CHEN Bo, GUO Yutao, et al
DOI: 10.3969/j.issn.1006-6233.2020.08.001
Objective: To investigate the effect of miR-1231 on meningioma cell proliferation by targeting the EGFR /PI3K /AKT pathway. Methods: Bioinformatics software and luciferase reporter gene experiments were used to analyze and verify the targeted binding between miR-1231 and EGFR. RT-PCR was used to detect miR-1231 expression and EGFR mRNA expression in meningiomas cells. Western blot was used to detect EGFR protein expression, PI3K and AKT phosphorylation levels; CCK-8 was used to detect meningioma cell viability; colony formation assay and EdU assay was used to detect meningioma cell proliferation. Results: Compared with normal meningeal cells, the expression of miR-1231 in meningiomas cells was significantly down-regulated (P<0.05), and the expression of EGFR was significantly up-regulated (P<0.05). EGFR was confirmed as a target gene of miR-1231. Compared with miR-NC group, miR-1231 significantly inhibited meningiomas cell proliferation and inhibited EGFR/PI3K/AKT pathway (P<0.05). Compared with anti-miR-NC group, anti- miR-1231 significantly promotes meningioma cell proliferation and activates the EGFR /PI3K /AKT pathway (P<0.05). Compared with miR-NC + EGFR-NC group, miR-1231 + EGFR-NC significantly inhibits meningiomas cells EGFR /PI3K /AKT pathway (P<0.05), significantly reduced cell proliferation (P<0.05). Compared with miR-NC + EGFR-NC group, miR-NC + EGFR significantly activated meningiomas cells PI3K /AKT pathway (P<0.05) ), significantly increased cell proliferation ability (P<0.05), and could significantly reverse the inhibitory effects of miR-1231 + EGFR-NC on PI3K /AKT pathway and cell proliferation ability of meningiomas cells. Conclusion: MiR-1231 can inhibit the proliferation of meningioma cells by targeting the EGFR to regulate the PI3K /AKT pathway.

DOI: 10.3969/j.issn.1006-6233.2020.08.014
Objective: To study the regulatory mechanism of miR-663b on the proliferation and apoptosis of human intervertebral disc degeneration nucleus pulposus cells induced by low glucose. Methods: The expression of miR-663b in degenerative nucleus pulposus cells of human disc herniation and glucose (5, 1 mmol/L) -treated discs was detected by real-time fluorescent quantitative reverse transcription polymerase chain reaction (qRT-PCR) experiments. The low glucose group + miR-NC group (transfected with miR-NC), the low glucose group + miR-663b group (transfected with miR-663b mimics), and the low glucose group + anti-miR-NC group (transfected with anti-miR -NC), low glucose group + anti-miR-663b group (transfected with anti-miR-663b), divided with the plastid method, were transfected into 1 mmol/L treated disc degeneration nucleus pulposus cells. Cell Counting Kit (CCK-8) method, flow cytometry, and western blot were used to detect the absorbance (OD) value of cells, the apoptosis rate of cells, and cell cycle-dependent protein kinase inhibitor 1A (P21) , protein expression of cysteine aspartic protease (Caspase-3) in cells. Results: The expression of miR-663b in tissues of human disc herniation and 1 mmol/L glucose (low glucose group) treated disc degeneration nucleus pulposus cells were significantly reduced (P<0.05). The cells of the low glucose group were inhibited from proliferating, their apoptotic capacity was improved, and the protein expression of P21 and Caspase-3 was increased. The expression of miR-663b was significantly increased in the low-glucose group cells overexpressing miR-663b, the cell proliferation ability was also significantly increased, and the apoptotic capacity was significantly reduced. The protein expression levels of P21 and Caspase-3 were inhibited, while miR-663b's low-glucose group cells had opposite effect. Conclusion: MiR-663b can promote the proliferation of human intervertebral disc degeneration nucleus pulposus cells induced by low glucose and inhibit apoptosis.

