Analgesic Effect of Sevoflurane Inhalation combined with Dizosin on Parturients with Gestational Hypertension during Cesarean Section and Prevention of Shivering

Objective:
To explore the analgesic effect of sevoflurane inhalation combined with dizosin on parturients with gestational hypertension during cesarean section and the prevention effect of shivering.
Methods:
80 cases of pregnant women with high blood pressure admitted to our hospital from May 2014 to May 2017 were selected as the study subjects. They were divided into observation group (n=45) and control group (n=35) by random number table method. The control group was anesthetized with sevoflurane, and after the birth of the fetus, tramadol was given intravenously, on the basis of the control group, and the observation group was injected with dezocin. The occurrence of shivering after anesthesia was compared between the two groups. Visual Analogue Scale/Score (VAS) was used to score the rest and exercise time, recovery time, incidence of restlessness and effective analgesia time at each time point after operation. The occurrence of adverse reactions in the two groups was recorded.
Results:
The incidence of cold war in the observation group was 8.89%, which was significantly lower than 34.29% in the control group (P<0.05). The VAS scores of both groups showed a decreasing trend in both maternal resting and activity, and the observation group was lower than the control group at 4h, 8h, 12h, 24h and 48h after surgery, and the difference was statistically significant (P<0.05). There was no significant difference between the two groups. The incidence of wakeful observation group was significantly lower than that in the control group (P<0.05). The effective analgesic time control group was significantly smaller than the observation group, and the difference was statistically significant (P<0.05). The incidence of adverse reactions in the observation group was 6.67%, which was significantly lower than that in the control group (P<0.05).
Conclusion:
Sevoflurane inhalation combined with diazoxin during caesarean section in hypertensive women during pregnancy has a significant analgesic effect and can effectively prevent the incidence of cold war after anesthesia. It is safe and effective and worthy of promotion and application.

Relationship between 25 (OH) D Level and Peripheral Neuropathy in Patients with Type 2 Diabetes Mellitus

Objective:
To analyze the relationship between the level of 25-hydroxyvitamin D [25 (OH) D] and peripheral neuropathy in patients with type 2 diabetes mellitus (T2DM).
Methods:
183 cases of T2DM admitted to our hospital were selected as research object. According to whether they were accompanied by peripheral neuropathy, the group was divided into group A with peripheral neuropathy in 86 cases, group B without peripheral neuropathy in 97 cases. The differences in the level of biochemical indexes of the two groups were compared, and the factors affecting the incidence of peripheral neuropathy in patients with T2DM were analyzed by multiple factor Logistic regression, and the relationship between the clinical indexes and the occurrence of peripheral neuropathy was analyzed by Pearson correlation.
Results:
The serum 25 (OH) D levels in the group A was significantly lower than that of the group B, while the ages, the course of the disease, the hemoglobin A1C (HbA1c) and the homeostasis model assessment for insulin resistance index (HOMA-IR) were significantly higher than those in the group B (all P<0.05). The results of multiple factor Logistic regression analysis showed that ages, course of disease and serum level of 25 (OH) D were the main factors affecting peripheral neuropathy in patients with T2DM (all P<0.05). The Pearson correlation analysis showed that the ages, course of disease and the level of HbA1c in patients with T2DM were positively correlated with the occurrence of peripheral neuropathy, while the level of serum 25 (OH) D and HOMA-IR was negatively correlated with the occurrence of peripheral neuropathy (all P<0.05).
Conclusion:
There is a significant relationship between vitamin D deficiency in serum and peripheral neuropathy in patients with T2DM. Serum 25 (OH) D deficiency causes the risk of peripheral neuropathy in patients, and the risk of perinatic neuropathy is also increased with ages, course of disease and HbA1c levels.

Therapeutic Effect of Bacillus Subtilis combined with Mesalazine Enteric-coated Capsules on Patients with Ulcerative Colitis and the effects of serum SOD MDA and TNF-alpha

Objective:
To analyze the effect of bacillus subtilis adn enterococcus faecium enteric-coated capsule joint mesalazine on SOD, MDA, and TNF-α in patients with ulcerative colitis.
Methods:
92 patients with ulcerative colitis was divided into research group (n=46) and control group (n=46) according to simple random methord. The control group was treated with mesalazine, qid. The study group was treated with mesalazine combined with Bacillus subtilis enteric-coated capsules, tid. The two groups were treated for 2 months. The clinical efficacy, colon inflammation and clinical symptom score, SOD, MDA, TNF-alpha, and adverse reactions were compared and observed between the two groups.
Results:
After treatment, the total effective rate of research group was 91.30% higher than the control group 76.08% (P<0.05). Colon inflammation score, clinical symptom scores, MDA, TNF -αof research group was lower than the control group (5.11±0.62)points, (0.64±0.08)points, (6.06±0.74) nmol/ml, (20.29±2.50) μg/L vs (7.80±0.93) points, (1.12±0.14) points, (7.23±0.92) nmol/ml, (23.76±2.88) μg/L, SOD of research group was higher than the control group (1.45±0.18) U/ml vs (1.28±0.16) U/ml (<0.05). The adverse reaction was not observed between two group (P>0.05).
Conclusion:
The effect of bacillus subtilis adn enterococcus faecium enteric-coated capsule joint mesalazine in treatment of ulcerative colitis is better than methadazine. It may be associated with better regulation of SOD, MDA, and TNF-α.

has-miR-1-3p Inhibits Proliferation and Invasion of Human Trophoblast Cells Via Regulating BDNF/TrkB Pathway

Objective:
To investigate the role and mechanism of has-miR-1-3p in proliferation and invasion of human trophoblast cell line HTR-8/Svneo.
Methods:
HTR-8/Svneo cells was transfected with has-miR-1-3p mimics as experimental group, and nonsense miRNAs were used as the control group. CCK8 and Transwell assays were used to detect the effects of has-miR-1-3p on cell proliferation and invasion. The target genes of has-miR-1-3p were predicted by an online analysis website. The regulation of has-Mi-1-3p on target genes was verified by dual luciferase report and Western blot assays.
Results:
has-miR-1-3p mimics significantly decreased cell proliferation and invasion of HTR-8/Svneo cells compared to the control cells. Moreover, the dual luciferase reporter and Western blot assays indicated that has-miR-1-3p mimics could directing target BDNF, and the phosphorylation level of TrkB was also inhibited. Furthermore, up-regulation of BDNF significantly reversed the inhibitory effects of has-miR-1-3p mimics on proliferation and invasion in HTR-8/Svneo cells.
Conclusion:
has-miR-1-3p inhibits proliferation and invasion of human trophoblast cells via regulating BDNF/TrkB pathway.

The Effect of Ropivacaine Combined with Sufentanil Epidural Anesthesia on Labor-Induced Abortion in Labor of Vaginal Delivery and its Influence on Maternal Birth and Maternal-Infant Outcome

Objective:
To study the application of ropivacaine combined with sufentanil epidural anesthesia in primipara of vaginal delivery of painless childbirth and its impact on labor, maternal
Methods:
108 cases of primiparous women receiving vaginal delivery in our hospital from January 2017 to December 2017 were enrolled in this study. All the patients were divided into control group and observation group according to random number table method, 54 cases in each group. The women in the control group were given epidural ropivacaine alone for anesthesia, and the women in the observation group were given epidural anesthesia with ropivacaine and sufentanil. The analgesic indexes of the two groups were compared, the degree of pain in different time, the duration of labor and the maternal and infant outcome.
Results:
The observation group maternal pain onset time, complete block time were lower than the control group, duration of analgesia was higher than the control group, the difference was statistically significant (P<0.05); The VAS scores of pain in two groups were lower than that before anesthesia 10min, 1h and 2h after anesthesia, and the difference was statistically significant (P<0.05). The observation group maternal first stage of labor, the second stage of labor, the third stage of labor time were lower than the control group, the difference was statistically significant (P<0.05); The incidence of adverse maternal-infant outcomes in the observation group was 5.6% (3/54) lower than that in the control group (18.5%, 10/54), the difference was statistically significant (P<0.05).
Conclusion:
Ropivacaine combined with sufentanil epidural anesthesia in vaginal delivery of primiparous painless childbirth application effect is very significant. It can effectively improve the analgesic effect of anesthesia, and reduce the pain of childbirth delivery, shorten the labor process, reduce the adverse of mother and child. The outcome is worth clinical promotion.

