication group (97.78%) was significantly higher than that in the single medication group (82.22%)(P<0.05).The arterial partial pressure of oxygen (PaO2), degree of blood oxygen saturation (SaO2) and oxygenation indexes in the two groups increased significantly after treatment (P<0.05), while levels of C-reactive protein (CRP) and procalcitonin (PCT) decreased significantly (P<0.05).The increase or decrease in the combined medication group was significantly greater than that in the single medication group (P<0.05).The APACHE Ⅱ score, duration of mechanical ventilation, lung symptom disappearance time and length of intensive care unit (ICU) stay of the combined medication group after treatment were significantly lower or shorter than those of the single medication group (P<0.05).The incidence rate of multiple organ failure (MOF) and 28-day mortality rate in the combined medication group (15.56% and 6.67%) were significantly lower than those in the single medication group (37.78% and 24.44%)(P<0.05).Conclusion: High-dose ambroxol hydrochloride combined with bronchoscopic lavage is effective in the treatment of elderly SP.It can obviously improve the oxygenation index, improve the condition, and reduce inflammation and mortality rate.

1438 The Application of Iohexo of Different Concentrations in Arterial Pase Dsplay with Enhanced Abdominal CT

Objective: To compare the application of iohexol of different concentrations in arterial phase display with enhanced abdominal CT.Methods: 289 patients with suspected abdominal lesions from March 2016 to February 2019 in the First People's Hospital of Xining City were retrospectively selected.According to the mass concentration of iohexol, they were divided into two groups: low concentration group (300mg/mL, n=145) and high concentration group (350 mg/mL, n = 144).The scanning time, image quality and adverse events of the two groups were statistically analyzed.Results: The differences of trigger time of arterial phase and actual scanning time between the two groups were not statistically significant (P>0.05), nor in the CT value of abdominal aorta (P>0.05).The differences of noise and signal-to-noise ratios of abdominal aorta in arterial phase between the two groups were not statistically significant (P>0.05).The incidence of adverse events was 1.4% (2/145) in the low concentration group and 5.6% (8/144) in the high concentration group.The former was significantly lower than the latter, and the difference was statistically significant (P<0.05).Conclusion: The application effect of iohexol of low concentration in arterial phase display with enhanced abdominal CT is safer than iohexol of high concentration, so it is worthy of promotion.

1441 The Correlation Between CD8+/FOXP3+ Ratio PD-L1 Expression in Tissues and Prognosis in Patients Undergoing Radical Operation of Esophageal Squamous Carcinoma

Objective: To explore the correlation between CD8+/FOXP3+, expression of programmed death ligand 1 (PD-L1) in tissues and prognosis of patients undergoing radical operation of esophageal squamous carcinoma (ESCC).Methods: During the period from January 2014 to January 2019, 110 archived wax specimens and 110 wax specimens of normal tissues adjacent to the cancer (longer than 5 cm away from lesion, confirmed as cancer-free infiltration) from patients who underwent radical operation of ESCC in the hospital were enrolled as study objects.The expression of CD8+, FOXP3+ and PD-L1 in ESCC tissues was detected by immunohistochemistry, and the relationship between pathological parameters and prognosis was analyzed.Results: The positive expression rate of CD8+/FOXP3+ in ESCC tissues was lower than that in adjacent tissues, while the positive expression rate of PD-L1 was higher than adjacent tissues (P<0.05).Univariate and multivariate analysis showed that TNM stage T3, lymph node metastasis, positive expression of PD-L1 and negative expression of CD8+/FOXP3+ were independent risk factors affecting prognosis of ESCC patients (P<0.05).Conclusion: Positive expression of CD8+/FOXP3+, negative expression of PD-L1, TNM stage T3 and lymph node metastasis are independent risk factors affecting prognosis of patients after radical operation of ESCC.

1446 Curative Effectiveness of Gamma Globulin and Azithromycin in Treating Severe Pediatric Mycoplasma Pneumoniae Pneumonia and Influence on the Levels of CRP T lymphocyte Subsets and Immunoglobulin

Objective: To explore the curative effectiveness of gamma globulin and azithromycin in treating severe pediatric mycoplasma pneumoniae pneumonia, and to analyze their effects on the levels of CRP, T lymphocyte subsets and immunoglobulin.Methods: 190 cases admitted in our hospital from February 2017 to February 2019 were selected prospectively and randomly divided into 2 groups with random number table method.The control group (n=95) was treated with azithromycin, and the observation group (n=95) was administered gamma globulin combined with azithromycin.Records were made of clinical effect of two groups (time index, pulmonary function index).Analysis was made on changes of CRP, immunoglobulin and T cells before and after interventional treatment.And the security of both treatments was compared.Results: Clinical effect was found better for the observation group, whose fever clearance time, the disappearance time of cough and pulmonary rales ,and length of hospital stay were respectively (2.58±0.78)d, (2.47±0.63)d, (3.95±1.15)d, (7.87±1.28) d, significantly lower than in the control group (3.72±1.31)d, (4.84±1.41) d, (6.35±2.25) d, (10.28±3.28)d, and the difference was statistically significant (P<0.05).FEV1, FVC and MMEF in the observation group were (65.41±6.14) %, (60.30±5.43) % and (65.96±7.19)%, respectively, which were significantly higher than those in the control group (51.67±5.46) %, (55.55±6.74) % and (52.25±7.25) %, with statistically significant differences (P<0.05).CRP and CD8+ were significantly decreased in both groups after treatment, and CD4+, CD4+/CD8+, IgA, IgG and IgM were significantly increased (P<0.05).After treatment, CRP and CD8+ in the observation group were (10.86±5.72) mg/L and (28.64±2.44) %, respectively, which were significantly lower than those in the control group (19.32±6.09) mg/L and (30.80±4.30) %, with statistically significant differences (P<0.05).CD4+, CD4+/CD8+, IgA, IgG and IgM of the observation group were (49.39±6.95) %, (1.88±0.65) %, (2.95±0.49) g/L, (13.37±2.80) g/L, (1.63±0.38) g/L, respectively, higher than the control group (44.87±6.31) %, (1.51±0.53) %, (2.41±0.48) g/L, (11.06±2.18) g/L, (1.34±0.38) g/L, and the differences were statistically significant (P<0.05).In addition, the incidence of adverse reactions in both groups was compared, and the difference was not statistically significant (P>0.05).Conclusion: Immunoglobulin combined with azithromycin can significantly improve the clinical effect, improve the function of T-lymphocyte subsets and increase the expression level of immunoglobulin in children.

