353 Research on Biological Role of LncRNA AK093407 in Colorectal Cancer

Objective: To study the biological role of LncRNA AK093407 in human colorectal cancer cells. Methods: QRT PCR was used to detect the expression of lncRNA AK093407 in human colorectal cancer and HCT-116 cells; siRNA silenced the expression of lncRNA AK093407 in colorectal cancer cells: MTT assay was used to detect cell proliferation; annexin V / PI staining was used to detect cell apoptosis; PI staining was used to detect cell cycle; Western staining was used to detect cell cycle. The expression of Caspase-3, caspase-9, caspase-10, Bax, Bcl-2, p21, p27 and p-STAT3 were detected by blot. Results: The expression of lncRNA AK093407 in human colorectal cancer was higher than that in adjacent tissues; the expression of lncRNA AK093407 in HCT-116 was higher than that in NM460; lncRNA was silenced After AK093407, the proliferation activity of HCT-116 cells decreased, apoptosis increased, cell cycle blocked in G1 / S phase; the expression of caspase 3, caspase 9, caspase 10, Bax, Bcl-2, p21, p27 and p-STAT3 decreased. Conclusion: LncRNA AK093407 is highly expressed in colorectal cancer and HCT-116 cells. Silencing lncRNA AK093407 can inhibit cell proliferation, induce cell apoptosis and cell cycle arrest.

358 Relationship between Serum TNF-α IL-6 sIL-2R Levels and the Severity and Prognosis of Children with Adenoid Hypertrophy

Objective: To explore relationship between serum TNF-α, IL-6, sIL-2R levels and the severity and prognosis of children with adenoid hypertrophy. Methods: The related data of 100 children with adenoid hypertrophy who were admitted to the hospital between August 2016 and August 2018 were retrospectively analyzed. The severity of hypertrophy and rate of adenoid re-hyperplasia were analyzed. The levels of serum indexes were compared among children with different severities of hypertrophy and different prognosis. The correlation between the three serum indexes and hypertrophy, and their predictive value for adenoid re-hyperplasia were analyzed. Results: Of the 100 children, there were 36, 32 and 32 cases with grade I, grade II and grade III hypertrophy, respectively. The rate of adenoid re-hyperplasia was 15.00% (15/100). Among them, there respectively were 2 cases (2.00%), 6 cases (6.00%), and 7 cases (7.00%) with adenoid re-hyperplasia (χ2 = 4.063, P>0.05). The order of levels of TNF-α, IL-6 and sIL-2R in children with different severities of hypertrophy from low to high was as follows: grade I, grade II and grade III (P<0.05). TNF-α, IL-6 and sIL-2R were positively correlated with the severity of adenoid hypertrophy (P<0.05). During follow-up, levels of TNF-α, IL-6 and sIL-2R in children with hyperplasia were significantly higher than those without (P<0.05). ROC curve analysis showed that the AUC of combined diagnosis (0.896) was better than that of single diagnosis with TNF-α, IL-6 or sIL-2R (0.845, 0.814, 0.709). Conclusion: The levels of serum TNF-α, IL-6 and sIL-2R are closely related to the severity of adenoid hypertrophy in children. All of them can be applied to predict the prognosis of children but the combined prediction of the three is better.

362 Effect of Budesonide Combined with Montelukast on IL-33 IL-17A and Induced Phlebitis in Peripheral Blood of Patients with Asthma

Objective: To explore the effect of budesonide combined with montelukast on interleukin-33 (IL-33), interleukin-17A (IL-17A) in peripheral blood and inflammatory factors in induced sputum of patients with bronchial asthma. Methods: 104 patients with bronchial asthma admitted to our hospital from March 2017 to March 2019 were randomly divided into control group and study group, 52 cases in each group. The control group was treated with Budesonide aerosol inhalation, while the study group was treated with budesonide and montelukast. The respiratory function, serum IL-33, IL-17A water of the two groups were compared before and after treatment. Levels of IL-5 and IFN-gamma in induced sputum were also measured. Results: Before treatment, there was no significant difference between the two groups in each index. After treatment, FEV1 and FEV1 / FVC ratio of the study group were (2.89 ± 0.84) L and (86.47 ± 7.62) %, respectively, which were significantly higher than those of the control group. The serum levels of IL-33 and IL-17A in the study group were (68.75 ± 8.42) ng / L and (4.02 ± 0.67) ng / L, respectively, which were significantly lower than those in the control group. The level of IL-5 in induced sputum of the study group was (71.52 ± 21.69) pg / mL, significantly lower than that of the control group, and the level of IFN - γ in induced sputum was (64.31 ± 16.83) pg / mL, significantly higher than that of the control group (P<0.05). Conclusion: Budesonide combined with montelukast in the treatment of bronchial asthma can significantly improve the level of inflammatory factors, reduce airway inflammation, improve respiratory function of patients, and is conducive to the improvement of clinical efficacy. The combined therapy is worthy of promotion and application.

366 Diagnostic Value of Serum MicroRNA 122 and Microrna-130a Levels in Occult Hepatitis B Virus Infection

Objective: To study the correlation between serum microRNA-122 (miR-122) and miR-130a and occult hepatitis B (OBI), and to explore their diagnostic value for OBI. Methods: 60 patients with OBI (OBI group), 60 carriers of inactive HBsAg (ASC) (ASC group), 60 patients with chronic hepatitis B (CHB) (CHB group) and 60 suspicious patients (control group) were selected. The expression of serum miR-122 and miR-130a was detected by real-time fluorescence quantitative method. Results: The relative expression level of serum miR-122 in OBI group was significantly lower than that in the other three groups (all P<0.05). However, the relative expression level of miR-130a was significantly higher than that in the three groups (all P<0.05). The relative expression level of serum miR-122 in OBI group, the control group, ASC group and CHB group increased in turn (all P<0.05), and the relative expression level of miR-130a in OBI group, CHB group, ASC group and control group decreased in turn (all P<0.05). The value of miR-130a/miR-122 in OBI group was significantly higher than that in the other three groups (all P<0.05). Pearson correlation analysis showed that the expression of serum miR-122 was negatively correlated with the expression of miR-130a in patients with OBI (r=0.421, P<0.05). ROC curve analysis showed that the areas under the curves of serum miR-122, miR-130a and miR-130a/miR-122 value for differentiating patients with OBI and suspicious patients were 0.859 (95% CI: 0.762~0.955), 0.844 (95% CI: 0.739~0.950) and 0.945 (95% CI: 0.018~0.989), respectively. The areas under the curves for differentiating OBI and ASC were 0.942 (95% CI: 0.021~0.995), 0.813 (95% CI: 0.699~0.927) and 0.964 (95% CI: 0.012~0.997), respectively. The areas under the curves for differentiating OBI and CHB were 0.998 (95% CI: 0.000~1.000), 0.646 (95% CI: 0.503~0.789) and 0.993 (95% CI: 0.000~1.000), respectively. Conclusion: The decreased expression of serum miR-122 and the increased expression of miR-130a are associated with OBI, which is of important diagnostic value for OBI. The diagnostic value of miR-130a/miR-122 is higher than that of serum miR-122 and miR-130a alone.

370 Effects of Wisconsin and Siruikang on Weight Gain Endocrine and Metabolism in Hospitalized Male Patients with Schizophrenia

Objective: To analyze the effects of wisconsin and Siruikang treatment on weight gain, endocrine and metabolism in hospitalized male patients with schizophrenia. Methods: A total of 126 male schizophrenia patients who were treated in the hospital from Aug. 2016 to Aug. 2018 were enrolled in the study. They were randomly divided into wisconsin group and the Siruikang group, 63 cases in each group. The curative effect, changes in body mass, fasting plasma glucose (FPG), blood lipids [total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL) and low density lipoprotein (LDL) ], endocrine hormones [prolactin (PRL), testosterone, free triiodothyronine (FT3), free thyroxine (FT4) ] before and after treatment were compared between the two groups. The occurrence of adverse reactions during treatment in both groups was statistically analyzed. Results: There was no significant difference in the effective rate of treatment between the wisconsin group and the Siruikang group (87.30% vs 88.89%, P>0.05). There was no significant difference in BMI, FPG, TC, TG, HDL, LDL, testosterone, FT3 or FT4 level in Siruikang group before and after treatment (P>0.05). There was no significant difference in FPG, HDL, LDL, PRL, testosterone, FT3 or FT4 level in wisconsin group before and after treatment (P>0.05). After treatment, level of PRL in Siruikang group was higher than that before treatment and that in wisconsin group (P<0.05). After treatment, levels of BMI, TC and TG in wisconsin group were higher than those before treatment and those in Siruikang group (P<0.05). The incidence of adverse reactions in siruikang group was significantly higher than that in wisconsin group (61.90% vs 33.33%, P<0.05). Conclusion: The curative effect of wisconsin and Siruikang is comparable on male schizophrenia patients. However, the effects of Siruikang are less on vertebral reaction and endocrine, with lower effects on sexual function, which increases treatment compliance of male patients.

