529 The Expression of CK20 HIF - 1α and PKM2 in Colorectal Cancer Tissues

Objective: To observe the expression of CK20, HIF-1α and PKM2 in colorectal cancer tissues. Methods: 73 cases of colorectal cancer tissue specimens in our hospital from December 2015 to December 2016 were selected as the research object (colorectal cancer group), and choose another 30 cases of normal colorectal tissue specimens as the control group. The expression of CK20, HIF-1α and PKM2 were detected by the immunohistochemical method. Results: In the colorectal cancer group, there was no significant differences of the expression rate of CK20, HIF-1α and PKM2 under different age, sex and differentiation degree(P>0.05), but the expression rate of different lymph node metastasis and Dukes stages of CK20, HIF-1α and PKM2 was statistically significant (P<0.05); In the colorectal cancer group, CK20 was positively related with HIF-1α (r=0.751, P<0.01); and PKM2 was positively related with HIF-1α, too (r=0.926, P<0.01)3. The expression of CK20, HIF-1α and PKM2 of the colorectal cancer group were significantly higher than those of the control group (P<0.05). Conclusion: The expression of CK20 and HIF-1α of the colorectal cancer tissue specimens are significantly higher than that of the colorectal normal tissue specimens, which showed certain prediction value on tumor value-added and metastasis for colorectal cancer patients.

532 Shuanghua Qingre Granules Combined with Ambroxol Hydrochloride in the Treatment of Geratic Chronic Tracheitis and Effects on the Serum IL-6 IL-17 TNF-α MMP-9 levels

Objective: To study the curative efficacy of Shuanghua Qingre granules combined with ambroxol hydrochloride in the treatment of geratic chronic tracheitis and its effects on the serum interleukin-6(IL-6), interleukin-17 (IL-17), tumor necrosis factor-α(TNF-α) and matrix metalloproteinase-9(MMP-9) levels. Methods: 80 patients of geratic chronic tracheitis admitted from April 2014 to April 2015 in our hospital were selected as research objects and divided into the observation group (n=40) and the control group (n=40). Both groups of patients were treated with conventional treatment after admission, including asthma, spasmolysis, fever etc. The control group was treated with ambroxol hydrochloride injection on the basis of conventional therapy, every time 30mg, 2 times a day, while the observation group was treated with Shuanghua Qingre granules on the basis of control group, once for 20g, 3 times a day. Both groups were treated for 2 weeks. Then the curative effect, clinical symptoms (cough, wheezing, dyspnea) disappearance time, changes of IL-6, IL-17, TNF-α, MMP-9 levels, pulmonary function FEV1, FEV1/FVC before and after treatment were compared between two groups. Results: The total effective rate of observation group was significantly higher than that of the control group [95.00% (38/40) vs 75.00% (30/40)] (P<0.05); The disappearance time of cough, wheezing, breathing difficulties were significantly shorter in the observation group than those of the control group [(2.80±0.67)d vs (5.38±1.20)d, (4.58±1.01)d vs (8.93±1.21)d (4.02±0.90)d vs (6.78±1.35)d] (P<0.05); the serum IL-6, IL-17, TNF-α, MMP-9 levels were significantly lower than those of the control group [(8.02±2.12)μg/L vs (12.32±2.30)μg/L, (15.87±3.42)pg/L vs(19.36±4.02)pg/L, (16.30±4.21)ng/L vs (23.76±4.60)ng/L, (0.30±0.12)ng/L vs (0.50±0.14)ng/L] (P<0.05); the FEV1,FEV1/FVC were significantly higher than those of the control group [(57.36±9.32)% vs (50.37±8.97)%,(66.86±7.80)% vs (61.92±6.84)%] (P<0.05). Conclusion: Effect of Shuanghua Qingre granule combined with ambroxol hydrochloride in treatment of senile chronic bronchitis is significantly better than the treatment with single, ambroxol hydrochloride injection. It can significantly improve the clinical symptoms and lung function of the patients, and reduce the level of serum IL-6, IL-17, TNF-α and MMP-9.

536 Curative Effect of Parecoxib Sodium Combined with Dexmedetomidine on the Blood Glucose Insulin Cortisol and Stress Response of Patients underwent Thoracic Surgery

Objective: To study the curative effect of parecoxib sodium combined with dexmedetomidine on the blood glucose, insulin, cortisol and stress response of patients underwent thoracic surgery. Methods: 80 patients of thoracic surgery who were treated from April 2014 to April 2015 in our hospital were selected as research objects. According to the draw method, these patients were divided into the observation group and the control group, 40 cases in each group. The control group was treated with 40mg parecoxib sodium through intravenous injection at half an hour before operation, while the observation group was treated with slow injection of dexmedetomidine 0.5μg/kg at 5 minutes before the end of operation. The intraoperative hemorrhage and blood transfusion volume, recovery time of breath, waking time, extubation time, the changes of blood glucose, insulin, cortisol, epinephrine and norepinephrine, mean artery pressure (MAP), heart rate (HR), respiration rate (RR), visual analogue scale/score (VAS), ramsay score before induction (T1), at the end of operation (T2) and immediately after extubation were compared between two groups. Results: There was no significant difference in the intraoperative blood loss, intraoperative blood transfusion, recovery time, recovery time of breath and extubation time between the two groups (P>0.05); the blood glucose, insulin and cortisol levels in the observation group at T1, T2 were significantly lower than those of the control group [(5.04±0.74)mmol/L vs (6.83±0.75)mmol/L, (6.73±0.61)mmol/L vs (7.69±0.83)mmol/L, (15.59±1.89)μIU/mL vs (20.86±2.03)μIU/mL, (14.93±1.82)μIU/mL vs (17.53±1.94)μIU/mL, (370.26±62.07)nmol/L vs (560.49±70.53)nmol/L, (402.86±63.75)nmol/L vs (694.27±78.36)nmol/L](P<0.05); the epinephrine and norepinephrine levels at T1, T2 were significantly lower than those of the control group [(48.28±16.20)ng/mL vs (79.36±18.37)ng/mL, (77.39±18.20)ng/mL vs (96.38±19.75)ng/mL, (316.25±43.55)ng/mL vs (402.17±53.47)ng/mL, (398.46±54.32)ng/mL vs (475.62±56.29)ng/mL] (P<0.05); the MAP, HR at T1, T2 were significantly lower than those of the control group [(77.34±6.19)mmHg vs (84.59±6.12)mmHg, (76.40±5.89)mmHg vs (81.49±5.97)mmHg, (89.80±9.46)time /min vs (95.70±9.80)time /min, (87.57±8.09) time/min vs (94.45±8.90) time/min] (P<0.05); VAS, Ramsay scores were significantly lower than those of control group [(2.56±0.35)score vs (5.30±0.78)score, (3.63±0.90) score vs (6.42±1.08) score] (P<0.05). Conclusion: Parecoxib sodium and dexmedetomidine had a synergic role, it could reduce the blood glucose, insulin, cortisol, relieve the pain, relieve restlessness, but wouldn't cause greater stress response and was conducive to the prognosis of patients.

540 Evaluation of Prognosis of Severe Pneumonia by Serum S-ChE PAB Apo A1 and APACHE II Score

Objective: To study the evaluation of prognosis of severe pneumonia by serum cholinesterase (S-ChE), prealbumin(PAB), apolipoprotein A1 (Apo A1) and Acute physiology and chronic health scoring system II (APACHE II) score. Methods: 70 patients of severe pneumonia admitted in our hospital from March 2015 to March 2017 were selected as research objects. According to the APACHE score<20 divided into A group, APACHE score≥20 were divided into B group, the incidence of multiple organ dysfunction syndrome (MODS) and fatality rate were compared between the two groups; depending on whether MODS occurred divided into MODS and non MODS groups, according to the prognosis after 28d, the patients were divided into the fatality group and the survival group, the differences of serum S-ChE, PAB, Apo A1 and APACHE II scores between the two groups were compared. Results: The incidence of the MODS and fatality rate in Agroup were significantly lower than those in group B [17.24%(5/29) vs 43.90% (18/41), 10.34% (3/29) vs 41.46% (17/41)] (P<0.05); the serum S-ChE, PAB, Apo A1 in MODS group were significantly lower than those in non MODS group [(2.84±0.33)kU/L vs (4.53±0.40)kU/L, (118.45±15.20)mg/L vs (204.61±17.92)mg/L, (0.60±0.11)g/L vs (1.15±0.21)g/L], the APACHE score was higher than those in non MODS group [(26.91±2.74) score vs (19.58±2.04)score], the difference was statistically significant (P<0.05); the serum S-ChE, PAB, Apo A1 in fatality group were significantly lower than those in survival group [(2.75±0.31)kU/L vs (4.46±0.29)kU/L, (109.86±15.73)mg/L vs (184.51±16.85)mg/L, (0.59±0.12)g/L vs (1.12±0.18)g/L], the APACHE score was higher than those in survival group [(28.95±2.40) score vs (18.58±1.97) score], the difference was statistically significant (P<0.05); the Logistic regression analysis showed that the scores of S-ChE, PAB, Apo, A1 and APACHE II were independent risk factors for the prognosis of severe pneumonia (OR=0.082, 0.137, 0.096, 1.798, P<0.05). Conclusion: The serum S-ChE, PAB, Apo A1 and APACHE II scores have a good evaluation value in the prognosis of severe pneumonia.

