Relationship between Levels of Serum Cholinesterase NT-pro BNP and PCT and Prognosis in Patients with Heart Failure and Pulmonary Infection

Objective
: To explore the clinical value of serum cholinesterase (CHE), N-terminal pro-brain natriuretic peptide (NT-proBNP) and procalcitonin (PCT) on diagnosis and prognosis assessment of patients with heart failure and pulmonary infection.
Methods
: The clinical data of 120 patients with heart failure and pulmonary infection were retrospectively analyzed. According to the New York Heart Association (NYHA) cardiac function grading and 28d survival, they were divided into grade II group (n=42), grade III group (n= 49) and grade IV group (n=29) and death group (n=23) and survival group (n=97), and the clinical data of 162 patients with simple heart failure were included in controls. The levels of serum CHE, NT-proBNP and PCT were compared among patients with different disease condition and prognosis. The correlation between the above indexes and cardiac function was analyzed by Spearman correlation coefficient model. Receiver operating characteristic (ROC) curves were used to analyze the diagnostic efficacy of the above indexes in predicting 28d survival of patients with heart failure and pulmonary infection.
Results
: Serum CHE level in patients with heart failure and pulmonary infection was significantly lower than that of patients without pulmonary infection while the levels of serum NT-proBNP and PCT were significantly higher than those of patients without pulmonary infection (P<0.05). Among patients with heart failure and pulmonary infection with different cardiac function, serum CHE level showed grade II group> grade III group> grade IV group, and the levels of serum NT-proBNP and PCT showed grade II group< grade III group< grade IV group (P<0.05). Serum CHE level was significantly negatively correlated with cardiac function in patients with heart failure and pulmonary infection (r<0, P<0.05) while levels of serum NT-proBNP and PCT were significantly positively correlated with cardiac function (r>0, P<0.05). Serum CHE level in death group was significantly lower than that in survival group while levels of serum NT-proBNP and PCT were significantly higher than those in survival group (P<0.05). The AUC values of ROC curves of CHE, NT-proBNP and PCT in predicting 28d death of patients with heart failure and pulmonary infection were significantly smaller than those of combination prediction (P<0.05), and the cut-off values were 4.18 kU/L, 11.61 μg/L and 10.04 μg/L respectively.
Conclusion
: Serum CHE, NT-proBNP and PCT levels are closely related to the prognosis of patients with heart failure and pulmonary infection. Early clinical combination testing can help guide the diagnosis and treatment plan, so as to take appropriate intervention measures to improve the prognosis as soon as possible.

Effects of Hemocoagulase combined with Omeprazole Sodium on Gastric Acid Secretion Function and Levels of Serum EGF VEGF and SOD in Patients with Peptic Ulcer

Objective
: To explore the efficacy of hemocoagulase combined with omeprazole sodium on gastric acid secretion function and levels of serum EGF, VEGF and SOD in patients with peptic ulcer.
Methods
: A total of 120 patients with peptic ulcer admitted to our hospital from February 2017 to February 2019 were selected and divided into observation group (Hemagglutinase combined with omeprazole sodium is injected intravenously) and control group (Intravenous infusion of omeprazole sodium) according to the treatment method, with 60 cases in each group. The blood loss volume, blood transfusion volume, hemostasis time, hospital stay, healing effects of ulcer wounds and indexes of gastric acid secretion function [basic gastric acid secretion (BAO), maximum gastric acid secretion (MAO) and peak gastric acid secretion (PAO)] and levels of serum EGF, VEGF and SOD before and after treatment were recorded and compared between the two groups.
Results
: The blood loss volume and blood transfusion volume in observation group were significantly less than those in control group, and the hemostasis time and hospital stay were significantly shorter than those in control group (P<0.05). The total effective rate of wound healing in observation group was significantly higher than that in control group (P<0.05). The levels of BAO, MAO and PAO in the two groups were significantly decreased after treatment, and the levels in observation group were lower than those in control group (P<0.05). The levels of EGF, VEGF and SOD after treatment were significantly increased in the two groups, and the levels in observation group were higher than those in control group (P<0.05).
Conclusion
: Hemocoagulase combined with omeprazole sodium has better hemostasis effects on patients with peptic ulcer, and it can promote ulcer wound healing.

The Relationship between the Awareness of Disease Status and Quality of Life Anxiety Depression Dignity in Chinese Cancer Patients

Objective
: To assess the levels of Chinese cancer patients’ awareness of disease status, and analyze the correlation between the awareness of disease status and quality of life, anxiety, depression, dignity.
Methods
: By cross-sectional investigation, cancer patients who were hospitalized in the fourth hospital of Hebei Medical University were selected as the research objects. According to the interview with patients, the degree of knowledge about the disease was evaluated. The self-assessment scale of anxiety and depression (SAS, SDS), dignity scale (PDI) and quality of life questionnaire (QLQ-C30) were used to investigate the relationship between the state of being informed and the patients' quality of life, anxiety, depression and dignity.
Results
: A total of 457 cancer patients were included in the study, 308 of whom (67.40%) were in advanced stage of cancer. Among them, 73 patients (15.97%) were completely unknown, 145 (31.73%) were partially informed, and 239 (52.30%) were fully informed. 85 patients (18.60%) had anxiety, 333 patients (72.87%) had depression, 69 patients (15.10%) had anxiety and depression at the same time, 423 patients (92.56%) had different degrees of dignity damage. Partial correlation analysis showed that there was a significant correlation between the degree of informed disease and multiple dimensions of quality of life (emotional function, cognitive function, social function, fatigue, appetite reduction symptoms and economic difficulties) (P<0.05), anxiety (rs = -0.132, P = 0.005) and dignity impairment (rs = -0.096, P = 0.041).
Conclusion
: Nearly 50% of cancer patients have no or incomplete knowledge of their condition. The lower the level of patients' knowledge of their condition, the worse the quality of life, the more easily their dignity is damaged, and the more likely they are to have negative emotions, especially anxiety. In clinical work, we should try our best to take appropriate methods to let patients know their own conditions, which is conducive to improving the quality of life of patients, improving their negative emotions, so as to better implement the whole process of cancer diagnosis and treatment strategy.

Effect of Amifostine combined with GP Regimen on the Short-term Efficacy and Adverse Reactions of Chemotherapy in Patients with Advanced Breast Cancer

Objective
: To analyze the effect of amifostine combined with gemcitabine + cisplatin (GP) regimen on short-term efficacy and adverse reactions of chemotherapy in patients with advanced breast cancer.
Methods
: The clinical data of 66 patients with advanced breast cancer diagnosed by pathology and cytology from July 2017 to December 2018 were analyzed retrospectively. 66 patients were divided into control group (30 cases) receiving GP treatment and combined group (36 cases) treated by amifostine combined with GP. The short-term efficacy and adverse reactions of chemotherapy were compared between the two groups.
Results
: There was no significant difference in total effective rate and disease control rate between the two groups (both P> 0.05). There was no significant difference in the incidence of nausea, vomiting, dizziness, liver dysfunction and renal dysfunction between the two groups (all P>0.05), but the incidence rate of peripheral neurotoxicity and myelosuppression in the combined group was significantly lower than that in the control group (both P<0.05).
Conclusion
: The short-term efficacy of amifostine combined with GP regimen in the treatment of advanced breast cancer is similar to that of GP regimen alone, but amifostine combined with GP regimen can effectively decrease the incidence rate of peripheral nerve toxicity and myelosuppression, and does not cause serious adverse reactions of chemotherapy, with good tolerance of patients.

Comparison of Efficacy and Safety of Recombinant Tissue Plasminogen Activator in Intravenous Thrombolysis for Cardiogenic and Non-cardiogenic Cerebral Infarction

Objective
: To compare the efficacy and safety of recombinant tissue plasminogen activator (rt-PA) in intravenous thrombolysis for cardiogenic cerebral infarction (CCI) and non-cardiogenic cerebral infarction (NCI).
Methods
: Retrospective study was conducted to collect the clinical data of 60 patients with hyperacute cerebral infarction treated by rt-PA intravenous thrombolysis from January 2017 to December 2018 in our hospital, and they were divided into NCI group (24 cases) and CCI group (36 cases). The differences of National Institutes of Health Stroke Scale (NIHSS) and modified Rankin scale (mRS) scores and adverse events before and after treatment were compared between the two groups.
Results
: Compared with before treatment, NIHSS scale scores of both groups were significantly decreased after treatment (P<0.01). The NIHSS scale scores of CCI group before, 1 day after and 7 days after treatment were not statistically significant compared with those of NCI group (P>0.05). After treatment, mRS scale scores of the two groups were significantly lower than before treatment (P<0.01). There was no significant difference in mRS scale between CCI group and NCI group before, 1 month after and 3 months after treatment (P>0.05). There was no significant difference between the two groups in the prognosis at 3 months after treatment (P>0.05). There was no statistical significance in the comparison of reinfarction, asymptomatic cerebral hemorrhage, symptomatic cerebral hemorrhage and mortality between the two groups (P>0.05). The incidence of skin mucosal bleeding and brain hernia in CCI group was significantly higher than that in NCI group (P<0.05).
Conclusions
: Rt-pa intravenous thrombolytic therapy in hyperacute CCI and NCI patients within 4.5h can effectively alleviate neurological defects, and the prognosis of patients is effective. The overall efficacy and prognosis effect of the two are similar, but the risk of bleeding and adverse events after thrombolysis in CCI patients is significantly increased.