Curative Efficacy of Leflunomide in Treatment of Systemic Lupus Erythematosus and Effects on Serum BAFF MCP-4 IL-8 and IL-10 Levels

Objective:
To investigate the clinical efficacy and safety of leflunomide in the treatment of systemic lupus erythematosus (SLE) and its effects on serum levels of B cell activating factor (BAFF), monocyte chemoattractant protein (MCP-4), interleukin-8 (IL-8) and interleukin-10 (IL-10).
Methods:
96 patients with systemic lupus erythematosus admitted to our hospital from September 2014 to September 2016 were selected. According to the simple random number table, all the patients were divided into the observation group (n=48) and the control group (n=48). The control group was treated with glucocorticoid, while the observation group was treated with leflunomide. Then the clinical effect, the changes of 24 h urinary protein, SLEDAI scores, serum levels of BAFF, MCP-4, IL-8 and IL-10, and the incidence of adverse reactions in the two groups before and after treatment were observed and compared between the two groups.
Results:
After treatment, the total effective rate of observation group was significantly higher than that of the control group (P<0.05). Before treatment, there was no significant difference in the 24h urinary protein, SLEDAI score, serum BAFF, MCP-4, IL-8 and IL-10 levels between the two groups (P>0.05); after treatment, the 24h urinary protein, SLEDAI score, serum BAFF, MCP-4, IL-8 and IL-10 of the two groups were significantly improved compared to those before treatment (P<0.05), the 24h urinary protein, SLEDAI scores, serum BAFF, MCP-4, IL-8 and IL-10 levels of observation group were significantly lower than those of the control group (P<0.05); the incidence of adverse reactions in the observation group was significantly lower than that of the control group (P<0.05).
Conclusion:
Leflunomide can enhance the clinical efficacy in the treatment of systemic lupus erythematosus, which may be related to the inhibition of serum BAFF, MCP-4, IL-8 and IL-10 levels.

Curative Efficacy of Qingkailing Dripping Pills on Chronic Pharyngitis and its Effects on Serum EGF IL-1 beta and VCAM-1

Objective:
To study curative efficacy of Qingkailing dripping pills in treatment of chronic pharyngitis and effects on serum epidermal growth factor (EGF), interleukin-1β (IL-1β) and Vascular endothelial cell adhesion molecule-1(VCAM-1) levels.
Methods:
120 patients of chronic pharyngitis admitted to our hospital from October 2015 to October 2017 were selected as research objects, according to the simple random number table, those patients were divided into the 62 cases of observation group and the 58 cases control group, the control group was treated with conventional dexamethasone sodium phosphate+gentamicin atomization treatment, and the observation group was treated with Qingkailing dripping pills on the basis of the control group. They were treated with 1 weeks for 1 courses, and 3 consecutive courses of treatment. The clinical efficacy, the changes of serum inflammatory factors, EGF, IL-1β and VCAM-1 were compared between the two groups, and the adverse reactions and the 3 months recurrence rate were compared.
Results:
After treatment, the total effective rate of the observation group was 93.55% (58/62), which was significantly higher than that of the control group 79.31% (46/58) (P<0.05); after treatment, the serum tumor necrosis factor-α (TNF-α), C reactive protein (CRP) and interleukin -6 (IL-6) in the two groups were significantly lower than those before the treatment (P<0.05), and the serum -α, CPR and IL-6 in the observation group were significantly lower in the observation group than that of the control group [(94.52±13.89)ng/L vs(21.37±17.43)ng/L,(13.42±1.68)mg/L vs(22.30±2.93)mg/L,(110.27±14.19)ng/L vs(158.33±19.34)ng/L](P<0.05). After treatment, the serum IL-1β in the two groups were significantly lower than those before the treatment, and the serum EGF and VCAM-1 was significantly higher (P<0.05), and the IL-1β in the observation group were significantly lower than that of the control group, and the EGF and VCAM-1 was significantly higher than that of the control group[(82.37±8.90)pg/ml vs(114.06±12.17)pg/ml, (4.42±0.72)μg/L vs(3.37±0.68)μg/L, (8.19±1.40)ng/ml vs(5.68±1.06)ng/ml](P<0.05). During the treatment period, the incidence of adverse reactions in the observation group and the control group was 9.68% (6/62) and 8.62% (5/58) respectively, and the difference was not statistically significant(P>0.05). The follow-up results of 3 months follow-up showed that 2 cases relapsed in the observation group, the recurrence rate was 5.26% (2/38), and 7 cases relapsed iin the control group, the recurrence rate was 25% (7/28), and the recurrence rate of the observation group was significantly lower than that of the control group (P<0.05).
Conclusion:
Qingkailingdi pills is well for chronic pharyngitis, which can effectively promote the recovery of the disease, and the recurrence rate is low in the short term. It's internal mechanism may be related to the regulation of the expression of serum EGF, IL-1β and VCAM-1, and it's highly safe and worthy of application.

Therapeutic Effect and Drug Safety of Safflower Yellow Intramural Injection in Patients with Myofascial Pain Syndrome

Objective:
To investigate the clinical effect of carthamin yellow body wall injection for myofascial pain syndrome, and to observe the safety of the drug.
Methods:
80 patients with myofascial pain syndrome diagnosed in our hospital from July 2016 to June 2017 were selected. The data of 80 patients were analyzed. They were randomly divided into the control group and the observation group, 40 cases in each group. The observation group was given carthamin yellow which use body wall injection, while the control group was treated with dexamethasone sodium phosphate injection. The improvement of clinical symptoms in the two groups was compared with the visual analogue scale (VAS) to evaluate the degree of pain in the two groups of patients after 2, 4, 6 and 8 weeks, and the clinical results were compared before and after treatment in the group.
Results:
The ratio of clinical symptoms improvement in the observation group (82.5%) was significantly higher than that in the control group (67.5%). There was no significant difference in the VAS scores between the two groups before and after treatment (P>0.05). After 2, 4, 6 and 8 weeks of treatment, the VAS score of the observation group was significantly lower than that of the control group (P<0.05). There was no significant difference in blood routine, liver and kidney function before treatment and after treatment in the observation group (P>0.05).
Conclusion:
The clinical application of carthamin yellow body wall injection can obviously reduce the pain degree of the patients, and the curative effect is stable. The quality of life of patients is greatly improved, and almost of the patients have no obvious side effects, and the drug safety is worth widely popularizing in clinical practice.

Effects of Azithromycin Combined with Pulmicort Respiratory Aspiration on Mycoplasma Pneumoniae Infection in Children and ts Effect on Immune Function

Objective:
To study the effects of azithromycin combined with pulmicort respiratory aspiration on mycoplasma pneumoniae infection in children and its effect on immune function.
Methods:
90 children with mycoplasma pneumonia diagnosed in our hospital were randomly divided into observation group and control group, 45 cases in each group. The observation group was treated with azithromycin and pulmicort Respiratory inhalation. The control group was treated with azithromycin alone. Observe and compared the time of complete disappearance of symptoms, changes in serum immunoglobulin (Ig) values, changes in T lymphocyte subsets, changes in hs-CRP and cytokines, and adverse events. occurrence in the two groups.
Results:
After treatment, the time of fever, cough, sputum, chest tightness, wheezing and lung tribal sounds disappeared in the observation group, which was significantly lower than that in the control group (P<0.05). The Ig values of the two groups were increased to varying degrees, and the increase in the observation group was significantly greater than that in the control group. The difference was statistically significant (P<0.05). After treatment, the levels of CD3+, CD4+, and CD4+/CD8+ increased in different degrees in both groups, and CD8+ decreased. The increase and decrease in the observation group were significantly greater than those in the control group, with a significant difference (P<0.05). The levels of hs-CRP and IL-6 in both groups were decreased, and the decrease in the observation group was significantly greater than that in the control group (P<0.05). Both groups had adverse reactions such as nausea, vomiting and abdominal pain, but the incidence of adverse reactions was significantly lower in the observation group than in the control group (P<0.05).
Conclusion:
Compared to azithromycin alone, the use of azithromycin combined with Pulmicort Respiratory Inhalation for the treatment of children with Mycoplasma pneumoniae infection can relieve the symptoms of children quickly disappear, and can improve the immune function of children with no serious adverse reactions. It is worth in clinical application.

