1761 Effect of TACE Combined with HIFU on Elderly Patients with Primary Liver Cancer and its Effect on Immune Function

Objective: To investigate the clinical efficacy of TACE (hepatic arterial chemoembolization) combined with HIFU (high intensity focused ultrasound) in elderly patients with primary liver cancer and its effect on immune function. Methods: 84 patients with primary hepatocellular carcinoma were randomly divided into control group and observation group (n = 42). The patients were randomly divided into control group and observation group. The patients in the control group were treated with TACE alone. The patients in the observation group were treated with TACE combined with HIFU. The changes of tumor volume and immune function were compared between the two groups. The median survival time and the survival rate after 3 years were compared. Results: After treatment with different methods, the tumor volume reduction and clinical treatment efficiency in the observation group were significantly better than those in the control group (P < 0.05). The median survival time and the survival rate of 1, 2 and 3 years after treatment in the observation group were significantly higher than those in the control group (P < 0.05). The levels of immune globulin IgA, IgM, IgG of patients in the observation group after treatment of 3 months were [(3.65 + 0.20) g/L], [(2.29 + 0.18) g/L], [(10.68 + 0.84) g/L], were significantly higher than those in the control group [(3.23 + 0.24) g/L], [(1.81 + 0.15 g/L]), [(8.76 + 0.67) g /L]; the difference between the two groups was statistically significant (t=8.71, 13.28, 11.58; P < 0.05); At three months after the treatment, the observation group patients complement C3 and C4 levels were [(1.49 + 0.06) g/L], [(0.34 + 0.03) g/L], the control group [(1.31 + 0.08) g/L], [(0.22 + 0.05) g/L], significant difference between the two groups (t=11.67, 13.34; P < 0.05). Conclusion: TACE combined with HIFU in elderly patients with primary liver cancer can effectively reduce the volume of the lesion, improve the immune kinetic energy of patients, improve the survival rate of patients, prolong the survival time, the effect is remarkable, worthy of clinical promotion.

1764 Study on the Correlation between Microscopic Appearances and Histopathological Changes of Actinic Keratosis

Objective: To study the skin microscopy and histopathological changes of solar keratosis. Methods: The lesion was examined by histopathological examination, and the skin lesions were observed by SLR camera and skin mirror. Results: 56 cases were diagnosed with suspected kyphotic keratosis patients, of which 46 cases of skin and pathology consistent with the results of solarization keratosis, 10 cases of dermatology diagnosis of solarization keratosis, and histopathological Results:2 cases of basal cell carcinoma, 7 cases of seborrheic keratosis, 1 case of eczema, dermoscopy and pathology coincidence rate was 82.14%. Among them, female patients in 22 cases (47.83%), male patients in 24 cases (52.17%). 6 patients (13.04%) from 51 to 60 years old, 24 patients (52.17%) from 61 to 70 years old, 71~ 82 years old patients in 11 cases (23.92%). Most of the elderly patients, the age of onset concentrated in the 60 to 80 years old, accounting for 74.23% of the number of this study. The longest course of the disease for more than 20 years. There were significant differences in the occupation of the patients, the largest number of farmers, 38 cases (82.61%). There were 6 cases (13.04%) of cervical disease and 3 cases (6.53%) of patients with dorsal disease. There were 6 patients (13.04%) with pain in the affected area, 32 patients (69.57%) with itching, 8 patients (17.39%) without obvious discomfort. Patients with 0.4cm ~ 3.3cm lesions, the performance of red or yellow brown patches or pimples, lesions more dry parts, damage was scaly, and often appear excessive keratosis and verrucous features. Some patients with ulceration of crust injury site symptoms. At the same time, skin lesions around the skin days of sun damage characteristics, such as skin atrophy, pigment changes and significant expansion of blood vessels. Histopathological findings showed that there were 8 cases (17.39%) of hypertrophic patients, 13 cases (28.26%) of atrophic patients and 4 cases (8.70%) of pigment patients, 5 cases of digitated patients (10.87%), Type of bowenoid patients in 13 cases (28.26%), acantholytic type in 3 cases (6.52%). Dermoscopic erythema, under the background of false grid structure in 33 cases (71.74%), hair follicle hornplug surrounded by white circles in 28 cases (60.87%), 25 cases of punctate structures around the hair follicle orifice (54.35%), performance scales in 39 cases (84.78%), showing erythema in 34 cases (73.91%), 36 cases of strawberry the structure of (78.26%), 37 cases had curled vessels (80.43%). Conclusion: There is a good consistency of dermoscopy on the diagnosis of solar keratosis and pathological findings of the results. Sunshine keratosis skin mirror performance and histopathological changes have obvious characteristics, through the relevant examination to diagnose the disease, early treatment, reduce the patient's condition deterioration, the risk of cancer, the detection method worthy of clinical promotion.

1768 Integrated State of HPV-16 in Precancerous Tissues and CervicalCancer and Application in Cervical Cancer Screening

Objective: To investigate the prevalence of physical state of HVP-16 DNA in cervical and cervical precancerous carcinoma. Methods: Multiplex PCR was adopted to detect the physical state of HPV in samples from 252 patients with cervical carcinoma, including 48 samples of cervical cancer, 204 cervical intraepithelial(CIN)(125 CIN I,46 CIN Ⅱ and 33 CIN Ⅲ)and 20 normal samples from the subjects with hysteromyoma undergoing hysterectomy ,respectively. Results: Not only patients with cervical cancer but also patients with precancerous were found to have integrated infection with HPV-16 and those with cervical cancer have the highest integration rate. With the elevating of the grades of CIN, the integrated infection rates of HVP-16 was significantly elevated, the difference was statistically significant (χ2=49.7359,12.6033, P<0.01). Conclusion: Among the patients with HPV-16 infection,the integrated state of HVP-16 is positively correlated with the severity of cervical lesions. Combined HVP typing test and detection of integrated viral contribute to predicting the prognosis of patients with cervical precancerous lesions and increasing the accuracy of screening cervical cancer on the basis of HPV DNA detection.