Effect of Compound Anisodine on Serum MDA and SOD Levels in Patients with Acquired Paralytic Strabismus

Objective
: To explore the effects of compound anisodine on serum malondialdehyde (MDA) and superoxide dismutase (SOD) levels in patients with acquiredparalytic strabismus.
Methods
: A total of 100 patients with acquiredparalytic strabismus from April 2017 to August 2018 in the hospital were selected and randomly divided into observation group and control group, with 50 cases in each group. Control group was given routine treatment, and observation group was given compound anisodine on the basis of control group. The clinical efficacy, MDA and SOD levels, hemodynamic indexes and occurrence of adverse reactions were compared between the two groups.
Results
: The effective rate of clinical treatment in observation group was higher than that in control group (96.00% vs 80.00%) (P<0.05). The non-diplopia area after treatment in observation group was higher than that in control group, and the prism strabismus was lower than that in control group (P<0.05). The SOD level after treatment in observation group was higher than that in control group while the MDA level was lower than that in control group (P<0.05). The whole blood viscosity, plasma viscosity, erythrocyte aggregation index and hematocrit value in observation group were lower than those in control group (P<0.05). There were no obvious adverse reactions during treatment in the two groups.
Conclusions
: Compound anisodine can increase the area without diplopia, reduce the degree of prism strabismus, decrease the content of MDA in serum, increase the level of SOD and improve the index of hemorheology.

Efficacy of Active Vitamin D combined with Cinacalcet on Secondary Hyperparathyroidism in Dialysis Patients with Chronic Renal Failure and its Effects on Levels of TGF-β1 and sTfR

Objective
: To observe the efficacy of active vitamin D combined with cinacalcet in the treatment of secondary hyperparathyroidism (SHPT) in dialysis patients with chronic renal failure and its effects on levels of serum transforming growth factor-β1 (TGF-β1) and serum transferrin receptor (sTfR).
Methods
: 90 patients with chronic renal failure who underwent MHD causing moderate-to-severe SHPT were randomly divided into control group and observation group, with 45 cases in each group. Control group was treated with calcitriol, and observation group was given calcitriol combined with cinacalcet. The clinical efficacy and changes of parathyroid size, blood calcium, blood phosphorus, intact parathyroid hormone (iPTH), renal function indexes, TGF-β1 and sTfR before and after treatment were compared between the two groups, and adverse reactions were observed in the two groups.
Results
: The total effective rate was 93.33% in observation group and 75.56% in control group (P<0.05). After treatment, the decreases of length, width and thickness of parathyroid glands in observation group were more significant than those in control group (P<0.05), and the increase of blood calcium was more significant than that in control group (P<0.05), and the decreases of blood phosphorus and iPTH were more significant than those in control group (P<0.05). There were no significant changes in renal function indexes [serum creatinine, urea nitrogen, urea clearance index] before and after treatment between the two groups (P>0.05). After treatment, the serum levels of TGF-β1 and sTfR were significantly decreased in the two groups (P<0.05), and the decreases in observation group were more significant than those in control group (P<0.05). There was no significant difference in the incidence rate of adverse reactions between the two groups (P>0.05).
Conclusion
: Calcitriol combined with cinacalcet has significant efficacy in the treatment of SHPT in MHD patients with chronic renal failure, and it can effectively improve the high iPTH status and calcium and phosphorus imbalance, reduce the expressions of TGF-β1 and sTfR, and will not adversely affect residual renal function, with good safety.

Effect of Intraperitoneal Perfusion in Patients with Ovarian Cancer and its Effect on Serum bFGF HE4 and AFP

Objective
: To research the effect of intraperitoneal perfusion in patients with ovarian cancer and its effect on serum fibroblast growth factor(Bfgf), Humanepididymisprotein4(HE4) and alpha fetoprotein(AFP).
Methods
:According to the treatment methods, 90 patients with ovarian cancer admitted to our hospital from March 2014 to March 2015 were divided into experimental group and control group, 45 cases in each group. Both groups were given hydration (intravenous drip of 20 mL potassium chloride + 1000 mL normal saline), diuresis, antiemetic, etc., and tumor cell reduction surgery. On this basis, the control group was treated with neoadjuvant chemotherapy. The experimental group was treated with neoadjuvant chemotherapy and intraperitoneal perfusion before operation, and continued to be treated with TP after operation. Each cycle was 28 days, and the treatment course of the two groups was 6-8 cycles. The operation indexes (operation time, intraoperative bleeding volume, ascites volume), serum bFGF, HE4, AFP levels, adverse reactions and therapeutic effect were compared between the two groups.
Results
: The operation time, the amount of blood loss and the volume of ascites in the experimental group were significantly lower than those in the control group[(136.08±17.35)min vs(160.26±20.64)min,(540.21±121.36)mL vs(610.36±134.08)mL,(510.32±113.65)mL vs(917.87±132.48)mL](P<0.05); The levels of bFGF, HE4 and AFP were significantly lower than those of the control group[(65.90±9.36)ng/L vs(96.34±12.75)ng/L,(42.69±8.17)pmol/L vs(75.38±13.49)pmol/L,(5.83±1.02)ng/mL vs(13.97±3.57)ng/mL](P<0.05); The total incidence of adverse reactions was significantly lower than that of the control group [20.00%(9/45)vs 46.66%(21/45)](P<0.05); Total remission rate was significantly higher than that of the control group [91.11%(41/45)vs 71.11%(32/45)](P<0.05).
Conclusion
: The application of intraperitoneal perfusion in patients with ovarian cancer has a significant effect, which can effectively improve the clinical symptoms of patients, reduce the level of serum bFGF, HE4, AFP, promote the apoptosis of tumor cells, inhibit the formation of blood vessels, reduce toxic and side effects, and improve the prognosis.

Clinical Value of Cervista HR-HPV DNA Combined with TCT in Cervical Cancer Screening

Objective
: To study the clinical value of cervista HR-HPV DNA combined with TCT in cervical cancer screening, 500 suspected cervical cancer patients were selected and tested with single cervista HR-HPV DNA, single TCT and cervista HR-HPV DNA combined with TCT.
Methods
: A total of 500 patients with suspected cervical cancer were selected. Time selection: February 2016 to February 2018, respectively, single cervista HR-HPV DNA test, single TCT test, cervista HR-HPV DNA combined with TCT test, pathological results as a diagnostic gold standards were used to analyze the data of diagnostic specificity, sensitivity, and accuracy of single cervista HR-HPV DNA assay, single TCT assay, and cervista HR-HPV DNA combined with TCT assay.
Results
: Compared with the single cervista HR-HPV DNA test and single TCT test data, the data of the cervista HR-HPVDNA parallel TCT test was higher than that of the single cervista HR-HPV DNA test. The data difference was significant, P<0.05. However, the cervista HR-HPV DNA was connected in parallel. The diagnostic specificity of TCT was lower than that of single cervista HR-HPV DNA and single TCT. The data was significantly different (P<0.05). The diagnostic specificity, sensitivity and accuracy of single cervista HR-HPV DNA were higher than single TCT. Data were detected and the data were significantly different, P<0.05.
Conclusion
: The clinical value of cervista HR-HPVDNA combined with TCT in cervical cancer screening is significant, which can improve the diagnostic value of patients and obtain higher diagnostic specificity, sensitivity and accuracy.

Changes of Serum CA125 VEGF-C and Sex Hormone Levels before and after Hysteromyoma Surgery and the Relationship with Prognosis

Objective
: To observe the changes of serum cancer antigen (CA125), vascular endothelial growth factor-C (VEGF-C) and sex hormone levels before and after the operation of hysteromyoma, and to study the relationship between these three indexes and the prognosis of patients.
Methods
: From January 2017 to may 2018, 120 cases of hysteromyoma in our hospital were selected for surgical treatment as the study object. All patients underwent laparoscopic myomectomy, Serum CA125, VEGF-C, follicular stimulating hormone (FSH), luteinizing hormone (LH), progesterone (P), estradiol (E2) levels were compared between the two groups before and 7 days after operation, and the pregnancy of patients with pregnancy requirements (n = 75) and those without pregnancy requirements (n = 75) within 12 months after operation were followed up (n = 45).
Results
: The levels of serum CA125, VEGF-C, P and E2 in all patients were significantly lower than those before operation (P<0.05 or P<0.01), and the levels of serum FSH and LH were significantly higher than those before operation (P<0.05 or P<0.01). The follow-up results showed that the menstrual volume, dysmenorrhea and other clinical symptoms of 120 patients were significantly improved.
Conclusion
: Surgical treatment of hysteromyoma can effectively improve the changes of serum CA125, VEGF-C and sex hormone levels, and improve the prognosis of patients.