545 Effect of Thymosin Adjuvant Therapy on the Treatment Effect of Diabetic Patients with Pulmonary Tuberculosis and the Effect of Serum Immune Cell Level

Objective: To investigate the effect of immune cells and serum level of clinical effect in patients with Thymopeptide Enteric Injection adjuvant treatment of diabetes complicated with pulmonary tuberculosis. Methods: 98 cases of diabetic patients with tuberculosis infection in our hospital from June 2014 to June 2016, diagnosed by T-spot TB in, according to the principle of voluntary patients were divided into the control group and the control group with conventional therapy combined with thymosin adjuvant therapy, immune cell level contrast effect, the treatment group of two patients with tuberculosis in serum. Results:In the observation group, the duration of intensive treatment and the end of treatment were 37 and 42 cases, respectively. There were 36 cases and 41 cases of negative culture. There was no difference between the control group and the control group, P>0.05. The level of serum immune cells in patients with different types of contrast, found that the normal serum level of Treg cells was 1.3±0.6, significantly lower than those in patients with diabetes and diabetes combined with pulmonary tuberculosis, and the level of Th17 cells was 3.5±1.2, significantly higher than that in patients with diabetes and diabetes combined with pulmonary tuberculosis was significantly different, There was no difference in the level of Treg cell nuclear Th17 cells in the two groups before treatment, which was comparable. There was no difference in the incidence of symptoms between the two groups after treatment. After treatment, the disappearance rate of fatigue, night sweats, loss of appetite, cough and sputum in the observation group were 88.89%, 87.10%, 86.36%, 75%, 88.89%, which was significantly higher than that in the control group, with significant difference. Conclusion: While strengthening the anti-tuberculosis treatment, thymosin injection can improve the immune function of patients, help to improve the T cell subsets and promote the relief of clinical symptoms, which is worthy of clinical promotion and application.

548 Comparation of Early Naso-jejunal Nutrition Giving Whole Protein Preparation and Short Peptide Preparation in the Treatment of Acute Severe Pancreatitis

Objective: To investigate the effects of early nasal jejunal nutrition on patients with acute severe pancreatitis (SAP) treated with whole protein preparations and short peptide preparations. Methods: Eighty patients with SAP who were admitted to hospital from May 2015 to May 2017 were randomly divided into two groups. The whole protein preparation group was given nutritional support of whole protein preparation, and the short peptide preparation group was short peptide preparation. The two groups of diseases severity parameter, infection index, nutrition index and complication. Results: The levels of APACHEⅡscore, SOFE score, nutrition risk score and CTSI score in the whole protein preparation group were significantly lower than those in the short peptide preparation group at 14th day after nutrition was turned on. The levels of plasma endotoxin, plasma DAO and urine IFABP The levels of PCT, amylase, plasma endotoxin, plasma DAO, urine IFABP and ferritin were significantly lower than those of the short peptide preparation group on the 14th day after nutrition. The pre-albumin and albumin levels were significantly higher than those of the short peptide (P<0.01). The overall complication rate was significantly lower than that of the short peptide preparation group (P<0.05). Conclusion: The effect of early nasal jejunal administration of whole protein preparation on SAP is significant and has reference value.

550 Serum inflammatory Factors Vascular Endothelial Function and Pulmonary Function of Ulinastatin Combined with Ipratropium Bromide in the Treatment of AECOPD

Objective: To observe the efficacy and effect on serum inflammatory factors, vascular endothelial function and pulmonary function of ulinastatin combined with ipratropium bromide in the treatment of AECOPD. Methods: 125 cases of patients with AECOPD were randomly divided into observation group (64 cases) and control group (61 cases).The control group were given ipratropium bromide based on conventional therapy,and the observation group were given ulinastatin based on the control group. After 1 week ,to observe the clinical efficacy and differences of serum inflammatory factors (hs-CRP, TNF-α, IL-1, IL-8), vascular endothelial function related indicators (ET-1, NO, TXB2, VEGF), and pulmonary function (TLC, PEF1, PEF25, PEF50) between the two group. Results: ①Comparison of curative efficacy the observation group, excellence got 32 cases, effective got 32 cases,invalid 4 cases failure,and those of the control group was 18 cases,37 cases and 6 cases. The clinical efficacy of the two groups showed significant difference(Z=-2.123 P=0.034); ②Comparison of serum inflammatory factors, and all indexs of the observation group improved more significantly than that of the control group, with statistical difference (t=5.419,5.184,4.893,7.241 P=0.000,0.002,0.006,0.000); ③Comparison of vascular endothelial function. The two groups of NO, VEGF were decreased and ET-1,TXB2 were decreased, and all indexs of the observation group improved more significantly than that of the control group, with statistical difference (t=9.182,18.831,6.271,11.382 P=0.000,0.000,0.000,0.000);④Comparison of vascular pulmonary function. The two groups of TLC, PEF1, PEF25, PEF50 were increased with before, and all the indexs of the observation group improved more significantly than that of the control group, with statistical difference (t=5.402, 4.927, 4.238, 5.701 P=0.009, 0.011, 0.019, 0.003). Conclusion: Ulinastatin combined with ipratropium bromide is effective in the treatment of AECOPD, which can effectively inhibit the inflammatory reaction, protect the vascular endothelial function and lung function.

552 The Effect of Microvascular Decompression and Gamma Knife Radiosurgery on Idiopathic Trigeminal Neuralgia in the Elderly

Objective: To investigate the effect of microvascular decompression and gamma knife radiosurgery on idiopathic trigeminal neuralgia in the elderly. Methods: 90 patients of idiopathic trigeminal neuralgia in the elderly who received therapy from November 2013 to November 2015 in our hospital were selected. According to different surgical methods,the patients were divided into the microvascular decompression group (n=45) and the gamma knife group (n=45). The curative effect was compared between the two groups after 3 months, and the patients were followed up for 12 months, and the visual analogue scale (VAS) was recorded before and after the operation, and the complication and pain recurrence rate were record. Results: There was no significant difference in the total remission rate between the two groups after 3 months [88.89%(40/45) vs 82.22% (37/45)](P>0.05); the VAS scores in the two groups were significantly lower than those before treatment (P<0.05), there was no significant difference in VAS score between the two groups after operation 1 month (P>0.05), but the VAS scores in the microvascular decompression group were significantly lower than those of the gamma knife group at after operation 3, 6, 9 and 12 months, the difference was statistically significant (P<0.05); there was no significant difference in the incidence of postoperative infection, nausea, vomiting, tinnitus, facial numbness and hearing impairment between the two groups [17.78%(8/45) vs 15.56% (7/45)] (P>0.05); the pain recurrence rate in the microvascular decompression group was significantly lower than those of the gamma knife group at 12 months follow-up [11.11%(5/45) vs 28.89%(13/45)], the difference was statistically significant (P<0.05). Conclusion: Microvascular decompression surgery and gamma knife radiotherapy can relieve senile idiopathic trigeminal neuralgia, but the microvascular decompression remission rate and recurrence rate were better than that of gamma knife radiation in long-term pain, it's more conducive to the treatment of elderly patients with good foundation, But for the basis of poor or can not tolerate surgery, gamma knife radiosurgery also can be used as a good palliative treatment.