1772 Effect of Tiotropium combined with Shah Mette Lo Tikason on T lymphocyte and Inflammatory Factors in patients with Moderate to Severe Stable COPD

Objective: To investigate the effect of tiotropium and Shah Mette Lo Tikason on inflammatory factors and T lymphocytes in patients with moderate to severe chronic obstructive pulmonary disease (COPD). Methods: Between February 2014 to February 2015 in our hospital respiratory outpatient department visits moderately severe plateau, 80 cases of COPD patients, all patients were randomly divided into two groups, the control group, 40 patients with routine western medicine treatment, the observation group 40 cases in the control group was given on the basis of the treatment. Consult for the occasional bromide combined salmeterol assigned for treatment. After 2 weeks treatment, and compared before and after treatment serum inflammatory factors [interleukin 8 (IL - 8) tumor necrosis factor alpha] (TNF alpha) in serum T lymphocytes (CD4 + CD8 +, CD4 + / CD8 +) and pulmonary function (FEV1, FVC FEV1 / FVC) level changes. Results: The clinical curative effect of observation group patients was significantly better than the control group patients (P < 0.05).Two groups after treatment in patients with IL - 8, TNF alpha value significantly reduced compared with before treatment (P < 0.05), the observation group after treatment in patients with IL - 8, TNF alpha value was significantly lower than the control group patients (P < 0.05).In the two groups after treatment in patients with serum CD4 + and CD4 + / CD8 + T lymphocytes was increased, the and falling CD8 + than before treatment (P < 0.05).Observation group of patients after treatment serum CD4 + and CD4 + / CD8 + T lymphocytes was higher than the control group, and CD8 + phase is lower (P < 0.05).Two groups of patients after two weeks of lung function were improved after treatment, improve is better than that of control group and observation group patients (P < 0.05). Conclusion: The seconds for the occasional bromide combined salmeterol calzone treatment period of COPD patients can effectively improve the patient's lung function, lower levels of inflammatory cytokines, enhance immunity.

1776 The Stability of L4/5 Interspinous Interbody Fusion Cage and its Effects on Biomechanical Properties and Biocompatibility of Adjacent Segments

Objective: To investigate the effects of stability of the L4/5 lumbar, biomechanical properties and biocompatibility of adjacent segments for interspinous fusion cage. Methods: 18 fresh adult frozen spine specimens were collected, including L4/5 lumbar for interspinous fusion cage as the study group, and normal specimens as the control group, with 9 specimens in each group. The specimens of two groups were numbered and implanted in vivo and fixed by fixators. Results: The strain changes in different implantation for anteflexion, center compression, lateroflexion and rear protraction in the study group were smaller than in the control group (P all<0.05). Under the maximum load of 600 N, the angular displacement and linear displacement in left lateral flexion, levorotation, anteflexion and rear protraction in the study group were lower than in the control group (P all<0.05). Conclusion: L4/5 lumbar for interspinous fusion cage can effectively improve the vertebral activity and keep its stability, which is helpful to shorten the stress of the intervertebral disc.

1779 The Relationship between the Expression of CD34 and VEGF and the Magnitude of CT Enhancement in Esophageal Carcinoma

Objective: To analyze the relationship between the expression of CD34 and VEGF and the enhancement of CT in esophageal carcinoma. Methods: The organization of esophageal mucosa in 40 cases of esophageal carcinoma and 20 cases of healthy people were collected from our hospital, the expression of CD34 and VEGF protein were detected by immunohistochemistry in tissue, and the MVD was counted, measured CT scan cancer CT enhancement value, will be between the indexes and clinical stages of the tumor and its relation to statistics analysis. Results: Compared with healthy control group, the positive expression rate of CD34 and VEGF in esophageal carcinoma group were higher (P<0.05), and they were positively correlated in esophageal cancer tissues (r=0.451, P<0.05); The expression of VEGF and MDV in esophageal carcinoma were correlated with the degree of cell differentiation, depth of invasion, lymph node metastasis and clinical TNM stage (P<0.05); the expression of VEGF in esophageal cancer tissues (r=0.434, P<0.05) and MVD (r=0.463, P<0.05) with spiral CT enhancement was positively correlated, the differences were statistically significant. Conclusion: CD34 and VEGF were highly expressed in esophageal cancer, and CD34, VEGF and CT enhancement in esophageal carcinoma are related, are important indicators of combined detection can be used as the invasion and metastasis of esophageal carcinoma and prognosis judgement.

1784 Effects of Different Anesthesia Methods on CRP and Complement C3 and C4in Patients Undergoing Laparoscopic Cholecystectomy

Objective: To observe the effects of different anesthesia methods on CRP and complement C3 and C4 in patients undergoing laparoscopic cholecystectomy. Methods: Patients with laparoscopic cholecystectomy treated in our hospital from March 2014 to June 2015 were observed. According to the methods of operation, All the cases were divided into general anesthesia group (45 cases) and general anesthesia combined with epidural anesthesia group (50 cases). Observed the level of CRP, stress indicators of the two groups, as well as the effects of complement C3, C4, compared the anesthesia effect of the two groups of patients. Results: The level of CRP and cortisol of the two groups before anesthesia had no significant difference, while general anesthesia group of these indicators in skin incision after 2h, 4h after the operation changes compared with the compound anesthesia group obviously, the difference was statistically significant (P < 0.05); There was no significant difference in the level of C3 and C4 between the two groups before and after anesthesia. The changes of 2h and 4h in the general anesthesia group were significantly higher than those in the combined anesthesia group, the difference was statistically significant (P<0.05); After surgery, the VAS score of the combined anesthesia group was lower than that of the general anesthesia group, and the Ramsay scores of the two groups were not significantly different. Conclusion: General anesthesia combined with epidural anesthesia can significantly reduce the stress reaction of patients, and has a good effect of anesthesia.

