Short and Long-term Effect of Small Bone Window Through Lateral Fissure Island Approach in the treatment of Hypertensive Cerebral Hemorrhage Less than 6h

Abstract:Objective: To observe the short-term and long-term effect of small bone window by the lateral fissure island approach for hypertensive intracerebral hemorrhage onset <6 h. Methods: 60 patients with basal ganglia hypertensive intracerebral hemorrhage were selected, and divided into the observation group and control group, 30 cases in both groups. The observation group was treated with small bone window through the lateral fissure island approach, and the control group was through the temporal lobe cortex. The hematoma removal, rebleeding rate, the GOS score at one month after surgery, the functional recovery (assessed by ADL scale)at one year after surgery were compared between two groups. Result: The success rate of hematoma evacuation in the observation group was 96.67%, which was significantly higher than 60.00% in the control group (P <0.05). The rebleeding rate was 13.33% in the control group and no recurrence in the observation group, the difference was not significant (P > 0.05). There was no significant difference in GOS prognosis between the two groups at one month after surgery (P > 0.05). The mortality rate of the observation group and the control group were both 3.33%, the difference was not statistically significant (P > 0.05). The proportion of ADL grade Ⅰ in the surviving patients of observation group was 48.28%, which was significantly higher than 24.14% in the control group (P < 0.05). Conclusion: The effect of the small bone window through the lateral fissure Island approach is better than that of the temporal cortex approach. The removal of hematoma is complete and the patient's long-term function is better.
