Abstract:Objective: To investigate the effect of bronchoalveolar lavage by fiberoptic bronchoscope in the treatment of severe pneumonia and the efficacy of arterial blood gas and respiratory dynamics. Methods: A total of 118 patients with severe pneumonia who were admitted to the hospital from September 2017 to September 2020 were enrolled as subjects. According to the simple random method, They were divided into control group (n=58) and observation group (n=60). The control group was given routine comprehensive medical treatment, while observation group was treated with bronchofibroscopic BAL. The duration of cough, disappearance time of lung rales, hospitalization time, clinical curative effect, Arterial blood gas index [Ⅱ acute physiology and chronic health evaluation system (APACHE Ⅱ), arterial blood CO2 partial pressure (PaCO2), blood oxygen saturation (SaO2), arterial blood oxygen partial pressure (PaO2)], respiratory dynamics indexes [dynamic compliance (Cdyn), airway resistance (Raw), work of breathing (WOB)] and levels of inflammatory factors [interleukin-6 (IL-6) interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α)] were compared between the two groups. Results: The duration of cough, disappearance time of lung rales and hospitalization time in observation group were shorter than those in control group (P<0.05), and response rate of treatment was higher than that in control group (88.33% VS 70.69%) (P<0.05). In observation group, SaO2, PaO2 level were higher than the control group after treatment, the APACHE Ⅱ score, PaCO2, Raw, WOB, IL - 6, IL - 8, TNF alpha were lower than the control group (P<0.05). Conclusion: Bronchoalveolar lavage via fiberoptic bronchoscope is effective in the treatment of severe pneumonia, which can improve arterial blood gas index and respiratory dynamics, and reduce inflammatory factors.