560 Value of Endocrine Hormone Levels in Evaluating the Effects of Surgical Treatment for Patients with Functional Pituitary Adenoma

Objective: To investigate the value of changes of endocrine hormone levels in evaluating the effects of surgical treatment for patients with functional pituitary adenoma. Methods: According to the degree of surgical resection, 500 patients with functional pituitary adenoma, including 273 patients with prolactin (PRL) type and 227 patients with growth hormone (GH) type were divided into the total resection group and the partial resection group. Serum levels of PRL and GH were determined before surgery, at 1 day, 3 days and 7 days after surgery. Above-mentioned indexes were compared between the total resection group and the partial resection group. The tumor control was evaluated during follow-up, and the value of PRL and GH in predicting the surgical effects on PRL type and GH type pituitary adenomas was analyzed by the receiver operating characteristic (ROC) curve. Results: Serum levels of PRL and GH in the total resection group at 1 day, 3 days and 7 days after surgery were significantly lower than those in the partial resection group (P<0.05). CT or MRI reexamination showed that the tumor control rates of patients with PRL type pituitary adenoma treated by total resection and patients not treated by total resection were 97.00% and 31.51% respectively. The tumor control rates of patients with GH type pituitary adenoma treated by total resection and patients not treated by total resection were 85.54% and 54.10%, respectively. The area under the curve (AUC), sensitivity and specificity of postoperative serum PRL in predicting the 2-year tumor control rate of patients with PRL type pituitary adenomas were 0.899, 78.52% and 88.10%, respectively. The optimal critical value, AUC, sensitivity and specificity of serum GH in predicting the 2-year tumor control rate of patients with GH type pituitary adenomas were 1.00nmol/L, 0.900, 79.00% and 88.14%, respectively. Conclusion: The monitoring of serum PRL and GH levels after operation has a certain predictive value for the degree of surgical excision and recurrence of PRL and GH pituitary adenomas.

564 Study of Changes of Plasma D-dimer and Lipoprotein(a) during Intravenous Thrombolysis with rt-PA in Patients with Acute Cerebral Infarction

Objective: To explore the changes and significance of plasma D-dimer and lipoprotein(a) in the process of intravenous thrombolysis with rt-PA in acute cerebral infarction. Methods: The plasma D-dimer and lipoprotein(a) were dynamically monitored by the technique of immunofluorescence and biochemical analysis before thrombolysis and 1, 2, 24 h after intravenous thrombolysis in patients with cerebral infarction. Results: Plasma D- dimer and lipoprotein (a) increased significantly after 1 hours and 2 hours after intravenous thrombolysis, while the levels of rt-PA decreased rapidly 24 hours after intravenous thrombolysis, but still higher than that before intravenous thrombolysis. Conclusions: After cerebral infarction, after intravenous thrombolysis, plasma D- dimer level and lipoprotein (a) level all show a concordance, which first increased and then decreased. It indicates that there is a certain change in coagulation, fibrinolytic system and lipid metabolism after cerebral venous thrombolysis in rt-PA. This change trend is of certain significance for guiding and judging intravenous thrombolysis.

567 The Effect of Sevoflurane in Radical Resection of Lung Cancer and its Effect on Serum E-selectin sIL-2R beta -EP and HMGB1

Objective: To study the anesthetic effect of sevoflurane in the treatment of radical procedure for lung cancer and its effects on the serum E-selectin, Soluble interleukin 2 receptor(sIL-2R), β-endorphin(β-EP) and high mobility group box protein B1(HMGB1) levels. Methods: 96 cases of lung cancer patients undergoing radical procedure who were treated from March 2015 to March 2017 in our hospital were selected as research objects. According to random number table, those patients were divided into the group A (n=48) and the group B(n=48). During the operation, group A was given 1~2% sevoflurane to maintain anesthesia, group B was given 1~1.5% isoflurane to maintain anesthesia. The condition of perioperative period, the changes of serum E-selectin, sIL-2R, beta -EP and HMGB1 levels at T0 (before operation), T1 (operation; start 1h), T2 (the end of the operation) and T3 (after the operation 1h), the incidence of postoperative restlessness and adverse reactions were compared between two groups. Results: There was no significant difference in the intraoperative blood loss, intraoperative infusion volume and operation time between the two groups (P>0.05), the recovery time, extubation time, spontaneous breathing recovery time and length of stay of group A were significantly shorter than those of group B (P<0.05); the levels of serum E-selectin, sIL-2R, β-EP and HMGB1 in both groups at T1, T2 and T3 points were significantly higher than those at T0 (P<0.05); the serum levels of E-selectin, sIL-2R, β-EP and HMGB1 of group A were significantly lower than those of group B at T1, T2 and T3 points (P<0.05); the incidence of postoperative restlessness in group A was significantly lower than those in group B (P<0.05); the incidence of postoperative adverse reactions in group A was significantly lower than that in group B (P<0.05). Conclusion: Sevofluran is well for patients with radical procedure for lung cancer, which can decrease the levels of serum E-selectin, sIL-2R, β-EP and HMGB1, reduce the postoperative restlessness and adverse reaction, and promote postoperative recovery.

571 The Effect of Safflower Injection in the Patients with Cerebral Infarction and Its Influence on Serum Inflammatory Factors

Objective: To observe the clinical curative effect of safflower injection combined with conventional rehabilitation therapy on hemodynamics and serum inflammatory factors in patients with cerebral infarction. Methods: 112 patients with cerebral infarction admitted in our hospital from September 2015 to May 2017 were divided into observation group and control group according to the random number table method. The control group was given routine rehabilitation treatment. On the basis of this, Combined with safflower injection treatment, each cycle included 28 days; compare the two groups of patients with clinical efficacy, hemodynamic changes, serum levels of inflammatory factors. Results: The total effective rate was 92.86% (52/56) in the observation group and 78.57% (44/56) in the control group. The difference was statistically significant (χ2 = 4.667, P<0.05).The scores of FMA and BI in the observation group were significantly higher than those in the control group, NIHSS score was significantly lower than the control group, the difference was statistically significant (P<0.05);After treatment, the high blood viscosity, low shear viscosity and plasma viscosity of the observation group were significantly lower than those of the control group. The maximal peak flow velocity and vascular resistance index of bilateral middle cerebral artery were significantly higher than those of the control group (P<0.05);The level of serum Hs-CRP in the observation group was significantly lower than that in the control group, and the levels of VEGF and TGF-β1 were significantly higher than those in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: On the basis of routine rehabilitation therapy, safflower injection can effectively improve the clinical efficacy of cerebral infarction patients, improve the blood flow status and reduce the level of serum inflammatory factors, which is worth to be spread.

576 Effect of Edaravone Injection on Plasma NO ET and Hemorheology in Patients with Cerebral Hemorrhage

Objective: To study the effect of Edaravone Injection on plasma NO, ET and hemorheology in patients with cerebral hemorrhage. Methods: From November 2015 to February 2017, a total of 80 patients with traumatic brain hemorrhage in our hospital were randomly divided into observation group and control group, 40 cases in each group. The patients were treated with routine hemostasis, fluid infusion, intracranial pressure reduction, nutrition, anti-inflammatory, dehydration, diuresis and other symptomatic treatment; the observation group was given Edaravone Injection on this basis. Observe and record the clinical treatment effect of two groups of patients, compare the two groups of patients before and after treatment FMA score and ADL score; and the nemorheological indexes and NO, ET levels were detected and compared. Results: The total effective rate of the observation group was 95%, which was significantly higher than that of the control group (72.50%), and the difference was statistically significant (P<0.05). After treatment, the scores of FMA and ADL in the observation group were significantly higher than those in the control group, the difference was statistically significant (P<0.05). Two groups of patients after treatment, the observation group of whole blood viscosity (high and low shear), plasma viscosity, erythrocyte sedimentation rate, platelet adhesion rate and erythrocyte aggregation index were significantly lower than the control group, the difference was statistically significant (P<0.05). After treatment, the level of plasma ET in the observation group was significantly lower than that in the control group, while the level of NO was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). Conclusion: Edaravone Injection has good clinical efficiency in the treatment of cerebral hemorrhage patients with craniocerebral trauma, which can effectively improve the hemorheological indexes and plasma NO and ET levels.