1450 Correlation of IL-1β and IL-33 Expression with Syphilis Cellular Immunity in Early Syphilis Patients

Objective: To study the correlation between expression of interleukin-1β (IL-1β), interleukin-33 (IL-33) and syphilis cellular immunity in patients with early syphilis.Methods: Fifty patients with early syphilis (observation group) who were admitted to the hospital from July 2018 to June 2019 were enrolled as study objects.Another 50 healthy volunteers were enrolled as control group.The levels of serum IL-1β and IL-33 in both groups were detected by enzyme-linked immunosorbent assay.Flow cytometry was applied in order to detect levels of T lymphocyte subsets (CD3+, CD4+, CD8+) and NK cells in peripheral blood.According to measured serum IL-1β and IL-33 levels in patients with early syphilis, they were divided into high-level IL-1β group, low-level IL-1β group, high-level IL-33 group and low-level IL-33 group by dichotomy.The levels of CD3+, CD4+, CD8+ and NK cells in peripheral blood were compared among all groups.Pearson correlation analysis was applied in order to analyze correlation of IL-1β and IL-33 expression with cellular immunity in patients with early syphilis.Results: The levels of serum IL-1β and IL-33 in observation group were higher than those in control group (P<0.05).The levels of CD3+, CD4+, CD8+ and NK cells in peripheral blood in observation group were lower than those in control group (P<0.05).The levels of CD3+, CD4+, CD8+ and NK cells in peripheral blood in high-level IL-1β group were lower than those in low-level IL-1β group (P<0.05).The levels of CD3+, CD4+, CD8+ and NK cells in peripheral blood in high-level IL-33 group were lower than those in low-level IL-33 group (P<0.05).Pearson correlation analysis showed that levels of serum IL-1β and IL-33 were negatively correlated with levels of CD3+, CD4+, CD8+ and NK cells in peripheral blood in early syphilis patients (P<0.05).Conclusion: The levels of serum IL-1β and IL-33 are increased in early syphilis patients, which are negatively related to cellular immunity.Changes in IL-1β and IL-33 may be of certain regulatory effect on cellular immunity.

Objective: To investigate the effects of butylphthalide soft capsule combined with brain protein hydrolysate on neurological function (NIHSS) score and Keap1-Nrf2/ARE signaling pathway in patients with acute cerebral infarction (ACI).Methods: From August 2016 to July 2019, 93 patients with ACI in our hospital were selected as the research objects, and they were simply randomized into 3 groups, with 31 cases in each.Besides conventional treatment, control group A was also treated with brain protein hydrolysate, and the control group B was additionally treated with butylphthalide soft capsules, and the observation group was treated with protein hydrolysate combined with butylphthalide soft capsule.The clinical efficacy and vascular pseudohemophilic factor (VWF), adiponectin (APN), interleukin (IL)-8, IL-19, tumor necrosis factor (TNF)-α, superoxide Dismutase (SOD), malondialdehyde (MDA), Keap1-Nrf2/ARE signalling pathway-related protein levels, NIHSS scores, and daily living (Barthel index) of the three groups were compared statistically.Results: The clinical efficacy of the observation group was higher than that of control groups A and B (P<0.05).After the course of treatment, the NIHSS score of the observation group was lower than those of control groups A and B, and the Barthel index was higher than that of control groups A and B (P<0.05).After the treatment course, the serum VWF and MDA levels in the observation group were lower than those in the control groups, and the APN and SOD levels were higher than those in the control groups (P<0.05).After the course of treatment, the serum levels of IL-19, IL-8 and TNF-α in the observation group were lower than those in the control groups (P<0.05).After the treatment course, the expression level of Keap1 in the observation group was lower than that in the control groups, and the expression levels of NQO1, ARE, and Nrf2 were higher than those in the control groups (P<0.05).Conclusion: Butylphthalide soft capsule combined with brain protein hydrolysate for the treatment of ACI can obviously improve the vascular endothelial function of patients, inhibit oxidative stress response and inflammatory response, regulate Keap1-Nrf2/ARE signaling pathway related proteins, improve neural function and ability of daily living.The effectiveness is significant.

Objective: To analyze the effects of local infiltration anesthesia with dezocine and ropivacaine on perioperative immune function, stress response and postoperative pain in patients with breast cancer.Methods: The clinical data of 74 patients with breast cancer admitted to our hospital from March 2018 to November 2019 and confirmed by surgery or puncture biopsy were retrospectively analyzed.According to the different medication administered, they were randomly divided into a control group and a study group, with 37 cases in each group.The control group was administered ropivacaine for local infiltration anesthesia.The study group was supplemented with dezocine.Peripheral blood T lymphocyte subsets, natural killer (NK) cell levels, and stress indexes were compared between the two groups at different points.Visual analogue scale(VAS) scores were used to assess pain 1h, 6h, and 24h after surgery.Results: The levels of CD3+, CD4+ cells, and NK cells in the two groups 1h, 24h, and 48h after surgery were significantly lower than those before anesthesia , and the levels were reduced to the lowest 24 hours after surgery, which was significantly different from that before anesthesia (P<0.05).The levels of CD3+, CD4+ cells, and NK cells in the study group were higher than those in the control group 1 hour, 24 hours, and 48 hours after surgery (P<0.05).The levels of stress indicators Cor, AngⅡ, and IL-6 were not different from those before anesthesia.In the control group, the levels of stress indicators Cor, AngⅡ, and IL-6 1h, 24h, and 48h after surgery were higher than those before anesthesia (P<0.05).And the levels of stress indicators Cor, AngⅡ, and IL-6 in the study group were significantly lower than those in the control group 1h, 24h, and 48h after surgery (P<0.05).And the VAS scores 1h, 6h, and 24h after surgery were lower than those in the control group(P<0.05).Conclusion: Perioperative anaesthesia with dizocine and ropivacaine in patients with breast cancer can effectively reduce the stress response caused by surgical trauma, protect peripheral blood T lymphocyte subsets and NK cells, restore the body's immune function, and reduce postoperative pain.

1464 Effects of Endoscopic Thyroidectomy via Breast Areola Approach on Stress Related Hormones in Patients with Benign Thyroid Lesions

Objective: To analyze the effects of endoscopic thyroidectomy via breast areola approach on stress related hormones in patients with benign thyroid lesions.Methods: The clinical data of 82 patients with benign thyroid lesions treated in the hospital from January 2016 to October 2018 were collected.40 cases treated with endoscopic thyroidectomy via breast areola approach were included into the laparoscopic group while 42 cases treated with open thyroidectomy were included into the open group.Changes of stress and related hormones such as serum cortisol (Cor), norepinephrine (NE), adrenocorticotropic hormone (ACTH), thyroid stimulating hormone (TSH), thyroid hormones (TT3, FT3, TT4, FT4) before and after operation were compared between the two groups.Changes of visual analogue scale (VAS) scores at different time points after operation were recorded.The operation time, length of incision, intraoperative blood loss, length of hospital stay, and drainage volume were compared between the two groups.The incidence of postoperative complications was statistically analyzed.Results: ①Before operation, there were no statistically significant differences between the two groups in levels of Cor, NE and ACTH (P>0.05).12 hours after operation, the levels of stress hormones increased in the two groups (P<0.05).But the levels of the hormones and the pre- and post-operation difference were lower in the laparoscopic group than in the open group (P<0.05).②There were no statistically significant differences between the two groups in levels of TSH, TT3, FT3, TT4 and FT4 before operation (P>0.05).12 hours after operation, TSH levels decreased while TT3 and FT3 levels increased in the two groups, which was statistically significant compared with pre-operation data (P<0.05), but differences between groups were not statistically significant (P>0.05).③Compared with the open group, the operation time was longer, the length of incision was shorter, the intraoperative blood loss, length of hospital stay, drainage volume, VAS scores at different time points after operation and the incidence of complications were less, shorter or lower in the laparoscopic group (P<0.05).Conclusion: The application of endoscopic thyroidectomy via breast areola approach in the treatment of benign thyroid diseases can reduce postoperative stress reactions and improve the thyroid function.The intraoperative blood loss is little and patients can recover quickly after operation, and the incidence of complications is low.

