绝经后女性2型糖尿病合并脂肪肝患者影响因素研究

摘要目的: 观察绝经后女性2型糖尿病合并脂肪肝患者代谢性指标变化,为临床治疗提供依据。方法: 收集2014年10月至2014年16月在我院诊断糖尿病患者病历资料,选取绝经后女性2型糖尿病患者274例作为研究对象,将经由腹部B超检查并结合临床表现诊断脂肪肝的136例患者作为病例组,138例单纯2型糖尿病患者作为对照组,对两组患者的临床资料进行收集,主要是包括了身高以及体重和血压;同时包括了甘油三酯以及高密度脂蛋白胆固醇和空腹血糖,、糖化血红蛋白以及总胆固醇和低密度脂蛋白胆固醇,应用SPSS软件进行单因素分析和多因素回归分析。结果: 影响绝经后女性2型糖尿病患者合并脂肪肝的单因素分析:两组患者血压、HbA1c、FPG、TC、LDL-C等各项指标相比较,差异均无统计学意义(P>0.05);BMI、TG、HDL-C等各项指标,病例组与对照组相比,差异均有统计学意义(P<0.05)。影响绝经后女性2型糖尿病患者合并脂肪肝的多因素logistic回归分析:结果显示BMI(OR＝4.069,95%CI＝2.465~6.74)是发生脂肪肝的危险因素。结论: 绝经后女性2型糖尿病患者合并脂肪肝的影响因素有甘油三酯、低密度脂蛋白胆固醇、体重指数,超重或肥胖是发生脂肪肝的危险因素。

Abstract：Objective: To observe the metabolic changes in the patients with type 2 diabetes mellitus combined with fatty liver disease, and to provide the basis for clinical treatment. Methods: The primary patients were divided into the case group (type 2 diabetes with fatty liver group 136 cases) and control group (simplex type 2 diabetes 138 cases), based on ultrasond diagnosis and clinical manifestation. We recorded the clinical data of the two groups, including height, weight, blood pressure; FBG, TG , TC, HDL-C, LDL-C. SPSS 19.0 statistical software was used for 2 test and multivariate regression analysis. Results: (1) Univariate analysis showed that there was no significant difference in blood pressure, HbA1c, FPG, TC and LDL-C between two groups in postmenopausal women with type 2 diabetes mellitus (P>0.05). The indexes of BMI, TG and HDL-C were significantly different between the case group and the control group (P<0.05). (2) Multivariate logistic regression analysis in postmenopausal women with type 2 diabetes mellitus: The results showed that BMI (OR = 4.069, 95% CI = 2.465 ~ 6.74) was a risk factor for fatty liver. Conclusion: Blood lipids, body mass index are the influence factors, overweight or obese are the risk factors of fatty liver in postmenopausal women with type 2 diabetes mellitus.