Effect of Pidotimod Adjuvant Therapy on T lymphocyte Subsets and Inflammatory Factors in Children with Hand Foot Mouth Disease

Objective:
To study the effect of pidotimod in treatment of children with hand-foot disease and its effects on T lymphocyte subsets and inflammatory factors.
Methods:
800 children with hand-foot-mouth disease admitted to Xi'an Children's Hospital were divided into observation group and control group according to the order of admission, 400 cases in each group. The control group was treated with ribavirin, and the observation group was treated with pidotimod. The herpes elimination time, body temperature return time, hospital stay, T-lymphocyte subsets, inflammatory factors, and immunoglobulin were compared between the two groups.
Results:
The herpes elimination time, normal body temperature return time, and length of stay in the observation group were significantly less than those in the control group (P<0.05). Before treatment, there was no significant difference in the CD3+, CD4+, CD8+, and ratios of CD4+/CD8+ cells between the two groups (P>0.05). After treatment, the CD3+, CD4+, CD8+, and ratios of CD4+/CD8+ cells in the observation group were significantly higher than those in the control group(P<0.05). Before treatment, there was no significant difference in the levels of IL-6, IL-10, IL-1β and TNF-α between the two groups (P>0.05). After treatment, the levels of IL-6, IL-1β and TNF-α in the observation group were significantly lower than the control group (P<0.05), IL-10 levels were significantly higher than the control group (P<0.05). Before treatment, the levels of IgA, IgG, and IgM in the two groups were not significantly different (P>0.05). The IgA and IgG levels in the observation group were significantly higher than those in the control group (P<0.05).
Conclusion:
Pidotimod adjuvant therapy can effectively improve T lymphocyte immune function and inflammatory factors in children with hand-foot and mouth disease, and promote the recovery of children disease, with good clinical efficacy.

Effect of Functional Exercise on Obese Patients with Lumbar Disc Herniation treated by Nerve Root Blocking combined with Intervertebral Foramen Endoscopy

Objective:
To explore the effect of functional exercise on nerve root closure combined with intervertebral foramen in the treatment of obese patients with lumbar disc herniation.
Methods:
From December 2015 to December 2017, 80 patients with lumbar disc herniation were selected as study subjects. All patients were treated with nerve root block combined with intervertebral foramen endoscopy. According to the different intervention methods, each group was divided into observation group and control group with 40 cases in each group. The patients in the control group were given traditional postoperative nursing care after treatment, and the patients in the observation group were treated with functional exercise intervention on the basis of the control group. After two courses of treatment, the patients were followed up within half a year after discharge, and the lower waist and leg functions of the two groups were observed and recorded. Spinal stability (including lumbar curvature). Kyphosis index, sacral obliquity angle, average intervertebral space height) And lumbar function (including residual lumbar leg pain, straight leg elevation, muscle strength, skin sensation and nerve reflex).
Results:
The total effective rate of lower lumbar leg function in the observation group was 95%, which was significantly higher than the control group's 72.5%. The difference between the two groups was statistically significant (P<0.05). And lumbar curvature (1.45±0.53) cm lower than the control group (1.81±0.59) cm, lordosis index (2.24±0.66) cm and sacral slope Angle (30.28±5.59) ° was significantly higher than in the control group (1.81±0.72) cm and (26.64±5.36) °respectively, and are statistically significant (P<0.05); In the mean intervertebral space height, there was no statistically significant difference between the two groups (P>0.05). Residual lumbar leg pain, straight leg elevation, muscle strength, skin sensation, nerve reflex and total score were significantly lower than those in the control group, and the difference between the two groups was statistically significant (P<0.05).
Conclusion:
Functional exercise intervention has a significant effect on the treatment of obese patients with lumbar disc herniation by nerve root closure combined with intervertebral foraminoscopy. It effectively improves the lower lumbar and leg motor function of patients, and largely improves the rehabilitation effect of patients.

The Clinical Effect of Minimally Invasive Combined with Open Surgery for Tibia Plateau Fractures with Meniscus Injury

Objective:
To investigate the incidence of tibia plateau fractures with meniscus injury and the clinical effect of minimally invasive combined with open surgery.
Methods:
The incidence of tibia plateau fractures combined with meniscus injury in the type of Schatzter of 360 patients was calculated.Then the patients of type Ⅰ and part of the joint surface didn't subsidence Ⅳ and Ⅴ tibial plateau fractures combined with meniscus injury were divided into minimally invasive open surgery treatment group and conventional open surgery treatment group. The surgical efficacy, the incidence of postoperative complications and the postoperative knee function were observed in the two groups of different injury type.
Results:
In 360 patients, the incidence of Schatzker Ⅰ, Ⅱ Ⅲ, Ⅳ Ⅴ, Ⅵ tibial plateau fractures combined with meniscus injury was 19.2%, 47.7%, 20.8%, 21.5%, 29.8% and 35.7%, the overall incidence of 28.6%. Compared with the conventional open surgery treatment group, minimally invasive combined with open surgery regimen showed no significant improvement in the incidence of postoperative complications (P>0.05), and significantly increased the surgical efficacy (P<0.01) and postoperative knee joint function (P<0.05).
Conclusion:

Changes of Serum CD40L and VCAM-1 Expression in Children with Bacterial Pneumonia and their Correlation with Disease Severity

Objectives: To analyze the changes of serum CD40L and VCAM-1 levels in children with bacterial pneumonia and their correlation with the severity of infection.
Methods:
From September 2014 to March 2016, 125 children with bacterial pneumonia were selected as the study subjects, 116 children with non-bacterial pneumonia and 138 children with outpatient health examination were selected as the control group. The differences of serum CD40L and VCAM-1 levels between the three groups were compared. The correlation between the level of serum CD40L and VCAM-1 and the severity of infection in children with bacterial pneumonia were analyzed.
Results:
Statistics results showed that the levels of CRP, PCT, CD40L and VCAM-1 in serum of children with bacterial pneumonia and non-bacterial pneumonia were significantly different from those of healthy children (P<0.05). The serum levels of PCT, CD40L and VCAM-1 in children with bacterial pneumonia were higher than those in children with non-bacterial pneumonia (P<0.05). The serum levels of CRP, PCT, CD40L and VCAM-1 in children with bacterial pneumonia of different infection degrees were (67.44+19.26 vs 102.06+39.22) (mg/L), (3.55+1.38 vs 11.66+2.07) (ng/mL), (2.47+0.58 vs 3.15+0.99) (ng/mL), (587.4+165.3 vs 668.1+240.9) (ng/mL) (P<0.05). Pearson analysis showed that serum CRP (r=0.58, P<0.05), PCT (r=0.61, P<0.05), CD40L (r=0.52, P<0.05), VCAM-1 (r=0.57, P<0.05) levels were positively correlated with the severity of infection. Multivariate logistic regression analysis showed that serum CRP, PCT, CD40L and VCAM-1 were all risk factors for the development of bacterial pneumonia (P<0.05).
Conclusion:
The serum levels of CD40L and VCAM-1 in children with bacterial pneumonia were significantly higher, and the more severe the infection was, the higher the levels of serum CD40L and VCAM-1 were. The higher levels of CD40L and VCAM-1 were risk factors for the development of bacterial pneumonia.

