DOI: 10.3969/j.issn.1006-6233.2020.02.007
Objective: To investigate the mechanism of dexmedetomidine in reducing early brain injury in rats with subarachnoid hemorrhage by down-regulating the expression of NLRP3 inflammatory body. Methods: 100 healthy SD rats were randomly divided into healthy control group, model group and dexmedetomidine group (low dose group, middle dose group and high dose group), 20 rats in each group. Rats in model group and dexmedetomidine group were used to construct subarachnoid hemorrhage model by internal carotid artery puncture. After the model was established, surviving rats were identified as successful subarachnoid hemorrhage model by Garcia method. After the completion of the model, rats in the model group were injected with normal saline and rats in the dexmedetomidine group were injected with high, medium and low doses of dexmedetomidine respectively. The rats were sacrificed after 7 days of continuous administration. The amount of subarachnoid hemorrhage and water content in the brain were measured and compared. The levels of serum inflammatory factors were detected by ELISA, and the levels of NLRP3 were detected by immunohistochemical method. Pearson correlation test was used to analyze the correlation between inflammatory factors such as interleukin-1 beta (IL-1 beta), tumor necrosis factor-alpha (TNF-alpha), interleukin-6 (IL-6) and NLRP3 levels. Results: Compared with healthy control group, the amount of subarachnoid hemorrhage and water content in brain of model group and dexmedetomidine group increased significantly, and the amount of subarachnoid hemorrhage and water content in brain of dexmedetomidine group decreased. The amount of hemorrhage and water content of dexmedetomidine group decreased with the increase of dosage. There was significant difference between the two groups (P<0.05). Compared with the healthy control group, the levels of IL-1beta, TNF-alpha, IL-6 and NLRP3 in the model group and dexmedetomidine group were significantly higher, and the levels of inflammatory factors in dexmedetomidine group were lower than those in the model group. The levels of inflammatory factors in dexmedetomidine group decreased gradually with the increase of dosage, and there were significant differences among the groups (P<0.05). Pearson correlation test showed that NLRP3 was positively correlated with IL-1beta, TNF-alpha and IL-6 (P<0.05). Conclusion: dexmedetomidine can reduce the degree of early brain injury, inflammatory response and NLRP3 level in rats with subarachnoid hemorrhage, and the mechanism may be related to down-regulation of the expression of NLRP3 inflammatory bodies.

DOI: 10.3969/j.issn.1006-6233.2020.02.008
Objective: To investigate the influence of Logistic regression analysis on the related factors of in-stent restenosis (ISR) in patients with coronary atherosclerotic heart disease (CHD) after stent implantation. Methods: The clinical data of 105 patients with CHD who underwent stent implantation were retrospectively analyzed. According to the occurrence of ISR after stent implantation, the related factors affecting the occurrence of ISR after stent implantation were analyzed by single factor and multi-factor logistic regression. Results: Among 105 patients with CHD, there were 19 cases of ISR with the incidence 18.10% after operation. Univariate analysis showed that the incidence of ISR in patients with family history of CHD, diabetes mellitushistory, hypertensionhistory, smoking history, stent ≥ 3 pieces, stent length ≥ 30 mm, stent diameter < 3.00 mm, tandem stent, low-density lipoprotein-cholesterol (LDL-C) ≥ 3.37 mmoL/L, routine dosage of statins and discontinuation of aspirin were higher than those without family history of CHD, no diabetes mellitus history, nohypertensionhistory, nosmokinghistory, stent < 3 pieces, stent length < 30 mm, stent diameter < 3.00 mm, noseries stent, LDL-C < 3.37 mmoL/L, high dose of statins, no discontinuation of aspirin (P<0.05). Multivariate logistic regression analysis showed that family history of CHD, history of diabetes mellitus, history of hypertension, smoking history, stent ≥ 3 pieces, stent length ≥ 30 mm, stent diameter < 3.00 mm, tandem stent, LDL-C ≥ 3.37 mmoL/L, routine dosage of statins and discontinuation of aspirin were independent risk factors for coronary ISR in patients with CHD after stent implantation (P<0.05). Conclusion: There are many factors influencing the occurrence of ISR in patients with CHD after stent implantation. Therefore, it is necessary to evaluate the factors reasonably in clinic, and to improve the prognosis of patients with possible ISR in time and with emphasis on the treatment of patients with underlying diseases and the results of coronary angiography.