DOI: 10.3969/j.issn.1006-6233.2020.08.016
Objective: To investigate the detection of plasma procalcitonin (PCT), neutrophil-to-lymphocyte ratio (NLR) and monocyte chemotactic protein-1 (MCP-1) in the diagnosis of liver cirrhosis with spontaneous bacterial peritonitis (SBP) and the value of prognosis evaluation. Methods: A total of 146 patients with liver cirrhosis and ascites treated in our hospital from December 2017 to December 2018 were selected and divided into SBP group (n=76) and non-SBP group (n=70) according to clinical manifestations and laboratory examination results. Another 51 healthy examinees in the same period in our hospital were randomly selected as control group. The levels of plasma PCT, NLR and MCP-1 were measured among the three groups, and receiver operating characteristic (ROC) curve was plotted to analyze the diagnostic value of PCT, NLR and MCP-1 on liver cirrhosis with SBP. The changes of plasma PCT, NLR and MCP-1 before and after treatment were analyzed in SBP group, and the value of PCT, NLR and MCP-1 in the prognosis evaluation of liver cirrhosis with SBP was investigated. Results: The levels of plasma PCT, NLR and MCP-1 in SBP group were significantly higher than those in non-SBP group and control group (P<0.05), and the level of plasma MCP-1 in non-SBP group was significantly higher than that in control group (P<0.05), and there were no significant differences in the PCT and NLR compared with those in control group (P>0.05). ROC curve results showed that when the optimal cutoff value of PCT was 0.94ng/ml, the AUC for diagnosis of liver cirrhosis with SBP was 0.850 (95% CI: 0.787-0.912), and when the optimal cutoff value of NLR was 13.72, the AUC was 0.721 (95% CI: 0.639-0.803), and when the optimal cutoff value of MCP-1 was 0.69μg/L, the AUC was 0.810 (95% CI: 0.741-0.879), and the AUC of combination of the three was 0.866 (95% CI: 0.807-0.926). The AUC, sensitivity and specificity of the combined diagnosis were better than those of PCT, NLR or MCP-1 alone. The levels of plasma PCT, NLR and MCP-1 in patients with liver cirrhosis and SBP after treatment were significantly lower than those before treatment (P<0.05). Conclusion: The levels of plasma PCT, NLR and MCP-1 are abnormally elevated in patients with liver cirrhosis and SBP. Detection of plasma PCT, NLR and MCP-1 has certain clinical value in the early diagnosis and prognosis evaluation of liver cirrhosis and SBP.

DOI: 10.3969/j.issn.1006-6233.2020.08.019
Objective: To analyze the effect of modified minimally invasive funnel chest correction (Nuss) in the treatment of children's funnel chest and the factors that lead to postoperative complications. Methods: From February 2016 to February 2018, 90 children undergoing Nuss operation in our hospital were analyzed retrospectively. the results were observed and divided into groups according to whether the complications occurred after operation. The general data of patients were analyzed by univariate analysis. The statistically significant indexes were included in multivariate Logistic regression, and the independent risk factors leading to complications were analyzed. Results: The operation was successfully completed in 90 cases. A total of 21 cases had postoperative complications, but after corresponding treatment, there was no serious problem when they were discharged from hospital. All the children were followed up. During the follow-up period, there was no obvious discomfort and no plate displacement occurred. Univariate analysis showed that age, the symmetry of funnel chest, the number of plates inserted and the time of operation were the influencing factors of complications, and the difference was statistically significant (P<0.05). Multivariate Logistic regression showed that the operation time was too long, asymmetric funnel chest and the number of plates placed was an independent risk factor for complications, and the difference was statistically significant. Learning significance (P<0.05). Conclusion: Nuss operation has a good effect on the treatment of pectus excavatum in children. The long operation time, asymmetric pectus excavatum and the number of plates placed are independent risk factors for postoperative complications. In clinical practice, high attention should be paid to this type of children to prevent serious complications caused by the above factors.

DUAN Yongdong, DAI Junli
DOI: 10.3969/j.issn.1006-6233.2020.08.021
Objective: To investigate the expression of mismatch repair protein (MMR) in colorectal cancer tissues in different parts of the large intestine and the significance of MMR expression in colorectal cancer. Methods: The expression and significance of mismatch repair proteins (msh1, MSH2, MSH6, and PMS2) in colorectal cancer tissues were analyzed by immunohistochemical method (SP method), and the expression significance of MMR protein in different parts of colorectal cancer was analyzed. Deletion of one or more of msh1, MSH2, MSH6 and PMS2 proteins was determined as mismatch repair gene defect (dmmr), and all expression levels were determined as mismatch repair gene integrity (PMMR). Results: In 168 cases of colorectal cancer, there were 141 cases (83.93%) of PMMR, 27 cases (16.07%) of dmmr, 56 cases (71.79%) of MMR protein expression loss group (dmmr) and 22 cases (28.21%) of MMR protein non deletion group (PMMR) in 78 cases of normal colorectal mucosa adjacent to carcinoma (P<0.05). In 47 cases of right colon cancer, there were 19 cases (40.42%) of dmmr, 5 cases (5.43%) of left colon cancer, 3 cases (10.34%) of 29 cases of rectal cancer. Furthermore, there was a certain relationship between dmmr and clinicopathological features of colorectal cancer. The dmmr in the group with lymph node metastasis and invasion into the muscular layer was significantly lower than that in the group without lymph node metastasis and the group with invasion of muscle layer (P<0.05). Conclusion: The expression of mismatch repair protein in colorectal cancer is higher than that in normal intestinal mucosa, and the expression of mismatch repair protein is different in different parts of colorectal cancer. Different status of mismatch repair protein is related to the progress of colorectal cancer, and suggests that the prognosis of colorectal cancer with mismatch repair protein deficiency may be better.

