Effects of Serum CEA CA125 and Cyfra21-1 Levels on the Prognosis of Patients with Advanced NSCLC

Objective
: To observe the correlation between serum CEA, CA125 and cytokeratin 19 fragment (cyfra21-1) levels and the survival of patients with advanced non-small cell lung cancer (NSCLC), and to evaluate the application value of three tumor markers in the prognosis of patients with advanced NSCLC.
Methods
: The clinical characteristics, short-term efficacy and prognosis of 105 patients with stage III B and IV NSCLC treated in our hospital from January 2015 to June 2018 were retrospectively analyzed. Serum CEA, CA125 and Cyfra21-1 levels were measured before and after treatment.
Results
:There were significant differences in serum CEA, CA125 and Cyfra21-1 levels between adenocarcinoma and squamous cell carcinoma, stage III B and IV, and the number of extrapulmonary metastasis sites < 3 and < 3 (all P < 0.05). The levels of serum CEA, CA125 and Cyfra21-1 after treatment were significantly different from those before treatment (all P < 0.05); the progression-free survival (PFS) and total survival (OS) of patients with high-level indicators (CEA < 5 ng/ml, CA125 < 15 ng/ml, and Cyfra21-1 < 5 U/ml) were less than those of patients with low-level indicators (CEA < 5 ng/ml, CA125 < 15 ng/ml, Cyfra21-1 < 5 U/ml) (all P < 0.05). 05).
Conclusion
: Serum CEA, CA125 and Cyfra21-1 levels have high clinical value in judging the short-term therapeutic effect and prognosis of patients with advanced NSCLC.

Clinical Value of Blood sCD14-ST Combined with Urinary NGAL on Early Diagnosis of Acute Kidney Injury in Patients with Sepsis

Objective
: To investigate the effect of serum soluble cluster of differentiation 14 subtype (sCD14-ST) combined with urinary neutrophil gelatinase-associated lipocalin (NGAL) on the early diagnosis of acute kidney injury (AKI) in patients with sepsis.
Methods
: 132 patients with sepsis from October 2015 to December 2018 were selected and divided into AKI group (n=41) and non-AKI group (n=91) according to presence or absence of AKI. Then the serum creatinine (Scr), sCD14-ST, and urinary NGAL levels at 0, 4, 8, 12, 24 hours after sepsis were measured and analyzed.
Results
: ①The hospitalization time and 30-day mortality rate in the AKI group were significantly higher than those in the non-AKI group (P<0.05). ②There was no significant difference in serum Scr levels at 0, 4 and 8 hours in AKI group (P>0.05). The serum Scr level at 12 hours was significantly higher than those at 0, 4, 8 hours and non-AKI group, and the serum Scr levels at 24 hours was significantly higher than those at 0, 4, 8, 12 hours and non-AKI group(P<0.05). There was no significant difference in serum sCD14-ST and urinary NGAL levels at 0, 4 hours in AKI group (P>0.05). The levels of sCD14-ST in blood at 8 hours, 12 hours and 24 hours were significantly higher than those at 0h, 4 hours and non-AKI groups, and the levels of sCD14-ST in blood at 24 hours were significantly higher than those at 8 hours(P<0.05), and the levels of urine NGAL at 12 hours and 24 hours were significantly higher than those in 0, 4 hours and non-AKI groups, and the levels of NGAL in urine at 12 hours and 24 hors were significantly higher than those in 8 hours, and the urine at 24 hours was significantly higher than those in 12 hours (P<0.05). The blood Scr, sCD14-ST and urinary NGAL levels within non-AKI group at the post 0h had no difference with those of any other time points (P>0.05). ③ ROC curve analysis showed that the serum sCD14-ST threshold value was 927.50pg/mL, the sensitivity and specificity were 90.12% and 83.63%, respectively; the urine NGAL threshold value was 85ng/mL, and the sensitivity and specificity were 89.36% and 84.34%, respectively. The sensitivity and specificity of serum sCD14-ST combined with urinary NGAL for early diagnosis of sepsis in patients with sepsis were 91.45% and 86.23%, respectively.
Conclusion
: Serum sCD14-ST combined with urinary NGAL has a high predictive value for early diagnosis of sepsis in patients with sepsis, which is worthy of clinical application.

Diagnostic Values of MiR-133a and MiR-208a in Myocardial Injury after PCI in Patients with Acute Coronary Syndrome

Objective
: To analyze the diagnostic values of miR-133a and miR-208a in myocardial injury after percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS).
Methods
: 83 patients with ACS who underwent PCI in Handan First Hospital from June 2017 to January 2019 were divided into 41 patients with myocardial injury group and 42 patients with non-myocardial injury group according to the myocardial injury after PCI. Real-time fluorescence quantitative polymerase chain reaction (RT-PCR) was used to detect the levels of serum miR-133a and miR-208a, serum troponin I (cTnI) levels were measured by chemiluminescence immunoassay, Pearson correlation method was used to analyze the correlations between levels of serum miR-133a and miR-208a with cTnI, subject operating characteristic curve (ROC) was used to evaluate the diagnostic value of miR-133a and miR-208a in patients with myocardial injury after PCI.
Results
: The levels of serum cTnI and the relative expressions of miR-133a and miR-208a in myocardial injury group were significantly higher than those in non-myocardial injury group (P < 0.05); the relative expressions of miR-133a and miR-208a in serum of patients were positively correlated with the level of cTnI (r = 0.741, 0.770, P < 0.001); the areas under ROC curve (AUC) of miR-133a and miR-208a in diagnosing myocardial injury after PCI in ACS patients were 0.809 (95%CI was 0.825-0.961) and 0.853 (95%CI was 0.890-0.990), respectively, the diagnostic thresholds were 0.95 and 0.31, the sensitivity was 93.60% and 95.12%, the specificity was 73.80% and 83.33%, respectively.
Conclusion
: MiR-133a and miR-208a have good diagnostic sensitivities and specificities for myocardial injury after PCI in ACS patients. They are effective biomarkers of myocardial injury in ACS patients after PCI.

Analysis of Correlation between Serum Vitamin A PCT and Hemoglobin Levels and Severity of Acute Pneumonia in Children

Objective
: To analyze the correlation between levels of serum vitamin A (VA), procalcitonin (PCT) and hemoglobin (Hb) and disease severity of children pneumonia at acute stage.
Methods
: 76 children patients with pneumonia at acute stage in our hospital from August 2016 to August 2018 were included in observation group. According to the severity of disease, they were divided into mild group (n=31) and severe group (n=45) two subgroups. Another 30 healthy examiners at the same time period were included in control group (n=30). The levels of serum VA, PCT and Hb were compared between subgroups of observation group and between observation group and control group, and their correlation with disease severity was analyzed in children pneumonia at acute stage, and the receiver operating characteristic curve (ROC curve) was used to evaluate the diagnostic value of the above indicators on children pneumonia at acute stage.
Results
: The serum VA level in mild group was higher than that in severe group while the serum PCT level was lower than that in severe group (P<0.05). There was no significant difference in the Hb level between the two groups (P<0.05). The serum VA level in observation group was lower than that in control group while the serum PCT level was higher than that in control group (P<0.05). There was no significant difference in the Hb level between the two groups (P<0.05). The increases of serum VA and PCT levels were closely related to the occurrence of children pneumonia at acute stage (P<0.05). The ROC curve of VA showed AUC was 0.754 (P<0.05), with sensitivity of 0.526 and specificity of 0.900 respectively. The ROC curve of PTC showed AUC was 0.843 (P<0.05), with sensitivity of 0.855 and specificity of 0.767, respectively. The ROC curve of the two combination showed AUC was 0.872 (P<0.05) with sensitivity of 0.776 and specificity of 0.867, respectively, which were better than those of respective predictions (P<0.05).
Conclusion
s: The decrease of serum VA and increase of PCT in children patients with pneumonia at acute stage are closely related to the progression of disease. The detection of indexes has certain value in evaluating patients’ condition and guiding clinical treatment.

Effects of Salmon Calcitonin Injection and Zoledronic Acid Injection on Bone Turnover Markers and Bone Mineral Density in Patients with Postmenopausal Osteoporosis

Objective
: To study the effects of salmon calcitonin and zoledronic acid on bone turnover markers (BTMs) and bone mineral density (BMD) in patients with postmenopausal osteoporosis (PMOP).
Methods
: 74 patients with PMOP in our hospital from April 2017 to April 2018 were selected and evenly divided into two groups according to the simple randomization method (each n=37). The two groups were given an intravenous infusion of 5 mg of zoledronic acid on the basis of basic treatments of calcium carbonate D3, calcitriol and lifestyle intervention (once a year), and observation group was additionally given an intravenous infusion of 50 IU/d of salmon calcitonin (changed to once for every other day after 2 weeks, twice for 1 week after 1 month, administration time of 3 months per year). The clinical effects were compared between the two groups at 1 year after treatment.
Results
: The effective rates of treatment in observation group and control group were 94.29% and 77.14% respectively (P<0.05), and there was a statistically significant difference in the clinical efficacy between the two groups (P<0.05). After treatment, the VAS scores of bone pain in the two groups were significantly decreased (P<0.05), and the VAS score in observation group was lower than that in control group. The levels of serum PINP and β-CTX were significantly decreased in the two groups (P<0.05) while the level of serum 25OHD was significantly increased in the two groups, and the levels of PINP and β-CTX in observation group were significantly lower than those in control group. The BMD values of lumbar vertebrae L
1~4
, femoral neck and Ward’’s triangle were significantly increased in the two groups (P<0.05), and the BMD of each part in observation group was higher than that in control group (P<0.05). There were no significant differences in the incidence rates of adverse reactions such as headache and dizziness, nausea and vomiting, fever, myalgia, arthralgia and influenza-like symptoms during treatment between the two groups (P>0.05), and the incidence rates of fractures during treatment were 8.57% and 17.14% respectively in the two groups (P>0.05).
Conclusion
: Zoledronic acid combined with salmon calcitonin in the treatment of PMOP can alleviate bone pain, reduce the expression of BTMs and promote BMD. The clinical effect of zoledronic acid alone has obvious advantages over zoledronic acid alone.