1788 Clinical Study of STARR Sequential Combined with Biofeedback in the Treatment of Pelvic Floor Relaxation Constipation

Objective: To observe the clinical efficacy of STARR sequential combined with biofeedback in the treatment of pelvic floor relaxation constipation. Methods: 90 cases of pelvic floor relaxation constipation were randomly divided into the treatment group and the control group. The treatment group was treated by STARR sequential combined with biofeedback, while the control group was justly treated by STARR. Patients' ODS score and result of rectal pressure test were recorded before surgery and 6 months after surgery. Results: Patients' ODS score and results of rectal pressure test and pelvic floor electromyography in the treatment group were better than those in the control group 6 months after surgery(P<0.05). Conclusion: STARR sequential combined with biofeedback can significantly improve the satisfaction rate and improve pelvic floor function of pelvic floor relaxation constipation.

1791 Foxiangyin on the Model Rats with Cold Coagulation and Blood Stasis Dysmenorrhea Pain Reaction and Blood Rheology

Objective: To investigate the effects of water extract from Foxiang Yin (FXY) on pain reaction and hemorheological parameters of rats with dysmenorrhe induced by cold coagulation and blood stasis to provide the experimental basis for clinical application of the prescription. Methods: The rats dysmenorrhea model induced by cold congealing and blood stasis were made by ice water bath and subcutaneous injection of adrenaline hydrochloride(Adr)combined with intraperitoneal injection of oxytocin. The inhibition rate of pain reaction was were calculated. The prothrombin time (PT), thrombin time (TT), activated partial thromboplastin time (APTT), the rate of platelet aggregation, hematocrit (HCT), K value of erythrocyte sedimentation equation, plasma viscosity and whole blood viscosity were detected. Results: The CT,PT,TT and APTT in 1.8g/kg,3.6g/kg and 7.2g/kg doses groups of FXY are significantly longer than that in the model control group(P<0.05 or P<0.01). The platelet aggregation rate in high and middle 3.6g/kg and 7.2g/kg dose of FXY group were significantly lower than that of model group (P<0.05 or P<0.01). The hematocrit ( HCT) and ESR equation K values in all doses of FXY groups were significantly lower than that of model group (P<0.05). The plasma viscosity and whole blood viscosity in all dosage of FXY groups were lower than that of model group (P<0.05 or< P 0.01). Conclusion: FXY has obvious analgesic effect on the cold coagulation and blood stasis dysmenorrhea rats and improve high agglutination condition of blood and hemorheology is one of its action mechanisms.

1795 Comparison of the Value of CT and MRI in the Diagnosis of Ovarian Cancer

Objective: To investigate the value of CT and MRI in the diagnosis of ovarian cancer. Methods: 90 cases of ovarian cancer patients in our hospital were studied. All patients were examined by CT and MRI before operation. The results of operation and pathology were compared and analyzed. The difference of CT and MRI in diagnosis of ovarian cancer was compared. Results: With reference to the postoperative pathological findings, the accuracy rates of MRI in the diagnosis of ovarian cancer and pathological stage were higher than CT (91.11% and 88.89% vs 80.00% and 67.78%) (P < 0.05). Follow-up showed that among 90 patients in the study, a total of 78 patients had recurrence and metastasis. The diagnostic rate and sensitivity of MRI in the diagnosis recurrence and metastasis were significantly higher than CT (P < 0.05). Image analysis showed that MRI had more advantages than CT in displaying lesions of ovarian cancer. Conclusion: Both of CT and MRI have advantages in the diagnosis of ovarian cancer, the accuracy rates of MRI in the diagnosis of ovarian cancer, recurrence and metastasis are higher than CT. Therefore, it is more valuable than CT.

1799 Trimebutine Combined with Alprazolam on InflammatoryFactors and Gastrointestinal Hormones in Diarrhea Type IBS

Objective: To evaluate the effect of trimebutine combined with alprazolam on serum inflammatory factor, immune function, gastrointestinal hormone, ghrelin and quality of life of IBS-D patients. Methods: 117 cases of IBS-D were randomly divided into control group (n=58) and treatment group (n=59).The control group was only given trimebutine treatment, the treatment group was treated with trimebutine combined with alprazolam, 2 groups were consecutively treated for 4 weeks. Detected and compared of the 2 groups of serum inflammatory factors, immune function, gastrointestinal hormon, ghrelin and quality of life changes before and after treatment; Statistics and analysis of 2 groups of SF-36 quality of life scores before and after treatment. Results: After treatment, the MOT, GAS, SP, 5-HT, IL-8, IL-12, CD8+T cell ratio of 2 groups were significantly decreased (P<0.05 or P<0.01), VIP, SS, CCK, CGRP, IL-10, CD3+, CD4+, CD4+/CD8+ and SF-36 scores increased significantly (P<0.05 or P<0.01), and the difference between 2 groups was significant (P<0.05 or P<0.01). Conclusion: Trimebutine combined with alprazolam can significantly improve gastrointestinal peristaltic function of IBS-D patients, balance the level of inflammatory cytokines, improve the immune function and quality of life, with significant curative effect and worthy of clinical application.