Effect of Octreotide combined with Alprostadil in the Treatment of Hyperlipidemic Acute Pancreatitis and the Effect on Gastrointestinal Function Recovery and Negative Conversion Rate of Blood Amylase

Objective
: To observe the curative effect of octreotide combined with alprostadil in the treatment of hyperlipidemic acute pancreatitis (HAP) and the effect on gastrointestinal function recovery and negative conversion rate of blood amylase.
Methods
: A total of 100 patients with HAP admitted to the hospital from May 2016 to May 2019 were selected and divided into 2 groups by the random number table method, 50 cases in each group. Both groups were given standardized medical treatment after admission. The control group was treatment with intravenous infusion of alprostadil and the observation group was additionally treated with continuous intravenous infusion of octreotide for 10 days. The negative conversion rates of blood amylase after 5 days and 10 days of treatment, gastrointestinal function recovery, time for blood amylase recovery, length of hospital stay, levels of serum triglyceride (TG), interleukin-6 (IL-6), tumor necrosis factor (TNF-α), C-reactive protein (CPR) and blood rheological indicators were compared between the two groups. The incidence of complications in both groups was recorded.
Results
: For mild patients, the total response rate of the observation group was significantly higher than that of the control group. The negative conversion rate of blood amylase in the observation group after 5 days of treatment was significantly higher than that in the control group (P<0.05). However, for severe patients, there was no significant difference in the total response rate or the negative conversion rate of blood amylase between the two groups (P>0.05). The time for gastrointestinal function recovery and blood amylase recovery in the observation group was significantly shorter than that in the control group. The length of hospital stay was also significantly shorter than the control group (P<0.05). The levels of TG, IL-6, TNF-α, CRP and blood rheological indicators in both groups were decreased significantly after treatment, and the levels were lower in the observation group than the control group (P<0.05). The incidence of complications in the observation group was significantly lower than that in the control group (8.00% vs 22.00%) (P<0.05).
Conclusion
: Octreotide combined with alprostadil can effectively reduce levels of blood lipids in patients with HAP, promote gastrointestinal function recovery, increase the negative conversion rate of blood amylase, reduce inflammation and improve the status of blood microcirculation, with high safety.

Effect of Laparoscopic Left Hepatic Pedicle Occlusion in the Treatment of Intrahepatic Bile Duct Stones and the Influence on Liver Function and Stress Index after Operation

Objective
: To study the effect of laparoscopic left hepatic pedicle occlusion in the treatment of intrahepatic bile duct stones and its influence on liver function and stress index after operation.
Methods
: 102 intrahepatic bile duct stones who received therapy from February 2017 to February 2018 in our hospital were selected as research objects. According to random number table,those patients were divided into the observation group (n=52) and the control group (n=50). The control group was treated with laparotomy, while the observation group treated with Laparoscopic left hepatic pedicle occlusion. Then the Surgical indicators (operation time, intraoperative blood loss, time of hepatic portal exhaust, time of getting out of bed, time of hospitalization), liver function indicators of Aspartate aminotransferase (AST), alanine aminotransferase (ALT), total bilirubin (TBIL), stress indicators (adrenaline, cortisol), incidence of adverse reactions of two groups after treatment were compared.
Results
: After treatment, The operation time, intraoperative blood loss, portal exhaust time, ambulation time and hospitalization time in the observation group were significantly lower than those in the control group[(204.85±31.87)min vs(273.75±36.52)min,(182.47±40.32)ml vs(227.38±51.28)ml,(21.08±6.37)h vs(46.24±11.76)h,(1.94±0.42)d vs(3.02±0.61)d,(12.05±2.53)d vs(17.32±3.97)d](P<0.05). AST, ALT and TBIL were significantly lower than those in the control group[(87.47±20.86)U/L vs(101.38±23.97)U/L,(71.97±16.42)U/L vs(90.43±17.94)U/L,(46.83±12.31)μmol/L vs(60.25±14.92)μmol/L](P<0.05). The levels of adrenaline and cortisol in each node were significantly lower than those in the control group(P<0.05). The total incidence of adverse reactions was significantly lower than that in the control group [5.76%(3/52)vs28.00%(14/50)](P<0.05).
Conclusion
: Laparoscopic left hepatic pedicle occlusion is effective in the treatment of intrahepatic bile duct stones. It can recover quickly after operation. It can reduce the stress reaction to the body and promote the recovery of liver function. It has fewer complications and is safe and reliable.

Curative Efficacy of Omeprazole Combined with Aluminum Sulfatein in the treatment of Chronic Gastritis in Children and Effects on Serum Inflammatory Factors and Immune Function

Objective
: To study the curative efficacy of Omeprazole combined with aluminum sulfatein in the treatment of chronic gastritis in children and its effects on the serum inflammatory factors and immune function.
Methods
:165 cases of pediatric chronic gastritis admitted to our hospital from September 2016 to September 2018 were selected for this study, the patients were divided into observation group (n=85) and control group (n=80) according to the simple random grouping method. The control group was treated with sucralfate and the observation group was treated with omeprazole on the basis of the control group. Clinical efficacy, serum interleukin-8 (il-8), interleukin-6 (il-6), tumor necrosis factor alpha (tnf-alpha), immune function and adverse reactions were compared between the two groups.
Results
:After treatment, the total effective rates of the two groups were 95.29% and 83.75%, respectively, with significant difference (P<0.05). After treatment, inflammatory factors in both groups were significantly improved compared with those before treatment, and serum il-8, il-6 and tnf-alpha levels in the two groups were significantly different after treatment (P<0.05). After treatment, the levels of CD3+, CD4+ and CD8+ in the two groups were significantly improved compared with that before treatment, and the levels of CD3+, CD4+ and CD8+ in the two groups were significantly different after treatment (P<0.05). The total incidence of adverse reactions in the two groups was 9.41% and 13.75%, respectively, with no significant difference (P>0.05).
Conclusion
: The use of omeprazole and sucralate in children with chronic gastritis has significant curative effect and can effectively improve the level of inflammatory factors and immune function, which is worthy of promotion and application.

Significance of Prethrombotic State Molecules and Cystatin C Level in Patients with Coronary Heart Disease for Prognosis after Intervention Operation

Objective
: To observe the evaluation significance of prethrombotic state molecules and cystatin C (Cys-C) level in patients with coronary heart disease (CHD) for prognosis after intervention operation.
Methods
: 72 CHD patients who were admitted to the hospital were enrolled. All underwent percutaneous coronary intervention (PCI). The prethrombotic state molecules [plasma D-dimer (DD), thrombotic precursor protein (TpP), fibrinogen (Fbg), serum von Willebrand factor (vWF)] and Cys-C levels before and at 3d after treatment were compared. They were divided into good prognosis group and poor prognosis group according to follow-up results. The prethrombotic state molecules and Cys-C before and at 3d after treatment were compared between the two groups.
Results
: The levels of plasma D-D, TpP, Fbg, serum vWF and Cys-C at 3d after PCI were significantly higher than those before treatment (P<0.05). The proportion of patients with diabetes, hyperlipidemia, hypertension and smoking history in poor prognosis group was significantly higher than that in good prognosis group (P<0.05). DD, TpP, Fbg, vWF and Cys-C in both groups at 3d after treatment were significantly higher than those before treatment (P<0.05). At 3d after treatment, D-D, TpP, Fbg, vWF and Cys-C in poor prognosis group were significantly higher than those in good prognosis group (P<0.05). D-D, TpP, Fbg, vWF and Cys-C were independent risk factors for prognosis of patients with CHD after PCI (P<0.05).
Conclusion
: The levels of prethrombotic state molecules and Cys-C in patients with coronary heart disease have an increasing trend, and the higher the degree of increase, the worse the prognosis of patients, which can be used for prognosis evaluation.

Comparison of the Efficacy of Single-port and Traditional Multi-hole Thoracoscopic Pulmonary Nodulectomy in the treatment of Solitary Pulmonary Nodules in the Elderly

Objective
: To compare the efficacy of single-port and traditional multi-hole thoracoscopic pulmonary nodulectomy in the treatment of solitary pulmonary nodules (SPN) in the elderly.
Methods
: 100 elderly patients with SPN underwent pulmonary nodule resection in our hospital were selected. According to the simple random group method, they were divided into the control group with conventional multi-hole thoracoscopic surgery, the observation group with single-port thoracoscopic surgery, 50 cases in each group. The changes of surgical indicators, lung function indicators before and after surgery, postoperative pain score and complications were compared between the two groups.
Results
: Compared with the control group, intraoperative blood loss and postoperative drainage volume were significantly reduced in the observation group, and postoperative hospital stay and drainage time were significantly shortened (P<0.05). There was no significant difference in operation time between the two groups (P>0.05). Compared with the control group, postoperative forced vital capacity (FVC), maximal voluntary ventilation (MVV) and forced expiratory volume in one second (FEV1) in the observation group were significantly improved (P<0.05). Visual analogue scale (VAS) was significantly reduced in the observation group 24 and 72 hours after surgery compared with the control group (P<0.05). There was no statistically significant difference in the incidence of postoperative complications such as pulmonary infection and atelectasis between the two groups (P>0.05).
Conclusion
: Compared with conventional multi-hole thoracoscopy, single-port thoracoscopic pulmonary nodule resection can effectively improve the treatment effect of elderly patients with SPN, improve the pulmonary function of patients, contribute to promote postoperative rehabilitation and improve the quality of life, so it has good clinical application value.

