The Clinical Application Value of Umbilical Cord Blood Serum Total Bilirubin Detection in the Early Diagnosis and treatment of ABO Hemolytic Disease of the Newborn

Abstract  Objective: To explore the clinical value of cord blood serum total bilirubin in the early diagnosis and treatment of neonatal ABO hemolytic disease, and to provide experience and reference for clinical diagnosis and treatment. Methods: From October 2015 to October 2017, 320 neonates with ABO blood group mismatch in our hospital were selected as the study samples to detect three indexes of neonatal ABO hemolysis. The serum total bilirubin level of umbilical cord blood was measured by automatic biochemical analyzer, and the relationship between serum total bilirubin and maternal and infant blood group, time of jaundice and hemolysis was analyzed. Results: There was no significant difference between maternal and infant O - A blood type and mother - to - infant O - B blood type in the incidence of ABO hemolytic disease , incidence of high bilirubin , total bilirubin in cord blood serum , anti - human globulin test and release test. In the age of less than or equal to 3 days of neonatal umbilical cord blood serum total bilirubin was significantly higher than that of age greater than 3 days in neonatal hyperbilirubinemia, the difference was statistically significant (P<0.05). Coombs test positive children's umbilical cord blood total bilirubin was significantly higher than that of antiglobulin test negative patients, the difference was statistically significant (P<0.05). Conclusion: The three detection of serum total bilirubin and hemolysis of umbilical cord blood in neonates with ABO incompatible blood group is of great significance in the early diagnosis and treatment of ABO hemolytic disease of the newborn.