374 Effect of Levothyroxine Tablet and Growth Hormone for Idiopathic Short Children and the Influence on Serum IGF-1 and IGFBP-3 Levels

Objective: To study the clinical efficacy of levothyroxine tablets and growth hormone in the treatment of idiopathic short stature in children, and the effects on serum IGF-1 and IGFBP-3 levels. Methods: A total of 110 children with idiopathic short stature admitted to our hospital from June 2014 to July 2018 were enrolled in the study. According to the parity of admission order, they were divided into observation group (odd array) and control group (even array). On the basis of conventional treatment regimen, control group was given recombinant human growth hormone therapy, and observation group was given levothyroxine tablets combined with recombinant human growth hormone, and they were treated for 12 months. The growth effects, thyroid function, levels of serum insulin-like growth factor-1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) and incidence rate of adverse reactions were compared between the two groups. Results: The height, weight, bone age and growth rate in the two groups on the 6th and 12th month of treatment were significantly higher than those before treatment (P<0.05), and the growth rate on the 3rd and 12th month of treatment and weight on the 12th month of treatment in observation group were significantly higher than those in control group (P<0.05). The level of thyroid stimulating hormone (TSH) on the 3rd, 6th and 12th month of treatment were significantly decreased in the two groups (P<0.05) while the levels of serum free triiodothyronine (FT3) and serum free thyroxine (FT4) were significantly increased (P<0.05), and the levels in observation group after treatment were significantly different compared with those in control group (P<0.05). The levels of serum IGF-1 and IGFBP-3 on the 3rd, 6th and 12th month of treatment in the two groups were significantly higher than those before treatment (P<0.05), and the level of IGF-1 in observation group was significantly different from that in control group at the same time (P<0.05). During treatment, the incidence rates of adverse reactions in the two groups were 10.91% and 9.09% respectively (P>0.05). Conclusion: Compared with growth hormone alone, levothyroxine tablets combined with growth hormone for idiopathic short stature in children has achieved better clinical results, and to some extent, it improves the growth effects, promotes thyroid function, and increases levels of serum IGF-1 and IGFBP-3.

379 Effect of Oseltamivir Capsule Combined with Jinye Baidu Granule on Influenza A (H1N1)

Objective: To investigate the efficacy and safety of oseltamivir capsules combined with Jinye Baidu granules in the treatment of influenza A (H1N1). Methods: 204 patients with influenza A (H1N1) were randomly divided into trial group (100 cases) and control group (104 cases). Patients in the control group were administered with oseltamivir phosphate capsules orally (75 mg each time, twice a day) ; the trial group received oseltamivir phosphate capsules combined with Jinye baidu Granules (10g each time, 3 times a day) for oral treatment. Both groups of patients were treated continuously for 5 days. Blood routine, C-reactive protein (CRP) and liver and kidney function were measured in 2 groups before and after treatment. The efficacy and adverse reactions of each group of patients were observed. Results: The therapeutic effect of the experimental group comparing with that of the control group, the normal temperature rate was 91% vs 79.8% on the third day, the clinical symptom relief rate was 61% vs 47%, 79% vs 61% on the third and fifth day, and the total cure rate was 40% vs 26%. These indexes in the experimental group were significantly better than those in the control group (P < 0.05) ; The total ineffectiveness of the experimental group was lower than that of the control group (3% vs 7.7%), and the overall effective rate was higher than that of the control group (97% vs 92.3%), but the difference was not statistically significant (P>0.05). There were no significant differences in the rate of temperature recovery (97% vs 94.2%), deterioration rate (2% vs 5.8%) and negative rate of nasal swab virus (99% vs 97.1%) between the two groups on the 5th day (P>0.05). The overall adverse reaction rate between the two groups was compared (24% vs 22.1%) and no significant difference was found (P>0.05). There were no significant differences in white blood cells (WBC), percentage of monocytes (M%), liver and kidney function between the two groups before and after treatment (P>0.05). No significant difference was observed in C-reactive protein (CRP) between the two groups before treatment, but CRP in the trial group was significantly lower than that in the control group after treatment[ (11.9±3.6) mg/L vs (14.2±4.3) mg/L] (P<0.05). Conclusion: The combination of oseltamivir capsule and Jinye baidu granule can improve the clinical symptoms of influenza A (H1N1) more effectively without increasing adverse reactions.

384 Correlation between Dialysis Methods and Quality of Life Efficacy and Microinflammation Indicators in Maintaining Dialysis Patients

Objective: To explore the correlation of quality of life, dialysis mode, efficacy and microinflammation index in maintaining dialysis patients. Methods: From January 2018 to January 2019, 90 patients with end-stage renal disease who underwent dialysis in our hospital were randomly divided into three groups: A, B and C. Each group consisted of 30 patients. They were treated with hemodiafiltration (HDF), high-flux hemodialysis (HFHD) and high-flux hemodialysis combined with hemoperfusion (HFHD+HP). 6 months after treatment, the clinical efficacy, microinflammation index, quality of life and complications of the three groups were compared before and after treatment. Results: After 6 months of treatment, ALB and Kt/V in the three groups were significantly higher than those before treatment (P<0.05), and increased from group A to group C in turn (P<0.0125), BUN, Scr, β2-MG, PTH, CRP,IL-10,Hcy and TNF-a levels were significantly lower than those before treatment (P<0.05), and decreased from group A to group C in turn (P<0.0125) ; There was statistical difference in the incidence of complications after treatment among the three groups (P<0.05), and the incidence of complications in group A was higher than that in group C (30.00% vs. 3.33%, P<0.0125) ; After treatment, the SF-36 and KDTA scores in the three groups were significantly higher than those before treatment (P<0.05), and increased from group A to group C (P<0.0125). Conclusion: Compared with HDF and HFHD, dialysis with HFHD and HP in series can significantly improve the clinical efficacy, control the level of inflammation and improve the quality of life, and the safety is guaranteed.

389 Correlation between Hcy VCAM-1 and NT-proBNP Levels and Prognosis of Intravenous Thrombolysis in Patients with Acute Ischemic Stroke

Objective: To explore the relationship between levels of plasma homocysteine (Hcy), vascular cell adhesion molecule-1 (VCAM-1) and N-terminal pro-brain natriuretic peptide (NT-proBNP) and prognosis of intravenous thrombolysis in patients with acute ischemic stroke (AIS). Methods: A retrospective analysis was performed on clinical data of 116 patients with AIS. The levels of plasma Hcy, VCAM-1 and NT-proBNP before treatment were compared among patients with different short-term efficacy of different intravenous thrombolysis [National Institutes of Health Stroke Scale (NIHSS) ] and long-term prognosis [modified Rankin scale (mRS) ]. The correlation between the above indicators and short-term NIHSS score reduction and long-term mRS score was analyzed. The receiver operating characteristic (ROC) curve was used to evaluate the efficacy of the above indicators in predicting long-term prognosis. Results: There was a significant difference in plasma Hcy level among patients with different reductions of short-term NIHSS score (P<0.05), but there were no significant differences in the levels of plasma VCAM-1 and NT-proBNP (P>0.05). The levels of plasma Hcy, VCAM-1 and NT-proBNP in patients with long-term mRS>2 points were significantly higher than those in patients with long-term mRS≤ 2 points (P<0.05). The plasma Hcy level was significantly negatively correlated with reduction of short-term NIHSS score (r<0, P<0.05), and the levels of plasma VCAM-1 and NT-proBNP were significantly correlated with reduction of short-term NIHSS score (P>0.05). The levels of plasma Hcy, VCAM-1 and NT-proBNP were significantly positively correlated with long-term mRS scores (r>0, P<0.05). The cut-off values of long-term poor prognosis predicted by plasma Hcy, VCAM-1 and NT-proBNP were ≥19.64 μmol/L, ≥994.01 ng/mL and ≥526.02 ng/L, and the values of area under the ROC curve were significantly smaller than those of combined prediction (P<0.05). Conclusion: The levels of plasma Hcy, VCAM-1 and NT-proBNP in AIS patients are closely related to the prognosis of intravenous thrombolysis. Early detection has high clinical reference value.