Somatostatin Combined with Dexamethasone on Early PostoperativeInflammatory Ileus after Abdominal Operation and its Influence on the Gastrointestinal Function and LPS IL-6 hs-CRP Levels in Patients

Objective:
To investigate the clinical efficacy of somatostatin combined with dexamethasone on early postoperative inflammatory ileus after abdominal operation and its influence on patients' gastrointestinal function and their serum lipopolysaccharide (LPS), interleukin -6 (IL-6), high-sensitivity C-reactive protein (hs-CRP) levels.
Methods:
The subjects of this study were selected from 138 cases of patients with early postoperative inflammatory ileus after abdominal operation admitted in our hospital from May 2015 to June 2017. The patients were according to simple random number table method were divided into observation group and control group, each with 69 cases. Besides conventional treatment given to both groups, dexamethasone was adopted to the control group in addition, and combination of somatostatin and dexamethasone was given to the observation group additionally. The clinical efficacy, the recovery status of gastrointestinal function, and changes of serum LPS, IL-6, hs-CRP levels before and after treatment were compared between the two groups.
Results:
The overall effective rate of the observation group was 91.30% (63/69), much higher than 75.36% (52/69) of the control group (P<0.05). The time for resuming of peristaltic sound, postoperative exhausting, release of abdominal pain and distention, extinction of nausea and vomitting that the observation group took was significantly shorter than the time the control took (P<0.01). After treatment, both groups saw great decrease in serum LPS, IL-6 and hs0CRP levels as compared with those before treatment (P<0.01), and observation group had remarkably lower levels of above indicators than the control group did at the identical time points after treatment (P<0.01).
Conclusion:
Application of somatostatin combined with dexamethasone in treating early postoperative inflammatory ileus after abdominal operation is effective in promoting the recovery of gastrointestinal function and mitigating inflammatory reactions.

Growth Condition after Auricle Reconstruction and Factors Affecting the Formation of Cranioauricular Angle

Objective:
To explore the growth of reconstructed auricles and analyze the related factors affecting the formation of cranioauricular angle.
Methods:
The clinical data of 120 patients with unilateral auricular reconstruction were retrospectively analyzed. The auricle projection areas of reconstructed ear and normal ear were compared before operation and at 3 months, 6 months and 12 months after operation. According to formation of cranioauricular angle of reconstructed ear, the patients were divided into poor group (n=24) and satisfactory group (n=96). The clinical data of the two groups were compared by univariate analysis, and the items with statistically significant differences in the comparison result were given Logistic multivariate stepwise regression analysis.
Results:
At 3 months, 6 months and 12 months after operation, the auricle projection area of reconstructed ear was significantly higher than that before operation, and there was no statistically significant difference compared with that in normal ear at the same time (P>0.05). There were no significant differences between the two groups in gender, reconstructed auricles, skin blisters, ear piercing or skin care (P>0.05), but age≥13 years old, scar hyperplasia and no use of ear protectors were independent risk factors for poor formation of cranioauricular angle (P<0.05).
Conclusions:
Auricle growth is good in patients after auricle reconstruction, and the surgical procedure is very feasible. And the effect of cranioauricular angle formation is closely related to age, scar hyperplasia and the use of ear protectors. Clinical diagnosis and follow-up interventions are required to cause enough attention.

The risk factors of type 2 diabetes mellitus combined with left wentricular diastolic dysfunction

Objective:
Although diabetes mellitus is an indepent risk of heart failure with preserved ejection fraction, the underlying mechanisms leading to c remain poorly understood. The study was aimed to access the risk factors for LVDD in patients with type 2 diabetes mellitus.
Methods:
101 asymptomatic patients with type 2 diabetes mellitus without overt heart disease were enrolled in this study. Patients with New York Heart Association class>1, ejection fraction<50%, history of coronary artery disease, severe valvulopathy, chronic atrial fibrillation, or creatinine clearance<30 mL/min, as well as those receiving insulin treatment, were excluded. Among all 101 patients, 43 patients were found combined with heart failure with preserved ejection fraction. Left ventricular diastolic function was estimated as the ratio of early diastolic velocity (E) from transmitral inflow to early diastolic velocity(e') of tissue Doppler at mitral annulus(E/e'). Total patients were divided into two groups, normal and heart failure with preserved ejection fraction. Parameters of glycemic control, plasma insulin concentration, lipid profile, treatment with antidiabetic drugs, and other clinical characteristics were evaluated, and their association with heart failure with preserved ejection fraction determined.
Results:
Results showed that sex, duration of diabetes, systolic blood pressure, pulse pressure, fasting insulin concentration, and sulfonylurea use have significant differences between two groups .Logistic analysis showed that Sex (female), duration of diabetes, hypertension, hyperinsulinemia and sulfonylurea use maybe risk factors in the development of LVDD in patients with type 2 diabetes mellitus .
Conclusion:
Sex (female), duration of diabetes, hypertension, hyperinsulinemia and sulfonylurea use maybe risk factors in the development of LVDD in patients with type 2 diabetes mellitus.

Effect of Pedicle Screw Fixation Combined with Posterolateral Bone Grafting and Fusion on Perioperative Indexes, JOA score, VAS score and Complications in Patients with Lumbar Spondylolisthesis

Objective:
To observe the effects of pedicle screw fixation combined with posterolateral lumbar fusion (PLF) on perioperative indexes, JOA score, VAS score and complications in patients with lumbar spondylolisthesis.
Methods:
A total of 90 patients with lumbar spondylolisthesis treated in the hospital from February 2013 to December 2016 were enrolled in the study. They were randomly divided into 2 groups with 30 patients in each group. Group A was treated with pedicle screw fixation combined with PLF, while group B was treated with pedicle screw fixation combined with intervertebral fusion. The spondylolisthesis rate, reduction rate, reduction loss rate and height loss rate were compared between the 2 groups at different time points. The JOA scores, VAS scores, therapeutic effects, operative indicators (intraoperative blood loss and operative time), postoperative bone graft fusion rate and complications in both groups were observed before treatment and at the end of follow-up.
Results:
The treatment was effective in both groups (P>0.05). There was no statistically significant difference in the operative time or intraoperative blood loss between the 2 groups (P>0.05). The bone graft fusion rate in group B was significantly higher than that in group A (P<0.05), but there was no significant difference in JOA score or VAS score between the two groups before operation and at the end of follow-up (P<0.05). JOA scores of both groups were significantly higher than while VAS scores were significantly lower than those before operation (P<0.05). The spondylolisthesis rate, height loss rate, reduction loss rate and the total incidence of complications in group A were significantly higher than those in group B during follow-up (P<0.05).
Conclusion:
Pedicle screw fixation combined with PLF or intervertebral fusion is effective in the treatment of lumbar spondylolisthesis. Compared with PLF, pedicle screw internal fixation combined with intervertebral fusion can maintain the correction of spondylolisthesis, with a advantage of reducing the risk of postoperative complications.

Effects of Sinomenine combined with Epirubicin on the Changes of Invasion Related Proteins in Melanoma Cells

Objective:
To analyze the effect of sinomenine (SIN) combined with epirubicin (EPI) on the changes of invasion-related proteins in melanoma cells.
Methods:
The invasion status of melanoma A375 cells under different conditions was detected by Transwell. They were divided into control group (given normal saline), SIN group (10 g/mL, 100 g/mL as low concentration and high concentration), EPI group (5 g/mL, 50 g/mL as low concentration and high concentration), SIN+EPI group (10 g/mL, 5 g/mL) and SIN+EPI group (5 g/mL). The expression of matrix metalloproteinase-2 (MMP-2) and matrix metalloproteinase-9 (MMP-9) proteins and their mRNA were detected by RT-PCR and Western blot.
Results:
The inhibition rate of SIN+EPI group on the invasion of melanoma A375 cells was significantly higher than that in group SIN and EPI (P<0.05). Moreover, with the increase of SIN and EPI concentration, the inhibition of SIN and EPI on the invasion of A375 cells increased significantly. Compared with the control group, the levels of MMP-2, MMP-9 protein and mRNA in SIN group, the EPI group and the SIN+EPI group were inhibited in varying degrees. Among them, the inhibition effect of SIN+EPI group was higher (P<0.05).
Conclusion:
The use of low concentration (10μg/mL) SIN can effectively promote the inhibitory effect of low concentration of EPI on the invasion of melanoma cells, and SIN can be used as an important drug for adjuvant chemotherapy. It can play a role in improving the clinical efficacy. SIN combined with EPI chemotherapy drugs can play a synergistic and synergistic effect, and can reduce the dosage of EPI, in order to prevent a series of toxic and side effects caused by high concentration of chemotherapeutic drugs, so it has good safety and effectiveness.

