1585 Serum Levels of Histone Deacetylase 2 Interleukin-1 beta Interleukin-6 and Interleukin-17A in Patients with COPD and Clinical Significance

Objective: To explore the plasma histone acetylation enzyme 2, interleukin 1, interleukin 6 and interleukin 17A levles of patients with COPD and the clinical significance. Methods: 64 COPD patients were retrospectively chosen as the observation group in our respiratory intensive care unit from June 2016 to June 2018. While, 64 cases of normal physical health people were selected as the control group. Serum HDAC2, IL-1β, IL-6 and IL-17A levels were determined by ELISA. FEV1/FVC% and FEV1% pred values were detected by the lung function detector. Pearson correlation analysis was performed to explore the correlation between serum HDAC2, IL-1β, IL-6 and IL-17A levels and lung function. Results: Compared with the control group, serum HDAC2, IL-1β, IL-6 and IL-17A levels were significantly increased in the observation group (P<0.05). Serum HDAC2, IL-1β, IL-6 and IL-17A levels of observation group were gradually increased with the lung function grade increase (P<0.05). FEV1/FVC%,FEV1% pred of COPD patients with different levels of lung function decreased gradually with the increase of grade, and the comparison difference results were statistically significant (P<0.05). Pearson correlation analysis showed that serum HDAC2 level was positively correlated with lung function. While, IL-1β, IL-6 and IL-17A levels were negatively correlated with lung function FEV1/FVC% and FEV1% pred (P<0.05). Conclusion: The expression of histone deacetylase 2 was decreased, IL-1β, IL-6 and IL-17A levels of COPD patients were significant increased. The inflammation of patient airway and alveolar tissue were activated, which might be one of the main factors of progressive decline of lung function.

1589 Effect of Aripiprazole Combined with Sertraline on Obsessive-compulsive Disorder and Effects of Brain-derived Neurotrophic Factor TNF and IL-2

Objective: To study the therapeutic effect of aripiprazole combined with sertraline on obsessive-compulsive disorder and effects of brain-derived neurotrophic factor, tumor necrosis factor (TNF) and interleukin-2 (IL-2). Methods: 82 received therapy from March 2017 to March 2018 in our hospital were selected as research objects. According to random number table,all the patients were divided into the observation group (n=42) and the control group (n=40). The control group was treated with Sertraline, while the observation group treated with Aripiprazole combined with sertraline. Then the Therapeutic efficacy, Yale Brown Obsessive-compulsive Disorder Scale (Y-BOCS) and Hamilton Anxiety Scale (HAMA) score, brain-derived neurotrophic factor, TNF, IL-2 level, incidence of adverse reactions of two groups after treatment were compared. Results: After treatment, The total effective rate of observation group was significantly higher than that of control group[92.85%(39/42)vs 70.00%(28/40)](P<0.05).Y-BOCS and HAMA scores were significantly lower than those of the control group [(9.86±3.13)scores vs(14.42±4.52)scores,(7.68±2.10)scores vs(12.06±3.09)scores](P<0.05). The levels of brain-derived neurotrophic factor and TNF were significantly higher than those of the control group [(40.02±5.87)μg/L vs(29.76±4.95)μg/L(38.49±8.98)μg/L vs(34.09±7.34)μg/L](P<0.05). The level of IL-2 was significantly lower than that of the control group[(50.32±6.28)ng/L vs(62.93±7.59)ng/L](P<0.05). Conclusion: Aripiprazole combined with sertraline is effective in the treatment of obsessive-compulsive disorder. It can improve the clinical symptoms, regulate the secretion of neurotransmitters in the brain, and improve the levels of brain-derived neurotrophic factor, TNF and IL-2. It is safe and reliable.

1593 Analysis of Risk Factors for Postoperative Respiratory Failure in Esophageal Cancer and Construction of Predictive Model

Objective: To investigate the risk factors of postoperative respiratory failure and the construction of predictive models for esophageal cancer. Methods: 114 patients underwent radical esophageal cancer surgery in the ICU of our hospital from Jan. 2015 to Dec. 2018 were enrolled. The follow-up was divided into two groups according to arterial blood gas analysis: respiratory failure and non-respiratory failure. Risk factors for respiratory failure after radical resection of esophageal cancer. The regression equation for predicting respiratory failure after radical resection of esophageal cancer was further obtained, and the ROC curve was used to evaluate the optimal diagnostic cutoff value and diagnostic efficacy of the predictive model. Results: Of the 114 patients who underwent radical esophageal cancer in the ICU of our hospital, 78 had respiratory failure and 36 had no respiratory failure. Gender, age, height, hyperlipidemia, body weight, FEV1, hypertension, and high There were no significant differences in blood glucose surgery anastomosis, preoperative infection, operation time and other surgical complications (P>0.05). The BMI, smoking index, lung function damage, intraoperative blood loss, lung surgery history and The postoperative mechanical ventilation time was statistically significant (P<0.05). The correlations between smoking index, intraoperative blood loss, lung surgery history and postoperative mechanical ventilation time were 35.427, 1.13, 106.661 and 3.21, respectively. Post-respiratory failure is highly correlated. The probabilistic predictive model of postoperative respiratory failure in esophageal cancer was P=1/[1+e (45.947-3.567 smoking index-0.122 intraoperative blood loss-4.67 lung surgery history-1.166 mechanical ventilation time)], new variable P the AUC is 0.994, which is higher than other variables, indicating that its diagnostic performance is the best. According to the maximum value of the Youden index, the critical value of the new variable P is 0.391. The sensitivity at this time is 96.70% and the specificity is 95.80%. Conclusion: Estimating the incidence of respiratory failure after radical resection of esophageal cancer combined with multiple independent risk factors can improve the diagnostic efficiency of respiratory failure after radical resection of esophageal cancer, improve its diagnostic sensitivity and specificity, and have high clinical value.

1598 Expression and Clinical Significance of VEGFp-Akt and PTEN in Non-small Cell Lung Cancer

Objective: To detect the expression of PTEN, p-Akt and VEGF protein in tissue of non small-cell lung cancer group and normal control groups, and discuss the mechanism of these three proteins in the pathogenesis of non-small cell lung cancer. Methods: 82 cases of tissue samples with non small-cell lung cancer and fifty eight cases of tissue samples from alveolus health control, and fifty six cases of tissue samples from bronchus health control were collected. The expression levels of them was measured by immunohistochemistry method and analyzed the relationship with the clinical and pathological data. Results: The expression of p-Akt and VEGF protein in lung cancer group, squamous cell carcinoma of the lung group and lung adenocarcinoma group was significantly higher than which in double normal control groups(P<0.01).p-Akt and VEGF expression in lung tissue were positively correlated. The expression levels of p-Akt protein were related to tumor size, differentiation degree, clinical stage and lymph node metastasis. The expression levels of VEGF protein were related to differentiation degree , clinical stage and lymph node metastasis. The expression levels of PTEN protein in lung cancer group, squamous cell carcinoma of the lung group and lung adenocarcinoma group is significantly lower than which in double normal control groups(P<0.01). The expression levels of PTEN protein were related to differentiation degree, clinical stage and lymph node metastasis. PTEN is negatively correlated with p-Akt and VEGF in tumor tissues. Conclusion: P-Akt and vascular endothelial growth factor play a synergistic role in the occurrence, invasion and metastasis of non-small cell lung cancer, and PTEN plays an inhibitory role.

1604 Effects of Foley Balloon combined with Oxytocin Intravenous Infusion on Stages of Labor and Maternal-infant Outcomes of Primiparae with Full-term Pregnancy

Objective: To explore the effects of Foley balloon combined with oxytocin intravenous infusion on stages of labor and maternal-infant outcomes of primiparae with full-term pregnancy. Methods: 190 primiparae with full-term pregnancy and indications of induction of labor were randomly assigned to control group and observation group according to the coin toss method, with 95 cases in each group. Control group was given intravenous infusion of oxytocin, and observation group was given Foley balloon combined with oxytocin intravenous infusion. The cervical maturity (Bishop) score was observed in the two groups before induction and after 6h and 12h of intervention, and the stages of labor, maternal-infant outcomes and adverse reactions were compared between the two groups. Results: After 6h and 12h of intervention, the Bishop scores in the two groups were significantly increased (P<0.05), and were higher than those before induction (P<0.05), and the scores in the two groups after 12h of intervention were significantly higher than those after 6h of intervention (P<0.05), and the change in observation group was significantly higher than that in control group (P<0.05). The latency and active phase in first stage of labor and total labor time in observation group were significantly shorter than those in control group (P<0.05), and the postpartum fatigue score was significantly lower than that in control group (P<0.05). The success rate of first induction and vaginal delivery rate in observation group were significantly higher than those in control group [(63.16% and 88.42%) vs (43.16% and 78.95%)] (P<0.05), and the cesarean section rate and incidence rates of postpartum hemorrhage, soft birth canal tear and neonatal asphyxia were significantly lower than those in control group [(11.58%, 0.00%, 1.05% and 1.05%) vs (21.05%, 5.26%, 6.32% and 5.26%)] (P<0.05). The total incidence rate of adverse reactions in observation group was significantly lower than that in control group (4.21% vs 8.42%) (P<0.05). Conclusion: Foley balloon combined with oxytocin intravenous drip can effectively promote cervical ripening of full-term pregnant primipara, shorten their labor process, effectively improve the success rate of first induced labor and vaginal delivery rate, improve maternal and infant outcomes, and have good clinical safety.