Changes of Expression of MiR-181a and CDX2 in Patients with Chronic Atrophic Gastritis and the Relationship with Prognosis

Objective
: To analyze the expressions of micro RNA-181a (miR-181a) and caudal type homologous transcription factor-2 (CDX2) in gastric mucosa, and to explore their relationships with clinicopathological parameters and prognosis in patients with chronic atrophic gastritis (CAG).
Methods
: 90 patients with CAG admitted from February 2017 to September 2018 were selected as the study subjects, atrophic mucosal biopsy tissues and surrounding normal mucosal biopsy tissues were retained during gastroscopy. Real-time fluorescence quantitative PCR (qRT-PCR) was used to detect the expression of miR-181a in biopsy tissues, immunohistochemical staining (SP method) was used to detect the expression of CDX2 protein; analyzed their expressions in different tissues and their relationships with clinicopathological parameters and prognosis of CAG patients.
Results
: The expression level of miR-181a in CAG gastric mucosa was significantly higher than that in normal gastric mucosa (P<0.05), and the positive rate of CDX2 protein was significantly higher than that in normal gastric mucosa (P<0.05). The expressions of miR-181a and CDX2 were not related to sex, Hp infection and chronic inflammation (P>0.05), but to atrophy of gastric mucosa, intestinal metaplasia and atypical hyperplasia in CAG patients (P<0.05). The expressions of miR-181a and CDX2 were related to the prognosis of CAG patients (P<0.05).
Conclusion
: MiR-181a and CDX2 are highly expressed in gastric mucosa of CAG, their expressions are related to the clinicopathological parameters such as atrophy of gastric mucosa and intestinal metaplasia, and prognosis of CAG patients, which can provide some help for clinical treatment.

Changes and Clinical Significance of Serum BGP and AKP Levels in Patients with Diabetes Mellitus before and after Artificial Dental Implant Restoration

Objective
: To explore the changes and clinical significance of serum osteocalcin (BGP) and alkaline phosphatase (AKP) levels in patients with diabetes mellitus (DM) before and after artificial dental implant restoration.
Methods
: A total of 106 patients with DM who underwent artificial dental implant restoration from June 2014 to June 2017 were selected for the study. According to the glycemic control, they were divided into glycemic control group (n=66) and poor glycemic control group (n=40). And 50 healthy people with artificial dental implant restoration treated in the same period were selected as control group. The levels of BGP and AKP in each group were evaluated before operation and at 6 months after operation, and the probing depth (PD) and depth of bone loss (DSB) were compared at 6 months after operation and at 1 year after operation. The incidence rate of inflammation around implant was recorded.
Results
: The levels of BGP and AKP in glycemic control group and poor glycemic control group at each time point were higher than those in control group (P<0.05). Thelevels of BGP and AKP were increased first and then decreased at the time period from before operation to 1 year after operation in glycemic control group and poor glycemic control group (P<0.05), and thelevels of BGP and AKP in poor glycemic control group at each time point were higher than those in glycemic control group (P<0.05), and there were no interactions of different groups and time in the BGP and AKP (P>0.05). The PD level at 1 year after operation was higher than that at 6 months after operation in glycemic control group and poor glycemic control group (P<0.05), and there was no significant difference in control group between at 6 months after operation and at 1 year after operation (P>0.05), and the difference between the two time points in poor glycemic control group was higher than that in glycemic control group and control group (P<0.05), and the difference between the two time points in glycemic control group was higher than that in control group (P<0.05). At 1 year after operation, the DSB levels in the three groups were higher than those at 6 months after operation (P<0.05), and the difference between the two time points in poor glycemic control group was higher than that in glycemic control group and control group (P<0.05), and the difference between the two time points in glycemic control group was higher than that in control group (P<0.05). The incidence rate of inflammation around implant in poor glycemic control group was higher than that in glycemic control group and control group (P<0.05), and there was no significant difference in the incidence rate of inflammation around implant between glycemic control group and control group (P>0.05).
Conclusion
: DM can have a variety of negative effects on dental implants. DM patients should actively control blood glucose after dental implant surgery so as to improve the prognosis of dental implants.

Application Value of Contrast-enhanced Ultrasound in Cerebral Perfusion Imaging of Ischemic Stroke

Objective
: To observe the effect of contrast-enhanced color ultrasound (CEUS) on carotid artery, vertebral artery and cerebral blood flow perfusion in patients with ischemic stroke.
Methods
: 50 patients with ischemic stroke from September 2018 to March 2019 were selected as the study group. Contrast-enhanced ultrasonography (CEUS) was used to measure the blood flow of right carotid artery (rICA), left carotid artery (LCA), right vertebral artery (rVA), left vertebral artery (lVA), anterior cerebral blood flow perfusion (aCBFV), posterior cerebral blood flow perfusion (pCBFV), and whole cerebral blood flow perfusion. (tCBFV).
Results
: The aCBFV of patients with anterior circulation cerebral infarction was significantly lower than that of patients with posterior circulation cerebral infarction (P < 0.05), and pCBFV was significantly higher than that of patients with posterior circulation cerebral infarction (P < 0.05). There were no significant differences in tCBFV, aCBFV and pCBFV among different types of cerebral infarction patients with anterior and posterior circulation (P > 0.05).
Conclusion
: Contrast-enhanced ultrasound can effectively check the arterial blood flow in patients with ischemic stroke, and can accurately evaluate cerebral perfusion blood flow, which has a good role in the diagnosis of disease.

Effect of Early Probiotics combined with Enteral Mutrition Intervention on Intestinal Tolerance and Inflammatory Mediators in Elderly Patients with SAP in ICU

Objective
: To explore the clinical effects of early probiotics combined with enteral nutrition (EN) in elderly patients with severe acute pancreatitis (SAP).
Methods
: The clinical data of 116 elderly SAP patients were retrospectively analyzed. All patients were treated with conventional comprehensive therapy. Patients only with early EN intervention were included in control group (n=54), and patients with early probiotics combined with EN intervention were included in combined group (n=62). Before intervention and after 7d of intervention, the serum inflammatory mediators [interleukin-6 (IL-6), interleukin-10 (IL-10), tumor necrosis factor-α (TNF-α)], intestine mucosal functional indicators [diamine oxidase (DAO), dextrolactic acid (D-Lac), endotoxin] and humoral immune functional indicators [immunoglobulin A (IgA), IgG, IgM] were compared between the two groups, and the hospitalization record indexes, survival time and occurrence of intestinal intolerance symptoms within 7d were analyzed in the two groups.
Results
: After 7d of intervention, the levels of serum IL-6, IL-10, TNF-α, DAO, D-Lac and endotoxin in the two groups were significantly lower than those before intervention, and the levels in combined group were significantly lower than those in control group at the same time period (all P<0.05). The levels of serum IgA, IgG and IgM in the two groups were significantly higher than those before intervention, and the levels in combined group were significantly higher than those in control group at the same time period (all P<0.05). Within 7d after intervention, the incidence rates of abdominal distension and diarrhea in combined group were significantly lower than those in control group (all P<0.05), but there were no significant differences in the incidence rates of nausea/vomiting and constipation between the two groups (all P>0.05). There was no significant difference in survival time between the two groups (P>0.05). Among surviving patients, the abdominal pain relief time, oral feeding time, ICU stay and hospital stay in combined group were significantly lower than those in control group (all P<0.05).
Conclusion
: Early probiotics can effectively improve the EN tolerance of elderly SAP patients, help to improve the nutritional intervention effects, and promote the outcomes of disease, and it has high clinical application value.