Effects of Shikonin on cell apoptosis of human liver cancer cell line SMMC-7721

Objective:
To study the effect and mechanism of shikonin on apoptosis of SMMC-7721 cells of liver cancer.
Methods:
Different concentration of shikonin were used to interfere with liver cancer SMMC - 7721 cells, determined by MTT method to observe the cell proliferation inhibition, and using flow cytometry to test the apoptosis rate after 24h, and further using PCR and Western blot to detect the mRNA and protein expression of caspase3 and caspase9.
Results:
the growth of liver cancer SMMC-7721 cells was inhibited significantly at 24h, 48h and 72h by shikonin (0, 0.5, 1.0, 2.0, 4.0μg/mL), indicating the dual dependence of time and concentration. According to the results of flow cytometry detection, the apoptosis rate of human liver cancer SMMC-7721 cells was significantly higher than that of the control group after 24 h of action of different concentrations of shikonin (5, 10, 20μg/mL), and the apoptosis rate was significantly increased with the increase of shikonin concentration. In addition, the increased concentration of shikonin can promote the expression of mRNA and protein of caspase3 and caspase9 in human liver cancer SMMC-7721 cells.
Conclusion:
Shikonin can inhibit the apoptosis of SMMC-7721 cells in liver cancer by promoting the mRNA and protein expression of caspase3 and caspase9.

Effects of Alprostadil Combined with Electromyographic Biofeedback Therapy on Limb Function and Excitability of Spinal Motor Neurons in Patients with Spastic Hemiplegia after Stroke

Objective:
To analyze the effects of alprostadil combined with electromyographic biofeedback therapy on limb function and excitability of spinal motor neurons in patients with spastic hemiplegia after stroke.
Methods:
91 patients with post-stroke spastic hemiplegia admitted to our hospital from January 2016 to October 2017 were divided into 43 cases of control group and 48 cases of observation group according to random number table method. The control group was treated with emg biofeedback therapy, and the observation group was treated with alprostadil based on the control group. Then modified Ashworth scale efficacy, limb function, Hmax, Mmax and H/Mmax, neurologic deficit (NDS) and daily life activity (ADL) scores in both groups were compared.
Results:
Effective rate in the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Before treatment, Fugl-Meyer function for upper and lower limbs in both group were no statistically significant difference (P>0.05). After treatment, the fugl-meyer functional scale of upper and lower limbs in both group were increased, and the observation group was higher than the control group, with statistically significant difference (P<0.05). Before treatment, Hmax, Mmax and H/Mmax in both group were no difference (P>0.05). After treatment, Hmax, Mmax and H/Mmax in both groups were decreased, and the observation group was lower than the control group (P<0.05). NDS score in both group was rised, ADL score was falled,and the observation group were more significant change (P<0.05).
Conclusion:
Alprostadil combined with emg biofeedback therapy has a positive effect on the improvement of limb function in patients with post-stroke paralysis. It can reduce muscle tension and reduce spasm by regulating excitability of spinal motor neurons.

Effect of Paclitaxel and Carboplatin combined with Kang Lai Te on Angiogenesis and Invasion in Patients with Cervical Cancer

Objective:
To study the effect of paclitaxel, Carboplatin combined with Kanglaite on angiogenesis and invasion of cervical cancer patients.
Methods:
Eighty-seven cervical cancer patients admitted to our hospital from August 2013 to July 2014 were selected and divided into control group (46 cases) and observation group (41 cases) according to the order of inpatient medical records has been successively. The control group was treated with paclitaxel and carboplatin, and the observation group was combined with Kanglaite injection to complete the treatment, 3 weeks for a course of treatment, continuous treatment of 4 courses. The clinical efficacy, tumor microvessel density (MVD), vascular endothelial growth factor (VEGF), immune function, quality of life, and survival were compared between the two groups.
Results:
After treatment, the total clinical efficacy of the observation group was significantly higher than that of the control group (P<0.05). Before treatment, there was no significant difference in tumor microvessel density (MVD) and vascular endothelial growth factor (VEGF) levels between the two groups (P>0.05). After treatment, the MVD and VEGF levels in the observation group were significantly lower than those in the control group (P<0.05). Objective:
To investigate the effect of perioperative fast track surgery (FTS) on postoperative inflammatory stress reaction and prognosis in patients with colorectal cancer and diabetes mellitus.
Methods:
According to the random number table method,eighty patients with colorectal cancer and diabetes mellitus were divided into two groups:the control group(D group)and the observation group (F group).The patients in the control group were performed with routine perioperative treatment,while the patients in the observation group were given FTS perioperative treatment.Prognosis and the difference of postoperative inflammatory stress reaction between the two groups were observed.
Results:
Therewere no significant differences in age and duration of diabetes between the two groups(P<0.05).The defecation time,anal exhaust time,wound healing time and hospitalization time in group F were significantly shorter than those in group D (P<0.05).At the same time,the weight,hemoglobin and albumin reduction of group F were significantly lower than those in group D (P<0.05);The incidence of postoperative complications in group F (7.50%) was significantly lower than that in group D (32.50%) (P<0.05),and the level of inflammation index (CRP,IL-6,SAA,TNF- a) in group F was significantly better than that in group Dat postoperative 5 days (P<0.05).
Conclusion:
It is of great significance to reduce the incidence of complications,control the level of inflammation,shorten the time of rehabilitation and improve the prognosis by using the FTS concept for the perioperative management of colorectal carcinomacomplicated with diabetes mellitus.

A Clinical Study on the Treatment of Maxillary Retraction Deformity with Modified Maxillary Protrusion Appliance

Objective:
To investigate the effect of maxillary protrusion appliance on the treatment of patients with maxillary retraction deformity during the mixed dentition period with the crowding of the posterior segment of the dental arch.
Methods:
The modified maxillary protrusion appliances were used to treat 40 cases of skeletal class III malocclusion with maxillary retraction during mixed dentition period. A facemask was used on the face to pull maxillary forward, palatal appliance was used to push molars back and palatal expansion in the mouth. For each patient, the lateral cephalogram was taken before and after treatment. Hard and soft tissue changes were analyzed to evaluate the effects of maxillary protrusion.
Results:
The anterior teeth overjet increased by 3.6mm, the average forward movement of point A subspinale was 2.1mm, mandibular plane angle was increased by 1.9° on the average, maxillary first molar average 2.5mm backwards. The position relation between maxillary and mandible was improved obviously, and the lateral appearance of soft tissue was improved obviously.
Conclusion:
The modified maxillary protrusion appliance could effectively treat skeletal class III malocclusion relationship and lateral appearance of the patients with maxillary retraction deformity, and made the first molars moving backward and relieve the dental arch crowding of the posterior segment.

Effect of Total Laparoscopy on Serum Levels of VEGF TGF-alpha and SICAM-1 in Elderly Patients with Primary Hepatocellular Carcinoma

Objective:
To analyze the clinical effect of complete laparoscopy in the treatment of elderly patients with primary hepatocellular carcinoma and its effects on serum vascular endothelial growth factor (VEGF), transforming growth factor-alpha (TGF-alpha), soluble intercellular adhesion molecule-1 (SICAM-1).
Methods:
84 patients with primary hepatocellular carcinoma according to simple random number table method were divided into 44 control group and 40 research group. control group was treated with open hepatectomy, and the research group was treated with complete laparoscopi. surgical index, serum VEGF, TGF-α, SICAM-1, liver function, complications and follow-up in both group were compared.
Results:
There was no significant difference in operative time between the two groups (P>0.05). 1 months and 3 months after operation, the blood loss, feeding time, getting out of bed and hospitalization time in the study group were less than those in the control group (P<0.05). Three days and seven days after operation, the liver function indexes of the two groups were higher than those before operation (P<0.05), but the change range of the study group was smaller, and there was statistical difference between the two groups (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). There was no significant difference in 1-year recurrence rate, 2-year recurrence rate, 1-year survival rate and 2-year survival rate between the two groups (P>0.05).
Conclusion:
Complete laparoscopic treatment of elderly patients with primary hepatocellular carcinoma can reduce serum levels of vascular endothelial growth factor, TGF-alpha and SICAM-1, achieve the same safety and survival rate as open surgery, and have more advantages in surgical trauma and liver function.