1469 Protective Effect of Edaravone on Nerve Function and Its Relationship with Serum D-Dimer and Total Bilirubin Levels in Patients Undergoing Intracranial Aneurysm Clipping

Objective: To explore the effects of edaravone on nerve function, serum D-dimer (D-D) and total bilirubin (TBil) levels in patients undergoing intracranial aneurysm clipping, and to analyze correlation between nerve function and serum D-D, TBil levels after treatment.Methods: Sixty patients who underwent intracranial aneurysm clipping from January 2018 to September 2019 were enrolled and divided into observation group and control group with the simple random grouping method, 30 cases in each group.Both groups were given routine treatment of albumin, dopamine, low-molecular dextran, mannitol) for 10 days.On this basis, observation group was given edaravone for 14 days.NIHSS score, levels of serum D-D and TBil, and prognosis were compared between the two groups.Results: The NIHSS scores, D-D and TBil levels in the observation group before treatment and 14d after treatment were higher than those in control group (P<0.05).14d after treatment, serum D-D level was positively correlated with NIHSS score (r=0.426, P<0.05), while serum TBil level was negatively correlated with NIHSS score (r=-0.359, P<0.05).3 months after treatment, good rate of prognosis in observation group was higher than that in control group (70.00% vs 43.33%) (P<0.05).Conclusion: Edaravone treatment after intracranial aneurysm clipping can protect nerve function, which is related to changes in serum D-D and TBil levels.

1473 Effects of Different Plate Internal Fixation Techniques on Serum SP, NPY Levels and Knee Joint Function in Patients with Tibial Plateau Fractures

Objective: To investigate the effects of different plate internal fixation techniques on serum substance P (SP), neuropeptide (NPY) levels and knee joint function in patients with tibial plateau fractures.Methods: From January 2017 to September 2018, 74 patients with tibial plateau fractures in our hospital were prospectively studied and randomly categorized into two groups with random number table method, 37 cases in each.Group A underwent minimally invasive internal fixation system (LISS) plate technology, and group B underwent anatomical plate internal fixation.The operative indexes, complication rate, perioperative pain stress (serum SP, NPY), varus angle, caster angle, knee joint range of motion and distinguished achiever ratio of knee joint function recovery at three months, six months and twelve months after the operation were compared.Results: The operation time, intraoperative blood loss, and postoperative drainage volume of group A were lower than those of group B, and the length of the incision was shorter than that of group B (P<0.05); serum SP and NPY levels in group A were lower than those in group B at the first, third, and seventh days after surgery (P<0.05); there was no significant difference between the two groups after three, six, and twelve months of varus angle, caster angle, knee joint range of motion (P>0.05); the excellent recovery rate of knee function in group A (81.08%) was not significantly different from that in group B (78.38%) (P>0.05); there was no significant difference in the incidence of complications between group A and group B (P>0.05).Conclusion: The anatomical plate internal fixation technology and LISS plate internal fixation technology have high safety and good knee joint function recovery.However, compared with the anatomical plate internal fixation technique, the LISS plate internal fixation technique has a shorter incision, which can obviously reduce the amount of intraoperative bleeding in patients with tibial plateau fractures, reduce the postoperative pain stress response, and accelerate the postoperative rehabilitation process, with high clinical promotion value.

1478 Efficacy of Intravitreal Injection of Ranibizumab on Neovascular Age-Related Macular Degeneration and Its Effects on Serum CRP VEGF Intraocular Pressure and Visual Acuity

Objective: To investigate the efficacy of intravitreal injection of Ranibizumab in the treatment of neovascular age-related macular degeneration (nAMD) and its effects on serum C-reactive protein (CRP), vascular endothelial growth factor (VEGF), intraocular pressure and visual acuity of patients.Methods: Seventy-four patients with nAMD in our hospital were selected as the research subjects, and they were categorized as the Ranibizumab group and the Conbercept group with random number table method, with 37 cases in each group.The two groups were treated by intravitreal injection.The visual acuity [best corrected visual acuity (BCVA)], central retinal thickness (CRT), intraocular pressure and levels of serum CRP and VEGF before treatment and 4 weeks and 12 weeks after treatment and choroidal neovascularization (CNV) leakage at 12 weeks after treatment were compared between the two groups, and the clinical efficacy was evaluated, and the occurrence of complications were recorded during treatment.Results: There was no significant difference in intraocular pressure before and after treatment between the groups and within the groups (P>0.05).Comparison of visual acuity, CRT, serum CRP and VEGF before and after treatment in the two groups all showed before treatment> at 4 weeks after treatment>at 12 weeks after treatment (P<0.05).The CRT, serum CRP and VEGF in Ranibizumab group 4 and 12 weeks after treatment were lower than those in Conbercept group (P<0.05).There was no significant difference in visual acuity between the two groups 4 and 12 weeks after treatment (P>0.05).There were no significant differences in clinical efficacy and occurrence of complications during treatment between the two groups (P>0.05).Conclusion: In the case of intravitreal injection for nAMD, the efficacy and safety of Conbercept are comparable to those of Ranibizumab, but Ranibizumab has significant advantages in improving CRT and reducing expressions of CRP and VEGF, and it has high clinical application value.

Objective: To analyze the curative effect of periodontal diorthosis in dental repair and its influences on periodontal indexes, dental inflammatory factors in peripheral gingival crevicular fluid (GCF) and the functional score.Methods: During the period from January 2017 to December 2018, patients who underwent elective oral repair and met inclusion and exclusion criteria in stomatology department of the hospital were divided into observation group (periodontal diorthosis before dental repair, n=41) and control group (routine oral repair, n=52) according to non-randomized clinical concurrent control study and voluntary principle of patients.The control group underwent routine oral repair, while observation group underwent periodontal diorthosis before dental repair on basis of control group.The curative effect, periodontal indexes, dental inflammatory factors in peripheral GCF and functional scores before and after treatment were compared between the two groups.Results: ①There were significant differences in curative effect grading of oral repair between the two groups.The total response rate in observation group was significantly higher than that in control group (P<0.05).②After treatment, probe depth (PD), periodontal attachment level (AL), dental plaque percentage and blooding of prob (BOP) were significantly decreased in both groups (P<0.05).PD, AL, BOP and PLI in observation group were significantly lower than those in control group (P<0.05).③After treatment, dental inflammatory factors in peripheral GCF were significantly decreased in both groups, which were significantly lower in observation group than in the control group (P<0.05).④After treatment, occlusion time (OT), bite force distribution balance between the left and right sides (BFDB) and Standard deviation of hue (SDHue) were decreased in both groups (<0.05), while maximum area frame bite force/max movie force (MABF/MMF) was increased (P<0.05).OT, BFDB and SDHue in observation group were lower than those in control group, while MABF/MMF was higher than those in control group.There were no complications during treatment in either group.Conclusion: Compared with routine oral repair, curative effect of periodontal diorthosis is better in dental repair, whose improvement was more significant on periodontal indexes, dental inflammatory factors in peripheral GCF and function.