393 Clinical Effect of Compound Cerebral Peptide Glycoside Injection on Acute Cerebral Infarction and the Effect on LP-PLA2 hs-CRP and NSE Levels in Patients

Objective: To investigate the clinical effect of compound cerebral peptide glycoside injection on acute cerebral infarction and its effect on lipoprotein related phospholipase (LP-PL) A2, hypersensitive C reactive protein (hs-CRP) and neuron-specific enolase (NSE) levels. Methods: 160 patients with acute cerebral infarction hospitalized in our hospital from January 2017 to June 2018 were randomly divided into control group and treatment group, 80 cases each. All the patients were treated according to "2018 Chinese guidelines for the treatment of acute ischemic stroke". Moreover, patients in the treatment group were given compound cerebral peptide glycoside injection for two weeks. All the patients were followed for 3 months. The NIHSS and ADL scores were evaluated before and after treatment and 3 months follow-up between two groups. Furthermore, the changes of Lp-PLA2, hs-CRP and NSE levels before and after treatment were detected in the two groups, and the occurrence of adverse reactions before and after treatment were compared in the two groups. Results: The NIHSS score and ADL of patients were significantly decreased in the two groups, and the levels of Lp-PLA2, hs-CRP and NSE were significantly decreased in the two groups when after treatment compared with before treatment, with statistically significant differences (P<0.05) ; After treatment, NIHSS score in the treatment group was significantly decreased and ADL was significantly increased compared with the control group. The levels of Lp-PLA2, hs-CRP and NSE in the treatment group were obviously lower than those in the control group, with statistically significant differences (P<0.05) ; There was no significant difference in the occurrence of adverse reactions between the two groups (P>0.05). Conclusion: The application of compound cerebral peptide glycoside injection can effectively improve the condition of neurological dysfunction and the quality of life of patients, significantly reduce the levels of Lp-PLA2, hs-CRP and NSE in patients with acute cerebral infarction, and do not increase the occurrence of adverse reactions. It is worthy of further promotion and treatment in clinic.

398 Effect of Xinmupei on Hemodynamics and Uterine Contraction of Cesarean Women Under Combined Spinal Epidural Anesthesia

Objective: To observe the influence of hemabate on hemodynamics of parturient undergoing cesarean section after administrated by different ways. Methods: 60 patients with high-risk cesarean section underwent combined spinal and epidural anesthesia were randomly divided into Xinmu Peigong injection group (M group) and deltoid injection group (D group), 30 cases in each group. Non-invasive hemodynamic monitoring system were used to investivagate mean artery pressure (MAP), heart rate (HR),stroke volume (SV), cardiac output (CO), total peripheral resistance (SVR) before hemabate administration (baseline) and every 5 minute following hemabate administration. Results: Compared with pre-dose, there was a decrease in TPR, an increase in CO, and an increase in SV (P<0.05) in both groups after injection, MAP decreased in group M after administration for 5 min,in group D after 10 min (P<0.05). The HR of patients in group M increased 5 minutes after administration (P<0.05), and there was no significant change in HR at any time after administration in group D (P>0.05) ; the incidence of palpitations and chest pain in group M higher than group D (P<0.05). The uterine contraction scores of the uterus injection group at 2.5 min and 5 min after treatment were higher than those in the deltoid injection group (P<0.05). The incidence of maternal chest tightness in the deltoid injection group was higher than that in the uterine injection group. (P<0.05). Conclusion: The changes of hemodynamics after injection of hemabate during cesarean section,patients showed high cardiac output and low resistance, and this influence continued for a long time. The injection of myometrium injected had more obvious influence on hemodynamics.

402 Analysis on Predictive Value of Peripheral Blood Cell Parameters on Different Airway Inflammation Phenotypes in Patients with Acute Exacerbation of Asthma

Objective: To investigate the predictive value of peripheral blood cell parameters on airway inflammation phenotypes in patients with acute exacerbation of asthma. Methods: The clinical data of 104 patients with acute exacerbation of bronchial asthma from May 2017 to May 2019 were collected. All patients were detected by induced sputum to identify airway inflammation phenotypes, and the induced sputum cell parameters and peripheral blood parameters were compared among different inflammation phenotypes. Spearman correlation coefficient was used to analyze the correlation between the induced sputum cell parameters and peripheral blood parameters, and ROC curve was drawn to analyze the predictive value of peripheral blood cell parameters on airway inflammation phenotypes. Results: There were significant differences in the parameters of eosinophil (EOS) and neutrophil (NE) in induced sputum and peripheral blood among patients with different airway inflammation phenotypes (P<0.05). The EOS count and percentage in peripheral blood were strongly positively correlated with sputum EOS percentage (P<0.05). The peripheral blood NE count and percentage were weakly positively correlated with sputum NE percentage (P<0.05). The values of area under the ROC curve of peripheral blood EOS count and percentage were 0.931 and 0.801 respectively in the prediction of sputum EOS≧3%. The values of area under the ROC curve of peripheral blood NE count and percentage were 0.651 and 0.706 respectively in the prediction of sputum NE≧61%. Conclusion: Peripheral blood parameters of EOS and NE in patients with acute exacerbation of asthma have certain predictive value on airway inflammation phenotypes. The peripheral blood EOS has higher predictive value, and analysis of peripheral blood cell parameters or can be used as the simple and rapid way to judge the phenotypes of airway inflammation in clinical practice.

407 The Value of Serum PCT and CRP / ALB Ratio in the Prediction of Perioperative Infection of Upper Urinary Tract Calculi

Objective: To detect the serum procalcitonin (PCT) and C-reactive protein (CRP) /serum albumin (ALB) ratio in patients with upper urinary tract calculi before and after operation, and to analyze their relationships with perioperative infection. Methods: 180 patients with upper urinary tract calculi who underwent surgery in the Department of Urology of Zhongxiang People's Hospital from April 2017 to May 2019 were selected as the study subjects. According to whether infection occurred at admission, the patients were divided into infection group (n=100) and non-infection group (n=80). The infection group was further divided into significant infection group (n=60) and hidden infection group (n=40). Postoperative infection was divided into infection group (n=42) and non-infection group (n=138). Serum PCT, CRP and Alb levels were measured by automatic biochemical analyzer, and CRP/Alb ratio was calculated. Acute physiology and chronic health score system II (APACHE II) was calculated according to the related indicators and clinical characteristics of patients, White blood cell count (WBC) and red blood cell count (RBC) were measured, and the temperature (T), heart rate (HR) and respiratory rate (RR) were recorded. Results: The numbers of WBC, RBC and APACHE score in the patients with hospital infection were significantly higher than those in the patients without hospital infection (P < 0.05). One day after admission, the levels of serum PCT, CRP, CRP/Alb in the non-infected group were lower than those in the infected and occult infection group (P < 0.05), while the level of Alb was higher (P < 0.05) ; the levels of serum PCT, CRP and CRP/Alb in the occult infection group were lower than those in the infected group (P < 0.05). One day after operation, the levels of PCT, CRP and CRP/Alb in the infected group were significantly higher than those in the non-infected group (P < 0.05), while the level of ALB was lower (P < 0.05). One day after admission, the serum CRP level and CRP/Alb ratio of patients were detected separately, but the sensitivity was not high. The AUC of combined detection for predicting preoperative infection was 0.909, the corresponding sensitivity was 90.50%, and the specificity was 81.70%; One day after operation, the serum PCT level and CRP/Alb ratio were detected separately, which had a certain value in predicting preoperative infection. The combined detection of them predicted preoperative infection with AUC of 0.977, corresponding sensitivity of 92.90% and specificity of 98.60%. Conclusions: The serum PCT and CRP/Alb ratio are increased in patients with infection before and after operation, which may be used as biomarkers for perioperative infection of upper urinary tract stones.

411 Effect of PS Combined with Noninvasive Positive Pressure Ventilation on Neonatal Respiratory Distress and Blood Gas Index

Objective: To study the effect of Lung surface active substances (PS) combined with noninvasive positive pressure ventilationin treatment of Neonatal respiratory distressand its effectson blood gas index. Methods: 100 newborn patients with respiratory distress admitted to our hospital from February 2017 to February 2019 were selected for the study,by stochastic indicator method, divided into the observation group (n = 52) and control group (n = 48) and control group using noninvasive positive pressure ventilation therapy, observation group on the basis of this combined therapy with PS, comparing two groups of clinical curative effect, the hydrogen ion concentration index (pH), CO2 partial pressure (PCO2), oxygenation index (PaO2 / FiO2) level, the aeration time level of complications, length of hospital stay, hospital expenses. Results: After treatment, the total effective rate of the observation group was 90.38% higher than that of the control group (P<0.05). Before treatment, there was no significant difference in blood gas index between the two groups (P>0.05). After treatment, blood gas index levels of children in both groups were significantly improved compared with those before treatment (P<0.05). PH, PaO2/FiO2 levels of children in the observation group were significantly higher than those in the control group, and PCO2 levels were significantly lower than those in the control group (P<0.05). Ventilation time, length of stay and cost of hospitalization in the observation group were significantly lower than those in the control group (P<0.05). The total incidence of adverse reactions in the two groups was 19.23% and 43.75%, respectively, with significant differences (P<0.05). Conclusion: PS combined with non-invasive positive pressure ventilation has a significant effect in the treatment of neonatal respiratory distress, which can effectively improve the blood gas index of children, and is worthy of promotion and application.

