1409 Clinical Observation of LCAT combined with Sodium Hyaluronate Eye Drops in the treatment of Pterygium and its Effect on Recurrence Rate and OSDI Score

Objective: To investigate the clinical observation of limbal stem cell transplantation (LCAT) combined with postoperative sodium hyaluronate eye drops in the treatment of pterygium and its effect on recurrence rate and ocular surface disease index (OSDI). Methods: 90 patients with pterygium were randomly divided into two groups. The control group received pterygium excision combined with LCAT, and the observation group was treated with sodium hyaluronate eye drops on the basis of the control group. The therapeutic effects of the two groups and the recurrence rate at 6 months after surgery were compared. Ocular surface function, OSDI score, and tear inflammatory factor levels were measured and compared between the two groups. Results: The cure rate of the study group was higher than that of the control group (P<0.05). The recurrence rate of the study group was significantly lower than that of the control group (P<0.05). The ocular surface function of both groups received significant effects at 1 month after surgery (P<0.05). The control group recovered to the preoperative level 3 months after operation (P>0.05). The study group was significantly better than preoperative (P<0.05). The OSDI scores of the two groups were significantly higher than those before surgery (P<0.05). Significantly decreased 3 months after surgery (P<0.05), The OSDI scores of the study group were significantly lower than those of the control group at 1 month and 3 months after surgery (P<0.05). The inflammatory factors of tears in the two groups were significantly increased at 1 month after operation and significantly decreased at 3 months after operation (P<0.05). The IL-1β and TNF-α in the study group were significantly lower than those in the control group at 1 month and 3 months after operation (P<0.05). Conclusion: LCAT combined with postoperative sodium hyaluronate eye drops can significantly reduce dry eye symptoms, reduce inflammatory response, improve clinical efficacy and improve prognosis, and can effectively reduce postoperative recurrence of pterygium.

1413 Effect of OsteoMed M3 Screw Plate on Stress Response and Inflammatory Factors in Benign Spinal canal Tumors

Objective: To investigate the effect of OsteoMed M3 screw plate on stress response and inflammatory factors in the treatment of benign spinal canal tumors. Methods: From January 2015 to June 2017, 85 patients with benign vertebral canal tumors treated by laminoplasty were retrospectively divided into observation group (45 cases) and control group (40 cases). The observation group was treated with Osteo Med M3 screw plate internal fixation and laminoplasty, while the control group was treated with conventional laminectomy. Cure. The perioperative conditions, post-operative stress response, inflammatory factors, functional recovery of spine and clinical efficacy were compared between the two groups. Results: The operation time, active time and cervical pick up time in the observation group were significantly shorter than those in the control group, and the intraoperative blood loss was significantly less than those in the control group(P<0.05); there were significant differences in the interaction of serum superoxide dismutase (SOD), malondialdehyde (MDA), serum tumor necrosis factor-α (TNF-α), interleukin(IL)-6, IL-8, Japanese Orthopaedic Association Assessment Treatment Score (JOA), spine Dysfunction Index (NDI) score, spine Cobb angle and spinal canal width between groups at inter-group, different time points, inter-group·different time points (P< 0.05); the follow-up until 31 May 2018, the excellent and good rate of clinical efficacy in observation group was significantly higher than those in the control group(P<0.05). Conclusion: OsteoMed M3 nail plate is well forlaminoplasty for benign spinal canal tumors, the postoperative stress response, less inflammatory reaction, fast recovery of cervical function and other advantages, and high safety, it is worthy of application and promotion.

1418 Curative Efficacy of Autologous Bone Marrow Stem Cells in Treatment of Biliary Cirrhosis and Effects on Serum AFP CA125 and Liver Fibrosis

Objective: To study the curative efficacy of autologous bone marrow stem cells in treatment of biliary cirrhosis and effects on serum alpha fetoprotein(AFP), carbohydrate antigen 125(CA125) and liver fibrosis. Methods: 95 patients of biliary cirrhosis who received therapy from January 2016 to January 2018 in our hospital were were divided into the 50 cases of the observation group and the 45 cases of the control group by the simlpe random number table, the control group was given routine treatment, including oral ursodeoxycholic acid capsules, polyene phosphatidylcholine capsules, Compound Glycyrrhizin tablets, etc, the observation group was treated with autologous bone marrow stem cell transplantation for after treatment 4 weeks on the basis of the control group. The clinical efficacy, serum AFP, CA125, liver function indexes, liver fibrosis indexes and adverse reactions were compared between the two groups. Results: After treatment, the total effective rate in the observation group was significantly higher than those in the control group[84.00%(42/50)vs62.22%(28/45)](P<0.05); the serum AFP and CA125 levels in observation group were significantly lower than those in control group[(4.42±0.75)ng/ml vs(5.88±0.91)ng/ml,(61.09±8.31)U/ml vs(75.63±10.51)U/ml](P<0.05); the indexes of aspartate aminotransferase (AST), alanine aminotransferase (ALT), total bilirubin (TBil), alkaline phosphatase (ALP), glutamyltransferase (GGT) and the indexes of liver fibrosis such as hyaluronic acid (HA), type III procollagen (PCIII), laminin (LN) in the observation group were significantly lower than those in the control group(P<0.05); there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion: Autologous bone marrow stem cells is well for biliary cirrhosi, which can effectivelyimprove liver function, liver fibrosis, reduce the expression of serum AFP and CA125 levels, and has high safety, it's worth for popularizing.

1422 Assessment Value of Combined Detection of Hepatointestinal Calcoprotein and Tumor Markers in Staging of Gastric Cancer and its Relationship with Clinicopathological Features

Objective: To analyze the value of detection of hepatoenteral calcoprotein(CDH17) combined with tumor markers in the evaluation of gastric cancer staging and its relationship with clinicopathological features. Methods: 147 patien ts with gastric cancer who according to the standard of TNM staging gastric carcinoma was divided into 8 cases of stage Ⅰperiod, 13 cases of Ⅱperiod, 57 cases of Ⅲ period, 69 cases of Ⅳ period; At the same time, 171 patients with benign gastric lesions and 130 healthy patients were selected as the control group. serum CDH17 and tumor marker levels, positive tissue expression rates in each group were compared,serum CDH17 and tumor markers of each group were compared, and their diagnostic value for gastric cancer staging was also compared. Results: CDH17 and serum tumor marker levels in gastric cancer group were higher than that of benign gastric lesion group, gastric benign lesion group were higher than the control group, there was a statistically significant difference (P<0.01). CDH17 area under the curve was 0.597, CEA area under the curve was 0.508, CA50 area under the curve was 0.689, the area under the curve was CA199, CA242 0.601 area under the curve was 0.553, CA724 area under the curve was 0.725, combined determination of area under the curve was 0.810, the joint detection area under the curve was higher than a single index. Conclusion: The expression of CDH17 and tumor markers in gastric cancer is correlated with the stage of gastric cancer, which is helpful for the diagnosis of gastric cancer and the evaluation of malignant biological behavior. The combined monitoring can make up for the deficiency of individual indicators and has high clinical application value.

1426 Relationship between Serum HEPCIDIN Expression and Prognosis in Patients with COPD Complicated with Respiratory Failure

Objective: To investigate the relationship between serum HEPCIDIN expression and prognosis in patients with COPD complicated with respiratory failure. Methods: 115 cases of COPD complicated with respiratory failure (case group) and 52 cases of health examinees (control group) in our hospital were selected. The levels of CRP, WBC, HEPCIDIN, FEV1%, FEV1/FVC, and DLCO% were measured; moreover, the HEPCIDIN levels of patients with different degrees of hypoxemia were also measured. Results: The case group had higher levels of CRP, WBC, HEPCIDIN and IL-6 than those of the control group (P<0.05). HbA1c,FEV1%, FEV1/FVC and DLCO% in the case group were lower than the control group (P<0.05). The HEPCIDIN level was positively correlated with the degrees of hypoxemia (P<0.01). HEPCIDIN was negatively correlated with HbA1c, FEV1%, FEV1/FVC, and DLCO%, and positively correlated with CRP, WBC and IL-6 (P<0.05). High levels of HEPCIDIN, IL-6 and low levels of HbA1c, FEV1/FVC were risk factors for death of COPD with respiratory failure (P<0.05). The AUC, sensitivity and specificity of HEPCIDIN and HbA1c were similar (χ2=1.219, 1.695, P>0.05), which were lower than those of the FEV1/FVC (χ2=16.249, 17.023, P<0.05). Conclusion: Elevated serum HEPCIDIN levels are closely related to the severity of COPD disease; furthermore, high levels of HEPCIDIN, IL-6 and low levels of HbA1c ,FEV1/FVC are risk factors for the death of COPD and respiratory failure; As a predictor, the sensitivity and specificity of HEPCIDIN in diagnosis of COPD with respiratory failure are high and is worthy of clinical application.