1487 Effect of B-Ultrasound Guided Lower Plexus and Sciatic Nerve Block on VEGF Level and Cognitive Function in Elderly Patients with Intertrochanteric Fracture of Femur

Objective: To investigate the effect of B-ultrasound guided lower plexus - sciatic nerve combined block on vascular endothelial growth factor (VEGF) level and cognitive function in elderly patients with intertrochanteric fracture of femur.Methods: A total of 200 elderly patients with intertrochanteric fractures diagnosed and treated in our hospital from June 2018 to June 2019 were selected as the subjects.With the random number table method, they were randomly divided into two groups, namely the study group (100 cases) and the control group (100 cases).Artificial femoral head replacement was applied in both groups.In the study group, B-ultrasound guided lower plexus and sciatic nerve combined block anesthesia was applied, while in the control group, subarachnoid block combined with epidural anesthesia was applied.The effects of two anesthesia methods on VEGF level and MMSE in elderly patients with intertrochanteric fractures were compared .Results: Repeated measures analysis of variance showed that there were statistically significant differences in MMSE scores, VEGF levels at time points, between groups, and between time points and groups (P<0.05).In addition, MMSE score and VEGF level of the patients in the study group were higher than those in the control group 1d, 3d and 7d postoperatively, and the difference between the groups was statistically significant (P<0.05).At the same time, the incidence of cognitive impairment on 1d, 3d and 7d after the operation of the two groups gradually decreased, and the study group was lower than the control group.The difference between the two groups was not statistically significant (χ2=2.606, P>0.05), and the time point difference was statistically significant (χ2=9.988, P<0.05).The other two groups were accompanied by nausea, vomiting, urine retention, headache and other adverse reactions after surgery, but the total incidence of the study group was lower than that of the control group, with no statistical significance (χ2=2.000, P>0.05).Conclusion: B-ultrasound guided lower plexus combined with sciatic nerve block anesthesia has a faster onset and a longer duration of anesthetic effect, which can effectively reduce the intraoperative pain in elderly patients with intertrochanteric fracture, reduce postoperative cognitive impairment in elderly patients, and promote VEGF secretion to a certain extent, which has a positive effect on postoperative rehabilitation.

Objective: To analyze the expression level, functions, and regulatory mechanisms of long non-coding RNA LINC01133 in laryngeal squamous cell carcinoma (LSCC).Methods: The relative expression levels of LINC01133 in 48 LSCC tissues were measured by qRT-PCR.And the associations between LINC01133 expression levels and clinic-pathological features were analyzed.After that, a series of in vitro assays were adopted to detect the effects of LINC01133 on cellular proliferation, migration, invasion ability and epithelial-mesenchymal transformation.Results: Compared to control groups, LINC01133 was highly expressed in LSCC tissues and cell lines (P<0.05).Correlation analyses showed that the expression levels of LINC01133 were associated with differentiation degree, clinical stage, and cervical lymph node metastasis (P<0.05).Besides, in vitro assays showed that LINC01133 could enhance the proliferation, migration, invasion ability and epithelial-mesenchymal transformation of LSCC cell lines.Conclusion: LINC01133 is highly expressed in LSCC tissues and cell lines, and related to the clinical stage, differentiation degree, and neck lymph node metastasis.And, LINC01133 could promote the malignant phenotype and epithelial-mesenchymal transformation of LSCC cells, which is of great significance in the occurrence and development of LSCC.

1499 Detection of Peripheral Blood Neuropeptide Y Cholecystokinin and LEP in Children with Anorexia and its Clinical Significance

Objective: To investigate the peripheral blood neuropeptide Y (NPY),cholecystokinin (CCK),and leptin (LEP) levels and their clinical significance in children with anorexia.Methods: A total of 105 children with anorexia pediatric patients treated in our hospital from January 2017 to August 2019 were prospectively selected as the observation group,and 102 children with healthy physical examinations in the same period were selected as the control group.Compared serum NPY,CCK,LEP levels and clinical symptom scores between two groups and children with different levels of illness,and analyzed the correlation between serum NPY,CCK,LEP and clinical symptoms,according to the average expression of serum NPY,CCK,and LEP,the patients were divided into high-expressed and low-expressed patients,the efficacy of children with different levels of serum NPY,CCK,and LEP was compared,and the relationship between the three was analyzed.Results: The serum NPY in the observation group was lower than that in the control group,and CCK and LEP were higher than those in the control group (P<0.05); serum NPY was lower in severe children than in moderate and mild children,and CCK and LEP were higher than in moderate and mild children (P<0.05); severe children with reduced food intake,fatigue,and fatigue scores were higher than those with moderate and mild children (P<0.05); serum NPY was negatively correlated with reduced appetite,fatigue,and fatigue scores,and CCK was positively correlated with reduced appetite,fatigue,and fatigue scores (P<0.05); the total effective rate of those with high expression of NPY in serum was higher than those with low expression,and the total effective rate of those with high expression of CCK and LEP was lower than those with low expression (P<0.05); serum NPY was positively correlated with curative effect,while CCK and LEP were negatively correlated with curative effect (P<0.05).Conclusion: The expressions of serum NPY,CCK and LEP are closely related to the severity of anorexia and treatment effect in children,clinical treatment can be formulated according to the expression of the above serum indicators to improve the treatment effect.

1505 Arthroscopic Reduction and Retrograde Cross-Kirschner Wire Fixation for Treatment of Tibial Intercondylar Dpine Fracture in Children

Objective: To observe the effect of arthroscopic reduction and retrograde cross-Kirschner wire fixation on tibial intercondylar spine fracture in children.Methods: A total of 80 children with tibial intercondylar spine fracture were treated in our hospital from October 2016 to October 2019.With the simple random number table method, they were categorized into a control group (40 cases) and an experimental group (40 cases).after arthroscopic reduction, two groups of children were treated with internal fixation.The control group was treated with hollow nail fixation, while the experimental group was treated with cross Kirschner wire fixation.After that, the two groups of children were followed up for 10 months.The operation time, bleeding volume, VAS score of pain and Lysholm score of knee joint function of the two groups before and 10 months after operation were compared.The excellent and good rate of knee joint recovery, complications and X-ray of knee joint were followed up.Results: The VAS, Lysholm score, length of operation, amount of bleeding during operation, and no fracture redisplacement or nonunion were found in both groups before operation, and there was no significant difference between the two groups (P>0.05).Compared with before treatment, the VAS scores of the two groups were significantly reduced 10 months after treatment, and the Lysholm scores were significantly improved (P<0.001).The indicators of the experimental group were more significantly improved than the control group (P<0.001).The excellent and good rate of knee joint recovery in the experimental group was 100.00%, significantly higher than that in the control group, 90.00% higher than that in the follow-up.The complication rate in the experimental group was 2.50%, significantly lower than that in the control group.The incidence of symptoms was 15.00%.The difference was statistically significant (P<0.05).Conclusion: In the treatment of tibial intercondylar spine fracture in children, arthroscopic reduction and internal fixation can significantly improve knee joint function, reduce the occurrence of post-operative fracture redistribution or nonunion.The treatment of retrograde cross-Kirschner wire fixation can obtain better knee joint function, relieve postoperative pain, improve the recovery of knee joint function and reduce recovery.The occurrence of complications during the second period is satisfactory.