415 Effect of Dynamic Hip Screw and Proximal Femur on Perioperative Index and Hip Function in Patients with Intertrochanteric Femoral Fractures

Objective: To observe the effect of dynamic hip screw (DHS) and proximal femoral intramedullary screw (PFNA) on perioperative index and hip function in patients with intertrochanteric fracture of femur. Methods: 106 patients with intertrochanteric fracture of the femur admitted to our hospital from May 2017 to May 2019 were selected. They were divided into control groups (N = 53) and observation groups (N = 53) according to the random digital method. The control group used DHS treatment. The observation group used PFNA treatment. The clinical efficacy, operation time, post-operative weight loading time, hospitalization time, fracture healing time, hip function (Harris) score, Bartel index and complication incidence were compared between the two groups. Results: The total effective rate of 92.45 % was significantly higher than that of the control group 73.58 % (P<0.05) ; The time of operation, weight loading, hospitalization and fracture healing were all significantly shorter in the observation group than in the control group, and the amount of bleeding in the operation was less than in the control group (P<0.05) ; The Harris score and Bartel index of the post-treatment observation group were significantly higher than those of the control group (P<0.05) ; The incidence of complications in the observation group was 7.55 % significantly lower than 28.30 % in the control group (P<0.05). Conclusion: PFNA is effective in the treatment of intertrochanteric fractures of the femur. It can improve hip joint function and self-care ability, and has higher safety, which is beneficial to promote patient prognosis.

Objective: To observe the diagnostic value of four-dimensional ultrasound imaging technique for fetal external anomalies. Methods: 200 pregnant women in the hospital were given two-dimensional and four-dimensional ultrasonography in the middle and late stages. With postpartum or induced labor pathological examination as the golden standard,the accuracy, sensitivity and specificity of two-dimensional ultrasound and four-dimensional ultrasound for diagnosis of fetal external anomalies were compared. Results: The sensitivity, specificity, accuracy and Kappa value of two-dimensional ultrasonography for fetal external anomalies were 64.29%,87.10%,85.50%and 0.32.However,those of four-dimensional ultrasonography were 92.86%,97.83%,97.50%and 0.83.The accuracy, specificity and detection rate of fetal external anomalies of four-dimensional ultrasonography were significantly higher than those of two-dimensional ultrasonography (P<0.05). Conclusion: Compared with two-dimensional ultrasound, the accuracy of four-dimensional ultrasound imaging technique is higher for diagnosis of fetal external anomalies. t can detect fetal external anomalies more effectively, with great application value.

421 Exploring the Therapeutic Effect of High Frequency Repetitive Transcranial Magnetic Stimulation and Electrical Stimulation on Dysphagia in Stroke Patients

Objective: To explore the therapeutic effect of high frequency repetitive transcranial magnetic stimulation and electrical stimulation on dysphagia in stroke patients. Methods: 60 stroke patients with dysphagia treated in our hospital from September 2014 to September 2017 were selected,30 cases were divided into the observation group and the control group by simple random grouping method. The control group was treated with electrical stimulation, while the observation group was treated with high-frequency repeated transcranial magnetic stimulation on the basis of the control group. After treatment, the therapeutic effect, drinking water experiment, SSA score, brain-derived neurotrophic factor (BDNF), neuron-specific enolase (NSE) and vascular endothelial growth factor (VEGF) levels of the two groups were compared. Results: After treatment, the total effective rates of the two groups were 96.67% and 73.33%, respectively, with statistical significance (P<0.05). After treatment, both the drinking water experiment and SSA score of the two groups were improved compared with that before treatment, and the drinking water experiment score of the observation group was higher than that of the control group, while the SSA score of the observation group was lower than that in the control group,while the levds of BDNF and VEGF in the observation group were higher than those in the control group (P<0.05). After treatment, the levels of BDNF, NSE and VEGF in the two groups were significantly improved compared with that before treatment, and the levels of NSE in the observation group were lower than those in the control group, while those in the observation group were higher than those in the control group (P<0.05). Conclusion: High frequency repetitive transcranial magnetic stimulation (TMS) can improve the swallowing function of patients with dysphagia after stroke.

425 Preventive Effect of Local Injection of Triamcinolone Acetonide on Nerve Root Edema after Microendoscopic Discectomy of Nucleus Pulposus

Objective: To study the preventive effect of local injection of triamcinolone acetonide on nerve root edema after microendoscopic discectomy of nucleus pulposus. Methods: 80 patients with lumbar disc herniation were divided into control group (n = 38) and observation group (n = 42) according to simple random method. All patients were treated with nucleus pulposus extraction under the intervertebral foramen microscope. 40 mg triamcinolone acetonide was injected into the sheath of compressed nerve root in the observation group, but not in the control group. The occurrence, clinical effect, pain media, VAS, ODI, JOA and safety of the two groups were compared. Results: The incidence of nerve root edema in the observation group was lower than that in the control group (P<0.05). Before operation, there was no significant difference in the levels of pain media, VAS, ODI and JOA scores between the two groups (P>0.05) ; three days and seven days after operation, the levels of pain media, vas and ODI scores in the two groups decreased, the observation group was lower than the control group, JOA scores increased, the observation group was higher than the control group, the levels of pain media, VAS, ODI and JOA scores in the two groups were statistically significant at different times and different treatment methods Significance (P<0.05). After operation, the safety indexes of the two groups had no significant change (P>0.05). Conclusion: Local injection of triamcinolone acetonide has a certain preventive effect on the edema of nerve root after the nucleus pulposus removal under the intervertebral foramen, which can reduce the degree of pain, improve the clinical effect, and is safe and reliable.

429 Effect of Proton Pump Inhibitor Combined with Antibiotic Therapy on Serum Gastrin sIL-2R and CRP Levels and Immune Function in Patients with Hp Positive Chronic Atrophic Gastritis

Objective: To analyze the effect of proton pump inhibitor combined with antibiotic therapy on serum gastrin (GAS), soluble interleukin-2 receptor (sIL-2R) and C-reactive protein (CRP) levels and immune functionin patients with Hppositive chronic atrophic gastritis (CAG). Methods: A total of 150 patients with Hppositive CAG admitted to the Danzhou People's Hospital from April 2016 to April 2018 were enrolled in the study. They were randomly divided into observation group and control group, 75 cases in each group. The control group were treated with proton pump inhibitor, while the observation group additionally took antibiotics (clarithromycin, amoxicillin). The total effective rate of treatment, Hp eradication rate, serum GAS, sIL-2R and CRP levels, immune function (T cell subsets CD4+, CD8+, CD4+/CD8+) before and after treatment and adverse reactions were compared between the two groups. Results: The total effective rate of treatment in the observation group was significantly higher than that in the control group (77.33% vs 61.33%) (P<0.05). The Hp eradication rate in the observation group was also higher than that in the control group (74.67%vs42.67%) (P<0.05). The serum GAS level in the observation group was higher than that in the control group while levels of sIL-2R and CRP were lowerthan the control group after treatment (P<0.05). The levels of CD4+ and CD4+/CD8+ in the observation group were higher than those in the control group, while the content of CD8+ was lower than that in the control group (P<0.05). The difference values of GAS, sIL-2R, CRP, levels of blood CD4+, CD8+ and CD4+/CD8+ in the observation group before and after treatment were higher than those in the control group (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (17.33% vs 13.33%) (P>0.05). Conclusion: Proton pump inhibitor combined with antibiotics is effective in the treatment ofHp-positive CAG. It can downregulate levels of GAS, sIL-2R and CRP, and improve the immune function andHp eradication rate, with safety.

