705 Effects of Propofol and Remifentanil on Oxygen Metabolism and S100β BDNF Aβ-42/tau in elderly Patients with Hypertension Undergoing Abdominal Surgery

Objective: To study the effects of propofol and remifentanil on oxygen metabolism and S100 β, BDNF,Aβ-42/tau in elderly patients with hypertension undergoing abdominal surgery. Methods: 80 elderly hypertensive patients undergoing general anesthesia tracheal intubation and abdominal surgery were selected from January 2017 to May 2018. They were randomly divided into control group and observation group, with 40 cases in each group. The control group was maintained with propofol anesthesia; the observation group was anesthetized with propofol and remifentanil. The sphygmomanometer heart rate changes at different time points during the operation were compared between the two groups. The intraoperative esmolol and nitroglycerin levels, respiration recovery time, resuscitation time, catheter removal time, and incidence of adverse reactions after resuscitation were recorded. And the levels of cerebral oxygen metabolism, serum S100 β, brain-derived neurotrophic factor (BDNF), Aβ -42, and tau-181 were compared. Results: The systolic blood pressure, diastolic blood pressure and heart rate of the control group and the observation group at the time of extubation (T3) were significantly higher than before (T1), after the completion of surgery (T2), and after the extubation (T4), and the differences were statistically significant. Significance (P<0.05). Respiratory recovery time, resuscitation time and organ extraction time of the control group were all later than those of the observation group, and the use of esmolol and nitroglycerin were both higher than the observation group, and the difference was statistically significant (t=10.300,P=0.000; t=13.172,P=0.000;t=9.077, P=0.000; t=6.314, P=0.000; t=11.744, P=0.000). There was no difference in CERO2 and Da-jvO2 in the two groups at T1 (P>0.05), and the indexes in the observation group were lower than those in the control group at the beginning of operation (T5), 0.5 h after operation (T6) and T2 (t=3.445, P=0.001, t=4.551, P=0.000, t=3.894, P=0.000; t=3.571, P=0.001, t=5.489, P=0.000, t=4.112, P=0.000). There was no difference in serum S100 β, BDNF, A β-42 and tau-181 between the two groups at T1 (t=0.735, P=0.465; t=0.588, P=0.558; t=0.262, P=0.794; t=0.261, P=0.795). The serum levels of S100 β, A β -42 and tau-181 in the observation group were lower than those in the control group, while the levels of serum BDNF in the observation group were higher than the control group (t=6.914, P=0.000, t=11.524, P=0.000, t=6.600, P=0.000; t=3.267, P=0.002, t=4.746, P=0.000, t=3.665, P=0.000; t=5.941, P=0.000, t=6.521, P=0.000, t=4.406, P=0.000; t=2.919, P=0.005, t=4.282, P=0.000, t=4.047, P=0.000). The incidence of lethargy, abnormal blood pressure, abnormal heart rate, nausea and vomiting were not statistically significant in the two groups (χ2=0.346, P=0.556; χ2=0.000, P=1.000; χ2=1.013, P=0.314; χ2=0.346, P=0.556). Conclusion: The blood pressure and heart rate were stable during the operation by propofol combined with remifentanil, and the amount of antihypertensive drugs was effectively reduced. It has little effect on cerebral oxygen metabolism and brain nerve cells. The postoperative recovery was rapid and it was helpful to shorten the operating room residence time.

711 Analysis on the Diagnostic Value and Correlation between Urinary NGAL Level and Serum pro-BNP in Patients with Ischemic Heart Failure

Objective: To investigate the diagnostic value and the correlation between urinary NGAL level and serum pro-BNP in patients with ischemic heart failure. Methods: 100 cases of ischemic heart failure (case group) and 96 cases of healthy individuals (control group) were selected. The levels of urinary NGAL and serum pro-BNP levels were detected, and the correlation with the disease severity was analyzed. Then the diagnosis value of the two indicators was further investigated. Results: The expression level of NGAL and NT-pro BNP was higher in the case group than in the control group (P<0.05). The expression level of CK, CTnL and hs-CRP was higher in the case group than in the control group (P<0.05). The case group had higher levels of LAD and RAD as well as lower level of LVE than those of the control group (P<0.05). With the increase of cardiac function grade, the levels of NGAL and NT-pro BNP gradually were increased (P<0.05); NT-pro BNP was positively correlated with CK, CTnL, hs-CRP, LAD, RAD, cardiac functional grading, and was negatively correlated with LVEF (P<0.01). High levels of NGAL, NT-pro BNP, as well as low levels of LVEF were risk factors for ischemic heart failure (P<0.05). The AUC of NGAL and NT-pro BNP were similar, suggesting that the sensitivity and specificity in the diagnosis of ischemic heart failure were similar (P>0.05), which was less than that of the LVEF (P<0.05). Conclusion: Urinary NGAL levels and serum pro-BNP are significantly elevated in patients with ischemic heart failure, which are positively correlated with the cardiac function grade and damage degree. High levels of NGAL, NT-pro BNP, as well as low levels of LVEF are risk factors for ischemic heart failure; The sensitivity and specificity of NGAL and NT-pro BNP for the diagnosis of ischemic heart failure are relatively high, which can used as biological markers and are worthy of promotion.

716 Effects of Acetylcysteine Effervescent Tablets combined with Salbutamol Sulfate on Pulmonary Function and Serum Soluble Myeloid Cell-triggering Receptor-1 in Patients with Stable COPD

Objective: To investigate the relationship between acetylcysteine effervescent tablets and salbutamol sulfate aerosol in the stable period lung function and serum soluble myeloid cell trigger receptor-1 (sTREM-1) in patients with chronic obstructive pulmonary disease (COPD). Methods: 82 patients with COPD in our hospital (November 2016-August 2018) were enrolled according to the treatment plan (n=41) and the control group (n=41). On the basis of conventional treatment, the control group was treated with salbutamol sulfate aerosol, and the study group was treated with acetylcysteine effervescent tablets on the basis of the control group for 10 days. The two groups of clinical symptoms (pulmonary signs, wheezing, dyspnea, cough) were relieved, clinical efficacy, lung function indicators before and after treatment [forced vital capacity (FVC), 1st s forced breathing volume (FEV1), FEV1 /FVC] levels, serum sTREM-1 and inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), IL-8] levels, adverse reactions (headache, gastrointestinal tract The incidence of discomfort, itching, vomiting and nausea were counted. Results: ①Clinical symptoms: The lung signs, wheezing, dyspnea, and cough disappeared in the study group were shorter than the control group (P<0.05). ②Clinical efficacy: the total effective rate of the study group was 95.12% (39/41). 78.05% (32/41) in the control group (P<0.05); ③Pulmonary function: The levels of FVC, FEV1, and FEV1/FVC increased after treatment at the end of the two groups, and the study group was higher than the control group (P<0.05); ④serum sTREM-1 and inflammatory factors: serum sTREM-1 and TNF-α, IL-6, IL-8 levels were lower than before treatment, and the study group was lower than the control group ( P<0.05); ⑤Adverse reactions: The incidence of adverse reactions in the study group was 14.63% (6/41) and there was no significant difference between the control group and 9.76% (4/41) (P>0.05). Conclusion: The combination of salbutamol sulfate aerosol and acetylcysteine effervescent tablets in the treatment of patients with stable COPD can effectively shorten the time of clinical symptom relief, reduce serum inflammatory factors and sTREM-1 levels, and reduce the degree of inflammatory response in vivo, inhibit the disease, promote the early recovery of lung function, and does not increase the risk of adverse reactions, and is safe.