1608 Changes and Clinical Significance of Serum Osteopontin Levels in Type 2 Diabetic Patients with Microangiopathy

Objective: To analyze the change of osteopontin (OPN) in type 2 diabetic (T2DM) patients with microangiopathy. Methods: 265 patients with type 2 diabetes mellitus (T2DM) were divided into simple type 2 diabetes mellitus (group A) 152 cases and microangiopathy group (group B) 113 cases. Eighty healthy volunteers who came to our hospital for physical examination were selected as control group (group N). The differences of OPN among the three groups were compared. The difference of serum OPN level between non-diabetic nephropathy group (NDN group) and diabetic nephropathy group (DN group) was compared. The difference of serum OPN level between non-diabetic retinopathy group (NDR group), non-proliferative phase group (NPDR group) and proliferative phase group (PDR group) was compared. Results: The level of OPN in group B was significantly higher than that in group A and group N (P < 0.05). The level of OPN in DN group was higher than that in NDN group, NPDR group was higher than that in NDR group, and PDR group was higher than that in NPDR group, with statistical significance (P < 0.05). Bivariate Logstic regression analysis showed that OPN was an independent risk factor for microangiopathy in patients with T2DM. ROC curve was used to evaluate the diagnostic value of serum OPN level in type 2 diabetes mellitus with microangiopathy, which was moderately predictive (AUC 0.787). Conclusion: Serum OPN level is correlated with the occurrence and severity of microangiopathy in patients with T2DM.

1612 The Relationships Between Preoperative Fibrinogen and D-dimer Levels with Prognosis in Patients with Debakey Type I Aortic Dissection

Objective: To detect the expressions of fibrinogen (FIB) and D-dimer (D-D) in patients with Debakey type I aortic dissection before operation, and to analyze their relationships with adverse prognosis. Methods: 56 patients with Debakey type I aortic dissection admitted to our hospital from January 2015 to December 2017 were retrospectively selected as the study subjects, according to whether death occurred one year after operation, they were divided into 41 cases in survival group and 15 cases in death group, another 40 healthy volunteers were selected as control group, the basic clinical data of the subjects were collected, preoperative plasma FIB and D-D levels were measured by Automatic Coagulation analyzer. Results: Preoperative CRP, SBP, DBP, WBC, PLT, FIB, and D-D levels in the death group were significantly higher than those in the survival group and the control group (P < 0.05), and the survival group were significantly higher than the control group (P < 0.05). The AUC of plasma FIB in predicting the adverse prognosis of Debakey type I aortic dissection was 0.893, sensitivity was 86.70%, and specificity was 85.40%; the AUC of D-D in predicting the adverse prognosis of Debakey type I aortic dissection was 0.810, sensitivity was 80.00%, and specificity was 75.60%; the AUC of FIB combined D-D in predicting the adverse prognosis of Debakey type I aortic dissection was 0.951, sensitivity was 93.30%, and specificity was 97.90%; FIB and D-D levels were independent risk factors for adverse prognosis in patients with Debakey type I aortic dissection (P < 0.05). Conclusion: FIB and D-D are highly expressed in plasma of patients with Debakey type I aortic dissection before operation, it is an independent risk factor for adverse prognosis, the predictive value of the two combined detection for adverse prognosis is higher than that of single index detections.

1616 Relationship Between the Serum Levels of IL-33 & IL-35 and Cardiopulmonary Function in Patients with Stable COPD

Objective: To investigate the relationship between the serum levels of IL-33 & IL-35 and cardiopulmonary function in patients with stable chronic obstructive pulmonary disease(COPD). Methods: 77 patients with COPD in Department of Respiratory Medicine of Danzhou People's Hospital from March 2017 and September 2018 were divided into stable chronic obstructive pulmonary disease group(n=46) and acute exacerbation chronic obstructive pulmonary disease group (n=31) according to the clinical diagnosis, and 40 normal subjects who underwent a physical examination during this period were included as normal group. Serum IL-33 and serum IL-35 were detected by enzyme-linked immunosorbent assay (ELISA). Results: Serum IL-33 levels in the case group were significantly higher than those in the normal group, and the acute exacerbation chronic obstructive pulmonary disease group was significantly higher than the stable chronic obstructive pulmonary disease group, and the difference was statistically significant (P<0.05). The serum IL-35 level in the case group was significantly lower than that in the normal group, and the acute exacerbation chronic obstructive pulmonary disease group was significantly lower than the stable chronic obstructive pulmonary disease group, and the difference was statistically significant (P<0.05). In the case group, the serum IL-33 level of patients with cardiopulmonary function I-II was significantly lower than that of patients with III-IV(P<0.05), and the serum IL-35 level was significantly higher than that of patients with III-IV(P<0.05). Serum IL-33 was negatively correlated with cardiopulmonary function (P<0.05), and serum IL-35 was positively correlated with cardiopulmonary function in Case group (P<0.05). Conclusions: The serum level of IL-33 in patients with stable chronic obstructive pulmonary disease is lower than that of patients with acute exacerbation COPD, while the serum level of IL-35 is higher. Serum IL-33 is negatively correlated with cardiopulmonary function, while serum IL-35 is positively correlated with cardiopulmonary function. The both have a certain influence on the clinical symptoms of patients with stable chronic obstructive pulmonary disease.

1620 Study on the Mechanism of siRNA Silencing BRM Enhancing the Sensitivity of Pancreatic Cancer Cells to Gemcitabine

Objective: To investigate enhancement of silencing BRM gene by siRNA on the sensitivity of human pancreatic cancer cell line to gemcitabine. Methods: Panc-1 cells from human pancreatic cancer in vitro were cultured. Negative control NC and BRM siRNA were transfected into Panc-1 cells cultured in vitro. The expression of BRM in Panc-1 cells was detected by RT-PCR and Western Blot, so as to identify the silencing effect of BRM-specific siRNA. Different concentrations of gemcitabine were used to detect the cell proliferation rate, and the half-inhibitory concentration (IC50) was calculated. The apoptosis rate of Panc-1 cells and the protein expression levels of JAK2, STAT3, p-STAT3 and Survivin in Panc-1 cells were detected. Results: The expression of BRM mRNA and protein in Panc-1 cells of BRM siRNA group was significantly lower than that of blank control group and NC control group (P<0.05). The inhibition rate of gemcitabine on Panc-1 cell proliferation in BRM siRNA group was higher than that in blank control group and NC control group (P<0.05); The IC50 of the blank control group, the NC control group and the BRM siRNA group were 5.18 ug/mL, 5.05 ug/mL and 3.05 ug/mL, respectively; After gemcitabine treatment with the final concentration of 3.0ug/mL, the apoptotic rate of Panc-1 cells in BRM siRNA group was significantly higher than that in blank control group and NC control group (P<0.05), STAT3 was not significantly changed (P>0.05), while the expression of JAK2, p-STAT3 and Survivin protein was significantly decreased (P<0.05). Conclusion: Silencing BRM expression by siRNA can increase the sensitivity of Panc-1 cells to citrate, which may be related to the activation of JAK2/STAT3 pathway by BRM to promote pancreatic cancer growth and drug resistance.

1624 Efficacy of Bisoprolol Combined with Fordostein in the Treatment of COPD and the Effect on Serum CC16 sE-SLT and SAA

Objective: To study the efficacy of Bisoprolol Combined with Fordostein in the treatment of COPD and its effect on serum human clara cell protein (CC16), soluble E-selectin (sE-SLT) and serum amyloid A (SAA). Methods: 81 patients of COPD who received therapy from March 2017 to March 2018 in our hospital were selected as research objects. According to random number table,those patients were divided into observation group (41 cases) and control group (39 cases) by random number table, Routine treatment was given in both groups,The patients in the observation group were treated with bisoprolol combined with fordostein, fordostein 0.4g, three times a day.Patients in the control group were treated with Bisoprolol. The course of treatment was 3 months in both groups. Then Serum CC16, sE-SLT, SAA, efficacy and adverse reactions were compared between the two groups. Results: The total effective rate of the observation group was 92.85% (3/42). The total effective rate of the control group was 66.66% (13/39). The difference has statistical significance(P<0.05). CC16 in the observation group and the control group were (0.50±0.02)and (0.85±0.05), respectively. sE-SLT in the observation group and the control group were(17.67±2.78)and(22.08±4.54), respectively. SAA in the observation group and the control group were(34.84±2.37)and(46.09±4.46), respectively, The difference has statistical significance(P<0.05). During the treatment, there were 1 case of wheezing, 1 case of dizziness and 1 case of fatigue in the observation group.There were 2 cases of wheezing, 3 cases of dizziness and 2 cases of fatigue in the control group. The total incidence of adverse reactions in the two groups was 7.14% (3/42) and 17.94% (7/39), respectively. There was no significant difference between the two groups(P>0.05). Conclusion: Bisoprolol combined with Fordostein is effective in the treatment of COPD. It can improve clinical symptoms, cardiopulmonary function, bronchiectasis, reduce serum levels of CC16, sE-SLT and SAA, with fewer adverse reactions, and is safe and reliable.

