529 Expression and the Correlation with Prognosis of Levels of Serum CYFRA21-1 TK1 and CA15-3 in Patients with Breast Cancer

Objective: To explore the expression levels of serum C cytokeratin 19 fragment antigen 21-1 (CYFRA21-1), serum thymidine kinase 1 (TK1) and carbohydrate antigen 15-3 (CA15-3) in patients with breast cancer and their correlation analysis with prognosis. Methods:The clinical data of 90 patients with breast cancer (breast cancer group) admitted from January 2013 to January 2015 were retrospectively analyzed. Another 50 patients with benign breast tumors at the same time period were selected as control group. The levels and positive rates of serum CYFRA21-1, TK1 and CA15-3 were compared between the two groups. The recurrence and metastasis at 3 years were recorded in breast cancer group. The correlation between levels of CYFRA21-1, TK1 and CA15-3 and prognosis of breast cancer was analyzed by logistic regression analysis. Results:The levels and positive rates of serum CYFRA21-1, TK1 and CA15-3 in breast cancer group were higher than those in control group (P<0.05). The levels of CYFRA21-1, TK1 and CA15-3 in recurrence group were higher than those in non-recurrence group (P<0.05), and the levels of CYFRA21-1, TK1 and CA15-3 in metastasis group were higher than those in non-metastasis group (P<0.05). Spearman analysis showed that the levels of CYFRA21-1, TK1 and CA15-3 were positively correlated with recurrence and metastasis of breast cancer (P<0.05). Conclusions:The high expression levels of serum CYFRA21-1, TK1 and CA15-3 are associated with occurrence, postoperative recurrence and metastasis of breast cancer, and can effectively predict the postoperative recurrence and metastasis

532 Inhibitory Effect and Mechanism of siRNA Targeting Nox2 on Adriamycin-induced Injury of Human Skin Fibroblasts

Objective: To explore the effect of Nox2 in human dermal fibroblasts damage induced by adriamycin. Methods: A human dermal fibroblasts injury model was established by exposure to adriamycin. Western blotting was used to detect Nox2 protein expression in the injury of human dermal fibroblasts induced by adriamycin. The cell survival rate was determined by MTT method. Superoxide dismutase (SOD) activity, malondialdehyde (MDA) and reactive oxygen species (ROS) levels were detected by biochemical kit. Moreover, the level of apoptosis was detected by Western blotting.Results: The relative expression of Nox2 in adriamycin group was higher than that in the control group, and the difference was statistically significant (P<0.05). Compared with adriamycin group, inhibition of Nox2 expression by siRNA decreased the level of MDA and ROS, as well as enhanced the activity of SOD (P<0.05). In addition, inhibition of Nox2 expression by siRNA decreased the apoptosis of human dermal fibroblasts induced by adriamycin (P<0.05). Conclusion: Targeting inhibition of Nox2 expression by siRNA can protect against the injury of human dermal fibroblasts induced by adriamycin, the mechanism is the reducing of oxidative stress and ROS level.

536 Expression and Clinical Significance of Mismatch Repair Proteins MLH1 MSH2 MSH6 and PMS2 in Colorectal Cancer

Objective: To detect the expression of mismatch repair proteins(MLH1, MSH2, MSH6 and PMS2)in the colorectal cancer and disscus the correlation between the clinicopathological characteristics and MMR protein. Methods: EnVision immunohistochemical method was used to detect the expression of MMR protein, and then analyze the relationship between the expression of mismatch repair protein of CRC patients and pathology features from 242 cases of colorectal cancer. Results: ①In 242 cases of colorectal cancer patients, total expression deletion rate was 14.04%, the expression deletion rate was of MLH1, hMLH2, hMSH6 and hPMS2 protein was respectively 10.03% (25/242), 1.2% (3/242), 1.2% (3/242) and 11.16% (27/242). hMLH1 and hPMS2 co-expression deletion rate was 9.50%(23/242), hMSH2 and hMSH6 co-expression deletion rate was 0.83% (2/242). ②All of MMR protein expression deletion was uncorrelated with sexuality, age, lymphatic metastasis in CRC (p>0.05), the expression of hMLH1 protein was related to the differentiation degree of CRC (p<0.05), the expression of hPMS2 protein was related to the location, size and differentiation degree of tumor (p<0.05). Conclusion: The location, size and differentiation of tumors affect the expression of mismatch repair proteins hMLH1 and hPMS2 in CCR. The co-expression of hMLH1 and hPMS2, hMSH2 and hMSH6 is more common, and the deletion of hMLH1 and hPMS2 is more common.

539 Comparative Analysis of SPECT-CT and MRI in Osteoporotic Vertebral Compression Fractures

Objective: To analyze the diagnostic value of single photon emission computed tomography-computed tomography (SPECT-CT) and magnetic resonance imaging (MRI) for osteoporotic vertebral compression fractures (OVCF). Methods: The clinical data of 82 patients with OVCF in our hospital from March 2016 to April 2018 were retrospectively analyzed. Among them, 67 cases were acute-phase fractures (disease course≤ 3 w) and 15 cases were subacute-phase fractures (disease course 4~12 w). The detection of responsible vertebral bodies by SPECT-CT and MRI was recorded in patients with acute-phase fractures and subacute-phase fractures, and 82 patients were given percutaneous kyphoplasty (PKP). The pain degree [visual analogue score (VAS)] in 82 patients were compared before operation and at 3d after operation. Results: A total of 83 vertebral bodies in 67 patients with acute OVCF were given PKP. Among them, MRI and SPECT-CT confirmed 82 responsible vertebral bodies, and Kappa value was 0.845 (P<0.001), and the consistency was extremely strong. 1 case (1.20%) was missed diagnosis and 1 case (14.29%) was misdiagnosed by SPECT-CT. 34 vertebral bodies in 15 patients with subacute OVCF were given PKP. Among them, MRI and SPECT-CT confirmed 29 responsible vertebral bodies, and Kappa value was 0.633 (P<0.001), and the consistency was high. 1 case (3.33%) was missed diagnosis and 1 case (33.33%) was misdiagnosed by SPECT-CT. At 3 d after operation, the VAS scores in 82 patients with OVCF were decreased compared with those before operation (P<0.05). Conclusions: SPECT-CT and MRI can confirm the responsible vertebral bodies of OVCF. When patients have MRI examination contraindications, SPECT-CT can be selected as the examination method for evaluating responsible vertebral bodies of OVCF.

543 Relationship between HPV16/18 Infection and Expression of PDGFR- and PDGFR- beta Protein in Cancer Tissues of Elderly Patients with Cervical Squamous Cell Carcinoma

Objective: The relationship between the expression of HPV16/18 infection and the expression of platelet derived growth factor receptor alpha (platelet-derived growth factor receptor-alpha, PDGFR- a) and platelet derived growth factor receptor beta (platelet-derived growth factor receptor-alpha, PDGFR- beta) in the elderly patients with squamous cell carcinoma of the cervix and the expression of platelet derived growth factor receptor beta (platelet-derived growth factor receptor-alpha, PDGFR- beta) and the relationship between the three The relationship between each other.Methods:75 cases of senile cervical squamous cell carcinoma and 32 of normal cervical tissue were selected. In situ hybridization was used to detect HPV16 /18 infection, and PDGFR- A and PDGFR- beta protein were detected by immunohistochemistry. Results:The infection rates of HPV16 /18 in cervical squamous cell carcinoma group and control group were 69.33% (52 /75) and 18.75% (6 /32) respectively.The infection rate of HPV16 /18 in elderly patients with cervical squamous cell carcinoma was significantly higher than that in the control group (P <0. 05). One case of PDGFR-α protein expression was detected in the control group, Positive expression rate 3.13%. In the elderly patients with cervical squamous cell carcinoma, 59 cases of PDGFR-α protein were detected positively, Positive expression rat 78.67%, and the difference was statistically significant. (P<0.05); The positive expression rate of PDGFR-α protein was related to the clinical stage of cervical squamous cell carcinoma patients (P<0.05); The positive expression rate of PDGFR-β protein was lower in the control group was 6.25%, and the cervical squamous cell carcinoma patients The expression rate of PDGFR-β protein in tissues was 62.67%, and the difference was statistically significant (P<0.05). The expression of PDGFR-β protein was correlated with the clinical stage of cervical scales (P<0.05); Among 72 cases of cervical squamous cell carcinoma, 47 cases (Positive expression rat 62.67%) were HPV16/18 positive, 45 cases (Positive expression rat 95.74%) were PDGFR-α protein positive, 40 cases (Positive expression rat 85.11%) were PDGFR-β protein positive.The number of HPV16/18 positive cervical squamous cell carcinoma patients was significantly higher than that of non-HPV16/18 positive cervical squamous cell carcinoma patients (P < 0.05).Spearman correlation analysis showed that the expression of PDGFR-α and PDGFR-β protein was positively correlated with HPV16/18 infection in patients with cervical cancer (rs=0.428,P<0.05; rs=0.562,P<0.05).PDGFR-α was positively correlated with the expression of PDGFR-β protein in cervical cancer patients (rs=0.639, P<0.05).Conclusions:Senile cervical squamous cell carcinoma is closely related to HPV16/18 infection. The expression of PDGFR-α and PDGFR-β protein in HPV16/18 infected cervical squamous cell carcinoma is significantly increased.It plays a synergistic role in the occurrence and development of cervical squamous cell carcinoma