433 Effects of Liraglutide on Endothelial Function and Immune Function in Elderly Patients with Type 2 Diabetes Mellitus Complicated with Hyperlipidemia

Objective: To investigate the effects of liraglutide on endothelial function and immune function in elderly patients with type 2 diabetes mellitus complicated with hyperlipidemia. Methods: 86 elderly patients with diabetes mellitus complicated with hyperlipidemia were enrolled in the general practice department of the hospital from January 2018 to February 2019. They were randomly divided into the control group and the liraglutide group, with 43 cases in each group. The control group was treated with metformin treatment. The liraglutide group was given subcutaneous injection of liraglutide. The two groups were observed for 2 months. Blood glucose and blood lipids were measured before and after the study [blood sugar (FBG), postprandial 2 h blood glucose (2 h PG), glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), low density lipid) Protein (LDL-C), high-density lipoprotein (HDL-C) and body mass index (BMI), comparing endothelial function indicators [ET-1 and NO] and immune function [hypersensitive C-reactive protein (hs-CRP), IgM and IgG]. Results: After treatment with liraglutide group, FBG, 2 h PG, TC and TG were lower than those in the control group (P<0.05). After treatment with liraglutide group, HbA1c, LDL-C and HDL-C were compared with the control group. There was no significant difference between BMI and BMI (P>0.05). After treatment with liraglutide, ET-1 was significantly lower than that before treatment (P<0.01). There was no significant difference in NO after treatment with liraglutide group (P>0.05). After treatment with liraglutide group, ET The level of -1 was lower than that of the control group (P<0.05), and the level of NO was higher in the liraglutide group than in the control group (P<0.05). The hs-CRP was decreased after treatment with the liraglutide group compared with the control group, while the IgG was higher than the control group (P<0.05). Conclusion: The effect of lilalutide on the elderly type 2 diabetes mellitus with hyperlipidemia is more obvious. It can alleviate the damage of endothelial function, increase the immune function and reduce the inflammatory response.

Objective: To observe the effects of salbutamol and ketotifen on serum levels of endothelin-1 (ET-1), nitric oxide (NO) and circulating endothelial cells (CEC) in children with asthma. Methods: 80 children with asthma enrolled in our hospital were chosen. According to the odevity of admission order, they were divided into 2 groups, 40 cases in each group. The control group was given routine treatment and salbutamol, the observation group was given ketotifen on the basis of the control group. The lung function indexes [force expiratory volume in the first second (FEV1), vital capacity (FVC), FEV1/FVC, peak expiratory flow (PEF) ], serum levels of ET-1, NO, CEC in the two groups before and after treatment were compared, the symptom regression time, efficacy and adverse reactions of the two groups were observed. Results: The total response rate of the observation group was significantly higher than that of the control group (P<0.05). The FEV1, FVC, FEV1/FVC and PEF of both groups increased significantly after treatment (P<0.05), and the increase in the observation group was significantly greater than that in the control group (P<0.05). However, the serum levels of ET-1, NO and CEC decreased significantly after treatment (P<0.05), and the decrease in the observation group was significantly greater than that in the control group (P<0.05). The fading away time of cough, lung rale, shortness of breath and wheezing symptoms in the observation group was significantly shorter than that in the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P <0.05). Conclusion: Salbutamol combined with ketotifen in the treatment of children with asthma can promote recovery of lung function and clinical symptoms, which nay be related to reduction of serum ET-1, NO and CEC levels, and the safety is high.

445 Observation on Clinical Effect of Baixuanxiatare Tablet Combined with Adapalene Gel on Acne Vulgaris

Objective: To observe the curative effect of Baixuanxiatare tablet combined with Adapalene Gel on acne vulgaris. Methods: 116 cases of patients with acne disseminata admitted to our hospital from January 2018 to April 2019 were selected and numbered according to the order of admission, 58 cases were randomly selected as the treatment group, and the other 58 cases were used as the control group. The control group was implemented Adapalene Gel treatment, the the treatment group was implemented Baixuan Xiatare Tablet treatment on the basis of control group. The clinical efficacy, TCM syndrome score, serum testosterone (T) and estradiol (E2) levels, sebum secretion status and incidence rate of adverse reactions were compared between the two groups. Results: After treatment, the total effective rate of the treatment group was higher than that of the control group, with statistical difference (94.83%vs70.69%, P < 0.05). Compared with before and after treatment, and the scores of TCM syndromes in the treatment group were lower than those in the control group (7.41±1.73vs13.62±1.95, P<0.05). Compared with before, the serum T levels of the two groups decreased on average, and the treatment group were lower than the control group (562.81±174.26 vs 681.93±194.17, 72.52±21.48 vs 95.49±23.62, P<0.05). The E2 levels were significantly higher than the before treatment (P < 0.05), the treatment group was higher than the control group (48.35±9.26 vs 43.64±6.01, 128.36±28.25 vs 113.94±26.61, P<0.05). The sebum secretion rates of the two groups after treatment were significantly lower than those before treatment (P < 0.05), and the sebum secretion rate of the treatment group was lower than that of the control group (1.04±0.13 vs1.39±0.21, P<0.05). There was no significant difference in the incidence of adverse reactions between the treatment group and the control group (15.52% vs 13.79%, P>0.05). Conclusion: Baixuanxiatare Tablet combined with Adapalene Gel in the treatment of acne disseminata has clinical curative effect. It can significantly improved the levels of serum testosterone and estradiol, effectively inhibits sebum secretion, and has high safety.

450 Comparative Analysis of Mid-and Long-term Effects of Traditional Metal Piles and Fiberglass Piles in the Repair of Oral Roots

Objective: To explore the long-term and long-term effects of traditional metal piles and glass fiber piles in the repair of oral roots. Methods: 94 patients with oral residual root disease admitted to our hospital from January 2016 to July 2018 were enrolled. The patients were divided into two groups according to the random number table method, 47 cases each. The study group (58 teeth) used glass fiber piles for residual root repair, while the control group (56 teeth) used metal piles.The effects of the mid-term (12 months after repair), long-term (24 months after repair) and the health index of the teeth at different times [sulcular bleeding index (SBI), gingival index (GI) ] were compared. Results: At 12 months after repair, the aesthetic effect, prosthetic strength, comfort, and chewing function scores of the study group were higher than those of the control group (P<0.05). The alkaline phosphatase (AKP) level in the control group was significantly higher than that before the repair. High (P<0.05), and there was no significant difference in the AKP level between the study group and the pre-repair (P>0.05).24 months after the repair, the repair effect of the study group (completed restoration, color matching, edge fit) was better than the control group, and the success rate of repair (94.83%) was higher than that of the control group (78.57%). The difference was statistically significant (P <0.05). The incidence of post-core fracture, root fracture, gingivitis, nucleus detachment or loosening, and gingival margin staining in the study group were lower than those in the control group (P<0.05).Repeated measures analysis of variance showed that the interactions between GI and SBI at the time point and group were significant (P<0.05). At 12 months and 24 months after repair, the GI and SBI of the study group were significantly lower. In the control group, the difference was statistically significant (P<0.05). Conclusion: Traditional metal piles and glass fiber piles have certain effects on the repair of oral roots. However, the long-term repair effect of glass fiber piles is better than that of traditional metal piles, which is more conducive to improving the periodontal health index and improving the success rate of repair.

455 Contrast analysis of CT Detection and Liver Biopsy Pathological Diagnosis of Liver Cirrhosis

Objective: To evaluate the diagnostic value of CT detection in the diagnosis of patients with liver cirrhosis by referring to liver biopsypathological diagnosis. Methods: A total of 102 patients with chronic hepatitis B (CHB) admitted to the hospital from March 2018 to March 2019 were selected as the subjects. All of them underwent ultrasound-guided liver biopsy and CT detection. With the results of ultrasound-guided liver biopsy as the golden standard, the diagnostic value of CT detection in patients with liver cirrhosis was evaluated. Results: With the results of liver biopsy pathological diagnosis as the golden standard, the sensitivity, specificity, accuracy and Kappa value of CT detection for liver cirrhosis were 89.23% (58/65), 78.38% (29/37), 85.29% (87/102) and 0.680, respectively.Among the 65 patients with liver cirrhosis confirmed by liver biopsy pathology, CT found that 47.69%had abnormal liver morphology (including hepatic fissure widening and stiffness of liver surface contour), 49.23% had decreased liver CT value lower than that of the same level spleen, 40.00% had abnormal ratio of left to right liver lobe, 38.46% had portal vein outer diameter and spleen vein outer diameter widening, and 63.08% had splenomegaly. The overall coincidence rate of CT detection with liver biopsy pathological diagnosis for grade of liver function was 87.69%. CT signs of benign nodules of liver cirrhosis showed no obvious enhancement signal, but CT of malignant nodules showed a fast-in and fast-out enhancement mode, and the lesions became larger and more in a short time. Conclusion: CT detection is consistent with liver biopsy pathological diagnosis for early diagnosis of liver cirrhosis, evaluation of the severity and differential diagnosis of benign and malignant nodules. It may be an effective means for the diagnosis of liver cirrhosis.