1627 Efficacy of Clove Oil Cement combined with Minocycline on Acute Retrograde Pulpitis and The Effect of Inflammatory Factors

Objective: To study effect of Clove oil cement combined with minocycline on Acute retrograde pulpitis and the effect of inflammatory factors. Methods: 84 reversible pulpitis admitted from March 2017 to April 2018 in our hospital were selected as research objects. According to random number table, those patients were divided into the observation group (n=45) and the control group (n=39). The control group was treated with Clove oil cement, while the observation group treated with clove oil combined with minocycline. Then the Therapeutic efficacy, tooth mobility, pain score and pain relief time, levels of Leukotriene, tumor necrosis factor, interleukin 8 in gingival crevicular fluid, and incidence of adverse reactions of two groups after treatment were compared. Results: After treatment, The total remission rate in the observation group was significantly higher than that in the control group[93.33%(42/45)vs69.23%(27/39)](P < 0.05). The degree of tooth loosening, the percussion score were significantly lower than those of the control group[(0.82±0.31)branch vs(1.56±0.40)branch,(1.16±0.34)branch vs(2.03±0.56)branch](P<0.05).The levels of LTB4、TNF-α、IL-8 were significantly lower than those in control group[(15.97±3.76)pg/mL vs(26.49±6.21)pg/mL,(21.02±5.57)mg/L vs(30.27±7.30)mg/L,(7.34±1.39)pg/mL vs(12.90±3.28)pg/mL](P<0.05). The total incidence of adverse reactions was significantly lower than that of the control group[6.66%(3/45)vs25.64%(10/39)](P<0.05). Conclusion: Clove oil hydrating combined with minocycline has a significant effect on retrograde pulpitis. It can improve clinical symptoms, relieve pain, reduce the levels of inflammatory factors LTB4, TNF-alpha and IL-8, with fewer complications and is safe and reliable.

1631 Screening Value of Serum Sex Hormones PRL FSH LH T and E2 for Non-chimeric Klinefelter Syndrome

Objective: To analyze the screening value of serum sex hormones pituitrin (PRL), follicle-stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T) and estradiol (E2) for non-chimeric klinefelter syndrome. Methods: 140 cases of non-chinmeric klinefelter syndrome patients who treated from January 2013 to January 2018 in our hospital, and 98 cases of male infertility with normal chromosome karyotype at the same time were randomly selected as the control group. The levels of serum sex hormones PRL, FSH, LH, T and E2 in both groups were compared, and analyze the screening value of the ROC curve for each indicator. Results: Levels of PRL, FSH, LH and E2 in non-chimeric klinefelter syndrome group were higher in the than the control group (11.34±1.90)vs(8.01±1.14)ng/mL,(25.71±4.30)vs(9.60±1.57)mIU/mL,(23.18±3.21)vs(5.79±0.85)mIU/mL,(22.09±3.86)vs(18.43±2.40)pg/mL, and levels of T in non-chimeric patients group was lower than those in the control group (all P<0.05). The ROC curve shows that the area under the curve of PRL and FSH, LH, T and E2 was 0.725, 0.690, 0.656, 0.774 and 0.663, the area under the combined measurement curve was significantly higher than that of a single index (P<0.05). Conclusion: serum sex hormone PRL, FSH, LH, T and E2 levels have certain diagnostic value for non-chimeric klinefelter syndrome and can be used as a preliminary screening index for such diseases, providing a basis for clinical diagnosis.

1634 Evaluation of Elastic Modulus of Quadriceps Femoris in Female Patients with Knee Osteoarthritis Using Supersonic Shear Imaging

Objective: To explore the value of real-time ultrasonic shear wave elastic imaging in evaluating the biological properties of quadriceps muscle in patients with knee arthritis by observing the changes of the elastic modulus of shear wave under the ultrasound of those patients’ quadriceps muscle. Methods: 40 female patients diagnosed with knee osteoarthritis (observation group) and 40 healthy female volunteers (control group) were examined by ultrasonic shear wave elastic imaging. The lower extremity muscle chain training treatment was given to the patients belong to the observation group, and ultrasonic shear wave elastic imaging was examined before and 3 months after treatment. The difference of elastic modulus of femoral rectus muscle, medial femoral muscle and lateral femoral muscle before and after treatment was compared between the observation group and the control group. After that, the hardness changes of lower extremity muscle in female knee osteoarthritis patients were quantitatively analyzed. Results: In observation group before treatment the mean elastic modules of femoral rectus muscle (7.03±1.07) kpa, medial femoral muscle (6.87±1.64) kpa, lateral femoral muscle (7.00±1.23) kpa, were lower than that rectus muscle (9.07±1.52) kpa, medial femoral muscle (7.96± 0.94) kpa, lateral femoral muscle (8.33±1.10) kpa in the control group, the difference is statistically significant (P<0.01).After treatment the mean elastic modules of femoral rectus muscle (8.17±1.00) kpa, medial femoral muscle (7.35±1.11) kpa, lateral femoral muscle (7.80±0.89) kpa in the observation group were higher than that before treatment, and the comparison was statistically significant (P<0.01 or P<0.05). Conclusion: The real-time ultrasonic shear wave elastic imaging technique could be used to quantitatively evaluate the biomechanical properties of knee arthritis patients, which supplements the new observation basis for the diagnosis and efficacy evaluation of knee osteoarthritis. It has high clinical application value and research prospect.

1638 Effect of Somatostatin combined with Lansoprazole on Serum Gastrin, Inflammatory Factors and Adverse Reactions in Patients with Peptic Ulcer

Objective: To investigate the effect of somatostatin combined with lansoprazole on serum gastrin, inflammatory factors and adverse reactions in patients with peptic ulcer (PU). Methods: 126 patients with PU in the hospital were selected as the subjects by simple random sampling method, and they were randomly divided into the control group and the observation group, 63 cases in each group. All patients were given basic treatments after admission. On the basis, the control group was treated with lansoprazole while the observation group was treated with somatostatin combined with lansoprazole. After 48h of medication, serum gastrin, motilin, somatostatin and other gastrointestinal hormones were compared between the two groups before and after treatment. The inflammatory factor, epidermal growth factor (EGF) and transforming growth factor alpha (TGF-ɑ) were detected, and adverse reactions in the two groups were statistically analyzed. Results: After treatment, levels of gastrin, motilin and somatostatin in the two groups were significantly lower than those before treatment, and the levels in the control group [(86.5±12.7) μmol/L, (208.4±31.2) pg/mL, (35.2±5.2) pg/mL] were significantly higher than those in the observation group [(71.3±10.6) μmol/L, (152.6±23.4) pg/mL, (27.9±4.1) pg/mL] (P<0.05). After treatment, inflammatory factors CRP and IL-6 in the two groups were decreased significantly, without significant difference between groups (P>0.05). Levels of EGF and TGF-ɑ in the two groups were significantly higher than those before treatment, and the levels in control group [(0.62±0.13) pg/mL, (2.45±0.47) pg/mL] were significantly lower than those in the observation group [(0.84±0.15) pg/mL, (2.63±0.51) pg/mL] (P<0.05). The total response rate of the control group was significantly lower than that of the observation group (66.7% vs 82.5%) (P<0.05), but there was no significant difference in the total incidence of adverse reactions between the two groups (15.9% vs 7.9%) (P>0.05). Conclusion: Somatostatin combined with lansoprazole can effectively relieve clinical symptoms of PU and promote the recovery of gastrointestinal function, without increasing adverse reactions and with high safety.

1642 Effects of Ropivacaine-Containing Expansion Fluid on Stress Response and Postoperative Pain in Endoscopic Thyroid Surgery Patients under General Anesthesia with Tracheal Intubation

Objective: To analyze the positive effects of ropivacaine-containing inflation fluid on stress response and postoperative pain in patients undergoing endoscopic thyroidectomy under tracheal intubation and general anesthesia. Methods: 72 patients underwent endoscopic thyroidectomy under tracheal intubation and general anesthesia were randomly divided into study group (n=36) and control group (n=36). anesthesia They were subcutaneously injected with inflation fluid for auxiliary anesthesia. The inflation fluid of the control group was normal saline. The inflation fluid of the study group was 20 ml ropivacaine hydrochloride at 0.75% concentration and normal saline. The concentrations of stress indexes such as MAP, HR and plasma cortisol (Cor) of the two groups before induction (T0), before injection of inflation fluid (T1), immediately after the end of surgery (T2) and at extraction time of tracheal catheterization (T3) were recorded. Postoperative pain was assessed by the related scores at 10 min after surgery (T4), 30 min after surgery (T5) and 12h after surgery (T6). The incidence of adverse events during the perioperative period was statistically analyzed. Results: MAP, HR and Cor in both groups were significantly increased from T0 to T2 in turn, and they significantly decreased at T3 (P<0.05). There was no significant difference in MAP, HR or Cor at T0 and T1 between the two groups (P>0.05). MAP, HR and Cor in the study group were significantly lower than those in the control group at T2 and T3 (P<0.05). The VAS score and Ramsay sedation score at T5 were significantly lower than those at T4 in the two groups (P<0.05). The VAS score and Ramsay sedation score in the study group were significantly lower than those in the control group at T4 and T5. The BCS comfort score was significantly lower than that in the control group at T5 (P<0.05). There was no significant difference in the incidence of adverse reactions during perioperative period between the study group and the control group (22.22% vs 16.67%) (P>0.05). Conclusion: The swelling solution containing ropivacaine hydrochloride can significantly alleviate the stress reaction caused by endoscopic thyroidectomy, provide satisfactory analgesic effect and improve the comfort of patients.

