177 Clinical Study of Elderly Head and Neck Cancer line CT Plan Dose Distribution in Two-way Influence on Radiation

Objective: To analyze the elderly head and neck cancer underwent CT treatment plans for two-way influence the dose distribution of radiation, in order to provide reference for the clinical. Methods: From March 2015 to March 2016, the elderly patients with head and neck cancer radiotherapy, were diagnosed by pathology for head and neck cancer, IMRT, a total of 12 patients were collected: 8 cases of nasopharyngeal carcinoma, 2 cases of tonsil carcinoma, 1 case of laryngeal carcinoma, 1 case of tongue cancer, among them 11 cases of male patients and 1 female patient, a comparison process and after radiotherapy of the following indicators: target and organ at risk, parotid gland volume, parotid mass and body midline distance. Results: A two-way process and after radiotherapy, the left and right sides of the parotid gland volume were reduced after treatment (P<0.05), left and right sides of parotid mass and body midline distance were reduced (P<0.05); GTV/PTV D2, Cheng D50, D95, D98, D100 is less than a ride, D95, D98, GTV in the second D100, PTV D50, D95, D98, D100, HI, CI is a low (P<0.05); two-way in bilateral parotid V26, mean dose and right parotid gland V26, the average dose was increased (P <0.05). Conclusion: The elderly head and neck cancer CT plan two-way radiation dose, V26, bilateral parotid mean dose of PTV and GTV, D95, D98 and D100 and PTV doses were significantly different. Clinical attention should be paid to the anatomical structure of patients with large differences, should be at the right time to modify the treatment plan to enhance the treatment effect of patients.

180 Atorvastatin Combined with Trimetazidine on the Expression Level of Caspase-1 and IL-18 of patients with CHD

Objective: To investigate the effects of atorvastatin combined with trimetazidine for the treatment of patients after the in vivo Caspase-1 and IL-18 expression changes. Methods: 100 cases of patients with coronary heart disease (CHD) admitted in our hospital from July 2013 to July 2015 were randomly divided into observation group and control group; the control group was treated with trimetazidine treatment, observation group were treated with atorvastatin combined with trimetazidine treatment sibutramine; patients after the treatment, assessment, intervention at the end of the observation before and after treatment in patients with blood rheology blood lipid levels, learning condition, and the levels of Caspase-1 and IL-18 in serum. Results: The effective rate of the observation group was significantly higher than the control group, and the difference has statistical significance (P<0.05); After treatment, The TC, TG, HDL-C and LDL-C levels of Patients were significantly decreased (P<0.05), and the observation group was significantly lower than that of control group (P<0.05); red blood cell hematocrit and plasma viscosity the whole blood viscosity and low shear rate and high shear rate of whole blood viscosity patients was significantly reduced (P<0.05), and the observation group was significantly lower than that of control group (P<0.05);The Caspase-1 levels of patients two groups were significantly decreased (P<0.05), and the observation group was significantly lower than that of control group (P<0.05); IL-18 in two groups significantly decreased (P<0.05), and the observation group was significantly lower than that of control group (P<0.05). Conclusion: The trimetazidine and atorvastatin on the prognosis of patients with CHD can improve the blood lipid and blood rheology in patients with learning condition, reduce the levels of Caspase-1 and lower the level of IL-18, it has high value in clinical application.

184 Study on the Changes of Tear Crystalline Lens and Aqueous Oxidative Stress State Indicators of Patients with Cataract

Objective: To investigate and study the change situation of tear, crystalline lens and aqueous humor oxidative stress state indexes of patients with cataract. Methods: 84 patients with cataract, From February 2012 to September 2014, were the observation group, and 84 patients with eye trauma treated in a short time at the same period were the control group. Then the tear, crystalline lens and aqueous humor oxidative stress state related indexes of two groups were compared. And the tear, crystalline lens and aqueous humor related indexes of observation group with cataract of incipient stage, immature stage, maturation stage and overripe stage were compared. Results: The expression of tear, crystalline lens and aqueous humor oxidative stress state related indexes of observation group were all worse than those of control group,and the expression of tear, crystalline lens and aqueous humor related indexes of observation group with cataract of incipient stage, immature stage, maturation stage and overripe stage had significant differences, P<0.05, all the items all had significant differences. Conclusion: The expression differences of tear, crystalline lens and aqueous oxidative stress state indexes of patients with cataract are obvious, and the influence of cataract stages for the indexes are great.

187 The Value of Inflammation Based Index in Hepatocellular Carcinoma Prognosistic Patient

Objective: To investigate the usefulness of the inflammation based index (IBI) as prognostic factors in patients with hepatocellular carcinoma (HCC). Methods: A total of 119 HCC patients admitted in our hospital from July 2011 to July 2014 were enrolled in the study. The HCC patients were assigned according to the grade (0, 1, 2) of IBI. The correlations between IBI and clinic-pathologic features were detected in the study. The prognostic significance of IBC and barcelona clinic liver cancer (BCLC) were analyzed by univariate survival analysis. Results: The AFP level in human serum (P<0.05) and inntrahepatic metastasis (P<0.01) significantly increased in the HCC patients of the high IBI group compared with 0 grade IBI group. Child-Pugh class and BCLC had a significant difference between 0, 1, 2 IBI groups (P<0.01). The IBI grade negatively correlated with tumor-free survival time. Conclusion: The IBI score is consistent with the clinicopathological features and BCLC staging of HCC, which could be used as an effective indicator for the prognosis of HCC patients.