722 The Roles of MiR-33a and MiR-133a in the Assessment of Liver Fibrosis in Patients with Chronic Hepatitis B Infection

Objective: To analyze the expressions of serum miR-33a and miR-133a in patients with chronic hepatitis B virus (HBV) infection, and to explore their relationships with hepatic fibrosis and their diagnostic values for severe hepatic fibrosis. Methods: From January 2017 to June 2018, 132 patients with chronic HBV infection (hepatitis B group) was conducted liver biopsy, liver fibrosis was graded according to the Metavir system score, including 78 cases of mild to moderate liver fibrosis group and 54 cases of severe liver fibrosis group. At the same time, 100 healthy people without family history of hepatitis B were selected as control group. RT-PCR was used to detect the expression levels of miR-33a and miR-133a in the subjects, Pearson correlation coefficient (r) method and receiver operating characteristic curve (ROC) were used to analyze the relationships between the levels of miR-33a and miR-133a with the grading of hepatic fibrosis in patients with severe hepatic fibrosis, and its diagnostic value for severe liver fibrosis. Results: The expression level of ALT、AST、TBIL、miR-33a in hepatitis B group was higher than that in the control group (P<0.05), and the expression level of miR-133a was lower than that in the control group (P<0.05). The expression level of serum miR-33a in severe liver fibrosis group was higher than that in mild to moderate liver fiber. The level of miR-133a was lower than that of mild to moderate liver fibrosis group (P<0.05); there was a positive correlation between serum miR-33a with liver fibrosis grading in hepatitis B group (r=0.613, P =0.005), there was a negative correlation between miR-133a and liver fibrosis grade (r=-0.598, P =0.009); the area under the ROC curve (AUC) of serum miR-33a and miR-133a in diagnosing severe hepatic fibrosis was 0.740 and 0.694 respectively, the diagnostic critical value of miR-33a was 1.79, the sensitivity and specificity was 74.97% and 89.39% respectively, the diagnostic critical value of miR-133a was 0.86, the sensitivity and specificity was 70.70% and 80.45% respectively. Conclusion: Serum miR-33a levels in patients with chronic HBV infection is increased, the level of miR-133a is decreased. It is related to the grading of liver fibrosis and it can be used as a biomarker for clinical assessment of liver fibrosis.

726 Effects of Leuprolide Acetate Microspheres Combined with Laparoscopy on Serum TIMP-1 MMP-9 Levels and Conception Rate in Patients with Endometriosis

Objective: To investigate the effect of pregnancy rate, serum matrix metalloproteinase-9 (MMP-9) and matrix metalloproteinase inhibitor-1 (TIMP-1) levels in endometriosis patients who were treated with leuprolide acetate microspheres and laparoscopy. Methods: 98 patients with endometriosis in our hospital (From December 2014 to March 2017) were randomly divided into the control group (n=49) and the study group (n=49) according to the treatment plan. The control group underwent laparoscopic conservative surgery. The study group was given leuprolide acetate microspheres on the basis of the control group (once / 4 weeks, a total of 6 injections). The clinical efficacy of the two groups, the sex hormone indicators before and after treatment [Luteinizing hormone (LH), progesterone (P), follicle stimulating hormone (FSH), estradiol (E2)] levels, serum MMP-9 and TIMP-1 levels, MMP-9/TIMP-1, pre-treatment and 1 month post-treatment estrogen status scores (Kupperman) and pelvic pain scores (VAS), followed up for 9 to 12 months, statistics of pregnancy rates were compared. Results: ①Clinical efficacy: The total effective rate of the study group was 93.88% (46/49) higher than that of the control group (79.59% (39/49) (P<0.05); ②Sex hormone indicators: serum LH, P after treatment The levels of FSH and E2 were lower than those before treatment, and the study group was lower than the control group (P<0.05). ③Serum MMP-9 and TIMP-1: serum MMP-9 and TIMP-1 levels and MMP after treatment -9/TIMP-1 was lower than before treatment, and the study group was lower than the control group (P<0.05); ⑤Kupperman and VAS scores: Kupperman and VAS scores were lower than those before treatment in the two groups after treatment. The study group was lower than the control group (P<0.05); ⑥Pregnancy rate: 9 months and 12 months after treatment, the pregnancy rate of the study group (36.73%, 51.02%) was higher than that of the control group (18.37%, 28.57%). (P<0.05). Conclusion: The use of leuprolide acetate microspheres combined with laparoscopic treatment of endometriosis can effectively improve serum sex hormone content, lower serum MMP-9 and TIMP-1 levels, and regulate MMP-9/TIMP-1 balance. It relieves clinical symptoms and pain, improves the overall therapeutic effect of the disease, and is conducive to improving the conception rate, which has certain positive significance for improving the prognosis of patients.

732 Observation of Carative Effect of Intravitreal Injection of Ranibizumab Combined with 532nm Laser Photocoagulation in Patients with Ischemic Retinal Vein Occlusion

Objective: To observe the short-term efficacy of intravitreal injection of ranibizumab combined with 532nm laser photocoagulation in patients with ischemic retinal vein occlusion (RVO). Methods: From October 2015 to October 2017, 78 patients (78 eyes) with ischemic RVO were enrolled. According to the random number table, 39 patients (39 eyes) and 39 patients (39 eyes) were included in the control group. The control group was treated with 532 nm laser photocoagulation. The experimental group was treated with intravitreal injection of ranibizumab plus 532 nm laser photocoagulation. Six-month follow-up after treatment was proceeded; the clinical effects, quality of life score (SF-36) retinal leakage and best corrected visual acuity (BCVA), macular central retinal thickness (CMT) during pre-treatment, 1 month, 3 months, 6 months after treatment were compared. Results: The total effective rate of the test group was 94.87% higher than that of the control group at 6 months after treatment, and the difference was statistically significant (P<0.05). The degree of retinal leakage in the test group was lower than that of the control group at 6 months after treatment. The difference was statistically significant. Significance (P<0.05); BCVA and SF-36 scores increased at 1 month, 3 months, and 6 months after treatment, and the test group increased significantly. The BCVA group and the different time points in the two groups. Compared with the interaction between groups and at different time points, the difference was statistically significant (P<0.05). The CMT of the two groups decreased at 1 month, 3 months and 6 months after treatment, and the test group decreased greatly. There was statistically significant difference between the two groups of CMT groups, at different time points, and between groups at different time points (P<0.05). Conclusion: Patients with ischemic retinal vein occlusion combined with intravitreal injection of ranibizumab and 532nm laser photocoagulation can improve clinical outcomes, improve visual acuity, reduce retinal leakage and macular edema, and improve patients' quality of life.

737 Application of GRACE Score Combined with Serum Lp-PLA2 and NT-proBNP Levels in Predicting MACE Events after PCI in Patients with AMI

Objective: To investigate the global acute coronary event registration score (GRACE) combined with serum N-terminal pro-brain natriuretic peptide (NT-proBNP) and lipoprotein-associated phospholipase A2 (Lp-PLA2) levels in patients with acute myocardial infarction (AMI) application value in the prediction of cardiovascular adverse events (MACE) after cortical coronary intervention (PCI). Methods: A total of 189 patients with acute myocardial infarction in our hospital (from November 2016 to March 2018) were enrolled. All patients were followed up for 6 months after PCI. 56 patients with myocardial infarction with MACE were assigned to MACE group, and the rest did not have MACE. Patients with myocardial infarction were set to non-MACE group. The general conditions of the two groups (gender, age, BMI, diastolic blood pressure, systolic blood pressure, history of heart failure, history of diabetes, heart rate) and GRACE scores were measured. Serum NT-proBNP levels were determined by double-antibody sandwich immunofluorescence assay. Serum Lp-PLA2 levels were determined by immunosorbent assay. The general conditions, GRACE scores, serum NT-proBNP, and Lp-PLA2 levels were statistically analyzed, and the risk factors associated with MACE in patients with acute myocardial infarction were statistically analyzed. Results: There were no significant differences in gender, age, BMI, diabetes history and non-MACE between the MACE group (P>0.05). The diastolic blood pressure, heart rate and heart failure and systolic blood pressure in the MACE group were lower than those in the non-MACE group(32.14%), higher than the Non-MACE group (3.76%) (P<0.05); GRACE score and serum NT-proBNP, Lp-PLA2 levels in MACE group were significantly higher than non-MACE group (P<0.05); Logistic multiple linear regression analysis showed that GRACE score ≥ 140 points, serum NT-proBNP level ≥1000 ng/nl, serum Lp-PLA2 level ≥210 ng/ml and the history of heart failure were important risk factors for MACE in patients with acute myocardial infarction (P<0.05). Conclusion: The GRACE score and the abnormal levels of serum NT-proBNP and Lp-PLA2 in patients with MACE after acute myocardial infarction have increased abnormalities, and the GRACE scores and serum NT-proBNP, Lp-PLA2 levels, and the history of heart failure are the important risk factors in patients with MACE. Clinical indicators can be used to predict the risk of MACE in patients with acute myocardial infarction after PCI, and to do the corresponding prevention and control work.