Application of Re-puncture Biopsy in Metastatic Castration-resistant Prostate Cancer

Objective:
To explore the clinical significance of secondary prostate biopsy in metastatic castration-resistant prostate cancer (CRPC).
Methods:
63 patients with CRPC in our hospital from July 2014 to July 2017 were randomly selected as the research subjects. All patients underwent secondary prostate biopsy. According to the secondary puncture results, the patients were divided into the combined intra-canal carcinoma (IDC-P) group (43 cases) and the unconsolidated IDC-P group (20 cases). The levels of prostate-specific antigen (PSA) and related parameters at the time of first puncture and second puncture were compared between the two groups. The follow up and prognosis of the patients were observed.
Results:
The first puncture results showed that 13 cases had IDC-P and 50 cases had normal adenocarcinoma. The second puncture results showed 43 cases had IDC-P and 20 cases had normal adenocarcinoma. The PSA, PSAD and PSATZ were (14.28 (+2.39) ng/mL, (0.26 (+0.41) ng/mL and (0.65 (+0.86) ng/mL respectively at the first puncture. At the second puncture, they were (17.32±2.73) ng/mL, (0.43±0.49) ng/mL, (0.99±1.01) ng/mL, respectively. The difference was statistically significant (t=70.968, 16.867, 17.991, P<0.05). The levels of PSA, PSAD, and PSATZ in the combined IDC-P group were (19.39±3.18) ng/mL, (0.57±0.52) ng/mL, and (1.27±1.03) ng/mL, respectively. Higher than the uncombined IDC-P group (15.26±2.59) ng/mL, (0.30±0.39) ng/mL, (0.71±0.81) ng/mL. The difference was statistically significant (t=5.072, 2.064, 2.140, P<0.05). At the mean follow-up (23.65±4.52) months, 11 patients died, and 52 patients (34 patients with IDC-P combined) received chemotherapy after the diagnosis of CRPC. There were 7 patients (20.59%) who had treatment responders in patients with IDC-P and 12 patients (66.67%) who had not responded to IDC-P. Conclusion:
The second biopsy is beneficial to the clinical understanding of the changes of metastatic prostate cancer (IDC-P) in a timely manner and has important guiding value for the treatment and prognosis of CRPC.

Analysis on Application Value of High Frequency Ultrasound Imaging in Diagnosing Chronic Compression Injury of Sciatic Nerve

Objective:
To study and analyze the application value of high-frequency ultrasound imaging in diagnosing chronic sciatic nerve chronic compression injury.
Methods:
60 cases of patients with suspected chronic sciatic nerve compression in our hospital from February 2016 to December 2017 were selected for the study. Among them, 30 cases with chronic compression injury of the sciatic nerve were confirmed as observation group by combined imaging and surgical examination, and 30 cases with chronic compression injury of non-sciatic nerve were control group. All the patients underwent high-frequency ultrasound and electromyography to assess the sciatic nerve injury. The diagnostic results of high-frequency ultrasound and electromyography in chronic compression injury of the sciatic nerve were observed and analyzed. The sensitivity and specificity of the two diagnostic tests were compared.
Results:
The sensitivity of high-frequency ultrasonography in the diagnosis of chronic compression injury of sciatic nerve was 93.33%, the specificity was 91.67%, the accuracy was 91.67%. The sensitivity of electromyography in diagnosis of chronic compression injury of sciatic nerve was 96.67%. The specificity was 93.33% and the accuracy was 91.67%. There was no statistically significant difference between the two groups (P>0.05).
Conclusion:
High-frequency ultrasound imaging has high application value in the diagnosis of chronic compression injury of sciatic nerve with high sensitivity, specificity and accuracy. It can be used as a routine examination method for sciatic nerve injury.

Study on Efficacy and Safety of Agomelatine combined with Transcranial Magnetic Therapy in the treatment of Depression and Anxiety in Perimenopausal Syndrome

Objective:
To study the efficacy and safety of agomelatine combined with transcranial magnetic therapy in the treatment of depression and anxiety in perimenopausal syndrome.
Methods:
70 cases of perimenopausal syndrome were diagnosed and treated in our hospital. According to the random number table method, the patients were randomly divided into the control group who was given the agomelatine, and the combined group was treated with transcranial magnetic therapy on the basis of the control group, with 35 cases in each group. All the patients were treated for 2 months. The clinical efficacy, the Hamilton Anxiety Scale (HAMA), the Hamilton Depression Scale (HAMD) before and after treatment and the incidence of adverse reactions were compared between the two groups.
Results:
The total effective rate of the combined group was 91.43%, which was significantly increased in comparison of the control group in 62.86% (P<0.05). The scores of HAMA and HAMD in the two groups were significantly decreased in comparison of those before treatment, and the rate of reduction in the combined group was more significant than that in the control group (P<0.01). There was no statistically significant difference in the incidence rate of adverse reactions between the two groups during treatment (P>0.05).
Conclusion:
The combined treatment of agomelatine and transcranial magnetic therapy can effectively improve the clinical symptoms of patients with perimenopausal syndrome, reduce negative emotions such as depression and anxiety, and do not increase the occurrence of adverse reactions, with good safety. Therefore, it is worthy of clinical application and promotion.

Clinical Efficacy, Safety and Quality of Life of Aripiprazole Combined with Fluvoxamine in the treatment of Obsessive-compulsive Disorder

Objective:
To study the effect of aripiprazole combined with fluvoxamine on clinical efficacy, survival therapy and safety in patients with obsessive-compulsive disorder.
Methods:
From April 2012 to March 2016, 216 patients with obsessive-compulsive disorder admitted to our hospital were divided into control group (n=103 cases) and study group (n=113 cases) according to different drug regimens. Patients in the control group were given non-convulsive electroconvulsive therapy combined with fluvoxamine. Patients in the study group were given aripiprazole on the basis of the control group. The scores of GQOLI-74, HAMA and Y-BCOS before and after treatment in two groups were compared, the clinical efficacy and safety of medication were compared.
Results:
There was no significant difference in the GQOLI-74 score between the two groups before treatment (P>0.05). The GQOLI-74 score of the study group after treatment was significantly higher than that of the control group, and there was a significant difference (P<0.05). The GQOLI-74 score of the two groups after treatment was significantly higher than that before treatment, and there were significant differences (P<0.05). There was no significant difference in the HAMA score between the two groups before treatment (P>0.05). The HAMA score of the study group after treatment was significantly lower than that of the control group, and there was a statistically significant difference (P<0.05). There was no significant difference in Y-BCOS score between the two groups before treatment (P>0.05). Clinical Analysis of Interventional Cryotherapy in Patients with Bronchial Tuberculosis by Fiberoptic Bronchoscopy

Objective:
To analyze the curative effect of interventional cryotherapy for bronchial tuberculosis patients by fiberoptic bronchoscope.
Methods:
Sixty-eight patients with bronchial tuberculosis admitted to our hospital from October 2016 to October 2017 were randomly divided into control group and experimental group. Both groups were given routine anti-tuberculosis chemotherapy, while the experimental group was given fiberoptic bronchoscopic interventional cryotherapy on this basis. The sputum negative rate, chest CT changes and changes under bronchoscope were compared between the two groups.
Results:
There were 30 sputum positive patients, 4 sputum negative patients, 28 sputum positive patients and 6 sputum negative patients in the control group before treatment. After treatment, there were 14 sputum-positive patients, 20 sputum-negative patients, 9 sputum-positive patients and 25 sputum-negative patients in the control group, respectively. Clinical Efficacy of Hemoperfusion in the Treatment of Severe Abdominal Allergic Purpura in Children