579 Effect of Huaier Granule Combined with Neoadjuvant Chemotherapy for Breast Cancer and Its Influence on P53 Ki-67 MMP-2 and MMP-9 Levels

Objective: To study the curative efficacy of huaier granule combined with neoadjuvant chemotherapy on the breast cancer and the serum levels of P53 gene(P53), Ki-67 antigen(Ki-67), matrix metalloproteinase (MMP)-2 and matrix metalloproteinase (MMP-9). Methods: 92 patients with breast cancer admitted in our hospital from March 2011 to March 2013 were selected and randomly divided into the observation group (n=46) and the control group (n=46). The patients in control group were treated with neoadjuvant chemotherapy (pirarubicin+paclitaxel intravenous injection), while the patients in observation group were treated with huaier granule. Then the positive expression rate of P53 and Ki-67, changes of MMP-2 and MMP-9, clinical efficacy, survival rate and complications in the two groups were observed and compared before and after treatment. Results: After treatment, the positive expression rate of P53 in the observation group was significantly increased, and the positive rate of Ki-67 was significantly decreased (P<0.05), the serum levels of MMP-2 and MMP-9 of both groups were significantly lower than those before treatment and the MMP-2 and MMP-9 in the observation group were significantly lower than those of the control group (P<0.05). The clinical efficacy of observation group was significantly higher than that of the control group (P<0.05). The incidence rate of gastrointestinal reaction, myelosuppression, alopecia and liver function injury of observation group was significantly lower than that of the control group (P<0.05);during the follow-up period, 4 cases were lost in the observation group and 5 cases were lost in the control group,the survival rate in the observation group in 3 years was significantly higher than that of the control group. (P<0.05). Conclusion: Huaier granule combined with neoadjuvant chemotherapy had better clinical efficacy on breast cancer, which could effectively reduce the side effects and improve the prognosis, its may be closely related to the regulation of P53, Ki67, MMP-2, and MMP-9 levels.

583 Relationship between BRAF Gene Status and Efficacy of Cetuximab Combined with Chemotherapy in the treatment of Advanced Colorectal Cancer

Objective: To explore the relationship between BRAF gene status and efficacy of cetuximab combined with chemotherapy in the treatment of advanced colorectal cancer. Methods: 120 patients with advanced colorectal cancer were randomly divided into 2 groups. In group A, 61 cases of KRAS, NRAS, BRAF genes were wild type, using cetuximab combined with FOLFIRI (irinotecan, leucovorin, 5- fluorouracil) scheme; in group B, KRAS, NRAS genes were wild type in 59 cases, and BRAF gene was mutant, also using cetuximab combined with FOLFIRI scheme. The short-term efficacy, time to progression (TTP), overallsurvival (OS) and adverse reactions were compared between the two groups. Results: The objective response rate (ORR) and disease control rate (DCR) in group A and group B were 45.9%&42.4% and 83.6%&78.0%, respectively. TTP and OS in group A and group B were 10.5&9.1 months and 18.8&18.5 months, respectively. The results of A group were slightly better than those of group B, but there were no significant differences (P>0.05). The main adverse reactions of the two groups were acne like rash, digestive tract and hematological toxicity, mainly I-II degrees, but there were no statistical differences (P>0.05). Conclusion: Detection of BRAF gene status is helpful to further predict the value of cetuximab in advanced colorectal cancer, which may influence the treatment choice and prognosis of patients.

588 Study on the Incidence and Influencing Factors of Intrahepatic Infection after Radiofrequency Ablation in Patients with Liver Tumors

Objective: To investigate the related factors of liver infection after radiofrequency ablation in patients with liver cancer. Methods: 140 cases of intrahepatic tumors in our hospital from March 2013 to January 2017 were selected and treated by radiofrequency catheter ablation. The incidence of postoperative intrahepatic infection was observed and recorded. The radiofrequency The incidence of intrahepatic infection after ablation related factors were statistically analyzed. Results: Univariate analysis showed that the type of tumor, the number of tumors, the type of abdominal surgery and RFA were closely related to the occurrence of intrahepatic infection after RFA in patients with liver cancer (P <0.05), age, gender, Child-Pug grade, There was no significant correlation between antimicrobial prophylaxis and intrahepatic infection after hepatic tumor RFA (P>0.05). Multivariate analysis showed that the type of tumor, the number of tumors, the type of abdominal surgery and the RFA method all affected the RFA treatment After the occurrence of intrahepatic infection of the key factors (P<0.05). 591 The Predictive Value of the Level of Serum S100B Protein Expression on the Risk of Brain Metastasis in Patients with Small Cell Lung Cancer

Objective: To evaluate the predictive value of serum S100B expression in brain metastasis risk for patients with small cell lung cancer (SCLC) and its association with clinicopathological characteristics of SCLC patients. Methods: 20 SCLC patients with brain metastases, 24 SCLC patients without brain metastases and 16 post-operative SCLC patients as control group were enrolled in this case-control study. The serum levels of S100B in three groups were detected by ELISA. Results: Serum S100B levels in SCLC patients with brain metastases were markedly increased compared with SCLC patients without brain metastasis and postoperative SCLC patients (all P<0.001). Receiver Operating Characteristic (ROC) showed that serum S100B level had a good predictive value for brain metastasis in SCLC patients(SCLC patients with brain metastases vs. SCLC patients without brain metastases: AUC 0.840 (95% CI: 0.721~0.958); SCLC patients with brain metastases vs. post-operative SCLC patients: AUC 0.906 (95% CI: 0.811~1.000)). Multivariate regression analysis displayed that high level of serum S100B was an independent factor in predicting brain metastasis risk in SCLC patients (P=0.025). In addition, serum S100B level was positive associated with N stage in SCLC patients with brain metastases (P=0.019). Spearman rank correlation analysis revealed that serum S100B level was positively correlated with Neuron Specific Enolase (NSE) in SCLC patients with brain metastases (r=0.508, P=0.022). Conclusion: Serum S100B could be a promising biomarker of brain metastasis risk in SCLC patients, and it is positively correlated with N stage and NSE level in SCLC patients with brain metastasis.

597 The Value of Wells Score in the Diagnosis of Deep Vein Thrombosis in Spinal Cord Injury Patients

Objective: To observe the value of Wells score in diagnosis of deep vein thrombosis after spinal cord injury. Methods: 196 patients admitted in our rehabilitation department between May 2014 to December 2015 were obversed. Lower extremity deep venous ultrasonography was carried to diagnosis deep vein thrombosis, and the Wells score was used to evaluate all patients. Results: The Wells score in evaluation of deep venous thrombosis of spinal cord injury patients in rehabilitation stage sensitivity was 64.3%, misdiagnosis rate was 35.7%, and the specificity was 99.3%, and the misdiagnosis rate was 0.7%, the Youden's index was 63.6%, the positive predictive value was 97.3%, the negative predictive value was 87.4%, the positive likelihood ratio is 91.9, negative likelihood ratio was 0.36. Conclusions: There is a higher Wells score of false negative diagnosis of deep venous thrombosis in patients with spinal cord injury. Wells score is not an effective method to evaluating deep venous thrombosis.

600 Effects of Stellate Ganglion Block with Aparoscopic Gastrectomyon for Gastric cancer on Serum Aβ-42 IL-1βTNF-α and Brain Oxygen Metabolism in Patients

Objective: To discuss effects of stellate ganglion block with aparoscopic gastrectomyon for gastric cancer on Serum Aβ-42、 IL-1β、TNF-α and brain oxygen metabolism in patients. Methods: A total of 98 patients who underwent radical gastrectomy from June 2015 to November 2017 were randomly divided into experimental group and control group (n = 49). The experimental group was treated with stellate nerve block and general anesthesia. The control group received normal saline combined with general anesthesia. The serum A beta -42, IL-1 beta, and TNF- alpha were compared between the two groups. Compared before anesthesia induction (T0), endotracheal intubation (T1), pneumoperitoneum establishment (T2), radical gastrectomy (T3) and recovery time after extubation (T4), cerebral oxygen supply and demand index was compared. The indexes of cerebral oxygen supply and demand were: internal jugular bulb oxygen saturation (SjvO2), internal jugular venous oxygen partial pressure (PjvO2), arterial oxygen saturation (SaO2), arterial oxygen partial pressure (PaO2), radial artery internal jugular bulb blood oxygen content difference (Da-jvO2) and cerebral oxygen uptake rate (CEO2) were compared. Results: After treatment, the levels of serum Aβ-42, IL-1β and TNF-α were increased in both groups (P<0.05). The levels of serum Aβ-42, IL-1β and TNF-α in the experimental group were significantly lower than those in the control group (P<0.05). The levels of SjvO2, PjvO2, SaO2 and PaO2 in the two groups were significantly lower than those in the control group (P<0.05). The levels of Da-jvO2 and CEO2 in the two groups were significantly higher than those before the operation (P<0.05), and the difference was significant (P<0.05). Conclusion: Laparoscopic radical gastrectomy for gastric carcinoma under stellate ganglion block can reduce the postoperative serum A beta -42, IL-1 beta, TNF- alpha and improve brain oxygen metabolism.