Effect of Concurrent Chemoradiotherapy and Radiotherapy on Serum TGF-beta 1 IGF-1 and Prognosis of Advanced Esophageal Cancer

Objective
: To analyze the efficacy of concurrent chemoradiotherapy in the treatment of advanced esophageal cancer and its influences on serum transforming growth factor-1 (TGF-β1), insulin-like growth factor-1 (IGF-1) and prognosis.
Methods
: 98 patients from June 2016 to January 2018 with advanced esophageal cancer were divided into 55 control group and 43 research group according to the random number table method. The control group was treated with sequential chemoradiotherapy, and the research group was treated with concurrent chemoradiotherapy.
Results
: The Objective remission rate of the study group was higher than that of the control group (79.09% vs 51.18% P < 0.05). After treatment, the levels of serum TGF-beta 1 and IGF-1 in both groups decreased, and the levels of serum TGF-beta 1 and IGF-1 in the study group were lower than those in the control group [(139.09+14.29) vs (285.71+36.10) mg/L, (55.43+6.75) vs (80.21+10.43) ug/L, P < 0.05]. The rate of nausea, vomiting and leukocyte decrease in the study group was higher than that in the control group (67.72% vs 86.04%, 20.00% vs 39.53%, P < 0.05). There was no significant difference in the rate of radioesophagitis and transaminase increase between the two groups (9.09% vs 13.95%, 5.45% vs 9.30%, P > 0.05). During the follow-up period, the median survival time of the study group was longer than that of the control group [26.8 (95% CI: 23.585-30.089) months vs 23.0 (95% CI: 20.142-25.895) months, P < 0.05].
Conclusion
: Concurrent chemoradiotherapy has a good effect in the treatment of advanced esophageal cancer. It can reduce the levels of serum TGF-beta 1 and IGF-1, prolong the survival time of patients, and tolerate the toxicity and side effects.

Clinical Efficacy of Cilostazol combined with Prostaglandin E1 in the treatment of Early Type 2 Diabetic Nephropathy and its Effects on Serum CysC beta 2-MG and Urinary Protein

Objective
: To analyze the clinical curative effect of cilostazol combined with prostaglandin E1 on early type 2 diabetic nephropathy and the influence on serum cystatin C (CysC), β2-microglobulin (β2-MG) and urine protein (ALB).
Methods
: A total of 90 patients with early type 2 diabetic nephropathy who were admitted to the hospital during the period from May 2016 to May 2018 were selected as subjects. They were randomly divided into the observation group and the control group, 45 cases in each group. The control group was treated with cilostazol and other conventional drugs. On this basis, the observation group was treated with prostaglandin E1. The treatment response rate, levels of serum CysC, β2-MG and glycosylated hemoglobin (HbA1c), renal function indexes [ALB, serum creatinine (Scr), blood urea nitrogen (BUN)] before and after treatment and adverse reactions were compared between the two groups.
Results
: The treatment response rate in the observation group was significantly higher than that in the control group (93.33% vs 75.56%) (P<0.05). The decrease oflevels of serum CysC, β2-MG and HbA1c, ALB, Scr and BUN in the observation group was significantly larger than thatin the control group after treatment (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (13.33% vs 8.88%) (P>0.05).
Conclusion
: The treatment of early type 2 diabetic nephropathy patients with cilostazol combined with prostaglandin E1 can significantly improve the clinical efficacy, improve the serum CysC, beta 2-MG and renal function, and is safe and reliable, which is worthy of clinical promotion and practice.

Therapeutic Effect of Interferon combined with Montelukast and Vitamin D on Asthmatic Bronchitis

Objective
: To observe the efficacy of interferon combined with montelukast and vitamin D in the treatment of asthmatic bronchitis.
Methods
: A total of 96 children patients with asthmatic bronchitis admitted to pediatric department of our hospital from March 2016 to September 2018 were selected and divided into observation group and control group according to the random number table method, with 48 cases in each group. Control group was given montelukast combined with vitamin D, and observation group was given interferon on this basis, and 7d was taken as one course of treatment. The efficacy and immune function were analyzed in the two groups after the end of one course of treatment, and the adverse drug reactions and recurrence rate within 6 months were observed.
Results
: The remission times of symptoms of wheezing, cough and lung wheezing rale and hospital stay in observation group were significantly lower than those in control group (P<0.05). There was a statistically significant difference in the clinical efficacy between the two groups (P<0.05), and the total effective rate in observation group was significantly higher than that in control group (92.00% & 76.00%) (P<0.05). After treatment, the levels of IgA, CD4+ and CD4+/CD8+ in the two groups were significantly higher than those before treatment (P<0.05) while the levels of IgE and CD8+ were significantly lower than those before treatment (P<0.05), and the increases of IgA, CD4+ and CD4+/CD8+ and decreases of IgE and CD8+ in observation group were greater than those in control group (P<0.05). There was no significant difference in the incidence rate of adverse reactions between the two groups (8.33% & 6.25%) (P>0.05). Within 6 months, the recurrence rate in observation group was significantly lower than that in control group (4.17% & 18.72%) (P<0.05).
Conclusion
: Interferon combined with montelukast and vitamin D in the treatment of asthmatic bronchitis can effectively alleviate symptoms, improve clinical efficacy, improve immune function and reduce the recurrence rate of children with asthmatic bronchitis. It is safe and has clinical application value.

Relationships Between Serum Prealbumin Albumin and Hemoglobin Levels with BODE Index in Patients with Stable COPD

Objective
: To detect the levels of serum prealbumin (PA), albumin (ALB) and hemoglobin (HB) in patients with stable chronic obstructive pulmonary disease (COPD) and to analyze their relationships with BODE index, i.e. bodymass index (B), degree of airflow obstruction (O), dyspnea (D), exercise capacity (E).
Methods
: 120 patients with stable chronic obstructive pulmonary disease hospitalized in the Zhujiajiao People’s Hospital of Qingpu District in Shanghai from January 2015 to November 2018 were selected as the observation group, during the same period, 86 healthy persons who came to Zhujiajiao People’s Hospital of Qingpu District in Shanghai for physical examination were selected as the control group, the BODE index score was determined according to the BODE index score standard, serum albumin level was measured by bromocresol green method, serum prealbumin level was measured by immunoturbidimetry, the hemoglobin level was determined by high iron cyanide method, analyzed the correlations between serum prealbumin, albumin, and hemoglobin levels with BODE levels in patients with stable COPD.
Results
: The BODE index of the observation group was significantly higher than that of the control group (P < 0.05); the levels of serum prealbumin, albumin, and hemoglobin in the observation group were significantly lower than those in the control group (P < 0.05); the levels of serum prealbumin, albumin, and hemoglobin in patients with BODE index grade 4 were significantly lower than those in patients with grade 1, grade 2, and grade 3 (P < 0.05), the levels of serum prealbumin, albumin, and hemoglobin in grade 3 patients were significantly lower than those in grade 1 and grade 2 patients (P < 0.05), the levels of serum prealbumin, albumin, and hemoglobin in grade 2 patients were significantly lower than those in grade 1 patients (P < 0.05); serum prealbumin, albumin, and hemoglobin levels in patients with stable COPD were negatively correlated with BODE index (P < 0.05).
Conclusion
: The levels of serum prealbumin, albumin, and hemoglobin in patients with stable COPD were significantly decreased, serum prealbumin, albumin, and hemoglobin levels were negatively correlated with BODE index, it may be an effective index for predicting the condition of COPD patients in stable stage.

Expression Imbalance of Helper T cells and the Change of NF - κ B in Premature Infants with White Matter Injury

Objective
: To demonstrate the expression of T helper cell 1 (Th1), T helper cell 2 (Th2) and nuclear factor-κB (NF-κB) in premature infants with white matter injury (WMI).
Methods
: Serum samples were collected from 70 WMI neonates, plus another control group of 70 premature babies without WMI. Patients were further divided into mild, moderate and severe WMI groups. Reverse transcription quantitative polymerase chain reaction was used to test mRNA expression levels of Th1/Th2 cytokines, including interleukin 2 (IL-2), tumor necrosis factor- α (TNF-α ), IL-4,IL-10 and nuclear factor (NF)- κB, whilst their serum levels were measured by ELISA.
Results
: Serum levels of IL-4 and IL-10 were significantly decreased in premature WMI babies, whilst IL-2, TNF-α and NF-κB were upregulated (P<0.05 vs. control group). With aggravated disease, IL-4 and IL-10 expression was further decreased while IL-2, TNF-α and NF-κB were increased (P<0.05 vs. mild WMI group). Th1 cytokines IL-2 and TNF-α and NF-κB were negatively correlated with Th2 cytokines IL-4 and IL-10. Disease severity was positively correlated with IL-2, TNF-αand NF-κB expression, and was negatively correlated with IL-4 and IL-10 (P<0.05).
Conclusion
: Upregulation of Th1/Th2 imbalance NF-kappa B may be a potential factor involved in the pathogenesis of WMI in premature infants for early diagnosis and treatment.

Effects of Lumbar Plexus Block combined with General Anesthesia on Vital Signs and Postoperative Analgesia Satisfaction in Elderly Patients undergoing Hip Replacement

Objective
: To analyze the effects of lumbar plexus block combined with general anesthesia on vital signs and postoperative analgesia satisfaction in elderly patients undergoing hip replacement.
Methods
: 80 elderly patients undergoing hip replacement in the hospital from August 2017 to August 2019 were enrolled in the study. They were divided into observation group and control group by random number table method and simple random grouping method, 40 cases in each group. The control group was given simple general anesthesia, while observation group was given lumbar plexus block combined with low general anesthesia. The surgery and anesthesia related indexes were recorded. The vital signs [mean arterial pressure (MAP), heart rate (HR), blood oxygen saturation (SpO

)], postoperative pain [visual analogue scale (VAS)], analgesia satisfaction and adverse reactions were compared between the two groups at tracheal intubation time (T0), at 1 min before surgical skin incision (T1), at incision suture (T2) and at 30 min after entering anesthesia recovery room (T3).
Results
: The sufentanil dosage in observation group was less than that in control group (P<0.05), and postoperative extubation time was significantly shorter than that in control group (P<0.05). There was no significant difference in operation time or hospitalization time between the two groups (P>0.05). MAP and HR at T1to T3 were increased compared with those at T0 in both groups, while SpO

was lower than that at T0. MAP and HR at T1, T2 and T3 in observation group were lower than those in control group, while SpO
2
was higher than that in control group (P<0.05). VAS scores of observation group were lower than those of control group at 2h, 12h, 24h and 48h after surgery (P<0.05). The postoperative analgesia satisfaction of observation group was significantly higher than that of control group (P<0.05). The incidence of adverse reactions in observation group within 48h after surgery was lower than that in control group (P<0.05).
Conclusion
: Lumbar plexus block combined with general anesthesia can effectively reduce the dosage of sufentanil, shorten the extubation time, reduce the fluctuation of vital signs and postoperative adverse reactions in elderly patients with hip replacement, and the analgesic effect is satisfactory, which is worthy of clinical popularization and practice.