458 Analysis of Sleep Monitoring Characteristics and Clinical Manifestations in Adult Patients with Obstructive Sleep Apnea Hypopnea Syndrome

Objective: To monitor the sleep monitoring characteristics and clinical manifestations of adult patients with Obstructive sleep apnea hypopnea syndrome (OSAHS), and to make a comparative analysis of the sleep monitoring characteristics and clinical manifestations of adult patients of different age groups, so as to provide a basis for the understanding, diagnosis and treatment of clinical OSAHS. Methods: 168 elderly patients with OSAHS were studied from January 2016 to December 2018. They were divided into two groups according to age, namely, 80 cases in the elderly group (elderly population) and 88 cases in the middle-aged and young group (middle-aged and young population). Polysomnography monitoring results, clinical manifestations, personal history and the occurrence of complicated diseases of the two groups were compared and evaluated. Results: The typical clinical manifestations of snoring, apnea, waking up at night and lethargy in young and middle-aged patients were higher than those in the elderly group. Young and middle-aged patients apnea hypopnea index (AHI) higher than the elderly group, elderly group patients CT90 %, micro awakening, oxygen reduction index, Ⅰ period as a percentage of total sleep time, Ⅲ period as a percentage of total sleep time and REM sleep time as a percentage of total sleep time, total sleep apnea time than young and middle-aged group, there were significant differences, P < 0.05;The proportion of patients with coronary heart disease in the elderly group was higher than that in the young and middle-aged group, with statistical difference (P < 0.05). Conclusion: The clinical manifestations of OSAHS patients in the young and middle-aged group were typical, AHI is high and the degree of disease is serious; the clinical manifestations of OSAHS patients in the old group are not typical, sleep quality is poor, the proportion of chronic diseases such as coronary heart disease is high and the risk is high.

463 Meta-analysis of Risk Factors for Acute Renal Injury in Patients with Septic Shock

Objective: To analyze the risk factors of acute kidney injury in patients with septic shock, and to provide reference for early detection of high-risk population and timely prevention of acute kidney injury. Methods: Computer-reviewed literature on risk factors for acute kidney injury in patients with septic shock in CNKI, Wanfang Data Knowledge Service Platform, VIP, CBM, PubMed, Web of Science, The Cochrane Library,using RevMan5.3 to analyze. Results: Nine articles were included, including two cohort studies and seven case-control studies, age [WMD=1.33, 95% CI (-1.73, 4.38), P=0.39], smoking [OR=1.43, 95% CI (1.08, 1.87), P=0.01], drinking [OR=0.90, 95% CI (0.77,1.05), P=0.19], hypertension [OR=1.39, 95% CI (1.19,1.62), P<0.0001], diabetes [OR=1.53, 95% CI (1.38,1.69), P<0.00001], mechanical ventilation [OR= 1.30, 95% CI (1.13, 1.48), P=0.0002], the use of pressure-boosting drugs [OR=2.51, 95% CI (1.35, 4.64).P=0.003],blood transfusion history[OR=3.55,95% CI (1.82,6.91),P=0.0002],contrast medium [OR=3.44, 95% CI (2.03, 5.84), P<0.00001], blood culture positive [OR=1.59, 95% CI (1.17,2.15),P=0.003], APACHEⅡ [WMD=1.96,95% CI (1.49,2.43), P<0.00001], CVP [WMD=3.55, 95% CI (2.89,4.21), P<0.00001]. Conclusion: According to the existing evidence, the risk factors of acute renal injury in septic shock patients are smoking, hypertension, diabetes, mechanical ventilation, use of booster drugs ≥ 2 kinds, blood transfusion history, contrast media, positive blood culture, high Apache Ⅱ score, high CVP.

468 Study on the Relationship between Adverse Pregnancy History and Hypertension in PerimeNopausal Women

Objective: To analyze the relationship between adverse pregnancy and perinatal hypertension, and to provide theoretical basis for the early prevention of cardiovascular disease. Method: 256 female patients aged 40-60 with hypertension who visited to Affiliated Hospital Of Chengde Medical University from August 2018 to August 2019 were selected as the case group, and 314 women aged 40-60 with no hypertension were selected as the control group. Using case-control method, questionnaire survey was carried out, and single factor and multi factor unconditional logistic regression analysis was carried out. Results: Body mass index (or =3.465, 95% CI: 2.175-5.526), gestational hypertension (or = 3.629, 95% CI: 1.080-12.140), premature birth (or = 2.468, 95% CI: 1.073-5.674), age of first delivery (or = 0.468, 95% CI: 0.233-0.938). Conclusions: After adjusting the confounding factors, the population with body mass index ≥ 28KG / m2, preterm delivery and pregnancy induced hypertension were all independent risk factors of perimenopausal hypertension. The women with the first delivery age between 20-35 years old were the protective factors of perimenopausal hypertension. Therefore, it is necessary to strengthen the management of risk factors of perimenopausal hypertension, improve the understanding of hypertension protective factors, and reduce the incidence of hypertension.

473 Analysis on Effects and Clinical Efficacy of Laparoscopic Appendectomy on Nutritional Status and Inflammatory Response of Elderly Patients with Appendicitis

Objective: To study the effects and clinical efficacy of laparoscopic appendectomy (LA) on nutritional status and inflammatory response of elderly patients with appendicitis. Methods: 80 elderly patients with appendicitis in our hospital from January 2017 to August 2018 were selected and divided into OA group and LA group according to the simple randomization, with 40 cases in each group. OA group was treated with OA, and LA group was treated with LA, and the two groups were observed until discharge. The levels of serum albumin, hemoglobin and serum total protein were measured before operation and at 3 d after operation in the two groups. Serum tumor necrosis factor (TNF-α), interleukin-6 (IL-6) and C-reactive protein (CRP) were measured before operation and at 3 d after operation were detected in the two groups. The perioperative indicators were counted in the two groups, and the first postoperative exhaust time, first feeding time, first ambulation time and hospital stay were counted in the two groups, and the incidence rate of postoperative complications during, follow-up was counted in the two groups. Results: On 3 d after operation, the levels of serum albumin, hemoglobin and serum total protein in LA group were higher than those in OA group (P<0.05). The levels of TNF-α, CRP and IL-6 in LA group were lower than those in OA group (P<0.05). The intraoperative blood loss and incision length in LA group were less than those in OA group (P<0.05). The first exhaust time, first feeding time, first ambulation time and hospital stay in LA group were shorter than those in OA group (P< 0.05). There was no significant difference in the incidence rate of postoperative complications between the two groups (P>0.05). Conclusions: LA is more beneficial to the recovery of elderly patients with appendicitis because of less trauma, and it can reduce inflammatory response and affect the nutritional status of patients.

478 Analysis on the Clinical Effect of Targeted Puncture Combined with Staged Injection of PKP in the treatment of Kümmell's Disease in the Thoracolumbar Vertebral Body of the Elderly

Objectives: To explore the clinical effects of targeted puncture combined with staged injection of PKP in the treatment of Kümmell's disease in thethoracolumbar vertebral body of the elderly. Methods: The clinical data of 63 single segment elderly patients with thoracolumbar Kümmell's disease from January 2007 to January 2018 were analyzed retrospectively. PKP was used in combination with targeted puncture and staged injection. The VAS score, the improvement of Cobb angle, the change of anterior height, the Oswestry disability index (ODI) and the leakage of bone cement were compared before and 1 day, 3 months, 6 months and 12 months. Results: 6 patients were lost to follow-up, the follow-up rate was 90. 5%. Fifty-seven patients underwent bilateral pedicle puncture, and the success rate was 100%. During the 12-16 months of follow-up (average 13.68±1.30).48 cases of low back pain basically disappeared, and 9 cases of pain significantly reduced. VAS score, ODI, Cobb Angle and vertebral anterior margin height were significantly improved at 1, 3, 6 and 12 months after surgery (P < 0.05). Bone cement leakage: there were 4 cases of type C, 1 case of type B and 1 case of type S, but there was no intraspinal leakage and no clinical symptoms. Conclusion: Targeted puncture combined with staged injection in the treatment ofKümmell's disease in the elderly can rapidly relieve pain, partially restore vertebral height, improve local kyphosis and improve quality of life, and significantly reduce the rate of bone cement leakage during PKP.

482 Study on the Efficacy and Side Effects of Alprostadil Injection in the Treatment of Acute Ischemic Cerebral Infarction

Objective: To investigate the prognosis and effect of conventional therapy combined with alprostadil injection in the treatment of acute ischemic cerebral infarction. Methods: According to a simple random group, 100 patients with acute ischemic cerebral infarction admitted to our hospital from April 2017 to June 2018 were divided into 50 patients in the study group and the control group. The control group received conventional therapy or oral aspirin. Lyric tablets and atorvastatin calcium tablets, intravenous infusion of Shuxuetong injection, the study group was treated with alprostadil injection on the basis of the control group, comparing the two groups of patients before and after treatment Differences in neurological deficit scores (NIHSS), clinical outcomes, and related indicators. Results: After 1 week and 2 weeks of treatment, the ADL and NIHSS scores of the two groups were significantly improved compared with those before treatment (P<0.05). After 1 week and 2 weeks, the score of the study group improved better than the control group. <0.05). After 1 week and 2 weeks of treatment, CRP, IL-6 and fibrinogen were significantly improved in both groups (P<0.05). After 1 week and 2 weeks, the study group improved. Control group (P<0.05). After 2 weeks of treatment, the total effective rate of the study group was 94%, and that of the control group was 80%. The study group was significantly higher than the control group (P<0.05). The adverse reactions after treatment were counted. In the study group, 1 patient developed mild diarrhea, 3 rashes occurred, 1 patient developed nasal mucosal hemorrhage, and 2 patients had epistaxis in the control group. There were no significant differences in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: The combination of alprostadil and conventional therapy in patients with acute ischemic cerebral infarction has a significant effect on improving clinical outcomes.