604 Effect of Hypertonic Sodium Chloride on the Level of Serum NO and CD18 in Patients with Hemorrhagic Shock

Objective: To study the effect of hypertonic sodium chloride on the treatment of hemorrhagic shock and effect on serum levels of NO and CD18. Methods: 106 cases with hemorrhagic shock who were treated in our hospital from August 2015 to December 2016 were selected and randomly divided into the control group and observation group with 53 cases in each group. The patients in the control group were treated with intravenous infusion of naloxone, while the patients in the observation group were treated with intravenous infusion of hypertonic sodium chloride on the basis of the control group. Then the fluid resuscitation, the incidence of complications and mortality, and the serum levels of nitric oxide (NO) and CD18 in the two groups were observed and compared before and after the treatment. Results: The concentration of Hb in the observation group was significantly higher than that of the control group (P<0.01), while the amount of blood transfusion, recovery time and MAP were significantly lower than those of the control group (P<0.01); After treatment, the serum levels of NO and CD18 in the observation group were significantly lower than those of the control group (P<0.01); The incidence of complications and mortality rate of the observation group were significantly lower than that of the control group (P<0.05). Conclusion: Hypertonic sodium chloride combined with naloxone in treatment of hemorrhagic shock has better clinical efficacy, which can significantly reduce the serum levels of NO and CD18, and decrease the incidence of complications and death, and it is worthy of clinical promotion.

608 Effect of Genital Tract Group B Streptococcus (GBS) Infection in Late Pregnancy on Pregnancy Outcomes

Objective: To analyze the effect of genital tract group B streptococcus (GBS) infection in late pregnancy on pregnancy outcomes. Methods: 500 pregnant women in late pregnancy (35~37 weeks) were divided into the positive group (positive GBS bacterial culture) and the normal group (no GBS infection) according to the results of GBS bacterial culture and drug sensitivity test. The positive rate of GBS infection was statistically analyzed. In the positive group, patients who were willing to accept the clinical treatment were included into the medication group, and they were treated according to the results of drug sensitivity analysis, while patients who had concerns about medication and were not given any drug treatment were included into the non-medication group. The pregnant outcomes were compared between groups. Results: In the 500 cases of pregnant women in late pregnancy, GBS was positive in 84 cases (positive group), accounting 16.8%, including 55 cases in the medication group and 29 in the non-medication group. A total of 106 strains of GBS were detected, and their resistance rates to penicillin, ampicillin and erythromycin were high, while the resistance rates to clindamycin, ceftriaxone and vancomycin were low. The rate of cesarean section showed the normal group < the medication group < the non-medication group (29.8%, 43.6%, 51.7%), and the natural delivery rate showed the normal group > the medication group > the non-medication group (65.6%, 50.9%, 41.4%), with statistically significant differences between groups (P<0.05). There was no significant difference between the normal group and the medication group in the incidence rates of amniotic fluid pollution, intrauterine infection, fetal distress, premature delivery, postpartum hemorrhage and puerperal infection (P>0.05), but the incidence rates of above-mentioned complications in the non-medication group were significantly higher than those in the normal group and the medication group (P < 0.05). There was no significant difference between the normal group and the medication group in the incidence rates of neonatal sepsis, neonatal GBS infection, neonatal pneumonia and neonatal asphyxia (P > 0.05), but the incidence rates of above-mentioned adverse perinatal outcomes in the non-medication group were significantly higher than those in the normal group and the medication group (P<0.05). Conclusion: The risk of genital tract GBS infection is high in late pregnancy. It can cause the increase of cesarean section rate, incidence rates of complications and adverse perinatal outcomes. Targeted medication can improve pregnancy outcomes.

612 The Study on the Correlation between NLR LMR and Coronary Heart Disease

Objective: To investigate the relationship between peripheral blood neutrophil/lymphocyte ratio (NLR) and lymphocyte/monocyte ratio (LMR) and coronary heart disease. Method: A total of 86 patients who are suspected as coronary heart disease (CHD) were recruited in this study. According to the results of coronary angiography (CAG), they were divided into CHD group (54 subjects) and control group (32 subjects), respectively. The baseline data and clinical laboratory data were collected. In terms of aboved data, NLR and LMR were calculated and the association between NLR and LMR and coronary heart disease were analyzed. Results: CHD group, compared to control group, had higher NLR and lower LMR (P<0.05);Spearman correlation analysis showed that NLR was positively correlated with CHD, r = 0.272, P = 0.011;LMR was negatively correlated with CHD, r = -0.219,P = 0.043. Conclusions: NLR and LMR in peripheral blood were associated with coronary heart disease.

615 Diagnostic Value of Combined Detection of CT and B-ultrasound in Para Uterine Infiltration and Lymph Node Metastasis of Cervical Cancer

Objective: To investigate the application value of CT and ultrasonography in the diagnosis of cervical cancer parametrial invasion and lymph node metastasis in cervical cancer patients. Methods: 102 cases were treated with CT detection, ultrasonic detection and combined detection, including cervical cancer parametrial infiltration and lymphatic metastasis, analysis of the test results. Results: There were 56 cases of cervical cancer and 60 cases of lymph node metastasis. The accuracy, specificity, positive predictive value and correct index of combined detection in the diagnosis of cervical cancer were 95.10%, 97.83%, 98.11%, 87.96% respectively. The accuracy, specificity, positive predictive value and correct index of lymph node metastasis were 93.33%, 94.12%, 95.24%, 96.55%, 90.91% and 88.57%, respectively, which were significantly higher than those of CT and B-mode ultrasonography. Conclusion: CT combined with B-ultrasound can effectively reduce the misdiagnosis, improve the accuracy, and have high application value.

Objective: To study the clinical effect of trimetazidine in treatment of chronic heart failure and its effect on cardiac function and high sensitivity c-reactive protein. Methods: 94 patients with chronic heart failure from August 2015 to October 2017 in our hospital were selected, according to random table method, those patients were divided into control group (n=47)and research group (n=47). control group were treated with routine therapy. research group were treated with trimetazidine based on the control group. Then clinical treatment effect, Left ventricular ejection fraction (LV), left ventricular ejection fraction (LVEF), cardiac function classification, serum interleukin-6 (IL-6) and tumor necrosis factor-α(TNF-α),hs-CRP and acute cardiac events between two group was compared. Results: After treatment, the effective rate of research group was significantly higher than the control group 95.74% vs 80.85%, there was statistically significant (P<0.05).the FS and LVEF in two group was up, the research group was higher than the control group, and LVEDD and NYHA in two group were decreased, and the decrease was more obvious in the research group, there was statistically significant (P<0.05). IL-6, TNF-α,hs-CRP in the two groups decreased, and the research group was lower than the control group (P<0.05). There were thrombosis and arrhythmia in both groups, but the rate of the research group was lower than the control group, and there was a difference between the groups (P< 0.05). Conclusion: Application of trimetazidine in the treatment of chronic heart failure can effectively reduce serum high-sensitivity C-reactive protein levels, improve their cardiac function, improve the clinical efficacy of comprehensive treatment.