190 Expression and Significance of CK7 CK20 CA125 and TTF-1 in Liquid Based Cytology Adenocarcinoma Cells

Objective: To study the expression of CK7, CK20, A125 and TTF-1 in liquid based cytology adenocarcinoma cells, and analyze its clinical significance. Methods: 137 cases serous effusion specimens of adenocarcinoma diagnosed in our hospital from February 2015 to March 2016 were selected. Using the liquid based cytology production technology and immunocytochemical staining method, the different tissue derived adenocarcinoma cells CK7, CK20, CA125 and TTF-1 antibodies were observed. Results: The positive rate of CK7 100% in lung adenocarcinoma, most ovarian serous carcinoma express, intestinal adenocarcinoma individual positive; CK20 expression in intestinal adenocarcinoma positive rate was 88.89% (32/36),There's no expression in lung adenocarcinoma; CA125 expression in ovarian serous adenocarcinoma rate was 70.45% (31/44), in lung adenocarcinoma and expression in ovarian serous carcinoma was less; The vera TTF-1 expression in lung adenocarcinoma positive rate was 80.70% (46/57), in ovarian serous carcinoma, and no expression in intestinal adenocarcinoma, differences were statistically significant (all P < 0.05). Poorly differentiated lung adenocarcinoma express the vera.TTF-1 positive rate was significantly lower than the high differentiation, differences were statistically significant (all P < 0.05). Poorly differentiated CA125 expression in ovarian serous carcinoma positive rate was significantly lower than the high differentiation, the difference was statistically significant (P < 0.05). Poorly differentiated CK20 expression in intestinal adenocarcinoma were significantly lower than the high differentiation, the difference was statistically significant (P < 0.05).Conclusion: The expression of CK7, CK20, CA125 and TTF-1 in the liquid based cytology adenocarcinoma cells is beneficial to identify the serous cavity effusion from lung, ovary and intestinal adenocarcinoma.

193 The Application of Dual Source Computed Tomography for Gout Diagnosis

Objective: In order to analyze the clinical applications of DSCT technique in the diagnosis of gout, the adoption of "dual source CT (dual source computed tomography, DSCT) dual energy imaging technology to detect the urate crystal deposition condition of patients with gout joints. Methods: Choosing 48 patients with clinical diagnosis of gout as the experimental group, 30 patients with other arthritis as control group (11 patients with rheumatoid arthritis, 16 patients with osteoarthritis and 3 cases of systemic lupus with lupus arthritis), it used DSCT to scan double hand-wrist joints, double knee joints and double ankle joints of patients from both groups. Using the instrumental configuration of gout recognition software to process the original images, analyzed by two experienced clinical image physicians on the data obtained, it was decided whether there's any urate crystal deposition within the checked parts of patients. Results: In the DSCT examination of 48 patients with gout, 46 cases were found in the joints with urate crystal deposition and the gout diagnostic sensitivity was 46/48 (95.8%). As for the 30 patients of the control group, there're 2 cases with pseudo signal and the specificity was 28/30 (93.3%). There's no obvious difference (P≥0.05) of the positive rate of urate crystals between gout patients who have bone destruction and those who do not have. It's found that the difference of detection rate of DSCT urate crystals has no statistical significance (P>0.05) between gout patients who have increased blood uric acid levels and normal cases. Conclusion: It has high sensitivity and specificity when using DSCT double energy imaging in the diagnosis of gout, and the urate crystals deposition condition of patients’ checked parts could be displayed intuitively with this noninvasive test. It prompts that DSCT can be applied to gout disease monitoring and has the important clinical application value.

197 Efficacy Comparison betwwen Locking Compression Plate Fixation from Volar Approach and Non-operation in Treatment of Distal Fracture of Radius in Elderly Patients

Objective: To study the clinical efficacy of locking compression plate fixation from volar approach vs. non-operation in treatment of distal fracture of radius in the old. Methods: Totally 71 old patients with distal fracture of radius were selected and divided into observation group (37 patients) and control group (34 patients) according to the wills of the patients. The observation group was treated with locking compression plate fixation from volar approach, while the control group treated with manipulative reduction and plaster external fixation. The conditions of reduction and healing of the fracture were recorded, and the wrist joint functions evaluated by Cooney wrist joint scale. Results: The mean fracture healing times of the observation group and control group were (12.1±2.4) weeks and (14.8±3.2) weeks, respectively, and the difference was statistically significant (P<0.05). Before treatment, the two groups had insignificantly different palmar tilts, ulnar inclinations, and radial shortening lengths (P>0.05); at 6 months after treatment, both groups had significantly improved palmar tilts, ulnar inclinations, and radial shortening lengths compared with those before treatment, but the observation group had significantly greater improving extents (P<0.05); the observation group had significantly better Cooney wrist joint scores than the control group, including pain, range of motion, hand strength, inflexion/expansion activity and function (P<0.05); and the observation group had a significantly higher excellent rate of wrist joint function than the control group (94.6% vs. 76.5%, P<0.05). The two groups had insignificantly different incidences of postoperative complications (P>0.05). Conclusion: Locking compression plate fixation from volar approach in treatment of distal fracture of radius in the old has advantages of rapider fracture healing and better wrist joint function recovery. Volar locking compression plate fixation in the treatment of elderly patients with distal radius fracture has healing fast and wrist joint function recovery advantages.

200 Sublingual Dust Mite Drops Combined with Traditional Chinese Soup Flavored Yupingfeng in Treatment of Allergic Rhinitis

Objective: To analyze clinical efficacy of dust mites sublingual drops with traditional Chinese medicine soup flavored Yupingfeng in treatment of allergic rhinitis. Methods: 60 patients with allergic rhinitis were divided into treatment group and control group randomly. The control group was treated with sublingual dust mite drops and the treatment group was treated with traditional Chinese medicine soup flavored Yupingfeng on the basis of the control group. The clinical effects, clinical symptom score, TNSS, TMS scores and adverse events before and after treatment of the two groups were compared. Results: The total effective rate of treatment group (93.33%) compared with 76.67% in the control group was significantly improved, and the effective rate was 73.33% of the patients treated, and the effective rate in the control group was only 43.34%. After 6 months, 1 year, 1 year and a half, patients treated sneezing, runny nose, nasal congestion, nasal itching, TNSS and TMS points was lower than the control group, the differences were statistically significant (P <0.05), adverse reactions in patients treated was statistical significant (P<0.05). Conclusion: Sublingual dust mite drops combined with traditional Chinese medicine treatment of allergic soup flavored Yupingfeng for rhinitis can significantly improve the clinical efficacy and improve clinical symptoms, and do not increase the allergic incidence of adverse reactions, with high security. It is worth to popularize in clinical practice.