1647 Effects of Nifedipine Sustained Release Tablets Combined with Cinepazide Maleate on Lipid Level, Cardiac Function and Endothelial Diastolic Function in Elderly Patients with Coronary Heart Disease Complicated with Hypertension

Objective: To investigate the effects of nifedipine sustained-release tablets combined with cinepazide maleate on blood lipid levels, cardiac function and endothelium dilation function in elderly patients with coronary heart disease and hypertension. Methods: 102 elderly patients with coronary heart disease and hypertension were divided into observation group and control group by random number table method. The control group was given irbesartan hydrochlorothiazide dispersible tablets and nitrendipine tablets for oral administration. The observation group was given nifedipine sustained-release tablets for oral administration cinepazide maleate injection therapy. The 24h mean systolic blood pressure (24hmSBP), 24h diastolic blood pressure (24hmDBP), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C) levels, left ventricular ejection fraction (LVEF), left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), endothelium-dependent dilation (EDD) and non-endothelium dependent dilatatio (NEDD) were measured before and after treatment. The antihypertensive effect was evaluated. The occurrence of adverse cardiovascular events was statistically analyzed. Results: The total antihypertensive response rates in the observation group and the control group were 96.08% and 88.24%, respectively. There was no significant difference between the two groups (P>0.05). After treatment, 24hmSBP, 24hmDBP, TC, TG and LDL-C were significantly decreased (P<0.05), while HDL-C level was increased. The increase or decrease in the observation group was greater than that in the control group (P<0.05). After treatment, LVEF, EDD AND NEDD in the observation group were significantly higher than those in the control group (P<0.05), while LVEDD and LVESD were significantly lower than thosein the control group (P<0.05). The incidence of adverse cardiovascular events during treatment period in the observation group was significantly lower than that in the control group (P<0.05). Conclusion: Nifedipine sustained-release tablets combined with cinepazide maleate can improve endothelial diastolic function, cardiac function and blood lipid levels in elderly patients with coronary heart disease and hypertension, with relatively better antihypertensive effect, which can reduce occurrence risk of cardiovascular events.

1651 Effect of Mannitol combined with Gamma Globulin on Leukocyte count and CRP in Children with Severe Hand-foot-mouth Disease

Objective: To study the curative effect of mannitol combined with gamma globulin on children with severe hand, foot and mouth disease and the influence of white blood cell count and C reactive protein (CRP). Methods: 98 cases of hand, foot and mouth disease from February 2014 to February 2018 were selected. According to simple random grouping method, the children were divided into observation group (n=51) and control group (n=47). The control group was treated with gamma globulin, while the observation group was treated with mannitol combined with gamma globulin. The curative effect, clinical symptom subsidence time, white blood cell count, CRP level and the incidence of adverse reactions of the two groups were observed. Results: After treatment, the total effective rate of the observation group was significantly higher than that of the control group [94.11% (48/51) vs 74.46% (35/47)] (P<0.05), and the time of clinical symptoms subsidence was significantly lower than that of the control group [(2.18±0.63) D vs (4.01±0.85) d, (2.41 (+0.65) D vs (5.24 ±1.23) d, (2.90±0.52) D vs (5.86±1.30) d] (P < 0.05). The number and CRP were significantly lower than those of the control group [(10.36±4.62) 109/L vs (10.64±5.09) 109/L, (1.23±0.34) mg/L vs (2.97±0.75) mg/L] (P < 0.05), and the total incidence of adverse reactions was significantly lower than that of the control group [(5.88% (3/51) vs 21.27% (10/47)] (P<0.05). Conclusion: Mannitol combined with gamma globulin in the treatment of children with severe hand, foot and mouth disease has a significant effect. It can effectively improve the clinical symptoms of children with severe hand, foot and mouth disease, reduce the level of white blood cell count and CRP, and is safe and reliable.

1654 Comparison of Anesthetic Effects of Remifentanil Etomidate or Lidocaine Combined with Propofol for Painless Induced Abortion in Outpatients

Objective: To explore the differences of anesthetic effects of remifentanil, etomidate or lidocaine combined with propofol for painless induced abortion in outpatients. Methods: 108 cases of patients with painless induced abortion admitted to our hospital from January 2017 to March 2018 were divided into three groups according to thecomputer random number method, including group I (remifentanil combined with propofol anesthesia, n=36), group Ⅱ (etomidate combined with propofolanesthesia, n=36) and group Ⅲ (lidocaine combined with propofol, n=36). The intraoperative hemodynamic changes, analgesic effects, anesthetic effects, postoperative recovery and adverse reactions were detected and compared. Results: There were significant differences in the betweee-groups effects, time effects and interaction effects of MAP and HR (P<0.05), but there were no significant differences in the betweee-groups effects, time effects and interaction effects of SpO2 (P>0.05). , The MAP and HR in groups I and Ⅲ at T1 and T2 were lower than those at T0 (P<0.05), and the MAP and HR in group Ⅱ at T1 and T2 were higher than those in group I, and the MAP and HR in group Ⅲ at T1 and T2 were lower than those in group Ⅱ (P<0.05). There were no significant differences among the three groups in Ramsay score and anesthetic effects (P>0.05), and the VAS score in group Ⅱ was significantly lower than that in the other two groups (P<0.05), and there was no significant difference in VAS score between group I and group Ⅲ (P>0.05). The blinking time, spontaneous breathing and recovery walking time in group Ⅱ were significantly longer than those in the other two groups (P<0.05), and there was no significant difference between group I and group Ⅲ (P> 0.05). Adverse reactions of injection pain, nausea, lethargy, dizziness and postoperative pain were observed in all three groups, but the difference was not statistically significant (P>0.05). Conclusion: All the three groups have good anesthetic effects. Compared with the other two groups, etomidate combined with propofol anesthesia has better anesthetic effects, smaller blood flow fluctuation during operation and lighter postoperative pain degree, but slightly longer postoperative consciousness recovery time.

Objective: To analyze the effect of ketamine combined with flurbiprofen axetil on preemptive analgesia in patients undergoing radical mastectomy and its postoperative pain mediator and cardiovascular response. Methods: 230 cases of breast cancer who treated from April 2017 to April 2018 in our hospital, according to the simple random number table method those patients were divided into 102 control groups and 128 observation groups. control group used flurbiprofen axe to induce analgesia, observation group used flurbiprofen axetil combined with ketamine to induce analgesia. Then awakening time, extubation time and waking time, pain medium, visual analog scale (VAS) and intravenous controlled analgesia (PCIA) compressions, blood flow at different time points, cardiovascular reactions, adverse reactions in both group were compared. Results: awakening time, extubation time and waking time in both group were no statistically significant differences (P>0.05). 12h after surgery and 24h after surgery, the levels of adenosine E2 (PGE2), substance P (SP), and 5-hydroxytryptamine (5-HT) in both groups were all increased, and the levels ofβ-endorphin (β-EP) was decreased, and the change range of the observation group were smaller (P<0.05). 3h, 12h and 24h after surgery, VAS score and PCIA presses in both groups were increased, and the observation group were lower than the control group (P<0.05). after operation and extubation,HR and MAP in both groups were increased, while the observation group had a smaller rised (P<0.05). The incidence of hypertension in the observation group was lower than that in the control group (P<0.05). Conclusion: Preemptive analgesia with ketamine combined with flurbiprofen axetilcan alleviate postoperative pain in patients undergoing radical mastectomy,it inhibits the secretion of pain-causing mediators, reduces cardiovascular reactions, and does not increase the incidence of adverse reactions.

Methods: 104 patients of senile hypertension complicated with heart failure who received therapy from January 2016 to September 2018 in our hospital were selected as research objects, according to simple random number table method, those patients were divided into 54 case of the observation group and the 50 cases of the control group, on the basis of routine treatment, the control group was treated with benazepril, while the observation group was combined with Qili Qiangxin Capsule on the basis of the control group, they were treated for 8 weeks. The clinical efficacy, serum neuroendocrine hormones, blood pressure, cardiac function and adverse reactions were compared between the two groups. Results: After treatment, the total effective rate in the observation group was significantly higher than those in the control group(P<0.05);there were significant differences in the interaction of serum angiotensin Ⅱ (AngⅡ), endothelin-1 (ET-1), norepinephrine (NE), systolic blood pressure (SBP), diastolic blood pressure (DBP), left ventricular ejection fraction (LVEF), left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), 6-minute walking distance (6MWT) between groups at interblock, different time points, interblock·different time points(P<0.05);there were no obvious adverse reactions during treatment in both groups. Conclusion: Qili Qiangxin Capsule combined with Benazepril is well for senile hypertension complicated with heart failure, which can effectively regulate the expression of serum neuroendocrine hormones, reduce blood pressure, improve cardiac function, and no increase in adverse drug reactions the clinical application value is high.