1431 The Diagnosis Valne of Combined Detection of Pepsinogen I Gastrin-17 and TOLL-like Receptors in Peripheral Blood in Early Laryngeal Cancer

Objective: To investigate the value of combined detection of pepsinogen-I (PG-I), gastrin-17 (G-17) L and Toll-like receptors (TLR4) in peripheral blood in the diagnosis of early laryngeal cancer. Methods: 270 cases of suspected laryngeal cancer and 90 cases of health check-up were selected from January 2013 to May 2017 in our hospital. The levels of PG-Ⅰ and G-17 in serum of the three groups were detected by enzyme linked immunosorbent assay (ELISA), and (peripheral blood mononuclear cell, in peripheral blood mononuclear cells (PBMC) was measured by real-time fluorescence quantitative polymerase chain reaction (real-time polymerase chain reaction,RT-PCR). TLR4 mRNA expression in PBMC. Results: The level of PG-I in vocal cord polyp group was significantly lower than that in healthy control group (P < 0.05), while that in laryngeal cancer group was much lower than that in other two groups (P < 0.05); there was no significant difference between healthy control group and vocal cord polyp group (P > 0.05), and the level of G-17 in laryngeal cancer patients was significantly higher than that in healthy control group and vocal cord polyp group (P < 0.05). The expression of TLR4 in cancer group was significantly higher than that in vocal cord polyp group and polyp group, and the difference was statistically significant (P < 0.05). The sensitivity and negative predictive value of combined detection of three indicators were significantly higher than that of single indicator detection (P < 0.05). Conclusion: The concentration of PGI and G-17 in peripheral serum and the expression of TLR4 in PBMC are of great significance for the diagnosis and differentiation of early laryngeal cancer. The combined detection method can significantly improve the diagnostic efficiency.

1434 Progesterone combined with Yunkang Oral Liquid in the Treatment of Habitual Abortion and its Effects on TGF-beta 1 EmAb PIBF and ACA

Objective: To analyze the effect of progesterone combined with Yunkang oral liquid on habitual abortion and its effects on transforming growth factor-beta 1 (TGF-beta 1), endometrial antibody (EmAb), progesterone-induced blocking factor (PIBF) and anticardiolipin antibody (ACA). Methods: 86 cases of habitual abortion patients from December 2013 to December 2016 in our hospital were divided into control group and experimental group according to random number method, 43 cases in each group. The control group was treated with progesterone, and the experimental group was treated with Yunkang oral liquid based on the control group. The curative effect, clinical symptom score, TGF-beta 1, EmAb, PIBF, ACA and adverse reactions were observed and compared between the two groups. Results: Total effective rate of experimental group was higher than the control group, the difference was statistically significant (P<0.05). One week and two weeks after treatment, the scores of clinical symptoms, serum EmAb and ACA levels in the two groups were significantly lower than those before treatment. The above indexes in the experimental group were lower than those in the control group. The levels of serum TGF-beta 1 and PIBF in the two groups were significantly higher than those before treatment. The above indexes in the experimental group were higher than those in the control group. The scores of clinical symptoms, TGF-beta 1, EmAb, PIBF and ACA were in the two groups. There were significant differences in different treatment methods and treatment time (P < 0.05). There was no significant difference in adverse reactions between the two groups (P > 0.05). Conclusion: Progesterone combined with Yunkang oral liquid is better than progesterone in the treatment of habitual abortion. It can promote the expression of TGF-beta 1 and PIBF, reduce the levels of EmAb and ACA, and play a role in fetal protection.

1438 Curative Efficacy of Tacrolimus combined with Irbesartan in Treatment of Diabetic Nephropathy and Effects on Serum Angptl4 GDF-15 and CTGF Levels

Objective: To study the curative efficacy of tacrolimus combined with irbesartan in treatment of diabetic nephropathy and effects on serum angiopoietin-like protein 4 (Angptl4), growth differentiation factor-15(GDF-15) and connective tissue growth factor(CTGF) levels. Methods: 105 patients of diabetic nephropathy who received therapy from August 2016 to August 2018 in our hospital were selected as research objects, according to the random number table,those patients were divided into the 55 cases of the observation group and the 50 cases of the control group, they were received routine treatment, the control group was treated with irbesartan on the basis of the above, while the observation group was treated with tacrolimus injection on the basis of the control group, they were treated for 21 days. The changes of serum Angptl4, GDF-15, CTGF (Enzyme-linked immunosorbent assay), blood sugar, renal function and adverse reactions were observed in both groups. Results: After treatment, the serum Angptl4, GDF-15 and CTGF levels in the observation group were significantly lower than those in the control group (P<0.05); there was no significant difference in fasting blood glucose (FBG), postprandial blood glucose (2hPG) and glycosylated hemoglobin (HbA1c) between the two groups before and after treatment (P>0.05); the urea nitrogen (BUN), serum creatinine (SCr) and 24-hour urinary microprotein excretion rate (UAER) in the observation group were significantly lower than those in the control group (P<0.05); there was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion: Tacrolimus combined with irbesartan is well for diabetic nephropathy, which can effectively reduce the expression of serum Angptl4, GDF-15 and CTGF, improve renal vascular function and promote the recovery of renal function, and without increasing adverse drug reactions, it has high clinical value.

1442 Application of Benazepril in Maintenance Hemodialysis Patients and Its Effect on Insulin Resistance and Lipid Metabolism Disorder

Objective: To study application of benazepril in maintenance hemodialysis patients and its effects on insulin resistance and lipid metabolism disorder. Methods: 85 patients who underwent maintenance hemodialysis in our hospital from March 2015 to March 2017 were selected for the study, the randomized table was divided into the observation group (n=45) and the control group (n=40). The control group was treated with maintenance hemodialysis, and the observation group was treated with benazepril on the basis of the control group. The changes of fasting insulin (FINS), insulin resistance index (homa-ir), triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (ldl-c), high-density lipoprotein cholesterol (hdl-c), connective tissue growth factor (CTGF), endothelin-1 level and adverse reactions were compared between the two groups after treatment. Results: Before treatment, there was no significant difference in the level of FINS and homa-ir between the two groups. After treatment, the levels of females and homa-ir in both groups significantly decreased, and the levels of FINS and homa-ir in the observation group were significantly lower than those in the control group (P < 0.05). Before treatment, there was no significant difference in serum lipid level between the two groups. After treatment, serum lipid levels of both groups increased, and HDL-C decreased. TC, TG and ldl-c levels of the observation group were significantly lower than those of the control group (P < 0.05). There was no statistically significant difference in HDL-C level (P>0.05). Before treatment, there was no significant difference in serum CTGF and et-1 level between the two groups. After treatment, the serum CTGF and et-1 levels of both groups were significantly decreased, and the serum CTGF and et-1 levels of the observation group were significantly lower than those of the control group (P<0.05). The total incidence of dizziness, diarrhea, hypotension and hyperkalemia in the two groups was 8.89% and 22.50%, respectively, without significant difference (P>0.05). Conclusion: Benazepril is effective in the treatment of maintenance hemodialysis patients, which can effectively improve insulin resistance and lipid metabolism disorders, and is worthy of clinical application.

1446 Effects of Different Doses of Oxycodone After Anesthesia Induction on the Stress Response of Tracheal Intubation and Hemodynamics in Patients with Surgery

Objective:To explore the clinical effects of different doses of oxycodone in general anesthesia of surgery. Methods:116 patients with general anesthesia of surgery were selected for the study and were randomly divided into high-dose group and low-dose group, with 58 cases in each group. High-dose group was given 0.3 mg/kg oxycodone after anesthesia induction, and low-dose group was given 0.15 mg/kg oxycodone. Hemodynamics parameters [heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP)] were compared between the two groups before intubation (T0), immediately after tracheal intubation (T1), at 1min after tracheal intubation (T2), at 3min after tracheal intubation (T3) and at 10min after tracheal intubation (T4). The stress response indicators [serum norepinephrine (NE), plasma cortisol (COR), blood glucose (GLU)] were analyzed at T0 and immediately after extubation (T5) in the two groups, and the basic indicators during peri-recovery period and incidence rate of adverse drug reactions within 10min after injection were observed in the two groups. Results:At T1, the levels of HR, SBP and DBP in the two groups were significantly decreased compared with those at T0 (P<0.05), but there were no significant differences between the two groups (P>0.05). At T2 and T3, the levels of HR, SBP and DBP in the two groups were significantly increased compared with those at T0, and the levels in high-dose group were significantly lower than those in low-dose group (P<0.05). At T4, the levels of HR, SBP and DBP in the two groups were significantly decreased compared with those at T0, and the levels in high-dose group were significantly lower than those in low-dose group (P<0.05). At T5, the levels of NE, COR and GLU in the two groups were significantly increased compared with those at T0, and the levels in high-dose group were significantly higher than those in low-dose group (P<0.05). There were no significant differences between the two groups in the spontaneous breathing time, duration of eye opening, duration of fist making and time of returning to the ward during peri-recovery period (P>0.05). Within 10 min after injection, there were no significant differences in the incidence rates of hypotension, glossocoma, bucking and hypercapnia between the two groups (P>0.05). Conclusions: High dose of oxycodone after anesthesia induction can effectively inhibit the stress response of tracheal intubation and promote hemodynamic stability, and it is beneficial to maintain the anesthesia depth suitable for surgical operation.