1803 Comparative Study of Periodontal Iltrasonic Therapeutic Apparatus and Gracey Scraper in the Treatment of Periodontitis

Objective: To compare the effect of Gracey scaler and periodontal ultrasonic therapeutic apparatus in the treatment of periodontitis. Methods: 80 periodontitis patients were randomly divided into a control group and an observation group according to random number table and 40 cases in each group. Gracey traditional manual scaler was used in the control group and Vector periodontal ultrasonic therapeutic apparatus were used in the observation group. The dental plaque index (PLI) , probe depth (PD) , gingival index (GI) and clinical attachment loss (CAL) and VAS were compared. Results: One month after the treatment, dental plaque index( PLI), probe depth (PD) and clinical attachment loss (CAL), gingival index(GI) were both improved and the difference was statistically significant (P < 0. 05) , the PLI, GI and PD in the observation group were better than that in the control group (P < 0. 05) . The VAS were both reduced in two groups and the difference was statistically significant (P < 0. 05) , and the VAS in the observation group reduced more than that in the control group (P < 0. 05) . Conclusion: Both of Gracey scaler and periodontal ultrasonic therapeutic apparatus has sound effect for periodontitis, periodontal ultrasonic therapeutic apparatus has better effect and it is of high clinical application value.

1805 Retrospective Analysis of Influencing Factors of Lymph Node Metastasis in 280 Cases of cT1aN0M0 Lung Adenocarcinoma

Objective: To explore the risk factors of lymph node metastasis in cT1aN0M0 lung adenocarcinoma. Methods: The clinical data of 280 cases of cT1aN0M0 lung adenocarcinoma patients admitted between January 2013 and December 2015 in our hospital were selected, and the lymph node metastasis and clinical factors were retrospectively analyzed. The pathological characteristics and clinical data of the patients, single factor and multiple factors analysis were done to explore the influence factors of lymph node metastasis of cT1aN0M0 lung adenocarcinoma. Results: After all the 280 patients with cT1aN0M0 lung adenocarcinoma underwent thin slice CT scan, 108 patients (38.6%) were found to have pure glass opacity in CT imaging. Ground glass opacity combined with solid components (<5mm) were found in 117 cases (41.8%). Ground glass opacity combined with solid nodules (solid component diameter <5mm) were found in 12 cases (4.3%), pure solid nodules (solid component diameter ≥5mm) were found in 43 cases (15.4%). Patients with lymph node metastasis were 28 (10%), of which 16 patients (5.7%) with N1 lymph node metastasis and 12 patients (4.3%) with N2 lymph node metastasis. In this study, all of the patients with a diameter of less than 5 mm in diameter or with pure ground glass opacity, no lymph node metastasis after surgery were found, P<0.05, the difference was significant. Conclusion: For cT1aN0M0 lung adenocarcinoma patients, if the imaging performance were pure grinding glass or solid components of the diameter is less than 5 mm, there is usually no lymph node metastasis occurred. In patients with tumor diameter greater than 1cm, mixed nodules and solid nodules and μg/L CEA>5 were important prognostic factors for lymph node metastasis. Analysis of the maximum standard uptake value in patients with PET-CT showed that patients were more likely to have lymph node metastasis when the maximum standard uptake value was greater than 5(P<0.05).

1810 Effects of Laparoscopic Cholecystectomy on Efficacy of Acute Calculous Cholecystitis and Immune Function and Postoperative Complications

Objective: To explore the effects of laparoscopic cholecystectomy (LC) on the efficacy of acute calculouscholecystitis and immune function and postoperative complications. Methods: 88 cases of patients with acute calculous cholecystitis treated in our hospital from March 2015 to February 2017 were selected for the study and divided into two groups: the observation group (n=42, treated with LC surgery) and the control group(n=46, treated with open cholecystectomy/OC) according to the random number table method. The operative time, intraoperative blood loss, postoperative exhaust time, hospital stays, the changes of postoperative serum C-reactive protein (CRP)levels and cortisol levels, postoperative levels of CD3, CD4, CD8 and CD4/CD8, NK cells changes and postoperative complications were compared between the two groups. Results: The operative time was (72.36±12.62) min in the observation group, which was significantly longer than that of the control group (P<0.05). The intraoperative blood loss amount was (173.63±22.65) ml in the observation group, which was significantly less than that in the control group (P<0.05). The postoperative exhaust time and hospital stays were (21.74±7.96) h and (6.35±2.01) d, which were significantly shorter than those of the control group (P<0.05). The levels of serum CRP and cortisol were (4.36±0.25) mg/dl and (257.83±10.74) ng/ml respectively in the observation group, which were significantly lower than those in the control group (P<0.05). The percentage of CD3, CD4, CD8, CD4/CD8 and NK cells were (64.39±5.37)%, (35.74±3.81)%, (23.65±3.17)%, (1.51±0.08) and (16.46±2.85) %in the observation group, which were significantly higher than those in the control group (P<0.05). The total incidence rate of postoperative complications was 4.76% in the observation group, which was significantly lower than that in the control group with19.57% (P<0.05). Conclusion: LC can have a significant effect, and can effectively reduce the patient's postoperative stress response and immune function inhibition, and reduce the incidence rate of complications in the treatment of acute calculous cholecystitis.

Objective: To compare the effect of using the drip bipolar coagulation forceps versus harmonic scalpel to protect parathyroid glands in thyroid surgery. Methods: Seventy two patients indicated for thyroid surgery were divided into two groups: the drip bipolar coagulation forceps group underwent surgery, the harmonic scalpel respectively. All operations were performed by the same group of doctors. The total operation time, intraoperative bleeding, postoperative drainage, and postoperative hypoparathyroidism were compared. Results: There were 36 valid cases in the drip bipolar coagulation forceps group,and 36 cases in the harmonic scalpel group. There were no significant differences between two groups in patients of age, gender disease composition, and thyroid nodule diameter(P>0.05).Also the total operation time,intraoperative bleeding make no differences(P>0.05). The postoperative drainage of the drip bipolar coagulation forceps group was less than that of harmonic scalpel group (P<0.05). The incidence of transient postoperative hypoparathyroidism in the drip bipolar coagulation forceps group were significantly lower than those in the harmonic scalpel group(P<0.05).Both groups do not exist patient of postoperative bleeding and permanent postoperative hypoparathyroidism. Conclusion: The drip bipolar coagulation forceps is better than the ultrasonic scalpel.