486 Clinical Observation of Wuzhi Capsule Combined with Tacrolimus in the treatment of Hepatitis B Virus-associated Nephritis

Objective: To observe the effect of Wuzhi capsule combined with tacrolimus on hepatitis B virus-associated nephritis. Methods: The prospective randomized controlled trial was used to observe the HBV-GN patients who visited the Affiliated Hospital of Weifang Medical College from July 2015 to August 2018. 62 patients meeting the inclusion criteria were divided into treatment group (33 cases) and control group (29 cases). Patients in the treatment group were treated with Wuzhi capsule combined with tacrolimus and entecavir for 12 months, while those in the control group were treated with entecavir alone for 12 months.Alt, AST, ALB, UTP (24h), SCR, HBV DNA titer were analyzed by statistical method. The clinical efficacy of Wuzhi capsule combined with tacrolimus and entecavir in the treatment of hepatitis B-related nephritis was studied. Results: The total remission rate of Wuzhi capsule combined with tacrolimus and entecavir was 87.88% after 12 months, compared with 62.07% in entecavir group (P < 0.05). The serum levels of ALT, AST, ALB, and uTP (24h) in the treatment group were significantly better than those in the control group at 6 and 12 months (P<0.05). After 12 months of treatment, HBV DNA titer decreased in the treatment group, and the difference was statistically significant (P<0.05). The Scr value was no significant change after treatment, there was no difference (P>0.05). Conclusion: Wuzhi capsule combined with tacrolimus and entecavir can effectively induce the remission of HBV-GN, better protect the function of liver and kidney, and have good safety, which is worthy of clinical reference.

490 Observation on EffIcacy of Agliptin Combined with Hyperbaric Oxygen Chamber Exercise Idea on Diabetic Asymptomatic Cerebral Infarction

Objective: To investigate the efficacy of intracavascular embolization (occult intracranial malform) for intracranial hemorrhage (OVMS). Methods: Using prospective research methods, 170 patients with type 2 diabetes mellitus and SBI in our hospital in recent years were selected. According to the different treatment methods, they were divided into two groups, 85 cases in each group. The control group was treated with agliptin. On this basis, the observation group was combined with hyperbaric oxygen chamber exercise ideation treatment to compare the treatment effect and cognitive function changes of the two groups. Results: The total effective rate of the observation group was 85.9%, higher than 65.9% of the control group (P < 0.05). The MOCA score of the observation group was (27.58 ± 0.23) higher than that of the control group (24.32 ± 0.28) (P < 0.05). Conclusion: The combination of agliptin and hyperbaric oxygen chamber exercise therapy can further improve the clinical effect, neurological deficit and cognitive function of patients with diabetes mellitus and SBI.

493 Effect of Intraoperative Real-time Ultrasound-guided Microscopy through Small Bone Window Lateral Fissure Approach for Hypertensive Basal Ganglia Cerebral Hemorrhage

Objective: To analyze the effect of microsurgical treatment of small bone window through lateral fissure in patients with hypertensive basal ganglia intracerebral hemorrhage guided by real-time ultrasound during operation. Methods: The clinical data of 100 patients with hypertensive basal ganglia cerebral hemorrhage admitted to neurosurgery department of our hospital from December 2015 to December 2018 were retrospectively analyzed. According to the surgical methods, 57 patients treated with ultrasound-guided microscopy through small bone window lateral fissure approach were as the research group, and 43 patients with simple microscopy through small bone window lateral fissure approach were as the control group. The changes of operation related indexes, hematoma clearance rate 1 day after operation, GOS score 1 month after operation, ADL grade 6 months after operation and the occurrence of complications were observed. Results: The operation time, bleeding volume and hospitalization time of the study group were significantly better than those of the control group (P < 0.05). The proportion of patients with hematoma clearance rate>85% in the study group was higher than that in the control group, while the proportion of patients with hematoma clearance rate of 50%-85% in the study group was higher than that in the control group (P < 0.05). Compared with the control group, the proportion of patients with GOS score 4 in the study group increased significantly (P < 0.05), the proportion of patients with 3 points decreased significantly (P < 0.05). The proportion of grade II patients in the study group was significantly higher than that in the control group (P < 0.05). The total incidence of postoperative complications was 5.26% in the study group and 13.95% in the control group, and there was no significant difference between the two groups (P > 0.05). Conclusions: Ultrasound-guided microscopy through small bone window lateral fissure approach for hypertensive cerebral hemorrhage in basal ganglia can shorten the operation time, reduce the amount of bleeding, shorten the hospital stay, and has high clearance rate of hematoma and fewer complications, which is conducive to the recovery after operation.

498 Effect of Osteonecrosis of the Femoral Head in the Period of Collapse with Dislocation of Head and Neck and Analysis of Follow-up Imaging Results

Objective: To explore the curative effect of dislocation opening bone grafting in head and neck on osteonecrosis of the femoral head (ONFH) during collapse and follow-up imaging results. Methods: 86 patients confirmed with ONFH during collapse who were admitted to the hospital from April 2015 to April 2017 were enrolled. They were randomly divided into control group and observation group by simple random grouping method 43 cases in each group. The observation group was treated with external dislocation opening bone grafting in head and neck, while control group was treated with core decompression bone grafting. The operation time, intraoperative blood loss, postoperative drainage volume, first leaving bed time and hospitalization time were recorded. Before surgery, at 3 months and 1 year after surgery, VAS and HHS scores were performed to evaluate hip function. At 1 year after surgery, clinical curative effect and imaging curative effect of patients were evaluated by HHS scoring standard and ARCO staging. The complications and occurrence of postoperative hip arthroplasty were recorded. Results: There was no significant difference in the good rate of clinical curative effect and imaging curative effect between observation group and control group (86.05% vs 72.09%; 81.04% vs 65.12%) (P>0.05). The first leaving bed time and hospitalization time in observation group were significantly shorter (P<0.05). At 3 months and 1 year after surgery, VAS score of observation group were significantly lower than those of control group (P<0.05), and HSS score were significantly higher than those of control group (P<0.05). There was no significant difference in complications or incidence of postoperative hip arthroplasty between the two groups (P>0.05). Conclusion: Dislocation opening bone grafting in head and neck can improve clinical curative effect and imaging curative effect of ONFH patients.

502 Study on the Effect of Perioperative Application of Cefotaxime Sodium on Infection in Emergency Trauma Patients

Objective: To investigate the effect of cefotaxime sodium on the infection of patients with trauma in perioperative period. Methods: A total of 120 trauma patients admitted to the emergency department were enrolled between January 2016 and December 2018. They were simple random grouping into two groups according to the random number table: 60 patients in the control group underwent routine clinical therapy. 60 patients in the group were treated with cefotaxime sodium anti-infective treatment based on the same clinical routine treatment as the control group. The changes of body temperature, white blood cells and CRP were measured in the two groups. The clinical efficacy and infection rate of the two treatment methods were evaluated. And wound healing rate, the data results were processed using statistical software SPSS19.0. Results: The study showed that there were no differences in white blood cell and body temperature and CRP levels between the two groups (P>0.05). On the 3rd postoperative day, the body temperature, WBC and CRP levels of the two groups were lower than those before the operation, but the experimental group was significantly lower than the control group (P<0.05). Compared with the control group, the treatment efficiency of the experimental group was significantly better (95.00% vs 76.67%) (P<0.05). After treatment, the infection rate of the experimental group was 3.33%, which was significantly lower than 16.67% of the control group (χ2=5.926, P<0.05). After treatment, the grade A healing rate was 51.67%, the grade B healing rate was 43.33%, and the grade C healing rate was 5.00%. The grade A healing rate was 31.67%, and the grade B healing rate was 38.33%. The rate was 30.00%; the two groups were compared (U=2.395, P=0.031). Conclusion: In the treatment of infection in perioperative emergency trauma patients, cefotaxime sodium has significant effect and high safety.