Comparative Analysis of Efficacy of Cardiopulmonary Bypass Repair with Right Anterolateral Thoracotomy Incision and Cardiopulmonary Bypass Rpair with Transthoracic Median Incision in the treatment of Atrial Septal Defect

Objective
: To analyze and compare the clinical efficacy of cardiopulmonary bypass repair with right anterolateral thoracotomy incision and cardiopulmonary bypass repair with transthoracic median incision in the treatment of atrial septal defect.
Methods
: The clinical data of each 46 patients with atrial septal defect treated by cardiopulmonary bypass repair with right anterolateral thoracotomy incision (right small incision group) and cardiopulmonary bypass repair with transthoracic median incision (median incision group) were retrospectively analyzed. The intraoperative conditions, postoperative recovery, occurrence of postoperative complications and patient satisfaction were recorded in the two groups, and the body image status, and quality of life were compared between the two groups before operation and at 6 months after operation.
Results
: There were no significant differences in the intraoperative cardiopulmonary bypass time and aortic crossclamp time between the two groups (P>0.05). However, the intraoperative blood loss, incision length and mechanical ventilation time, postoperative intensive care time, chest drainage volume and postoperative hospital stay in right small incision group were lower than those in median incision group (P<0.05). There was no significant difference in the occurrence of postoperative complications between the two groups (P>0.05). The patient satisfaction in right small incision group was significantly better than that in median incision group (P<0.05). At 6 months after operation, the BISS scores in the two groups were higher than those before operation (P<0.05), but the score in median incision group was higher than that in right small incision group (P<0.05). The QLI scores in the two groups were also higher than those before operation (P<0.05), but the score in right small incision group was higher than that in median incision group (P<0.05).
Conclusion
: Cardiopulmonary bypass repair with transthoracic median incision and cardiopulmonary bypass repair with right anterolateral thoracotomy incision both have good efficacy and safety in the treatment of atrial septal defect. However, right anterolateral thoracotomy incision procedure is more advantageous in reducing the surgical trauma, promoting postoperative rehabilitation and improving postoperative quality of life and body image, and it can improve patient satisfaction.

Comparison of Three Different Imaging Methods in the Diagnosis of Non-calcified Dense Breast Mass

Objective
: To compare the value of DBT, DM and ultrasound in the diagnosis of dense non-calcified breast mass.
Methods
: From September 2016 to September 2018, 235 patients with dense non calcified breast masses admitted to the Second Affiliated Hospital of Liaoning University of Traditional Chinese Medicine and Liaoning Cancer Hospital and confirmed by surgery and pathology were selected as the study objects, all with DBT, DM and ultrasound imaging data. Three imaging methods were compared with pathological results to observe the detection rate and coincidence rate of benign and malignant imaging, and to analyze the diagnostic efficacy of three imaging methods.
Results
: Compared with the pathological results, the kappa value of DBT was 0.710, the consistency was general; the kappa value of DM was 0.505, the consistency was poor; the kappa value of ultrasound was 0.992, the consistency was good. There were significant differences in the detection rate of benign and malignant (χ 2 = 55.05, 48.876, 8.36, P<0.05) between the three imaging methods (P<0.05). The detection rate of benign, coincidence and malignant was significantly higher than that of DM and DBT (P<0.05), while that of DBT was significantly higher than that of DM (P<0.05). On diagnostic performance, the positive predictive value, negative predictive value of the three kinds of diagnostic methods, diagnostic coincidence rate significant difference (χ

Conclusion

Objective
: To study the effects of rosuvastatin or atorvastatin combined with clopidogrel on serum levels of MIF,CKLF1 and FKN and neurological function in patients with ischemic stroke.
Methods
: 186 cases of patients with ischemic stroke admitted to our hospital from June 2015 to January 2018 were selected for the study and randomly divided into observation group and control group by random table method, with 93 cases in each group. The two groups were given clopidogrel, and control group was combined with atorvastatin, and observation group was combined with rosuvastatin. The treatment effects were compared between the two groups, and the adverse reactions were recorded.
Results
: Compared with control group, the improving degrees of levels of TG,TC,LDL-C and HDL-C were greater after treatment in observation group (P<0.05). The levels of serum MIF, CKLF1 and FKN in observation group after treatment were significantly lower than those in control group (P<0.05). After treatment, the improvements of NIHSS score and Barhel index score in observation group after treatment were better than those in control group (P<0.05). There was no significant difference in the occurrence of adverse reactions between the two groups after treatment (P>0.05).
Conclusions
: Rosuvastatin combined with clopidogrel for patients with ischemic stroke can effectively improve the levels of serum MIF,CKLF and FKN and neurological function. Therefore, it is worthy of promotion.

Study on Effects of Coronary Tortuosity, Diabetes Mellitus and Coronary Atherosclerosis on the Occurrence of Cmvd

Objective
: To study the effects of coronary artery tortuosity, coronary atherosclerosis (CA) and diabetes on coronary microvascular disease (CMVD).
Methods
: A retrospective study was conducted to collect the clinical data of 257 patients with typical angina pectoris symptoms from June 2014 to June 2019 in our hospital, and the clinical data of patients without obvious coronary artery stenosis indicated by coronary angiography were collected. According to the results of resting load myocardial perfusion imaging, 105 patients in group A (without CMVD) and 152 patients in group B (with CMVD). The differences in the basic clinical data, coronary angiography results and echocardiography parameters between the two groups were compared, and the influencing factors of CMVD were analyzed by univariate and multivariate Logistic regression.
Results
: Compared with group A, the proportion of diabetes history was significantly increased in group B, and the concentrations of homocysteine (Hcy), low-density lipoprotein cholesterol (LDL-C) and hypersensitive C-reactive protein (hs-CRP) were all significantly increased (P<0.01). Compared with group A, the detection rate of CA and coronary artery tortuosity in group B was significantly increased, and the corrected TIMI blood flow frame number was significantly increased (P<0.05). Multivariate Logistic regression analysis showed that CA, diabetes and coronary artery tortuosity were all independent risk factors for patients with CMVD (P<0.01).
Conclusion
: CA, diabetes and coronary artery tortuosity are all independent risk factors for CMVD in patients with angina pectoris but without obvious coronary artery stenosis, suggesting that CA, diabetes and coronary artery tortuosity are closely related to the occurrence of CMVD.

Clinical Manifestations and Laboratory Examination of Infectious Mononucleosis in Children with Epstein Barr Virus Infection and Cytomegalovirus Infection

Objective
: To compare the clinical manifestations and laboratory examinations of children with infectious mononucleosis (IM) infected by EB virus (EBV) and cytomegalovirus (CMV).
Methods
: The clinical data of 35 children with IM caused by EBV infection (EBV group) and 40 children with CMV infection (CMV group) in our hospital from September 2016 to March 2019 were retrospectively analyzed. The main clinical manifestations and laboratory examination indexes of the two groups were compared.
Results
: There was no significant difference in the incidence of fever, lymph node enlargement, splenomegaly, liver enlargement, eyelid edema and skin rash between the two groups (P> 0.05), but the incidence of cheek-pharyngitis in EBV group was significantly higher than that in CMV group (P<0.05). There was no significant difference in the proportion of lymph node enlargement, cough, fever, pharyngeal cheek inflammation, eyelid edema and wheeze as the first symptoms between the two groups (P> 0.05), but the proportion of nasal obstruction/runny nose as the first symptoms in EBV group was significantly higher than that in CMV group (P<0.05). There was no significant difference in RBC, WBC, lymphocyte and neutrophil count between the two groups (P> 0.05), but platelet count (PLT), alanine aminotransferase (ALT) and atypical lymphocyte count (A-lym) in EBV group were significantly higher than those in CMV group (P<0.05).
Conclusion
: Fever is the first symptom of IM children infected with EBV and CMV, but compared with CMV, the incidence of nasal obstruction/runny nose is higher in children infected with EBV. The increase of platelet, alanine aminotransferase and atypic lymphocyte is increased, and the damage of liver function is more serious.

Comparison of two Methods of Treatment of Appendiceal Root and Mesentery in Laparoscopic Appendectomy

Objectives
:To study the comparison of two treatments of appendix root and mesentery in laparoscopic appendectomy.
Methods
: From July 2017 to May 2019, 60 patients with appendicitis who underwent appendectomy in our hospital were selected as the research objects for retrospective analysis. According to the different treatment methods of patients, they were divided into observation group (31 cases) and control group (29 cases). The observation group was treated with heme-o-lok clamp to remove the root and mesentery. The control group was treated with thread ligation to remove the root and mesentery. The operation time, the amount of bleeding during operation, the time of getting out of bed, the time of exhaust and the cost of hospitalization were observed, and the postoperative complications of the two groups were observed and recorded.
Results
: The amount of bleeding in the observation group was significantly lower than that in the control group, and the hospitalization cost was significantly higher than that in the control group (P<0.05). There was no significant difference in the time of getting out of bed, exhaust time and hospitalization time after operation (P> 0.05), but the detection rate of abdominal distension in the observation group was significantly lower than that in the control group (P<0.05). There was no significant difference in the incidence of diarrhea, vomiting, pain, intestinal obstruction and infection between the two groups (P> 0.05).
Conclusion
: The method of the hem-o-lok clamp closure and the ligation of the silk thread under the laparoscope is safe and reliable, but the operation time of the hem-o-lok clamp is shorter and the amount of blood loss during the operation is less, but the hospitalization expense of the patients with the ligation of the silk is lower and the cost is more economical.