548 Clinical Value of Modified Early Warning Score combined with Electrocardiogram and SaO2 in Evaluating the Severity of Acute Chest Pain

Objective: To explore the clinical value of modified early warning score (MEWS) combined with electrocardiogram (ECG) and blood oxygen saturation (SaO2) in evaluating the severity of acute chest pain. Methods:A total of 1080 patients with acute chest pain received in the emergency department from September 2017 to September 2018 were studied. MEWS score and MEWS combined with electrocardiogram and SaO2 score were performed. A special person was assigned to follow up the prognosis of patients. Cox regression model was used for survival analysis. Results:The worse the prognosis, the higher the MEWS score and the combined score, the difference was statistically significant (P<0.05). The combined scores of ICU and emergency death patients were significantly higher than those of MEWS (t = 3.241, 4.006, P<0.05). Cox regression survival analysis showed that MEWS score (P=0.038), electrocardiogram score (P=0.022), SaO2 score (P=0.015) and combined score (P=0.014) were all independent risk factors for death of patients with acute chest pain, and the combined score (HR=3.720) had higher predictive value for prognosis death of patients. Conclusion: MEWS score combined with electrocardiogram and SaO2 monitoring can help to timely and accurately evaluate the severity of acute chest pain and predict potential risks.

551 Effects of Tissue CD3+ CD68+ MMP-2 and MMP-9 on the Development of Abdominal Aortic Aneurysm in Patients

Objective: To explore the influence on development of tissue T lymphocytes (CD3+), macrophages (CD68+), matrix metalloproteinase-2 (MMP-2) and matrix metalloproteinase-9 (MMP-9) in patients with abdominal aortic aneurysm (AAA). Methods: The abdominal aortic aneurysm tissue obtained from 101 patients with AAA admitted to our hospital from May 2017 to June 2018 was selected as the observation group, at the same time, 20 normal abdominal aorta tissues were selected as the control group.The expression of CD3+, CD68+, MMP-2 and MMP-9 in the above tissues was detected by immunohistochemistry, and quantitative analysis was performed by image analysis technique. Results: The expressions of CD3+ (100.43±105.81) and CD68+ (233.01±195.31) in the AAA tissues of the observation group were significantly higher than those in the control group, and the difference was statistically significant (P<0.05);the expression of MMP-2 (0.098±0.047) and MMP-9 (0.025±0.015) in the AAA tissues of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Conclusion: The large expression of CD3+, CD68+, MMP-2 and MMP-9 in AAA tissue can further promote the formation and expansion of AAA

554 Diagnostic Value of Lysophosphatidylcholines in Patients with Acute Myocardial Infarction

Objective: To investigate the diagnostic value of lysophosphatidylcholine (LPCs) in acute myocardial infarction (AMI). Methods: From December 2015 to June 2016, a total of 44 AMI patients in Tianjin Third Central Hospital were recruited as AMI group. And 25 healthy individuals were selected as control group. According to the results of coronary angiography, the patients in AMI group were divided into anterior myocardial infarction AAMI patients (n=21) and inferior myocardial infarction AIMI patients (n=23). The biochemical parameters such as hs-CRP in the serum of each group were measured. The contents of five LPCs in LPC 14:0-LPC 18:0 were determined by UPLC-MS metabolomics platform. Principal Component Analysis (PCA) Model and Orthogonal Partial Least Squares Discriminant (OPLS-DA) Model were used to analyze serum metabolic profiles, and ROC Curves were used to analyze the potential diagnostic significance of five LPCs. Results: Fasting blood glucose (GLU) and triglyceride (TG) levels were significantly lower in AMI patients than in healthy subjects in Control group. Serum brain natriuretic peptide (BNP), high-sensitivity C-reactive protein (hs-CRP), creatine kinase (CK), and creatine kinase isoenzyme (CK-MB) levels were significantly higher (P<0.05). The levels of five LPCs in serum LPC 14:0-LPC 18:0 in AMI patients were also lower than those in healthy subjects (P<0.05). The metabolic profile can better distinguish between healthy subjects and AMI patients. All five LPCs have high diagnostic values for the diagnosis of AMI patients, especially LPC 15:0-LPC 18:0. LPC 14:0, LPC 15:0, LPC 17:0 and LPC 18:0 have high diagnostic value for distinguishing between AAMI and AIMI. The combination of LPC 14:0 and LPC 18:0 enables more efficient diagnosis of AAMI and AIMI. Conclusion: LPCs have high diagnostic values for AMI. Combination of different LPCs can more effectively diagnose AMI patients with different sites.

558 The Influence of Different Surgical Approaches and Operation Timing of Microsurgery on the Curative Effects of Posterior Circulation Aneurysms

Objective: To investigate the influence of surgical approach and operation timing of microsurgery on the curative effects of posterior circulation aneurysms. Methods: Clinical data of 40 cases of patients with posterior circulation aneurysms admitted into our hospital from May 2011 to November 2017 who received microsurgery were retrospectively analyzed. According to the duration of onset to operation, they were divided into three groups, that is, early stage group (within 3d), middle stage group (4 to 14d), and delayed group (over 14d), 15 cases, 13 cases and 12 cases respectively.In addition, 40 cases of patients with posterior circulation aneurysms treated by microsurgical treatment in our hospital were selected according to surgical approaches. All the patients were divided into extensive pterional approach (11 cases), pterional approach (9 cases), subtemporal approach (13 cases) and retrosigmoid approach (7 cases). And all patients were arranged with 6 months of follow-up. The clinical efficacy and prognosis of different surgical approaches and timing were compared. Results: The clinical efficacy of early stage group was significantly better than middle stage group and delayed group (P<0.05); There was no statistical difference in the rate of good prognosis compared among the three groups (P>0.05); The total effective rate and the rate of good prognosis of pterional approach surgery were significantly lower than extensive pterional approach, subtemporal approach and retrosigmoid approach (P<0.05); There was no statistical difference in the clinical efficacy and prognosis among extensive pterional approach, subtemporal approach and retrosigmoid approach (P>0.05). Conclusion: Different microsurgical approaches and timing will have a certain impact on the curative effect of posterior circulation aneurysms. Choosing appropriate surgical timing and approaches can significantly improve the curative effect, good prognosis and improve the quality of life of patients.

563 Analysis on the Effect of Methylpyridine combined with Mifepristone on Serum Progesterone and β-HCG Levels in Patients with Ectopic Pregnancy

Objective: To study the effect of meteropteridine combined with mifepristone on serum progesterone and β-HCG levels in patients with ectopic pregnancy. Methods: 86 cases of ectopic pregnancy treated in our hospital from January 2016 to February 2017 were randomly divided into two groups: control group (methotrexate treatment) and observation group (methotrexate mifepristone treatment). Adverse reactions, mass, abdominal pain disappearance time, serum β-HCG negative time, progesterone recovery time and menstrual resuscitation time were analyzed in both groups. Results: The success rate in the observation group (95.35%) was significantly higher than that in the control group (74.42%) (P<0.05). In the observation group, the mass, the time of disappearance of abdominal pain, the negative time of serum β -HCG, the time of progesterone returning to normal and the time of menorrhagia were shorter than those of the control group (P<0.05). After treatment, the levels of serum progesterone and β-HCG in the observation group were lower than those in the control group (P<0. 05).But there was no significant difference in the rate of intrauterine pregnancy and the rate of ectopic pregnancy between the two groups within one year (P>0.05). Conclusion: Methamotrexil combined with mifepristone for ectopic pregnancy has a high success rate, rapid disappearance of clinical symptoms, little influence on fertility outcome, and high safety.