623 Effect of Shenmai Injection on Serum PCT IL-6 TNF-alpha and Myocardial Function in Patients with Breast Cancer after Operation

Objective: To study the effects of Shenmai injection on serum PCT, IL-6, TNF-α and myocardial function in postoperative with breast cancer and to evaluate its effect on prognosis.. Methods: 90 patients of mammary cancer who received therapy from March 2014 to December 2016 in our hospital were selected as research objects. According to draw method were divided into the experimental group and the control group. Two groups of patients were treated with chemotherapy after operation, Intravenous infusion of cyclophosphamide, fluorouracil, epirubicin. The experimental group on this basis, was treated with Shenmai Injection, each 50ml, add 5% Glucose Injection 500ml to the intravenous drip, 1 times a day. Then the curative effect, clinical symptom score, procalcitonin (PCT) and interleukin -6 (IL-6), tumor necrosis factor alpha (TNF- alpha) levels of troponin (cTnI), creatine kinase (CK), creatine kinase (CK-MB) level, superoxide dismutase (SOD), glutathione (GSH), malondialdehyde (MDA) level of two groups were compared. Results: The total remission rate of the experimental group was significantly higher than the control group [86.66%(39/45)vs66.66%(30/45] (P<0.05); Fatigue, sweating, dry mouth and throat, spiritlessness scores were significantly lower than the control group [(1.53±0.23) scores vs(3.10±0.63)scores, (1.40±0.32)scores vs(3.15±0.80)scores,(1.30±0.31)scores vs(3.04±0.70)scores,(1.52±0.27)scores vs(3.02±0.60)scores](P<0.05); PCT、IL-6、TNF-α level were significantly lower than the control group [(1.62±0.32)μg/L vs(5.83±1.05)μg/L,(5.72±0.98)pg/mL vs(10.89±1.52)pg/mL,(1.23±0.15)ng/mL vs(2.79±0.23)ng/mL](P<0.05); cTnI、CK、CK-MB level were significantly lower than the control group [(450.76±58.83)ng/L vs(702.17±83.18)ng/L,(1.87±0.40)IU/mL vs(5.20±1.34)IU/mL,(158.90±42.10)IU /L vs(910.38±97.94)IU /L](P<0.05); SOD, GSH level were significantly higher than the control group [(109.89±13.68)μIU /L vs(78.89±10.29)μIU /L,(140.83±20.82)μg/L vs(117.49±17.89)μg/L](P<0.05), MDA level were significantly lower than the control group [(5.39±1.21)μmol/L vs(12.76±3.81)μmoL/L](P<0.05).Conclusion: Shenmai injection has remarkable effect on myocardial protection after operation for breast cancer patients. It can reduce the levels of inflammatory factors PCT, IL-6 and TNF- alpha, reduce the incidence of postoperative infection and benefit the prognosis of patients.

Objective: To study the application of carbon nanoparticle in VI region lymph nodes dissection and parathyroid gland protection for patients with cN0 papillary thyroid carcinoma. Methods: 79 cases of cN0 thyroid papillary carcinoma admitted to Department of general surgery in Jiangsu Province Traditional Chinese Medicine Hospital from July 2016 to July 2017 were randomly divided into nano carbon group (41 cases) and control group (38 cases). The general situation of the two groups was compared. Comparative analysis of two groups of patients after obtaining the average total number of lymph nodes, the diameter of >1.0cm, 0.5~1.0cm and <0.5cm lymph node number and the number of metastatic lymph nodes and lymph node metastasis rate, detection rate, pathological parathyroid first day after the operation, PTH and blood Ca, operation time, postoperative drainage. Results: There was no significant difference in the general condition between the carbon nanoparticle trace group and the control group. Comparative analysis of two groups of patients after obtaining the average total number of lymph nodes, the diameter of >1.0cm, 0.5~1.0cm and <0.5cm lymph node number and the number of metastatic lymph nodes and lymph node metastasis rate, detection rate, pathological parathyroid first day after the operation, PTH and blood Ca, operation time, postoperative drainage. There was no difference between the operation time and the postoperative flow rate. Conclusion: Nano carbon can increase the number of lymph node dissection and the number of positive lymph nodes in VI area of cN0 thyroid papillary carcinoma, especially the smaller diameter lymph nodes. It can improve the detection rate of lymph node metastasis, reduce parathyroid damage and reduce the temporary low PTH rate after operation.

Objective: To explore the influence factors of vascular disease in type 2 diabetes mellitus, and provide the basis for early effective prevention and treatment of diabetic foot and heart and cerebrovascular diseases. Methods: 187 patients with type 2 diabetes who were treated in Department of endocrinology of Chengde College Affiliated Hospital from October 2015 to April -2017 were selected as subjects. According to the level of hyperhomocysteinemia (Hcy), the patients were divided into high Hcy group and normal Hcy group. The effects of low density lipoprotein cholesterol (LDL-C), homocysteine (Hcy), triglyceride (TG) and glycosylated hemoglobin (HbAlc) and other risk factors on diabetic peripheral vascular disease in two groups were compared. Results: When high Hcy was combined with high SBP, high LDL-C, high TG and high HbAlc, the risk of diabetic peripheral vascular disease was significantly higher than that of single risk factor (P<0.05). Conclusion: The level of plasma homocysteine is related to the severity of peripheral vascular disease in type 2 diabetes, and plasma high Hcy concentration combined with metabolic syndrome is more prone to diabetic peripheral vascular disease.

633 Application of InterTan Intramedullary Nail and Hip Replacement in Elderly Patients with Intertrochanteric Fracture

Objective: To study the different therapeutic effects of InterTan intramedullary nail and replacement hip replacement in the treatment of intertrochanteric fractures of the aged femur. Methods: 100 cases of intertrochanteric fracture of the femur in our hospital from May 2013 to May 2016 were selected and divided into the observation group and the control group according to the operation mode. 50 in the observation group were the replacement hip joint replacement group and 50 in the control group, which was InterTan intramedullary nail group. The Harris scores, activities of daily living (ADL), complication rate, operative time, intraoperative blood loss, hospitalization time and the time of postoperative onset between the two groups were observed and compared. Results: There was no significant difference in the Harris score, the ADL score and the incidence of complications in the two groups (P>0.05). The time of operation, the amount of bleeding and the time of hospitalization in the observation group were all higher than those of the control group, and there was a significant difference between the two groups (P<0.05). The activity time of the observation group was earlier than that of the control group, and there was a significant difference between the two groups (P<0.05). Conclusion: The replacement of hip joint with hip joint can shorten the time of postoperative activity, but Inter Tan intramedullary nail can shorten operation time and reduce intraoperative blood loss.

640 Clinical Efficacy of Electronic Bronchoscopy in Children with Refractory Mycoplasma Pneumoniae Pneumonia

Objective: To observe the effect of OLYMPUS BF3C-40 type electron fiber bronchoscopy in the treatment of children with refractory Mycoplasma pneumoniae pneumonia. Methods: Sixty children with refractory mycoplasmalpneumonia were divided into the medicine group and the electron branchofiberoscope group (30 cases in each group) The medicine group was given antiinfection, ntipyretic, and relieve cough medicine. The electron branchofiberoscope was added in the electron branchofiberoscope group, the clinical effect were observed in two groups. Results: The cough duration, moist rales, and hospitalization time were significantly shorter in the electron branchofiberoscope group (t=3.87, 3.67, 7.65, P<0.05). After treatment, the difference of CRP, WBC, ESR among two groups were significantly better in the electron branchofiberoscope group, there were significant difference (t=1.12, 1.03, 1.17;t=2.33, 2.47,3.58;t=1.65,3.13,3.93, P<0.05). The total effective rate in the electron branchofiberoscope group were significantly higher than that in the drug group (χ2=10.708,P< 0.05). Conclusion: The electron branchofiberoscope lavage therapy can significantly improve the effect of treatment in children with refractory mycoplasmal pneumonia and improve the symptom in the lung.

644 The Application of Digital Mammography in the Diagnosis of Benign and Malignant Breast Lesions

Objective: To study the value of digital mammography in the diagnosis of benign and malignant breast lesions. Methods: 80 patients with breast lesions who were admitted to the hospital from Jan.2015 to De.2017 were selected as the subjects. All of them were diagnosed by imaging examination, surgery or aspiration cytology. The breast digital mammography was performed before confirmation of pathological results, and the imaging data were complete and clear. The results of digital mammography were compared with those of histopathological examination, and the findings of digital mammography were analyzed. Results: Among the 80 patients, there were 59 cases (73.75%) with benign breast lesions and 21 cases (26.25%) with malignant lesions diagnosed by pathology. The sensitivity, specificity and accuracy of digital mammography in the diagnosis of breast lesions were 89.83%, 85.71% and 88.75% respectively, compared with pathological diagnosis. Compared with patients with benign breast lesions, the digital mammography showed that the proportions of patients with lesions ≥ 1cm, irregular shape and blurred edges in patients with malignant lesions were significantly higher (P< 0.05). There were few calcified benign breast lesions, with high-density and regional/diffuse distribution. However, there were many calcified malignant lesions, showing equal-density and segmental / linear distribution. There was a statistically significant difference in the number, density and distribution of calcification between the two (P< 0.05). Conclusion: The digital mammography is valuable in clinical diagnosis of benign and malignant breast lesions, especially the number, density and distribution characteristics of breast calcification, which can help the differential diagnosis.