1817 Efficacy of Fluticasone Propionate Gelatin Sponge in Postoperative Endoscopic Sinus Surgery

Objective: To observe the clinical efficacy of fluticasone propionate gelatin sponge in chronic sinusitis patients after functional endoscopic sinus surgery. Methods: 120 patients underwent bilateral endoscopic sinus surgery and CRSwNP and CRSsNP patients were followed up for the study. They were divided into experimental group (the left nasal cavity) and the control group (the right side of the nasal cavity). In the experimental group, the nasal passages were filled second days after operation and the gelatin sponge filled with fluticasone propionate was used to fill the cavity. After 2 weeks, February and April, the dressing change was the same as before. By using visual analogue scale (VAS) and subjective assessment LundKennedy objective assess the efficacy of the 2 week of February, and April as the standard observation after operation. Results: The difference between the two groups was statistically significant at 2 weeks, three weeks, February and April (P < 0.05). Conclusion: The application of gelatin sponge filled with fluticasone propionate in the dressing cavity has good anti-inflammatory, mucosal edema and reduced cyst formation. It can promote the recovery of the cavity as soon as possible.

1819 Effect of Buprenorphine Combined with Femoral Nerve Block on Fast-track Surgery after Primary Total Knee Arthroplasty

Objective: To observe the analgesic effect of buprenorphine combined with femoral nerve block for the primary total knee arthroplasty (TKA) and the short-term postoperative rehabilitation. Methods: Sixty-three cases of total knee arthroplasty were retrospectively analyzed from June 2016 to June 2017. They were divided into two groups according to the method of femoral nerve anaesthetised. The patients in group A were given ropivacaine with the addition of 0.3 mg of buprenorphine. The patients in group B were given ropivacaine. Postoperative resting and activities of the visual Analogue Score(VAS)at 12、24、36、48、60、72 hours, the range of motion(ROM) in 7 days, the consumption of oxycodone acetaminophen in 3 days and the duration of the hospital stay were compared between two groups. Results: The resting state VAS in group A was significantly lower than that in group B at 12, 24, 36, 48 hours (P<0.05), and there was no significant difference in 60, 72 hours (P>0.05). The activities state VAS in group A was significantly lower than that in group B at 12, 24, 36, 48, 60, 72 hours (P<0.05); The consumption of oxycodone acetaminophen group A was significantly lower than group B in 1, 2 day and total consumption (P<0.05); The ROM of group A was significantly greater than that of group B in 7 days post-surgery (P<0.05); There was no significant difference in hospital stay between the two groups(P>0.05). Conclusion: The results showed that buprenorphine combined with femoral nerve block can improve the short-term analgesic effect and reduce the consumption of morphine drugs, which is beneficial to the functional exercise and rehabilitation of the patients.

1823 Study on the Application of 3D Printing Technology in the Treatment of Complex Bone Fracture

Objective: To investigate the application of 3D printing technology in the treatment of complex bone fracture. Methods: 76 cases of complex fractures of the distal end of our hospital were selected and randomly divided into two groups. In the control group, 38 cases were treated with plate screw.38 cases in the experimental group were treated with 3D printing technology combined with plate screw fixation. The clinical efficacy of two groups of bone callus score, operative time, intraoperative bleeding volume, fracture healing time, the recovery of knee function, pain score, postoperative complications and the incidence of adverse reactions were compared, the results using statistical software SPSS 17.0 to deal with. Results: After treatment, the two groups of patients with PRI induction, emotional score, total score, VAS score and PPI score decreased; The effective rate of the treatment group was 92.11%, and the effective rate of the control group was 68.42%. The effective rate of the treatment group was higher than that of the control group (P < 0.05) . The score of callus in the experimental group was significantly higher than that in the control group, the operation time, intraoperative blood loss and fracture healing time were lower than those in the control group, the difference was statistically significant (P<0.05); After the treatment, the recovery rate of joint function was 89.47%, and the recovery rate of the control group was 65.79%.The experimental group was higher than the control group, with statistical significance (P < 0.05); In the experimental group, the scores of PRI induction, emotion score, total score, VAS score and PPI score were lower than those of the control group, the difference was statistically significant (P<0.05); There was no significant difference between the experimental group and the control group in the complications and adverse reactions (P > 0.05). Conclusion: 3D printing technology in the treatment of complex fracture of the bone has a significant effect, good safety, suitable for clinical application.