567 Influences of Bone Cement Combined with IlizarovTechnology on DiseasedLimb Function and Bacterial Content of Wound in Patients with Tibia Chronic Osteomyelitis

Objective: To investigate the influences of bone cement combined with Ilizarovtechnology on the diseasedlimb function and bacterial content of wound in patients with chronic osteomyelitis. Methods: 62 cases of patients with chronic osteomyelitis of tibia from January 2015 to February 2018 were randomly divided into two groups, 31 patients in the control group were treated with Ilizarov bone graft technology, and 31 patients in the observation group were treated with antibiotic bone cement chain bead implantation on the basis of the control group.The times of dressing change, the time of using antibiotics, the time of wound healing and the length of stay, the diseasedlimb functional recovery, the recovery of joint function, the detection rate of bacteria and the quality of life were compared. Results: The times of dressing change, the time of using antibiotics, the time of wound healing and the length of stay in the observation group were less than those in the control group (P<0.05); the excellent and good rate of diseased limbfunctional recovery in the observation group was 93.55%, which was higher than 74.19%in the control group (P<0.05); after treatment, the two groups of knee joint score of New York Special Surgery Hospital (HSS) and ankle joint function score (Baird-Jackson) were higher than those before treatment (P<0.05), and the observation group was higher than that of the control group (P<0.05); the detection rate of bacteria in the observation group was 6.45%, which was lower than 32.26% in the control group (P<0.05); after the treatment of the two groups, the score of the quality of life of WHO (WHOQOL-BREF) was higher than that before the treatment (P<0.05), and the observation group was higher than that of the control group (P<0.05). Conclusion: Bone cement combined with Ilizarov technology in treating chronic osteomyelitis can not only promote the recovery of diseased limb function, reduce pathogenic bacteria, but also effectively improve the quality of life of patients.

572 Changes of CD3+, CD3+CD4+, CD3+CD8+lymphocyte Percentage and hs-CRP Level in Children with Hand-foot-mouth Disease

Objective: To investigate the changes of CD3 + CD4+ and CD3+CD8+ lymphocyte percentage in children with hand, foot and mouth disease (HFMD). Methods: 90 children with severe hand, foot and mouth disease admitted to our hospital from June 2016 to June 2018 were selected as the patient group, and 90 healthy children who came to our hospital for physical examination during the same period were selected as the control group. The levels of peripheral blood lymphocyte subsets, zinc in erythrocyte and high-sensitivity C-reactive protein were measured in two groups. The changes of CD3+, CD3+CD4+, CD3+CD8+lymphocyte percentage and hs-CRP level were analyzed. Results: The percentages of CD3+CD8+, CD3+CD4+, CD3+CD3+in the patients group were significantly lower than those in the control group (t=5.349, t=4.384, t=5.304, all P<0.05). The percentages of CD3+CD8+, CD3+CD4+, CD3+CD3+in the patients group were significantly lower than those in the control group (t=5.349, t=4.384, t=5.304, all P<0.05). The level of high-sensitivity C-reactive protein in the patients group was (5.54 +1.98) mg/L, which was significantly higher than that in the control group (1.67 +1.21) mg/L (t=4.958, P<0.05). The zinc level of erythrocyte in the children of the same age group was significantly lower than that in control group (all P<0.05). Conclusion: The humoral and cellular immune function of children with severe hand, foot and mouth disease is in disorder to a certain extent. The body lacks zinc of red blood cells, and can not form effective resistance to virus infection, and can further form secondary bacterial infection.

575 Effect of Chlorhexidine Gargle combined with Orthodontic treatment on Anterior Teeth Displacement caused by Periodontal Disease and its Influence on Serum Levels of IL-2 IL-6 and TNF-alpha

Objective: To investigate the effect of chlorhexidine gargle combined with orthodontic treatment on anterior teeth displacement caused by periodontal disease and the effects of chlorhexidine gargle on serum levels of interleukin-2, interleukin-6 and tumor necrosis factor-alpha. Methods: 108 patients with anterior teeth displacement caused by periodontal disease were selected from January 2015 to January 2018 in our hospital. They were divided into two groups by simple random number table. The control group (54 cases) was treated with orthodontic treatment, while the observation group (54 cases) was treated with chlorhexidine mouthwash. The clinical efficacy, periodontal indexes, serum levels of IL-2, IL-6 and TNF-alpha, and adverse reactions were compared between the two groups before and after treatment. Results: After receiving treatment for 12 months, overall effective rate of observation group was 94.4% which was significant higher than control group's 81.5% (P<0.05). After receiving treatment for 12 months, Two groups' periodontal index including probing bleeding rate, anterior teeth covering, periodontal pocket depth values and serum IL-2, IL-6 and TNF-α decreased significantly if comparing with those before treatment (P<0.01), and observation group's above index decreased more significantly (P<0.01). Serum IL-2 of two groups' increased significantly after receiving treatment for 12 months if comparing with that before treatment (P<0.01). Observation group's serum IL-2 was significant higher after treatment if comparing with control group at same time (P<0.01). There was no statistical significance if comparing control group adverse reaction rate (13.0%) with observation group's (9.3%) (P>0.05). Conclusion: The treatment of anterior teeth displacement caused by periodontal disease with chlorhexidine mouthwash combined with oral malformation can effectively regulate the level of inflammatory cytokines in patients, improve periodontal status and have significant clinical effects.

Objective: To assess the effects and ADR of SCAVB6 on the treatment of breast cancer in the real world. Methods:The breast cancer cases from February 2016 to February 2017 are the subjects of research, from which patients who have completed periodic chemotherapy are screened, based on whether or not to use the SCAVB6 to be treated at the same time as the treatment group (38 cases) and the control group (22 cases), the of short-team effects and the adverse reactions was be recorded. Results:The treatment group and the control group had a recently shown efficiency of 71.05%, 68.18%; Leukocyte decrease, thrombocytopenia, intestine and stomach reaction, and alopecia were higher than those in that treatment group (P < 0.05). Conclusions:SCAVB6 combined with chemotherapy has no significant effect on increasing the treatment effect of breast cancer, but it can alleviate that occurrence of adverse reaction and ascension the quality of life of patient.

582 Effect of Modified Elbow High Arteriovenous Fistula on Success Rate and Complications in Patients with Cardiac Insufficiency

Objective: To study and discuss the success rate and complications of modified elbow high arteriovenous fistula in patients with cardiac insufficiency uremia, and to provide reference for clinical study. Methods: 40 uremic patients with cardiac insufficiency treated from January 2014 to December 2016 were selected for the study. They were divided into observation group and control group by simple random method, 20 cases in each group. The control group was treated with traditional high arteriovenous fistula; the observation group was treated with modified high elbow arteriovenous fistula. The effect of the two groups after treatment was compared. Results: The success rate of the observation group was 100.0%, obviously higher than that of the control group (85.0%), and the difference was significant (p<0.05). The postoperative bleeding volume was obviously better than that of the control group (p<0.05); the operation time of the control group was significantly higher than that of the observation group, and the difference was statistically significant (p<0.05). The incidence of heart failure in the control group was 20.0%, 25.0% and 15.0%, respectively, and the incidence of complications in the observation group was significantly lower than that of the control group (p<0.05). The rate of postoperative bowel movement in the observation group was superior (P>0.05). After 24 months follow-up, the long-term defecation rate of the two groups was significantly higher than that of the control group (P<0.05). Discussion: The application of modified high elbow arteriovenous fistula in patients with new dysfunction uremia has many advantages, such as high success rate, long-term use patency rate, fewer complications, no impact on cardiac function and so on. It is an ideal autologous internal fistula operation scheme, which is worthy of clinical application.