204 TE and TEC Neoadjuvant Chemotherapy on CTCs and TSCs in Peripheral Blood of Patients with Breast Cancer

Objective: To analyze the effect of TE and TEC neoadjuvant chemotherapy on CTCs and TSCs in peripheral blood of patients with breast cancer. Methods: 155 cases of patients with breast cancer were selected from January 2015 to June 2016 in our hospital. The patients were divided into control group (77 cases) and observation group (78 cases) according to the random number table method. The patients in control group were treated with TE regimen, the observation group received TEC regimen, the curative effect, adverse reaction, CTCs and TSCs level in the two groups were observed. Results: Compared with the control group, the effective rate of the observation group was 89.7%, the difference was not significant (χ2=0.932, P>0.05), the neutrophils in the observation group were significantly more, the difference was significant (χ2=8.923,P<0.05), but there was no significant difference in gastrointestinal tract reaction, cardiotoxicity, liver function abnormality, thrombocytopenia, alopecia and serum albumin (P>0.05). Compared with the control group, there was no significant difference in CTCs level between the observation group and the control group (χ2=1.293, 1.441, 0.992, P>0.05). There was no significant difference in TSCs between the two groups (χ2=1.903, 1.783, P>0.05), but the level of TSCs in stage Ⅲ was significantly lower than that in stage Ⅲ (χ2= 7.543, P<0.05). Conclusion: The TE and TEC neoadjuvant chemotherapy for breast cancer patients with similar clinical efficacy, TEC program to reduce stage Ⅲ breast cancer patients with peripheral blood TSCs may be better than the TE program.

208 Short and Long-term Clinical Efficacy of Montelukast Sodium in the Treatment of Cough Variant Asthma and the Effect on Serum Inflammatory Factors in Children

Objective: To investigate the short and long- term clinical efficacy of montelukast sodium in the treatment of cough variant asthma and the effects on serum levels of tumor necrosis factor -α (TNF-α), interleukin-8 (IL-8), interleukin-10 (IL-10), and immune globulin E (IgE) in children. Methods: Totally 80 eligible children were selected, and randomly divided into observation group and control group, with 40 children in each group. Both groups were given conventional therapies, and beclometasone dipropionate inhalation aerosol was used additionally to the control group, while montelukast sodium chewable tablets was used additionally to the observation group, both used successively for 3 months. The clinical efficacy was assessed, and the serum levels of TNF-α, IL-8, IL-10, and IgE were determined before and after treatment. Results: In the observation group, 21 children were clinically controlled, of which 9 were responsive to the therapy excellently, 7 were responsive to the therapy, and 3 were unresponsive to the therapy,so the overall response rate was 75.00%; while in the control group, 13 children were clinically controlled, of which 8 were responsive to the therapy excellently, 13 were responsive to the therapy, and 6 were unresponsive to the therapy, so the overall response rate was 52.50%. The differences were statistically significant (P<0.05). The disappearance time of common symptoms such as cough, angina, and itching throat in the observation group were significantly shorter than those in the control group (P<0.05). Before the treatment, the serum level of TNF-α, IL-8, IL-10, and IgE were similar in two groups, the difference was statistically insignificant (P >0.05); After treatment, the serum level of TNF-α, IL-8, IL-10, and IgE were significantly improved in both groups compared with those before treatment, while the improving extents in the observation group were greater than those in the control group, the difference was statistically significant (P<0.05). 1 year after the treatment, totally 7 children of the observation group experienced relapse and the relapse rate was 17.50%; while totally 14 children experienced relapse and the relapse rate was 35.00%; and the difference was statistically significant (P<0.05). Conclusion: In the treatment of cough variant asthma in children, Montelukast sodium has good short-term clinical efficacy, and can shorten the symptomatic improvement time, lighten the inflammatory reactions, and reduce the long-term relapse rate.

212 The New Root Canal Filling Medium in Treatment of Root Canal Disease and the Effect on the Level of CypA and RANKL

Objective: To analyze the clinical efficacy of the new root canal filling medium in the treatment of root canal disease and the effect on the level of CypA and RANKL in the lesion area. Methods: 120 cases of pulpitis and periapical periodontitis in our hospital from January 2013 to January 2016 were randomly divided into observation group and control group, with 60 cases in each group, the patients in observation group were treated with gutta percha and epoxy paste intervention, while the patients in control group were treated by using gutta percha and calcium hydroxide paste filling; 8 weeks after the treatment, the clinical efficacy and pain of patients were evaluated; during the treatment and 8 weeks after the treatment, the periodontal tissue was taken to make biopsy samples, the level of CypA and RANKL in tissue sections were examined by using immunohistochemical staining method. Results: The clinical efficacy of patients in the observation group was significantly higher than that in the control group, and the difference was statistically significant (P<0.05); after the treatment, the ratio of the number of the patients with severe pain in the observation group was significantly lower than that in the control group (P<0.05), the ratio of the number of the patients with no pain was significantly higher than that in the control group (P<0.05); after the treatment, the level of CypA and RANKL in the two groups were all significantly lower (P<0.05), after the treatment, the level of CypA and RANKL in the observation group were significantly lower than those in the control group, and the difference was statistically significant (P<0.05).Conclusion: The new root canal filling medium can significantly improve the clinical efficacy of the treatment of root canal disease, and can reduce the level of CypA and RANKL in the lesion area, which is conducive to the rehabilitation of patients.

215 Influence of Laparoscopic Minimally Invasive Surgery on the Body Trauma and Ovarian Function in Patients with Benign Ovarian Tumor

Objective: To study the influence of laparoscopic minimally invasive surgery on trauma and ovarian function in patients with benign ovarian tumors. Methods: From October 2010 to October 2016, 78 cases with benign ovarian tumors in our hospital were numbered according to the order of treatment, and they were randomly divided into the observation group and the control group by the random number table method, 39 cases in each group. The observation group was treated with laparoscopic minimally invasive surgery, while the control group was treated with open surgery. The levels of perioperative related indicators and changes of ovarian function indexes [estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH)] were compared between the two groups. Results: The intraoperative blood loss and time of postoperative exhaust recovery of the observation group were significantly less and shorter than those in the control group, and the postoperative analgesia rate was significantly lower than that in the control group (23.08% vs 82.05%) (P < 0.05). There were no significant changes in the levels of E2, FSH and LH in the two groups before and after treatment (P>0.05). Conclusion: Laparoscopic minimally invasive surgery will cause small trauma in the treatment of patients with ovarian teratoma. The effect on ovarian function is relatively small, and it can better protect the immune function in patients. It is worthy of clinical application. October