1667 Relationship between Brain Natriuretic Peptide Level and Depression in Patients with Acute ST-segment Elevation Myocardial Infarction

Objective: To investigate the relationship between the relationship between the BNP level and depression in the acute st segment of the myocardial infarction.Methods: 308 cases of patients in the department of cardiology of the affiliated hospital of chengde medical college with acute st-segment elevation myocardial infarction was enrolled in this study.They were divided into the group of BNP greater than 100pg/ml 124 cases and the group of BNP less than 100pg/ml 184 cases according to BNP level.The patients in the two groups were evaluated on the PHQ-9 scale within72h after admission, and the patients with depression were divided into mild depression, moderate depression, moderate and severe depression and severe depression according to their scores.The proportion of people without depression and the proportion of people with different degrees of depression in the two groups were analyzed.Results: The proportion of patients in the BNP ≥ 100pg/ml group was higher than that in the BNP< 100pg/ml group (P<0.001).The proportion of depression in BNP ≥ 100pg/ml group was 79.8%, including 47.6% for mild depression, 22.6% for moderate depression, 8.0% for moderate and severe depression, and 1.6% for severe depression;the proportion of depression in BNP < 100pg/ml group was 60.9%, including 44.6% for mild depression, 12.5% for moderate depression, 3.3% for moderate and severe depression, and 0.5% for severe depression (P < 0.001).Linear regression analysis of BNP level and depression score showed that BNP level was positively correlated with depression score (r = 0.45, P < 0.001).Binary logistic regression showed that BNP level was an independent predictor of depression and the probability of depression in BNP > 100pg/ml group was higher than that in BNP < 100pg/ml group [adjust OR =2.09, 95%CI (1.19-3.67), P = 0.01]. Conclusion: The BNP level of patients with acute ST elevation myocardial infarction is closely related to depression. The higher the BNP level, the higher the proportion of people with depression, Depression was positively correlated with BNP level.

1671 Application Effects of Cook Balloon Combined with Misoprostol in Induced Labor of Full-term Pregnant Primiparas and its Influence on VASA Score

Objective: To explore the application effects of Cook balloon combined with misoprostol (Mis) in induced labor of full-term pregnant primiparas and its influence on Visual Analogue Scale of Anxiety (VASA) score. Methods: 420 full-term pregnant primiparas from February 2015 to May 2017 were divided into combined group and Mis group by simple random sampling, with 210 cases in each group. The maternal effective contraction time of induction and labor time, cervical maturity (Bishop score at 12h of intervention) and effects of induced labor, delivery modes, perinatal outcomes and maternal adverse reactions were recorded. Maternal anxiety status (VASA) was assessed at delivery, at the end of delivery and at 2h after delivery. Results: The effective contraction time of induction, labor time and cesarean section rate in combined group were lower than those in Mis group (P<0.05). The Bishop score and effects of induced labor in combined group at 12h of intervention were higher than those in Mis group (P<0.05). But there were no significant differences in the occurrence of perinatal complications and maternal adverse reactions between the two groups (P>0.05). The VASA scores at delivery, at the end of delivery and at 2h after delivery in combined group were lower than those in Mis group (P<0.05). Conclusion: Cook double-lumen balloon combined with Mis is safe and effective in induced labor of full-term pregnant primiparas. It can make pregnant women get better delivery experience, and it is conductive to improving delivery emotions.

1674 Significance of CD69 and RANTES Expression in Nasal Polyps of Patients with Chronic Rhinosinusitis

Objective: To investigate the significance of CD69 and RANTES expression in nasal polyps of patients with chronic rhinosinusitis (CRS). Methods: 198 cases of CRS and 87 cases of health examinees (control group) in our hospital were selected. CRS patients were divided into non-nasal polyp group (n=92) and nasal polyp group (n=106). Then the mRNA and protein levels of CD69, RANTES, and levels of IL-4, IL-10, IL-12, IFN-γ, TNF-α, and hs-CRP were measured and compared between groups. Results: The mRNA and protein levels of CD69 and RANTES ranking in a descending order was nasal polyp group, non-nasal polyp group and control group (P<0.05).The levels of IL-4, IL-10, IL-12, TNF-α, and hs-CRP ranking in a descending order was nasal polyp group, non-nasal polyp group and control group (P<0.05).The IFN-γ levels ranking in an ascending order was nasal polyp group, non-nasal polyp group and control group (P<0.05). The mRNA levels of CD69 and RANTES were positively correlated with IL-4, IL-8, IL-10, TNF-α and hs-CRP, and negatively correlated with IFN-γ (P<0.05). High levels of TNF-α, CD69, and RANTES mRNA were risk factors for nasal polyps in CRS patients (P<0.05). The AUC, sensitivity and specificity of CD69 and RANTES mRNA in the diagnosis of CRS were similar (χ2=1.219, 1.695, P>0.05), which were less than TNF-α (P<0.05). Conclusion: Elevated levels of CD69 and RANTES in nasal polyps cause a series of inflammatory factors imbalance, which may be the pathogenesis of CRS; moreover, the high levels of CD69 and RANTES are risk factors for nasal polyps in CRS patients.

1680 Efficacy and Safety of Intramedullary Fixation and Inverted LISS in the treatment of Elderly Unstable Intertrochanteric Fractures

Objective: To explore the effects of intramedullary fixation and inverted less invasive stabilization system (LISS) on elderly patients with unstable intertrochanteric fractures. Methods: 98 elderly patients with unstable intertrochanteric fractures in our hospital from July 2015 to July 2017 were selected for the study. 98 patients were divided into group A (odd number, 49 cases) and group B (even number, 49 cases) according to the odd and even hospitalization number. Group A was given intramedullary fixation, and group B was given inverted LISS. The operation indexes (total bleeding volume, operative time, hospital stay, first full weight-bearing time, fracture healing time), Harris score at 1 year after operation and incidence rate of complications were compared between the two groups. Results: The total bleeding volume in group A was more than that in group B, and the operative time, hospital stay first full weight-bearing time and fracture healing time were shorter than those in group B (P<0.05). There were no significant differences in the Harris score at 1 year after operation and incidence rate of complications between the two groups (P>0.05). Conclusion: Intramedullary fixation and inverted LISS both have good efficacy and high safety in the treatment of elderly patients with unstable intertrochanteric fractures, and there is no difference in hip recovery after operation. However, intramedullary fixation can shorten the operative time and hospital stay, and can more rapidly complete recovery after operation.

1684 Application of CTA Combined with DSA in Diagnosis and Treatment of Thoracoabdominal Aortic Aneurysm

Objective: To investigate the application value of CT angiography (CTA) combined with digital subtraction angiography (DSA) in the diagnosis and treatment of thoracoabdominal aortic aneurysms. Methods: A total of 190 patients with suspected thoracoabdominal aortic aneurysm who were admitted to the hospital from July 2016 to May 2018 were enrolled. All underwent DSA and CTA. The diagnosis result and imaging result of the two methods were compared. Results: There was no significant difference in sensitivity, specificity and accuracy for aneurysms between CTA and DSA (P>0.05). There was no significant difference in tumor morphology, classification and location between the two methods (P>0.05). Conclusion: The combined examination with CTA and DSA for patients with thoracoabdominal aortic aneurysm can more accurately show the pathological features of aneurysms, and improve the accuracy of preoperative evaluation. In particular, conducting CTA to define diagnosis for patients with thoracoabdominal aortic aneurysm before the interventional treatment, choosing best projection angle according to the aneurysm morphology displayed by CTA in the surgery, and guiding DSA to select the appropriate angle could show more accurately show pathological features of aneurysms, improve the accuracy of evaluation before covered stent insertion. At the same time, the intraoperative radiation dose and contrast dose are significantly less.

1687 Analysis and Study on the Effect, Safety and Satisfaction of Patients with Facial Plastic and Cosmetic Surgery by Lattice Laser Technology

Objective: To analyze the satisfaction and safety of dot array laser technology on facial plastic surgery and cosmetology patients. Methods: 70 cases of facial plastic surgery and cosmetology patients admitted to our hospital from June 2015 to June 2016 were selected as the research objects, randomized number table method was used to divide the observation group (n=37) and the control group (n=33). The control group was treated with conventional surgery, and the observation group was treated with dot matrix laser technology on the basis of the control group. The efficacy, visualanalogue scale (VAS), quality of life, satisfaction and adverse reactions of the two groups after treatment were compared. Results: After treatment, the total effective rate of the observation group was 91.89%, significantly higher than that of the control group (69.70%) (P < 0.05). After treatment, VAS scores and quality of life scores in both groups were significantly improved, and the quality of life scores and VAS scores in the observation group were significantly better than those in the control group (P < 0.05). After treatment, the satisfaction of patients in the observation group was 91.89%, significantly higher than that in the control group (66.67%, P < 0.05). The total incidence of adverse reactions was 16.22% and 54.55%, respectively (P < 0.05). Conclusion: Dot-matrix laser technology is effective in facial plastic and cosmetic patients. It can effectively improve pain and quality of life, reduce adverse reactions, and is worthy of promotion and application.