586 Therapeutic Effect of Methimazole combined with Bisoprolol on Hyperthyroidism and its Influence on Serum OCN and beta-CTx

Objective: To analyze the therapeutic effect of methylthimidazole combined with bisoprolol on hyperthyroidism and its effect on serum osteocalcin (OCN) and specific sequence of collagen type I carboxyterminal peptide beta (beta-CTx). Methods: 85 patients with hyperthyroidism admitted to our hospital from February 2015 to February 2017 were selected as the subjects of this study. They were divided into observation group (n=50) and control group (n=50) by simple random number table method. The control group was treated with methimazole combined with propranolol, while the observation group was treated with methimazole combined with bisoprolol for 14 consecutive days. The clinical efficacy, thyroid hormone function, serum OCN, beta-CTx changes and adverse reactions were compared between the two groups. Results: After treatment, the total effective rate of the observation group was significantly higher than that of the control group (P<0.05); After treatment, the thyroid stimulating hormone (TSH) of two groups were significantly higher than before treatment. TSH in the observation group was significantly higher than that in the control group, FT4 and FT3 were significantly lower than those in the control group (P<0.05). After treatment, the levels of serum OCN and beta-CTx in the two groups were significantly lower than those before treatment (P<0.05). The levels of serum OCN and beta-CTx in the observation group were significantly lower than those in the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion: After treatment, the levels of serum OCN and beta-CTx in the two groups were significantly lower than those before treatment (P<0.05). The levels of serum OCN and beta-CTx in the observation group were significantly lower than those in the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).

590 Efficacy of Baitangping combined with Liraglutide in the treatment of Type 2 Diabetes Mellitus and its Effect on Serum Hcy and APN

Objective: To investigate the efficacy of Baitangping combined with Liraglutide in the treatment of type 2 diabetes mellitus and its effect on serum homocysteine (Hcy) and adiponectin (APN). Methods: 95 patients with type 2 diabetes admitted to our hospital from March 2016 to March 2017 were selected, 50 cases were randomly divided into observation group and 45 cases in control group, according to the simple random grouping method. The observation group was treated with baitangping and lilalutin. The control group was treated with metformin. Compare the clinical efficacy of two groups of patients, fasting blood sugar, 2 h postprandial blood glucose, and glycosylated hemoglobin (HbAlc), serum creatinine, serum creatinine, Scr) and glomerular filtration rate (glo - merular filtration rate, GFR), Hcy, APN levels; Body mass index and adverse reactions were recorded before and after treatment. Results: After treatment, the effective rate of the observation group was 94.00%, significantly higher than that of the control group (P<0.05). Before treatment, there was no significant difference in the level of each indicator between the two groups. After treatment, the level of each indicator decreased in both groups, compared with that before treatment (P<0.05). Fasting blood glucose, 2h postprandial blood glucose and HbAlc in the observation group were all lower than those in the control group (P<0.05). There was no significant difference in Scr, GFR level and body mass index between the two groups before treatment. After treatment, the Scr level and body mass index of the observation group were significantly lower than that of the control group, and the GFR level was higher than that of the control group (P<0.05). There was no significant difference in serum Hcy and APN levels between the two groups before treatment. After treatment, the serum Hcy level of the two groups significantly decreased and the APN level significantly increased, and the serum Hcy level of the observation group was significantly lower than that of the control group, and the APN level was significantly higher than that of the control group (P<0.05). The incidence of hypoglycemia in the observation group was significantly lower than that in the control group (P<0.05). There was no statistically significant difference in the incidence of nausea, vomiting, headache and diarrhea between the two groups (P>0.05). Conclusion: Baitangping combined with liraglutide is effective in the treatment of type 2 diabetes mellitus. It can significantly improve the level of serum Hcy and APN without increasing the occurrence of adverse reactions. The patients can basically tolerate it. It is worth popularizing and applying.

Objective: To investigate the clinical efficacy observation of preoperative debridement and detoxification combined with local microwave therapy before cervical disease. Methods:100 patients with cervical intraepithelial neoplasia admitted to our hospital from January 2015 to March 2018 were treated with preoperative detoxification and detoxification combined with local microwave therapy as the treatment group. The hospital was hospitalized without heat-clearing and detoxification. One hundred patients with cervical intraepithelial neoplasia were used as the control group, and the clinical effects of the two groups were compared. Results:The intraoperative and postoperative bleeding in the treatment group was significantly lower than that in the control group. The average wound healing time in the treatment group was significantly lower than that in the control group. The complication rate in the treatment group was 2%,which was significantly lower than that in the control group,9%. <0.05 difference was statistically significant. 598 Clinical value of 24h dynamic electrocardiogram combined with NLR value in predicting malignant arrhythmia after acute myocardial infarction

Objective: To investigate the clinical value of 24h dynamic electrocardiogram [heart rate variability (HRV), QT interval variability (QTV)] combined with the neutrophil lymphocyte ratio (NLR) in predicting malignant arrhythmia after acute myocardial infarction (AMI). Methods:96 patients with AMI (AMI group) who were admitted to cardiology department in the hospital from August 2014 to August 2018 and 60 normal volunteers (control group) who underwent physical examination in the hospital during the same period were included in the study. Patients in AMI group were divided into the malignant group (38 cases) and the non-malignant group (58 cases) according to the occurrence of malignant arrhythmia. All subjects were given 24h dynamic electrocardiogram examination. Changes of HRV , QTV and NLR value were monitored and compared. The value of HRV, QTV indicators and NLR values in predicting the occurrence of malignant arrhythmia was analyzed by receiver operating characteristic (ROC) curve. Results:The values of SDNN, SDNN-index, SDANN-index, rMSSD, 24h-QTV, Day-QTV, Night-QTV and NLR in AMI group were significantly lower than those in the control group (P<0.05). Except rMSSD, the values of SDNN, SDNN-index, SDANN-index, 24h-QTV, Day-QTV, Night-QTV and NLR in the malignant group were significantly lower than those in the non-malignant group (P<0.05). ROC curve analysis indicated that the value of HRV and QTV indicators combined with NLR values in predicting the occurrence of malignant arrhythmia after AMI was greater, and the area under ROC curve was the largest (0.892). Conclusion: HRV and QTV indicators combined with NLR values can predict the occurrence of malignant arrhythmia after AMI.

602 Effects of Dexamethasone Combined with Methylprednisolone Sodium Succinate on Neurological Function NSE NO and NE after Craniotomy for Traumatic Brain Edema

Objective: To study the effects of dexamethasone combined with methylprednisolone sodium succinate on neurological function, NSE, NO and NE after craniotomy for traumatic brain edema. Methods: Fifty patients with traumatic brain edema admitted to our hospital from August 2016 to July 2017 were randomly divided into observation group and control group with 25 cases in each group. The control group was treated with dexamethasone after craniotomy, while the observation group was treated with methylprednisolone sodium succinate. The clinical efficacy, brain edema volume and neurological function, levels of NSE, NO and NE were compared between the two groups. Results: After treatment, the total clinical effective rate of the observation group was significantly higher than that of the control group (P<0.05). Before treatment, there was no significant difference in the volume of brain edema and NIHSS score between the two groups (P>0.05). The volume of brain edema and NIHSS score of the two groups showed a downward trend on the 3rd, 7th and 14th day after treatment. The volume and NIHSS score of the observation group were significantly lower than those of the control group on the 3rd, 7th and 14th day after treatment (P<0.05). The two groups were compared in the aspects of group, time point and time point. There were significant differences (P<0.05). Before treatment, there was no significant difference in NSE, NO and ET levels between the two groups (P>0.05). After treatment, the NSE levels of the two groups showed an upward trend on the 3rd and 7th day. After treatment, the NSE levels of the two groups showed a downward trend on the 14th day. After treatment, the levels of NO and ET of the two groups showed a downward trend. The observation group was significantly lower than the control group on the 3rd, 7th and 14th day (P<0.05). There were significant differences in different time points, between groups and at different time points (P<0.05). Conclusion: Traumatic brain edema patients treated with dexamethasone combined with methylprednisolone sodium succinate after craniotomy can effectively improve brain edema and neurological function, effectively reduce the levels of NSE, NO and NE, and have good clinical efficacy.

606 Effect of Perioperative Infusion of FFP on Coagulation in Patients Undergoing Scoliosis Orthopedic Surgery

Objective:To investigate the effects of perioperative infusion of fresh frozen plasma (FFP) on coagulation function in patients undergoing scoliosis surgery. Methods:104 patients with scoliosis were selected from January 2016 to October 2018. The proportions of autologous blood transfusion, allogeneic suspended red blood cell transfusion and FFP transfusion were compared between the two groups, and the status of PT, TT, APTT, FIB, D-D, ACT, CR and PF at 3 days before operation and 3 days after operation were compared. The hospitalization time and wound healing of the two groups were counted. Results:The proportions of autologous blood transfusion, allogeneic red blood cell infusion and FFP transfusion in FFP group were significantly higher than those in non-FFP group (P<0.05). There was no significant difference in PT, TT, APTT, FIB, D-D, ACT, CR and PF between the two groups before and 3 days after operation (P>0.05). There was no significant difference in the length of hospital stay and Grade I wound healing between the two groups (P>0.05). Conclusion: Perioperative infusion of FFP in patients undergoing posterior scoliosis surgery does not improve their coagulation function, nor does it have a significant impact on the prognosis of patients.