647 Evaluation of the Effect of the Optimal Scheme of Youdujing Suppository for Condyloma Acuminata

Objectives: To explore the curative effect of the combination of Chinese medicine youdujing suppository combined with western medicine in the treatment of condyloma acuminata of vagina. Methods: 92 female patients with genital condyloma acuminatum were enrolled and divided into treatment group and control group randomly. After warts bodies were removed, both groups received 5-aminolaevulinic acid-photodynamic therapy at different time. Youdujing suppository was applied by vagina in treatment group. The wound healing time and recurrence of two groups were compared in three months respectively. Results:The healing time of the treatment group was shorter than that of the control group, and the recurrence rate of the treatment group was lower than that of the control group (P<0.05). Conclusions: The treatment of Western medicine has been optimized by the combination of youdujing, which can better inhibit the recurrence of condyloma acuminata, and it is worth popularizing.

650 Clinical Observation of Double Incision Combined with 360 Internal Fixation in Treatment of High Energy Closed Pilon Fracture

Objective: To study the clinical effect of double incision combined with 360 internal fixation for closed Pilon fracture. Methods: 80 patients with high energy closed Pilon fracture treated in our hospital from June 2012 to October 2016 were randomly divided into study group ( n = 40 ) and control group ( n = 40 ). The control group was treated with posterior lateral approach internal fixation and the study group was treated with double incision combined with 360 internal fixation. Results:The incidence of complications in the study group and the control group were 15 %, 42.5 %, respectively, and the clinical effect in the study group ( 92.5 % ) was significantly better than that in the control group ( 62.5 % ). there was significant difference between the two groups (P<0.05). There were significant differences in pain and soft tissue recovery time between the two groups, and the functional score of the study group ( 3.68 1.07 ) was lower than that of the control group ( 5.72 1.23 ), the fracture healing time of the study group was significantly shorter than that of the control group (P<0.05). Conclusion: The treatment of closed Pilon fracture with double incision and 360 internal fixation through combined approach is effective, which can not only improve the healing condition of the fracture patients, but also reduce the incidence of complications. it is worthy of clinical application.

654 Application of Seven Fluorane Combined with Remifentanil Target Controlled Infusion in Anaesthesia of Elderly Patients with Tumor

Objective:To investigate the clinical effect of target controlled infusion of sevoflurane combined with remifentanil in elderly patients with cancer. Methods: From January 2015 to August 2017, 98 elderly patients undergoing general anesthesia were divided into observation group and control group according to different anesthesia regimens. The patients in the control group were anesthetized with propofol combined with seven flurane target controlled infusion. The patients in the observation group were anesthetized with remifentanil combined with target controlled infusion of seven halothane. The changes of vital signs before and after anesthesia were observed in two groups, and the postoperative waking time and extubation time were compared and analyzed. Results:The systolic blood pressure, diastolic blood pressure and MAP of observation group and control group were significantly different from those of T0 (P<0.05), but at T0, T1, T2 and T3, systolic blood pressure, diastolic blood pressure, heart rate and MAP level were not significantly different (P>0.05). The awake time and extubation time in the observation group were significantly shorter than those in the control group, the difference was significant (P<0.05). Conclusion: Targeted infusion of sevoflurane combined with remifentanil in elderly patients with tumor surgery can achieve a good anesthetic effect while shortening the recovery time of patients after surgery, is conducive to follow-up treatment and care, it is recommended in the clinical application .

657 Clinical Analysis of Combined Anterior and Posterior Approach for the treatment of Fracture and Dislocation of the Olecranon Joint

Objective: To discuss the clinical efficacy of combined anterior and posterior approach surgery in the treatment of transolecranon fracture-dislocation of the elbow. Methods: 36 patients with transolecranon fracture-dislocation of the elbow treated in the hospital from January 2015 to March 2017 were selected as study subjects, and they were divided into the control group and the study group with 18 cases in each group by the random number table method. The control group was treated by elbow posterior open reduction and internal fixation while the study group was treated by combined anterior and posterior approach surgery. The general situation of surgery, fracture healing, recovery of elbow function (Broberg-Morrey function score) and the incidence of complications in the two groups after surgery were observed. Results:There was no significant difference in the surgical time between the two groups (P>0.05). The intraoperative blood loss and fracture healing time of the study group [(26.49±3.28) mL, (6.09±1.18) weeks] were less or shorter than those of the control group (P<0.05). The fracture healing rate of the study group in 6 months after surgery was higher than that of the control group (100.00% vs 77.78%) (χ2=4.500, P < 0.05). There was no significant difference in Broberg-Morrey scores between the two groups before surgery (P>0.05). After surgery, Broberg-Morrey scores of the two groups increased significantly, and the Broberg-Morrey scores of study group after surgery [(78.82±4.88), (85.34±5.36), (93.32±6.08)] were significantly higher than those of the control group (P<0.05). The incidence of complications in the study group after surgery was significantly lower than that in the control group (27.78% vs 72.22%) (Test value=4.685, P<0.05). Conclusion: The clinical efficacy of combined anterior and posterior approach for the treatment of fracture and dislocation of the olecranon joint is clear, and it has clinical application value.

661 Correlation Analysis between High Pressure of Anal Canal during Defecation and Formation of Rectocele

Objective: To explore the correlation between the high pressure of anal canal during defecation and the formation of rectocele. Methods: 60 patients with the criteria of rectocele selection were selected as the case group, and 60 normal persons were selected as the control group, the anorectal pressure of two groups were measured, and the data were recorded and analyzed. Results:The anal canal defecation diastolic pressure of the case group was significantly higher than the control group (P<0.01), and the anal canal resting pressure was higher than the control group (P<0.05), further more, the case group action correlation was positively correlated. however, the maximum systolic pressure, the anal rectal resting pressure and the rectal systolic pressure of two groups were showed without significant difference (P>0.05). Conclusions: The anal canal defecation diastolic pressure,the anal canal resting pressure and the formation of rectocele are closely related. The formation of rectocele is due to the high pressure of anal during defecation,so that feces can not be excreted,Which pushes the anterion wall of the return.

664 Clinical Observation on the Treatment of Chronic Eczema with Dot Matrix Laser and Flumethasone

Objective: To investigate the clinical efficacy of dot-matrix laser combined with flumethasone in the treatment of chronic eczema. Methods: 100 patients with chronic eczema who were treated in outpatient department from July 2016 to September 2017 were randomly divided into two groups. Patients in the single drug group were treated with flumethasone, while patients in the combined group were treated with laser dextrose injection. VAS score was compared between the two groups , EASI score, overall treatment effect, cytokines and quality of life. Results:After treatment, the VAS score and EASI score in the combined group were significantly lower than those in the single drug group. The effective rate and total effective rate were significantly higher in the combination group than those in the single drug group. The levels of IL-2, IL-4, IL -10, IgE and LTB4 levels were significantly lower than the single drug group, IFN-γ levels were significantly higher than the single drug group; after treatment EPQOLS total score was significantly lower than the single drug group, there was a significant difference (P<0.05). Conclusion: The clinical efficacy of dot-matrix laser combined with flumethasone in the treatment of chronic eczema is significant and has reference value.

667 The Relationship between Vitamin D Levels and the Survival Status of Patients with Ovarian Cancer

Objective: To explore the relationship between vitamin D levels and the survival status of patients with ovarian cancer. Method: Total 168 patients with ovarian cancer were included during August 2009 to January 2011, then 5 years follow up was conducted, Log rank test and variance analysis were used to compare the progression free survival (rate), 5-year overall survival (rate), and Karnofsky score among patients with different serum 25(OH)D levels. Results: The median progression-free survival and 5-year progression-free survival were not significantly difference among patients with different serum 25(OH)D levels (χ2=2.298, log-rank P=0.513); the median overall survival and 5-year overall survival were significantly difference among patients with different serum 25(OH)D levels (χ2=12.342, log-rank P=0.006), and patients with serum levels of 25.0~49.9 nmoL/L had longest median overall survival, while 50.0~74.9 nmoL/L had highest overall survival rate; Karnofsky scores was also no significantly difference among patients with different serum 25(OH)D levels (P>0.05). Conclusion: The vitamin D levels are associated with the 5 year total survival rate in patients with ovarian cancer, and the survival status of 25.0~74.9 nmoL/L in serum 25 (OH) D was better.