HU Yu, ZHOU Mengjun, et al
DOI: 10.3969/j.issn.1006-6233.2020.02.015
Objective: To explored the proliferation and migration of (PASMC) in rat pulmonary artery smooth muscle cells under the intervention of connective tissue growth factor (CTGF), PD98059 and LY294002. Method: PASMC was isolated and cultured, and then were divided into blank group, CTGF group, CP Group, CL group and CPL group. The effects of CTGF, PD98059 and LY294002 on cell proliferation were detected by WST-1, and the effects on cell migration were detected by Transwell, cell scratch and cytoskeleton. Results: Compared with the blank group, the proliferation level of WST-1 cells increased after CTGF (50NG/ml) stimulation of pasmc48, 72, 96 and 120 hours, the number of transmembrane cells increased after 6h and 12h, the scratch width of cells decreased after 12h and 24h, and the cytoskeleton was fuzzy, the difference was statistically significant (P<0.05). After PD98059 (20 μ mol / L) and / or LY294002 (10 μ moL/L) intervention for 24, 48, 72, 96 and 120 hours, WST-1 showed a decrease in cell proliferation; after 6 and 12 hours, Transwell showed a decrease in the number of transmembrane cells, and after 12 and 24 hours, the scratch width and cytoskeleton of the cells were increased, the difference was statistically significant (P<0.05). Conclusion: CTGF can stimulate the proliferation and migration of PASMC and may be one of the factors to promote the pulmonary vascular remodeling of pulmonary hypertension (PAH). PASMC proliferation and migration can be inhibited by PD98059 and / or LY294002, suggesting that ERK1 / 2 and PI3K pathways blocked by CTGF may participate in the pulmonary vascular remodeling of PAH stimulated by CTGF.

WU Xu, YAO Mengwei, GUO Zhen
DOI: 10.3969/j.issn.1006-6233.2020.02.029
Objective: To analyze the correlation between the levels of inflammatory factors in synovial fluid and X-ray classification and knee function in patients with traumatic arthritis. Methods: 85 patients with traumatic arthritis (case group) and 45 health examinees (control group) were selected from January 2017 to January 2019. The levels of interleukin-6 (IL-6), interleukin-8 (IL-8) and tumor necrosis factor-α (TNF-α) in the synovial fluid of the two groups were compared. The differences of IL-6, IL-8 and TNF-α level in synovial fluid of patients with different X-ray grades were compared. Pearson correlation was used to analyze the relationship between KSS, HSS and IL-6, IL-8 and TNF-α level in synovial fluid. Results: The levels of IL-6, IL-8 and TNF-alpha in the synovial fluid of the case group were significantly higher than those of the control group (P<0.01). X-ray examination showed that there were 35 cases of grade I, 21 cases of grade II, 19 cases of grade III and 10 cases of grade IV. The levels of IL-6, IL-8 and TNF-α in synovial fluid of patients with different X-ray grading were significantly different (P<0.01), suggesting that the levels of IL-6, IL-8 and TNF-α in synovial fluid would increase with the increase of X-ray grading and the aggravation of bone injury. The average KSS score and HSS score were (63.47±10.46) and (67.98±13.28) respectively. Pearson correlation analysis showed that KSS score and HSS score at admission were negatively correlated with IL-6, IL-8 and TNF-α in synovial fluid (P<0.01), but not with ages and body mass index (P>0.05). DOI: 10.3969/j.issn.1006-6233.2020.02.031
Objective: To study the effects of MRI scan combined with serum GP73 and AFP levels on the diagnostic efficacy of primary hepatic carcinoma (PHC). Methods: 92 patients with suspected PHC from June 2018 to June 2019 in our hospital were selected for the study. According to the results of pathological examination, the patients were divided into PHC group (47 cases) and benign group (45 cases). 50 healthy examiners in our hospital at the same period were selected as control group, and the MRI scan was performed, and the expression levels of serum alpha-fetoprotein (AFP) and Golgi glycoprotein 73 (GP73) were measured and compared. Kappa consistency test was used to analyze the value of each method for the diagnosis of PHC alone or in combination. Results: MRI showed 48 patients with PHC, including 26 cases of nodular type, 17 cases of massive type and 5 cases of diffuse type. T1WI mainly showed even low or slightly low signal, and T2WI mainly showed uneven high signal, and DWI showed obviously enhanced tumor tissues, and the signal intensity was higher than that of surrounding normal tissues, and some cases showed high signal necrosis and low signal hemorrhage. The ADC value in PHC group was significantly lower than that in benign group and control group (P<0.05), and the levels of serum AFP and GP73 were significantly higher than those in benign group and control group (P<0.05), and the ADC value in benign group was significantly lower than that in control group (P<0.05), and the levels of serum AFP and GP73 were significantly higher than those in control group (P<0.05). Taking pathological results as the gold standard, and consistency Kappa test showed that levels of serum AFP and GP73 had lower diagnostic efficacy for PHC (Kappa value <0.5), and combined with MRI examination could effectively improve the diagnostic efficacy (Kappa value 0.5~0.7), and MRI had good diagnostic value for PHC (Kappa value>0.7), and the combined diagnostic accuracy of the three could be further improved (Kappa value > 0.7). Conclusion: MRI scan combined with serum GP73 and AFP levels can effectively improve the diagnostic accuracy of PHC, and it is of great significance for early treatment of PHC and improvement of prognosis.