Objective: To explore the effects of off-pump coronary artery bypass grafting (OPCABG) and on-pump coronary artery bypass grafting (ONCABG) on inflammatory factors and cardiac function in elderly patients with coronary heart disease (CHD) complicated with valvulopathy.Methods: The clinical data of 116 elderly patients with CHD complicated with valvulopathy who underwent coronary artery bypass grafting (CABG) between January 2015 and December 2017 were retrospectively analyzed.The patients were divided into OPCABG group (n=55) and ONCABG group (n=61) according to the surgical methods.The perioperative conditions (operative time, intraoperative blood transfusion volume, postoperative ventilator use time, postoperative hospital stay) were compared between the two groups, and the perioperative serum inflammatory factors [interleukin-6 (IL-6), interleukin-8 (IL-8), tumor necrosis factor α (TNF-α)] and cardiac function [cardiac troponin I (cTnI), creatine kinase (CK), creatine kinase isoenzyme (CK-MB)] were evaluated before operation and 1d and 3d after operation, and the incidence rate of postoperative complications was recorded in the two groups, and the incidence rates of major cardiovascular and cerebrovascular adverse events (MACE) were evaluated at 1 year of follow-up.Results: The operative time, intraoperative blood transfusion volume, postoperative ventilator use time and postoperative hospital stay in OPCABG group were all lower than those in ONCABG group (P<0.05).The inflammatory factors (IL-6, IL-8, TNF-α) and cardiac function (cTnI, CK, CK-MB) at 1d after operation and at 3d after operation were all increased first and then decreased (P<0.05), and the increases in OPCABG group were smaller than those in ONCABG group (P<0.05) while the decreases were greater than those in ONCABG group (P<0.05), and there were interaction effects between treatment methods and time of IL-6, IL-8 and TNF-α (P<0.05).In 1 year follow-up, there were no significant differences in the incidence rate of reoperation, cardiac death, myocardial infarction and stroke and total incidence rate of MACE between the two groups (P>0.05).Conclusion: OPCABG can improve perioperative status, postoperative inflammation and cardiac function in elderly patients with CHD complicated with valvulopathy, and it has similar prognosis with ONCABG.

Objective: To study the curative efficacy of different anesthesia methods radical gastrectomy and the effects on liver and kidney function and oxygenation function.Methods: 100 patients admitted to our hospital for radical gastrectomy from August 2016 to August 2019 were included in this study and divided into the observation group (n=53) and the control group (n=47) with simple random grouping method.The control group received general anesthesia, and the observation group received epidural block combined with general anesthesia.The two groups were compared with postoperative clinical indicators, cereal third transaminase (ALT), aspertate aminotransferase (AST), total bilirubin (TBIL), urine nitrogen (BUN), serum creatinine (Cr), pulmonary blood FenLiuLv (QS/QT), lactic acid (ABL), c-reactive protein (CRP) and interleukin 2 (IL 2) and interleukin 8 (IL - 8) level changes and the occurrence of adverse reactions.Results: The respiratory recovery time, eye-opening time, extubation time and restlessness score of the observation group were significantly lower than those of the control group (P<0.05); before operation, there was no significant difference in the level of liver and kidney function between the two groups; after operation, the liver and kidney function of the two groups were increased, but there was no significant difference in the level of BUN and Cr between the two groups; ALT, AST and TBIL of the observation group were significantly lower than those of the control group (P<0.05) Before operation, there was no significant difference in the level of oxygenation index between the two groups; after operation, the level of oxygenation index in the two groups increased, and the QS / QT and ABL levels in the observation group were significantly lower than those in the control group (P<0.05); before operation, there was no significant difference in the level of stress reaction between the two groups; after operation, the level of stress reaction in the two groups increased on average, and the level of CRP, IL-2 and IL-8 in the observation group were significantly lower than those in the control group (P<0.05).The total incidence of adverse reactions in the two groups was 5.66% and 19.15%, respectively, with significant difference (P<0.05).Conclusion: Epidural block combined with general anesthesia in patients with radical gastrectomy has a significant effect, and can effectively improve the liver function and oxygenation function level of patients, which is worthy of promotion and application.

1518 Effect Study of Microchannel Percutaneous Nephrolithotomy on Calculus Removal and Renal Function in Patients with Complicated Renal Calculi

Objective: To study the effect of micro-channel percutaneous nephrolithotomy (PCNL) on calculus removal and renal function in patients with complicated renal calculi.Methods: 121 patients with complicated renal calculi were divided into control group (n=58) and research group (n=63)with simple random method.The control group was treated with standard channel PCNL, while the research group was treated with micro-channel PCNL.The operation effects, stone-free rate, renal function before and after operation, and complications were compared between the two groups.Results: There was no significant difference in hospital stay between the two groups (P>0.05).The operation time in the research group was longer than that in the control group, and the intraoperative blood loss and visual analogue scale (VAS) 6 hours after operation were lower than those in the control group (P<0.05).The success rate of primary lithotripsy in the control group and the research group was 87.93%(51/58) and 82.54%(52/63), and the difference was not statistically significant (P>0.05).Before operation, there was no significant difference between the two groups in creatinine and urea nitrogen, and there was no statistical difference (P>0.05).After operation, creatinine and urea nitrogen in the two groups did not change significantly compared with corresponding data before operation, and there was no significant difference in creatinine and urea difference between the two groups before and after operation (P>0.05).Bleeding, fever, urinary fistula and organ injury occurred in both groups, and the total incidence rate between the two groups was not statistically significant (P>0.05).Conclusion: Microchannel PCNL has similar stone-free rate to standard channel PCNL in the treatment of complex renal calculi, and has no obvious effect on early postoperative renal function, less postoperative complications and more obvious advantages of minimally invasive surgery.

1522 Study of the Effects of Hemodialysis and Hemofiltration on Cardiovascular Stability in Patients with Chronic Renal Failure

Objective: To study the effects of hemodialysis (HD) and hemofiltration (HF) on cardiovascular stability in patients with chronic renal failure (CRF).Methods: A total of 104 CRF patients who underwent blood purification treatment from August 2018 to August 2019 were selected for the study and were randomly divided into CVVH group and HD group (each n=52).The two groups were given routine symptomatic support, and CVVH group was given CVVH on this basis, and HD group was given MHD, and the two groups were treated for 3 months.The dialysis effects, expression levels of inflammatory factors, hemodynamic indexes and prognosis were compared between the two groups.Results: After 3 months treatment, there were no significant differences in the GFR, Kt/V, nPCR and TACU between the two groups (P>0.05).Serum Scr and BUN levels were significantly decreased in the two groups (P<0.05), and the levels of serum PTH and β2-MG were significantly decreased in CVVH group (P<0.05), and the levels of PTH and β2-MG in CVVH group were lower than those in HD group (P<0.05).The serum IL-10 level in the two groups was significantly increased (P<0.05) while the levels of serum CRP and IL-6 levels were significantly decreased (P<0.05), and the level of IL-10 in CVVH group was higher than that in HD group while the levels of CRP and IL-6 were lower than those in HD group (P<0.05).The GEDVI The ITBVI and EVLWI were significantly decreased in the two groups (P<0.05), and the GVVI, ITBVI and EVLWI in CVVH group were lower than those in HD group (P<0.05).Results of 3 years follow-up showed that the incidence rates of CVD in CVVH group and HD group were 32.69% and 53.85% (P<0.05), and the cardiogenic death rates were 15.38% and 28.85% respectively (P>0.05).Conclusion: HD and CVVH can effectively promote the clearance of small molecule toxins in CRF patients, and CVVH has a good scavenging effect on medium macromolecular toxins, and it is beneficial to improve the body microinflammation states and maintain cardiovascular stability, thereby reducing the risk of adverse events such as CVD and cardiac death.