Effect of Alprostadil Combined with Aspirin on Vascular Endothelial Function in Coronary Slow Blood flow in Elderly Patients with Angina Pectoris

Objective
: To study the effect of alprostadil combined with aspirin on vascular endothelial function in coronary slow blood flow in elderly patients with angina pectoris.
Methods
: 115 patients with coronary slow flow angina admitted to our hospital from January 2015 to October 2018 were randomly divided into observation group (60 cases) and control group (55 cases). On the basis of routine treatment, the control group was given aspirin treatment, and observation group was treated with Alprostadil for 3 consecutive months. The total effective rate of the observation group was significantly higher than that of the control group (P<0.05). No, ET-1, FMD, the number and duration of angina pectoris attack per day, the number of corrected tmit blood flow frames were statistically significant (P>0.05).
Results
: After treatment, the total effective rate in the observation group was significantly higher than that of the control group (P<0.05). There were significant differences in serum nitric oxide (NO), endothelin-1 (ET-1), Endothelial Dependent Vasodilation Function (FMD), the number and duration of angina pectoris daily attacks and Corrected TMIT Blood Flow Frames between the two groups at interblock, different time points (P<0.05), and they all have interaction (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).
Conclusion
: Alprostadil combined with aspirin has a significant effect in the treatment of elderly patients with coronary slow flow angina, which can effectively improve the endothelial function, promote the recovery of coronary blood flow, relieve clinical symptoms, and has a high clinical application value.

Rlationship between Platelet Volume Distribution Width, Severity and Prognosis in Patients with Stable Coronary Heart Disease Treated by PCI

Objective
: To explore the relationship between platelet volume distribution width, severity and prognosis in patients with stable coronary heart disease (SCAD) treated by PCI.
Methods
: From April 2015 to September 2017, 160 patients with stable coronary heart disease treated by PCI in our center were divided into two groups: group A (platelet volume distribution width ≤ 14.2%) and group B (platelet volume distribution width above 14.2%), 80 in each group. The results of angiography and biochemical indexes were collected, and the incidence of adverse events was compared.
Results
: Compared with patients in group A, the severity of coronary artery stenosis, lesion score and lesion number were significantly lower than those in group B (t=12.819, 6.707, 2.508, P<0.05). In addition, TG, LDL, TC and Cr in group A were significantly lower than those in group B (t=2.305, 4.476, 3.286, 2.741, P<0.05), while HDL was significantly higher than those in group B (t=2.280, P<0.05). Conclusion
: The higher the platelet volume distribution width of SCAD patients, the more serious the disease and the more adverse prognosis. Comparison of the Curative Effect of Minimally Invasive Percutaneous Nephroscopy combined with Ureteroscopy in the treatment of Complicated Upper Ureteral Calculi

Objective
: To compare the curative effect of minimally invasive percutaneous nephroscope combined with ureteroscope for the treatment of complicated upper ureteral calculi.
Methods
: 70 patients with complicated upper ureteral calculi in our hospital from January 2016 to December 2018 were selected and randomly divided into the control group for minimally invasive percutaneous nephrolithotomy and the research group for combined ureteral soft and hard laparoscopy, with 35 cases in each group. The perioperative indicators, stone clearance rate, serum inflammatory factor level and postoperative complications were compared between the two groups.
Results
: The amount of blood loss in the study group was significantly reduced compared with the control group, the operation time was significantly prolonged compared with the control group, and the length of hospital stay was significantly shortened compared with the control group (P<0.01). The stone clearance rate (94.29%) in the study group was significantly higher than that in the control group (71.43%) (P<0.05). However, there was no significant difference in the stone clearance rate (100.00% vs. 91.43%) between the two groups at 12 weeks after surgery (P>0.05). Serum levels of C-reactive protein (CRP) and interleukin-6 (IL-6) were significantly higher in the two groups one week after surgery than those before surgery, while serum levels of CRP and IL-6 in the study group were significantly lower than those in the control group (P<0.01). There was no significant difference in the incidence of postoperative fever, pyelonephritis and low back pain between the two groups (P>0.05).
Conclusion
: Compared with minimally invasive percutaneous nephroscopy, combined ureteroscopy with soft and hard ureteroscopy can effectively improve the treatment effect of patients with complicated upper ureteral calculi, with significant clinical efficacy, and can alleviate the inflammatory reaction of the body, with good efficacy and safety, so it is worthy of clinical application.

Objective
: To investigate the expression of long non-coding RNA-CECR7 (lncRNA-CECR7) in esophageal cancer cells and its effect on the expression of p2l-activated kinase 2 (PAK2), Cyclin D1 (Cyclin D1) and cyclin-dependent kinase 4 (CDK4) genes.
Methods
: The expression of lncrna-cecr7 in human esophageal cancer cells Eca109, EC9706, kyse30, TE-13 and het-1a were detected by real-time fluorescence quantitative PCR. The esophageal cancer cells with the highest expression were transfected, and the control group (siRNA of negative control) and the intervention group (siRNA of lncrna-cecr7) were set up respectively. The expression of lncrna-cecr7 was detected by qRT-PCR, the apoptosis rate was detected by flow cytometry, and the expression of PAK2, cyclin D1 and CDK4 were detected by qRT-PCR.
Results
: Compared with het-1a, the expression of lncrna-cecr7 in esophageal cancer cells was significantly up-regulated (P<0.01), and the expression of lncrna-cecr7 was the highest in TE-13 cells. The expression of lncrna-cecr7 in TE-13 cells in the intervention group was significantly lower than that in the control group (P<0.01). After silencing lncrna-cecr7, the apoptosis rate of TE-13 cells in the intervention group was significantly higher than that in the control group (P<0.01). The expression of PAK2, cyclin D1 and CDK4 mRNA in the intervention group was significantly lower than that in the control group (P<0.01).
Conclusion
: lncRNA-CECR7 is highly expressed in esophageal cancer cells, which can induce apoptosis by silencing lncRNA-CECR7, and its mechanism may be closely related to the down-regulation of PAK2, Cyclin D1 and CDK4 gene expressions.

Diagnostic Value of OCT in Central Serous Chorioretinopathy

Objective
: To study the clinical value of optical coherence tomography (OCT) in the diagnosis of central serous chorioretinopathy (CSC).
Methods
: 98 patients suspected as CSC admitted to our hospital from September 2015 to March 2018 were retrospectively analyzed. Fluorescein fundus angiography (FFA) and OCT were performed at the same time. The results of FFA examination were taken as references by CSC, and the diagnostic efficacy and consistency analysis of the two examination methods were compared, and the FFA leakage point distribution and OCT images at leakage point were observed when patients were diagnosed as CSC.
Results
: Among the 98 patients, 85 cases (86.73%) were diagnosed as CSC by FFA, and 82 cases (83.67%) were diagnosed as CSC by OCT, and there was no significant difference between the two methods (P>0.05), and the sensitivity, specificity and accuracy rate of the two diagnostic methods were 96.47%, 76.92% and 93.88%, and the diagnostic consistency was good (Kappa>0.7). The number of FFA fluorescence leakage points was 104 among 85 patients, mainly 87 cases (88.78%) in one leakage point, and the leakage point morphology was mostly ink-spot type in 61 cases (70.11%). Among 82 cases of affected eyes, there were abnormalities of FFA fluorescence leakage point in OCT images, including 21 eyes (25.61%) of PED, 2 eyes (9.52%) of defects in RPE, 28 eyes (34.15%) of small polypoid in RPE and33 eyes (40.24%) of rough RPE. OCT was used in 82cases of affected eyes without FFA leakage points, and there were abnormal changes in RPE layer in 7 cases of images, including 2 eyes (28.57%) of PED, 1 eye (14.29%) of RPE defect, 2 eyes (28.57%) of small polypoid in RPE and 3 eyes (42.86%) of rough RPE.
Conclusions
: There were fluorescent leakage points in all patients diagnosed by CSC using FFA, and abnormal changes of RPE were found in both fluorescent leakage points and non-fluorescent leakage points of patients diagnosed by CSC using OCT, which were in good agreement with the results of FFA.

Objective
: Specific IgE (sIgE) in serum of patients with Chronic urticaria (CU) were detected to provide basis for the prevention of the disease.
Methods
: The serum sIgE of 300 cases with spontaneous chronic urticaria were detected by Medwiss allergen detection system imported from Germany.
Results
: Among the 300 patients, 193 cases were positive for sIgE ,with a positive rate of 64.3%. The top three inhaled allergens were dust mite, grass combination, mold combination. The top three food allergens were cashew nuts, amaranth, milk.Compared with the male group and the female group, there was no difference in the total sIgE detection rate and the sIgE detection rate of inhalation and food(P>0.05). There was no difference between the total sIgE detection rate and the inhalation sIgE detection rate between the children group and adults group(P>0.05),while the food sIgE detection rate of children group was higher than that of the adult group(P<0.05) .
Conclusion
: The occurrence of spontaneous CU is related to serum sIgE, allergen detection is helpful for the treatment and prevention of patients with spontaneous chronic urticaria.

Effect of Folic Acid-assisted Butylphthalide on Patients with Mild Cognitive Impairment

Objective
: To analyze the effect of folic acid-assisted butylphthalide on patients with mild cognitive impairment.
Methods
: Eighty patients with mild cognitive dysfunction admitted to our hospital from June 2017 to January 2019 were enrolled in the observation group and the control group, 40 cases each. The control group was treated with butylphthalide, and the observation group was combined with folic acid treatment to compare the effects of the two groups on cognitive performance, mental state and adverse hair growth.
Results
: There was no significant difference in the MoCA scores between the two groups before treatment (P>0.05). The MoCA scores of the two groups increased in the first month and the second month after treatment, and the MoCA scores in the observation group were higher than those in the control group (P<0.05). There was no significant difference in the MMSE scores between the two groups (P>0.05). The MMSE scores were improved in both groups after 1 month and 2 months after treatment, and the scores of the observation group were higher than those in the control group (P<0.05). There was no significant difference between the amplitude and latency (P>0.05). After treatment, the amplitude of both groups increased and the latency decreased (P<0.05). The amplitude of the observation group was higher than that of the control group and the latency was lower than that of the control group (P< 0.05); The incidence of adverse reactions in the observation group was lower than that in the control group (P<0.05).
Conclusion
: Folic acid-assisted butylphthalide treatment in MCI can effectively improve the mental state and cognitive function of patients, reduce the incidence of adverse reactions, and has high safety and is worthy of application.