DOI: 10.3969/j.issn.1006-6233.2020.08.025
Objective: To investigate the therapeutic effects of endoscopic submucosal dissection (ESD) in patients with early gastrointestinal cancer and precancerous lesions. Methods: 77 patients with early gastrointestinal cancer and precancerous lesions who underwent endoscopic mucosal resection (EMR) or ESD in the Department of Gastroenterology of the hospital from August 2015 to August 2017 were selected as subjects. The size and type of lesions in the two groups were recorded. The complete resection rate, en bloc resection rate, operation time, length of hospital stay, intraoperative and postoperative complications, and follow-up recurrence were recorded. Results: There were 32 patients (35 lesions) in ESD group and 45 patients (50 lesions) in EMR group. The lesion diameter in ESD group was significantly larger than that in EMR group (P<0.05). The complete resection rate of ESD group and EMR group was 97.14% and 68.89%, respectively, and the whole resection rate was 100.00% and 75.56%, respectively, with significant difference between the two groups (P<0.05). There was no significant difference in the complete resection rate and the whole resection rate of lesions ≤ 20 mm between the two groups (P>0.05). The complete resection rate and the total resection rate of lesions ≥ 20 mm in ESD group were significantly higher than those in EMR group (P<0.05). The operation time in ESD group was higher than that in EMR group (P<0.05). There was no significant difference in bleeding, perforation and postoperative recurrence between the two groups (P>0.05). Conclusion: The curative effect and safety of ESD are good in the treatment of early gastrointestinal cancer and precancerous lesions. Compared with EMR, the resection effect of ESD is better. However, the duration of ESD is longer and there's a high risk for the surgeon. Clinically, the patient's clinical data, condition of lesions and indications can be combined for choosing surgical method reasonably.

Objective: To analyze the application of contrast-enhanced ultrasound combined with quantitative analysis in the diagnosis of benign and malignant thyroid nodules. Methods:Retrospective analysis was conducted of clinical data of 72 patients with thyroid nodules who underwent contrast-enhanced ultrasound examination in our hospital from May 2019 to December 2019. The characteristics of contrast-enhanced ultrasound enhancement mode of benign and malignant thyroid nodules were observed. The time-intensity curve was used to analyze blood flow perfusion parameters such as contrast medium transit time, peak intensity, peak time, and area under the curve. Using the postoperative pathological results as a control, the diagnostic value of contrast-enhanced ultrasound combined with quantitative analysis technology was analyzed. Results: Contrast-enhanced ultrasound showed that among 72 patients with thyroid nodules, 43 were benign nodules and 29 were malignant nodules. The pathological types of benign nodules are thyroid adenomas and nodular goiters. Their contrast-enhanced ultrasound features are uniform enhancement or no enhancement inside, peripheral ring enhancement, and clear boundaries. The pathological types of malignant nodules are papillary thyroid carcinoma. Contrast-enhanced ultrasound is characterized by low or uneven enhancement, irregular morphology, and blurred borders. Postoperative pathology confirmed 47 benign nodules and 25 malignant nodules. The sensitivity of contrast-enhanced ultrasound in diagnosing malignant thyroid nodules was 88.00%, the specificity was 85.11%, and the accuracy was 86.21%.Compared with benign thyroid nodules, Peak and AUC in diagnosing malignant thyroid nodules were relatively lower, and had a later TIP (P<0.05). LI Yuanyuan, YANG Daping, SUN Yanyan
DOI: 10.3969/j.issn.1006-6233.2020.08.027
Objective: To investigate the effect of extensive hysterectomy combined with pelvic lymphadenectomy for cervical cancer, and to analyze the influencing factors of postoperative pelvic floor dysfunction (PFD). Methods: 63 patients with cervical cancer who were treated in our hospital from January 2015 to June 2019 were divided into study group (32 cases) and control group (31 cases) with random table method. All patients underwent extensive hysterectomy combined with pelvic lymphadenectomy. As to reatment, the study group took laparoscopy, and the control group took conventional open surgery. The intraoperative and postoperative conditions and complications were compared between the two groups. Binary logistic regression was used to analyze the risk factors of postoperative PFD. Results: The operation time, intraoperative blood loss, time to get out of bed, anal exhaust time, postoperative defecation time, and hospitalization time of the study group were significantly lower than those of the control group. The number of lymph node dissection in the study group was significantly higher than that of the control group. It is tatistical significance (P<0.05). The complication rate of the study was 56.25%, which was significantly lower than that of the control group (87.10%). The difference was statistically significant (P<0.05). A total of 38 cases of PFD occurred after operation. The factors of age, tumor diameter, presence or absence of ejaculation, and indwelling catheter were the influencing factors of postoperative PFD in patients with cervical cancer (P<0.05). Multivariate binary logistic analysis reveals that age ≥ 45 years, tumor diameter ≥ 4cm, menopausal, urethral catheter indwelling time ≥ 7d were risk factors for postoperative PFD in patients with cervical cancer (P<0.05). Conclusion: Laparoscopic extensive hysterectomy combined with pelvic lymphadenectomy for cervical cancer patients has a good effect, but incidence rate of postoperative PFD is high. Proper measures should be taken to aged menopausal patients whose tumor diameter is too large, and urethral catheter is long indwelled.