1528 Influential Factors of Anterior Chamber Depth and Anterior Chamber Exudation After Phacoemulsification and IOL Implantation

Objective: To investigate the changes of anterior chamber depth (ACD) after phacoemulsification cataract extraction (PE) combined with intraocular lens (IOL) implantation, and to analyze the influencing factors of anterior chamber (AC) exudation.Methods: The clinical data of 315 cataract patients (342 eyes) who underwent PE combined with IOL implantation in our hospital from January 2016 to December 2018 were selected, and the patients were divided into two groups according to whether AC exudation occurred after surgery.There were 19 patients (21 eyes) in the AC exudative group and 296 patients (321 eyes) in the AC non-exudative group.The ACD changes of cataract patients before and after 1 day, 1 week, 1 month, and 2 months were compared, and the influencing factors of AC exudation were analyzed by binary logistic regression.Results: The ACD at different time points after surgery was significantly higher than that before surgery, and the ACD at 2 months after surgery was significantly lower than that at 1 day, 1 week, and 1 month after surgery, with significant differences (P<0.05).As to intraoperative pupil diameter, history of alcohol abuse, smoking history, history of hypertension, history of heart disease, history of diabetes, history of prostatic hyperplasia, history of rheumatoid arthritis in the AC exudation group, the difference was not significant in the AC non-exudation group, which was not an influencing factor (P>0.05).High myopia, preoperative intraocular pressure (IOP), glaucoma, preoperative visual acuity, intracapsular rupture of the lens (PCR), and ultrasound cumulative energy, the composite parameters (AECP), uveitis, and lens nucleus classification were significantly different between the groups, all of which were factors influencing AC exudation (P<0.05).By binary logistic analysis, high myopia, preoperative IOP>21mmHg, glaucoma, intraoperative PCR, ultrasound AECP>7.25 (% × min), uveitis, and lens nucleus grading ≥ Ⅳ are risk factors for AC exudation ( P<0.05).Conclusion: ACD gradually increased after surgery, and it was close to normal 2 months after surgery.For patients with risk factors for postoperative AC exudation, preventive measures should be taken and treated in a timely manner.

Objective: To analyze the effects and safety of lanthanum carbonate in improving calcium and phosphorus levels in patients undergoing maintenance hemodialysis.Methods: A total of 92 patients who were expecting maintenance hemodialysis were divided into control group (n=46) and treatment group (n=46) with the simple random grouping method.Control group was treated with calcium carbonate, and treatment group was treated with lanthanum carbonate.After 2 months of treatment, levels of blood calcium, blood phosphorus, intact parathyroid hormone (iPTH) and serum 25-hydroxyvitamin D (25OHD) and changes of vascular endothelial function were compared between the two groups.Safety was evaluated.Results: There were no obvious changes in levels of blood calcium or iPTH before and after treatment (P>0.05), but blood phosphorus level was significantly decreased after treatment (P<0.05).The pre- and post-operative differences of blood calcium level and blood phosphorus level in treatment group were obviously greater than those in control group (P<0.05), but the pre- and post-operative difference of iPTH of the two groups was not statistically significant (P>0.05).The serum 25OHD level in treatment group was significantly increased after treatment (P<0.05), but there was no significant change in control group (P>0.05).The pre- and post-operative difference of serum 25OHD level in treatment group was greater than that in control group (P<0.05).The flow-mediated dilation (FMD) in treatment group was significantly increased after treatment (P<0.05) while the fibroblast growth factor-23 (FGF23) was significantly decreased after treatment (P<0.05).There were no obvious changes in FMD or FGF23 in control group before and after treatment (P>0.05).The pre- and post-operative differences of FMD and FGF23 in treatment group were greater than those in control group (P<0.05).At 1 week after the end of treatment, the review results of blood, liver and kidney function and electrocardiogram in the two groups were in normal range.There was no significant difference in the incidence rate of adverse reactions between the two groups (P>0.05).Conclusion: Effects of lanthanum carbonate are more obvious than calcium carbonate in the treatment of patients undergoing maintenance hemodialysis.The former can significantly reduce the blood phosphorus level, increase the serum 25OHD level and improve the vascular endothelial function, without causing changes in blood calcium level and with high safety.

Objective: To investigate the effect of 3D CT reconstruction combined with 3D printing to assist internal fixation in the treatment of acetabular fractures.Methods: From May 2013 to March 2018, 88 patients with acetabulum fracture in our hospital were selected as the research objects and categorized into the control group (n=44) and the observation group (n=44) with the random number table method.The patients in the control group were treated with 3D CT reconstruction and internal fixation, while the patients in the observation group were treated with 3D CT reconstruction and 3D printing.The operative indexes, knee joint score (HSS) and postoperative complications were recorded and analyzed.Results: The operation time, fracture healing time, intraoperative blood loss, length of hospital stay, fluoroscopy irradiation times and postoperative Hss score in the observation group were significantly lower than those in the control group (P<0.05), and the excellent and good rate of Matta score in the observation group was 88.64% (39/44), which was not significantly different from 81.82% in the control group (36/44).After operation, there were 1 case of abnormal healing, 1 case of internal fixture broken and 1 case of incision infection in the observation group, 1 case of joint adhesion and 3 cases of internal fixture broken in the control group.There was no statistically significant difference between the number of complications in the observation group(3/44) and the control group (4/44) (P>0.05).Conclusion: The application of 3D CT reconstruction combined with 3D printing in acetabular fracture internal fixation can improve the quality of operation, promote the recovery of fracture site after operation, and reduce the incidence of complications.It is worth promoting in related clinical treatment.

Objective: To explore the clinical effect of improved dacryocystorhinostomy in treating chronic dacryocystitis.Methods: One hundred and fifty patients (150 eyes) with chronic dacryocystitis admitted to the Affiliated Hospital of Chengde Medical College from January 2018 to December 2019 were chosen as the observation group to undergo improved dacryocystorhinostomy.Another fifty patients(50 eyes) admitted during the same period of time were chosen as the control group to undergo traditional dacryocystorhinostomy.The patency of lacrimal passage and the duration of operation of the two groups were compared.Results: The cure rate of the observation group was 99.3% while that of the control group was 78.0%.The difference between the two groups was statistically significant (P<0.05).Conclusion: The improved dacryocystorhinostomy has significant curative effect,short surgery time and low recurrence rate, and therefore it is comparatively an ideal operation method to treat chronic dacryocystitis.

1544 An Analysis of the Curative Effectiveness of B-Ultrasound Guided Holmium Laser Lithotripsy on Upper Urinary Tract Calculi in Children

Objective: To analyze the effect of laser lithotripsy under the guidance of B-ultrasound on children's upper urinary tract calculi.Methods: The clinical data of 210 cases of upper urinary tract calculi admitted to our hospital from November 2013 to November 2018 were retrospectively analyzed.They were divided into observation group (n=105) and control group (n=105).The control group was treated with ureterolithotomy and the observation group was treated with holmium laser lithotripsy under theguidance of B-ultrasound.The perioperative indexes, stone-free rate, white blood cell count (WBC), hemoglobin (Hb), serum inflammatory cytokines [interleukin-6(IL-6),interleukin-2(IL-2),tumornecrosis factor-α(TNF-α)], incidence of complications and recurrence rate were compared between the two groups.Results: The operation time in observation group was longer than that in control group, postoperative hospital stay was shorter than that in control group, intraoperative blood loss was less than that in control group (P<0.05), and stone free rate was lower than that in control group (P<0.05).After 24h of surgery, WBC level in observation group was lower than that in control group, while Hb level was higher than that in control group (P<0.05).The serum levels of IL-6 and TNF-α in the observation group were lower than those in the control group, and IL-2 was higher than that in the control group(P<0.05).The incidence of complications in observation group was lower than that in the control group (P<0.05).There was no significant difference in recurrence rate between the two groups (P>0.05).Conclusion: The operation time of holmium laser lithotripsy guided by B ultrasound is shorter, the amount of bleeding is less, the length of hospital stay can be shortened, and the safety is higher.The stone free rate of ureterolithotomy is higher, but the complications are more, so the appropriate operation scheme should be selected according to the actual situation of children.