218 Correlation Analysis on EGFR c-Met and Ki-67 in Gastric Cancer with Intravascular Cancer Embolus

Objective: To analyze the expression of EGFR, c-Met and Ki-67 in gastric cancer with intravascular cancer embolus and their influence on the prognosis. Methods: The expression of EGFR, c-Met and Ki-67 in 88 gastric cancer cases and 39 cases of intravascular cancer embolus were detected and the difference between 1-year and 3-year survival rate in cancer embolus positive and negative patients group were analyzed. Results: In gastric cancer tissue with intravascular cancer embolus, the strong positive expression rates of EGFR, c-Met and Ki-67 were all higher than those in gastric cancer tissue without intravascular cancer embolus (P<0.05). The strong positive expression rates of EGFR, c-Met and Ki-67 in gastric cancer tissue without intravascular cancer embolus 1-year and 3-year survival rate were obviously higher than those of cancer embolus positive group (P<0.05). Conclusions: The expression and prognosis of EGFR, c-Met and Ki-67 are related to the gastric cancer with intravascular cancer embolus, and has rather poor prognostic significance in gastric cancer.

221 Effects of Laparoscopic and Open Cholecystectomy on the Immune Function and Sserum Inflammatory Factors in Patients with Cholecystolithiasis

Objective: To investigate the effects of laparoscopic and open cholecystectomy on the immune function and serum inflammatory factors in patients with cholecystolithiasis. Methods: Totally 80 eligible patients were selected and randomly divided into observation group and control group, with 40 patients in each group. The observation group used laparoscopic cholecystectomy, while the control group used open cholecystectomy, and the levels of immune globulin A (IgA), immune globulin M (IgM), immune globulin G (IgG), T lymphocyte subpopulations CD3+, CD4+, and CD8+, hypersensitive C reactive protein (hs-CRP), interleukin-6 (IL-6), and human tumor necrosis factor (TNF-α) and so on in the venous blood were tested before surgery, at 1 d and 5 d after surgery, respectively. Results: At 1 d after surgery, the patients of both groups had significantly decreased serum levels of IgA, IgM, and IgG, CD3+ and CD4+ in peripheral blood, and serum levels of hs-CRP, IL-6, and TNF-α, as well as significantly increased CD8+ level in peripheral blood compared with those before surgery (P<0.05). At 5 d after surgery, the observation group had insignificantly different levels of IgA, IgM, IgG, CD3+, CD4+, CD8+, hs-CRP, IL-6, and TNF-α compared with those before surgery (P>0.05); while the control group had significantly different levels of the above-mentioned factors compared with those before surgery (P<0.05). Before surgery, the two groups had insignificantly different levels of IgA, IgM, IgG, CD3+, CD4+, CD8+, hs-CRP, IL-6, and TNF-α (P>0.05); while at 1 d and 5 d after surgery, the observation group had significantly better levels of the above-mentioned factors than the controlled group (P<0.05). Conclusion: Laparoscopic cholecystectomy and open cholecystectomy have certain trauma, and can affect the immune function. However, the former has the characteristics of less trauma and faster postoperative recovery.

225 Application Value of Spiral CT Pulmonary Angiography with Low Radiation Dose and Low Dose Contrast Agent

Objective: To investigate the application value of spiral CT pulmonary angiography with low radiation dose and low dose contrast agent. Methods: Fifty-six patients received spiral CT pulmonary angiography in our hospital from May 2015 to May 2016 were selected as the research objects. All of the patients were randomly assigned into routine scan group (n=28) and double low dose scan group (n=28). Toshiba ONE Aquilion 320 slice spiral CT was used to check, the routine scan group (120kv, Omnipaque 350mgI/ml), the double low dose scan group (100kV, Visipaque 270mgI/ml), the other parameters were the same. The following parameters of two groups were compared: the CT values of each segment of the pulmonary artery, the values of noise, the signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR), the quality of the pictures, the CT dose index (CTDIvol), dose length product (DLP), effective radiation dose (ED) and iodine intake. Results: The CT values of each segment of the pulmonary artery of two groups had no significant difference (P>0.05). The values of SD [(7.7±2.1) vs (8.3±2.4)], SNR [ (23.9±7.3) vs (25.1±7.8)], CNR[(24.9±6.6) vs (27.1±7.2)], and the scores of picture quality [(4.1±0.8) vs (3.8±0.8)] of two groups had no difference (P<0.001). The CTDIvol [(6.1±0.4) vs (8.4±0.6)mGy], DLP [(293.4±21.7) vs (391.5±33.2) mGy.cm], ED [(4.3±0.4) vs (5.7±0.6)mSv/mGy.cm] and Iodine intake [(6.2±0.9) vs (8.3±1.1)g] of the double low dose scan group were lower than the routine scan group (P<0.001). Conclusion: Spiral CT with low radiation dose and low dose contrast agent can get good picture quality and meet the needs of clinical diagnosis, but also significantly reduce the radiation and iodine intake.

Objective: To discuss insulin-like growth factor receptor 1 (IGF-1) and its receptor (IGF-1R) expression in scar tissue and the effect of ribozyme on the expression level. Methods: 18 cases of Pathological scar tissue from operation excision (9 cases of scar keloid, 9 cases of scar hyperplasia ) were selected in Department of Burn and Plastics, Guangzhou Red Cross Hospital from 2015 May to 2016 July, meanwhile 20 cases of normal skin tissue specimens were collected. The expression level of tissue was observed by using hematoxylin eosin staining, the expression level of IGF-1 mRNA in each tissue specimen and its receptor mRNA were detected by using real-time fluorescence quantitative PCR, scar tissue fibroblasts were derived to culture cells to obtained IGF-1 and its receptor ribozyme, the cells in the logarithmic growth phase with good growth state in 2-4 generation + ribozyme were selected to culture,72h later, the expression level of IGF-1 mRNA and its receptor mRNA were detected by using real-time fluorescence quantitative PCR. Results: The differences were statistically significant in the expression level of IGF-1 mRNA in normal skin, hypertrophic scar and keloid tissue and its receptor mRNA (P <0.05), the comparison between groups showed that normal skin hypertrophic scar tissue < hypertrophic scar tissue < scar keloid tissue and the differences were statistically significant (P <0.05).Conclusion: The expression level of IGF-1 mRNA in scar tissue and its receptor mRNA are significantly higher than those in normal tissues, ribozyme has the effect on blockade of IGF-1 mRNA and its receptor mRNA expression, which may play a role in inhibiting scar formation.