1828 Application of Dexmedetomidine with Non-intravenous Route in Postoperative Analgesia in Pediatric Patients Undergoing Abdominal Surgery

Objective: To investigate the effect of dexmedetomidine intravenous administration on postoperative analgesia in pediatric patients undergoing abdominal surgery. Methods: 60 patients who were undergoing the general anesthesia for lower abdomen surgery were randomly devided into there groups, each group 20 cases. Patients in group B were received dexmedetomidine 1ug/kg nasal drops before operation 30min; Patients in group A, C were received volume normal saline nasal drops. The three groups were given atropine 0.01-0.02mg/kg, lidocaine 1mg/kg, propofol 2mg/kg, sevoflurane 6-8%, oxygen flow 2L/min, waiting for the eyelash reflex disappeared, placed the laryngeal mask, sevoflurane dropped to 1-2%. In group A and group B, 0.25% ropivacaine 1ml/kg was injected after the success of sacral block, group C was given 0.25% ropivacaine 1ml/kg+ dexmedetomidine 1ug/kg, sacral block was given total amount of 20ml cap. 1. Each patient general information respectively were record; 2. The time of operation, induction, extubation and anesthesia awake were recorded; 3. The adverse events such as bradycardia, delayed awakening, laryngismus, urine retention were and awakening period agitation score were recorded. 4. Postoperative 4, 8, 12, 16, 20, 24 hours analgesic score, sedation score and analgesia time were recorded. Results: 1. The general situation have no statistical significance (P>0.05); 2. The time of operation, extubation and anesthesia awake have no statistical significance (P>0.05), the induction time of B group are shorter than A and C groups (P<0.05); 3. All the three groups have no hypoxemia, delayed awakening and urine retentionwere; Compared with group B and C, the emergence laryngismus and awakening period agitation score were higher in group A (P<0.05); the analgesia time were shorter, group C was the longest. 4. All the analgesic score of the three groups were less than 4 points, sedation score for 2-3 minute in postoperative 4 h. Compared with group A, analgesic and sedation score of the group B and C are lower in postoperative 8 h(P<0.05). Compared with group A and B, analgesic and sedation score of the group C were lower in postoperative 12, 16, 20h (P<0.05). There was no statistical significance in postoperative 24h (P>0.05). Conclusion: Intranasal dexmedetomidine (DEX) application used in pediatric anesthesia induction period undergoing the lower abdomen and lower extremity surgery could significantly reduce induction time and have early postoperative analgesia sedation. For caudal block, which received the ropivacaine mixed with 1ug/kg dexmedetomidine can prolonge analgesia calm times and reduce adverse reaction during anesthesia recovery period .

Objective: To investigate the clinical effect of laryngeal mask general anesthesia combined with lumbar plexus - sciatic nerve block in total knee arthroplasty. Methods: 111 patients undergoing total knee arthroplasty were enrolled from June 2014 to June 2017 were randomly divided into two groups. Group A patients were given laryngeal mask general anesthesia, group B patients with complex lumbar plexus - sciatic nerve block Perioperative monitoring of two groups of patients, postoperative pain, comfort, sedation, inflammatory indicators, propofol maintenance dose and adverse drug reaction rate. Results: The levels of SBP and MAP in group B were significantly lower than those in group A at T2, T3 and T4. 1864 Clinical Effect of Mesenchymal Stem Cells on Ankylosing Spondylitis

Objective: To evaluate the clinical efficacy of mesenchymal stem cells in patients with ankylosing spondylitis (AS). Methods: A retrospective study was conducted on 82 patients with AS from May 20 to May 2017. Among them, 42 patients were treated with mesenchymal stem cells as observation group and 40 patients treated with anti-rheumatic drugs as control group (BASF) score and the brachial ankylosis disease activity index (BASDAI) score were measured before and after treatment, and the changes of liver function, blood pressure index, nighttime pain VAS score, Bath ankylosing spondylitis function index (BASFI) score and Bath ankylosing spondylitis disease activity index Variety. 1868 Comparison of Two Kinds of Early Disconnection Methods for Intraocular Pressure Control after Complex Trabeculectomy

Objective: To explore the clinical effect of early laser disconnection and needle disconnection for intraocular pressure control after trabeculectomy. Methods: Fifty primary glaucoma patients (50 eyes) were enrolled in the study from August 2014 to February 2017 in the affiliated hospital of Chengde Medical College. All patients accepted complex trabeculectomy. All patients were observed closely after surgery and accepted early needle or laser suture cut if necessary. Subjective symptoms, vision, intraocular pressure and anterior chamber were observed to compare effect of these two disconnection methods for intraocular pressure control after glaucoma surgery. Results: Within one month after surgery, 18 cases accepted laser disconnection (laser group), 19 cases accepted needle disconnection (needle group). To the end of follow-up, 38 cases of all(76%) were successful , 12 cases (24%) were effective. 14 cases (77.78%) in the laser group were successful, 15 cases (78.95%) in needle group were successful, there was no significant difference in the success rate between the two groups (P>0.05).There was no significant difference in scores of subjective symptoms between the two groups before disconnection (P>0.05). The scores of subjective symptoms in layser group was significantly lower than that in needle group at 1 day after disconnection ,the difference was statistically significant (P <0.05). There was no significant difference in scores of subjective symptoms between the two groups at 3days and7days (P>0.05).There was no significant difference in best corrected visual acuity and anterior chamber depth in the two groups at 1 week after disconnection compared with before disconnection (P>0.05). There was significant difference in intraocular pressure at 1 week after disconnection and the last follow-up compared with before disconnection(P<0.05). Conclusion: Complex trabeculectomy combined with early laser or needle disconnection is safe and effective. Clinicians can choose it appropriately according to hospital and patient conditions.

1872 Fiber Bronchoscopy in the Treatment of Elderly Patients with Aspiration Pneumonia

Objective: To investigate the clinical effect of routine treatment and routine treatment plus fiber bronchoscopy in the treatment of elderly patients with aspiration pneumonia (AP). Methods: 40 cases of elderly patients with AP who was admitted in this hospital from January 2012 to January2015 were selected as the objects, among them, 22 cases were male, 18 female patients, randomly divided into two groups, and the compared group (20 cases) were given routine treatment (including common sputum suction catheter for sputum), while the observation group (20 cases) received conventional treatment plus with fiber bronchoscope bronchial lavage and continuous observation until discharge, to observe the effect of the two groups before and after treatment in patients with white blood cell total (WBC) and oxygen index (PaO 2 / FIO 2), after a week of treatment X Ray sternum inflammatory lesions of the change. Results: After treatment, the two groups' FiO2 /PaO2 increased, WBC decreased, all the treatment of two groups were effective, and the observation group was significantly better than the compared group (P < 0.05). After the treatment for one week, the observation group was significantly better than the control group 50% (10/20) (P) (18/20 < 0.05). Conclusion: After treatment, the pulmonary shadow of FiO2/ PaO2 and WBC is significantly improved which proved that in the treatment of elderly patients with AP, the curative effect of bronchial lavage with bronchial tube is better than that of ordinary sputum suction tube.