1451 Clinical Value of Combined Detection of sTREM-1 ESM-1 and PCT in Early Diagnosis and Prognosis of Sepsis

Objective: To observe the combined detection of soluble myeloid trigger receptor-1 (sTREM-1), endothelial cell-specific molecule-1 (ESM-1) and procalcitonin (PCT) in the early diagnosis and prognosis in patient with sepsis. Methods: 136 patients with sepsis in ICU, from January 2015 to December 2017, were selected as sepsis group. In the same period, 56 patients with systemic inflammatory response syndrome (SIRS) who had not developed sepsis in the ICU and 30 healthy people were selected as non-sepsis group and control group. The differences of serum sTREM-1, ESM-1 and PCT levels were compared among sepsis group, non-sepsis group and control group. The analysis of variance was used to compare the serum levels in sepsis patients with different severity. The Spearman correlation analysis was used to compare the indicators. Logistic regression and receiver operating characteristic curve of sTREM-1, ESM-1 and PCT were used to value the efficacy in diagnosing sepsis and predicting prognosis. Results: The serum levels of sTREM-1, ESM-1, and PCT in the sepsis group were significantly higher than those in the non-sepsis group and the control group (P<0.01), whereas levels in the non-sepsis group were significantly higher than those in the control group (P<0.01). The serum sTREM-1, ESM-1, and PCT levels increased in sepsis patients with the severity of sepsis (P<0.01). The serum levels of sTREM-1, ESM-1 and PCT in the death group were significantly higher than those in the survivor group (P<0.01). The serum levels of sTREM-1 in sepsis patients were positively correlated with ESM-1 (r=0.684, P<0.01) and PCT (r=0.716, P<0.01), and the positive correlation between ESM-1 and PCT levels (r= 0.754, P < 0.01). In the predictive diagnosis of sepsis, the area under the curve of the combined detection was 0.957, the sensitivity was 82.4%, and the specificity was 98.2%. It was significantly better than sTREM-1 (Z=4.840, P<0.01), and ESM-1 (Z=4.350, P<0.01) and PCT (Z=4.554, P<0.01), but there was no significant difference in the area under the curve among sTREM-1, ESM-1 and PCT (P>0.05). In predicting the prognosis, the area under the curve of the combined detection was 0.969, the sensitivity was 95.8% and the specificity was 96.9%, it was significantly better than sTREM-1 (Z=3.9923, P<0.01), and ESM-1 (Z=4.487, P<0.01) and PCT (Z=2.617, P<0.01), but there was no significant difference in the area under the curve among sTREM-1, ESM-1 and PCT (P>0.05). Conclusion: sTREM-1, ESM-1 and PCT are involved in the development of sepsis diseases. The combined detection is obviously superior to the detection of single markers. It has important clinical value for early diagnosis and prognosis prediction.

Objective: To study clinical value of serum creatine kinase, hs-CRP, lipoprotein a and homocysteine in hypertensive patients. Methods: 120 patients of hypertension who received therapy from April 2017 and April 2018 in our hospital were selected as research objects as the high blood pressure group, the same choice in our normal physical examination of 100 patients of healthy subjects as control group, the detection of two groups of serum CK, hs - CRP, Lp(a), Hcy levels, using ROC curve analysis of CK, hs - CRP, Lp(a), Hcy predict clinical value in high blood pressure. Results: The serum CK, hs-crp, Lp(a) and Hcy levels of the high blood pressure group were significantly higher than those of the control group (P<0.05). According to the ROC curve analysis, the area under the hypertensive curve predicted by CK, hs-crp, Lp(a) and Hcy was 0.638, 0.781, 0.725 ,0.731. The accuracy rate was 54.74%, 75.45%, 70.09% , 69.09%. The curve area of the four groups combined to predict hypertension was 0.878, and the accuracy rate was 83.18, both of which were higher than the single prediction, and the difference was significant (P<0.05). Conclusion: Serum CK, hs-crp, Lp(a) and Hcy levels of hypertensive patients were significantly increased, and the detection of CK, hs-crp, Lp(a) and Hcy had an important clinical effect on the treatment and prevention of hyperemia patients.

1459 Evaluation of Prognosis of Severe Pneumonia by Serum S-ChE LPO Routine Blood

Objective: To study evaluation of prognosis of Severe Pneumonia by Serum S-ChE, LPO, Routine blood. Methods: 175 patients of severe pneumonia who received therapy from May 2016 and May 2018 in our hospital were selected, According to the condition of the disease, it was divided into 75 cases of delayed deterioration group and 100 cases of improvement group. Serum S-ChE, LPO and blood routine (WBC, PLT) levels of the two groups were detected. Results: Serum s-ChE and LPO levels in the worsening group were higher than the improving group (P<0.05), but there was no significant difference between the two groups (P<0.05). According to ROC curve analysis, the area under the ROC curve of s-ChE in evaluating the prognosis of severe pneumonia was 0.857, with sensitivity of 75.34%, specificity of 79.39%, accuracy rate of 78.29%, evaluation curve area of LPO was 0.772, sensitivity of 73.68%, specificity of 72.03% and accuracy of 72.57%. Blood routine evaluation curve area was 0.643, sensitivity 51.92%, specificity of 60.98% , accuracy 58.29%, and the combined prediction curve area was 0.913, sensitivity 81.58%, specificity 86.87% and accuracy 84.57%, all of which were higher than individual prediction, with significant difference (P<0.05). Conclusion: The prognosis of severe pneumonia in the elderly is related to changes in serum S-ChE and LPO levels of patients, and has no obvious relationship with blood routine. It is of great clinical significance to jointly evaluate the prognosis of severe pneumonia in the elderly.

1462 Study on the Efects of Different Types of Cerebral Infarction on Progressive Ischemic Stroke

Objective: To study the predictive value of different cerebral infarction classification on progressive ischemic stroke. Methods: 143 patients of acute cerebral infarction who received therapy from in May 2017 to June 2018 in our hospital were selected as research objects , according to the divided into progression with the progress group, were performed on two groups of patients were analyzed, and the statistics of the number of cases, the single factor analysis of parting, selected from progressive ischemic stroke risk factors, multiple factors analysis, set up different combination, so as to calculate the prediction accuracy. Results: After analysis, LAA and SAO subtypes in TOAST classification, and large infarction, middle infarction and lacunar infarction in CT classification. The comparison between the four subtypes of OCSP and LAA, PAD and UE subtypes of CISS subtypes had statistical significance (P<0.05). After analysis, the CAP scores of PACI subtype, TACI subtype, CT classification, large infarction, lacunar infarction subtype, CISS classification, UE subtype is an independent risk factor for ischemic stroke progression (P< 0.05). ROC curve analysis showed that the area under the ACU of PACI subtype, TACI subtype, large infarction, lacunar infarction subtype and UE subtype combined detection was significantly different (P>0.05), sensitivity and specificity of combined detection. Both were better than the individual detection (P<0.05). Conclusion: There is a close relationship between cerebral infarction classification and progressive ischemic stroke, which can predict the progression of stroke, and the combination model of TACI subtype +PACI subtype +UE subtype + lacunar infarction is the optimal model to predict progressive ischemic stroke.

1470 Expression of Foxp3 and beta-catenin in Breast Cancer and the Relationship with Clinical Prognosis

Objective: To discuss the expression of forked head / wing like spiral transcription factor (Foxp3) and beta catenin in breast cancer and the relationship with clinical prognosis. Methods: The expression of Foxp3 and beta-catenin in 80 cases of breast cancer and adjacent normal tissues in our hospital from August 2016 to August 2018 was detected by immunohistochemistry. The relationship between the expression of Foxp3 and beta-catenin and pathological characteristics and survival and prognosis of breast cancer was analyzed. Results: The positive rates of Foxp3 and beta-catenin expression in breast cancer tissues were significantly higher than those in adjacent normal tissues, the difference was statistically significant (P < 0.05). The expressions of Foxp3 and beta-catenin in breast cancer tissues were correlated with clinical stage, pathological grade and lymph node metastasis (P < 0.05). The expressions of Foxp3 and beta-catenin in breast cancer tissues were positively correlated (P < 0.05). The survival curve analysis showed that, the 1-year survival rate of patients with positive expression of Foxp3 and beta-catenin was significantly lower than those patients with with negative expression of Foxp3 and beta-catenin, the difference was statistically significant (P < 0.05). Conclusions: Foxp3 and beta-catenin are related to the occurrence and development of breast cancer, and their expressions are positively correlated, suggesting that the detection of their expressions can be used as the important index for evaluating the clinical prognosis of breast cancer.

Objective: Discuss the dynamic changes of TNF-α and INF-γ in peripherd blood of patients with lung cancer before and after different dose radiotherapy. Methods: To detect the content of IFN-r、TNF-a in Peripheral blood of 30 lung cancer radiotherapy patients by ELISA before radiotherapy 1 weeks after radiotherapy, 2 weeks, 4 weeks and 6 weeks. Results: The concentration of IFN-r increased gradually at 2 000cGy, 4000cGy and 6000cGy by radiotherapy. F=961.32, P < 0.001. The concentration of TNF-a decreased gradually at 2 000cGy, 4000cGy and 6000cGy by radiotherapy. The F values were 734.85, respectively, and P values were P < 0.001. Conclusion: Radiotherapy can enhance the level of INF-r with lung cancer patients, and reduce the level of TNF-a.