Effect of Ulinastatin on Inflammatory Factors and Coagulation Function in Patients with Severe Sepsis

Objective
: To study effect of ulinastatin on inflammatory factors and coagulation function in patients with severe sepsis.
Methods
: 127 severe sepsis received therapy from February 2017 to February 2018 in our hospital were selected as research objects. According to the simple random grouping table, those patients were divided into the observation group (n=65) and the control group (n=62). The control group was treated with Xuebijing injection, while the observation group treated with Ulinastatin combined with Xuebijing injection. Then the Therapeutic efficacy, Interleukin-6 (IL-6), Interleukin-8 (IL-8), Tumor Necrosis Factor (TNF-alpha) levels, coagulation function indicators of platelet(PLT), fibrinogen (FIB), prothrombin time (PT) and activated partial thromboplastin time (APTT), Physiology and Chronic Health Score (APACHE II) Systemic Infection Related Organ Dysfunction Score (SOFA) of two groups after treatment were compared.
Results
: After treatment, The total effective rate of observation group was significantly higher than that of control group [92.30%(60/65)vs74.19%(46/62)](P<0.05). The levels of IL-6, IL-8 and TNF-alpha in the control group were significantly lower than those in the control group [(24.86±3.12)ng/L vs(61.85±5.67)ng/L,(24.16±5.80)ng/L vs(51.79±10.73)ng/L,(85.16±11.10)g/L vs(116.98±13.67 g/L)](P<0.05). Efficacy and Toxic and Side Effects of Vacuum Pad combined with Breast Bracket on Set-up Radiotherapy of Breast Cancer

Objective
: To observe the effects of vacuum pad combined with breast bracketon set-up errors, efficacy and toxic and side effects of patients with set-up radiotherapy of breast cancer.
Methods
: The clinical data of 86 patients with breast cancer who underwent radiotherapy from April 2013 to April 2016 were retrospectively analyzed. According to the different Methods of radiotherapy position fixing, the patients were divided into observation group (45 cases) and control group (41 cases).Observation group was given vacuum pad combined with breast bracket for fixation, and control group was only given breast bracket for fixation. The set-up errors and pass rate of set-ups, cardiopulmonary irradiation dose, toxic and side effects of radiotherapy and short-term and long-term efficacy were compared between the two groups.
Results
: The set-up errors of X-axis and Y-axis in observation group were significantly lower than those in control group, and the pass rate of set-ups was significantly higher than that in control group (85.33% vs 63.90%) (P<0.05). The cardiopulmonary irradiation dose in observation group was significantly lower than that in control group (P<0.05), and the total incidence rate of radiotherapy-related toxic and side effects was significantly lower than that in control group (42.22%vs 78.05%) (P<0.05). There were no statistically significant differences in the short-term efficacy and local recurrence, distant metastasis and survival rate within 3 years between the two groups (P>0.05).
Conclusion
: Vacuum pad combined with breast bracket for set-up radiotherapy of breast cancer can reduce the set-up errors reduce the cardiopulmonary irradiation dose and toxic and side effects of radiotherapy.

Effects of YAG Laser Peripheral Iridotomy on Intraocular Pressure after Refractory Glaucoma Treatment

Objective
: To explore the effects of YAG laser peripheral iridotomy on intraocular pressure after refractory glaucoma treatment.
Methods
: The treatment data of 79 patients with refractory glaucoma in our hospital from January 2016 to January 2017 were retrospectively analyzed. Study group (n=40, 45 eyes) was given YAG laser peripheral iridotomy, and control group ( n=39, 43 eyes) was given routine peripheral iridotomy. The intraocular pressure, visual acuity, peripheral anterior chamber depth and anterior chamber angle before and after surgery, occurrence of postoperative complications and surgical success rate were compared between the two groups.
Results
: Postoperative intraocular pressure was significantly decreased in the two groups (P<0.05), and the index after operation in study group was significantly lower than that in control group (P<0.05). The changes of visual acuity in study group after surgery was significantly ,lower than that in control group (P<0.05). The anterior chamber depth and anterior chamber angle in the two groups was significantly improved after surgery compared with that before surgery (P<0.05), and the index in study group after surgery was significantly higher than that in control group (P<0.05). The incidence rate of postoperative complications in study group was lower than that in control group (7.5% vs 15.38%) (P>0.05). The surgical success rate in study group was higher than that in control group (100% vs 64.1%) (P<0.05).
Conclusion
s: YAG laser peripheral iridotomy has significant pressure-reduction effects in the treatment of refractory glaucoma, and it has high surgical success rate.

Effect of Preconditioning with Rosuvastatin on Myocardial Reperfusion Left Ventricular Remodeling and Myocardial Injury after PCI for Acute Myocardial Infarction

Objective
: To investigate the effect of different preconditioning with rosuvastatin on myocardial reperfusion, left ventricular remodeling and serum creatine kinase isoenzyme (CK-MB) and troponin T (cTnT) levels after percutaneous coronary intervention (PCI) for acute myocardial infarction (AMI).
Methods
: 150 patients with AMI were randomly divided into the observation group and the control group. The observation group was treated with oral rosuvastatin of 20 mg/d before operation and at 14 days after operation, and the dose was adjusted to 10 mg/d 14 days later. The control group was treated with oral rosuvastatin of 10mg/d before and after PCI. The intervention lasted for 1 month after operation. The thrombolysis in myocardial infarction (TIMI) grading in each group was observed at 1 week after operation. Left ventricular remodeling parameters [high-sensitivity C-reactive protein (hs-CRP), matrix metalloproteinase-9 (MMP-9)], serum CK-MB and cTnT levels were determined before and after operation. The major adverse cardiac events and adverse drug reactions in both groups during treatment were statistically analyzed.
Results
:The proportion of TIMI grade 3 in the observation group at 1 week after operation was significantly higher than that in the control group (93.33% vs 80.00%) (P<0.05). The levels of hs-CRP and MMP-9 in both groups increased significantly at 1 week after operation but the levels at 1 month after operation were significantly lower than those at 1 week after operation (P<0.05). The levels of serum CK-MB and cTnT in both groups decreased significantly at 1 week after operation, and the levels at 1 month after operation were significantly lower than those at 1 week after operation (P<0.05). The levels of hs-CRP, MMP-9, CK-MB and cTnT were significantly lower in the observation groupthan in the control group at 1 week and 1 month after operation (P<0.05). The incidence of severe arrhythmia in the observation group was significantly lower than that in the control group (P<0.05), but there was no significant difference in adverse drug reactions between the two groups (P>0.05).
Conclusion
:Compared with conventional dose, high-dose rosuvastatin can better improve myocardial reperfusion in patients with AMI after PCI, inhibit left ventricular remodeling, relieve myocardial injury, and reduce the incidence of severe arrhythmia, without increasing adverse drug reactions.

Efficacy Evaluation of Propranolol and Methimazole in the Treatment of Hyperthyroidism

Objective
: To analyze and evaluate the clinical efficacy of propranolol and methimazole in the treatment of hyperthyroidism.
Methods
:80 patients with hyperthyroidism were selected from the fourth affiliated hospital of Harbin medical university.All patients were divided into control group and observation group by random number method, with 40 cases in each group.Patients in the control group were treated with thimidazole, and patients in the observation group were treated with thimidazole combined with the control group.The improvement of thyroid function index, clinical treatment effect and the incidence of adverse reactions after treatment were compared between the two groups.
Results
:Before treatment, there was no significant difference in thyroid function indicators between the two groups (P>0.05). After treatment, the improvement of thyroid function in the observation group was significantly better than that in the control group, with statistically significant differences (P<0.05). The effective rate of clinical treatment in the observation group was significantly higher than that in the control group (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).
Conclusion
: The combined use of propranolol and methimazole in the treatment of hyperthyroidism has a significant effect, which can effectively improve the clinical symptoms of patients without increasing the incidence of adverse reactions.