610 Effects of Ropivacaine combined with Different Doses of Sufentanil on Hemodynamics and Anesthetic Effect of Cesarean Section Women

Objective: To investigate the effect of ropivacaine combined with different doses of epidural sufentanil on hemodynamics and anesthetic effects in women undergoing cesarean section. Methods: 80 women undergoing cesarean section in the hospital during the period from May 2015 to October 2017 were selected as the subjects. They were randomly divided into the control group (given ropivacaine anesthesia), study group 1 (given ropivacaine combined with 2.5 μg of sufentanil anesthesia), study group 2 (given ropivacaine combined with 2.5 μg of sufentanil anesthesia) and study group 3 (given ropivacaine combined with 7.5 μg of sufentanil anesthesia), 20 cases in each group. Changes of hemodynamic parameters [systolic blood pressure (SBP), diastolic blood pressure (DBP)], anesthetic effects (duration of analgesia, time to the highest block level), maternal Bromage motor score, neonatal Apgar score and drug safety in all groups were observed. Results: There was no significant difference in SBP and DBP between groups before anesthesia (P>0.05). After anesthesia, the SBP and DBP of control group, study group 1 and study group 3 were increased significantly (P<0.05), but there was no obvious changes in SBP and DBP of study group 2 (P>0.05). The analgesic maintenance time of the control group, study group 1, study group 2 and study group 3 were significantly prolonged in turn, while the time to reach the highest block plane was shortened in turn (P<0.05). There was no significant difference in the Bromage exercise scores among the 4 groups (P>0.05). Only the Bromage exercise scores of study group 3 were significantly higher than those of the control group. The Apgar scores of the control group, study group 1, study group 2 and study group 3 were significantly decreased in turn (P<0.05). There was no significant difference in the incidence of adverse drug reactions among the four groups (P>0.05). Conclusion: Ropivacaine combined with 5.0 UG sufentanil epidural used in cesarean section can effectively improve the anesthetic effect and safety without increasing the influence on hemodynamics and maternal and infant outcomes.

Objective:To analyze the value of color Doppler ultrasound detection of fetal umbilical artery (UA) and middle cerebral artery (MAC) blood flowin late pregnancy on the evaluation of fetal growth restriction (FGR). Methods:120 cases of pregnant women in late pregnancy who were examined in our hospital from June 2016 to March 2018 were selected. Among them, 30 cases suspected as FGR by preoperative routine ultrasound were set as observation group, and other 90 cases without abnormalities were selected as control group. The two groups were given color Doppler ultrasound. The differences in UA and MAC blood flow parameters were compared, and they were followed up until the end of pregnancy, and the pregnancy outcomes of fetuses were compared between the two groups. Results: The UA EDV in observation group was significantly lower than that in control group (P<0.05) while the UA PI, RI, PSV/EDV and MCA PSV were higher than those in control group (P<0.05), and there was no significant difference in UA PSV and MCA EDV, PI, RI and PSV/EDV (P>0.05). The incidence rates of fetal distress, neonatal asphyxia, premature delivery and deathin observation group were significantly higher than those in control group (P<0.05). Conclusions: Color Doppler ultrasound can evaluate FGR and prognosis in late pregnancy by detecting UA and MAC blood flow of fetus in late pregnancy, and provide reliable basis for effective clinical intervention.

617 Clinical Application Value of Neonatal Amplitude Integrated Electroencephalography in Evaluating Brain Function Development Status in Premature Infants

Objective:To study the clinical application value of neonatal amplitude integrated electroencephalography (aEEG) in evaluating the status of brain functiondevelopment in premature infants. Methods:The clinical data of 274 preterm infants in our hospital from July 2016 to July 2018 were retrospectively analyzed. The preterm infants were divided into early stage group, middle stage group and late groupaccording to gestational age (GA). Another 120 healthy full-term neonates were selected as control group. And aEEG was monitored and the aEEG background activity, sleep-waking cycle (SWC) and amplitude voltage. 20 items of Neonatal Behavioral Neurological Assessment (NBNA) score and Bayley Scales of Infant Development (BSID) score were used to evaluate the development level of neonatal nervous system, and the correlation between aEEG indicators and scores of NBNA and BSID was analyzed. Results:The upper and lower bounds of aEEG amplitude voltage in the four groups at AS and QS stages were increased significantly with the increase of birth GA (P<0.05), and the differences between the two groups were statistically significant (P<0.05). There were significant differences in the background activity forms of aEEG in the four groups (P<0.05), and there were statistically significant differences in early stage group compared with those in middle stage group, late stage group and control group (P<0.05). There was a significant difference in the SWC maturity in the four groups (P<0.05), and there were significant differences between early stage group and middle stage group, late stage group and control group and between middle stage group, late stage group and control group (P<0.05). The scores of neonatal NBNA, MDI and PDI in the four groups were significantly increased with the increase of birth GA (P<0.05), and the differences between the groups were statistically significant (P<0.05). The results of Spearman rank correlation analysis showed that aEEG background activity forms, SWC mode and amplitude voltage were significantly positively correlated with NBNA, MDI and PDI (P<0.05). Conclusions:The proportion of CNV, the proportion of mature SWC and the amplitude of pop band were significantly increased with the increase of GA, and had a good correlation with NBNA, MDI and PDI scores, which had important reference significance for evaluating the development of brain function in premature infants

622 Comparison of Atorvastatin and Rosuvastatin in the treatment of Elderly Patients with Coronary Heart Disease

Objective: To explore the difference of efficacy between atorvastatin and rosuvastatin in the treatment of elderly patients with coronary heart disease (CHD), and to provide reference for clinical optimization of treatment. Methods: 180 elderly patients with CHD were treated in our hospital from March 2015 ~2017 to September. The age was more than 65 years old, and all were confirmed by coronary angiography (CAG). The patients were divided into the control group and the observation group by the random number table method, and the 2 groups were 90 cases each. On the basis of routine treatment, the control group took atorvastatin, 20mg/ times, 1 times /d, and the observation group took rosuvastatin, 10mg/ times and 1 times /d for 6 months. The changes of triglyceride (TG), total cholesterol (TC), high density lipoprotein (HDL-C) and low density lipoprotein cholesterol (LDL-C) were detected in 2 groups of CHD patients. The hypersensitive C reactive protein (hs-CRP) and brachial artery blood flow mediated diastolic function (FMD) were observed, and the adverse reactions of the 2 groups were statistically analyzed. Results: The 2 groups completed the treatment according to the doctor's advice, without withdrawal or falling off. After treatment, the blood lipid index TG, TC and LDL-C in the observation group were significantly lower than those in the control group and the control group (P<0.05). The serum hs-CRP in the observation group decreased significantly after treatment, and the FMD increased significantly (P<0.05) after treatment. The 2 groups were all stable and only a small amount of minor adverse symptoms appeared. The adverse reaction rate of the observation group was 8.89% (8/90), which was lower than that of the control group 11.11% (10/90), but the difference between the 2 groups was not statistically significant (P>0.05). Conclusion: Atorvastatin and rosuvastatin are both safe and effective drugs for the treatment of elderly CHD, but rosuvastatin has better effects on lipid regulation, inhibition of inflammatory reaction and improvement of vascular endothelial function.

Effective rate ,surgical rate ,complicaions and thevarious pathlogical risk factors incidence were analyzed and recompared .Results:Compared with IVCT group, the effective rate and surgical rate of TACE group was increased remarkably ,which was difference significantly(36.7%和53.3%,χ2=14.217,P=0.042; 73.3%和93.3%,χ2=12.905,P=0.047); Compared with IVCT group, The incidence rate of myelosuppression, gastrointestinal reaction and White blood cells reduce in TACE group were lower than those of IVCT group,which were difference significantly(χ2=16.718,36.904,18.412,P=0.038,0.0079,0.031); The incidence rate of parsmetrial involvement and deep muscleinfiltration,lymphnodemetastasis and vascularcercinomaboltransferring in TACE group were lower than those of IVCT group,which were difference significantly (χ2=17.370,21.090,31.813,P=0.036,0.024,0.0081).Conclusion: TACE is better than that of IVCT in neoadjuvant chemotherapy on cervical cancer,especially in reducing the thevarious pathlogical risk factors incidence.