742 Application Value of Transrectal Ultrasonography combined with Serum CYFRA21-1 Level in Preoperative Diagnosis of Rectal Cancer Patients

Objective: To investigate the value of transrectal ultrasonography combined with serum C-keratin 19 fragment antigen 21-1 (CYFRA21-1) level in preoperative diagnosis of rectal cancer patients. Methods: A total of 91 patients with rectal cancer from November 2016 to October 2018 were enrolled in the study group. 91 healthy controls were selected as the control group. All patients underwent transrectal ultrasonography and serum CYFRA21-1 levels. The hemodynamic parameters [End-diastolic velocity (EDV), pulsatility index (PI), resistance index (RI)] and serum CYFRA21-1 levels in the clinical stage of the study group and the control group and the study group were statistically analyzed. The hemodynamic parameters of the study group and the control group [end diastolic flow rate (EDV), pulsatility index (PI), resistance index (RI)] and serum CYFRA21-1 level, analysis of accurate clinical diagnosis of rectal cancer by rectal ultrasound were compared between transrectal ultrasound and serum CYFRA21-1 for the diagnosis of rectal cancer alone and in combination. Results: The EDV and serum CYFRA21-1 levels in the study group were higher than those in the control group, and the RI and PI were lower than the control group (P<0.05). The accuracy of T-stage diagnosis of rectal cancer by rectal ultrasound was 89.01% (81/91); Degree (97.80%) and accuracy (96.70%) were higher than those of transrectal ultrasound (86.81%, 92.31%) and serum CYFRA21-1 (69.23%, 82.42%) (P<0.05). Conclusion: Transrectal ultrasonography combined with serum CYFRA21-1 level detection can effectively improve the sensitivity and accuracy of rectal cancer diagnosis, and can accurately assess the disease stage, and provide a reliable basis for clinical development and adjustment of treatment options.

747 Curative Efficacy of Permetazole Sodium combined with Cisplatin in treatment of Advanced Lung Adenocarcinoma and its Effects on Serum cyfra21-1 CA125 and CEA

Objective: To study curative efficacy of permetazole sodium combined with cisplatin in treatment of advanced lung adenocarcinoma and its effectson serum cytokine fragment antigen 21-1 (cyfra21-1), carbohydrate antigen 125 (CA125), carcinoembryonic antigen (CEA). Methods: 90 patients with advanced lung adenocarcinoma from May 2014 to May 2017 for research, they were randomly divided into observation group (n=50) and control group (n=40). The observation group was treated with the combination of permetazole and cisplatin. The control group was treated with paclitaxel combined with cisplatin. Clinical efficacy, CEA, CA125, cyfra21-1, neuron-specific enolase (NSE) and adverse reactions were compared between the two groups. Results: After treatment, the effective rate of the observation group was 72.00% after treatment, which was significantly higher than 42.50% of the control group (P<0.05). Before treatment, there was no significant difference in the serum cyfra21-1, CA125 and CEA levels between the two groups. After treatment, the serum cyfra21-1, CA125 and CEA levels of both groups decreased (P<0.05). The serum cyfra21-1, CA125 and CEA levels of the observation group were all lower than those of the control group (P<0.05). Before treatment, there was no significant difference in serum NSE level between the two groups. After treatment, serum NSE level decreased (P<0.05). The serum NSE level of the observation group was lower than that of the control group (P<0.05). During the treatment, the adverse reaction rate of the observation group was significantly lower than that of the control group (P<0.05). Conclusion: Patients with advanced lung adenocarcinoma treated by pmetraxer disodium combined with cisplatin have higher efficacy, which can effectively improve the levels of cyfra21-1, CA125 and CEA in serum of patients with less adverse reactions, and the patients are basically tolerable and worthy of popularization and application.

751 Curative Effect of Gamma Globulin combined with Prednisone in treatment Myasthenia Gravis and its Effect on Serum AchRAb TGF-β1 and Complement

Objective: To discuss the curative effect ofgamma globulin combined with prednisone in treatment myasthenia gravis and its effect on serum Acetylcholine receptor antibody (AchRAb), transforming growth factor-β1 (TGF-β1) and complement. Methods: 99 patients with myasthenia gravis who treated from January 2014 to April 2018 in our hospital, according to the simple random number table method these patients were divided into 56 control group and 43 observation group. The control group was treated with prednisone, and the observation group was treated with gamma globulin on the basis of the control group. Then clinical efficacy,absolute score of muscular weakness, levels of AchRAb, TGF-β1, complement at different time points,and adverse reactions occurred in both groups were compared. Results: After treatment, the total effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). Before treatment, absolute score, levela of AchRAb, TGF-β1 and complement in both group were no statistically significant difference (P>0.05). 3 months after treatment and at the end of treatment, the absolute score of muscular weakness and level of AchRAb in both groups were decreased, and the levels of TGF-β1 and complement in the observation group were lower than those in the control group, while those in the observation group were higher than those in the control group, there were statistically significant differences in absolute scores, AchRAb, TGF- 1 and complement levels between the two groups in terms of treatment methods and treatment time (P<0.05). Fatigue, appetite loss, blood pressure drop and weight gain occurred in both groups, and there was no statistical difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: The curative effect of gamma globulin combined with prednisone in the treatment of myasthenia gravis is superior to that of prednisone, which can reduce the serum AchRAb level, promote the expression of TGF-β1 and complement, and is safe and reliable.

Objective: To explore the effect of endoscopically-assisted lateral fissure surgery on hypertensive cerebral hemorrhage in basal ganglia region and the influence of patients' NIHSS score. Methods: 88 patients with hypertensive cerebral hemorrhage in basal ganglia region admitted to our hospital from January 2016 to January 2018 were selected and randomly divided into two groups. The control group was treated by transcortical microneurosurgical technique, and the research group was treated by translateral fissure surgery under endoscopy. The postoperative state, NIHSS score and surgical indicators of the two groups were compared. Result: the postoperative status, NIHSS score and surgical index of the study group were better than those of the control group (P<0.05). Conclusion: During the treatment of hypertensive intracerebral hemorrhage in basal ganglia region, endoscope-assisted surgery through lateral fissure has a significant effect, reducing the incidence of adverse reactions, high patient satisfaction and treatment compliance. Therefore, this method should be widely used.

759 Correlations Between Levels of Serum HIF-1α and VEGF with Coronary Artery Calcification in Maintenance Hemodialysis Patients

Objective: To detect the levels of serum hypoxia inducible factor-1α (HIF-1α) and vascular endothelial growth factor (VEGF) in patients with maintenance hemodialysis (MHD), and to explore the relationships between them with coronary artery calcification (CAC). Methods: 88 patients with MHD in our hospital were selected as MHD group and divided into low-risk group, middle-risk group and high-risk group according to Framingham risk score (FRS); at the same time, 56 healthy persons were selected as the control group. Enzyme linked immunosorbent assay (ELISA) was used to detect the levels of serum HIF-1 and VEGF in two groups, CAC was evaluated by coronary artery calcification score (CACs), the relationships between levels of serum HIF-1, VEGF with CACs in MHD patients were analyzed. Results: Compared with the control group, the smoking rate, hemoglobin (Hb), blood glucose (GLU), C reactive protein (CRP), triglyceride (TG) and high-density lipoprotein cholesterol (HDLC) levels in MHD group were significantly different (P<0.05); the levels of serum HIF-1 and VEGF in group MHD were much higher than those in reference group (P<0.05); the levels of serum HIF-1α, VEGF and CACs score in high-risk group and middle-risk group were significantly higher than those in low-risk group (P<0.05), the levels of serum HIF-1α, VEGF and CACs in high-risk group were significantly higher than those in middle-risk group (P<0.05); the levels of serum HIF-1α and VEGF in MHD patients were positively correlated with CACs (r=0.778, 0.747, P<0.05); multivariate regression analysis showed that HDLC, FRS, HIF-1α, VEGF and CRP were the main influencing factors of CACS. Conclusion: The levels of serum HIF-1α and VEGF are high in patients with MHD, which is related to CACS, and may be involved in the occurrence and development of coronary artery calcification in MHD patients.