Effects of Minimally Invasive Surgical Resectionand Interventional Embolization on Perioperative Indexes and Short-term Prognosis in Patients with Hepatic Hemangioma

Objective
: To investigate the effects of minimally invasive surgical resection and interventional embolization on perioperative indexes and short-term prognosis in patients with hepatic hemangioma.
Methods
: The clinical data of 60 patients with hepatic hemangioma in the hospital from January 2017 to June 2018 were retrospectively analyzed. According to surgical Methods, 28 patients undergoing minimally invasive surgical resection were included in group B. 32 patients undergoing interventional embolization were included in group A.The perioperative indexes and short-term prognosis were compared between the two groups.
Results
: Theoperation time and hospital stay in group A were shorter than those in group B, and intraoperative blood loss was less than that in group B (P<0.05). The total treatment cost of group A was higher than that of group B (P<0.05). There was no significant difference in level of total bilirubin (TB) between the two groups after treatment (P>0.05). After treatment, levels of alanine aminotransferase (ALT) and aspartate transaminase(AST) in group A were lower than those in group B, while albumin (ALB) level was higher than that in group B (P<0.05). There was no significant difference in incidence of postoperative short-term complications between the two groups (P>0.05).
Conclusion
: The surgical trauma is smaller, prognosisrecovery is quickly and liver function damage is smaller with interventional embolization in treatment of hepatic hemangioma compared with minimally invasive surgical resection.

Objective
: To investigate the effect and complications of single-port laparoscopic with Pediatric activity retractor and surgery via small incision on inguinal hernia in children.

: 120 cases of pediatric indirect inguinal hernia operated in our hospital from June 2017 to June 2018 were selected by stratified sampling. 60 cases in the control group were treated with hernia sac high ligation through small incision of lower abdominal wall from June 2017 to January 2018. 60 cases in the observation group were treated with single-hole laparoscopic hernia sac high ligation under single-hole laparoscopy from February 2018 to June 2018. The operation time, blood loss, hospital stay and complications were compared between the two groups.
Results
: The operation time, intraoperative bleeding volume and postoperative free time in the observation group respectively was(10.5±2.2、1.2±0.2、2.7±0.4),which were lower than those in the control group,the differences were significant(P<0.05); The post-operative complications in the observation group respectively was(χ

Objective
: To investigate the effect of endoscopic metal titanium clip and injection hemostasis on acute non-variceal upper gastrointestinal bleeding.
Methods
: 100 patients with acute non-variceal upper gastrointestinal bleeding who were enrolled from February 2015 to February 2017 were randomly divided into two groups. Endoscopic patients were given endoscopic titanium titanium capsules. Patients in the drug group were given Injection of drugs to stop bleeding, compared the two groups of clinical symptoms score, vital signs, blood indicators, the overall hemostatic effect and quality of life.
Results
: The clinical symptoms of the patients in the endoscopic group were significantly lower than those in the drug group. The heart rate level was significantly lower than that of the drug group. The levels of diastolic blood pressure, systolic blood pressure, erythrocyte and hemoglobin were significantly higher than those of the drug group (P <0.01). The effective rate of bleeding was significantly higher than that of the drug group (P <0.05). There was significant difference between the two groups (P <0.05).
Conclusion
: Endoscopic titanium titanium tourniquet has a significant effect on acute non-variceal upper gastrointestinal bleeding.

Effect of Sevelamer Carbonate on Microinflammation and Prognosis in Patients with Chronic Renal Failure Complicated with Hyperphosphatemia

Objective
: To investigate the effect of Sevelamer carbonate on microinflammation and prognosis in patients with chronic renal failure complicated with hyperphosphatemia, and to explore its microinflammation status and treatment measures.
Methods
: 80 patients with chronic renal failure complicated with hyperphosphorus were selected from August 2017 to August 2018 in our hospital. The patients were divided into observation group (40 cases) and control group (40 cases), according to the simple random grouping method. The control group was treated with calcium carbonate on the basis of routine hemodialysis, while the control group was treated with calcium carbonate on the basis of routine hemodialysis. Four weeks after treatment, the levels of TNF-alpha, CRP, IL-6, blood phosphorus, calcium and parathyroid hormone (iPTH) and the incidence of adverse reactions were compared between the two groups before and after treatment.
Results
: After treatment, the levels of TNF-a, CRP and IL-6 in the two groups were significantly lower than those before treatment, and the levels of each index in the observation group were significantly lower than those in the control group (P < 0.05); the levels of blood phosphorus, iPTH, calcium-phosphorus product in the two groups were significantly lower (P < 0.05), and the levels of each index in the observation group were significantly lower than those in the control group (P < 0.05). The difference was statistically significant (P < 0.05); the levels of serum calcium and bicarbonate in the two groups were significantly higher than those in the control group, and the level of bicarbonate in the control group was significantly lower than that in the observation group (P < 0.05); there was no statistical difference in the incidence of adverse events between the two groups (P < 0.05).
Conclusion
: Sevelamer carbonate can effectively reduce the level of inflammatory factors and regulate the level of blood phosphorus and calcium in patients with chronic renal failure combined with hyperphosphatemia, with good safety.

Effects and Adverse Reactions of Dexmedetomidine Combined with Combined Spinal-epidural Anesthesia on Sedative Effects in Pregnant Women Undergoing Cesarean Section

Objective
: To explore the sedative effects and adverse reactions of dexmedetomidine combined with combined spinal-epidural anesthesia on pregnant women undergoing cesarean section.
Methods
: 68 cases of pregnant women with cesarean section admitted to our hospital from January 2017 to September 2018 were selected for the study and were given simple random grouping by the random number table, and were divided into observation group and control group, with 34 cases in each group. The two groups were given combined spinal-epidural anesthesia, and observation group was given ropivacaine+dexmedetomidine, and control group was given ropivacaine+normal saline. Mean arterial pressure (MAP), heart rate (HR), oxygen saturation (SpO
2
) and Ramsay sedation score were recorded before anesthesia (T1), after 5 min (T2) and 20 min (T3) of anesthesia and at fetal delivery (T4). And the neonatal Apgar score and NBNA score were compared between the two groups, and the occurrence of maternal adverse reactions was recorded.
Results
: The MAP and HR at time points of T2, T3 and T4 in control group were lower than those at T1, and lower than those in observation group (P<0.05). There were no statistically significant differences in the MAP and HR in observation group and SpO
2
in the two groups before and after anesthesia (P>0.05). There were no significant differences in the Ramsay sedation scores between the two groups at T1 and T2 (P>0.05), and the Ramsay sedation scores at T3 and T4 in observation group were higher than those in control group (P<0.05). There were no significant differences in the Apgar score at 1min and 5min after delivery, NBNA score at 2d and 7d after delivery and incidence rate of maternal adverse reactions between the two groups (P>0.05).
Conclusion
s: Dexmedetomidine combined with combined spinal-epidural anesthesia has good sedative effects on pregnant women with cesarean section, and it is beneficial to maintain intraoperative hemodynamic stability, and has small adverse effects on puerperae and neonates, and it has good safety.

Efficacy and Safety of Thoracic Endovascular Aortic Repair combined with Drug in Patients with Stanford B Type Aortic Dissection

Objective
: To study the efficacy and safety of thoracic endovascular aortic repair (TEVAR) combined with drug and absolute drug therapy in patients with Stanford B type aortic dissection (Stanford B type AD).
Methods
: The clinical data of 86 patients with Stanford B type AD admitted to our hospital from2010.01 to 2015.01 were retrospectively analyzed. According to the treatment Methods, they were divided into combined group (TEVAR+drug therapy, n=56) and drug group (pure drug therapy, n=30). The treatment effects were observed in the two groups.
Results
: The success rate of surgical technique was 100% in combined group. There were no significant differences in the various complications during hospitalization between the two groups (P>0.05). There was no significant difference infatality rate during hospitalization (P>0.05). At 12~48 months of follow-up after operation, there were no significant differences in the fatality rate, major complications and maximum diameter of true lumen between the two groups (P>0.05), and the thrombosis formation rate and thrombus absorption rate in combined group were significantly higher than those in drug group (P>0.05), and the maximum diameter of false lumen was significantly less than that in drug group (P<0.05). The median survival time in drug group and combined group both were 48 months, and the Log Rank test showed there was no significant difference in survival rate between the two groups (χ

=0.50, P>0.05).
Conclusion
: TEVAR combined with drug therapy can effectively improve the thrombosis formation rate and thrombus absorption rate in patients with Stanford B type aortic dissection, and reduce the maximum diameter of false lumen. However, it has more postoperative complications and can not effectively extend the survival time of patients.