232 The Clinical Value of Endoscopic Retrograde Cholangiopancreatography in the Acute Reaction of Acute Biliary Pancreatitis

Objective: To explore the clinical value of Endoscopic Retrograde Cholangiopancreatography in the acute reaction of acute biliary pancreatitis. Methods: 108 cases of acute biliary pancreatitis treated in our hospital from June 2013 to May 2016 were retrospectively analyzed. According to the different treatment methods, the patients were divided into control group and observation group. The control group received conventional non-surgical treatment,the observation group supplemented by emergency line ERCP treatment on the basis of conventional non-surgical treatment . The APACHE Ⅱ score and the incidence of complications were compared between the two groups at different time points. The laboratory indexes of the two groups were measured after 7 days of treatment. Results: The recovery time of normal body temperature, abdominal pain relief time and intestine ventilation time in the observation group were significantly lower than those in the control group (P <0.05). The APACHE Ⅱ score of the two groups were improved gradually after admission. The APACHE Ⅱ score of the observation group was significantly better than that of the control group on the 7th day after admission (P <0.05). The incidence of complications in the observation group was significantly lower than that of the control group P <0.05). After 1 week of treatment, the infection index (white blood cell count and C-reactive protein), liver function (total bilirubin, alanine aminotransferase and γ-glutamyltranspeptidase) and electrolytes (blood glucose and serum calcium ) were significantly improved compared with the control group (P <0.05). Conclusion: ERCP is an effective treatment for acute biliary pancreatitis, which can improve the laboratory indexes and improve the clinical curative effect, which is helpful for the outcome of acute biliary pancreatitis.

235 Risk Factors for Infections in Breast Cancer Patients after Surgery

Objective: To explore risk factors for infections in breast cancer patients chemotherapy after surgery in Southwest of China and carries on the analysis in order to provide theoretical basis for clinical infection prevention work. Methods: 56 GERD patients treated in Laoximen community health service center were divided into two groups. The treatment group patients used PPI combined with low intensity aerobic exercise, and the control group patients applied only PPI for treatment. 32 breast cancer patients in chemotherapy after surgery were analyzed on June 6, 2014- March 2016. All the materials were researched including oral ulcers, such as blood, urine specimens, to identify the bacteria and drug sensitive test. Results: 32 cases of breast cancer patients after chemotherapy were detected, with infection rate of59.3% during the second and third period of treatment. A total of 40 cases of infection, infection type is given priority to with respiratory tract infection, 20 cases (50%), digestive tract infection and skin soft tissue infection, 8 cases of each time each accounted for 20%; Pathogen of 32 cases of infection were detected 56 strains, with 30 strains of gram-negative bacteria, (53.6%), gram positive strains, fungi strains accounted for 28.6% and 17.9% respectively. Conclusion postoperative breast cancer chemotherapy patients lowered immunity, is people at high risk of infection, the infection is given priority to with the respiratory tract, targeted to develop effective nursing measures can reduce the incidence of infection, can improve the therapeutic effect of treatment. Objective: To investigate the clinical effect of retinal laser photocoagulation in the treatment of various ocular fundus diseases. Methods: 187 fundus diseases cases admitted in our hospital from November 2013 to December 2015 were randomly divided into observation group and control group, 89 cases in the control group given Woli Ting drug treatment, observation group, 98 cases (by retinal laser photocoagulation for treatment, after 2 months of treatment comparison. Results: The total effective rate of patients in the observation group of 90.82% was significantly higher than 76.40% (P<0.05); the adverse reaction rate of group observed was about 1.02% and it was significantly lower than the 4.49% in the control group (P<0.05). Conclusion: Treatment of ophthalmic surgery by laser photocoagulation treatment of different retinal diseases can significantly improve the overall efficiency and reduce the incidence of adverse reactions.

Objective: To investigate the application value of the low tidal volume combined with low levels expiratory positive airway pressure to soft lenses kidney gravel. Methods: 80 cases were divided into two groups, with 40 cases in each group. In breath control, low tidal volume combined with low levels expiratory positive airway pressure was used in the observation group, while blot's breathing treatment was used in the control group, 30min after the intervention, the blood gas analysis, TNF-a, IL-6 and CRP levels as well as procedure-related indicators in the two groups were compared. Results: 30min after the intervention, pH level in the observation group was lower than that in the control group(P<0.05), PCO2 level in the observation group was lower than that in the control group(P<0.05). After the surgery, TNF-a, IL-6 and CRP level in the observation group were significantly lower than those in the control group(P<0.05), The operative time of the observation group was shorter than that of the control group(P<0.05), the number of cases of respiratory and failure proportion of soft lenses treatment in the observation group were lower than those in the control group(P<0.05). Conclusion: The low tidal volume combined with low levels expiratory positive airway pressure for soft lenses kidney gravel can maintain normal blood gas analysis, can significantly reduce the influence of surgery, anesthesia stress on patients, which is conductive to the surgical procedure.