629 Clinical Efficacy Observation of PFNA Combined with PFN Internal Fixation in the treatment of Intertrochanteric Fractures in the elderly

Objective: To investigate the clinical efficacy observation of PFNA combined with PFN internal fixation in the treatment of femoral intertrochanteric fracture in the elderly and its effect on VAS scores. Methods: 76 elderly patients with intertrochanteric fracture of femur admitted to our hospital from January 2016 to January 2018 were randomly divided into study group and control group. The control group was treated with PFN internal fixation, while the study group was treated with PFNA internal fixation on the basis of the control group. The clinical efficacy, complications, hip function scores and VAS scores of the two groups before and after treatment were compared. Results: The clinical treatment effect of the study group was significantly better than that of the control group (P<0.05), and the postoperative complications of the study group were less than the control group (P<0.05). There was no significant difference in hip function scores between the two groups before surgery (P>0.05). The postoperative hip function score was higher in the study group than in the control group (P<0.05), and there was no significant difference in VAS scores between the two groups before surgery (P>0.05). The postoperative VAS score was lower in the study group than in the control group. (P<0.05). Conclusion: In the course of clinical treatment of elderly patients with intertrochanteric fracture of femur, the combination of PFNA internal fixation has better clinical effect, less pain and lower VAS score. It has a very positive role in improving the quality of life of elderly patients. It should be further applied and promoted in clinical treatment.

Objective: To observe the efficacy of conventional radiotherapy combined with intracavitary brachytherapy combined with external high frequency hyperthermia combined with traditional Chinese medicine in the treatment of advanced cervical cancer. Methods: 66 patients with stage II b ~ III B of cervical cancer diagnosed by pathological diagnosis were treated in our hospital (Jan. 2015 to Jan. 2017). The random digital method was divided into experimental group and control group. The experimental group (n=33) and conventional radiotherapy and chemotherapy (n=33) were used in the experimental group (n=33). The short-term efficacy, side effects, and the end of follow-up were evaluated for the two year survival rate and quality of life. Results: In the experimental group, the rate of CR was 90.91%, the rate of PR was 33.33%, the rate of NC was 9.09%, the total effective rate was 90.91%, the rate of CR in the control group was 90.91%, the rate of PR was 33.33%, the rate of NC was 9.09%, the total effective rate was 90.91%. The side reaction rate of the experimental group was 9.09% and the control group was 36.64%. The increase of the quality of life after the treatment was also increased with the follow-up time, and the experimental group was also obviously improved. The survival rate of the experimental group was 100%, while that of the control group was only 72.73%. The difference was statistically significant (P < 0.05). Conclusion: For children with middle and advanced cervical cancer, conventional radiotherapy combined with intracavity close range therapy, combined with external high-frequency thermotherapy combined with traditional Chinese medicine can achieve better short-term effect. Lower toxic and side effects can improve the one year survival rate, improve the quality of life after treatment and result in ideal results.

Objective: To investigate the volume of hemorrhage after lung biopsy in the different lung segments under bronchoscope of lung cancer patients. Methods: A analysis was made about the volume of hemorrhage in different lung segments biopsy under bronchoscope in 79 patients with lung cancer between March 1, 2015 and April 1, 2018. Result: Not patients death in the 79 cases because of lung biopsy , according to the degree of bleeding after different lung lobe biopsy classification (20ml each of the following is a small amount of bleeding, 20 ml to 100 ml for the medium amount of bleeding, more than 100ml for a large number of bleeding) ,there are no significant statistical differences in the three groups , but from The specific amount , Mean blood loss at the posterior segment of apicoposterius segment in upper lobus(22.30±28.378) ml, lobus medius or lingulare segment(36.69±40.145) ml, and lower lobe basale segment (13.69±15.785) ml.the comparison in the three lobus were statistically significant (P<0.05). Conclusion: Lung biopsy under bronchoscope is generally safe, and the amount of hemorrhage in the lobus medius or lingulare segment is significantly greater than that in the upper and lower lobe basale segments, which have the lowest amount of bleeding in each segment.

639 Curative Effect of Thin-tube Transgastric Esophagus on Prevention of Esophagogastric Anastomotic Leakage and its Effect on Short-term Quality of Life after Operation

Objective: To investigate the curative effect of thin-tube transgastric esophagus on prevention of esophagogastric anastomotic leakage and its effect on short-term survival quality. Methods: 120 patients undergoing esophageal cancer resection in the hospital from December 2016 to February 2018 were divided into the thin tubular stomach group (n=60) and the thick tubular stomach group (n=60) by simple randomization method. The thin tubular stomach was made after esophageal cancer resection in the thin tubular gastric group. The thick tubular stomach was made after esophageal cancer resection in the thick tubular gastric group. the general conditions and postoperative complications of the two groups were compared, and the European Cancer Research and Treatment Organization (EORTC), the QLQ-C30 scale for cancer patients and the QLQ-OES18 scale for esophageal cancer were used to establish a quality of life assessment form to investigate the quality of life of patients 6 months after surgery. Results: There were no significant differences in operation time, intraoperative blood loss, postoperative drainage, and postoperative hospital stay between the two groups (P>0.05). The incidence of anastomotic leakage and reflux esophagitis in the thin tubular group was significantly lower than that in the thick tubular group (P<0.05), the incidence of anastomotic stenosis, pulmonary infection, arrhythmia, and chest and stomach syndrome had no statistically significant difference (P>0.05). At 6 months after surgery, the functional dimension scores in Quality of life scale of the thin tubular stomach group were significantly higher than those of the thick tubular stomach group (P<0.05), while the symptomatic dimension scores were significantly lower than those of the thick tubular stomach group (P<0.05). Conclusion: Thin-tube transgastric esophagectomy can effectively reduce the occurrence of postoperative anastomotic leakage and improve the short-term quality of life.

643 Analysis of the Operation time, Clinical Effect and Prognosis of Total Hip Arthroplasty in Elderly Patients with Femoral Neck Fracture

Objective: To explore the effects of different surgical timings on hip replacement of femoral neck fractures in the elderly for clinical efficacy and prognosis. Methods: A retrospective analysis of 180 hip replacement patients undergoing femoral neck fractures in our department was performed with hip arthroplasty. All patients were divided into group A and 60 according to different operation time (operation time was 3-5 days after fracture). 60 patients in group B (operation time was 5-7 days after fracture) and 60 patients in group C (operation time was more than 7 days after fracture). The intraoperative blood loss, operation time, length of hospital stay, and postoperative complications were compared among the three groups; the Harris hip scores of the hip joints were observed, preoperatively and postoperatively; the comparison and overall efficacy were compared. Results: Harris function scores at 3, 6 and 9 months after operation in group B were significantly higher than those in group C (P<0.05). Postoperative Harris function score in group A was significantly higher than that in group C at the same time (P<0.05). ) Harris function scores at 6 and 9 months after operation were significantly higher than those at the same time point in group B (P<0.05); postoperative acceptance rate in group A was significantly higher than that in group C (P<0.05); intraoperative bleeding in the three groups There was no statistically significant difference in the volume, operation time, and length of hospital stay between the three groups (P>0.05). The incidence of complications in group A and B was significantly lower than that in group C (P<0.05). Conclusion: The earlier the treatment time of hip replacement for elderly femoral neck fracture is, the better the surgical effect is.

648 Clinical Study of Locking Compression Plate Internal Fixation Through Palmar Approach in the Treatment of Unstable Distal Right Radius Fractures

Objective: To explore the clinical effect of locking compression plate internal fixation through palmar approach in the treatment of unstable distal right radius fractures. Methods: 53 patients with unstable distal right radius fractures in our hospital from June 2014 to June 2017 treated with locking compression plate internal fixation through palmar approach were used as study objects, the clinical efficacy, fracture healing, wrist recovery and postoperative complications were observed. Results: According to the Gartland and Werlkey functional score at the last follow-up, the excellent and good rate of surgical treatment was 94.34%; the palmar inclination, ulnar angle and radius length were significantly improved in the last follow-up (P<0.05); no complications were found in 53 patients after operation; the time of fracture healing was (8.45±1.47) weeks; at the end of follow-up, wrist mobility returned to 78%~100% of the healthy side, and the grip strength returned to 72%~91% of the healthy side. Conclusion: The treatment of unstable distal radius fracture with locking compression plate through palmar approach has good clinical effect. It can restore radius length, palmar inclination angle and ulnar deflection to the greatest extent, and wrist joint recovers well after operation. No complications occur after operation.