764 Logistic Regression Analysis of Influencing Factors of Fetal Birth Failure after IVF-ET Successful Pregnancy

Objective: To explore the related factors influencing the failure of fetal birth after successful pregnancy of vitro fertilization-embryo transfer (IVF-ET) and to put forward preventive suggestions. Methods: The clinical datas of 247 cases of pregnant women after successful pregnancy of IVF-ET in our hospital were retrospectively analyzed from August 2015 to January 2018, and the patients were divided into successful group and failure group according to hether or not success baby birth, then the risk factors of infant success after successful pregnancy of IVF-ET were analyzed by single factor analysis and multiple factor Logistic regression analysis method, and put forward the prevention suggestions. Results: After follow-up, there were 206 cases with success baby birth in the 247 cases after successful pregnancy of IVF-ET and there were 41 cases with failed baby birth, and the failure rate of live births was 16.60%. There was no significant difference of the constituent ratios of the infertility years ≥ 6 years, primary infertility, with birth history, with ectopic pregnancy history, the number of embryos transferred ≥ 2, embryo transfer during freeze-thaw cycles, artificial cycle of embryo transfer and the short protocol, long protocol, super long protocol and antagonist protocol of the control ovary hyperstimulating protocol between the successful group and the failure group (P>0.05). The constituent ratios of the age ≥ 35 years old, history of abortion, history of assisted reproduction, history of uterine surgery, endometrial thickness on HCG injection day ≤10mm, depression, LH/FSH > 2 and fetal reduction of the successful group were significantly lower than those of the failure group, and the differences were statistically significant (P<0.05), and Logistic regression analysis showed that these factors were the risk factors for successful baby birth of the patients after successful pregnancy of IVF-ET (OR = 3.518, 2.962, 5.147, 5.007, 5.425, 4.932, 4.763, 4.294, all Ps<0.05). Conclusion: Older pregnant women, abortion history, assisted reproductive history, uterine surgery history, endometrial thickness on HCG injection day ≤10mm, depression, LH/FSH > 2 and fetal reduction all are risk factors of affecting the success baby birth after successful pregnancy of IVF-ET, which can increase the risk of infant failure, and the clinical work needs to strengthen the management of such patients.

769 Effect Observation of Tumor Patient Implantation of PICC Anterior Chest Radiography Combined with Body Surface Measurement

Objective: To explore the application value of chest radiography combined with body surface measurement in the measurement of central venous catheter (PICC) length in tumor patients. Methods: 60 cases of tumor patients requiring PICC catheter were selected from the oncology department of haian people's hospital of jiangsu province from May 2017 to April 2018, and were divided into the experimental group and the control group in 30 cases according to the random number table method. In the experimental group, the length of the PICC catheter was calculated by chest radiography combined with body surface measurement. In the control group, the length of the PICC catheter was calculated by body surface measurement formula alone. The accuracy rate of catheterization, length adjustment rate, incidence of ectopic catheter tip, incidence of postoperative complications and satisfaction of catheterization were compared between the two groups. Results: The accuracy rate of catheterization in the experimental group was 100% (30/30) higher than 86.7% (26/30) of the control group, P<0.05. The adjustment rate of catheter length and ectopic incidence of catheter tip in the experimental group were 6.7%(2/30) and 3.3% (1/30), respectively, lower than 30% (9/30) and 20% (6/30) of the control group, P<0.05. The complication rate of patients in the experimental group was 6.7% (2/30) lower than that of the control group (26.7% (8/30),P<0.05. Patients' satisfaction with catheterization in the experimental group was 96.7% (29/30) higher than that in the control group (24/30), P<0.05. Conclusion: Before the implantation of PICC in tumor patients, the application of chest radiography combined with body surface measurement can effectively improve the accuracy rate of catheterization, reduce the incidence of complications after catheterization in patients, and improve the satisfaction of patients, which is worthy of clinical application.

772 Effects of Omeprazole combined with Alpha Lipoic Acid on Gastrointestinal Hormones and Oxidative Stress in Type 2 Diabetic Patients with Gastroparesis

Objective: To investigate the effects of omeprazole combined with αlipoic acid on gastrointestinal hormones and oxidative stress in patients with type 2 diabetic gastroparesis. Methods: 92 patients with type 2 diabetic gastroparesis who were admitted to the hospital during the period from February to February 2018 were enrolled in the study. They were divided into the control group and the observation group by the simple random grouping method, 46 cases in each group. The control group was treated with αlipoic acid, and the observation group was treated with omeprazole combined with α lipoic acidfor 2 weeks. Blood glucose fluctuation[daily mean amplitude of glycemic excursions (MAGE), standard deviation of blood glucose (SDBG) and the largest amplitude of glycemic excursions (LAGE)], levels of gastrointestinal hormones [gastrin (GAS), motilin (MTL), somatostatin (SS)], oxidative stress indicators [glutathione peroxidase ( GSH-Px), superoxide dismutase (SOD), reactive oxygen species (ROS)] and gastric motility indexes (stomach contraction frequency, gastric emptying time, gastric emptying rate) and adverse reactions were compared between the two groups. Results: The MAGE, SDBG, LAGE, GAS, MTL and ROS levels in the observation group were lower than those in the control group, while SS, GSH-Px and SOD were higher than the control group(P<0.05). The gastric contraction frequency and gastric emptying rate in the observation group were higher than those in the control group, and the gastric emptying time was shorter than that in the control group(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (4.35% vs 2.17%) (P>0.05). Conclusion: Omeprazole combined with αlipoic acid can effectively regulate gastrointestinal hormones in patients with type 2 diabetic gastroparesis, relieve oxidative stress and clinical symptoms and promote gastrointestinal function recovery.

776 Screening Efficiency about the First Trimester Screening of PAPP-A Combined with the Second Trimester Screening of AFP for Down's Syndrome

Objective: To observe the screening effect of the first trimester serum PAPP-A combined with the second trimester serum AFP for Down's syndrome. Methods: A retrospective analysis was made of 1300 pregnant women who underwent prenatal screening and were followed up in the obstetric clinic of our hospital. This study is divided into two programs. First trimester program: Serum PAPP-A and AFP levels were measured during first trimester (10~13+6 weeks). Combined program: Serum PAPP-A levels were measured during first trimester (10~13+6 weeks), and AFP levels were measured during second trimester (15-20+6 weeks).Prenatal screening software Muldeale was used to estimate the risk of Down's syndrome in combination with the age, gestational age, weight, pregnancy history and previous medical history of pregnant women.The high-risk cutoff value of DS is 1/270, and more than 1/270 is high-risk. The screened high-risk pregnant women were given genetic counseling, and the fetal chromosome karyotype was analyzed by diagnostic amniocentesis. The abnormal fetus is selected by the pregnant woman and their family to choose therapeutic abortion. Calculate the positive rate and false positive rate of DS screening for both programs. . Results: The combined program screened 41 high-risk and 81 critical-risk DS patients. In the first trimester program,63 high-risk and 109 critical -risk DS patients were identified. The high risk, critical-risk of DS and total positive rate of screening results of combined program were lower than those of the first trimester program (P < 0.05).6 cases were diagnosed as DS by chromosome karyotype analysis of amniocentesis.5 cases of DS positive in the combined program,the positive rate was 83.3%. 3 cases of DS positive in the first trimester program,the positive rate was 50%. Compared with the two programs, the combined program is better than the first trimester program. Conclusion: The detection of serum PAPP-A in the first trimester combined with serum AFP in the second trimester has a definite effect for Down's syndrome screening. It is worthy of clinical application.

779 Analysis of Prognostic Risk Factors in Patients Infected by Acinetobacter Baumannii in Hospital's Intensive Care Unit

Objective:To investigate clinical status and Antibiotics for drug-resistance of Acinetobacter baumannii (AB) infection in ICU. Methods: The clinical data of 158 patients with Acinetobacter baumannii infection hospitalized in the East Intensive Care Unit (ICU) of Dongzhimen Hospital, Beijing University of Traditional Chinese Medicine from January 2014 to December 2017 were retrospectively analyzed, including annual isolation rate, strain source, prognosis and drug resistance. Results: The infection rate of AB was 12.2%, the mortality was 19.6%, and the isolation rate of strains decreased year by year. The source of the strain was mainly respiratory sputum, which was the main cause of pulmonary infection in ICU patients, especially ventilator-associated pneumonia. The antibiotics drug-resistant of AB accounted for 65.2%, and the resistance rate of various broad-spectrum antibiotics showed an increasing trend annual. The death group infected by AB was older、shorter remained in ICU、higher usage of mechanical ventilation, However There is no statistically significant (P>0.05). There were significant differences in drug resistance between multiple antibiotics and strains. Conclusion: The ICU of our hospital has been established for a short time, the infection rate of Acinetobacter baumannii is high, the isolation rate shows a downward trend, the drug resistance rate increases year by year, and the effective treatment drugs are few. The use of multiple antibiotics and strain resistance are risk factors for the death of Acinetobacter baumannii infected patients. In order to reduce the infection and transmission of Acinetobacter baumannii, we should strengthen the concept of infection control, interrupt the transmission of Acinetobacter baumannii and strengthen the clinical management of antibiotics.