Objective:
To investigate the clinical effect of blood perfusion on children with severe abdominal allergic purpura.
Methods:
28 children with severe abdominal allergic purpura were enrolled in our hospital from August 2013 to March 2018. They were divided into observation group A (blood perfusion) and observation group B according to different treatment methods. Treatment), another 14 healthy children were selected as healthy control group, serum tumor necrosis factor (TNF-α), interleukin-6 (IL-6), plasma malondialdehyde (MDA), superoxide before and after treatment in three groups of children. The level of dismutase (SOD) and total plasma antioxidant capacity (T-AOC) were measured.
Results:
The levels of MDA, TNF-α and IL-6 in group A were significantly lower than those in group B. The levels of T-AOC and SOD were significantly higher than those in group B. The difference was significant (P<0.05). The incidence of rash and gastrointestinal symptoms disappeared significantly in group A, and the difference was significant (P<0.05). The incidence of abdominal pain/rash recurrence and renal damage during follow-up were observed in group A. Compared with the observation group B, the average daily dose of hormone was significantly lower than the latter, and the difference was significant (P<0.05).
Conclusion:
For children with severe abdominal allergic purpura, combined with blood perfusion on the basis of conventional treatment, can effectively improve the clearance of inflammatory mediators and oxidative stress products, and is very positive for restoring the immune mechanism and oxidation balance of the body. To study the effect of traditional bracket and self-ligating bracket on the shape of dental arch in the treatment of crowded patients with maxillary dentition.
Methods:
A total of 82 patients with maxillary dentition admitted to our hospital from Feb. 2016 to Aug. 2017 were enrolled. All patients were divided into control group (n=36 cases) and study group (n=46). example). The patients in the control group were treated with traditional brackets, and the patients in the study group were treated with self-locking brackets. Comparing the upper and lower arch circumferences of the two groups of patients, the maxillary arch length index: the upper segment of the upper arch, the anterior segment of the upper arch, the middle segment of the upper arch, the entire segment of the lower arch, the anterior segment of the lower arch, the middle segment of the lower arch, etc. The maxillary arch width index: the first molar central fossa, the first molar cheek tip, the second premolar fovea, the second premolar cheek tip, the first premolar fovea, the first premolar cheek tip and the canine.

The operation time, intra-operative hemorrhage, hospitalization days ,VASscores and the recurrence rate of the patients in each group were observed.
Results:
The operation time, intra-operative hemorrhage, hospitalization days in the observation group were significantly lower than those in the control group (P<0.05) and the VAS scores on the first day and fifth day after operation in the observation group were significantly lower than those in the control group (P<0.05).
Conclusion:
The curative effect of the treatment of Hemorrhoids with five-point ligation method combined with Xiaozhiling injection sclerotherapy is significant, it is easy to operate with small pain, less blood lossand the patients recover fast.

Methods:

combined with zoledronic acid injection. All patients were treated for 12 months. The clinical effects bone mineral density (BMD), visual analogue scale (VAS), the change of creatinine ratio (U-Ca/Cr)] before and after treatment and the incidence of adverse reactions were compared between the two groups.
Results:
After 12 months of treatment, the total effective rate of the observation group was 93.5% (58/62) significantly higher than that of the control group [80.6% (50/62), P<0.05]. Compared with before treatment, the BMD value of each position (femoral neck, total hip, Wards triangle, lumbar L2 ~ 4) in the two groups increased significantly after treatment (P<0.05), and the BMD value in the observation group increased more significantly (P<0.05). After treatment, the VAS score, serum IL-6, BALP, TRACP-5b concentration and urinary U-Ca/Cr value of the two groups were significantly lower than those before treatment (P<0.05), and the observation group decreased more significantly (P<0.05). The adverse reaction rate of the observation group was 9.7% (6/62) compared with the control group [4.8% (3/62)]. There was no significant difference (P>0.05).
Conclusion:
Zoledronic acid injection combined with calcium carbonate D

To study the clinical efficacy of mirtazapine in the treatment of patients with sleep disorders in perimenopausal period.
Methods:
129 cases of perimenopausal syndrome with sleep disorder were selected and divided into control group (n = 64 cases) and research group (n = 65 cases) according to random number table method. Patients in the control group were treated with oral oryzanol tablets. Patients in the study group were treated with mirtazapine tablets. Efficacy of Simple Thrombectomy and Bridging in treatment of Acute Large Arterial Occlusive Stroke

Objective:
To compare the efficacy of simple mechanical thrombectomy and intravenous thrombolysis in the treatment of acute macrovascular occlusive stroke.
Methods:
80 patients with anterior circulation aortic occlusion stroke were admitted to our hospital from August 2014 to May 2018. They were divided into a bridging treatment group and a simple mechanical thrombectomy group according to different treatment methods. The two groups were compared before treatment and 24 hours after treatment. NIHSS score at 2 weeks after treatment, mRS score at 3 months after treatment, incidence of aICH, incidence of sICH, and mortality.
Results:
In the group, the NIHSS scores of the two groups were significantly improved before the treatment (P<0.01). There was no significant difference in the NIHSS score between the two groups after 24 hours of treatment and 2 weeks after treatment. >0.05); There was no significant difference in the incidence of aICH and sICH between the two groups (P>0.05); mortality after 3 months of treatment, survivor mRS score (0~1) and mRS score (0~2) There was no significant difference in the number of patients between the two groups (P>0.05). There was no significant difference between the bridging group and the simple group (P>0.05). The simple group was significantly different from the bridging group in terms of admission to treatment and onset to recanalization time. Conclusion:
In the treatment of acute large vessel occlusion stroke, bridging therapy and mechanical thrombectomy alone have better effect of revascularization. There was no significant difference in prognosis, mortality, incidence of complications and quality of life between bridging therapy and mechanical thrombectomy alone. Compared with mechanical thrombectomy alone, it has a shorter start-up time and can complete endovascular therapy in a shorter time window. However, further clinical trials are needed to compare the benefits and risks of both methods.

Analysis of Clinical Characteristics and Prognostic Factors in Patients with Bullous Pemphigoid

Objective:
To analyze the clinical features and prognostic factors of patients with bullous pemphigoid.
Methods:
The clinical data of 160 patients with bullous pemphigoid admitted to our hospital from February 2014 to January 2018 were collected and analyzed. The patients were followed up and they were divided into the good proup group and the poor prognosis group. Factors affecting the prognosis were explored.
Results:
Tension blisters were observed on normal skin or erythema of patients. Skine lesions were mainly located at trunk and limbs. 75 patients had consciously itching. The main type of skin lesions was simple skin lesions, and 10 patients had positive Nissl's sign. Histopathological examination showed subepidermal blister or fissure formation. Immunofluorescence showed that most patients had IgG+C3 deposition in the basement membrane, and some had IgM deposition. Of the 160 patients, 125 had good prognosis and 35 had poor prognosis. Clinical Observation on Treatment of Old Anterior Cruciate Ligament Fracture with Synovial Clearance and Retention of Anterior Ligament Reconstruction under Knee Arthroscopy

Objective:
To explore the clinical effect of arthroscopic synovial debridement combined with anterior cruciate ligament (ACL) reconstruction for old ACL rupture.
Methods:
The clinical data of 76 patients with old ACL were retrospectively analyzed. All patients were given knee arthroscopic synovectomy, and patients who were combined with ACL reconstruction with remnant preservation were included in observation group (n=42), and patients who were combined with ACL reconstruction with remnant removal were included in control group (n=34). The knee stability, knee function (Lysholm knee joint score) and life affected degree [2000 International Knee Documentation Committee (IKDC) Subjective score] were compared between the two groups before operation and at 3 months, 6 months and 12 months after operation. The treatment effects at 12 months after treatment and the complications within 12 months were analyzed in the two groups.
Results:
At 3 months, 6 months and 12 months after operation, the anterior displacement relaxation of knee joints in the two groups was significantly lower than that before operation, and the index in observation group was significantly lower than that in control group (all P<0.05). The Lysholm knee score and the IKDC subjective score were significantly improved compared with those before operation, and the scores in observation group were significantly higher than those in control group (all P<0.05). At 12 months after operation, the effective rate of treatment in observation group was significantly better than that in control group (P<0.05). There was no significant difference in the incidence rate of complications between the two groups within 12 months after operation (all P>0.05).
Conclusions:
Knee arthroscopic synovectom+ ACL reconstruction with remnant preservation has prominent treatment effects, and it does not increase the risk of complications compared with the method of remnant removal, and it is beneficial to the early development of mechanism of knee stability and function recovery.