Objective
: To investigate the effect of cell activation factor expression on disease progression and secondary venous thromboembolism in patients with malignant lymphoma.
Methods
: Between January 2016 and December 2018, 30 cases of malignant lymphoma patients as research group in our hospital, in the same period health check-up 30 cases as control group, the comparative analysis research group and the control group before and after the treatment serum soluble thrombus regulatory proteins (sTM), the surface of the whole blood neutrophils CD11b factor and platelet CD62p factor on the surface level.
Results
: The levels of sTM, CD11b and CD62p in the study group before treatment were significantly higher than those in the study group and control group (P<0.05), and the levels of each factor were significantly lower after treatment than before treatment, with statistically significant differences in the group (P<0.05), but no statistically significant differences in the levels of each factor after treatment compared with the control group (P>0.05).
Conclusion
: Endothelial cell injury, abnormal activation of neutrophils and platelet surface factors are likely to lead to thromboembolism, disease progression and secondary venous thromboembolism in patients with malignant lymphoma.

Clinical Value of Detection of High-sensitivity C-reactive Protein and Cystatin C in Patients with Ventricular Remodeling after Acute Myocardial Infarction

Objective
: To investigate the correlation between the levels of serum hypersensitive C-reactive protein (Hs-CRP) and cystatin C (CysC) and the degree of ventricular remodeling after acute myocardial infarction (AMI).
Methods
: 160 patients with AMI were selected as the observation group, and 80 patients with physical examination at the same time were selected as the control group. The serum levels of Hs-CRP and CysC were measured within 24 hours after the onset of the disease in the observation group and in the control group. According to the concentration levels of Hs-CRP and CysC, the observers were divided into four groups: group A had elevated serum Hs-CRP and CysC levels, group B had elevated serum Hs-CRP levels and CysC levels were normal, group C had elevated normal CysC levels of Hs-CRP, and group D had normal Hs-CRP and CysC levels. The follow-up period was 6 months. The left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), left ventricular ejection fraction (LVEF) were measured by echocardiography within 96 hours after onset and at the end of follow-up (6 months) as well as in the control group. The left ventricular remodeling was defined as the growth rate of left ventricular end-diastolic volume (LVEDV%>20%) at the end of follow-up (6 months). The indexes of echocardiography and the incidence of ventricular remodeling were compared among the five groups. The correlation between the degree of ventricular remodeling and the levels of hs CRP and CysC in serum was analyzed.
Results
: The serum levels of Hs-CRP and CysC in the observation group were significantly higher than those in the control group, LVEDV and LVESV were higher and LVEF was lower than those in the control group (all P<0.05). Compared with group B, C and D, the levels of LVEDV and LVESV were higher, the incidence of ventricular remodeling was lower and the incidence of ventricular remodeling was higher in group A (all P<0.05). Compared with group D, the levels of LVEDV and LVESV in group B and C were higher, the LVEF was lower and the ventricular remodeling was lower. Incidence rate increased (P<0.05). Multivariate logistic regression analysis showed that elevated serum levels of Hs-CRP and CysC were independent predictors of ventricular remodeling after AMI (P<0.05).
Conclusion
: Serum levels of Hs-CRP and CysC are closely related to ventricular remodeling after AMI, and they are reliable indicators for predicting the degree of ventricular remodeling after AMI.

Objective
: To explore the relationship KIF2A LDS and the occurrence, development and biological behavior of RCCC by detecting the protein expression of KIF2A and LSD1 in renal clear cell carcinoma (RCCC) and adjacent non tumor tissues,.
Methods
: The expression of KIF2A and LSD1 in 120 cases of renal clear cell carcinoma and 40 cases of adjacent non-tumor tissues to renal clear cell carcinoma were detected by immunohistochemistry.
Results
: The expression of KIF2A in renal clear cell carcinoma (60.83%) was significantly higher than that in non-tumor tissues (32.5%). The difference was statistically significant (P<0.05). The expression of LSD1 in renal clear cell carcinoma (55.0%) was significantly higher than that in non-tumor tissues (30.0%). The difference was statistically significant (P<0.05). The expression of KIF2A in RCCC was significantly higher than that in who / ISUP pathological grade and clinical significance The expression of LSD1 protein in RCCC was correlated with WHO / ISUP pathological grade (P<0.05), but not with clinical stage (P > 0.05). The expression of KIF2A and LSD1 protein was not correlated with gender and age (P > 0.05). The results of correlation test showed that the expression of KIF2A and LSD1 was positively correlated (rs = 0.235, P = 0.010).
Conclusion
: KIF2A and LSD1 may be involved in the occurrence and development of RCCC and have synergistic effect.

Comparison of Anterior and Posterior Approaches and Simple Posterior Approaches in the treatment of Thoracolumbar Spinal Fractures in Adults

Objective
: To study the effect of combined anterior and posterior approaches and simple posterior approaches in the treatment of adult thoracolumbar vertebral fractures.
Methods
: 60 adult patients with thoracolumbar vertebral fracture admitted to our hospital from May 2016 to August 2018 were selected as the research objects. According to the different surgical treatment methods, they were divided into group A (32 cases), group B (28 cases), group B (28 cases), group B (simple posterior treatment), group A (anterior combined posterior treatment) on the basis of group B. To observe the clinical efficacy of the two groups after treatment, the motor function and tactile function score, vertebral height and Cobb angle of the two groups before and after treatment were observed and recorded.
Results
: After treatment, there were significant differences in marked, effective and ineffective effects between the two groups (P<0.05), and the total effective rate of group A was significantly higher than that of group B (P<0.05). After treatment, the scores of motor function and tactile function in two groups were significantly higher than those before treatment, but after treatment, the scores in group A were significantly higher than those in group B, with statistical significance (P<0.05); after treatment, the height of injured vertebrae in group A and group B was significantly higher than that before treatment, and after treatment, the scores in group A were significantly higher than that in group B, with statistical significance (P<0.05); after treatment, the Cobb angle in two groups was significantly decreased, with treatment. There was significant difference before treatment (P<0.05), but group A was significantly lower than group B (P<0.05).The operative time and intraoperative blood loss in group A were significantly higher than those in group B (P<0.05), but there was no significant difference in hospitalization time between the two groups (P>0.05).
Conclusion
: The combined treatment of thoracolumbar spine fracture by anterior and posterior approach has significant effect.

Efficacy and Safety Analysis of Endoscopic Papillary Myotomy combined with Balloon Dilation in the treatment of Choledocholithiasis

Objective
: To explore the endoscopic papillary muscle of small incision (SEST) with balloon dilation (EPBD) therapy bravery to summarize the clinical efficacy and safety of the stone.
Methods
: From June 2015 to July 2018, 34 patients with choledocholithiasis treated in our hospital by SEST combined with EPBD were selected as SEST combined with EPBD group 46 patients treated by EST wereselected as EST group. The stone removal rate, stone size, pancreatitis, hyperamylasemia and cholangitis were compared between the two groups. The effect of SEST combined with EPBD was evaluated in the treatment of choledocholithiasis. The stone removal rate, stone size, pancreatitis, hyperamylasemia and cholangitis were compared between the two groups.
Results
: The results showed that the stone removal rate of EST group (97.83%) was higher than that of EST combined with EPBD group (97.06%) (P> 0.05). The stone diameter was significantly smaller than that of the SEST combined with EPBD group (P<0.05). The results showed that the level of serum amylase in EST group was higher than that in EST combined with EPBD at different time points, and it was the most obvious increase at 3 hours after operation, and it was decreased after 24 hours, the difference was statistically significant (P<0.05). In addition, gastrointestinal bleeding, PEP, hyperamylasemia and cholangitis occurred in both groups, and the total incidence of complications in EST group was higher than that in SEST combined with EPBD group (41.3%, 17.65%, χ 2 = 5.093, P<0.05).
Conclusion
: Compared with the simple EST treatment, the success rate of single stone removal is almost the same, but the stone removal diameter of SEST combined with EPBD is significantly larger than that of simple EST, and the postoperative complications are less. Therefore, SEST combined with EPBD is more effective and safe in the treatment of choledocholithiasis.

Effect of Rosuvastatin Combined with Tegrarol in the treatment of Acute Coronary Syndrome and its Effect on YKL-40 and GMP-140

Objective
: To study curative efficacy of risuvastatin in combination with tigreglio in treatment of Acute coronary syndromeand its effectson Plasma chitinase protein 40 (ykl-40), plasma alpha - granular membrane protein (gmp-140).
Methods
: 100 patients with acute coronary syndrome admitted to our hospital from April 2016 to April 2018 were selected for this study, the patients were divided into observation group (n=52) and control group (n=48) by simple random grouping. The control group was treated with tigrelol, and the observation group was treated with risuvastatin on the basis of the control group. The clinical efficacy, serum ykl-40, gmp-140, BNP, LVEF and adverse reactions were compared between the two groups.
Results
: After treatment, the total effective rates of the two groups were 90.38% and 72.92%, respectively (P<0.05). After treatment, the levels of ykl-40 and gmp-140 in both groups were significantly improved, and the observation group was significantly lower than the control group (P<0.05). Before treatment, there was no significant difference in BNP and LVEF levels between the two groups. After treatment, BNP and LVEF in both groups were significantly improved compared with that before treatment, and BNP level in the observation group was significantly lower than that in the control group, while LVEF was significantly higher than that in the control group (P<0.05). The incidence of adverse reactions in the two groups was 7.69% and 29.17% respectively (P<0.05).
Conclusion
: The use of rosuvastatin combined with tigrelol in acute coronary syndrome has significant curative effect and can effectively improve the levels of ykl-40 and gmp-140, which is worthy of promotion and application.

