DOI: 10.3969/j.issn.1006-6233.2019.08.001
Objective: To analyze the expression level and regulation mechanism of microRNA-23a (miR-23a), microRNA-24-2 (miR-24-2) and microRNA-27a (miR-27a) in patients with primary gouty arthritis (PGA). Methods: 50 patients with PGA in our hospital were selected as case group and 30 health physical examination personnel as control group. The expression levels of miR-23a, miR-24-2 and miR-27a were detected by real-time fluorescent quantitative PCR. Pearson correlation analysis was used to analyze the relationship between the expressions of miR-23a, miR-24-2 and miR-27a. The data were included in statistical analysis. Results: The plasma fasting blood glucose, uric acid, white blood cell count, neutrophils, triglycerides and apolipoprotein B100 in the case group were significantly higher than those in the control group (all P<0.05), apolipoprotein A1 was significantly lower than those in the control group (P<0.01), but there was no significant difference in total cholesterol between the two groups (P>0.05). Compared with the control group, the expression levels of miR-23a, miR-24-2 and miR-27a in the case group were significantly decreased (P<0.01). Pearson correlation analysis showed that there was a positive correlation between the expression levels of miR-23a, miR-24-2 and miR-27a in peripheral blood mononuclear cells of patients (P<0.01). Urate (100 g/ml) was used to stimulate peripheral blood mononuclear cells in healthy people. The results showed that the expression levels of miR-23a, miR-24-2 and miR-27a in peripheral blood mononuclear cells in the stimulation group were significantly lower than those in the non-stimulation group (P<0.01). Conclusion: Mi-23a, mir-24-2 and mir-27a may play an important role in the inflammatory immune response of PGA as negative inflammatory regulators, and mir-24-2 may regulate the lipoprotein of PGA.

Methods: 110 patients with acute non-ST-segment elevation myocardial infarction (NSTEMI) admitted to our hospital from June 2016 to June 2018 were divided into control group and experimental group according to TIMI shunt of target lesions after percutaneous coronary intervention. Among them, patients with TIMI no more than grade 2, i.e. slow flow/no reflow, were divided into experimental group (25 cases), while patients with TIMI grade 3, i.e. patients with normal reflow, were divided into control group (85 cases). The general data of two groups of patients were recorded, and the related indicators of coronary interventional surgery in two groups were observed, the occurrence of major adverse cardiac events in two groups during hospitalization were compared, and the blood flow indicators before and after the application of nicorandil in the experimental group were analyzed. Logistic regression analysis was used to analyze the related factors affecting slow/no-reflow flow during interventional procedures. Results: The proportion of patients with hyperlipidemia, diabetes history, smoking history, etc. in the experimental group was significantly higher than that of the control group (P<0.05). There was no significant difference in the intraoperative center rate between the two groups (t=0.241, P>0.05). The thrombus signs and incidence of intraoperative hypotension in the experimental group were significantly higher than those in the control group (χ2=8.273, χ2=6.394, χ2=6.028, t=22.349, χ2=5.273, all P<0.05). During the hospitalization period, 1 patient in the experimental group died due to cardiogenic shock, and there was no recurrence of myocardial infarction in both groups and no significant difference in the incidence of major cardiac adverse events in the two groups (all P>0.05). The ratio of TMPG3 level and TIMI blood flow level 3 in the experimental group was significantly higher than that before the treatment in this group (χ2=47.232, χ2=53.234, P<0.05). After treatment with nicorandil, CTFC of the experimental group was significantly lower than that of this group (t=8.273, P<0.05). Thrombosis, multiple stents, intraoperative hypotension, diabetes history, and smoking history were all risk factors for slow blood flow/no complex flow (all P<0.05). Conclusion: Thrombosis, signs of hypotension, multiple stents, history of diabetes mellitus and smoking during PCI are all related risk factors affecting the occurrence of slow/no-reflow coronary flow in patients with NSTEMI. Intracoronary nicorandil injection can effectively improve slow/no-reflow coronary flow in patients with NSTEMI.

DOI: 10.3969/j.issn.1006-6233.2019.08.033
Objective: To analyze the relationship between resting heart rate (RHR) and metabolic syndrome (MS) in patients with hypertension. Methods: A total of 126 patients with essential hypertension admitted to the hospital from December 2016 to December 2018 were included in the study. According to whether patients with hypertension were complicated with MS, they were divided into simple hypertension group (n=72), hypertension with 1 item MS group (n=31) and hypertension with MS group (n=23). Age, gender, history of hypertension, BMI, waist circumference, resting heart rate and laboratory biochemical indicators [including fasting blood glucose (FBG), total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), fasting insulin concentration (FINS), insulin resistance index (IR)] were compared among the three groups, and the risk factors affecting MS in hypertension were analyzed. Results: There were significant differences in the age, history of hypertension, BMI, waist circumference, RHR, FBG, TC, TG, HDL-C, LDL-C and IR among simple hypertension group, hypertension with 1 item MS group and hypertension with MS group (P<0.05), and there was no significant difference in the gender proportion among the three groups (P>0.05). Correlation analysis showed that RHR value was positively correlated with waist circumference, FBG, TG, LDL-C, FINS and IR in patients with hypertension (r=0.14, 0.13, 0.61, 0.45, 0.41, 0.54, 0.63, P<0.05). Logistic regression analysis suggested that waist circumference, RHR and IR were independent risk factors for hypertension with MS. Conclusions: Hypertensive patients with MS are closely related to IR, waist circumference and RHR. It is suggested that hypertensive patients exercise more to reduce abdominal fat and pay attention to regulating RHR, reduce insulin resistance index, so as to reduce the risk of cardiovascular disease.

DOI: 10.3969/j.issn.1006-6233.2019.08.044
Objective: To study the continuous renal replacement therapy (CRRT) in patients with kidney syndrome (CRS) 1 core and analyzes the factors affecting prognosis. Methods: 66 patients with type 1 cardiorenal syndrome admitted to our hospital from July 2017 to September 2018 were retrospectively collected. According to the death during hospitalization, 31 cases were divided into survival group and 35 cases into death group. The clinical indicators, laboratory examination indicators, treatment parameters and left ventricular ejection fraction levels of the two groups were compared, and multivariate logistic regression analysis was made. Results: The interval between diagnosis and treatment in survival group was (3.2 +0.8) days, which was lower than that in death group (5.1 +1.2) days, with significant difference (P<0.05). The levels of alanine aminotransferase, total bilirubin and serum creatinine in survival group were lower than those in death group, and there were significant differences (all P<0.05). The left ventricular ejection fraction of survival group was (60.4 +10.3)%, which was higher than that of control group (50.7 +9.2)%. The difference was significant (P<0.05). Multivariate logistic regression analysis showed that the interval between diagnosis and treatment was a risk factor for death in CRRT patients with type 1 cardiorenal syndrome, while left ventricular ejection fraction was a protective factor. Conclusion: CRRT has a poor prognosis for type 1 CRS patients. The interval between diagnosis and treatment is an independent risk factor for death, while left ventricular ejection fraction is a protective factor.