Objective: To analyze the influence of low-calcium dialysate on maintenance hemodialysis patients' secondary hyperparathyroidism and carotid atherosclerosis. Methods: 76 cases of maintenance hemodialysis patients were randomly divided into two groups, control group:high dialysate calcium concentration (DCa1.75), 3 times/week for 6 months; observation groups: low-calcium dialysate (DCa1.25), 3 times/week for 6 months. The peripheral blood PTH, calcium and phosphorus metabolism, lipid metabolism, ultrasound carotid artherosclerosis parameters before and after 6 months of two group patients were detected. Results: The PTH, calcium and phosphate metabolism, lipid metabolism, ultrasound carotid atherosclerosis parameters before treatment of two group patients showed no significant difference (P>0.05), The serum tCa, iCa, PTH contents of observation group patients were less than control group patients after 6 months treatment. P level was higher than control group patients (P<0.05); lipid metabolism parameters TC, TG,ApoB, LDL-C contents were less than control group patients, HDL-C level was higher than control group patients (P<0.05); serum content of SOD was higher than that in control group, contents of ROS and MDA were lower than those of control group (P<0.05); carotid ultrasound parameters IMT, CA-RI were lower than control group patients, Vmax, Vmin levels were higher than control group patients (P<0.05). Conclusion: For uremic patients, the concentration of calcium in the normal range, application of low calcium dialysate concentration helps to maintain the stability of calcium and phosphorus and the content of PTH, further progress in inhibition of carotid atherosclerosis, individualized dialysate calcium concentration is an important link to ensure the effect of dialysis in uremic patients, reduce serious complications.

Objective: To explore the clinical features of cognitive, behavioral and motor function in patients with cerebrovascular disease complicated with sleep disorders. Methods: A retrospective study on 226 cases patients with cerebrovascular disease admitted in our hospital from June 2015 to June 2016, according to whether patients with sleep disorders as cerebrovascular disease and sleep disorder group (study group) and cerebrovascular disease group (control group).The differences in behavior and motor function of the two groups of patients, cognitive were analyzed. Results: Sleep quality scale (PSQI) and sleepiness scale (ESS) scores were significantly higher in the study group than in the control group (P<0.01) in the study group (). The simple mental state examination scale (MMSE), Fugl-Meyer Motor Scale (FMA) and cognitive function rating scale (LOTCA) score in the study group were significantly lower than those in the control group (P<0.05). Patients in the study group were significantly higher than those in the control group (P<0.05) in the aspects of somatic, obsessive compulsive symptoms, interpersonal sensitivity, depression, anxiety and terror. The scores of quality of life scale (GQOL-74) in the study group were significantly lower than those in the control group (P<0.01) in cognitive function, mental function, physical function and social function. Conclusion: Cerebrovascular disease complicated with sleep disorders in patients with severe daytime sleepiness and nocturnal insomnia, and cognitive and motor function in patients with such loss more, poor mental health, quality of life of patients decreased significantly compared with the same type of disease, clinical treatment and nursing care should be paid attention to.

Objective: To study the effect of high dose of vitamin D injection combined with injection of insulin aspart on the parathyroid hormone level, insulin sensitivity and maternal and infant outcomes in the treatment of gestational diabetes mellitus (GDM). Methods: 238 gestational diabetes patients selected as object were divided them into two groups, called control group (n=109 cases) and research group (n=129 cases) by digital table. The patients of control group were treated by insulin aspart combined with low dose of vitamin D, while the patients of research group were treated by insulin aspart combined with high dose of vitamin D. the parathyroid hormone related indexes , insulin sensitivity related indexes, vitamin D level, maternal and infant outcomes and incidence of hypoglycemia during pregnancy of two groups were compared. Results: The iPTH, product of calcium and phosphorus , phosphorus, calcium of two groups before treatment had no statistically difference (P>0.05),after treating four weeks the iPTH, product of calcium and phosphorus, phosphorus of research group were much less than control group (P<0.05),the calcium of two groups had no statistically difference (P>0.05). The iPTH, product of calcium and phosphorus, phosphorus of two groups after four weeks' treatment were all much less than pre-treatment respectively (P<0.05),while calcium of two groups after treatment had no statistically difference with pre-treatment respectively (P>0.05). The FPG, FINS, 2hPG of two groups before treatment had no statistically difference (P>0.05); the FPG, FINS, 2hPG of research group had no statistically difference with control group (P>0.05); The FPG, FINS, 2hPG of two groups after four weeks' treatment were all much less than pre-treatment respectively (P<0.05). The HOMA-βand HOMA-IR of two groups before treatment had no statistically difference (P>0.05), the HOMA-βof research group after treatment was much higher than control group. HOMA-IR much less than control group (P<0.05). The HOMA-βand HOMA-IR of two groups after four weeks' treatment were all much less than pre-treatment respectively (P<0.05). The vitamin D level of two groups before treatment had no statistically difference (P>0.05). The moderate lack rate of research group after treatment was less than control group (P<0.05). The normal rate of research group after treatment was more than control group(P<0.05); the slight lack rate of research group after treatment of two groups had no statistically difference (P>0.05). The moderate lack rate and slight lack rate of two groups after four weeks' treatment were all much less than pre-treatment respectively (P<0.05); normal rate was much higher than pre-treatment respectively (P<0.05). The rate of maternal and infant outcomes and hypoglycemia of research group was 20.93%, while control group was 31.19%; they had statistically difference (P<0.05). Conclusion: High dose of vitamin D injection combined with injection of insulin aspart in the treatment of GDM can prevent the occurrence of physiological hyperparathyroidism in late pregnant period ,increase the insulin sensitivity, effectively control blood sugar and improve the deficiency degree of vitamin D, and definite effect, decrease maternal and neonatal complications rate, which is worthy of being clinically promoted.

255 Effect of Dezocine Combined with Ropivacaine on Postoperative Analgesia, Stress Response and the Immune Function of Patients with Gastric Cancer

Objective: To explore the effects of Dezocine combined with Rropivacaine on postoperative analgesia, stress response and the immune function of patients with gastric cancer. Methods: 74 cases of gastric cancer patients in our hospital undergone surgery were divided into dezocine combined with ropivacaine group (study group) and fentanyl ropivacaine group (control group) according to different drugs used by the patients. Postoperative analgesia, stress reaction and immune function of the two groups were compared. Results: 4h, 8h, 24h and 48h after the operation, the scale scores of the patients from visual fuzzy scale (VAS) in the study group were significantly lower than those in the control group (P<0.05). 4h, 8h and 24h after the operation, the levels of ET and AngⅡin the study group were significantly lower than those in the control group (P<0.05). 24h after the operation, the levels of CD3, CD4 and NK cells in the study group were significantly higher than those in the control group (P<0.05). The total adverse reactions after the operation in the study group were significantly lower than those in the control group (P<0.05). Conclusion: Dezocine combined with Ropivacaine has better analgesia effect in postoperative patients with gastric cancer, can reduce patients' stress response level and enhance patients' immune function,whose clinical effect is better.