From March 2017 to December 2018, 200 primiparas who were required to perform labor analgesia with nerve and muscle stimulator in our hospital were used as control group (group A) and 400 primiparae who were required to undergo combined spinal-epidural anesthesia for labor analgesia. According to the order of admission of puerpera, they were randomly divided into B1 group and B2 group, 200 patients in each group. The analgesic effect, labor duration, uterine contraction, postpartum hemorrhage, neonatal Apger score and the degree of motor block were observed and recorded.
Results:
There was no significant difference in age, height, weight, gestational age and operation time between the study group and the control group (P>0.05). The analgesic effect of group B1 and B 2 was significantly higher than that of group A (P<0. 05), but the analgesic effect of group 0 was significantly higher than that of group A and group B1 (, (P<0. 05). The time of active period and the second stage of labor in group B1 and B2 were significantly longer than those in group A (P<0.05) and the active period in group B1 was significantly shorter than that in group B2 (P<0.05), but there was no significant difference among the three groups (P> 0.0). 5) the amount of postpartum hemorrhage in group B 1 and group B2 was significantly higher than that in group A (P<0 05), and the amount of postpartum hemorrhage in group B1 was significantly lower than that in group B 2 (P<0 05), and there was no difference in Apger scores among the three groups (P> 0 05). The degree of motor block in group A was significantly higher than that in group B1 (P<0.05).
Conclusion:
The analgesic effect of injection of ropivacaine hydrochloride by injection of lumbar anesthesia is better than the analgesic effect of the guide instrument. In group B1, the regimen of lumbar anesthesia is shorter than that in the B2 group, and the effect on the uterine contraction is small, and the effect of labor analgesia is the best.

Comparative Analysis of Clinical Effects of External Fixator and DVR Anatomical Locking Plate in the treatment of Type C Distal Radius Fracture

Objective:
To explore the clinical effects of external fixation and DVR anatomical locking plate in the treatment of type C distal radius fractures.
Methods:
Eighty patients with type C distal radius fracture treated in our hospital from August 2015 to June 2017 were retrospectively analyzed. They were divided into group A (DVR anatomical locking plate) and group B (external fixator) according to different surgical methods. The operative effect and postoperative recovery of the two groups were analyzed. Evaluation analysis.
Results:
The ulnar and palmar angles of group A were significantly better than those of group B (P<0.05). At 3 months, 6 months and 12 months after operation, the Gartland_Werlev score of wrist joint in group A was significantly lower than that in group B (P<0.05).
Conclusion:
The use of DVR anatomical locking plate and external fixation for the treatment of type C distal radius fractures can effectively promote the recovery of postoperative wrist function, but compared with external fixation, DVR anatomical locking plate can be used to treat such patients. More effective fixation effect is conducive to the early development of functional exercise in patients, and has a very positive effect on promoting the recovery of wrist function. It is recommended to be applied in clinic.

Analysis on Interventional Effects of Preventive Interventions on Postoperative complications of Children with Hypospadias

Objective:
To investigate the effect of preventive interventions on postoperative complications of children with hypospadias.
Methods:
80 children undergoing hypospadias admitted to hospital from March 2017 to March 2018 were randomly divided into two groups. The traditional group of children received traditional nursing, and the preventive group of children received preventive intervention. Pain, sedation, child behavior, urinary tract related parameters, and complications were reported in both groups.
Results:
The scores of Wong-Baker and Ramsay scores in the prevention group were significantly lower than those in the traditional group; the depression, hyperactivity, and aggressiveness scores after the intervention were all significantly lower than the traditional group; The urethral pressure, maximum urethral closure pressure and VV levels were significantly higher than the traditional group; the overall complication rate was significantly lower than the traditional group. The difference was statistically significant (P<0.05).
Conclusion:
Preventive intervention has significant effect on the postoperative complications of hypospadias in children. It can be used for reference.

Effect of Liujunzi Decoction on Lung and Spleen Deficiency Syndrome in Stable Chronic Obstructive Pulmonary Disease and Its Influence on Lung Function and Blood Gas Analysis

Objective:
To investigate the effect of Liujunzi Decoction on lung and spleen deficiency syndrome in stable chronic obstructive pulmonary disease (COPD) and its influence on lung function and blood gas analysis.
Methods:
From January 2017 to January 2018, 112 patients with stable COPD with deficiency of both lung and spleen, who met the inclusion and exclusion criteria of this study, were selected. They were divided into control group (simple western medicine) and observation group (western medicine combined with Liujunzi decoction). After 3 months of treatment, pulmonary function, arterial blood gas analysis, TCM syndrome score, clinical treatment effect and the number of acute attacks within six months were compared between the two groups.
Results:
There was no significant difference in arterial blood gas index, pulmonary function index and TCM syndrome score before treatment between the two groups (P>0.05). After treatment, the scores of pH value, PaCO
2
, cough, expectoration, shortness of breath and chest tightness in the observation group were lower than those in the control group, and the scores of SaO
2
, PaO
2
, PEF, FEV1%, FEV1 / FVC in the observation group were higher than those in the control group (P<0.05). The total effective rate was 89.29% in the observation group and 73.21% in the control group. There was significant difference between the two groups (P<0.05). No serious adverse reactions occurred in the two groups during treatment.
Conclusion:
Liujunzi Decoction has a significant effect on the treatment of stable chronic obstructive pulmonary disease with deficiency of both lung and spleen. It can effectively improve clinical symptoms and blood gas analysis index, improve lung function and control the development of the disease.

Application of Standardized Operation in Quality Control of Chinese Herbal Medicine Decocting

Objective:
To provide reference for the realization of normalization and standardization of quality control and management of traditional Chinese medicine(TCM) decoction service.
Methods:
Through establishing quality control organization ,analyzing each link of quality control and developing quality standard rules,quality control system was constructed and total quality management was conducted.
Results:
According to checking 41quality items from 12 link of quality control of examing once a quarter, on receiving and storing of decoction pieces, prescription checking and preparation, decoction, distribution, transportation, environment and disinfect etc, holding 1 quarter quality control and safety management meeting and commenting on quality control work. The qualified percentage of quality control index was increased from 76%(Sep.2017) to (Dec.2017).
Conclusion:
The construction of standardization of quality control can effectively regulate each link of management of TCM decoction service and ensure the quality of TCM decoction.

Tiaoshentongbi Decoction Senile Combined with Herb Residue Ironing Treatment of Primary Osteoporosis

Objective:
To observe the clinical effects of tiaotongbi decoction combined with herb residue ironing for treating primary osteoporosis.
Methods:
80 patients diagnosed with primary osteoporosis were randomly divided into observation group and control group, 40 cases in the control group by using calcitonin and vitamineD3 treatment, the observation group by using calcitonin and vitamineD3 andtiaoshentongbi decoction combined herb residue ironing treatment, compared two groups of clinical effects, VAS pain score and to improve the situation.
Results:
After 4 weeks of intervention, VAS pain score was significantly reduced in both groups (P<0.05), and the improvement degree of VAS pain in the observation group was significantly better than that in the control group (P<0.05). The total effective rate of the observation group was 87.5%, which was significantly higher than 57.5% of the control group (P<0.05).
Conclusion:
The combined treatment of calcitonin and vitamineD3 and tiaoshentongbi decoction combined herb residue ironing can effectively improve the clinical effect of osteoporosis pain and alleviate pain, which is worthy of clinical promotion.