Objective
: To analyze the incidence of osteoporosis around hip prosthesis in the elderly and the effect of zoledronic acid.
Methods
: From September 2017 to September 2018, 60 elderly patients with hip replacement who were hospitalized in our hospital were divided into three groups according to their ages, i.e. group A, group B and group C, to evaluate whether there is any difference in the clinical efficacy of patients at different ages, including 10 patients in group A, aged 60~69; 28 patients in group B, aged 70~79; 22 patients in group C, aged 60~69 The age is 80~89 years old. All the elderly patients with osteoporosis were treated with zoledronic acid once a week after the operation, once a year after the operation, and twice a day with 600 mg of calcic D. One year follow-up was conducted to compare the BMD of lumbar spine and contralateral hip joint before and after operation.
Results
: After taking zoledronic acid sodium for 1 year, the BMD of the three groups was significantly better than that of the group before operation (P<0.05). The bone mineral density of the contralateral femoral neck of the three groups was significantly higher than that of the group before operation (P<0.05). The bone mineral density of the contralateral total hip joint of the three groups was significantly higher than that of the group before operation (P<0.05).
Conclusion
: The use of sodium zolaiphosphate in elderly patients after hip arthroplasty can effectively prevent the occurrence of osteoporosis around the prosthesis, which is worth popularizing in clinic.

Efficacy and Adverse Reactions of compound Flumetasone combined with Calcipotriol Ointment and UVA1 Phototherapy in the treatment of Nail Psoriasis

Objective
: To analyze the efficacy and adverse reactions of compound flumetasone combined with calcipotriol ointment and UVA1 phototherapy in the treatment of nail psoriasis.
Methods
: 88 patients with nail psoriasis were collected and randomly divided into two groups,with 44 cases in each group. Single group was given calcipotriol ointment and UVA1 phototherapy, and combined group was given compound flumetasone combined with calcipotriol ointment and UVA1 phototherapy, and they were treated for 3 months. The nail severity index, related cytokines, transcription factor mRNA expression, quality of life, and occurrence of adverse reactions were compared between the two groups.
Results
:There were significant differences in group effects, time effects and interaction effects of NAPSI (P<0.05), and the NAPSI scores in combined group at 1 month and 3 months of treatment were significantly lower than those in single group (P<0.05), and the NAPSI scores in the two groups at 1 month and 3 months of treatment were significantly decreased successively (P<0.05). After treatment, the changes of IL-17, IL-22, IL-10 and INF-γ in combined group were larger than those in single group (P<0.05). After treatment, the decreases of mRNA expressions of RORγt and STAT3 in combined group were greater than those in single group (P<0.05). After treatment, the decrease of DLQI score in combined group was greater than that in single group (P<0.05). There was no significant difference in the incidence rate of adverse reactions between the two groups during treatment (P>0.05).
Conclusion
:Compound flumetasone combined with calcipotriol ointment and UVA1 phototherapy for nail psoriasis can effectively improve clinical symptoms, regulate serum-related cytokines levels, down-regulate mRNA expressions of transcription factors, and improve quality of life of patients.

Analysis of the Value of Combined TCT and Colposcopy in the Diagnosis of Cervical Carcinoma

Objective
: To analyze the diagnostic value of colposcopy combined with thinprep cytological test (TCT) in screening precancerous lesions of cervical cancer.
Methods
: A total of 735 women who underwent colposcopy and TCT examination in our hospital were selected. To evaluate the coincidence of colposcopy and TCT results with pathological results, and to evaluate the diagnostic efficacy of colposcopy and TCT alone or in combination in screening cervical precancerous lesions.
Results
: A total of 232 cases were positive in colposcopy and TCT, among which 163 cases were positive in both methods. Histopathological examination was performed on 232 patients, and the results showed 45 cases were negative, including 39 cases with normal cervix and 6 cases with cervicitis. Positive for 187 cases, including the cervical intraepithelial neoplasia (CIN) Ⅰ 70 cases, CIN Ⅱ 64 cases, CIN Ⅲ 48 cases, ductal carcinoma in 5 cases. The results of colposcopy were negative in 56 cases and positive in 176 cases. The consistency coefficient between colposcopy and pathological diagnosis was Kappa=0.565, P<0.05. The TCT findings of 232 cases were within the normal range, including 53 cases of atypical squamous cells of undetermined significance (ASCUS) and above, and 179 cases including ASCUS 73, 54 cases of low-grade squamous intraepithelial lesions (LSIL), 44 cases of high-degree squamous intraepithelial lesions (HSIL), and 8 cases of squamous cell carcinoma (SCC). Using pathological examination results as the gold standard, the consistency coefficient of TCT and pathological diagnosis results Kappa=0.773, P<0.05. The sensitivity, specificity and Yoden index of colposcopy and TCT in the diagnosis of cervical precancerous lesions were 78.61%, 91.44% and 90.91%, 57.78%, 82.22% and 88.89% respectively, and 0.36, 0.74 and 0.80, respectively.
Conclusions
: Colposcopy and TCT can be used to screen and diagnose the precancerous lesions of cervical cancer.

: To explore the value of SiLiankang in the treatment of myocardial damage in children with rotavirus diarrhea, and to summarize the clinical experience.
Methods
: 74 children with rotavirus diarrhea diagnosed and treated in our department from September 2018 to March 2019 were divided into control group (37 cases) and observation group (37 cases). The control group was treated with routine treatment. The observation group was treated with SiLiankang on the basis of routine diagnosis and treatment. The duration of persistent diarrhea, the level of GAS, MOT, VIP in serum and the therapeutic effect of the two groups were recorded and analyzed before and after treatment.
Results
: After 7 days of treatment, the duration of persistent diarrhea in the observation group (3.69 ±0.98d) was lower than that in the control group (4.40 ±1.04 d) (P<0.05). There was no significant difference in serum hormone indexes among the three groups before treatment. The level of GAS (725.45 ±123.74 μ g), MOT (266.41 ±58.47 μ g), VIP (144.28 ±40.19 μ g) in the observation group was lower than that in the control group before treatment, and there was no statistics between the observation group and the control group (P<0.05). The duration of persistent diarrhea in the two groups decreased after 4 days of treatment and 7 days after treatment, and the duration of persistent diarrhea in the observation group (3.69 ±0.98d) was lower than that in the control group (4.40 ±1.04d), and the duration of persistent diarrhea in the observation group (3.69 ±0.98d) was lower than that in the control group (4.40 ±1.04d), and the duration of persistent diarrhea in the observation group (3.69 ±0.98d) was lower than that in the control group. After treatment, the total effective treatment rate in the observation group (94.59%) was significantly higher than that in the control group (75.68%) (P<0.05).
Conclusion
: Siliankang plays an important role in the treatment of rotavirus diarrhea. It can obviously reduce the pathogenic hormones and inflammatory factors in children's body, and speed up the routine treatment. It is worth popularizing in related clinical diseases.

Analysis of the Effect of Bronchoscopic Cryotherapy and High Frequency Electrocautery on Airway Stenosis Caused by Central Lung Cancer

Objective
: To investigate the efficacy of transbronchoscopic cryotherapy and high-frequency electrocautery in treatment of airway stenosis caused by central lung cancer.
Methods
: 90 patients with airway stenosis caused by central lung cancer were divided into the high-frequency electrocautery group (30 cases treated with transbronchoscopic high-frequency electrocautery). The cryotherapy group (30 cases treated with transbronchoscopic cryotherapy) and the combined group (30 cases treated with transbronchoscopic cryotherapy and high-frequency electrocautery). The clinical effect, shortness of breath score, pulmonary function (FVC, FEV1, MVV) of the three groups were observed.
Results
: The total treatment effective rate of the combined group was significantly higher than that of high-frequency electrocautery group or the cryotherapy group (P<0.05). After treatment, shortness of breath scores were decreased significantly while FVC, FEV1 and MVV were increased significantly, and changes were the most obvious in the combined group, followed by high-frequency electrocautery group and changes were the smallest in cryotherapy group (P<0.05).
Conclusion
: The short-term effects of transbronchoscopic cryotherapy and high-frequency electrocautery are similar in treatment of airway stenosis caused by central lung cancer. The combined use can effectively improve the clinical efficacy, significantly relieve shortness of breath and improve lung function.

Screening Maternal Serum Protein Markers of Down's Syndrome Based on Mass Spectrometry

Objective
: To detect specific maternal serum protein markers for Down's syndrome early.
Methods
: Ten maternal sera (14~20 weeks) and 10 normal maternal sera at the same time point were collected. The differential expression proteins were analyzed by mass spectrometry and further verified by Western blot.
Results
: By mass spectrometry analysis, 13 proteins with abnormal expression of more than 1.0 times were found in maternal serum of Down syndrome fetuses. DAVID functional analysis showed that 13 differentially expressed proteins were mainly involved in lipoprotein biosynthesis and decomposition, apoptosis and platelet degranulation. The abnormal expression of FN1 and IGFBP3 in maternal serum of Down's syndrome fetus was 2.5 times higher than that in normal control group (P<0.05).
Conclusion
: Mass spectrometry is an effective method for screening maternal serum protein markers of Down syndrome, and FN1 and IGFBP3 may be potential new markers for screening Down syndrome fetuses.