1690 Effects of Hyperandrogenemia and Insulin Resistance on Lipid Metabolism in Patients with PCOS

Objective: To investigate the effects of hyperandrogenemia and insulin resistance on lipid metabolism in patients with polycystic ovary syndrome. Methods: 593 cases of PCOS diagnosed from March 2016 to June 2018 were retrospectively analyzed. 83 healthy women were selected as control group. PCOS was divided into HA group and NHA group according to testosterone and hyperandrostenedione values. PCOS was divided into IR group and NIR group according to HOMA-IR. Blood lipid levels of each group were measured and the relationship between lipid metabolism and hormone and insulin resistance was analyzed. Results: TC, TG, HDL, LDL, Apo-A, Apo-B, Apo-B/Apo-A values in PCOS group were higher than those in control group. TC, TG, LDL, Apo-A, Apo-B, Apo-B/Apo-A values in HA group were higher than those in NHA group, and HDL values in HA group were lower than those in NHA group. TC, TG, LDL, Apo-A and Apo-B values in IR group were higher than those in NIR group. T and TC, TG, LDL, Apo, Apo, Apo-B, Apo-A/Apo-A showed positive phase. Guan was negatively correlated with Apo-A and HDL; AND was positively correlated with TC, TG, HDL, LDL, Apo-A and Apo-B, and negatively correlated with Apo-B/Apo-A; HOMA-IR was positively correlated with TC, TG, LDL, Apo-A, Apo-B and Apo-B/Apo-A, and negatively correlated with HDL. Conclusion: PCOS patients had severe abnormal lipid metabolism, and PCOS patients with HA/IR had more abnormal lipid metabolism. T, AND and HOMA-IR in PCOS patients were correlated with TC, TG, HDL, LDL, Apo-A, Apo-B, Apo-A.

1695 Central Nervous System Tuberculosis After In Vitro Fertilization and Embryo Transfer: a report of 7 Cases and Literature Analysis

Objective: To discuss the clinical characteristics of the central nervous system tuberculosis(CNS-TB) in those women after in vitro fertilization-embryo transfer (IVF-ET), and literature review. Methods: Clinical datas of 7 cases of CNS-TB after IVF-ET from January of 2009 to August of 2018 in the Department of Tuberculosis Research Institute in the Eighth Medical Center of PLA were analyzed retrospectively. Results: 7 cases were diagnosed as CNS-TB after IVF-ET , 4 received IVF-ET once, 2 twice and 1 trice. One failed of transfering, 2 early abortion, 3 mid-term induced abortion and 1 premature fetus died. Among them, 1 case had tuberculous pleurisy in the past and had received full-course anti-tuberculosis treatment, 1 case had tuberculosis of fallopian tube and had not completed treatment due to adverse drug reactions, 2 cases had contact history of tuberculosis patients, and the other 3 cases had no clear history of tuberculosis and contact history. The final diagnoses of 7 cases were as follows, 6 cases with hematogenous pulmonary tuberculosis, 1 case with double pulmonary tuberculosis (sputum positive). Six cases of tuberculous meningoencephalitis had brain tuberculoma, 4 cases combined tuberculous perimyelitis (1 case of endometrial tuberculosis and 1 case of tuberculous polyserositis),and 1 case conbinined pelvic tuberculosis .Four cases had intracranial hypertention and 3 cases had normal intracranial pressure. Interferon-gamma release assays (IGRAs) were positive in 5 cases(one case was not checked), tuberculosis antibodies 38 kD positive in 6 cases, PPD positive in 4 cases and CA125 positive in 6 cases. Conclusion: IVF-ET can significantly increase the incidence of CNS-TB. Once it occurs, the prognosis of pregnancy is poor, which can cause fetal death and maternal dysfunction.

1700 Analysis on Effect of Sanhuang Lotion on Wound infection after Tibiofibular Fracture surgery

Objective: To investigate the effect of Sanhuang lotion on wound infection after tibiofibular fracture surgery. Methods: 108 patients with wound infection after tibiofibular fracture surgery were selected from January 2016 to March 2018 in our hospital. The patients were divided into study group and control group by random number table method, with 54 cases in each group. The control group was treated with conventional Western medicine, and the research group was treated with Sanhuang lotion on this basis. The therapeutic effect of the two groups was compared. Results: The levels of serum CRP (10.90+4.57) mg/L, IL-6 (72.76+11.96) ng/L, TNF-alpha (28.66+4.42) ng/L [CRP (10.90+4.57) mg/L, IL-6 (72.76+11.96) ng/L, TNF-alpha (28.66+4.42) ng/L] in the study group were significantly lower than those in the control group (P<0.05). The number of dressing changes in the study group was significantly less than that in the control group (8.89 (+2.16). Incision closure rate was 100%, limb function score (91.55 (+5.79) was significantly higher than that in the control group. Incision healing time (12.42 (+2.77) days) and rehabilitation cycle (15.96 (+3.90) weeks) were significantly shorter than those in the control group. There was a significant difference between the two groups (P < 0.05). The pain score of the study group (4.90 +0.80) was significantly lower than that of the control group (6.37 +0.93), and the difference between the two groups was statistically significant (P < 0.05). Conclusion: Sanhuang lotion can effectively control wound infection after tibiofibular fracture surgery, reduce inflammation reaction and promote wound healing. The therapeutic effect is good, and it is worth recommending.

1703 Cross-sectional Anatomy and Reconstructed CT Parameters of Pedicle Screw Placement in Atlas

Objective: To reconstruct the CT measurements and cross section anatomy, explore the atlas of the nail into the position accurately, to determine the nail into the constant reference frame, and based on the selected reference frame and into the nail Angle relationship of measurement to determine the direction and point of view, for the atlas pedicle nailing technology to provide the anatomical and clinical operation reconstruction CT data reference. Methods: 20 atlas specimens of 40 sides were measured with vernier calipers and protractors. The atlas three-dimensional CT images of 20 normal subjects were measured with GE Light Speed 64 Pro spiral CT operating software (ADW4.2). Results: The reconstructed CT and sectional anatomical measurements showed that: the thinnest width of the posterior arch at the base of the atlas vertebral artery sulcus (w) > its height (H), the height of the arch was H4.6±1.5mm), the height of the arch was 4.5±1.3mm, the width of the arch was 9.5±2.0mm, and the width was 8.9±1.9mm. The transverse distance between the screw entry point and the posterior tubercle of the posterior arch of the atlas is S1:21.2±3.5 mm and 20.8±3.6 mm. The length of atlas screw track (AB): 26.1±4.4 mm; 25.6 ±3.9 mm. The distances between the inner and outer edges of the dorsal mass of the atlas are S2:11.1±1.1 mm and 11.5±1.2 mm. Longitudinal angle of nail track (Y): 4.1±0.7, 4.7±0.9, transverse angle of nail track (X): 10.9±5.2, 10.2±4.6. The distance from the medial wall of the transverse foramen to the outer edge of the spinal canal was S3:8.0+1.4 mm and 7.8±1.5 mm. There was no significant difference between the two parameters (P > 0.05). Conclusion: The results of anatomical measurement and reconstructed CT were consistent. The projection distance between the middle point of the distance between the inner edge and the outer edge of the lateral mass of the upper arch and the thinnest point of the posterior arch at the bottom of the vertebral artery sulcus was 2 mm. The longitudinal angle of the screw was 3 to 5 degrees, and the transverse angle was 6 to 16 degrees; the length of the screw was 21.7 degrees. 3.5mm in diameter and 30.5mm in diameter are objective, safe and reliable. Meanwhile, three-dimensional reconstruction CT can provide more specific and individualized reference for nail placement.

1706 Clinicopathological Features and Risk Factors of Bone Metastases in 60 Patients with Radical Resection of Colorectal Cancer

Objective: To analyze the clinicopathological features and risk factors of bone metastases in 60 patients with radical resection of colorectal cancer. Methods: The clinical data of 60 cases with bone metastases and 65 cases without bone metastases after radical resection of colorectal cancer in our hospital from June 2014 to June 2017 were retrospectively analyzed. The clinicopathological features and risk factors of 60 patients with postoperative bone metastasis were analyzed. Results: Among the 60 patients with bone metastasis after radical resection of colorectal cancer, most of them were middle-aged and elderly patients, mostly men. There were 16 cases (26.67%) of bone metastasis and 44 cases (72.33%) of multiple bone metastases. Bony changes: there were 50 cases (83.33%) of osteolytic lesions, 4 cases (6.67%) of osteogenic lesions and 6 cases (10.00%) of mixed bone destruction. There were 5 cases of simple bone metastases (8.33%), and 55 cases (91.67%) of combined distant metastasis. There were significant differences between patients with bone metastases and patients without bone metastases in terms of primary tumor location, infiltration depth, primary tumor diameter, whether postoperative chemotherapy was or not, preoperative CA199 level, whether lymph node metastasis was or not, and whether the combined lung metastasis was or not (P<0.05). The primary tumor location, infiltration depth, without postoperative chemotherapy, preoperative CA199 level, lymph node metastasis and lung metastasis were independent risk factors for bone metastases after radical resection for colorectal cancer (P<0.05). Conclusions: The location of primary tumors, depth of invasion, non-chemotherapy after operation, preoperative CA199 level, lymph node metastasis and lung metastasis are independent risk factors for bone metastasis in patients with colorectal cancer after radical operation. The monitoring of risk factors after operation should be strengthened, and corresponding preventive measures should be taken actively.

