Analysis on the Correlation between the Levels of TNF-alpha, IL-6 and IL-8 in Synovial Fluid and X-ray Classification and Joint Function in Patients with Traumatic Arthritis

Abstract:Objective: To analyze the correlation between the levels of inflammatory factors in synovial fluid and X-ray classification and knee function in patients with traumatic arthritis. Methods: 85 patients with traumatic arthritis (case group) and 45 health examinees (control group) were selected from January 2017 to January 2019. The levels of interleukin-6 (IL-6), interleukin-8 (IL-8) and tumor necrosis factor-α (TNF-α) in the synovial fluid of the two groups were compared. The differences of IL-6, IL-8 and TNF-α level in synovial fluid of patients with different X-ray grades were compared. Pearson correlation was used to analyze the relationship between KSS, HSS and IL-6, IL-8 and TNF-α level in synovial fluid. Results: The levels of IL-6, IL-8 and TNF-alpha in the synovial fluid of the case group were significantly higher than those of the control group (P<0.01). X-ray examination showed that there were 35 cases of grade I, 21 cases of grade II, 19 cases of grade III and 10 cases of grade IV. The levels of IL-6, IL-8 and TNF-α in synovial fluid of patients with different X-ray grading were significantly different (P<0.01), suggesting that the levels of IL-6, IL-8 and TNF-α in synovial fluid would increase with the increase of X-ray grading and the aggravation of bone injury. The average KSS score and HSS score were (63.47±10.46) and (67.98±13.28) respectively. Pearson correlation analysis showed that KSS score and HSS score at admission were negatively correlated with IL-6, IL-8 and TNF-α in synovial fluid (P<0.01), but not with ages and body mass index (P>0.05). Conclusion: The levels of inflammatory factors (IL-6, IL-8, TNF-α) in synovial fluid are closely related to X-ray classification and knee function in patients with traumatic arthritis. With the increase of X-ray classification, knee function is significantly weakened, and the levels of inflammatory factors (IL-6, IL-8, TNF-α) in synovial fluid are significantly increased.