1477 Effect Analysis of Aspirin Combined with Edaravone on Serum Hcy in Patients with Acute Cerebral Infarction

Objective: To explore the effect of aspirin combined with edaravone on serum Hcy in patients with acute cerebral infarction. Methods: 70 patients with acute cerebral infarction who were treated in our hospital from March 2015 to December 2015 were selected as the research objects,the patients were divided into observation group (n=37) and control group (n=33) by random number table method. The control group was treated with aspirin, and the observation group was treated with edaravone on the basis of the control group. The clinical efficacy, serum Hcy, NIHSS score, modified Barthel index and the incidence of adverse reactions were compared between the two groups. Results: The total effective rate of the two groups was 94.59% and 69.70%, respectively (P < 0.05). After treatment, serum Hcy level in both groups was significantly decreased, and serum Hcy level in the observation group was significantly lower than that in the control group (P < 0.05). After treatment, NIHSS and improved Barthel index were significantly improved in both groups, and the improvement degree of NIHSS and improved Barthel index in the observation group was better than that in the control group (P < 0.05). The total incidence of adverse reactions in the two groups was 8.11% and 36.36%, with significant difference (P < 0.05). Conclusion:edaravone combined with aspirin in patients with acute cerebral infarction has a significant effect, which can improve the serum Hcy level of patients and improve the prognosis of patients with acute cerebral infarction, which is worthy of clinical promotion.

1480 Expression and Clinical Significance of Artemin, CXCL13 and MMP-9 in Endometrial Carcinoma

Objective: To investigate the expression of Artemin, CXCL13 and MMP-9 in endometrial carcinoma, atypical hyperplasia, simple hyperplasia and normal endometrial tissue, and in different clinicopathological features of endometrial carcinoma. The relationship between differential expression and clinical pathological staging of endometrial carcinoma. Methods: A total of 154 cases of endometrial specimens were removed from the Department of Gynecology, Affiliated Hospital of Chengde Medical College from June 2016 to April 2018, including 78 cases of endometrial carcinoma, 56 cases of stage I (EC-I group), 15 cases of stage II (EC-II group), 7 cases of stage III (EC-III group), 32 cases of atypical proliferative endometrial tissue (EAH group), 46 cases of simple proliferative endometrial tissue (ESH group), normal endometrial tissue in 44 cases (NE group). The expression of Artemin, CXCL13 and MMP-9 in endometrial carcinoma, atypical hyperplasia, simple hyperplasia and normal endometrium was detected by Western Blot. Real-time quantitative PCR was used to detect Artemin, CXCL13 and MMP-9 mRNA. The expression of different clinicopathological features in endometrial carcinoma tissues was analyzed, and the relationship between differential expression and clinical pathological staging of endometrial carcinoma was analyzed. Results:: The expression of Artemin, CXCL13 and MMP-9 in the EC group were higher than those in the EAH group, ESH group and NE group (F=38.54, F=98.74, F=72.46, all P<0.05). The expression of Artemin, CXCL13 and MMP-9 mRNA in endometrial carcinoma was correlated with depth of myometrial invasion, pathological differentiation, FIGO stage and lymph node metastasis (all P <0.05), which was not correlated with age (P >0.05). Conclusion: Artemin, CXCL13 and MMP-9 play a role in the development of endometrial carcinoma, which may provide new research direction for early diagnosis and prognosis of endometrial carcinoma.

1484 Comparison of Effects and Safety of Hydrochlorothiazide or Indapamide Combined with Benazepril in the treatment of Patients with Hypertension Complicated with Heart Failure

Objective: To compare the effects and safety of hydrochlorothiazide or indapamide combined with benazepril in the treatment of patients with hypertension and heart failure. Methods: 120 patients with hypertension and heart failure from January 2016 to July 2018 were selected and randomly divided into two groups. Control group (60 cases) was given hydrochlorothiazide combined with benazepril, and observation group (60 cases) was given indapamide combined with benazepril, and they were treated for 12 w. The changes of diastolic blood pressure, systolic blood pressure and 6min walking distance before and after treatment were compared between the two groups. The heart function heart rate, left ventricular early diastolic and advanced diastolic blood flow peak velocity ratio (E/A), left ventricular ejection fraction (LVEF), left ventricular end-systolic diameter (LVESd) and left ventricular end-diastolic diameter (LVEDd) were evaluated. The levels of serological indicators of serum endothelin 1, cortisol, brain natriuretic peptide, angiotensin II (Ang II) and norepinephrine (NE) and oxidative stress indicators of malondialdehyde (MDA), lipid hydrogen peroxide (LHP), glutathione peroxidase (GSH-Px) and total antioxidant capacity (T-AOC) were detected. The incidence rate of adverse reactions was analyzed in the two groups. Results: The systolic blood pressure, diastolic blood pressure, E/A, LVESd and LVEDd index index, levels of serum endothelin 1, cortisol, brain natriuretic peptide, NE, Ang II, MDA and LHP in observation group were decreased significantly after treatment. The 6min walking distance was significantly prolonged. The LVEF, GSH-Px level and T-AOC index were significantly increased (P<0.05). The adverse reactions of the two groups were mainly hyponatremia, hypokalemia and hyperuricemia (P>0.05). Conclusions: Indapamide combined with benazepril in the treatment of hypertension patients with heart failure is better than hydrochlorothiazide combined with benazepril, the total effective rate is higher.

Objective: To study the curative efficacy of coenzyme Q10 injection combined with levocarnitine in treatment of senior chronic pulmonary heart disease with cardiac insufficiency. Methods: 86 patients of senior chronic pulmonary heart disease with cardiac insufficiency who received therapy from January 2015 to May 2018 in our hospital were selected as research objects, according to random number table, those patients were divided into the 45 cases of the observation group and 41 cases of the control group, on the basis of routine treatment, the control group was treated with levocarnitine, while the observation group was treated with coenzyme Q10 injection on the basis of the control group, they were treated for 3 weeks. The clinical efficacy, hemorheology, blood gas analysis, cardiac function and adverse reactions were compared between the two groups. Results: After treatment, the total effective rate of the observation group was significantly higher than those in the control group (P<0.05); The whole blood viscosity, high shear rate, low shear rate, plasma viscosity and whole blood reduced viscosity in the observation group were significantly lower than those in the control group (P<0.05); the partial pressure of oxygen (PaO2) in the observation group was significantly higher than those in the control group, and the partial pressure of carbon dioxide (PaCO2) in the observation group was significantly lower than those in the control group (P<0.05); the heart rate (HR) and left ventricular end-diastolic diameter (LVEDD) in the observation group were significantly lower than those in the control group, the left ventricular ejection fraction (LVEF) was significantly higher than those in the control group, and the 6-minute walking distance was significantly longer than those in the control group (P<0.05); there was no significant difference in adverse reactions between the two groups during treatment (P>0.05). Conclusion: Coenzyme Q10 injection combined with levocarnitine is well for senior chronic pulmonary heart disease with cardiac insufficiency, and has good drug safety

1493 Expression of Vascular Endothelial Growth Factor in Breast Cancer Patients with Different BMI and its Effect on Prognosis

Objective: To investigate the expression of vascular endothelial growth factor in breast cancer patients with different body mass index and its effect on prognosis. Methods:450 cases of female breast cancer patients who met the criteria for surgical treatment from January 2016 to January 2017 were selected as the study subjects. They were divided into the normal body mass index group (18 kg/m2< BMI < 25kg/m2) and the obesity group (BMI ≥ 25kg/m2) according to their body mass index. Vascular endothelial cell growth factor (VEGF) was detected by standard streptomycetin protein-peroxidase affinity immunohistochemistry in cancer tissues of subjects, and was divided into 4 levels according to the ratio of positive cells in the tissues: No positive cells are negative (-); Positive cells < 20% showed low expression (+). 20%~50% of the positive cells were moderately expressed (++) >50% of the positive cells were highly expressed (+++). - ~ + was regarded as low expression of vascular endothelial growth factor, while ++ ~ ++ was highly expressed. All subjects were followed up for 2 years postoperatively, and whether the subjects had recurrence or metastasis during the follow-up period was recorded. Results: The expression of vascular endothelial growth factor (VEGF) was 38.9% in the normal BMI group and 75.4% in the overweight and obese groups. In the patients with normal BMI, there was no statistically significant difference in 2-year disease-free survival between the patients with high VEGF expression and those with low VEGF expression (P >0.05). In the overweight and obese breast cancer group, the 2-year disease-free survival period of the patients with high VEGF expression was shorter than that of the patients with low VEGF expression, and the difference was statistically significant (P <0.05). Compared with the patients with low VEGF expression in the normal BMI group, the difference in 2-year disease-free survival was statistically significant in the overweight and obese breast cancer group (P <0.05). Compared with patients with high VEGF expression in the normal BMI group, patients with high VEGF expression in the overweight and obese breast cancer group also had a statistically significant difference in 2-year disease-free survival (P <0.05). Conclusion: The expression of vascular endothelial growth factor (VEGF) is increased in obese breast cancer patients, and its expression level is significantly correlated with the prognosis of breast cancer, which suggests that overweight and obese breast cancer patients should actively control their weight to prevent recurrence and metastasis of breast cancer.