WU Changhui, et al
DOI: 10.3969/j.issn.1006-6233.2020.08.029
Objective: To investigate the value of the evaluation of intestinal trefoil factor (TFF3) in early intestinal dysfunction in septic shock patients and its relationship with the severity of the disease. Methods: From November 2016 to October 2019, 150 patients with septic shock within 24 hours after admission in the Department of critical medicine of Dazhou Central Hospital were selected as the study objects. According to whether early intestinal dysfunction was complicated during hospitalization, they were divided into non intestinal dysfunction group (n = 72) and intestinal dysfunction group (n = 78). Clinical data were collected and serum TFF3 was detected by double antibody sandwich enzyme-linked immunosorbent assay (ELISA). Pearson test was used to analyze the correlations between serum TFF3 level and Acute Physiology And Chronic Health Evaluation Ⅱ (APACHE Ⅱ), C-reactive protein (CRP), blood urea nitrogen (BUN), blood creatinine (SCr) and gastrointestinal function score; multivariate logistic regression analysis of binary variables was used to screen the independent risk factors of early intestinal dysfunction in septic shock patients; and the ROC curve was drawn to evaluate the predictive values of APACHE Ⅱ score, SCr and TFF3 in patients with septic shock complicated with early intestinal dysfunction. Results: The APACHE Ⅱ score, levels of CRP, BUN, SCr and serum TFF3 of patients in intestinal dysfunction group were significantly higher than those of patients in non intestinal dysfunction group (P<0.05). There was a positive correlation between serum TFF3 level and APACHE Ⅱ score, CRP, BUN, SCr and gastrointestinal function score (P<0.05). Multivariate logistic regression analysis showed that the increases of APACHE Ⅱ score, SCr and serum TFF3 level were independent risk factors for septic shock complicated with early intestinal dysfunction (P<0.05). The ROC curve showed that the area under the curve (AUC) of serum TFF3 level in patients with septic shock complicated with early intestinal dysfunction was higher than that of APACHE Ⅱ score and SCr, and the sensitivity and specificity were 82.10% and 98.60% respectively when the best cutoff value was 12.97 ng/ml. Conclusions: The serum TFF3 level of septic shock patients with early intestinal dysfunction is increased, which can be used as a serum marker to evaluate the early intestinal dysfunction of septic shock patients.

DOI: 10.3969/j.issn.1006-6233.2020.08.031
Objective: To evaluate the effect of abnormal sperm morphology on the outcome of assisted pregnancy and the rate of neonatal deformities during the in vitro fertilization cycle. Methods: From January 2013 to January 2020, 1820 patients were selected who were accepting assisted pregnancy during the in vitro fertilization cycle for the first time in our reproductive medical center. The percentage of normal morphological sperm of patients was measured and they were divided into three groups (<1.0%, 1.0~4.0% and> 4.0%). The outcomes of assisted pregnancy among the three groups, including clinical pregnancy rate, live birth rate, and natural abortion rate, were compared. Then the safety, including preterm birth, infant weight and deformity rate, were further compared. Results: The three groups included 145, 726, and 949 cases, respectively. There was no significant difference in general information (age of women, body mass index, basic FSH, age of men and infertility years.) among the three groups before assisted pregnancy (P>0.05); the percentage of morphological sperm had no significant effect on the outcomes of assisted pregnancy, such as clinical pregnancy rate, live birth rate and spontaneous abortion rate. Follow-up of 777 deliveried couples showed that there was no statistical difference in preterm birth rate and infant weight among the three groups (P>0.05), and abnormal sperm morphology will not increase the deformity rate of the newborn. Conclusion: Abnormal sperm morphology did not affect the assisted pregnancy outcome (including clinical pregnancy rate, live birth rate and spontaneous abortion rate), nor the safety of the newborn (including premature birth, infant weight and deformity rate). The study could provide certain reference for the safety of patients with abnormal sperm morphology on selecting assisted pregnancy.