Features of Montreal Classification in 134 Cases with Crohn's Disease and Related Risk Factors of Poor Prognosis

Objective
: To investigate the features of Montreal classification in 134 cases with Crohn's disease (CD) and related risk factors of poor prognosis.
Methods
: The clinical data of CD patients who were admitted to the hospital from July 2015 to February 2018 were retrospectively analyzed. Montreal classifying was performed on them. The clinical features of different classification patients were compared. The risk factors of prognosis were analyzed.
Results
: Among the 134 patients, there were 82 (61.19%) male, and 52 (38.81%) female, with male to female ratio of 1.58:1. The confirmed age was mainly on A2, accounting for 76.12%. The lesion location was mainly on L3, accounting for 55.22%. The disease behavior was mainly on B1 type, accounting for 67.91%. There were significant differences in disease behavior distribution among patients with different lesions location subtypes (P<0.05). The proportion of B1 type in L4 patients was significantly lower than that in other types, while proportion of B2 type was higher than that in other types. The proportion of B1 type in L3 patients was higher than that in other types, while proportion of B2 type was lower than that in other types. All patients were followed up. There were 56 cases with poor prognosis and 78 cases with good prognosis. Univariate analysis showed that perianal lesions and disease behavior were associated with prognosis of CD patients (P<0.05). Logistic multivariate analysis showed that perianal lesions, B2 and B3 types as disease behaviors were independent risk factors for poor prognosis of CD patients (P<0.05).
Conclusion
: There are certain differences in clinical features among patients with different Montreal classifications. The perianal lesions and disease behavior are closely related to poor prognosis of patients. Montreal classification can be applied as a reference index for disease assessment, diagnosis and prognosis judgement.

Changes of Serum miR-183, E-selectin and AMBP in Patients with Gastric Cancer and their Relationship with Clinical Pathology

Objective
: To analyze the changes of serum Microrna-183(mir-183), E-selectin and α1-microglobulin/precursor trypsin inhibitor (AMBP) in patients with gastric cancer and their relationship with clinical pathology.
Methods
: 96 patients with gastric cancer who treated from January 2016 to January 2017 in our hospital and 30 patients with outpatient health examination were selected at the same time. Then serum levels of miR-183, E-selectin and AMBP between the gastric cancer group and the control group were compared, and the correlation between the above indicators and clinical pathology was analyzed, and the predictive value of gastric cancer diagnosis was also analyzed.
Results
: Serum levels of miR-183, E-selectin and AMBP in gastric cancer group were higher than the control group, there were a statistically significant difference (P<0.05).The levels of miR-183, E-selectin and AMBP in serum of patients with different TNMⅢ-Ⅳ stages, lymph node metastasis and infiltration depth T3-T4 were higher than those of patients with TNM-Ⅱ stages, no lymph node metastasis and infiltration depth T1-T2, with statistical difference (P<0.05). Serum miR-183, E-selectin and AMBP levels showed no difference among different ages, genders, tumor distribution sites, HP infection and tumor differentiation (P>0.05). The area under the curve of miR-183 was 0.687, and the sensitivity and specificity were 0.833 and 0.625; The area under the E-selectin curve was 0.854, and the sensitivity and specificity were 0.831 and 0.829;The area under the AMBP curve was 0.752, and the sensitivity and specificity were 0.867 and 0.635;The area under the curve was 0.908, and the sensitivity and specificity were 0.900 and 0.843.
Conclusion
: Serum levels of miR-183, E-selectin and AMBP are correlated with the clinical pathology of gastric cancer, it can also facilitate the diagnosis of gastric cancer, combined monitoring can make up for the deficiency of single index and has high clinical application value.

Comparative Study on Immunoglobulin, Complement Levels and Immunopathology in Patients with Primary Glomerular Disease

Objective
: To investigate the relationship between immunoglobulin A nephropathy (IgAN) and membranous nephropathy (MN) in renal pathology and serum immunoglobulin and complement values, so as to provide scientific basis for assistant diagnosis of primary glomerular diseases.
Methods
: 332 patients with IgAN and MN, 85 in IgAN group and 247 in MN group were enrolled in our hospital. The blood biochemical and pathological indexes were measured respectively, and the results were compared.
Results
: The intensity of complement C3 pathological immune complex in IgAN group was stronger than that in MN group (P<0.01), and the deposition level of IgG pathological immune complex was weaker than that in MN group (P<0.01). The sIgA and sIgG values in IgAN group were higher than those in MN group (P<0.01, P<0.01), and the values of sC3 and sC4 were lower than those in MN group (P<0.01, P<0.01). The levels of BUN and SCR in IgAN group were higher than those in MN group (P<0.01, P<0.05). The levels of sC3 and sC4 in MN group were negatively correlated with the intensity of C3 pathological immune complex (P<0.05, P<0.05); the levels of sC3 in MN group were negatively correlated with the intensity of C3 pathological immune complex (P<0.05); the levels of Scr and sIgA in IgAN group were negatively correlated with the intensity of IgG (P<0.05, P<0.05); the levels of sIgG in MN group were negatively correlated with the intensity of IgG (P<0.05).
Conclusion
: There are differences in immunoglobulin, complement pathology and serological levels in patients with IgAN and MN, and their manifestations are different in different diseases. Immunoglobulin, complement pathology and serological levels are closely related to IgAN and MN. Therefore, the intensity of pathological grade can be predicted by monitoring serological indicators.

Objective
: To understand the quality of drinking water in rural areas, to ensure the safety of drinking water for rural residents, and to provide scientific basisfor improving the quality of drinking water in rural areas, effective and reasonable advice and measures.
Methods
: 34 items of physical and chemical indexes and microbiological indexes of rural drinking water in dry season and wet season from 2015 to 2018 were tested in accordance with the national standard of the people's Republic of China GB / T 5750-2006, the national standard of the people's Republic of China GB / T 5749-2006, and the water was determined in accordance with the national standard of the people's Republic of China GB 5749-2006 The qualified rate and unqualified items of sample test results are analyzed and compared.
Results
: From 2015 to 2018, the qualified rate of rural drinking water quality in Shuangluan district has no significant difference (χ

=8.69, P<0.05). The unqualified indexes of microorganism are mainly the total number of colonies and total coliform bacteria, and the unqualified indexes of physical and chemical are mainly sulfate and nitrate (calculated by n ), fluoride, chroma, total hardness and sulfate.
Conclusion
: The testing pass rate of drinking water quality in rural areas is not high, mainly with the geological structure of the water source, the lack of effective protection measures around the water source, and the lack of disinfection measures for rural water sources. Related, the water quality of drinking water in rural areas should be given sufficient attention.

Clinical Value of Plasma CT-1, Ghrelin, Lp-PLA2 Detection in Coronary Heart Disease

Objective
: To analysis of clinical value of plasma cardiotrophin -1(CT-1), growth hormone releasing polypeptide (Ghrelin), lipoprotein-associated phospholipase A2(Lp-PLA2) in coronary heart disease.
Methods
: 176 cases of coronary heart disease was from March 2015 to June 2018, 43 cases of control group, 45 stable angina pectoris (SAP), 42 cases of unstable angina pectoris (UAP), 46 cases of acute myocardial infarction (AMI). Comparison between groups of plasma CT-1, Ghrelin, Lp-PLA2 level, left ventricular ejection fraction (LVEF), left ventricular end systolic volume (LVESV), left ventricular end-diastolic volume (LVEDV),and analyze the correlation between them, and the relationship between cardiac adverse events.
Results
: plasma CT-1,Lp-PLA2 levels, LVEDV and LVESV of AMI group was higher than the UAP group, SAP group, control group, plasma Ghrelin of AMI group was lower than the UAP group, SAP group, control group, there was statistically significant differences (P<0.05). Plasma CT, Lp-PLA2 level and LVEF was negatively correlated, (r=-0.786, -0.654, P<0.05), and both LVEDV and LVESV was positively correlated, (r=0.881, 0.702, 0.621, 0.514, P<0.05). plasma Ghrelin level and LVEF was positively correlated (r=0.620, P<0.05), and LVEDV, LVESV were negatively correlated, (r=-0.433, 0.-594, P<0.05). Plasma CT-1 and Lp-PLA2 levels in MACE group were higher than those in non-MACE group, and plasma Ghrelin was lower than that in non-MACE group (P<0.05). The areas under the curve predicted by CT-1, Ghrelin, Lp-PLA2, CT-1+Ghrelin +Lp-PLA2 for MACE were 0.816, 0.797 and 0.699, and the areas under the curve of CT-1+Ghrelin +Lp-PLA2 were larger than a single index. Multifactorial Logistic regression analysis shows, plasma levels of CT-1 and Lp-PLA2 was independent risk factors for adverse cardiac events in patients with coronary heart disease, Ghrelin was a protective factor.
Conclusion
: Plasma CT-1, Ghrelin, Lp-PLA2 are related to coronary heart disease, and their changes may indicate adverse cardiovascular events.