1496 Study on the Value of GeneXpert MTB /RIF Assay in the Diagnosis of Tuberculous Pleurisy

Objective: To investigate the value of GeneXpertMTB/RIF system in the diagnosis of tuberculous pleurisy. Methods: 100 cases of highly suspected tuberculous pleurisy were collected. The positive rate of pleural effusion samples detected by acid-fast staining smear method, adenosine deaminase (ADA) detection method and GeneXpert MTB/RIF system were compared. The magnitude of detection results by GeneXpert MTB/RIF system was compared, and the drug resistance of samples to rifampicin was detected. Results: Among the 50 patients in the control group, none of the three methods were detected. In the experimental group, the positive detection rates of the smear staining method, the ADA method, and the GeneXpert MTB/RIF system were 39%, 61%, and 71%, respectively. The positive detection rate of GeneXpert MTB/RIF system was higher than that of smear staining and ADA staining, and the difference was statistically significant. GeneXpert MTB/RIF assay was more sensitive to the test specimens; GeneXpert MTB/RIF assay specimens were 83.3% (10/12) sensitive to rifampicin resistance and 95.74% (45/47) specific. Conclusion: GeneXpert MTB/RIF technology is a simple, safe and efficient molecular biological detection method. It has great value for the early diagnosis and treatment of tuberculous pleurisy, and it is worthy of clinical promotion.

1499 The Correlations Between Serum 25-(OH)D3 with IFN-γ and IL-10 Levels in Patients with Tuberculous Pleurisy

Objective: To analyze the correlations between serum 25-hydroxyvitamin D3 [25-(OH)D3] with interferon-γ (IFN-γ) and interleukin-10 (IL-10) levels in patients with tuberculous pleurisy and explore its significance. Methods: 73 patients with tuberculous pleurisy who first diagnosed in our hospital from January 2017 to May 2018 were selected as the case group, and 60 another healthy people were as the control group, the levels of serum 25-(OH)D3, IFN-γ and IL-10 were detected by enzyme-linked immunosorbent assay (ELISA), the correlations between serum 25-(OH)D3 with IFN-γ and IL-10 were analyzed. Results: The levels of serum 25-(OH)D3 and IL-10 in the case group were lower than those in the control group (P < 0.05), and the level of serum INF-γ was higher than that in the control group (P < 0.05); the levels of serum 25-(OH)D3, INF-γ and IL-10 in different stages of tuberculous pleurisy were significantly different (P<0.05); with the aggravation of the disease, the serum 25- (OH) D3 and IL-10 levels gradually decreased (P < 0.05), the level of INF-γ increased gradually (P < 0.05); the levels o serumf 25-(OH)D3 and IL-10 were increased (P < 0.05) and INF-γ was decreased (P < 0.05) in the case group after treatment; serum 25-(OH)D3 was negatively correlated with INF-γ (r=-0.562, P=0.007), and positively correlated with IL-10 (r=0.668, P=0.001) in patients with tuberculous pleurisy. Conclusion: Serum 25-(OH)D3 level is decreased in patients with tuberculous pleurisy, it is related to the abnormal levels of INF-γ and IL-10, it is suggested that 25-(OH)D3 deficiency may be an important mechanism of immune dysfunction in tuberculous pleurisy.

1503 Analysis on Related Factors of Capsule Retention and Incomplete in Patients with CD by Small Intestine Capsule Endoscopy

Objective: To analyze the clinical characteristics of patients with Crohn's disease (CD) who underwent small intestine capsule endoscopy and the related factors of capsule retention and incompleteness. Methods: A retrospective study was conducted to collect clinical data of 106 CD patients who underwent small intestinal capsule endoscopy in our hospital. The clinical characteristics of the patients were recorded. The number of cases of capsule retention and incompleteness was counted. Multivariate logistic regression was used to analyze the influencing factors of capsule retention and incompleteness in small intestine capsule endoscopy. Results: Among 106 CD patients, 13 cases had not completed the small intestine capsule endoscopy, and the rate of incompleteness was 12.26%. Multivariate logistic regression analysis showed that gender, albumin, hemoglobin and diarrhea were related factors affecting the incompleteness of small intestinal capsule endoscopy in CD patients (P < 0.05). Among 106 CD patients, 6 cases were retained in small intestine capsule endoscopy, the retention rate was 5.66%. Of the 6 patients with capsule detention, 4 had no clinical symptoms (capsules were discharged by themselves after drug treatment), 2 had secondary intestinal obstruction (capsules were taken out by endoscopy). After symptomatic treatment, they were discharged smoothly. Conclusion: Gender, albumin, hemoglobin and diarrhea are all related factors affecting the incompleteness of small intestinal capsule endoscopy in CD patients. Before capsule endoscopy, we should pay close attention to the gastrointestinal motility and nutritional status of patients, exclude intestinal stricture, reduce the incomplete rate of examination as much as possible, and reduce the incidence of capsule retention.

Objective: To study the optimal minimum dose of cis-atracurium in laryngoscopic surgery for polyps of vocal cord. Methods: 120 patients who underwent laryngoscopic surgery for polyps of vocal cord from January 2017 to September 2018 were divided into two groups: group I (cis-atracurium 0.05 mg/kg during operation) and group II (cis-atracurium 0.075 mg/kg during operation), 60 cases in each group. The changes of heart rate (HR), mean arterial pressure (MAP), systolic blood pressure (SBP) and diastolic blood pressure (DBP) at different time points in different patients were compared. The degree of muscle relaxation at different time points was monitored and compared. The time from anesthesia to tracheal intubation, operation, drug withdrawal to spontaneous breathing recovery, extubation and eye opening were also compared between the two groups. Results: There was no significant difference in MAP, DBP and HR between the two groups at different time points (P> 0.05). SBP and DBP decreased significantly 1 min before intubation (T2) compared with after stable entry (T1) (P< 0.05). The onset time of T1 in group I was significantly longer than that in group II. The lowest value in T1 operation, T1 value in tracheal intubation, T1 value in laryngoscope insertion, T1 value in laryngoscope removal and TOF value in series were significantly higher than those in group II (P< 0.01). The time from induction of anesthesia to tracheal intubation, from drug withdrawal to recovery of spontaneous breathing, extubation and eye opening in group I were significantly shorter than those in group II (P< 0.01), but there was no significant difference in the operation time between the two groups (P> 0.05). Conclusion: The application of cis-atracurium in resection of polyp of vocal cord under self-retaining laryngoscope is in line with remifentanil and propofol target-controlled intravenous anesthesia. The intraoperative circulation is stable. Although the low dose of cis-atracurium (0.05 mg/kg) can not completely prevent muscle tremor, it can meet the needs of muscle relaxation in tracheal intubation and self-retaining laryngoscope operation and reduce the anesthesia time of operation.

1512 Determinations and Significances of PlGF, IL-1β and TGF-β2 in Aqueous Humor and Plasma of Patients with Neovascular Glaucoma

Objective: To detect the levels of placental growth factor (PlGF), interleukin-1β (IL-1β), and transforming growth factor (TGF-β2) in aqueous humor and plasma of patients with neovascular glaucoma (NVG), and to explore the relationships between the levels of PlGF, IL-1β and TGF-β2 in aqueous humor and plasma. Methods:42 patients (42 eyes) diagnosed as NVG in our hospital from April 2016 to September 2018 were selected, 50 cases (50 eyes) of patients with age-related cataract (ARC) were selected as control group, enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of PlGF, IL-1β and TGF-β2 in aqueous humor and serum. Results: The levels of PlGF, IL-1β and TGF-β2 in aqueous humor and serum of patients in group NVG were significantly higher than those of ARC patients (P < 0.05); the levels of PlGF and TGF-β2 in aqueous humor were significantly higher than those in serum in group NVG (P < 0.05); there was no significant change in the levels of PlGF and TGF-β2 between aqueous humor with serum of ARC patients (P>0.05); the level of IL-1β in the two groups was significantly higher than that in the serum (P<0.05). Pearson test showed that the levels of PlGF, IL-1β and TGF-β2 in aqueous humor and serum of NVG patients were positively correlated (P < 0.05). Conclusion: The levels of PlGF, IL-1β and TGF-β2 in aqueous humor and serum of patients in group NVG are significantly higher than those in control group, it may be related to NVG disease.

1515 Comparative Analysis of two Different Surgical Approaches for the treatment of Lumbar Disc Herniation by Foramen Endoscopy

Objective: To compare the curative effect and safety of foraminal endoscope on lumbar disc herniation (LDH) through two different surgical approaches. Methods: The clinical data of 157 patients with LDH who were admitted to the hospital from March 2015 to October 2018 were analyzed retrospectively. The patients were divided into foraminal group (foraminal endoscope through foraminal approach surgery, n=76) and laminar group (foraminal endoscope through laminar space approach surgery, n=81) according to different treatment methods. The general surgical conditions, surgical effects, score of visual analogue pain scale (VAS), fear visual analogue scale (FAVS), Oswestry dysfunction index (ODI), incidence of complication and recurrence rate were compared between the two groups. Results: There was no significant difference in operation time or intraoperative blood loss between the two groups (P>0.05). The leaving bed time in foraminal group was significantly shorter than that in laminar group, while treatment cost was significantly lower than that in laminar group (P<0.05). There was no significant different in postoperative total good rate between the two groups (P>0.05). VAS score and ODI score of both groups were significantly decreased after surgery. The intraoperative VAS score in foraminal group was significantly higher than that in laminar group, and preoperative FAVS was significantly higher than that in laminar group (P<0.05). There was no significant difference in VAS score or ODI score between the two groups (P>0.05). There was no significant difference in total incidence of complications or recurrence rate between the two groups (P>0.05). The incidence of tissue damage in the posterior spine in laminar group was significantly higher than that in foraminal group (P<0.05). Conclusion: The effects of the two surgical approaches are comparable in terms of clinical curative effect, reducing postoperative pain and function improvement for LDH patients. However, laminar approach can more significantly reduce intraoperative pain and preoperative fear, suitable for patients with L5S1 segment and high ilium cliff. And foraminal approach surgery is suitable for patients who cannot tolerate general anesthesia.