1711 Effects of Type 2 Diabetes Mellitus on Blood Fat, Cardiac Function and Cardiovascular Disease with Hypothyroidism

Objective: To explore the effects of type 2 diabetes mellitus (T2DM) on cardiovascular-related complications in patients with subclinical hypothyroidism (SCH). Methods: 344 patients with SCH who were diagnosed in our hospital from April 2015 to April 2018 were retrospectively analyzed and were divided into T2DM-SCH group (64 cases) and SCH group (280 cases) according to whether patients were complicated by T2DM. The general data of age, gender and body mass index (BMI) were retrospectively analyzed, and the FPG, OGTT 2h blood glucose (2hPG), glycosylated hemoglobin (HbA1c) concentration, total cholesterol (TC), triglyceride (TG), low-density Lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), free triiodothyronine (FT3), free thyroxine (FT4), thyroid-stimulating hormone (TSH), left ventricular systolic function (LVEF) and diastolic function (E/A) were detected, The complications of cardiovascular diseases were recorded in the two groups. Results: Compared with SCH group, the levels of serum FPG, 2hPG, HbA1c, TC, TG, HDL-C, LDL-C, SBP and DBP were significantly increased in T2DM-SCH group (P<0.05) while the levels of cardiac function LVEF and E/A were decreased significantly (P<0.05). There were 31 cases (48.44%) of cardiovascular events in T2DM-SCH group and 76 cases (27.14%) of cardiovascular events in SCH group (χ2=14.752, P=0.005). Conclusions: T2DM can increase the risk of lipid metabolism disorder, cardiac dysfunction, and cardiac adverse events in SCH patients.

Objective: To study effects of Closed reduction and elastic intramedullary nail fixation treatment with radial neck fracture in children and Its Influence on elbow joint function. Methods: 81 radial neck fracture in children who received therapy from March 2017 to March 2018 in our hospital were selected as research objects. According to random number table,those patients were divided into the observation group (n=42) and the control group (n=39). The control group was treated with , while the observation group treated with Closed reduction and elastic intramedullary nail fixation. Then the Therapeutic effect, operative indicators (operation time, intraoperative blood loss, hospital stay, fracture healing time), elbow flexion and extension, rotation range of motion of two groups after treatment were compared. Results: After treatment, The total effective rate of observation group was significantly higher than that of control group[88.09%(37/42)vs61.53%(24/39)](P<0.05). The operation time, intraoperative blood loss, hospitalization time and fracture healing time were significantly lower than those in the control group[(30.48±8.25)min vs(46.02±10.40)min,(9.65±2.10)ml vs(14.08±3.07)ml,(7.39±2.13)d vs(11.83±3.86)d ,(40.38±4.29)d vs(51.56±5.10)d](P<0.05). The flexion, extension and rotation of elbow joint were significantly higher than those of control group[(140.98±22.13)°vs(130.12±19.61)°,(8.37±1.53)°vs(4.97±1.02)°,(86.02±12.29)°vs(80.75±10.34)°,(84.08±12.84)°vs(76.40±9.73)°](P<0.05). Conclusion: Closed reduction and elastic intramedullary nail fixation in the treatment of radial neck fracture in children has a significant effect, can achieve satisfactory reduction effect, can effectively improve the elbow function of children, fewer complications, improve life treatment.

1718 Curative Effect of Three Different Minimally Invasive Methods for the Treatment of Common Bile Duct Stones without Gallstones

Objective: To investigate the curative effect of different minimally invasive methods for the treatment of common bile duct stones without gallstones. Methods: Retrospective analysis of clinical data of 96 patients with common bile duct stones were performed, Patients were divided into groups A, B, and C according to different surgical procedures. Group A undergoing laparoscopic choledocholithotomy, primary suture, group B undergoing laparoscopic cholecystectomy , laparoscopic choledocholithotomy , primary suture and group C undergoing laparoscopic cholecystectomy , laparoscopic primary , T tube drainage. The intraoperative blood loss, surgical time, postoperative exhaust time, incidence of postoperative complications, hospitalization time and costs were compared among the three groups. Results: The intraoperative blood loss of group A was significantly less than that of group B and group C (P<0.05). The exhaust or defecation time and hospitalization time of group C was significantly longer than that of group A and group B (P<0.05). There was no significant difference in the incidence of postoperative residual stones, stone recurrence or postoperative complications among the three groups (P>0.05). Conclusion: The three minimally invasive procedures have their own advantages and disadvantages. For patients with indications for preservation of gallbladder and primary suture, it is recommended to preserve gallbladder and use primary suture.

1721 Exploring the Effect of Vildagliptin on Osteoporosis and Microangiopathy in Diabetic Patients

Objective: To explore the effect of vildagliptin on diabetic osteoporosis and microangiopathy in diabetic patients. Methods: 90 patients with diabetes treated in our hospital from November 2016 to November 2017 were selected as the subjects. The patients were divided into the control group and the observation group according to the random number table. The control group was treated with the routine treatment of diabetes. The patients in the observation group were treated with the treatment vildagliptin on the basis of routine treatment in the control group, and the comparison was two. The therapeutic effects were evaluated and the osteoporosis and microangiopathy in the two groups before and after treatment were evaluated. Results: After treatment, the FPG, 2hPG, HbAIC of the observation group were 5.3 ±1.1, 6.3±2.7 and 4.6±2.1, respectively. The bone mineral density of the observation group was 2.31±0.62, 2.29±0.57, 1.98±0.65 and 1.83±6.3, respectively. The number of vildagliptin retinopathy I, II, III and IV in the observation group after the treatment was significantly lower than that of the control group. The incidence of 40% was significantly lower than that of the control group. Conclusion: Vildagliptin's use in diabetic patients can help improve osteoporosis and microangiopathy in patients.

1724 Clinical Analysis of High Cesarean Section Rate after Two-child Policy

Objective: To analyze the changes of cesarean section rate and cesarean section indications in a third-class a hospital of a city from 2014 to 2018 in order to analyze the reasons for the high cesarean section rate after the two-child policy. Methods: A retrospective analysis was conducted on the clinical data of 15,531 puerpera who were hospitalized in childbirth from 2014 to 2018 to analyze the cesarean section rate and the changes of cesarean section indications after the second child policy was gradually opened. Results: From 2014 to 2018, the highest rate of cesarean section was scar uterus (28.93%), which increased year by year, with statistically significant difference (P < 0.05). The proportion of pregnancy complications and complications in the first cesarean section increased year by year. The proportion of social factors in 2017 (23.88%), 2018 (24.17%), and the proportion of giant fetuses in 2017 (8.30%) and 2018 (4.92%) all decreased from 30.27% and 13.61% in 2014.According to the opening time of the second fetus, the ratio of scar uterus was statistically significant (P < 0.05), social factors and giant fetal factors were decreased, but pregnancy complications and complications were increased. Conclusion: The first cesarean section is the primary factor of high cesarean section rate. Control of the rate of the first cesarean section is the key, at the same time needs a variety of measures to intervene together, for pregnant women to give pre-pregnancy, pregnancy and prenatal health education is crucial.

1729 Experimental Study on Determination of Total Solubility Parameters of Chinese Traditional Medicine by High Performance Liquid Chromatography

Objective: The theory and experiment of HPLC determination of total solubility parameters of Chinese traditional medicine were studied. Methods: By high performance liquid chromatograph to caffeine, aloe emodin, rhein, emodin and emodin and methyl ether in the liquid phase detection, through the theory of solubility parameter and Hildebrand -Scatchard solution related equation and principle of high performance liquid chromatography (HPLC) combined with each other, treat total component parameter measurement AT, BT, CT and the related fingerprint parameters average retention time analysis the relationship between a, b, c and so on. Results: According to the fingerprint between apparent solubility parameter average retention time and amount of related function and a mathematical model, based on high performance liquid chromatographic peak time correlation formula (1) and (2) different elution conditions of caffeine solubility parameter values were 32.04 J1/2/cm3/2, 35.07 J1/2/cm3/2, 39.04 J1/2/cm3/2, 43.26 J1/2/cm3/2, 45.78 J1/2/cm3/2, 46.22 J1/2/cm3/2. The correlation values calculated by the formula were 0.09256, 0.14365, 27.39, 0.1533 and 0.16743, respectively. According to the fingerprint average retention time and rhubarb components between the solubility parameter of the related function and a mathematical model, based on high performance liquid chromatographic peak time in combination with related formula (1) and (2) different elution conditions of aloe emodin solubility parameter values were 38.56 J1/2/cm3/2、36.54 J1/2/cm3/2、40.25 J1/2/cm3/2、44.25 J1/2/cm3/2、43.65 J1/2/cm3/2、44.58 J1/2/cm3/2. The correlation values calculated by the formula were 1.09584, 0.5869, 26.58, 0.53647 and 0.12356, respectively. Rhubarb acid solubility parameter values were 43.25 J1/2/cm3/2, 44.26 J1/2/cm3/2, 44.58 J1/2/cm3/2, 45.44 J1/2/cm3/2, 44.98 J1/2/cm3/2, 45.23 J1/2/cm3/2, respectively. The solubility parameters of emhutin were 44.73 J1/2/cm3/2、42.16 J1/2/cm3/2、47.69 J1/2/cm3/2、43.21 J1/2/cm3/2、44.78 J1/2/cm3/2、43.26 J1/2/cm3/2, and 0.57657, 0.43256, 0.44569, 0.43225, and 0.44154, respectively. The solubility parameters of emodin methyl ether were 42.16 J1/2/cm3/2、47.56 J1/2/cm3/2、46.21 J1/2/cm3/2、44.38 J1/2/cm3/2、47.56 J1/2/cm3/2、44.23 J1/2/cm3/2, respectively. The relationship between the total parameters AT, BT, CT and the average retention time of the related fingerprint parameters a, b and c was strongly fitted. Through the P value analysis, there were significant differences among the indexes (P<0.01). Therefore, combined with the total solubility parameter theory of related TCM components and high performance liquid chromatography, the average values of fingerprint parameters can be calculated and analyzed. Conclusion: HPLC has a good application value in determining the solubility parameters of multiple components of traditional Chinese medicine and can lay a theoretical foundation for improving the application of single traditional Chinese medicine.