DOI: 10.3969/j.issn.1006-6233.2020.02.039
Objective: To research Shenshao Oral Liquid Inhibits Secondary Inflammatory Injure Induced by CD14/TLR4 after Intracerebral Hemorrhage in Experimental Rats. Methods: Experimental rats were randomly divided into sham operation group, model group and intervention group. Each group further was subdivided into 6 h group, 12 h group, 24 h group, 72 h group, and 7 d group. ICH group and SS group were used to duplicate the model of cerebral hemorrhage. SS group was given Shenshao oral liquid (8ml/d) by gavage after waking up. The number and protein expression of CD14, TLR4 and IL-8 positive cells were detected by immunohistochemistry and Western blotting. Results: The edema was appeared after 6h after the ICH model was established, and the water content of brain was gradually increased at 12h to 72h, and reached to the highest level at 72h, then the brain edema was gradually decreased, and ultimately got right original level at 7d. Compared with the ICH group, the water content of brain in the rats of the intervention group was obviously decreased (P<0.05). The expression of CD14 in brain tissue cells of sham group was slight; CD14 protein began to express around hematoma in ICH group at 6h after operation, and a small number of positive cells increased with time, reached the peak at 72h, and then decreased gradually. The trend of TLR4 and IL-8 protein expression was consistent with that of CD14. The expression level of TLR4 and CD14 was positively correlated with brain water content (P<0.05), and the expression level of TLR4 and IL-8 was significantly decreased after Shenshao oral liquid. Conclusion: Shenshao oral liquid may reduce the inducing factors of CD14 / TLR4 Signal pathway, reduce the secondary inflammatory injury of brain tissue around hematoma after cerebral hemorrhage, and play a protective role in brain tissue.

ZHENG Chunjiao, WU Zhiming, HUANG Sujing, et al
DOI: 10.3969/j.issn.1006-6233.2020.02.041
Objective: To explore the relationship between cytomegalovirus infection and serum TNF-alpha, IL-1 and IL-6 in patients with spontaneous abortion. Methods: The clinical data of 110 cases of spontaneous abortion pregnant women (observation group) and 70 cases of normal pregnant women (control group) were collected from January 2017 to January 2019. The observation group was divided into two subgroups according to the detection results of HCMV-IgM, HCMV-DNA and HCMV-RNA in peripheral blood, and either of them was positive. For HCMV active infection, 26 cases were active infection and 84 cases were inactive infection. The positive rates of HCMV-IgM, HCMV-DNA and HCMV-RNA, the levels of inflammatory cytokines (TNF-a, IL-1, IL-6) and the levels of inflammatory cytokines in active infection group and inactive infection group were compared between the observation group and the control group. The relationship between HCMV active infection and TNF-a, IL-1, IL-6 was analyzed. Results: The positive rates of HCMV-IgM, HCMV-DNA and HCMV-RNA in the observation group were 37.37%, 16.36% and 2364% respectively, which were significantly higher than those in the control group (7.14%, 4.29% and 5.71%), the differences were statistically significant (P<0.05). The serum levels of TNF-alpha, IL-1 and IL-6 in active infection group were higher than those in inactive infection group, the differences were statistically significant (P<0.05), it is suggested that serum TNF - α, IL-1 and IL-6 levels are related to active infection, and the higher the level, the higher the risk of active infection. Conclusion: HCMV infection is one of the important causes of spontaneous abortion, and HCMV infection is related to the levels of TNF - α, IL-1 and IL-6. The higher the level, the higher the risk of active infection, may play an important role in the occurrence of abortion.