Effects of Microecologics combined with Antiviral Therapy on Immune Function and Liver Fibrosis Indexes in Patients with Hepatitis B Cirrhosis

Objective
: To explore the effects of microecologics combined with antiviral therapy on immune function and liver fibrosis indexes in patients with hepatitis B cirrhosis.
Methods
: 84 patients with hepatitis B cirrhosis admitted to our hospital were randomly divided into control group and combined group, with 42 cases in each group. Control group was given antiviral drugs, and combined group was given microecologics combined with antiviral therapy. The HBV-DNA level, liver function, liver fibrosis indexes, immune function and inflammatory factors levels were compared between the two groups.
Results
: After treatment, the levels of hepatitis B virus nucleic acid (HBV-DNA) in the two groups were significantly decreased (P<0.05), and the difference of HBV-DNA level before and after treatment in combined group was significantly greater than that in control group (P<0.05), and there was no statistically significant difference in the negative conversion rate of HBV-DNA between the two groups (P<0.05). The levels of liver function indexes [aspartate aminotransferase (AST), alanine aminotransferase (ALT) and total bilirubin (TBIL)] were significantly decreased in the two groups (P<0.05), and the differences of liver function indexes before and after treatment in combined group were significantly greater than those in control group (P<0.05). The levels of liver fibrosis indexes [serum hyaluronic acid (HA), laminin (LN), type III procollagen N-terminal peptide (PIIIP) and type IV collagen (IV-C)] were significantly decreased in the two groups (P<0.05), and the differences of liver fibrosis indexes before and after treatment in combined group were significantly greater than those in control group (P<0.05). The levels of immune function indexes of T cell subsets (CD3+, CD4+ and CD4+/CD8+) in the two groups were significantly increased (P<0.05) while the CD8+ level was significantly decreased (P<0.05), and the differences of immune function indexes before and after treatment in combined group were significantly greater than those in control group (P<0.05). The levels of inflammatory factors [interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), high-sensitive C-reactive protein (hs-CRP)] were significantly decreased in the two groups (P<0.05), and the differences of inflammatory factors before and after treatment in combined group were significantly greater than those in control group (P<0.05).
Conclusion
: Microecologics combined with antiviral therapy has significant effects in improving liver function and immune function and reducing liver fibrosis and inflammatory factors in patients with hepatitis B cirrhosis, and it can improve the efficacy.

Effects of Intravenous Anesthesia with Propofol combined with Remifentanil on Perioperative Immune Function in Patients with Gastric Cancer

Objective
: To investigate the effects of intravenous anesthesia with propofol combined with remifentanil on perioperative immune function in patients with gastric cancer (GC).
Methods
: A total of 157 patients undergoing GC radical surgery who were admitted to the hospital from March 2017 to May 2018 were enrolled in the study. They were numbered according to the treatment order and divided into the control group (intravenous anesthesia with propofol combined with fentanyl, n=78) and the observation group (propofol combined with remifentanil intravenous anesthesia, n=79) by the random number table method. The anesthesia effects, heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP) , T cell subsets (CD3+, CD4+, CD8+) levels and drug safety before and after anesthesia induction [before anesthesia induction (T1), at 60 minutes after anesthesia maintenance (T2), at 10 minutes after extubation (T3)] were compared between the two groups.
Results
: The anesthesia time, duration of eye opening and extubation time of the observation group were significantly shorter than those of the control group (P<0.05). Compared with those at T1, HR, SBP and DBP at T2 and T3 in both groups were significantly increased. At T2 and T3, HR, SBP and DBP in observation group were significantly lower than those in control group (P<0.05). Compared with those at T1, CD3+ and CD4+ at T2 and T3 in both groups were significantly decreased, while CD4+/CD8+ was significantly increased. At T2 and T3, CD3+, CD4+ and CD4+/CD8+ in the observation group were significantly significantly higher than those in the control group (P<0.05). There was no significant difference in total incidence of adverse reactions between the two groups (χ

Conclusion
: Compared with propofol combined with fentanyl intravenous anesthesia, the anesthesia effect of propofol combined with remifentanil intravenous anesthesia is better for patients undergoing GC radical surgery, which can more effectively stabilize patients’ vital signs, and reduce perioperative immunosuppression.

Expressions and Clinical Significances of GATA6 and BMP2 in Oral Squamous Cell Carcinoma

Objective
: To detect the expressions of GATA6 and bone morphogenetic protein (BMP2) in oral squamous cell carcinoma (OSCC) tissues, analyze their relationship and explore their clinical significance.
Methods
: From April 2014 to December 2016, paraffin specimens from 56 patients with oral squamous cell carcinoma confirmed by pathology in our hospital were collected as observation group, and oral mucosa tissues from 42 normal persons were taken as control group. The expressions of GATA6 and BMP2 in oral tissues were detected by immunohistochemistry, and the relationship between GATA6 and BMP2 expression and clinicopathological characteristics of oral squamous cell carcinoma was analyzed. All patients were followed up and the prognostic effects of GATA6 and BMP2 expression on oral squamous cell carcinoma were analyzed.
Results
: The positive expression rates of GATA6 and BMP2 in oral squamous cell carcinoma tissues were significantly higher than those in normal tissues (P < 0.05); the expressions of GATA6 and BMP2 were correlated with TNM stage and tumor differentiation in OSCC patients (P < 0.05), but not with lymph node metastasis and tumor size (P > 0.05); the 2-year survival rate of patients with high expressions of GATA6 and BMP2 in oral squamous cell carcinoma were significantly lower than those in patients with low expression (P < 0.05); Cox model multivariate analysis showed that TNM stage, tumor differentiation, GATA6 and BMP2 expression were independent risk factors affecting the prognosis of patients.
Conclusion
: The expressions of GATA6 and BMP2 in oral squamous cell carcinoma are up-regulated, which is related to clinical stage, tumor differentiation and 2-year survival, it may be used as a biomarker for judging prognosis of patients.

Study on the Correlation between Serum Calcium and Renin-angiotensin-aldosterone System in Patients with Diabetic Nephropathy

Objective
: To explore the correlation between serum calcium and index of in vivo renin-angiotensin-aldosterone system (RAAS) in patients with diabetic nephropathy (DN).
Methods
: 60 patients with type 2 diabetes mellitus (T2DM) admitted between January 2013 and July 2014 were selected for the study. According to the proteinuria level, the patients were divided into observation group 1 (normal proteinuria, n=18), observation group 2 (micro-proteinuria, n=20) and observation group 3 (large proteinuria, n=22), and another 30 healthy people at the same time period for physical examination were selected as control group. The levels of RAAS indexes [serum renin (PRA), angiotensin II (Ang II), aldosterone (ALD)] and serum calcium were compared in each group. The relationship between serum calcium and proteinuria was analyzed by Spearman correlation analysis. Pearman correlation analysis was used to analyze the relationship between PRA, AngII and ALD and serum calcium in DM patients.
Results
: The levels of PRA, AngII and ALD in observation groups 1, 2 and 3 were higher than those in control group (P<0.05), and the levels of PRA, AngII and ALD in observation group 1 were lower than those in observation groups 2 and 3 (P<0.05), and the levels of PRA, AngII and ALD in observation group 2 were lower than those in observation group 3 (P<0.05). The serum calcium level in observation group 1 was higher than that in observation groups 2 and 3 and control group (P<0.05), and the serum calcium level in observation groups 2 and 3 was lower than that in control group (P<0.05), and the serum calcium level in observation group 2 was lower than that in observation group 3 (P<0.05). Spearman correlation analysis showed that the serum calcium level was negatively correlated with urine protein degree of T2DM patients (P<0.05). Pearson correlation analysis showed serum calcium was negatively correlated with PRA, AngII and ALD (P<0.05).
Conclusion
: Serum calcium in DN patients is closely related to RAAS system. Monitoring serum calcium level is beneficial for diagnosis and treatment.

Comparative Study of Efficacy of Trepanation and Drainage and Bone Flap Craniotomy Hematoma Evacuation combined with Decompressive Craniectomy in the Treatment of Aspirin-related Supratentorial Cerebral Hemorrhage

Objective
: To study the efficacy and safety of trepanation and drainage and bone flap craniotomy hematoma evacuation combined with decompressive craniectomy in the treatment of aspirin (ASA)-related supratentorial cerebral hemorrhage (SCH).
Methods
: 84 patients with ASA-related SCH in our hospital from September 2015 to September 2018 were selected and divided into trepanation group and craniotomy group according to the simple randomization method, with 42 cases in each group. Trepanation group was given trepanation and drainage, and craniotomy group was given small bone window craniotomy hematoma evacuation and decompressive craniectomy. The hematoma clearance rate, the NIHSS score, GCS score and GOS score at 24 h after operation, ability of daily living at 6 months after operation, incidence rate of postoperative complications and mortality rate of patients were compared between the two groups.
Results
: The operative time, intraoperative blood loss, drainage time and hospital stay in trepanation group were significantly lower than those in craniotomy group (P<0.05), and there was no significant difference in the hematoma clearance rate between the two groups (P>0.05). At 24h after operation, the NIHSS score in the two groups was significantly decreased (P<0.05) while the scores of GCS and GOS were significantly increased (P<0.05), and the NIHSS score in trepanation group was lower than that in craniotomy group while the scores of GCS and GOS were higher than those in control group (P<0.05). At 6 months after operation, the ability of daily living in trepanation group was significantly better than that in craniotomy group (P<0.05), and the excellent and good rate of ability of daily living in the two groups were 90.24% and 84.21% respectively (P>0.05). The incidence rates of complications in trepanation group and craniotomy group were 21.43% and 19.05% respectively (P>0.05), and the mortality rates were 2.38% and 4.76% respectively (P>0.05).
Conclusion
s: Compared with craniotomy hematoma evacuation combined with decompressive craniectomy, trepanation and puncture drainage has simpler operation, smaller trauma and quicker postoperative recovery, and it can effectively reduce NIHSS score, enhance scores of GCS and GOS and improve postoperative daily living ability, and increase significantly the incidence rate of postoperative complications.