783 Effects of Nicergoline Combined with Vinpocetine on Hemorheology and Inflammatory Response in Patients with Cerebral Infarction

Objective: To study the effects of nicergoline combined with vinpocetine on hemorheology and inflammatory response in patients with cerebral infarction. Methods: 116 cases of patients with cerebral infarction in our hospital were selected and randomly divided into control group and observation group, with 58 cases in each group. Both groups were treated with routine therapy, while the control group was treated with vinpocetine; the observation group was treated with nicergoline tablets on the basis of the control group. The observation group was treated with nicergoline tablets on the basis of the control group, and the curative effect was evaluated after 2 weeks of treatment. The hemorheological parameters, inflammatory reaction factors and functional scale scores of the two groups were observed before and after treatment, and adverse drug reactions during treatment were recorded. Results: After treatment, the whole blood ratio, high shear viscosity, low shear viscosity and plasma specific viscosity in the observation group were lower than those in the control group (P<0.05). The levels of TNF-a, IL-6 and hs-CRP in the observation group were significantly lower than those in the control group (P<0.05). The NIHSS and improved Barthel index scores of the observation group were significantly improved compared with those of the control group (P<0.05). And the incidence of adverse drug reactions was similar between the two groups during the treatment period (P>0.05). Conclusions: Nicergoline combined with vinpocetine for patients with cerebral infarction can significantly reduce the hemorheology parameters and serum inflammatory factors levels, improve the patients' coagulation function, and does not increase adverse drug reactions.

787 Effects of Alprostadil combined with Edaravone on Coagulation Function, Levels of Plasma cTnI Hcy and PCT and Neurological Function in Patients with Acute Cerebral Infarction Complicated with Hypertension

Objective: To explore the effects of alprostadil combined with edaravone on coagulation function, levels of plasma troponin I (cTnI), homocysteine (Hcy) and procalcitonin (PCT) and neurological function in patients with acute cerebral infarction (ACI) complicated with essential hypertension (EH). Methods: 126 patients with ACI+EH were prospectively studied and randomly grouped (observation group, n=63; control group, n=63). All patients were given conventional symptomatic basic treatment, and control group was given edaravone, and observation group was given alprostadil based on above treatment. The coagulation function indexes, levels of plasma cTnI, Hcy and PCT and neurological function were observed in the two groups before treatment and after 14 d of treatment. The adverse reactions of the two groups were evaluated. Results: After treatment, the activated partial thromboplastin time (APTT), thrombin time (TT) and prothrombin time (PT) were significantly shortened, the contents of D-dimer (DD) and fibrinogen (Fib) were significantly decreased (P<0.05), the levels of plasma cTnI, Hcy and P CT were significantly decreased (P<0.05), and the scores of neurological deficit scale (mNIHSS) were significantly decreased (P<0.05) and the levels of serum S100B were significantly decreased in both groups (P<0.05). The change range in the observation group was significantly larger than that in the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between groups (P>0.05). Conclusions: Alprostadil combined with edaravone can effectively improve the coagulation function of ACI + EH patients, alleviate craniocerebral injury and neurological function damage, and is beneficial to the prognosis of patients. It is safe and effective in clinical application.

791 Effects of Peritoneal Transport Characteristics on Endothelial Function, Inflammatory Factors and Oxidative Stress Status in Patients with Maintenance Peritoneal Dialysis

Objective: To investigate the effects of peritoneal transport characteristics on endothelial function, inflammatory factors and oxidative stress in patients with maintenance peritoneal dialysis (MPD). Methods: According to the basic dialysate/plasma creatinine ratio (D/P), 128 MPD patients were divided into two groups: high transport (HT, 72 cases) and low transport (LT, 56 cases). Another 60 healthy persons matched in sex and age were included in the control group. The correlation between peritoneal transport characteristics and various factors was analyzed. Results:The systolic blood pressure, serum creatinine, triglyceride, uric acid and incidence rate of malnutrition in HT group and LT group were significantly higher than those in control group (P<0.05) while the albumin was significantly lower than that in control group (P<0.05). The serum creatinine, albumin and triglyceride in LT group were significantly higher than those in HT group (P<0.05). The FMD in HT group and LT group was significantly lower than that in control group (P<0.05), and the FMD in HT group was significantly lower than that in LT group (P<0.05). The levels of serum CRP, IL-6 and TNF-α in HT group and LT group were significantly higher than those in control group (P<0.05), and the levels of serum CRP, IL-6 and TNF-α in HT group were significantly higher than those in LT group (P<0.05). Serum SOD level in HT group and LT group was significantly lower than that in control group (P<0.05) while the levels of serum MDA and AOPPs were significantly higher than those in control group (P<0.05). Serum SOD level in HT group was lower than that in LT group (P<0.05) while the levels of serum MDA and AOPPs were higher than those in LT group (P<0.05). Peritoneal transport characteristics were negatively correlated with FMD and SOD (P<0.05), and were positively correlated with CRP, IL-6, TNF-α, MDA and AOPPs (P<0.05). Conclusions:There are obvious vascular endothelial dysfunction, oxidative stress and inflammatory reactions in patients with MPD. The degrees of vascular endothelial dysfunction and oxidative stress and inflammatory response are higher in patients with high peritoneal transport characteristics than those in patients with low peritoneal transport characteristics.

800 Clinical Effect of Leizhu Monoclonal Antibody Injection combined with Compound Xueshuantong Capsule on age-related macular degeneration

Objective: To investigate the effects of Leizhuman injection combined with compound Xueshuantong Capsule on the changes of vascular endothelial growth factor, intraocular pressure and visual acuity in patients with age-related macular degeneration. Methods: 60 patients with age-related macular degeneration in our hospital were randomly divided into treatment group and control group, 30 cases in each group. The control group was given intravitreal injection of Leizhumab injection. The treatment groupwas given compound Xueshuantong capsule once a month on the basis of the control group. All patients were treated for 3 months. After treatment, the control group was given intravitreal injection of Leizhumab. Serum levels of vascular endothelial growth factor, CRP, CMT, CNV, intraocular pressure and visual acuity were measured. Results: After treatment, compared with the control group, the serum levels of vascular endothelial growth factor in the treatment groupwere significantly lower (P<0.05); the serum CRP levels in the treatment groupwere significantly lower (P<0.05); the levels of CMT, CNV in the treatment group were lower (P<0.05); The visual acuity (logMAR) and IOP level were significantly lower (P<0.05). Conclusion: The experimental results show that for age-related macular degeneration patients, Leizhumab injection combined with compound Xueshuantong capsule ophthalmic injection can reduce the levels of vascular endothelial growth factor and CRP in peripheral blood, and also reduce the contents of CMT and CNV, so as to lower the ocular blood pressure and improve the visual acuity. It has guiding significance for clinical practice.

804 Study on the Influencing Factors of Postmenopausal Women with Type 2 Diabetes Complicated with Fatty Liver

Objective: To observe the metabolic changes in the patients with type 2 diabetes mellitus combined with fatty liver disease, and to provide the basis for clinical treatment. Methods: The primary patients were divided into the case group (type 2 diabetes with fatty liver group 136 cases) and control group (simplex type 2 diabetes 138 cases), based on ultrasond diagnosis and clinical manifestation. We recorded the clinical data of the two groups, including height, weight, blood pressure; FBG, TG , TC, HDL-C, LDL-C. SPSS 19.0 statistical software was used for 2 test and multivariate regression analysis. Results: (1) Univariate analysis showed that there was no significant difference in blood pressure, HbA1c, FPG, TC and LDL-C between two groups in postmenopausal women with type 2 diabetes mellitus (P>0.05). The indexes of BMI, TG and HDL-C were significantly different between the case group and the control group (P<0.05). (2) Multivariate logistic regression analysis in postmenopausal women with type 2 diabetes mellitus: The results showed that BMI (OR = 4.069, 95% CI = 2.465 ~ 6.74) was a risk factor for fatty liver. Conclusion: Blood lipids, body mass index are the influence factors, overweight or obese are the risk factors of fatty liver in postmenopausal women with type 2 diabetes mellitus.