1734 Effect of Transverse Bone Transfer Technique Combined with PRP on Repairing Effect and Quality of Life in Patients with Diabetic Foot Ulcer

Objective: To investigate the effect of transverse bone transfer technique combined with PRP on repairing effect and quality of life in patients with diabetic foot ulcer . Methods: 134 patients with diabetic foot from January 2018 to October 2018 were chosen and divided into observation group and control group , 67 cases in each group . The control group was treated with PRP topical wound after debridement , the observation group was treated with PRP external wound combined with transverse tibial bone transport . Compare the expression changes of PDGFR , wound reduction rate , bacterial clearance rate , granulation tissue coverage , granulation tissue thickness and quality of life between the two groups . Results: Before the treatment , there was no significant difference in PDGFR-α and PDGFR-β between the two groups ( P>0.05 ) . After treatment , the levels of PDGFR-α and PDGFR-β in both groups were higher than before treatment , and the observation group increased more significantly than the control group , P<0.01 . The rate of wound reduction in the observation group was significantly higher than that in the control group , the rate of bacterial clearance and the coverage rate of granulation tissue were significantly higher than that in the control group , and the growth thickness of granulation tissue was significantly thicker than that in the control group , P all <0.01 . After treatment , the scores of treatment influence , psychological or spiritual , physiological function , and social relationship of the two groups were reduced , and the reduction of the observation group was greater than that of the control group , P<0.01 . Conclusion: Transverse-bone transfer technique combined with PRP in the treatment of DFU can effectively improve the expression of PDGFR , improve the repair effect of ulcer wounds , and improve the quality of life.

1737 Value of CRP combined with SAA in differential diagnosis of bacterial pneumonia and Mycoplasma pneumonia in Children

Objective: To analyze the value of CRP combined with SAA in the differential diagnosis of bacterial pneumonia and mycoplasma pneumonia in children. Methods: :From December 2016 to December 2018, 100 cases of children hospitalized due to pulmonary infection in our hospital were selected and divided into two groups according to their infection status. Group a was a total of 75 cases of children with mycoplasma infection, while group b was a total of 25 cases of children with bacterial infection. Twenty healthy children who came to our hospital for physical examination in the same period were selected as the control group. The c-reactive protein level and serum amyloid A level of the three groups were determined and analyzed by ROC curve. Results: The CRP level and SAA level of children in the control group were significantly lower than those in group a and b, and the CRP level and SAA level of children in group a were significantly lower than those in group b (all P < 0.05). SAA/CRP ratios were not significantly different among the three groups (all P > 0.05). SAA test has a high diagnostic accuracy for children with bacterial infection and mycoplasma infection, while CRP test only has a high diagnostic accuracy for children with bacterial infection and has little significance for the differential diagnosis of children with mycoplasma infection. Conclusion: The detection of CRP and SAA has high accuracy in the diagnosis of bacterial infection in children, and the detection of SAA in the diagnosis and differential diagnosis of mycoplasma pneumoniae infection in children is also of high application value. It is worth popularizing and applying in clinic.

1741 Application of Bone Marrow Cytology and Peripheral Blood Routine Examination in the Diagnosis of Aplastic Anemia

Objective: To study the application value of bone marrow cytology combined with peripheral blood routine examination in the diagnosis of hypo-myelodysplastic syndrome (Hypo-MDS) and aplastic anemia (AA). Methods: The laboratory data of 20 patients with Hypo-MDS and 26 patients with AA admitted to our hospital from January 2014 to December 2018 were retrospectively analyzed. The results of bone marrow cytology and peripheral blood routine examination were compared between the two groups, and the application value of the two methods in identifying the two diseases was evaluated. Results: The results of peripheral blood test showed that the levels of platelet (PLT), reticulocyte (Ret), erythrocyte volume distribution width (RDW) and mean corpuscular volume (MCV) in AA group were significantly lower than those in Hypo-MDS group (P < 0.05). The morphological examination of peripheral blood microcells showed that the proportion of abnormal mature red blood cells in Hypo-MDS patients was significantly lower than that in Hypo-MDS group (P < 0.05). The frequencies of erythropoiesis, megakaryocyte hematopoiesis and trilineage hematopoiesis in Hypo-MDS patients were significantly higher than those in AA patients (P < 0.05). There was no significant difference in the frequencies of granulocyte hematopoiesis between the two groups (P > 0.05). Conclusions: It is difficult to distinguish Hypo-MDS from AA by a single laboratory examination, and a reasonable combination of bone marrow cytology and peripheral blood routine examination results can provide some reference for the identification of the two diseases, but for those who can not be identified, chromosome examination should be recommended for patients.

1744 Changes and Significance of apoA1/apoB and Lp(a) in Patients with Hypercholesterolemia

Objective: To study the changes and significance of apolipoprotein(apo)A1/apoB and lipoprotein(Lp)(a) in patients with hypercholesterolemia. Methods: 150 patients with hypercholesterolemia received from January 2017 to November 2017 were selected as the observation group, and 150 healthy were selected in our hospital for the same period as the control group. All personnel were collected from fasting venous blood at 2d after admission, the expression of low density lipoprotein cholesterol (LDL-C), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), triglyceride (TG), apoA1, apoB, apoA1/apoB, Lp(a) were detected, to analyze the changes and significance of apoA1/apoB and Lp(a) in patients with hypercholesterolemia. Results: The LDL-C, TC, HDL-C, apoA1, apoB, apoA1/apoB and Lp(a) in the observation group were(5.71±0.36)mmol/L,(6.83±0.47)mmol/L,(1.04±0.07)mmol/L,(1.07±0.18)g/L,(1.07±0.23)g/L、(1.06±0.32), (258.18±20.95)mg/L respectively, in the control group were(1.78±0.12)mmol/L,(3.62±0.29)mmol/L,(1.34±0.07)mmol/L,(0.38±0.22)g/L,(0.84±0.08)g/L,(1.67±0.32),(154.82±17.49)mg/L respectively, the differences has statistically significant(P<0.05); the Pearson correlation analysis showed that there was negatively correlated between apoA1/apoB and LDL-C and TC(r=-0.682, -0.679, P<0.05), there was a positive correlation between Lp(a) and LDL-C and TC(r=0.926, 0.909, P<0.05). Conclusion: There is obvious disorder of lipid metabolism in patients with hypercholesterolemia, and the decrease of apoA1/apoB and the increase of Lp (a) can reflect the disorder of lipid metabolism, this study also provides a new way for clinical treatment of hypercholesterolemia.

1753 Effect of Fire Needle combined with Sodium Hyaluronate Injection Intra-articular Injection on Lysholm Score and TLR4 Concentration in Peripheral Blood of Patients with Knee Osteoarthritis

Objective: To observe the influence on Lysholm score the TLR4 concn of peripheral blood on the method of fire needles combined with interarticular injection of sodium hyaluronate in the treatment of knee osteoarthritis. Methods: 80 patients with knee osteoarthritis were randomly divided into treatment group and control group. The treatment group was treated with fire needle combined with sodium hyaluronate injection intra-articular injection. The control group was treated with sodium hyaluronate injection alone intra-articular injection. The changes of Lysholm score and TLR4 concentration in peripheral blood were compared before treatment, 5 and 12 weeks after treatment. Results: At 5 weeks and 12 weeks after treatment, the Lysholm score of knee joint in treatment group and control group was compared (P=0.000, P=0.000), and the TLR4 concentration in peripheral blood of treatment group and control group was compared (P=0.000, P=0.000). Conclusion: Fire needle combined with sodium hyaluronate intra-articular injection can improve Lysholm score, reduce TLR4 concentration in blood, alleviate knee pain and improve knee function.

1757 The Efficacy and Safety of Xiaoer Chaigui Qure Granule in the Treatment of Herpetic Angina and its Effect on Serum LGA and LGM

Objective: To investigate effect of Xiaoer Chai GuiTuire granule on symptoms and serum lgA and lgM in children with herpangina. Methods: 50 cases of herpangina with accumulated heat in spleen and stomach type treated in our hospital from May 2016 to November 2017 were selected and divided into two groups according to the different treatment methods, each with 25 cases. The control group received the routine western medicine, while the treatment group received the Xiaoer Chai GuiTuire granule combined with western medicine. Then the TCM syndrome scores, clinical symptoms, immunoglobulin A (lgA) and immunoglobulin M (lgM) levels, clinical efficacy and adverse reactions were determined. Results: TCM syndrome scores had no difference before treatment (Z=-1.084, P>0.05), which were lower in the treatment group than in the control group after treatment (P<0.05). The improvement of clinical symptoms in the treatment group was significantly better than that in the control group (P<0.05). The levels of IgA and IgM had no difference before treatment (P>0.05), which were increased after treatment, and were higher in the treatment group than in the control group (P<0.05). The clinical efficacy of the treatment group was higher than that of the control group (96.00% vs 68.00%; χ2=6.640, P<0.05). The incidence rate of the adverse reaction in the treatment group was lower than that of the control group (4.00% vs 28.00%, χ2=5.357, P<0.05). Conclusion: The application of Xiaoer Chai GuiTuire granule is an effective and safe way to alleviate the various TCM symptoms, improve the lgA and lgM levels for children with herpangina, which is worthy of promotion.
