Abstract：Objective: To study the expression of soluble costimulatory molecule B7 homologue 4 (sB7-H4) in sera of patients, in order to investigate the clinical significance of sB7-H4 in the diagnosis of esophageal precancerous condition. Methods: Esophageal pathological changes were evaluated by hematoxylin-eosin (HE) staining. sB7-H4 and squamous cell carcinoma antigen-2 (SCCA-2) concentrations were detected by enzyme-linked immunosorbent assay (ELISA). Results: The precancerous lesions of the esophagus were divided into low grade intraepithelial neoplasia (LGIN) and high grade intraepithelial neoplasia (HGIN). The serum SCCA-2 concentration in HGIN patients was significantly higher than that in LGIN patients and healthy subjects (SCCA-2, P<0.01, P<0.01). The serum sB7-H4 concentration in LGIN patients was significantly higher than that in healthy subjects (sB7-H4) (P<0.05), and significantly lower than that in HGIN patients (P<0.01). The expression of sB7-H4 in serum was positively correlated with the histopathological grade of esophagus (P<0.0001), but not related to age and sex (P=0.7777, P=0.6068). Conclusion: The concentration of serum sB7-H4 increased progressively in the course of precancerous lesions of the esophagus. sB7-H4 is a valuable serum marker for judging the progression of precancerous lesions.