808 Study on the Control Current Status and Affecting Factors of Elderly Bronchial Asthma

Objective:To explore the control current status of elderly bronchial asthma and analyze the influencing factors for asthma control and attack severity. Methods:The clinical data of 82 elderly patients with bronchial asthma in our hospital from April 2017 to April 2018 were retrospectively analyzed. The general data, asthma control and attack severity of 82 elderly patients with bronchial asthma were recorded, and multivariate Logistic regression analysis was used to analyze the influencing factors for asthma control and attack severity. Results: Among 82 cases, males were in great proportion of 72.73%. Age was mainly 65-74 years old, accounting for 77.27%. The basic diseases were mainly diabetes and hypertension, accounting for 50.00% and 30.49% respectively. Asthma was completely controlled in 26.83% patients, well controlled in 47.56% patients and uncontrolled in 25.61% patients. The severity of asthma attack was mild in 54.88%, moderate in 14.63%, severe 25.61% and critically severe in 4.88%. Gender was not associated with asthma control and severity of asthma attack (P>0.05), but age, obesity, smoking and acute respiratory infection in the past 2 months were independent risk factors for asthma control and severity of asthma attack (P<0.05), and regular use of inhaled glucocorticoid and regular follow-up were independent protective factors affecting asthma control and severity of asthma attack (P<0.05). Conclusions:The control current status of bronchial asthma is good in our hospital, but age, obesity, smoking, acute respiratory infection in the past 2 months, use of inhaled glucocorticoid and follow-up status all are influencing factors for asthma control and attack severity. Clinically targeted interventions should be given based on the characteristics of elderly patients.

811 Analysis on Long-term Prognosis and Influencing Factors of Regular and Irregular Resection of Single Small Liver Cancer

Objective: To analyze the long-term prognosis and influencing factors of regular and irregular resection of single small liver cancer. Methods: 89 patients with single small liver cancer who treated from May 2010 to May 2013 in our hospital, according to random number table method, those patients were divided into 40 cases of A group and 49 cases of B group,group A used regular resection and group B used irregular resection. The operation and complications in both groups were compared, and the influencing factors of long-term prognosis were analyzed. Results: The operation time, intraoperative blood loss, margin size and length of hospital stay in the irregular resection group were lower than those in the regular resection group (P<0.05). the total complication rate in the both groups was no significant difference (P>0.05). Univariate analysis showed that gender, age, Child-pugh grade, serum HbsAg, CGT, AEP, total bilirubin, ALT level, tumor margin, hepatic occlusion time, bleeding volume, and whether the rule was removed for 5 years were no significant difference in survival rate (P>0.05). there were significant differences in 5-year survival rates between different albumin levels, tumor size, tumor capsule, long-term smoking, and intraoperative blood transfusion (P<0.05). Cox regression analysis showed that tumor size and long-term smoking were independent risk factors for the prognosis of patients with single hepatocellular carcinoma, tumor envelope integrity and albumin levels were protective factors. Conclusion: The long-term prognosis of small hepatocellular carcinoma is related to various factors. Regular hepatectomy is not an independent prognostic factor, and the optimal operation should be selected clinically according to the tumor anatomical location and liver function reserve of patients.

816 Analysis on Related Factors of Nosocomial Infection after Ureteroscope Holmium Laser Lithotripsy in Patients with Upper Urinary Calculi

Objective: To analyze the correlation between intraoperative infection, ureteroscopy and ureteral calculi in patients with intraoperative surgery, diabetes and upper urinary calculi. Methods: A total of 110 patients with upper urinary tract calculi from September 2016 to May 2018 were enrolled in our hospital. All patients underwent ureteroscopic holmium laser lithotripsy. According to whether or not nosocomial infection occurred after operation, they were divided into infected group and uninfected group. The clinical data of the patients were collected, and the risk factors of nosocomial infection after ureteroscope holmium laser lithotripsy were analyzed by Logistic regression. Results: Distribution of infection sites and pathogens: Among 24 cases of infection, urinary system infection accounted for 54.17% (13/24), digestive system accounted for 16.67% (4/24), and circulatory system infection accounted for 16.67% (4/24) ), a small number of respiratory infections 8.33% (2/24), incision infection 4.17% (1/24), 38 strains of co-cultured strains, including Escherichia coli 21.05% (8/38), Staphylococcus aureus 21.05% (8/38), the highest proportion of Enterococcus faecalis 15.79% (6/38); clinical data of the two groups: age of infection group > 60 years, length of operation > 120 min, intra-renal pressure > 30 mmHg, surgery The disinfection time of the former device was more than 10 h and the proportion of diabetic patients was higher than that of the non-infected group. The difference was statistically significant (P<0.05). There was no significant difference in the incidence and gender of the two groups (P>0.05). Multivariate regression analysis: Logistic regression analysis showed that age >60 years, duration >120 min, intra-renal pressure >30 mm Hg, preoperative instrument disinfection time <10 h, and patients with upper urinary calculi with diabetes Risk factors for nosocomial infection after ureteroscopy and holmium laser lithotripsy (P<0.05). Conclusions: Diabetes mellitus, duration of surgery >120 min, age >60 years, intrarenal pressure >30 mmHg, preoperative disinfection time <10 h, preoperative urinary tract infection are risk factors for nosocomial infection after ureteroscope holmium laser lithotripsy .

820 Analysis on Correlation between HBeAg and HBV-DNA Levels in Patients with Hepatitis B

Objective: To analyze the correlation between HBeAg and HBV-DNA levels in patients with hepatitis B. Methods: A totol of 586 HBsAg positive serum samples were selected. The levels of the HBeAg and HBV-DNA were measured by the ELISA and fluorometric PCR, respectively. The relationship between the markers were analyzed. Results: Among the 586 patients, the detection rate of HBV-DNA in HBeAg positive group and negative group was 86.19 % and 29.46 % respectively, which showed there was significant difference (χ2=194.46, P<0.05) between the HBeAg positive group and negative group. The content of HBV-DNA in the HBeAg positive group was higher than in the HBeAg negative group. With the increase of the HBeAg content, the amount of HBV-DNA in the serum also increased(F=6.67, P<0.05). Conclusion: The HBeAg level is positive correlation with the HBV-DNA load. Simultaneous detection of HBeAg and HBV-DNA would be clinical most desirable.

823 Analysis of Curative Effect and Safety of Olanzapine Tablets combined with Chlorpromizide in the treatment of Refractory Obsessive-compulsive Disorder

Objective: To analyze the efficacy and safety of olanzapine tablets combined with clomipramine in the treatment of refractory obsessive-compulsive disorder. Methods: 176 patients with refractory obsessive-compulsive disorder who treated from January 2014 to January 2018 in our hospital, according to simple random method those patients were divided into control group (n=81) and observation group (n=95) , the control group was treated with chlorpromizide, and the observation group was treated with olanzapine tablets on the basis of the control group. Then clinical efficacy, Yale-Brown Forced Scale (YBOCS), Hamilton Anxiety Scale (HAMA), and Hamilton Depression Scale (HAMD) scores, quality of life scores, blood routine, liver and kidney function at different time points before and after treatment, and adverse reactions in both group were compared. Results: The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Before treatment, YBOCS, HAMA and HAMD scores, quality of life scores, blood routine and liver and kidney functions in both group were no statistically different (P>0.05). There were significant differences in YBOCS, HAMA, HAMD and quality of life scores between the two groups in different treatment methods and treatment time (P<0.05). Blood routine and liver and kidney functions in the groups were no significantly changed, there were no statistically significant differences (P>0.05). the incidence of adverse reactions in th both groups was no statistical difference (P>0.05). Conclusion: Olanzapine combined with clomipramine has synergistic effect on the treatment of refractory obsessive-compulsive disorder, which can effectively improve the symptoms of patients, improve the quality of life, and tolerate side effects.