Objective: To study the efficacy and safety of pregabalin combined with intradermal injection in the treatment of acute herpetic pain and postherpetic neuralgia (PHN). Methods: A retrospective analysis was made of 195 patients with acute herpes zoster pain and postherpetic neuralgia hospitalized in the Pain Department of the First Affiliated Hospital of Harbin Medical University from January 2014 to August 2018. Among them, 68 patients suffered from acute herpes zoster pain and 127 patients suffered from postherpetic neuralgia. After admission, all patients were treated with oral pregabalin combined with intradermal injection. Visual analogue scale (VAS) and quality of sleep (QS) were observed before treatment, 7 days, 14 days, 1 month and 3 months after treatment. The therapeutic effect and adverse reactions were analyzed comprehensively. Results: The VAS score was significantly lower (P<0.01) and the quality of sleep was significantly improved (P<0.01) after 7 days, 14 days, 1 month and 3 months treatment .No serious adverse reactions was observed during the treatment. Conclusion: Oral prebahrain combined with intradermal injection has significant clinical effect on both acute herpetic pain and PHN, and can significantly improve the patients' sleep quality with less adverse reactions.It is worthy of clinical promotion.

1523 Clinical Study on Shock Wave Acupoint Therapy for Knee Osteoarthritis

Objective: To observe the clinical effect of shock wave stimulation acupoint therapy in the treatment of osteoarthritis of knee joint. Methods: 150 patients with osteoarthritis of knee joint were randomly divided into two groups, 75 cases of treatment group, shock wave acupuncture point therapy, along the foot Yangming stomach meridians, foot Taiyin spleen meridians, foot Shaoyang bile meridians and corresponding acupoints impact;75 cases of contrast group, shock wave conventional therapy was used to impact the pain points around the knee joint. In both groups, 4d was treated once, 5 times was 1 course, and 3 courses were treated. The knee joint function was evaluated by WOMAC score. Results: The pain and knee function of the two groups improved after treatment. The WOMAC score acupuncture point group was lower than the normal pain point group, and the total effective rate(92.00 %) was higher than that of the pain point impact group(76.00%). Conclusion: After treatment, the pain and knee joint function of the two groups were improved. WOMAC score was lower in the acupoint group than in the conventional pain point group, and the total effective rate in the acupoint impact treatment group (92.00%) was higher than that in the conventional pain point impact control group (76.00%).

1526 Analysis of Early Risk Factors for Pleural Effusion in Patients with Acute Pancreatitis

Objective:To investigate the early risk factors for pleural effusion in patients with acute pancreatitis. Methods: Clinical data of 73 patients with acute pancreatitis treated in our hospital from October 2016 to August 2018 were retrospectively analyzed. Independent risk factors for pleural effusion in patients with acute pancreatitis were analyzed by chi-square test and logistic regression. Results: Univariate analysis showed that pleural effusion was associated with WBC, CRP, MCTSI scores and peritoneal effusion (all P<0.05), but not with gender, age, BMI, blood glucose, blood Ca2+, blood amylase and alanine aminotransferase (all P>0.05). The WBC, CRP and MCTSI scores in the pleural effusion group were significantly higher than those in the non-pleural effusion group (all P<0.05). However, there were no significant differences in blood glucose and blood Ca2+ levels between the two groups (all P>0.05). Multivariate analysis showed that MCTSI score was an early independent risk factor for pleural effusion in patients with acute pancreatitis (P=0.034, OR=5.311, 95% C.I. : 1.133-24.893). Conclusion: MCTSI score is an early independent risk factor for pleural effusion in patients with acute pancreatitis, and has clinical guiding significance for early prediction and intervention.

1529 Improvement of Facial Aging in Patients with Skin Aging treated by Ultra-pulsed Carbon Dioxide Lattice Laser combined with Wrinkle Eeduction by Injection

Objective:To study improvement of facial aging in patients with skin aging treated by ultra-pulsed carbon dioxide lattice laser combined with wrinkle reduction by injection. Methods:58 patients from March 2017 to March 2018 were selected. The patients were divided into observation group (n=30) and control group (n=28) according to the random number table method. The control group was treated with wrinkle reduction by injection, while the observation group was treated with super pulsed carbon dioxide lattice laser on the basis of the control group. The curative effect, skin aging score and total incidence of adverse reactions were observed. Results: After treatment, The total effective rate of observation group was significantly higher than that of control group[93.33%(28/30)vs 64.28%(18/28)](P<0.05). Skin aging score was significantly lower than that of control group[(0.98±0.30)scores vs(4.26±1.03)scores](P<0.05). The total incidence of adverse reactions was significantly lower than that in the control group[13.33%(4/30)vs 39.28%(11/28)](P<0.05). Conclusion:Ultra-pulsed carbon dioxide lattice laser combined with injection wrinkle-removing therapy has remarkable clinical effect on skin aging patients. It can improve skin aging status, restore gloss, improve wrinkles, shrink pores, etc., with fewer adverse reactions and is safe and reliable.

1532 Clinical Study on Diabetic Intestinal Disease Treated by Pinaverium Bromide Combined with Epalrestat and Methylcobalamin

Objective: To investigate the efficacy and feasible mechanism of pinaverium bromide combined with epalrestat and mecobalamin in the treatment of diabetic bowel disease. Methods: A total of 208 patients with type 2 diabetes mellitus complicated with diabetic bowel disease were selected in outpatient and inpatient treatment in Langfang People’s Hospital. 208 outpatients and inpatients meeting the above criteria were randomly divided into two groups. Each group received 104 patients in the treatment group. In both groups, insulin was administered to control blood glucose, and drugs that affect gastrointestinal motility were discontinued before observation. The observation group was treated with methylcobalamin 0.5 μg 3/day orally; the treatment group was on the basis of 0.5 μg of original mecobalamin 3/day orally, combined with pinaverium bromide 50 mg 3/day and epalrestat 50 mg 3/ Oral treatment daily for 3 months. Results:There was no significant difference in clinical data between the two groups (P>0.05). The efficacy of the two groups was significantly higher than that of the observation group. The difference was statistically significant (P<0.05). The scores of diarrhea and constipation and after treatment were compared between the two groups. The scores after treatment in the treatment group were significantly lower than those in the observation group after treatment. The differences were statistically significant. Significance (P<0.05). Conclusion: Pinaverium bromide combined with Epalrestat and Methylcobalamin have significant curative effects on diabetic bowel disease, which is worthy of popularization and application.

1535 Comparison of the Effects of PASS Appliance and MBT Self-locking Appliance in the treatment of Class I Malocclusion

Objective: To compare the clinical effects of physiological anchorage control (PASS) appliance and MBT self-locking appliance in the treatment of moderately crowded class I extraction cases. Methods: 53 cases of Angle I with moderate crowded dentition requiring extraction were selected. PASS group was treated with PASS appliance and MBT self-locking appliance was treated with MBT self-locking appliance. Lateral cephalometric films were taken before and after treatment, and the difference of curative effect was evaluated by cephalometric analysis. Results: Compared with the MBT self-locking group, the average treatment time of the PASS group, the anterior skull base plane-upper sulcus seat angle (SNA), the anterior skull base plane-lower sulcus seat angle (SNB), the upper coronal seat point-nose root point-lower alveolar seat angle (ANB), mandibular plane-anterior skull base plane angle (MP-SN) of the PASS group showed no significant change (P>0.05); the long axis of the upper central incisor and the angle of the saddle point-nasal root point (U1-SN), the angle of the long central incisor with the mandibular plane (L1-MP), upper middle incisor Long axis and upper coronal seat-nose root point line intersection angle (U1-NA), lower middle incisor long axis and lower alveolar seat point - nose root point line intersection angle (L1-NA) and U1-NA, L1-NA distance of the PASS group was significantly lower (P<0.05); the anterior lower angle of the longitudinal axis of the maxillary first molar and the vertical plane of the PM plane (U6/PM) , and the anterior lower angle of the longitudinal axis of the maxillary first molar and the PP plane (U6/PP) increased, the distance from the mid-abdominal first molar to the PM plane (U6-PM), and the distance from the mesial apical apex of the maxillary first molar to the PM plane (U6R-PM) reduced (P<0.05); the nasolabial angle increased, the upper lip point to the E-line distance (UL-E line), and the lower lip point to the E-line distance (LL-E line) reduced (P<0.05). 1539 Curative Efficacy of Articular Cavity Injection of Sodium Lyaluronate and Ropivacaine combined with Arthroscopic Debridement in treatment of Osteoarthritis of the Knee