Objective: To observe the clinical effect of limited open reduction and percutaneous Kirschner wire fixation for the treatment of complex fractures of children with Gartland supracondylar humerus. Methods: During the September 2016 to May 2017, the clinical controlled study was conducted in 80 cases of fractures in children in our hospital, according to patients with single and double were as the experimental group (closed reduction method) and control group (limited open reduction method), 40 patients in each group. The statistical difference between the two groups of surgical indexes, the activity of the elbow joint before and after treatment, the ROM score of the elbow joint, and the complications were compared. Results:The difference between the two groups was statistically significant(P<0.05). The difference between the two groups was statistically significant (P<0.05), and the experimental group was less than the control group. Two groups of patients after surgery(before), at discharge (buckling, pronation and supination) index differences statistically significant (P<0.05); the experimental group and control group two groups of patients with different time points (buckling, pronation and supination) index differences statistically significant (P<0.05); Two groups of patients discharged from hospital when unbend spasm, scope of flexed index differences statistically significant (P<0.05), the experimental group and control group two groups of patients with different point straight spasm, buckling cramps, scope of flexed index differences statistically significant (P<0.05); The difference between the two groups of patients was statistically significant (P<0.05). Conclusion: Limited open reduction and Kirschner wire fixation in treatment of children with complex compared to closed reduction and internal fixation of Gartland type III supracondylar fracture of the humerus, its curative effect is safe and effective, and patients in the recovery of limb function has certain clinical advantages.

Objective: To investigate the clinical efficacy of pedicle screw fixation combined with improved expansive open-door laminoplasty in treatment of chronic compressive cervical myelopathy. Methods: 80 patients with chronic compressive cervical myelopathy treated in our hospital from May 2015 to February 2017 were selected as research object. According to random number table, patients were divided into control group (n=40) which purely took improved expansive open-door laminoplasty; research group (n=40) which took pedicle screw fixation combined with improved expansive open-door laminoplasty. The nerve function, cervical curvature, range of motion (ROM) changes and incidence of complications after surgery for 3 months in two groups were compared. Results:After surgery for 3 months, the Japanese orthopaedic association (JOA) scores of two groups were higher than before surgery, and research group was higher than control group (P<0.05); After surgery for 3 months, the cervical curvature, ROM of two groups were lower than before surgery, but research group was higher than control group (P<0.05); After surgery, the incidence of axial symptoms of research group was lower than that of control group (P<0.05); There was no statistical difference in the incidence of added cervical instability, posterior neck myasthenia, infection compared between the two groups (P>0.05).Conclusion: Pedicle screw fixation combined with improved expansive open-door laminoplasty for treating chronic compressive cervical myelopathy has significant efficacy, which can effectively improve nerve function, reduce the loss of cervical curvature and ROM, decrease risks of postoperative complications.

The cleaning brush of gastroscopy and colonoscope biopsy pipe used steel wire - plastic brush head, brushing time 5 seconds, water temperature 35 -38 C, and brushing endoscopic biopsy pipe 3 times.

682 A Report of 10 Cases of Giant Nerve Schwannoma in the Mediastinum and Literature Review

Objective: To investigate the clinical features, diagnosis, treatment and prognosis of the giant neurilemmoma of the mediastinum. Methods: The clinical data and follow-up data of 10 cases of giant neurilemmoma were analyzed retrospectively. Results:In 10 patients with giant schwannoma of the mediastinum, 8 were male and 2 were female. All the tumors were completely removed. The pathology proved to be neurilemmoma. There was no serious complication after operation without surgical death. After 1 months of operation, 2 patients were admitted again to hospital for a large amount of pleural effusion and dyspnea, and were discharged after closed thoracic drainage. Conclusion: Surgery is the main method for the treatment of giant nerve schwannoma in the mediastinum. Should pay more attention on pulmonary edema, respiratory failure and other complications of postoperative.

689 Performance Evaluation of Glycosylated Hemoglobin by Ion Exchange High Performance Liquid Chromatograph MQ-2000PT

Objective: To evaluate the performance of automatic glycohemoglobin analyzer MQ-2000PT for the determination of HbA1c (glycosylated hemoglobin A1c) by the method of HPLC (ion-exchange highperformance liquid chromatography). Methods: The linearity, carryover rate and precision of MQ-2000PT were calculated. Among 41 healthy subjects, the reference interval of HbA1c was also verified.Results: The within-run of both high and low levels of HbA1c were 0.45%、0.58%,and between-run coefficients of variation ( CV) were 0.92%、1.12%,respectively. An perfect linearity was found for HbA1c in the range of 4.6%~15.95% (R2 =0.9998, P<0.001). There was no significantly carryover rate between high and low level samples, the carryover rate was only 0.997%.The distribution range of HbA1c among the 41 healthy subjects were 4.4%-6.2 %, the determination results of 39 subjects(95.1%) were in the range(4.0%-6.0 %) provided by manufacturers. Conclusions: The performance of MQ-2000PT for the determination of HbA1c is good, and can be used for routine clinical determination.

697 Study on Improving the Quality of Life of Cancer Patients after Discharge from Patients with Cancer Pain

Objective: To investigate the effects of transitional care intervention on the quality of life and the effect of the transitional care on the patients with cancer pain,explore the new transitional care model to fit the patients with cancer pain. Methods: Patients with cancer pain hospitalized and discharged from our hospital from October 2015 to October 2017 were randomly divided into two groups (n=35). The control group was given traditional nursing intervention and the observation group was given multi-faceted transitional care, then we compare the differences of the two groups in the pain grade, pain relief and quality of life after different nursing interventions . Results:After different nursing interventions, NRS scores of the observation group were significantly better than that of the control group (P<0.05). The rate of pills in the observation group was higher than that of the control group (P<0.05). After different care intervention in the two groups, the remission rate of the observation group reached 77.1%, significantly higher than the remission rate of the control group (57.1%).The difference between the two groups was statistically significant (P<0.05). After different nursing interventions, the appetite, sleep, relatives' understanding and coordination, attitude toward treatment and adverse reactions of the observation group was significantly better than that in before-intervention (P<0.05). But only appetite and sleep of control group were better than that in before-intervention. The scores of quality of life in both groups increased after intervention, and the scores of quality of life in the observation group was higher than that of the control group (P<0.05). Conclusion: The multi-faceted transitional care model can effectively relieve pain in patients with cancer pain and improve the quality of life after discharge.

701 Therapeutic Effect of Acupuncture Combined with Modified BuYang HuanWu Soup on PSD and its Influence on the Results of HAMD SDS and BECK Scales

Objective: To explore the effect of acupuncture combined with Jiawei Buyang Five Decoction on post-stroke depression (PSD) and its effect on the results of Hamilton Depression Scale (HAMD), self rating Depression Scale (SDS) and Beck self rating Depression Scale (BECK). Methods: 84 patients with PSD were selected and divided into 2 groups according to the random number table method, with 42 cases in each group. The patients in the control group were treated with oral administration of Buyang Huanwu Decoction and five decoction. The observation group was treated with acupuncture combined with modified Buyang Huanwu Decoction and five decoction. The clinical efficacy, serum inflammatory factors, HAMD, SDS and BECK scales were compared between the two groups after 4 weeks of continuous treatment. Results:After treatment, the control group was markedly effective in 15 cases, effective in 18 cases, ineffective in 9 cases, the total effective rate was 78.57%. The observation group was markedly effective in 21 cases, effective in 19 cases and ineffective in 2 cases, the total effective rate was 95.24%, and there was a statistically significant difference between the two groups (P<0.05). Two groups of patients before treatment of HAMD, SDS and BECK scores showed no significant difference (P>0.05); after treatment, the observation group were HAMD, SDS and BECK scores were lower than the control group, there was a significant difference between the groups (P<0.05). Conclusion: Acupuncture combined with modified Buyang Huanwu five decoction is effective in the treatment of post-stroke depression. It can reduce the depression score, and improve the depressive symptoms of patients.