506 Curative Effect of Pemetrexed Combined with Cisplatin on Advanced Non-small Cell Lung Cancer and the Influences onToxicity and 1-year Survival Rate

Objective: To explore curative effect and safety of pemetrexed combined with cisplatin on advanced non-small cell lung cancer (NSCLC) and the influences on 1-year survival rate. Methods: A retrospective analysis was performed on related data of 85 patients with advanced NSCLC who were admitted to the hospital from June 2016 to June 2018. Patients who were treated with pemetrexed combined with cisplatin or docetaxel combined with cisplatin were included into pemetrexed group (n=42) and docetaxel group (n=43), respectively. The short-term curative effect, changes in tumor marker levels before and after treatment, incidence of toxicity, and long-term curative effect were compared between the two groups. Results: The response rate of treatment in pemetrexed group was higher than that in docetaxel group (54.76% vs 48.84% (P>0.05). The disease control rate in pemetrexed group was significantly higher than that in docetaxel group (92.86% vs 81.40%) (P<0.05). After treatment, levels of tumor markers such as NSE and CA125 in both groups were significantly decreased (P<0.05). After treatment, levels of four tumor markers in pemetrexed group were significantly lower than those in docetaxel group (P<0.05). The incidence of toxicity such as thrombocytopenia, gastrointestinal discomfort, abnormal liver function, anemia and leukopenia in pemetrexed group was lower than that in docetaxel group. There were significant differences in incidence of various toxicity except for abnormal liver function (P<0.05). There was no significant difference in metastatic rate of half a year disease progression between the two groups (P>0.05). The metastatic rate of 1-year disease progression in pemetrexed group was significantly lower than that in docetaxel group (P<0.05). Kaplan-Merier results showed that 1-year survival rate in pemetrexed group was significantly higher than that in docetaxel group (78.6% vs 53.5%). (Log-rank χ2= 5.297, P<0.05). Conclusion: The short-term effective rate of pemetrexed combined with cisplatin in the treatment of advanced non-small cell lung cancer is equivalent to that of docetaxel combined with cisplatin, but its disease control rate and long-term effect are better, and the incidence of complications is low, which can significantly reduce the level of tumor markers.

511 Clinical Efficacy of Yishen Bugu Decoction Combined with PVP in the treatment of Osteoporotic Thoracolumbar Vertebral Compression Fractures

Objective: To study the efficacy of Yishen Bugu decoction combined with percutaneous vertebroplasty (PVP) in the treatment of thoracolumbar osteoporotic vertebral compression fractures (OVCF) and its effects on quality of life of patients. Methods: A total of 132 patients with thoracolumbar OVCF admitted to our hospital from May 2012 to May 2019 were enrolled in this study and divided into two groups according to simple random method, with 66 cases in each group. The two groups were treated with PVP, and control group was given conventional anti-osteoporosis therapy after operation, and observation group was treated with Yishen Bugu decoction on the basis of control group, and they were treated for 3 months. The two groups were followed up for 6 months and the therapeutic effects were observed. Results: The effective rates of treatment in the two groups were 97.26% and 87.76% respectively (P<0.05), and the clinical efficacy in observation group was significantly better than that in control group (P<0.05). The VAS scores in the two groups after operation were decreased significantly with time (P<0.05), and the difference was significant at different time points within the groups (P<0.05), and the VAS scores in observation group at 1 month, 3 months and 6 months after operation were lower than those in control group (P>0.05). At 6 months after operation, the ODI in the two groups were significantly decreased (P<0.05) while the BMD was significantly increased (P<0.05), and the ODI in observation group were lower than those in control group while the BMD was higher than that in control group, and the scores of dimensions of GQOL-74 scale were significantly increased in the two groups (P<0.05), and the scores of physical health, mental health and social function in observation group were higher than those in control group (P<0.05). Conclusion: Yishen Bugu decoction combined with PVP in the treatment of thoracolumbar OVCF is beneficial to relieve postoperative pain and improve spinal deformity and dysfunction, thus improving the quality of life of patients and having a positive effect on enhancing the therapeutic effects.

515 Clinical Observation on 40 Cases of Thumb Radial Retrograde Fascia Flap Repairing the Defect of Finger Tip

Objective: To explore the surgical method and clinical effect of retrograde fascia flap of thumb radial side in repairing the defect of finger tip. Methods: From August 2014 to may 2018, 40 cases of finger tip defect in our hospital were repaired with reverse thumb radial fasciocutaneous flap. Results: All the 40 finger flaps survived in 40 cases. Postoperative follow-up was 12 to 24 months, with an average of 18.8 months, the index finger abdomen is full and the skin flap is of good color and texture. The two-point resolution of the flap was 3.0-5.0mm. According to the trial standard of upper limb function evaluation of hand surgery branch of Chinese medical association, 31 cases were excellent and 9 cases were good, the excellent and good rate were 100%. Conclusion: It is a simple method to repair the defect of the index finger with the reverse fasciocutaneous flap of the radial side of the thumb, which can obtain good function and shape. It is an alternative operation method and is worth popularizing.

518 Effects of Different Proportions of Blood Component Transfusion on Coagulation Parameters, Hemostasis Rate and Short-term Prognosis in Patients with Severe Trauma

Objective: To study the effects of different proportions of blood component transfusion on coagulation function, hemostasis rate and short-term prognosis in patients with severe trauma. Methods: The general data of patients with severe traumaadmitted to our hospital from June 2016 to June 2018 were retrospectively analyzed. According to the infusion proportion of plasma (FP) and red blood cell (RBC), they were divided into group A (FP: RBC≤1:2, n=21), group B (1:2 < FP: RBC < 1:1,n=26) and group C (FP: RBC ≥1:1, n=20).The coagulation function, blood routine, hemostasis rate, survival rate and blood product usage were observed in the three groups before and after blood transfusion treatment. Results: The prothrombin time (PT) and activated partial thromboplastin time (APTT) in the three groups were significantly prolonged compared with those before blood transfusion, and the international normalized ratio (INR) level was increased significantly while the fibrinogen (FIB) level was significantly decreased (P<0.05), and there were significant differences in the PT, INR, APTT and FIB among the three groups after blood transfusion (P<0.05). The level of hemoglobin (Hb) in group C after blood transfusion was significantly lower than that before blood transfusion (P<0.05), and there was no significant difference in Hb level between group A and group B before and after blood transfusion (P>0.05), and the platelet count (PLT) level in the three groups after blood transfusion was significantly lower than that before blood transfusion (P<0.05), and there was a significant difference in the PLT levels among the three groups after blood transfusion (P<0.05). There was no significant difference in the 28d survival rate among the three groups (P>0.05). There were significant differences among the three groups in the FP and cryoprecipitate usage (P<0.05), and there was no significant difference in the RBC usage (P>0.05). Conclusion: Blood component transfusion with proportion of 1:2<FP:RBC<1:1 can effectively correct coagulation disorders and blood routine level and improve the prognosis of patients with severe trauma, thus it is worthy of clinical promotion.

522 Relationship between the Detection Rate of Extended Spectrum β - lactamases (ESBLs) Produced by Gram Negative Bacilli and Bacterial Resistance

Objective: To study the relationship between the incidence of ESBLs production by gram-negative bacilli and the bacterial resistance with clinical experience. Methods: For the gram-negative bacilli isolated in our hospital, the improved three-dimensional test was used to detect the esbls-producing strain, and the isolation rate of the ESBLs producing strain was tested. The drug sensitivity test of gram-positive bacilli was used to find out the possible drug production of the ESBLs strain, and the relationship between it and bacterial resistance was analyzed. Results: The clinical detection rate of 732 strains showed that 224 strains of Communist ESBLs, and the detection rate of ESBLs production was 30.60%. There was significant difference in ESBLs detection rate among strains (χ2= 8.162, P < 0.05). According to the number of ESBLs producing strains, Escherichia coli, Klebsiella pneumoniae, Proteus and Klebsiella acidogenes can be arranged from more to less. After comparison, the detection rate of ESBLs producing strains in each department was statistically significant (χ2=19.863, P < 0.05). ESBLs producing strains were mainly distributed in ICU (47.62%), urology (40.00%), respiratory medicine (30.00%) and Neurology (28.57%). The difference of resistance rate of ESBLs producing strains to antibiotics was statistically significant (χ2= 29.016, P < 0.05). The difference of resistance rate of ESBLs producing strains to antibiotics was statistically significant (χ2= 29.016, P < 0.05). The resistance rate of ESBLs producing strains is higher than that of penicillins, but it is highly sensitive to carbapenems. Among them, ampicillin (98.21%), amikacin (96.43%), cefazolin (91.07%),sulbactam (67.86%) and ciprofloxacin (60.71%) are the top three. Conclusion: The detection rate of ESBLs producing bacteria was high in our hospital, mainly Escherichia coli, Klebsiella acidogenes and Klebsiella pneumoniae. The detection rate of ESBLs of gram-negative bacilli is high and it has multiple drug resistance.