Objective: To study curative efficacy of Articular cavity injection of sodium hyaluronate and ropivacaine combined with arthroscopic debridement in treatment of Osteoarthritis of the knee. Methods: 100 cases of knee osteoarthritis patients admitted to our hospital from February 2016 to February 2018 were selected as the research objects,randomized number table method was used to divide the observation group (n=51) and the control group (n=49). The control group was treated with arthroscopic debridement, and the observation group was treated with sodium hyaluronate injection and ropivacaine combined with arthroscopic debridement. After treatment, the efficacy, inflammatory indexes [interleukin-1 beta (il-1), tumor necrosis factor alpha (tnf-alpha), interleukin-6 (il-6)], knee joint function score (HSS), visualanalogue scale of pain (VAS), postoperative recovery time and quality of life of the two groups were compared. Results: After treatment, the excellent and good rates of patients in the two groups were 84.31% and 65.31% respectively, with statistically significant difference (P < 0.05). There was no significant difference in pre-treatment inflammatory index between the two groups. After treatment, the levels of inflammatory indexes in both groups were significantly decreased, and the levels of il-1 beta, tnf-alpha and il-6 in the observation group were significantly lower than those in the control group (P < 0.05). There was no significant difference in VAS and HSS scores between the two groups before treatment. After treatment, VAS score in both groups decreased significantly, and HSS score in the observation group was significantly higher than that in the control group, and VAS score was significantly lower than that in the control group (P < 0.05). After treatment, the postoperative recovery time of the observation group was significantly lower than that of the control group, and the quality of life score was significantly higher than that of the control group (P < 0.05). Conclusion: Intra-articular injection of sodium hyaluronate and ropivacaine combined with arthroscopic debridement in patients with knee osteoarthritis has a significant effect, which can effectively improve the knee function and inflammatory index level of patients, and is worthy of promotion and application.

Objective: To evaluate the difference in Edge sealing effect of Nano-resin filling caries cavity before and after radiotherapy. Methods: 45 teeth were randomly divided into three groups with 15 teeth in each group, i.e. filling before irradiation group, filling after irradiation group and control group.Teeth in the filling before irradiation group were prepared cavities on the buccal side, filled with Filtek Z350, and then irradiated fractionally up to 70Gy.Teeth in the filling after irradiation group were irradiated fractionally up to 70Gy, and then the cavities were prepared and filled.Teeth in the control group were directly prepared and filled.After 500 thermocycle,all samples shall be placed in methylene blue solution with 2.0% of mass fraction, for dyeing for 48 hours. All the teeth were cut through the filling center with the direction of perpendicular to the surface of obturator, vertical to the long axis of the teeth.Finally, the dye penetration depth of samples in each group was scored under microscope and the results were tested by non-parametric method. Results: There was no significant difference in microleakage between filling before irradiation group, filling after irradiation group and control group ( P>0.05). Conclusion: The nano-resin shows a stable filling effect under the influence of radiation. Therefore, no matter before or after radiotherapy, the nano-resin can be used to fill the caries cavity of the patients, which has achieved a better filling effect.

Objective: To investigate the expression of ENA polypeptide antibody spectrum in patients with autoimmune diseases and its correlation with T lymphocyte level. Methods: A total of 100 patients diagnosed with autoimmune diseases from November 2016 to November 2017 were selected as the study group, and 40 healthy medical personnel from our hospital during the same period were selected as the control group. The detection rate of ENA polypeptide antibody and the expression of T lymphocytes of the two groups were analyzed. Results: The positive rate of ENA polypeptide antibody was significant in the study group. At the same time, two or more autoantibodies were detected. The serum of the study group contained a variety of autoantibodies, indicating that the ENA antibody contained the corresponding disease antibody spectrum. Conclusion: Because of the clinical manifestations of autoimmune disease variety, increase the difficulty of early diagnosis, and the ENA polypeptide antibody detection repeatability, no special equipment, less YongXieLiang time, simple operation, good specificity, but also for a variety of autoantibodies to sieving and recognition at the same time, is beneficial to the patients with early diagnosis, is of great help to the follow-up treatment, worth clinical promotion.

1549 Observation of Effect of MSCTA and Intraoperative Ultrasound in Partial Nephrectomy with Retroperitoneal Laparoscopic Occlusion of Accessory Renal Artery

Objective: To investigate the effect of preoperative multi-slice spiral CT angiography (MSCTA) and intraoperative ultrasound on laparoscopic obstruction of renal artery partial nephrectomy (LPN). Methods: According to whether MSCTA and intraoperative ultrasound were performed, 45 cases of small renal cell carcinoma from May 2014 to May 2017 in the Affiliated Hospital of Chengde Medical College were retrospectively reviewed. They were divided into two groups, 23 cases in the study group and 22 cases in the control group. Relevant data were compared between the two groups, including bleeding volume, operation time, intraoperative warm ischemia time, detection rate of microcarcinoma, conversion and opening rate, leakage of urine after operation, positive rate of incision margin, and determination of GFR in operation kidney. Results: The research group and the control group operation time was 115±5.0min and 132±4.8 min, respectively. The intraoperative blood loss was 80±3.0 ml and 108±3.8 ml, respectively. The mean time was 25±2.2 min and 32±2.6 min, respectively, The number of cases of open surgery was 1 and 3, respectively. and the difference was statistically significant (p<0.05). The number of cases of microscopic cancer lesions was 1 and 0, respectively. The number of cases of postoperative leakage was 0 and 1 respectively. The number of positive margins was 0 and 1 respectively. The preoperative and postoperative renal GFR of the research group were 57.1±7.2 ml/min and 56.1±7.7 ml/min (P>0.05), respectively. Renal GFR was 57.8±7.0 ml/min and 53.4±6.5 ml/min (P<0.05), respectively. Conclusion: MSCTA can detect the accessory renal artery provided by the tumor before operation, and guide the timely and accurate control during operation to reduce the amount of bleeding. At the same time, intraoperative ultrasound can quickly and accurately locate the tumor tissue, preserve the nephron as much as possible, and reduce the positive margin of the margin. And is conducive to the recovery of renal function after surgery.

Objective: To study the curative efficacy of laparoscopic appendectomy on appendicitis and its effectsonserum Hypersensitive c-reactive protein (hs-crp). Methods: 95 cases of patients with appendicitis treated in our hospital from April 2015 to April 2018 were selected as study objects,The patients were divided into observation group (n=50) and control group (n=45) according to the simple random number table method. The control group received laparotomy, while the observation group received laparoscopic resection. Operation time, postoperative visual analogue scale (VAS) score, hospital stay, blood loss, postoperative gastrointestinal recovery time, hs-crp level, tumor necrosis factor alpha (tnf-alpha) and postoperative adverse reactions were compared between the two groups. Results: The operation time, intraoperative blood loss and postoperative VAS score of the observation group were significantly lower than those of the control group (P<0.05). After treatment, the levels of hs-crp and tnf-alpha increased significantly in both groups. The levels of hs-crp and TNF- alpha in the observation group were significantly lower than those in the control group (P<0.05). The recovery time of bowel sound, anal exhaust time, anal defecation time and hospitalization time in the observation group were significantly lower than those in the control group (P<0.05). The total incidence of complications in the observation group was 8.00%, significantly lower than that in the control group (P<0.05). Conclusion: the application of laparoscopic appendectomy in patients with appendicitis has a significant effect and a small effect on serum hs-crp, which is worthy of promotion and application.

Objective: To explore the clinical effects of hemodialysis (HD) and high-flux hemodialysis (HFHD) in the treatment of uremia with hyperphosphatemia. Methods: The clinical data of 126 patients with uremia complicated with hyperphosphatemia were retrospectively analyzed. The patients with HFHD for blood purification therapy were included in observation group (n=74), and the patients with HD were included in control group (n=52). The serotoxin indexes [creatinine (SCr), urea nitrogen (BUN), parathyroid hormone (PTH), β2-microglobulin (β2-MG)], calcium-phosphorus metabolism indicators (blood calcium, blood phosphorus, calcium-phosphorus product) and nutritional status indicators [serum albumin (ALB), serum prealbumin (p-ALB), upper arm muscle circumference (AMC)] were compared between the two groups before treatment and after 3 months of treatment. Results: After 3 months of treatment, the serum levels of SCr and BUN in the two groups were significantly decreased compared with those before treatment while the serum calcium level was significantly increased compared with that before treatment (P<0.05), but there were no statistically significant differences between the groups at the same time (P>0.05). The levels of serum PTH, β2-MG and blood phosphorus and calcium-phosphorus product in the two groups were significantly lower than those before treatment, and the levels in observation group were significantly lower than those in control group at the same time (P<0.05). The levels of ALB, p-ALB and AMC in the two groups were significantly increased compared with those before treatment, and the levels in observation group were significantly higher than those in control group at the same time (P<0.05). Conclusions: Compared with HD, HFHD for patients with uremia and hyperphosphatemia can effectively remove PTH in the body, more actively correct calcium-phosphorus metabolism, improve the body nutritional status, and benefit its prognosis and rehabilitation.