259 Effect of PDGFRA on Cell Proliferation and Invasion in Lung Cancer Cell Lines

Objective: To evaluate the effect of platelet derived growth factor receptor alpha(PDGFRA) on the proliferation and invasion of lung cancer cells. Methods: The mRNA expression level of PDGFRA in eight lung cancer cell lines and normal lung cell HBE was detected by PT-PCR. Two groups of lung cancer lines(A549) from the high expression level of PDGFRA were selected, then stable knock down and over-expression PDGFRA cell lines were constructed through lentiviral transfection. Furthermore, the effect of PDGFRA on cell proliferation and invasion in these cells were evaluated by MTS, scratch test, Transwell and western-blot test. Results: Over-expression of PDGFRA showed that cell proliferation and invasion significantly increased, meanwhile, E-candherin and Vimentin also increased, scratch test and transwell revealed that PDGFRA significantly improved the cell invasion. Whereas, knock down of PDGFRA decreased the cell proliferation, the expression of E-candherin and Vimentin decreased and the cell invasion also significantly also decreased compared with the control group. Conclusion: PDGFRA can promote the cell proliferation and contribute to the cell invasion in lung cancer cells.

263 The Application of Serum Golgi Membrane Protein 73 in Early Diagnosis of Primary Hepatocellular Carcinoma

Objective: To investigate the clinical value of serum Golgi membrane protein 73 (GP73) in the diagnosis of primary hepatocellular carcinoma (HCC).Methods: 300 people including healthy people, outpatient and inpatient in our hospital from July 2014 to July 2015 were divided respectively into five groups as HCC group, posthepatitic b cirrhosis group, other malignant tumor group, chronic hepatitis B group and healthy control group. The level of Serum GP73 and alpha fetoprotein (AFP) were determined and the ROC curve of its diagnostic HCC was drawn. Result: There was no significant difference in the level of serum GP73 of the patients with different age, tumor size, gender and concentration of serum AFP in the HCC group (P>0.05). The level of serum AFP and GP73 of the patients in the HCC group were significantly higher than that in the other groups(P<0.05), while the level of serum AFP and GP73 of the patients in the chronic hepatitis B, posthepatitic b cirrhosis group and other malignant tumor group were significantly higher than that of the healthy control group(P<0.05). The specificity and sensitivity of GP73 in the diagnosis of HCC were significantly higher than those of AFP, P<0.05. Conclusion: The sensitivity and specificity of serum GP73 in the diagnosis of HCC are better than those of AFP. GP73 can be a serum marker in the early diagnosis of HCC.

266 The Value of Super-thin Perforator Flaps in the Reconstruction of the Extensive Defects and Deformities in the Face and Neck

Objective: To observe the value of super-thin perforator flaps in the reconstruction of the extensive defects and deformities in the face and neck. Methods: From January 2003 to January 2016, 61 cases with extensive facial and cervical scars were collected in our hospital. The patients in control group were treated by conventional pre-expanded flaps, total 32 cases. The patients in experimental group were treated by super-thin perforator flaps. The skin flap healing and skin repair was analyzed. Results: The control group has 6 cases of flap necrosis and the flap necrosis rate were 20.69%.And the experimental group has 1 cases of flap necrosis and the flap necrosis rate were 3.13%,which was significantly lower than the control group(P<0.05).The control group has 10 cases of flap retraction and the flap retraction rate were 34.48%,and the experimental group has 3 cases of flap retraction and the flap retraction rate were 9.38%,there difference was significant(P >0.05).All patients were followed up for 2 years showed that the super-thin perforator flaps thinner than the conventional pre-expanded flap, thus avoiding secondary flap debulking and revisions. And the appearance and muscle stretch function of experimental group were better than the control group. Conclusion: The super-thin perforator flaps is a safe and effective surgical method to repair the extensive defects and deformities in the face and neck. It can reduce the flap necrosis rate, restore facial appearance and function.

269 The Efficacy of Vacuum Sealing Drainage Combined with BFGF in the Treatment of Sacrococcygeal Bedsores

Objective: To investigate the efficacy of vacuum sealing drainage combined with BFGF in the treatment of sacrococcygeal bedsores. Methods: 86 cases of sacrococcygeal bedsores were randomly divided into observation group and control group. Vacuum sealing drainage combined with BFGF and vacuum sealing drainage therapy alone were given respectively to the two groups. The clinical efficacy and the basic treatment between the two groups were compared. Results: The total effective rate of the observation group was significantly higher than that of the control group. The hospitalization time, antibiotic use time, disappearance time of secretions, blood recovery time, dressing frequency and treatment cost were significantly lower in the observation group than in the control group, the difference was statistically significant (P<0.05). Conclusion: Vacuum sealing drainage combined with BFGF can effectively improve the clinical efficacy, shorten the hospitalization time, promote the wound healing, accelerate the recovery of patients, reduce the cost of hospitalization and relieve the patients' economic pressure, therefore, is worthy of further clinical application.

271 Observation of Therapeutic Effect of Laparoscopy combined with Fiber Bile Duct Endoscopy in the treatment of elderly patients with Common Duct Stones and Gallbladder Stones

Objective: To investigate the clinical effect of laparoscopic combined with fiber bile duct endoscopy in the treatment of elderly patients with common bile duct stones and gallbladder stones. Methods: 88 cases of cholecystolithiasis and choledocholithiasis patients admitted in our hospital during the period from May 2011 to May 2014 were selected and were randomly divided into control group (treated with traditional open surgery, n=44) and observation group (given laparoscope combined with choledochoscope in the treatment, n=44), observed the clinical curative effect of two groups. Results: The two groups were all successful, the observation group patients without laparotomy, postoperative examination showed no residual stones; the amount of bleeding, hospitalization time and postoperative gastrointestinal function recovery time was significantly lower than the control group (P<0.05); The incidence of postoperative complications was significantly higher in the control group than in the observation group, with significant difference (P<0.05). Conclusion: The elderly patients with common bile duct stones and gallbladder stones are treated by laparoscopy combined with fiber bile duct endoscopy. The effect is remarkable, and the patients recover quickly, the wound is small. So it has the promotion value.