1888 Effect of Tangwei Capsule on Lipid Metabolism, Vascular Endothelial Function and Microcirculation in Patients with Type 2 Diabetes Mellitus

Objective: To investigate the effect of Tangwei capsule on lipid metabolism, vascular endothelial function and microcirculation in patients with type 2 diabetes mellitus (T2DM). Methods: A total of 200 patients with T2DM were randomly divided into the control group (100 cases) and the observation group (100 cases). The control group were treated with oral hypoglycemic agents or insulin, and the observation group were additionally treated with Tangwei capsule. Both groups were treated for 12 weeks. The levels of glucose and lipid metabolism, vascular endothelial function and microcirculation were compared between the two groups before and after treatment. Results: After the treatment, the insulin resistance (IR), total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) levels in the observation group were lower than those in the control group, while the high-density lipoprotein cholesterol (HDL-C) level was higher than that in the control group (P < 0.05). There were no significant differences in fasting blood glucose (FBG), postprandial 2h plasma glucose (2hPG) and glycosylated hemoglobin (HbA1c) between the two groups after treatment (P > 0.05). After treatment, the nitric oxide (NO) and endothelium dependent vasodilation (FMD) were higher, and plasma endothelin -1 (ET-1) was lower in the obsercation group than the control group (P < 0.05). After treatment, the diameters of afferent loop, efferent loop and top of loop were larger in the observation group than the control group, and the number of tube loop malformation was smaller than that in the control group (P < 0.05). Conclusion: Compared with routine hypoglycemic therapy, Tangwei capsule combined with routine hypoglycemic therapy has more advantages in regulating abnormal lipid metabolism and vascular endothelial function and relieving microcirculation disturbance in patients with T2DM.

Objective: To study the effect of early application of Xingnaojing Injection on patients with long-term rehabilitation efficacy and safety of cerebral hemorrhage. Methods: 94 cases of cerebral hemorrhage patients admitted in our hospital from July 2014 to September 2015 were selected. According to the random number table method, all the patients were divided into observation group and control group, each had 47 cases. The control group were given routine treatment, the observation group was treated with Xingnaojing injection treatment on the basis of the control group. The clinical efficacy, activities of daily living, neurological function and complications were compared between the two groups after treatment. Results: The total effective rate of the observation group was 93.62% (44/47), which was significantly higher than 78.72% of the control group (37/47), the difference was statistically significant (P < 0.05). After treatment, the number of barrier free activities of daily living in the observation group was significantly higher than that in the control group, while the number of severe functional impairment was significantly lower than that of the control group, the differences were statistically significant (P < 0.05). After treatment, the neurological deficit scores of the two groups were significantly lower than before treatment, while the observation group was significantly lower than that of the control group, the differences were statistically significant (P < 0.05). There were no significant differences in the incidence of pneumonia, renal function damage and cerebral edema between the two groups, and the difference was not statistically significant (P > 0.05). Conclusion: Early application of Xingnaojing Injection in treatment of cerebral hemorrhage has significant clinical efficacy, and can improve the activities of daily living of patients and the nerve function, good safety.

Objective: To investigate the effect of compound danshen dripping pills on renal function in patients with type 2 diabetic nephropathy at stage III - V. Methods: On the basis of intensive hypoglycemic, antihypertensive and lipid-lowering therapy, type 2 diabetic nephropathy patients were randomly divided into the control group and the experimental group. The patients were randomly divided into control group and experimental group. The changes of glycosylated hemoglobin, urinary albumin excretion rate, urea nitrogen and serum creatinine before and after the intervention were compared between the two groups. Results: After treatment, glycosylated hemoglobin, urea nitrogen, creatinine, urinary albumin excretion rate of two groups were significantly decreased (P < 0.05); In the experimental group, the blood urea nitrogen, serum creatinine and urinary albumin excretion rate of patients with type 2 diabetic nephropathy in stage V were significantly lower than those in stage IV and stage IV patients (P < 0.05). Conclusion: Compound Danshen dripping pills have significant effect on improving the renal function of patients with type 2 diabetic nephropathy, and the higher the staging of patients with type 2 diabetic nephropathy, the more obvious the stage is.

Objective: To investigate the efficacy of ambroxol hydrochloride and azithromycin in the treatment of mycoplasmal pneumonia in children. Methods: From April 2015 to April 2017, 200 cases of Mycoplasma pneumoniae pneumonia were randomly divided into two groups. The children in the control group were given azithromycin. The children in the experimental group were treated with ambroxol hydrochloride, Group of clinical indicators, the overall treatment effect, laboratory indicators, pulmonary function, coagulation index and immune function. Results: The time of fever, the time of disappearance of cough, the disappearance of rales and the time of disappearance of X-ray infiltration were significantly lower than those of the control group. Methods: 35 cases of drug hypersensitivity syndrome were collected, clinical data were collected and analyzed retrospectively, suspicious drugs were finded and epidemiological characteristics were analyzed. There was a strong correlation between most of the DHS and suspicious drugs, Suspicious drugs were purine tablets (20 cases,60.00%), antiepileptic drugs (6 cases,17.14%), anti-tuberculosis drugs (5 cases,14.29%), non-steroidal anti-inflammatory drugs (4 cases,11.43%); In terms of disease outcome, after a comprehensive treatment, 91.43% (32/35) of the patients' condition were significantly improved and they all discharged, 8.57% (3/35) of patients died due to multiple organ failure. Fever or rash was the first symptom of most DHS, followed by typical clinical manifestations of DHS, such as oral, mucous membrane, genital damage, liver and kidney function damage, lymphadenopathy, pneumonia and so on. ② The most involved organs by DHS was the skin and its accessories (30.60%), followed by oral system (18.03%), digestive system (13.66%) and nervous system (13.66%), blood, immune, urinary and respiratory system less involved. Conclusion: DHS should be on the alert when the patient showed high fever, rash and visceral damage, the general anti-infective treatment is invalid, and they are with allopurine tablets, anti-TB drugs, antiepileptic drugs and other medication history.