828 Comparative Study on Percutaneous Transforaminal Nucleus Pulposus and Small Incision Interlaminar Nucleus Pulposus Extraction for Lumbar Disc Herniation

Objective: To compare the clinical value of percutaneous transforaminal nucleus pulposus and small incision interlaminar nucleus pulposus removal for lumbar disc herniation (LDH). Methods: 60 patients with LDH in our hospital from November 2016 to February 2018 were enrolled. Percutaneous transforaminal nucleus pulposus removal was divided into intervertebral foramen. The nucleus pulposus extraction was divided into small incision groups, 30 cases in each group. The operation and postoperative complications were compared between the two groups. The follow-up to 6 months after surgery, the efficacy and functional improvement of the two groups were compared [Oswestry dysfunction index (ODI), Japanese orthopedic surgery score (JOA)] and quality of life (SF-36) score were counted. Results: Surgery and postoperative recovery: The length of the incision in the intervertebral foramen group, the time of surgery, the time of bed rest and the length of hospital stay were shorter than that of the small incision group. The amount of surgical bleeding and discectomy was less than that of the small incision group (P<0.05). Postoperative complications: The incidence of postoperative complications in the vertebral foramen group was 6.67% (2/30), which was lower than that in the small incision group (26.67% (8/30) (P<0.05); Surgical efficacy: The excellent and good rate of surgery in the intervertebral foramen group was 86.67% (26/30), and there was no significant difference between the small incision group and 83.33% (25/30) (P>0.05). Functional improvement: postoperative 6 The ODI index and JOA score of the intervertebral foramen and small incision group were improved compared with those before operation (P<0.05), but there was no significant difference between the two groups (P>0.05). Quality of life: postoperative At 6 months, the SF-36 scores of the intervertebral foramen group and the small incision group were higher than those before the operation, and the intervertebral foramen group was higher than the small incision group (P<0.05). Conclusion: Percutaneous transforaminal nucleus pulposus and small incision interlaminar nucleus pulposus extraction for LDH can achieve significant surgical results, improve patient dysfunction and improve lumbar function, but the former is more than the latter. The advantages of minimally invasive surgery, small trauma and quick recovery after surgery are more conducive to improving the quality of life of patients after surgery.

834 Clinical Study on the Therapeutic Effect of Early Rehabilitation Functional Exercise in the Treatment of Senile Heart Failure Complicated with Diabetes Mellitus

Objective:To investigate the therapeutic effect of early rehabilitation functional exercise on senile heart failure complicated with diabetes. Methods:148 patients with senile heart failure and diabetes admitted in our hospital between March 2017 and June 1818 were randomly divided into the control group (72 cases, conventional nursing) and the study group (76 cases, conventional nursing plus early rehabilitation functional exercise). Before and after treatment, the left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDd), NYHA classification, 6-min walk test (6MWT), Minnesota Minnesota Living With Heart Failure (MLHF) and changes in blood glucose and blood lipids were compared between the two groups. Results: After treatment, LVEF, LVEDd, NYHA II and III were significantly better than those before treatment (P<0.05), and LVEF, LVEDd, NYHAII and III were significantly better in the study group than in the control group (p<0.05). 6MWD and MLHF scores in the two groups were significantly better than those before treatment (P<0.05), and the 6MWD and MLHF scores in the study group were significantly better than those in the control group. The blood glucose and blood lipids in the two groups were significantly lower after treatment than before treatment (P<0.05); the blood glucose and blood lipids in the study group were significantly lower than those in the control group (P<0.05). Conclusions:Heart failure in elderly patients with diabetes mellitus and early rehabilitation training and blood sugar control drug therapy can significantly improve cardiac function and exercise ability, improve the quality of life of patients, and be conducive to prognosis.

838 Study on the Correlation of TCM Syndrome Differentiation of Acute Exacerbation of COPD with Lung Function, Blood Gas Analysis and Dyspnea Scale

Objective:To study the correlation of TCM syndrome differentiation with lung function, blood gas analysis and dyspnea scale in patients with acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods:A total of 120 patients with acute exacerbation of COPD who were admitted to the hospital from October 2017 to August 2018 were selected as study subjects. According to common TCM syndrome differentiation, the patients were divided into wind-cold attacking lung type (n=30), hot-heat obstructing lung type (n=30), phlegm-dampness obstructing lung type (n=30) and water attacking heart type (n= 30). The lung function [forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1)], blood gas indexes [partial pressure of blood oxygen (PO2), partial pressure of carbon dioxide (PCO2)]and dyspnea scale (MMRC) scores were determined and their correlation with TCM syndrome differentiation was analyzed. Results:The FEV1, FEVl/FVC and PO2 of wind-cold attacking lung type were significantly higher than those of the other three types while PCO2 and MMRC score were lower than those of the other three types (P<0.05). Conclusion: The pulmonary function, blood gas analysis and dyspnea manifestations of patients with acute exacerbation of COPD of different syndrome types are different, which can be used as an auxiliary index for the dialectical typing of TCM.

841 Clinical Value of Combined Detection of Serum Erythrocyte Distribution width Uric Acid and Brain Natriuretic Peptide in the Diagnosis of Chronic Heart Failure

Objective: To explore the diagnostic value of combined detection of serum erythrocyte distribution width (RDW), uric acid (UA) and brain natriuretic peptide (BNP) levels in patients with chronic heart failure(CHF).. Methods: Eighty patients with CHF admitted to the Department of Cardiology, Affiliated Hospital of Chengde Medical College from August 2017 to June 2018 were selected as CHF group.Seventy-five patients with normal cardiac function were selected as control group.Peripheral blood samples were collected from patients.The baseline database was established.. Results:Compared with the control group, the levels of serum RDW, UA and BNP in CHF group were higher and positively correlated with the grading of cardiac function. With the increase of the grading of cardiac function, the levels of serum RDW, UA and BNP gradually increased, with statistical significance. ROC curve analysis showed that the area under the curve of serum RDW, UA and BNP was 0.744, 0.761 and 0.984, respectively. The area under the curve of combined detection of serum RDW, UA and BNP was 0.985, with statistical significance.. Conclusion: Combined detection of serum levels of RDW, UA and BNP can help clinicians better diagnose CHF in early stage, comprehensively evaluate the risk stratification of patients' condition, and provide objective and comprehensive diagnostic value.

844 Effects of Individualized Diet Cntrol and Kuangqi-sijunzi Decoction on Glucose Lipid, RBP4 levels and Maternal and Infant Complications in Pregnant Women with Gestational Diabetes Mellitus

Objective: To analyze the effects of individualized diet control combined with huangqi-sijunzi decoction on the levels of glycolipid, retinol binding protein 4 (RBP4) and maternal and infant complications in pregnant women with gestational diabetes mellitus. Methods: 60 cases of gestational diabetes mellitus patients in our hospital from August 2016 to August 2018 were divided into two groups according to simple random grouping method. The control group was given individualized diet control, and the combined group was given individualized diet control combined with huangqi-sijunzi decoction, with 30 cases in each group. The changes of blood glucose, blood lipid, RBP4 levels before and after treatment and the occurrence of complications of puerpera and infant were compared between the two groups. Results: One month after treatment, the levels of fasting blood glucose (FBG), 2-hour postprandial blood glucose (2hPBG), triglyceride (TG), total cholesterol (TC) and low-density lipoprotein-cholesterol (LDL-C) in the two groups were significantly decreased compared with those before treatment, and the levels of high-density lipoprotein-cholesterol (HDL-C) were significantly increased compared with those before treatment in the combined group. After treatment, the levels of FBG, 2h PBBG, TG, TC and LDL-C were significantly decreased compared with those of the control group, and the level of HDL-C was significantly increased compared with the control group (P<0.05); compared with before treatment, the levels of RBP4 in the combined group were significantly lower after treatment and significantly lower than those in the control group (P<0.01), while there was no significant difference in the control group between and after treatment (P>0.05). The incidence of fetal growth restriction, intrauterine infection, intrauterine distress, amniotic fluid infection, cesarean section rate and macrosomia delivery rate in the combined group were significantly lower than those in the control group (P<0.05), but there was no significant difference in the incidence of postpartum hemorrhage, premature delivery and neonatal hypoglycemia between the two groups (P>0.05). Conclusion: Individualized diet control combined with huangqi-sijunzi decoction can effectively improve the levels of blood sugar, blood lipid and RBP4 in patients with gestational diabetes mellitus, reduce the occurrence of perinatal complications, and have important clinical significance in improving maternal and infant outcomes.