Therapeutic Effect of Early Rehabilitation Combined low Molecular Weight Heparinfor Deep Venous Thrombosis in Patients with Cervical Spinal Cord Injury

Objective
: To study the effect of early rehabilitation combined low molecular weight heparin (LMWH) for deep venous thrombosis (DVT) in patients with cervical spinal cord injury.
Methods
: The clinical data of 113 patients with cervical spinal cord injury admitted from May 2014 to December 2015 were retrospectively analyzed. Among them, 58 patients received early rehabilitation combined with low molecular weight heparin therapy (observation group), and 55 patients received simple Low molecular weight heparin treatment (control group).
Results
: The incidence rate of DVT in the observation group was 1.72%, which was lower than that in the control group (16.36%) (P<0.05). The decreased levels between after treatment and before treatment of D-Dimer and the blood rheology indicators hb, Fib, PAG, MST were all greater in the observation group than in the control group (P<0.05).
Conclusion
: It can prevent the occurrence of deep venous thrombosis with proper early rehabilitation combined low molecular weight heparin for patients with cervical spinal cord injury.

Inhibition of Baicalin on the Growth of Esophageal Cancer Cells and Related Mechanism

Objective
: To investigate the inhibitory effect of baicalin on the growth of esophageal cancer cells andrelated mechanism.
Methods
: ECA109 human esophageal cancer cell line was used for in vitro experiments,cultured cells were treated by thedifferentconcentration of baicalin groups andcontrol group. Cell proliferation was determined by MTT.TUNEL assay was used to determine the effect of baicalin on apoptosis of esophageal cancer cells. The expressions of cIAP- 1 and BAD were detected by Western Blot after adding baicalin 0, 6, 12, 24, 48h.
Results
: Baicalin was found to significantly inhibit the proliferation of ECA109 cells in a dose and time dependent manner.TUNEL experimental Results showed that compared with the control group, the apoptosis rate of baicalin group was significantly increased (P<0.05). The expression of cIAP- 1 was significantly decreased along with the time after adding baicalin, while Bad expression was increased (P<0.05).
Conclusion
: Baicalin can inhibit the proliferation and promote the apoptosis of esophageal cancer cells ECA109, and its mechanism may be related to the up-regulation of Bad protein expression and the inhibition of cIAP-1 protein expression.

Comparative Study of Clinical Efficacy of Autologous Platelet-rich Geland Conventional Dressing Change in Refractory Diabetic Foot

Objective
: To compare the clinical efficacy of autologous platelet-rich gel (APG) and conventional dressing change in refractory diabetic foot (DF).
Methods
: 60 patients with refractory DF admitted to hospital from March 2017 to March 2019 were randomly divided into observation group (standard treatment+APG, n=30) and control group (standard treatment+conventional treatment, n=30). The treatment effects were compared between the two groups.
Results
: After 2 weeks, 4 weeks and 8 weeks of treatment, the ulcer area in the two groups was decreased in turn, and the difference between the two groups was significant (P<0.05), and the ulcer area in observation group after 2 weeks,4 weeks and 8 weeks of treatment was smaller than that in control group(P<0.05). The treatment effects in observation group were significantly better than those in control group(P<0.05). The ulcer healing time and hospital stay in observation group were significantly shorter than those in control group (P<0.05). During treatment, the incidence rate of adverse reactions in observation group was significantly lower than that in control group (P<0.05).
Conclusion
: APG combined with standard therapy can effectively improve the healing effect of ulcer in refractory DF patients.

Effect of Cetirizine Combined with Pinellia Xiexin Decoction on Clinical Efficacy and Serological Indexes in the Treatment of Allergic Rhinitis

Objective
: To study the effect of treatment with cetirizine and Pinellia Xiexin Decoction on the clinical efficacy and serological parameters of allergic rhinitis.
Methods
: The clinical data of 147 patients with allergic rhinitis who were treated in our hospital from March 2016 to October 2017 were selected. All patients were divided into control group (n=73) and study group (n=73) according to the random number table method. = 74 cases). Patients in the control group were given cetirizine tablets orally. Patients in the study group were given cetirizine tablets orally and combined with Banxia Xiexin Decoction. T-cell subsets before and after treatment were compared between the two groups of patients. Interleukin-10 (IL- 10), Cytokines and clinical efficacy of transforming growth factor (TGF-β), tumor necrosis factor-α (TNF-α) and serum total IgE.
Results
: There was no significant difference in T-cell subsets before treatment between the two groups (P>0.05). In the study group, CD4+CD25+ T cells increased significantly after treatment, CD4+ T cells decreased significantly (P<0.05), and CD3+T and CD8+T levels did not change significantly (P>0.05). CD4+ T cells in the control group were significantly lower than before treatment, and CD3+T, CD8+T, and CD4+CD25+ T cells did not change significantly. There was no statistical difference (P>0.05). After treatment, the patients in the study group had statistically significant differences in the levels of CD4+T and CD4+CD25+T compared with the control group (P<0.05). There was no significant difference in serum IgE, TGF-β, IL-10 and TNF-α levels between the two groups before treatment (P>0.05). The levels of IL-10 and TGF-β in the study group were significantly higher than those in the control group, and the levels of TNF-α and IgE were significantly lower than those in the control group. The levels of TGF-β and IL-10 in the two groups after treatment were significantly higher than before treatment, and the levels of TNF-α and IgE were significantly lower than those in the control group, and there were statistical differences (P<0.05). The total effective rate in the control group was 94.52%; the total effective rate in the study group was 95.95%. There was no significant difference in the total effective rate between the two groups (χ
2
=0.627, P>0.05). The recurrence rate was 31.51% in the control group and 10.81% in the study group. The relapse rate in the study group was significantly lower than that in the control group, and there was a statistically significant difference (χ
2
=6.637, P<0.05).
Conclusion
: Cetirizine combined with Banxia Xiexin Decoction has similar effect on alleviating symptoms as cetirizine alone. At the same time, it can promote the normal proportion of different T cell subsets, improve inflammatory cytokines, inflammatory cytokines and IgE content in serum are significantly reduced, reduce the recurrence rate and improve the efficacy of anti-allergic drugs, which can effectively treat allergic rhinitis.

Efficacy of 595 nm Dye Laser combined with Intense Pulsed Light in the treatment of Facial Telangiectasia

Objective
: To explore the efficacy of 595nm pulseddye laser (PDL) combined with intense pulsed light (IPL) on facial telangiectasia.
Methods
: A total of 144 patients with facial telangiectasia treated in our hospital from January 2017 to December 2018 were selected and given simple random grouping by random number table method and divided into groups A, Band C, with 48 cases in each group. Group A was given 595 nm PDL + IPL, and group B was given 595 nm PDL, and group C was treated with IPL. The clinical efficacy, incidence rate of adverse reactions, recurrence rate, pain degree after treatment [visual analogue scale (VAS)] and self-satisfaction were compared among the three groups.
Results
: The total effective rate of treatment and self-satisfaction in the three groups showed group A > group B > group C (P<0.05). There were no significant differences in the incidence rate of adverse reactions, recurrence rate and VAS score among the three groups (P>0.05).
Conclusion
: 595nm PDL has better efficacy than IPL in the treatment of facial telangiectasia. The combination therapy can significantly improve the efficacy of single treatment, and does not increase the incidence rate of adverse reactions and the number of recurrence, and it has higher safety.

The pain score (VAS), knee function score and osteoarthritis index score before and after surgery were compared between the two groups. The changes of medial proximal tibial angle (MPTA) and femoral tibial angle (FTA) before and after surgery were measured in the two groups. The incidence of surgical complications was statistically analyzed.
Results
: The visual analogue scale (VAS) scores in the two groups decreased at 3 months after surgery, and the differences were statistically significant between groups (P<0.05). The Hospital for Special Surgery (HSS) scores in the two groups were increased at 3, 6 and 12 months after surgery while the Western Ontario McMaster Universities (WOMAC) scores were decreased (P<0.05), and there were no statistically significant differences between groups (P>0.05). At 12 months after surgery, FTA was decreased while MPTA was increased in the two groups (P<0.05), and there were no significant differences between groups (P>0.05). There was no significant difference in the incidence rate of complications between the two groups (P>0.05).
Conclusion
: Open medial tibial osteotomy and closed lateral osteotomy can alleviate the pain of patients with medial compartment of knee OA after operation, correct lower limb line of force, and improve joint varus deformity, and the former has relatively few complications.