274 Study on Transforaminal Endoscopic BEIS Technique for the Treatment of Lumbar Foraminal Stenosis early Curative Effect

Objective: To study the clinical effect of spinal canal stenosis with transforaminal endoscopic technique in the treatment of lumbar BEIS. Methods: 80 cases of lumbar foraminal stenosis patients admitted in our hospital from January 2014 to May 2016 were randomly divided into minimally invasive group (transforaminal endoscopic BEIS) and conventional group (traditional surgery) in 40 cases, compared two groups of patients with operation time, blood loss, hospital stay, pain, ODI function postoperative score. Results: The operation time, minimally invasive group bleeding volume and hospitalization time were lower than the traditional group, the difference was statistically significant (P<0.05); 1 weeks after surgery, after 1 months, 3 months, minimally invasive group VAS, ODI scores and the traditional group was not statistically significant (P>0.05), two groups of patients, VAS compared with the preoperative ODI score were significantly decreased (P<0.05). Conclusion: Transforaminal endoscopic BEIS technique in the treatment of lumbar spinal stenosis has less trauma, quicker recovery advantages. And the surgical effect is equivalent to traditional surgery.

278 Clinical Value of Serum Procalcitonin and C-reactive Protein on Middle-aged and Elderly Chronic Obstructive Pulmonary with Acute Aggravating Concurrent Infection

Objective: To explore the clinical value of serum procalcitonin and C-reactive protein on middle-aged and elderly chronic obstructive pulmonary with acute aggravating concurrent infection. Methods: 127 cases patients of chronic obstructive pulmonary disease met the inclusion criteria were chose in our hospital from December 2014 to April 2016, according to patient's condition, divided the COPD disease remission group(remission group) and COPD acute aggravating group (aggravating group). Then subdivided into COPD with acute aggravating infection group (infection group) and COPD with acute aggravating infection-free group (infection-free group) at the COPD acute aggravating group, infection group was graded according to the patient's lung function. The different level of serum PCT and CRP on different grouping and classification were compared. Results: The levels of CRP and serum PCT at COPD acute aggravating group was significantly higher than those in stable phase (P <0.05). The level of PCT and CRP at COPD with acute aggravating infection group was significantly increased than infection-free group (P <0.05). The level of PCT and CRP at level Ⅰ, Ⅱ, Ⅲ lung grade function of COPD with acute aggravating infection group, showed a gradual upward trend, and the statistical difference was significant between different classification (P <0.05). Conclusion: The level of serum PCT and CRP on COPD with acute aggravating infection group are significantly increased, and increase more significantly with the deterioration of lung function.

281 The Effect of Sevoflurane on Cerebral Oxygen Consumption by Controlled Hypotension in Intracranial Aneurysm Clipping Operation

Objective: To study the effect of sevoflurane on cerebral oxygen consumption by controlled hypotension in intracranial aneurysm clipping operation. Methods: 90 patients treated by intracranial aneurysm clipping operation in our hospital from April 2015 to February 2013 were selected and sevoflurane was used for depressurization in the operation, the changes of blood flow parameters before anesthesia, before depressurization, during depressurization and after depressurization were recorded and blood gas was analyzed. Results: Before depressurization, during depressurization and after depressurization, the arterial and venous partial pressure of blood oxygen all significantly increased compared with those before anesthesia (P < 0.05). At each stage patient's heart rate didn't change significantly (P > 0.05). The difference of arterial and venous blood oxygen content and oxygen uptake rate of the neurocranium were all significantly lower than those before anesthesia (P < 0.05). Conclusion: The application of sevoflurane to the neurosurgery operation by controlled hypotension can effectively reduce blood pressure and the amount of oxygen in the brain, therefore, can reduce the occurrence of postoperative complications.

328 Clinical Prognosis Effect of Multiple Lacunar Infarction, Psychological Status and Quality of Life in Elderly Patients

Objective: To explore the clinical prognosis effect of multiple lacunar infarction, psychological status and quality of life in elderly patients. Methods: Through a retrospective study on 45 patients with multiple lacunar infarction in our hospital from June 2015 to June 2016 and 43 cases of elderly patients with non infarction symptoms during the same period, the clinical prognosis effect, psychological status and quality of life of the two groups were analyzed and compared. Results: After the treatment in the hospital, the efficiency rate and effective rate in the study group was significantly lower than those in the control group (P<0.05). The incidence of infection, depression, cognitive disorder and hemiplegia in the study group were significantly higher than those in the control group (P<0.05). The scores in somatization, interpersonal sensitivity, depression, anxiety of the patients in the study group were significantly higher than those in the control group (P<0.05). 3 months after the patients discharged from the hospital, the scores in physical function, role physical function, bodily pain, general health, vitality, social function, emotional role function and mental health in the study group were significantly lower than those in the control group (P<0.01). Conclusion: The effect in clinical prognosis of multiple lacunar infarction in elderly patients is poor, the mental health is seriously affected, the depression and anxiety is severer, the quality of life significantly reduces, therefore the clinical treatment and nursing care to the patients should be paid more attention.

332 Humanistic Nursing Care in Patients with Advanced Liver Cancer

Objective: To observe the effect of humanistic care nursing for pain, mood and the sleep quality of patients with terminal cancer, to explore the clinical value of humanism care. Methods: 260 cases of terminal cancer patients treated in our hospital from February 2013 to June 2016 were selected as the research objects, and were randomly divided into observation group and control group, each group was 130 cases. The situation of pain, mood and sleep quality before and 4 weeks after treatment of patients in two groups were comparatively analyzed. Results: There was no difference of patients between two groups in pain VAS score, SDS and SAS scores and Pittsburgh sleep therapy index before treatment(P > 0.05), the pain VAS score, SDS and SAS scores and Pittsburgh sleep treatment index of the observation group after treatment were significantly lower than that of the control group (P < 0.05), the difference was statistically significant. Conclusion: The implementation of humanistic care for patients with terminal cancer can reduce the pain, reduce the occurrence of negative emotions such as anxiety and depression, and improve the sleep quality of patients, it has widely practical application value.