848 Studies on the Correlation between islet Function Serum Interleukin-6 and Hypersensitive C-reactive Protein in Patients with Sleep Disorders with Different Glucose Tolerances

Objective: To observe the changes of IL-6 and hsCRP levels and pancreatic islet function in patients with different glucose tolerance , sleep disorders, to explore the correlation between sleep disorders , hyperinsulinemia, insulin resistance and inflammatory responses. Methods: 108 patients with sleep disorders who had no previous history of diabetes were selected. All patients underwent OGTT and insulin release test, tested hsCRP, IL-6 and blood lipids, assessed their sleep status in parallel with the international Pittsburgh sleep quality scale (PSQI).The results of OGTT were divided into three groups: NGT, IGR and DM. Results:The median PSQI level of NGT group, IGR group and DM group was significantly higher than that of the control group (P < 0.05). The median levels of FPG, 2h PG and HbA1c in DM group were higher than those in NGT group, IGR group and control group (all P <0.05).The median FPG level of IGR group was higher than that of control group (P<0.05).The median PG level of IGR group at 2 h was higher than that of NGT group and control group (all P <0.05).The median FINS level of DM group was higher than that of control group (P<0.05).The median levels of 2 hours insulin and HOMA-IR (homeostasis model assessment of insulin resistance) in DM group were higher than those in NGT group and control group (P<0.05).The median level of 2hIns in IGR group was higher than that in NGT group and control group (P<0.05). The median TG level of DM group was higher than that of NGT group, IGR group and control group (all P <0.05);In the process from NGT to IGR and DM, the median levels of hsCRP and IL-6 gradually increased, and the median levels of IL-6 in the NGT group, IGR group and DM group were all higher than those in the control group (P< 0.05).The median hsCRP level of IGR group and DM group was higher than that of NGT group and control group (P< 0.05). Spearman correlation analysis showed that PSQI total score was positively correlated with IL-6, hsCRP, FPG, 2h PG, HbA1c, FINS, 2 hIns, HOMA-IR and waist circumference (P<0.05). Logistic correlation analysis showed that sleep disorders were positively correlated with 2hPG [OR=1.277, 95.0% confidence interval (1.046,1.558)] and hsCRP [OR=3.203, 95.0% confidence interval (1.028,9.980)].. Conclusion: Sleep disorders are associated with hyperinsulinemia, insulin resistance, glucose metabolism disorder, and inflammatory response. Improving sleep disorders may improve pancreatic islet function and reduce inflammatory response, thus reducing the risk of impaired glucose regulation or diabetes.

853 Effect of 6-minute Walk Test Training on Anxiety and Depression in Patients with Chronic Heart Failure Outside Hospital

Objective: To explore the effect of 6-minute walk test training on anxiety and depression in patients with chronic heart failure (CHF) outside hospital. Methods: 60 patients with CHF were divided into two groups randomly: the control group (n = 30) and the experimental group (n = 30). The control group was given routine drug therapy after discharge. In addition to routine drug therapy, the experimental group also increased the 6 minute walk test instruction and training. The self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were used to evaluate the anxiety and depression of the two groups at discharge and 6 months after training. Results: There was no significant difference in anxiety and depression scores between the two groups at discharge. After 6-months of intervention, the anxiety and depression scores of the control group were significantly higher than those of the experimental group. Conclusions: 6-minute walk test can significantly alleviate anxiety and depression in patients with chronic heart failure outside hospital.

862 Effect of Individualized Continuing Nursing on Caregiver Burden and Quality of Life of Traumatic Spinal Cord Injury patients with Paraplegia

Objectives: To explore the effect on the primary caretaker of patients with paraplegia due to traumatic spinal cord injury by individual continued-nursing care. Methods: The 86 patients with paraplegia due to traumatic spinal cord injury and the primary caretaker were selected and divided into two equal groups, treatment and control groups, randomly. The trial group was received intervening by individual continued-nursing care and the control group was intervened by routine nursing care. The ZBI and SF-36 scales were used to evaluate the interventions. Results: There were no significant statistical difference between two groups in physical data, caring burden and quality of life scores before intervening (p>0.05). The caring-burden of trial group was lower than control group after intervening and meanwhile the quality of life was greater than control group. There was significant statistical difference in comparison of the above items between two groups (p<0.01). Conclusions: The caring-burden can be reduced and the quality of life can be improved by individual continued-nursing care of project for the primary caretaker of patients with paraplegia due to traumatic spinal cord injury.

866 Analysis of oprD Gene in Imipenem-resistance Clinical Isolates of Pseudomonas Aeruginosa in Dongguan

Objective: To analyze the mechanism of imipenem resistance in Pseudomonas aeruginosa clinical isolates. Methods: 110 imipenem-resistance Pseudomonas aeruginosa clinical isolates were collected from 2 hospitals of Dongguan between 2016 and 2017, and antibiotic resistance was analyzed using VITEK2 system. PCR was performed to amplify gene oprD. The amplified products were subject to sequencing analysis. Results: 110 strains of bacteria were identified as drug-resistant strains of imipenem, of which 3 were negative for PCR amplification of oprD gene. The oprD gene sequencing of the remaining 107 positive strains showed that 7 of them had no mutation, 84 had frameshift mutations(Eight of them were found to carry insertion sequences ISPpu21, IS1394, ISPpu29 and ISPst2), and 16 with a premature stop codon which is a new termination codon in advance of a position causes its peptide chain abnormalities. The rate of the mutation was 90.91%. Conclusions: oprD gene mutations result amino change and/or frame shift,hampering the binding of imipenem and oprD was the main imipenem resistance mechanism in Pseudomonas aeruginosa in Dongguan, Guangdong Province.

871 Clinical Observation of Acupuncture and Moxibustion Combined with Chinese Medicine in the Treatment of Nervous Tinnitus

Objective: To observe and analyze the clinical efficacy of acupuncture and moxibustion combined with traditional Chinese medicine in the treatment of nervous tinnitus. Methods: A total of 120 patients with neurogenic tinnitus (TCM syndrome differentiation is internal obstruction of phlegm and blood stasis) in the traditional Chinese medicine outpatient department of the affiliated hospital of chengde medical college were randomly divided into control group and observation group according to the order of visit. The control group was treated with oral mecobalamine tablets combined with ginkgo biloba leaves, while the observation group was treated with traditional Chinese medicine comprehensive therapy (acupuncture combined with oral Chinese medicine decoction), and the clinical therapeutic effects of the two groups were compared. Results: The effective rate of the observation group was 86.7%, which was significantly higher than that of the control group (66.7%), and the difference was statistically significant (P<0.01). The severity of tinnitus was significantly improved in both groups after treatment, while the improvement in the observation group was better than that in the control group, and the difference was significant (P<0.01). After treatment, the sf-36 quality of life scale score of both the observation group and the control group was significantly increased, and the increase rate of the sf-36 quality of life scale of the observation group was more significant, with a significant difference compared with the control group (P<0.05). After treatment, the total scores of Hamilton anxiety scale in both groups were reduced, and the degree of reduction was more significant in the observation group and the control group, with statistically significant difference (P<0.05). Conclusion: Acupuncture and moxibustion combined with traditional Chinese medicine in the treatment of neurogenic tinnitus has significant efficacy, can improve the quality of life of patients, reduce anxiety symptoms, clinical application value.

875 Effect of "Transport Hub" Massage Based on Meridian and Acupoint Theory on Clinical Curative Effect and Growth and Development of Children with Anorexia

Objective: To analyze the effect of massage on the clinical curative effect and growth and development of anorexia based on the theory of meridian and acupoint, and to accumulate evidence-based experience for the quality care of this kind of group. Methods: From April 2016 to March 2017, 53 cases of anorexia admitted in our hospital were treated as the control group, and the chiropractic therapy was given. From April 2017 to March 2018, 56 cases of anorexia in our hospital were treated as the study group, and the "transport hub" massage was carried out on the basis of the control group intervention program. The scores of TCM syndrome, clinical efficacy and growth and development indexes of the two groups before and after intervention were compared. Results: The TCM syndrome score of the study group was lower than that of the control group, and the clinical curative effect of the children was higher than that of the control group. The indexes of body mass, height, head circumference and chest circumference in the dry prognosis were higher than those of the control group. Conclusion: Massage based on meridian and acupoint theory can improve the clinical efficacy and promote the growth and development of children with anorexia.