1560 Effect of Different Doses of Dizosin Under BIS Monitoring in Patients undergoing General Anesthesia for Laparoscopic Surgery

Objective: To observe the effect of different dosages of dizocine in patients undergoing general anesthesia during laparoscopic surgery under BIS monitoring. Methods: 76 patients undergoing elective laparoscopic surgery in our hospital from February 2017 to May 2018 were prospectively selected and randomly divided into two groups according to the digital random table method. The low dose group (38 cases) was given 0.10 mg/kg of dizosin combined with propofol, and the high dose group (38 cases) was given 0.15 mg/kg of dizosin combined with propofol. The changes of hemodynamics, time index, propofol dosage, incidence of restlessness and adverse reactions during anesthesia were compared between the two groups. Results: In hemodynamic indexes, MAP, HR and SpO2 were decreased in both groups compared with T0 (t=13.818、6.059、12.821、4.775、33.466、12.903,P<0.05). T2 and T1 MAP, HR, SpO2 increased (t=6.539、5.695、4.060、3.089、29.878、15.137,P<0.05). Group 2 sets of patient groups, the high dose group T1, T2 has a significantly higher SpO2 than the simultaneous low dose group (t=12.821、3.627,P<0.05). 2 groups of patients in the induction time, awake time and directional force recovery time time on indicators had no significant difference (P>0.05). But high doses of propofol are significantly lower than the low dose group (t=8.256,P<0.05). There was no significant difference in the incidence of early agitation and adverse reflux rate between the two groups (P>0.05). Conclusion: Dizosin can effectively control hemodynamics, alleviate stress response and reduce the dosage of propofol, while 0.1 mg/kg of Dizosin can effectively reduce the incidence of restlessness with fewer adverse reactions.

Objective: To explore the effect of cardic rehabilitation treat on quality of life in patients with chronic heart failure. Methods: 96 patients with chronic heart failure diagnosed and treated by Department of Cardiology, Chengde Central Hospital from January 2014 to December 2017 were randomly divided into two groups, 48 in the control group and 48 in the rehabilitation group. Both groups were given routine treatment for chronic heart failure, while the rehabilitation group was given cardiac rehabilitation treatment at the same time. The quality of life and the level of NT-proBNP were compared between the two groups. Results: The subjects were 96 patients with chronic heart failure diagnosed and treated by Department of Cardiology, Chengde Central Hospital from January 2014 to December 2017. They were randomly divided into two groups, 48 in the control group and 48 in the rehabilitation group. Both groups were given routine treatment for chronic heart failure, while the rehabilitation group was given cardiac rehabilitation treatment at the same time. 1567 Analysis of Influencing Factors of Postpartum Stress Urinary Incontinence in Postpartum Women

Objective: To investigate the incidence of postpartum stress urinary incontinence(SUI) and analyze its influencing factors. Methods: A total of 836 women who gave birth at the Second Hospital of Anhui Medical University from December 2017 to December 2018 and were given 6-8 weeks postpartum assessment of pelvic floor muscle strength were selected.The postpartum women were divided into SUI group (n=294) and non-SUI group (n=542) according to the questionnaire survey used to investigate the incidence of postpartum urinary incontinence. The clinical data of the two groups were compared retrospectively, and the influence of related factors on postpartum SUI was analyzed. Results: The incidence of SUI in postpartum women was 35.2% and the pelvic floor muscle strength in SUI group was significantly lower than that in non-SUI group (P<0.05). Univariate analysis: the maternal age, gravidity, parity, the number of abortions, prenatal weight, prenatal body mass index (BMI), weight gain during pregnancy, mode of delivery, SUI during pregnancy,pregnancy and postpartum complications, newborn birth weight, macrosomia showed significant difference. Logistic regression showed that parity, prenatal BMI, mode of delivery, SUI during pregnancy, newborn birth weight, macrosomia, pregnancy and postpartum complications were all independent risk factors for postpartum SUI (P<0.05). Conclusion: Postpartum SUI is related to various factors of pregnancy and childbirth. Controlling pregnant women's weight gain during pregnancy and reducing prenatal BMI can reduce the incidence of postpartum SUI.

1571 Study on the Correlation between Background Enhancement of Breast and Molecular Subtypes of Breast Cancer on Magnetic Resonance Imaging

Objective: To investigate the correlation between BPE (Background parenchymal enhancement) and molecular subtypes and common prognostic markers of breast cancer. Methods: A retrospective analysis was made on the clinical data of 60 female patients examined by breast MRI in our hospital from January 2017 to April 2018. All patients were examined by GE MR750 3.0T superconducting magnetic resonance scanning system in the United States, and had complete dynamic enhanced image results. All patients received immunohistochemical tests, including estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER-2) and Ki-67 index. Pathological and molecular subtype results were analyzed to investigate the correlation between BPE, FGT and molecular subtypes and prognostic markers. Results: 60 cases of breast cancer were classified as invasive ductal carcinoma in 54 cases, intraductal papillary carcinoma in 2 cases and ductal carcinoma in situ in 4 cases. Molecular subtypes: 19 cases of Luminal A, 26 cases of Luminal B, 12 cases of HER-2 overexpression and 3 cases of TNBC. The distribution of BPE and FGT: BPE was BPE 1 (14,23.3%), BPE 2 (27,45%), BPE 3 (12,20%) and BPE 4 (7,11.7%). The distribution of FGT was FGT 1 (2,3.4%), FGT 2 (15,25%) and FGT 3 (23,38.3%) FGT 4 (20,33.3%). The average age of patients with high BPE and FGT was lower than that of patients with low BPE and FGT, and the difference was statistically significant (t value was 3.8, 2.5, P value was 0.008 and 0.01, respectively). The average Ki-67 index was not significantly correlated with BPE and FGT in high and low groups (t value was 6.1, 5.8, P value was 0.45 and 0.43, respectively). The results of correlation analysis showed that there was no correlation between high BPE and FGT in Luminal B, HER-2 overexpression breast cancer and TNBC compared with Luminal A breast cancer (P > 0.05). In the comparison of menstrual status, BPE and FGT in premenopausal patients were higher than those in postmenopausal patients (2 values were 20.5, 18.23, P values were 0.007 and 0.004 respectively). In other prognostic markers, high BPE had no significant correlation with lymph node status, histological grade and Ki-67 expression (P > 0.05). Conclusion:Some patients with poor prognosis, such as HER-2 overexpression breast cancer or TNBC, have no increased risk of BPE. There was no significant correlation between BPE elevation and negative prognostic markers such as positive axillary lymph node, high expression of Ki-67 index and high histological grade.

1576 Analysis of Urinary Iodine Level and Thyroid Function in Early Pregnancy Women in Chengde Area

Objective: To study the correlation between iodine nutrition status and thyroid function based on the established reference range of pregnancy specific thyroid indexes in Chengde area. Methods: In January 2017 to December 2017 showed there there were 2091 cases of pregnant women met the criteria, and their urine iodine, serum TSH,FT3 and FT4 were measured. Finally, the SPSS21.0 system is used to analyze the data to discuss its correlation. Results:①The median urinary iodine of pregnant women in Chengde area was 132.1 ug / L, belonging to iodine deficiency state. ②There was a significant difference in the incidence of thyroid disease between iodine appropriate group and iodine deficiency group, iodine beyond appropriate group and iodine excess group (P < 0.05),among the thyroid disease in low T4 concentration was statistically significant(P < 0.05). ③There was no significant difference in FT3 and TSH between the groups with different urinary iodine nutrition levels (P > 0 05);The levels of FT4 in iodine deficiency group and iodine deficiency group were significantly higher than those in iodine beyond appropriate group and iodine excess group, and the differences were statistically significant. Conclusion: Most of women who are in the first trimester of pregnancy in Chengde area suffered an iodine deficiency and adequate supplementation of iodine should be taken into place,in the mean time, the screening of thyroid function should be popularized so as to provide a better guidance during pregnancy.

1580 Analysis of Risk Factors Affecting the Increase of Systolic Blood Pressure Variability in Acute Phase in Patients with Stereotactic Minimally Invasive Puncture Catheter Drainage

Objective:To study the risk factors for the increase of systolic blood pressure variability(SBPV)in acute phase in patients with stereotactic minimally invasive puncture catheter drainage. Methods: 200 patients with hypertensive intracerebral hemorrhage (HICH) who underwent stereotactic minimally invasive puncture catheter drainage in the hospital from January 2014 to December 2017 were included in the study.According to whether there was increased SBPV in acute phase, they were divided into increased SBPV group(n=102)and smaller SBPV group(n=98).The general data and blood biochemical indicators were compared between the two groups,and the independent risk factors affecting the increase of SBPV in acute phase were analyzed. Results: There were no significant differences between increased SBPV group and smaller SBPV group in terms of gender,age,smoking history,drinking history,history of stroke,history of diabetes,history of hypertension,regular use of antihypertensive drugs and dehydration and antihypertensive drugs(P>0.05),and there were significant differences in the NIHSS score and intracranial hematoma volume at admission and first SBP value at admission between the two groups(P<0.05).There were significant differences in the levels of urinary nitrogen,creatinine,total cholesterol,triglyceride and apolipoprotein between increased SBPV group and smaller SBPV group(P<0.05),and there was no significant difference in the level of low-density lipoprotein between the two groups(P>0.05).Logistic regression analysis indicated that the first SBP,NIHSS score,intracranial hematoma volume and triglyceride were independent risk factors for the increase of SBPV in acute phase of patients with stereotactic minimally invasive puncture catheter drainage. Conclusion: Clinically,the blood pressure of patients with higher NIHSS scores and higher SBP levels at admission should be actively monitored,and antihypertensive treatment should be done.It is recommended that patients with hypertension should pay attention to healthy diet,maintain normal lipid metabolism,and prevent the increase of SBPV in acute phase.