Objective: To evaluate the patent ductus arteriosus (PDA) on left ventricular (LV) systolic myocardial function in preterm infants with vector velocity imaging (VVI).Methods: Infants whose gestational age was less than 30 weeks and who were delivered in our hospital were enrolled in the study.Babies were categorized into three groups with the degrees of ductal patency evaluated with echocardiography on the third day after birth, ① Group hsPDA (hemodynamically significant PDA): PDA diameter≥1.5 mm and left atrium to aorta (LA:Ao) ratio ≥1.5; ② Group PDA: PDA diameter <1.5mm or LA:Ao ratio <1.5; ③ Group nPDA: No PDA was detected through examination.The clinical data, conventional echocardiographic parameters and LV longitudinal global peak systolic strain (LVS) and strain rate (LVSR) were recorded and compared within three groups, and analysis was conducted with one-way ANOVA.Results: The gestational age, birth weight and left ventricular ejection fraction (LVEF) were not different within the three groups (p>0.05).The conventional parameters such as LA:Ao, LV end-diastolic diameter (LVD) and E/e were significantly higher in group hsPDA than the other two groups (p<0.01), while those were not different between Group PDA and nPDA.The LV systolic myocardial function parameters LVS and LVSR were significantly lower in group hsPDA than in the other two groups (p<0.01), but those were not significant between group PDA and group nPDA although the former had lower trend (p>0.05).Conclusion: PDA in preterm infants have LV volume overload, the LV myocardial deformation already decreased despite preserved LVEF.

Objective: To observe the effectiveness of early sequential hemoperfusion (HP) combined with glucocorticoid in the treatment of acute rhabdomyolysis (RM) with bee stings.Methods: A total of 108 patients with RM with bee stings in our hospital from May 2017 to October 2019 were selected and evenly divided into control group and study group with the simple random number table method.Patients in control group had the conventional treatment plus early sequential HP, and patients in study group had glucocorticoid on the basis of control group.The biochemical indicators [creatine kinase (CK), myoglobin (Myo), serum creatinine (SCr), blood urine nitrogen (BUN)], inflammatory indicators [tumor necrosis factor (TNF-α), interleukin-6 (IL-6), procalcitonin (PCT)] and disease status (APACHEⅡ score) were compared between the two groups before treatment and 72h after treatment, and the prognosis status [hospital stay, Acute Kidney Injury stage 3 (AKI stage 3), Multiple Organ Dysfunction Syndrome (MODS) and occurrence of mortality] were recorded.Results: 72h after treatment, the levels of CK, Myo, SCr, BUN, TNF-α, IL-6 and PCT and APACHEⅡ score in the two groups were significantly decreased compared with those before treatment (P<0.05), and there were statistically significant differences in the above indexes in study group compared with those in control group (P<0.05).The hospital stay, incidence rate of AKI stage 3, incidence rate of MODS and mortality rate in study group were significantly lower than those in control group (P<0.05).Conclusion: For patients of acute rhabdomyolysis with acute bee stings, early sequential hemoperfusion combined with glucocorticoid has significant curative effects, and it can significantly improve biochemical indicators, reduce inflammatory response, and improve prognosis, with good application value.

1556 Application Study of Pectoralis Major Myocutaneous Flap and Free Anterolateral Femoral Flap in Defect Repairing After Oral Cancer Operation

Objective: To evaluate the application effects of pectoralis major myocutaneous flap and free anterolateral femoral flap in defect repairing after oral cancer surgery.Methods: The clinical data of 90 patients with oral cancer from June 2016 to June 2019 were retrospectively analyzed.They were catagorized into pectoralis major group (34 cases) and anterolateral femoral group (56 cases) according to the different transplanted flaps.Tumor enlargement resection+cervical lymph node dissection were performed on all patients, and pectoralis major myocutaneous flap was used to repair the defects in pectoralis major group, and free anterolateral femoral flap was used to repair the defects in anterolateral femoral group.The perioperative indicators, occurrence of vascular crisis, flap survival and occurrence of postoperative complications were recorded in the two groups.The quality of life of the patients before surgery and 6 months after surgery was evaluated with University of Washington Quality of Life Questionnaire (UW-QOL).Results: There were no significant differences in flap area and postoperative drainage volume between the two groups (P>0.05), and the flap preparation time and defect repairing time in pectoralis major group were significantly shorter than those in anterolateral femoral group (P<0.05).The incidence rate of vascular crisis and overall flap survival rate were 11.76% and 91.18% in pectoralis major group, which were not statistically different from 12.50% and 91.07% in anterolateral femoral group (P>0.05).The total incidence rate of postoperative complications in pectoralis major group was significantly higher than that in anterolateral femoral group (23.53% vs 7.14%) (P<0.05).At 6 months after surgery, the UW-QOL scores in the two groups were higher than those before surgery, but there was no significant difference in the increase between the two groups (P>0.05).Conclusion: Pectoralis major myocutaneous flap and free anterolateral femoral flap have high flap survival rate in defect repairing after oral cancer surgery, and they can effectively improve the quality of life, but free anterolateral femoral flap can reduce the risk of postoperative complications.

Objective: To explore the effect of PCNL, FURL and spiral basket manipulation assisted FURL in the treatment of upper ureteral calculi with unilateral diameter > 1.0cm.Methods: From January 2017 to September 2019, 80 cases of upper ureteral calculi with unilateral diameter >1.0cm were prospectively selected for study and categorized into three groups with the random number table method: group A (26 cases) was treated with PCNL; group B (27 cases) was treated with FURL; and group C (27 cases) was treated with spiral basket manipulation assisted FURL.The operating time, intraoperative hemorrhage, length of hospital stay, success rate of lithotripsy, calculus removal rate, calculus migration rate, postoperative fever and hematuria, and incidence of lumbar discomfort were compared among the three groups.Results: There were significant differences in operation time, intraoperative hemorrhage and hospitalization time among the three groups (P<0.05); the operation time, intraoperative hemorrhage and hospitalization time in group B were lower than those in group A, the hospitalization time in group C was lower than that in group A, and the operation time was longer than that in group B (P<0.05).There were significant differences in the success rate of lithotripsy and stone clearance rate among the three groups (P<0.05); the success rate of lithotripsy and stone clearance rate of group B were lower than those of group A and group C respectively (P<0.05).The incidence of complication hematuria in group A was significantly higher than that in group B and group C (P<0.05).Conclusion: PCNL and spiral stone basket assisted furl lithotripsy has a high success rate and stone clearance rate.Furl has a shorter operation time, less bleeding during operation and a shorter hospital stay.However, the incidence of complications of PCNL is higher.That is to say, spiral stone basket assisted furl is a treatment method that takes into account the stone clearance rate, treatment effect and safety, and is worthy of clinical promotion.