1905 Clinical Observation of Shengxuening Tablets in the Treatment of Maintenance Hemodialysis Patients with Renal Anemia

Objective: To observe the clinical efficacy of Shengxuening tablets to improve the law to maintain iron metabolism in hemodialysis patients with renal anemia and erythropoietin and its synergistic effect. Methods: 60 cases uremic patients with renal anemia selected as the research object, randomly divided into Shengxuening group and succinic acid ferrous group, 30 cases in each group. The patients of shengxuening group were given oral Shengxuening tablets, and oral ferrous succinate group was given Ferrous Succinate Tablets. In accordance with the guidelines, the patients of 2 groups were given the recommended dose individualized combination of erythropoietin treatment. After 16-weeks treatment, the clinical curative effect of the patients of 2 groups were observed and the changes of hemoglobin, hematocrit, serum ferritin and transferrin saturation were recorded before and after treatment in 2 groups. Meanwhile, the adverse reactions in the 2 groups were monitored. Results: After 16 weeks of treatment, the hemoglobin, hematocrit, serum ferritin, transferrin saturation of the 2 groups were increased compared with before treatment, the difference was statistically significant (P < 0.05). However, after treatment, the hemoglobin, hematocrit, serum ferritin, transferrin saturation of Shengxuening group was significantly higher than that of ferrous succinate group, the difference was statistically significant (P < 0.05). Compared with ferrous succinate group, Shengxuening group of adverse reaction is less. Conclusion: Compared with ferrous succinate group, Shengxuening can effectively supplement renal anemia in patients with uremia for iron, and less adverse reactions of blood safety.

1926 Effect of Calcium Dobesilate combined with Traditional Chinese Medicine on Diabetic Retinopathy

Objective: To analyze the treatment results through the combination of Chinese and Western medicine treatment of diabetic retinopathy. Methods: Patients who had been diagnosed with type 2 diabetes were divided into 20 patients with diabetic retinopathy at the background I, II and III, and 0.3 g of calcium benzenesulfonate was given orally three times. At the same time, Chinese medicine was given before and after treatment. The number of retinal hemorrhage and OCT were examined by fundus photography. The results were analyzed statistically. Results: After three months of treatment, diabetic retinopathy background I, II, III patients have varying degrees of improvement, bleeding and bleeding spots, microvascular tumor significantly reduced the number of background III macular retinal thickness has been reduced, Statistical analysis results are meaningful. Conclusion: Dihydroxybenzenesulfonate combined with traditional Chinese medicine in the treatment of diabetic retinopathy at the background.

1931 Influence of Nursing Intervention on Constipation Incidence in Elderly Patients with Intertrochanteric Fracture

Objective: To explore the nursing intervention for elderly patients with femoral fractures between rotor incidence of constipation. Methods: 108 elderly patients with femoral fracture admitted in orthopedic department of our hospital were randomly divided into control group and observation group, 54 cases of each group. The control group was given routine nursing care of patients with femoral intertrochanteric fracture, the observation group based on the implementation of nursing interventions to prevent constipation. The occurrence of constipation of two groups of lying in bed patients was observed and recorded and analyzed statistically. The incidence of constipation in the observation group was 12.96%, and the incidence of constipation in the control group was 48.15%. The difference between the two groups was statistically significant (P<0.05). Conclusion: Prevention of constipation nursing intervention can effectively reduce the incidence of constipation in elderly patients with intertrochanteric fracture, improve the quality of life of patients.

1933 Analysis of Nursing Intervention of 60 Cases of Vaginal Delivery of Maternal Perineal Laceration

Objective: To analyze and analyze the nursing intervention of third degree parturient with vaginal delivery and laceration of perineum. Methods: 60 perineal laceration of pregnant women admitted in our hospital from March 2014 to May 2016 were selected as the research object, and divided into intervention group and control group. The control group was given basic nursing, the intervention group was given targeted nursing program, and the quality of care, wound healing effect and complications were evaluated in the two groups. Results: The intervention group's maternal nursing efficiency, nursing attitude, nursing care and humanistic care scores were significantly higher than those of the control group, the overall quality of the two groups of nursing scores were statistically significant(P<0.05);There was no significant difference in the wound healing between the intervention group and the control group (Z=-1.515, P>0.05); Again, laceration wound infection, fever, fecal incontinence and other complications, the difference was not statistically significant (χ2=3.733, P>0.05), but the rate of complications of the intervention group (6.6%) was significantly lower than the control group (16.6%) (χ2=4.167, P<0.05). Conclusion: The effect of nursing intervention on parturient with grade III perineal laceration of vaginal delivery is significant, which can promote the healing of perineal laceration, reduce wound infection and greatly reduce postoperative complications. It can make the quality of nursing effectively improved, in the vaginal delivery perineal laceration treatment value is higher.