651 Correlation between 1H-MRS and 5-ALA Fluorescence Intensity in Malignant Gliomas

Objective: To investigate the correlation between the proton magnetic resonance spectroscopy (1H-MRS) and the 5-aminolevulinic acid (5-ALA) fluorescence intensity in malignant gliomas. Methods: From September 2012 to January 2018. 32 patients with malignant glioma were selected, all the patients were examined by 1H-MRS and 5-ALA fluorescence intensity, and were given the correlation analysis. Results: In the 32 patients, the conventional MRI were showed uneven enhancement, high signal around T2, unclear tumor border and significant edema around the tumor. The typical spectrum of tumor parenchyma of glioma were showed significant reduction in NAA peak, Cho peak significantly were increased, Cr peak were decreased not significantly, the Cho/NAA, Cho/Cr ratio were increased, NAA/Cr ratio were decreased, and compared the contralateral normal region were statistically significantly difference (P<0.05). The Cho/NAA, Cho/Cr and NAA/Cr ratios in the low level group were also significantly different from those in the high level group (P<0.05). The PpIX fluorescence intensity in the tumor parenchyma of glioma were 2.79±0.49, and the contralateral normal area were 1.84±0.51, so the tumor parenchyma region were significantly higher than that of the contralateral normal region (t=6.398, P=0.008). The Pearson coefficient analysis showed that the fluorescence intensity of PpIX in the tumor parenchyma were negatively correlated with NAA/Cr (P<0.05), and were significant positive correlation with Cho/Cr and Cho/NAA (P<0.05). Conclusion: There was a significant correlation between 1H-MRS and 5-ALA fluorescence intensity in malignant glioma.

658 Clinical Effect of Laparoscopy in Rectal Cancer and its Effect on the third Space Effect

Objective: To study clinical effect of laparoscopy in rectal cancer and its effect on the third space effect. Methods: 95 cases of rectal cancer patients received in our hospital from December 2014 to December 2017 were selected as subjects, the randomized table method was divided into the observation group (n=50) and the control group (n=45). The control group was treated by laparotomy and the observation group by laparoscopic rectal cancer resection. The operation time, intraoperative blood loss, number of lymph nodes dissection, time of first ventilation, time of getting out of bed, time of hospitalization, serum albumin, total protein, anterior albumin, electrolyte, fluid inflow and outflow, complications and patient satisfaction were compared between the two groups. Results: There was no significant difference in the number of lymph node dissection between the two groups. The operation time of the observation group was higher than that of the control group, and the intraoperative blood loss, time of first ventilation, time of out of bed activity and time of hospitalization were significantly lower than that of the control group (P<0.05). There was no significant difference in postoperative Na+, K+, Ca+ and protein levels between the two groups (P>0.05). The fluid volume and difference between the two groups were statistically significant (P<0.05). The total incidence of complications was 6.00% in the observation group, significantly lower than 28.89% in the control group. The patient satisfaction of the observation group was 98.00%, significantly higher than that of the control group (P<0.05). Conclusion: In patients with rectal cancer, the third space effect after laparotomy and laparotomy is abnormal in different degrees. However, laparotomy with laparotomy can reduce hospitalization time and complications, and the patient satisfaction is high, which is worthy of promotion and application.

663 Clinical Evaluation of Extracorporeal Circulation without Plasma Pre-filling in Children with Congenital Heart Disease

Objective: To explore the clinical evaluation of extracorporeal circulation without plasma pre-filling in children with congenital heart disease. Methods: 54 cases with Congenital heart disease in children failures were researched ,who from May 2013 to May 2016. According to random number table, they were divided into observation group and control group, 27 cases in each group.The levels of hematological parameters, coagulation function and liver and kidney function were measured before and after treatment. The duration of ICU, ICU and hospital stay were also observed. Results: After treatment, the blood parameters of the two groups were significantly higher than those before treatment (P<0.05). After treatment, the two groups of patients with blood coagulation index level was significantly higher than before treatment (P<0.05), and the plasma coagulation index level was no significant difference (P>0.05). After treatment, the liver and kidney function of the two groups were significantly improved compared with before treatment (P<0.05), and the plasma group had no significant difference in liver and kidney function (P>0.05). The recovery of the two groups after treatment was observed. The results showed that: there was no significant difference in the ventilation time, ICU time and hospital stay between the two groups (P>0.05). Conclusion: Pre-filling of congenital heart disease children with Galaxy has the same effect as plasma in maintaining coagulation function, liver and kidney function and blood parameters. Therefore, Galaxy can replace plasma for in vitro pre-filling of congenital heart disease children and reduce the use of plasma.

668 Curative Effect and Safety Analysis of two Ventilation Modes of Non-invasive Positive Pressure Ventilation (NIPPV) in the treatment of Patients with COPD and type II Respiratory Failure

Objective: To analyze the curative effect and safety of continuous positive airway pressure (CPAP) and bi-level spontaneous breathing and time-controlled ventilation (ST) in the treatment of patients with chronic obstructive pulmonary disease (COPD) complicated with type II respiratory failure. Methods: 86 patients with COPD and type II respiratory failure in the hospital were enrolled in the study. According to simple random grouping method, they were divided into CPAP group and ST group, 43 cases in each group. The two groups were treated with non-invasive ventilator of CPAP mode and bi-level ST mode, respectively. The levels and safety of vital signs indexes , blood gas analysis indexes, respiratory mechanics indexes [breathing frequency (f), tidal volume (VT), inspiratory/expiratory cycle time ratio (Ti/T)] in the two groups were compared. Results: After treatment for 3 days, HR, SBP, DBP, PaCO2 and f in both groups were all decreased (P<0.05), while SpO2, PaO2, pH, VT and Ti/T were all increased (P<0.05). The differences in HR, SpO2, SBP, DBP, PaO2, PaCO2, pH, f and VT before and at 3d after treatment of ST group are greater than those of CPAP group (P<0.05). There was no significant difference in the incidence of adverse reactions between the ST group (9.30%) and the CPAP group (13.93%) (P>0.05). Conclusion: The effect of non-invasive ventilator with ST mode is superior to that with CPAP in the treatment of patients with COPD combined with type II respiratory failure, which can more improve the vital signs, blood gas analysis and respiratory mechanics of patients without obvious adverse effects on patients.

671 Study on the Correlation between Dermatomyositis and Myositis-specific Antibodies in Children

Objective: To investigate the clinical manifestations of juvenile dermatomyositis (JDM) and correlation with myositis specific autoantibodies (MSAs). Methods: 182 cases of JDM in our hospital from June 2012 to March 2018 were collected. MSAs were detected and the relationship between MSAs and clinical data was analyzed. Results: Among 182 children, 136 (74.73%) were MSAs positive. Anti-ARS, MDA51 and TIF1-gamma were the three most common subtypes. The positive rates were 20.88%, 18.68% and 18.13% respectively. Single factor analysis and logistic regression analysis showed that anti-NXP2 antibody positive [OR = 2.764, 95% CI (1.268-7.013), P = 0.013], anti-TIF1-gamma antibodpositive [OR = 2.812, 95% CI (1.567-4.923), P = 0.000, P = 0.000, P = 0.000] and anti-SAE antibodpositive [OR = 4.758, 95% CI (1.079-79-19.587, 95% CI (1.079-19.587-19.587), P = 0.035] were all independent risk factors for dysphagia in JDM children. Anti-NXP2, 95% CI (OR = 2, 95% CI (1.268-95% CI (1.95% CI (0.173-0.745), P = 0.006] did not. Easy to swallow disorders. Positive anti-Jo-1 antibody [OR = 6.321, 95% CI (1.684-20.357), P = 0.007] and anti-MDA5 (OR = 5.349, 95% CI (2.513-10.687), P = 0.003) were independent risk factors for interstitial lung disease (ILD) in JDM children. ILD was not easy to occur in patients with positive anti-Mi-2 antibody [OR = 0.213, 95% CI (0.068-0.579), P = 0.004] and anti-TIF1-gamma antibody [OR = 0.175, 95% CI (0.084-0.382), P = 0.000]. Conclusion: The positive rate of serum MSAs in children with JDM is high. Different MSAs subtypes are related to the related symptoms. The detection of MSAs is important for the diagnosis and treatment of diseases.