Effects of CRRT at Different Times on Inflammatory Indicators,Hemodynamics and Prognosis in Patients with Sepsis

Objective
: To study the effects of different timing of continuous renal replacement therapy (CRRT) on inflammation markers, hemodynamics and prognosis in patients with sepsis.
Methods
: The clinical data of 54 patients with sepsis admitted to the hospital from January 2016 to December 2018 were retrospectively analyzed. The patients were divided into simple sepsis group (n=27) and septic shock group (n=27) according to the condition of patients with CRRT therapy. The infection status, acute physiology and chronic health evaluation II (APACHE II), inflammation markers, hemodynamics and prognosis were observed in the two groups before and after treatment.
Results
: The levels of white blood cells (WBC), procalcitonin (PCT) and body temperature in septic shock group before treatment were significantly higher than those in sepsis group (P<0.05). After treatment, the levels of WBC, PCT and body temperature in the two groups were significantly lower than those in the same group before treatment (P<0.05), and the levels of WBC and PCT in septic shock group after treatment were significantly higher than those in sepsis group (P<0.05). Before treatment and after 3d and 7d treatment, the APACHE II scores in the two groups showed a downward trend (P<0.05), and the APACHE II scores in septic shock group were significantly higher than those in sepsis group before treatment and after 3d and 7d of treatment (P<0.05). Before treatment, the levels of tumor necrosis factor-α (TNF-α) and C-reactive protein (CRP) in septic shock group were significantly higher than those in sepsis group (P<0.05), and there was no significant difference in the level of IL-6 between the two groups before treatment (P>0.05). After treatment, the levels of serum TNF-α, CRP and IL-6 in the two groups were significantly lower than those in the same group before treatment (P<0.05), and the levels of TNF-α, CRP and IL-6 in septic shock group after treatment were significantly higher than those in sepsis group (P<0.05). There were no significant differences in the heart rate (HR), mean arterial pressure (MAP) and central venous pressure (CVP) between the two groups before treatment (P>0.05). After treatment, the HR level in sepsis group was significantly lower than that in the same group before treatment (P<0.05), and there were no significant changes in the levels of MAP and CVP (P>0.05), and the HR level in septic shock group after treatment was significantly lower than that in the same group before treatment, and the MAP level was significantly higher than that in the same group before treatment (P<0.05), and there was no significant change in CVP level (P>0.05), and the HR level in septic shock group after treatment was significantly higher than that in sepsis group while the MAP level was significantly lower than that in sepsis group (P<0.05), and there was no significant difference in CVP level between the two groups (P>0.05). The 28d mortality rate in sepsis group after CRRT therapy was significantly lower than that in septic shock group (P<0.05).
Conclusion
: CRRT can effectively eliminate the inflammatory mediators and maintain hemodynamic stability in patients with sepsis and septic shock. Early CRRT intervention can effectively improve the prognosis and prolong the survival time of patients with sepsis.

Effect and Safety of Modified Miccoli Procedure combined with Carbon Nanotracer in Total Thyroidectomy

Objective
: To study curative efficacy of the improved Miccoli surgery combined with nano-carbon tracer in treatment of total thyroidectomy and its effects on safety analysis.
Methods
: 80 patients of thyroid carcinoma who received therapy from January 2016 to February 2018 in our hospital were selected as research objects, according to the random number table, each group of 40 cases, the control group was treated with the improved Miccoli surgery, while the observation group was combined with nano-carbon tracer injection on the basis of the control group .postoperative indicators, lymph node dissection, lymph node metastasis rate, misresection rate of parathyroid gland, parathyroid hormone, changes of serum calcium and postoperative complications were compared between the two groups.
Results
: There was no significant difference in the operative time, intraoperative bleeding volume, length of stay between the two groups(P>0.05). The number of lymph node dissection in the observation group was more than the control group(P<0.05). The misresection rate of parathyroid gland in the observation group was lower than those in the control group(P<0.05). There was interaction between parathyroid hormone and blood calcium in the two groups. (P<0.05). The postoperative complications in the observation group was lower than those in the control group(P<0.05).
Conclusion
: The modified Miccoli procedure combined with nano-carbon tracer can alleviate thyroid injury and is safe and effective.

Study on the Correlation between Ultrasonographic Characteristics of Carotid Atherosclerotic Plaques and Coronary Heart Disease by Color Doppler Ultrasound

Objective
: To explore ultrasonic features of carotidatherosclerosis plaques explored by color Doppler ultrasound, and to further analyze their relationship with coronary heart disease (CHD).
Methods
: A retrospective analysis was made of the imaging data of 83 patients with coronary heart disease who underwent color Doppler ultrasound examination in our hospital from April 2016 to April 2018 and 79 healthy persons who underwent physical examination at the same time. They were divided into coronary heart disease group and healthy group respectively. All the participants in the study received color Doppler ultrasound examination. The IMT value, carotid plaque status and hemodynamic parameters were compared between the two groups.
Results
: IMT valuesof carotidjunction, internal carotid artery and common carotid artery in CHD group were higher than those in healthy group. There were significant differences in IMT values between the two groups (P<0.05). The detection rateof plaquesin CHD group was higher than that in healthy group (85.54%vs 11.39%)(P<0.05),and plaque score was significantly higher than that in healthy group (P<0.05). PSV and EDV in CHD group were lower than those in healthy group, while PI and RI were higher than those in healthy group.There were significant differences incarotid hemodynamic parameters between the two groups (P<0.05).
Conclusion
: The ultrasonographic features of carotid atherosclerosis plaque and blood flow detected by color Doppler ultrasonography are correlated with coronary heart disease, which has reference significance for the diagnosis of coronary heart disease.

Clinical Efficacy of Mucopolysaccharide Polysulfate Cream combined with Triamcinolone Acetonide and Econazole Cream in the treatment of Chronic Eczema

Objective
: To explore the clinical efficacy of mucopolysaccharide polysulfate cream combined with triamcinolone acetonide and econazole cream in the treatment of chronic systemic eczema.
Methods
: 82 patients with chronic systemic eczema were randomly grouped by 1:1ratio, each 41 cases in group B and group A. They were given oral loratadine, and group B was treated with triamcinolone acetonide and econazole cream, and group A was given mucopolysaccharide polysulfate cream on the basis of treatment in group B. The symptoms and signs scores before treatment and after 4w of treatment were observed in the two groups. The efficacy of the two groups was evaluated. The Results of non-invasive skin physiological tests and levels of serum chemokines CCL17 and CCL18 were compared before treatment and after 4w of treatment.
Results
: After treatment, the scores of four aspects of conscious pruritus, skin thickness, skin softness and rash recovery and total score were significantly decreased in the two groups (P<0.05), and the decreases in group A were significantly greater than those in group B (P<0.05). The total clinical effective rate in group A was significantly higher than that in group B (87.80% vs 68.29%). After treatment, the transepidermal water loss (TEWL) values in the two groups were significantly reduced (P<0.05), and the cuticle water content of skin lesions was increased significantly (P<0.05), and the OD values of chemokines CCL17 and CCL22 at skin lesion tissues in the two groups were significantly decreased (P<0.05), and the changes in group A were significantly greater than those in group B (P<0.05).
Conclusion
s: Mucopolysaccharide polysulfate cream combined with triamcinolone acetonide and econazole cream has significant efficacy in the treatment of chronic systemic eczema. And it can effectively improve the symptoms and signs of rash, effectively maintain skin moisture, repair skin barrier function and promote the regeneration of damaged skin tissue.

Clinical Effect of Posterior Transmuscular Pedicle Screw Internal Fixation and Traditional Open Pedicle Screw Internal Fixation in the treatment of Thoracolumbar Fracture and Dislocation Injury

Objective
: To explore the clinical efficacy of posterior transforaminal pedicle screw fixation and conventional open pedicle screw fixation on thoracolumbar fracture-dislocation injury.
Methods
: A total of 100 patients with thoracolumbar fracture-dislocation injury who underwent elective surgery were divided into observation group(posterior transforaminal pedicle screw fixation,n=45) and control group(traditional open pedicle screw fixation, n=55) according to the surgical procedures. The operation conditions and pain degree, orthopedic imaging indexes changes and Oswestry dysfunction index score (ODI) before operation, at 24h after operation and at the last follow-up were recorded and compared between the two groups.
Results
: There was no significant difference in the operative time between the two groups (P>0.05). The intraoperative blood loss and postoperative drainage volume in observation group were significantly less than those in control group, and the hospital stay was significantly shorter than that in control group (P<0.05). The VAS scores in observation group were significantly lower than those in control group at 24h after operation and at the last follow-up(P<0.05).There were no statistical differences in the orthopedic imaging indexes between the two groups before operation and at the last follow-up (P>0.05). At the last follow-up, the change range of ODI score in the observation group was larger than that in the traditional group (P<0.05).
Conclusion
: Posterior transmuscular pedicle screw fixation has less bleeding and less trauma, which can reduce incision pain and promote functional recovery.

A Method for Rapid Determination of Anionic Synthetic Detergent in Natural Mineral Water for Drinking

Objective
: To establish a rapid method for the determination of anionic synthetic detergents in natural mineral water.
Methods
: Accurately draw 20mL of water sample to be tested into 25mL plug colorimetric tube, add 4 mL of methylene blue buffer solution, mix well, add 4 mL of chloroform, and close the lid. The mixture was shaken continuously for 120 times (45 S), and then poured into a 100 ml separatory funnel, standing and stratified. A small mass of absorbent cotton was push into the neck of the separatory funnel, The absorbance of chloroform phase was measured at a wavelength of 650 nm using the absorbance of chloroform as a reference.
Results
:The linear relationship between anionic synthetic detergents in natural mineral water was quickly determined by optimized methylene blue spectrophotometry with good Results. The correlation coefficient (r) was 0.9998, the lowest detection concentration was 0.02 mg/L, and the samples were low, medium and high. The spiked concentration test was carried out at a concentration level of 97.4% to 106.3%, and the precision was 0.68% to 2.34% (n=6). The certified standard materials were determined and the Results were satisfactory.
Conclusion
: This test method is easy to operate, has good precision, high accuracy and low detection limit. It is suitable for the detection of anionic synthetic detergents in all kinds of drinking natural mineral water.