1565 Coagulation Function and Thromboelastography Detection in Patients with Acute Ischemic Stroke Treated with Blood Transfusion and Its Clinical Significance

Objective: To study the coagulation function and thromboelastography (TEG) detection in patients with acute ischemic stroke treated with blood transfusion and its clinical significance.Methods: 120 patients with acute ischemic stroke who were treated with endovascular therapy in our hospital from January 2017 to June 2019 were selected as the research object, and 34 health controls were selected as the control group.The indexes of TEG and conventional coagulation test, and TEG parameters including R time, K time, α angle, and maximum amplitude (MA) between the two groups were compared.Correlation between TEG and parameters of onventional coagulation test in patients with ischemic stroke was analyzed.The sensitivity, specificity, and accuracy of different methods in the diagnosis of complications such as cerebral hemorrhage and early neurological deterioration were calculated.Results: Compared with those in the control group, the T and R times of the TEG test indicators in the ischemic stroke group were significantly shorter, while the α angle and the MA value were significantly larger(P<0.05).The conventional coagulation test index APTT was significantly shorter than that of the control group, and the FIB concentration increased (P<0.05), indicating the hypercoagulable state.The T time and K time of the TEG test indexes in the complication group were significantly shorter than those in the non-complication group, while the α angle and MA value were significantly increased compared with those in the non-complication group (P<0.05).The index of routine coagulation test APTT was significantly shorter than that of the non-complication group, and the FIB concentration increased (P<0.05).R time was significantly positively correlated with PI and ATPP (P<0.05), but there was no significant correlation between R time and FBI (P>0.05).K time was negatively correlated with FBI, and α angle and MA were positively correlated with FIB (P<0.05).The above parameters of TEG test had no significant correlation with PI and ATPP (P>0.05).The sensitivity, specificity, and accuracy of TEG in the diagnosis of symptomatic cerebral hemorrhage were 77.78%, 89.19%, and 88.33%, respectively, and in the diagnosis of early neurological deterioration, 16.67%, 92.59% and 85.00%, respectively.The sensitivity, specificity, and accuracy of conventional coagulation tests in the diagnosis of symptomatic cerebral hemorrhage were 46.15%, 72.90%, and 70.00%, respectively, and in the diagnosis of early neurological deterioration, 58.33%、66.67%、65.83%, respectively.Conclusion: The parameters of TEG detection can effectively reflect the process of coagulation changes in patients with acute ischemic stroke, and have some correlation with the indicator levels of conventional coagulation test.In clinical practice, the appropriate detection method can be selected according to the actual situation of the patient.

1570 The Evaluation of Applying Periodontal Membrane Injection and Apical Area Infiltration to Children's First Permanent Molar Anesthesia

Objective: To compare the effects of periodontal membrane injection and apical area infiltration for the treatment of children pulp disease of the first permanent molar and to observe the patients' sense of pain during injection.Methods: A total of 100 children six- to twelve-year-old suffering from pulp disease of the first permanent molar were included in the study and randomly divided into two groups with computerized random number method, respectively using distal periodontal membrane injection (the experimental group) and apical area infiltration anesthesia (the control group).Pain visual analog scale was used to determine pain values during injection, Venham rating scale of clinical anxiety and level of cooperative behavior was used to evaluate children anxiety levels and behaviors during treatment, statistical analysis were performed.Results: Pain values of the experimental group during periodontal membrane injection were significantly lower than the control group, meanwhile scores of clinical anxiety and levels of cooperation in the experimental group were slightly lower than the control group, and the differences were statistically significant.Conclusion: Distal periodontal membrane injection, when compared with apical area infiltration anesthesia, produces less pain, has better accessibility, exhibits better anesthetic effect and causes less trauma during treatment of the first permanent molar pulp disease in children 6- to 12-year-old.

1573 Study on the Medical and Economic Values of High Dose Bovine Alveolar Surfactant in the Treatment of Neonatal Respiratory Distress Syndrome

Objective: To investigate the clinical and economic values of high dose bovine alveolar surfactant (PS) in the treatment of neonatal respiratory distress syndrome (NRDS).Methods: 90 cases of NRDS diagnosed and treated in the Department of Pediatrics of our hospital from February 2017 to August 2019 were prospectively selected and divided into the routine group and the study group with the simple random number table method, 45 cases in each group.All NRDS patients were treated with CPAP, expectoration and anti infection.70 mg / kg standard dose and 100 mg / kg high dose PS were injected into tracheal intubation in routine group and study group respectively.The changes of blood gas analysis indexes (PaO2, PaO2, pH) and ventilator parameters (PEEP, FiO2, RI) were observed before and 48 hours after treatment, and the CPAP time, PS medication times, complication rate, mortality, length of hospital stay and total medical expenses were recorded.Results: Values of PaO2, pH and PEEP were significantly increased and PaCO2, FiO2 and RI were significantly decreased in the two groups 48 hours after treatment, and the difference was statistically significant (P<0.05).Values of PaO2, pH, PEEP, PaCO2, FiO2 and RI of the study group were significantly higher than those of the control group.The difference was statistically significant (P<0.05).The complication rate and mortality rate of NRDS in the study group were 8.89% and 2.22% respectively, which were slightly lower than those in the control group (13.33% and 4.44%).There was no significant difference (P>0.05).The time of CPAP and the times of PS administration in the study group were lower than those in the control group (P<0.05).The length of stay and the total cost of medical treatment in the study group were less than those in the routine group, and the difference was statistically significant (P<0.05).Compared with the standard dose of 70 mg / kg PS in the treatment of NRDS, the high dose of 100 mg / kg PS can save about 7200 yuan / case of medical expenses, and the total savings of 45 NRDS patients in the study group are about 324000 yuan.Conclusion: Compared with the standard dose of 70 mg / kg PS in the treatment of NRDS, the effect of high dose of 100 mg / kg PS is better, with significant clinical and economic values.

Objective: To study the curative effectiveness of ginkgo terpene lactones meglumine injection combined with indapamide in treatment of hypertension coronary heart disease and its effects on the serum monocyte chemoat-tractant protein-1(MCP-1), Heart fatty-acid-binding protein(H-FABP) and soluble ST2(sST2) levels.Methods: 85 patients of hypertension complicated with coronary heart disease who received therapy from February 2016 to February 2018 in our hospital were selected as research objects, and with the simple random number table, the patients were divided into the observation group (n=43) and the control group (n=42).On the basis of routine treatment, the control group was treated with indapamide.On the basis of the control group, the observation group was given ginkgo terpene lactones meglumine injection for a continuous treatment of 2 courses.The effect of antihypertensive and angina pectoris and the changes of TCM syndrome score, serum MCP-1, H-FABP and sST2 and adverse reaction were compared between the two groups.Results: There was no significant difference between the two groups in the hypotensive effect (P>0.05); the total effective rate of angina pectoris in the observation group was significantly higher than that of the control group[90.70%(39/43)vs73.81%(31/42)](P<0.05); after treatment, chest pain, shortness of breath, palpitation syndrome score, serum MCP-1, H-FABP and sst2 were significantly lower than before treatment, the difference in the observation group was significantly greater than that in the control group(P<0.05);during treatment, the total incidence of adverse reactions in the two groups was 9.30% (4/43) and 7.14% (3/42) respectively, and the difference was not statistically significant(P>0.05).Conclusion: Ginkgo terpene lactones meglumine injection combined with indapamide is well for hypertension combined with coronary heart disease, which can effectively reduce the expression of serum MCP-1, H-FABP and sST2 levels, improve cardiac function, relieve clinical symptoms, and has high safety, it's worthy of application and promotion.