Methods: 300 patients with acute cerebral infarction who were hospitalized from January 2015 to October 2017 were randomly divided into two groups. Patients in the two groups were compared Symptoms, overall treatment, coagulation, endothelial function and safety. 679 Analysis of Related Factors Influencing the Efficacy of Arthroscopic Surgery of Early Primary Frozen Shoulder

Objective: To explore the related factors affecting the efficacy of arthroscopic surgery of early primary frozen shoulder (PFS). Methods:The clinical data of 152 patients with early PFS admitted to our hospital from May 2015 to May 2018 were retrospectively analyzed. Patients were divided into good efficacy group (n=122) and poor efficacy group (n=30) according to the efficacy at 4 w after operation. Univariate analysis was performed clinical data, and the related factors affecting the efficacy of early arthroscopic surgery for PFS were recorded, and Logistic multivariate regression analysis was performed. Results:Logistic multivariate regression analysis showed that diabetes, cervical spondylosis, disease season, preoperative internal posterior rotation angle, preoperative shoulder joint score and postoperative functional exercise were independent related factors influencing the efficacy of arthroscopic surgery of early PFS (P<0.05). Conclusions:Arthroscopic surgery is effective in the treatment of early PFS, which is closely related to many factors, such as diabetes mellitus, cervical spondylosis, onset season, preoperative internal and posterior rotation angle, preoperative shoulder joint score and postoperative functional exercise.

683 Predictive Value of Erythrocyte Distribution Width and Modified Shock Index for Mace Prognosis in ST-segment Elevation Myocardial Infarction

Objective: To explore the predictive value of modified shock index (MSI) and red biood cell distribution width (RDW) to in predicting short-term major cardiac adverse events (Mace) in patients with acute ST elevation myocardial infarction (STEMI). Methods: STEMI patients (n=586) were chosen from the Department of Emergency in the emergency department of our hospital from June 2016 to March 2018 were selected, and general data, blood routine, blood pressure and heart rate were collected, shock index (SI), MSI and RDW were calculated, and were divided into Mace group and non-Mace group. General data, heart rate, blood pressure, RDW,SI and MSI were compared between the two groups. Results: STEMI patients were divided into Mace group (n=132) and non-Mace group (n=454). There were no statistically significant differences in the general data, heart rate, left ventricular ejection fraction (LVEF) and other indicators between the two groups (P>0.05). systolic pressure, diastolic pressure and The difference in systolic blood pressure, diastolic blood pressure, heart rate and Killip class Ⅲ and Ⅳ had statistical significance between two groups (P<0.05). SI, MSI and RDW in the MACE group were significantly higher than those in the non-mace group, and the difference was statistically significant (P<0.01). The incidence of Mace in hospital was 22.52%, including 6.48% (n=38) of all-cause death, and the ROC area under SI, MSI and RDW were 0.701 0.707 and 0.713, respectively, which was of statistical significance in predicting the incidence of Mace in STEMI patients (P<0.001). Conclusion: SI, MSI and RDW may be simple, economical and effective indicators for predicting short-term prognosis of STEMI patients in emergency department.

687 Screening of Cervical Cancer in Garze area and Women' s Cognition Degree Towards Cervical Cancer and HPV Related knowledge

Objective: To explore the screening of cervical cancer in women in Garze area and their cognition degree. Methods: The clinical data of 1913 women who underwent colposcopy biopsy after cervical cancer screening in the People' s Hospital of Ganzi Tibetan Autonomous Prefecture in the past 2 years were retrospectively analyzed. The results of colposcopy biopsy were used as the gold standard to analyze the diagnostic efficacy of each screening method. A total of 330 patients who participated in cervical cancer cognition survey were selected, and the differences in scores of cervical cancer screening belief scales among women of different ages, education levels, marital status and family monthly income. Results: Simple Thinprep cytology test (TCT)<simple human papillomavirus (HPV) examination <combined examination (all P<0.05), and the consistencies of HPV examination and combined examination with colposcopy biopsy results both were good (both Kappa>0.7). The results of cervical cancer screening belief scales scores showed age ranged from 19 to 29 years old >30~ years old >40~ years old (all P<0.05), and there was no statistical significance between age groups of 40~ years old, 50~ years old and 60~ years old (all P>0.05), and education level with junior high school and below>high school or technical secondary school>juniorcollege and above (all P<0.05), and married <unmarried/divorced (P<0.05), and the family monthly income with ~1500 yuan>~ 3000 yuan>~ 4500 yuan>4500 yuan or more (all P<0.05). The screening service access channels provided by the unit/community >physician recommendation >voluntary offer (all P<0.05). Conclusions: TCT combined with HPV examination is more effective in the diagnosis of cervical cancer. Conditional women recommend it as a screening project to investigate women's poor knowledge of cervical cancer and HPV. It is necessary to carry out targeted education and intervention in combination with individual conditions.

691 Effects of Dizosin combined with Ropivacaine on Fetus JZand Parturient for Epidural Labor Analgesia

Objective: To explore the effect of ropivacaine combined with dizosin on epidural labor analgesia on fetus and puerpera. Methods: The subjects were selected from March 2018 to March 2019 in the obstetrics department of Chengde Maternal and Child Health Hospital. 180 cases of primipara with spontaneous delivery were selected. All the primipara were divided into two groups according to the random arrangement method of the patient's admission number. The study group (group D) and the control group (group C) had 90 cases in each group. Group C: no epidural labor analgesia; Group D: PECA analgesia pump: 0.125% ropivacaine + 5 mg of Dzosin + 0.9% sodium chloride injection, a total of 100 ml, background dose of 10 ml/h, additional dose of 8 ml, locking time of 20 minutes. Observed indicators: the duration of labor, the amount of bleeding during delivery, umbilical artery blood gas results and neonatal Apgar score in the two groups. The PI, RI and S/D values of umbilical artery, middle cerebral artery and uterine artery of the two groups were observed by ultrasonography. The adverse reactions of the two groups were recorded. The observation time was 30 minutes before analgesia (T1), 10 minutes after analgesia (T2), 30 minutes after analgesia (T3), 1 hour after analgesia (T4), 2 hours after analgesia (T5), and uterine opening (T6). Maternal adverse reactions were recorded in two groups. Results: There were no significant differences in labor duration, blood gas analysis and Apgar score between the two groups (P>0.05). At T2, the S and D values of uterine artery blood flow in the study group were higher than those in the control group (P<0.05). At different time points, there were no significant differences in P I, RI and S/D between the two groups (P>0.05). There was no significant difference in adverse reactions between the two groups (P>0.05). Conclusion: epidural analgesia with ropivacaine combined with dizosin can significantly relieve the pain of parturient without adverse effects on infants and parturient.

699 Expression and Significance of Serum Decoy Receptor 3, Interferon-gamma and Anti-cyclic Citrullinated Peptide Antibodies in Patients with Rheumatic Arthritis

Objective: To investigate the expression and significance of serum decoy receptor 3, interferon-gamma and anti-cyclic citrullinated peptide antibodies in patients with rheumatoid arthritis. Methods: Seventy patients with rheumatoid arthritis admitted to our hospital from February 2017 to December 2017 were selected as observation group, and 65 healthy subjects who received health examination in our hospital during the same period were selected as control group. Fasting venous blood was collected from all subjects on the morning of the 2nd day after admission. The expression of serum decoy receptor 3, interferon-gamma and anti-cyclic citrullinated peptide antibody was detected. The changes and significance of serum decoy receptor 3, interferon-gamma and anti-cyclic citrullinated peptide antibodies in patients with rheumatoid arthritis were analyzed. Results: The serum levels of decoy receptor 3, interferon-gamma and anti-cyclic citrullinated peptide antibodies in the observation group were (4631.05 (+3180.24) pg/mL, (1341.23 (+482.94) pg/mL and (86.51 (+6.38) U/mL, respectively, while those in the control group were (481.53 (+150.07) pg/mL, (248.79 (+62.63) pg/mL and (45.41 (+8.35) U/mL, respectively, with statistical differences (P<0.05); Pearson correlation analysis showed that the serum levels of decoy receptor 3, interferon and anti-cyclic Cit Deception receptor 3, interferon-gamma and anti-cyclic citrullinated peptide antibody levels were positively correlated (r = 0.658, 0.744, P<0.05). Conclusion: The serum levels of decoy receptor 3, interferon-gamma and anti-cyclic citrullinated peptide antibodies in patients with rheumatoid arthritis were significantly increased, and the serum levels of decoy receptor 3, interferon-gamma and anti-cyclic citrullinated peptide antibodies were positively correlated, which could assist in the diagnosis of rheumatoid arthritis.
