A Controlled Study of the Effects of Cryo-Balloon Catheter Ablation on the Coagulation System Inflammatory Response and Prognosis of Patients with Atrial Fibrillation

Abstract:Objective: To analyze the effects of cryo-balloon catheter ablation on the coagulation system,inflammation and prognosis of patients with atrial fibrillation. Methods: A total of 93 patients with atrial fibrillation in our hospital from Jul. 2017 to Nov.2021 were selected and divided into cryo group (n=47) and RF group (n=46) according to different treatment plans.Radiofrequency ablation was performed in the RF group,and cryo-balloon catheter ablation was performed in the cryo group.The two groups were compared in terms of operative time,ablation time,success rate of pulmonary vein isolation (PVI),complications and preoperative,1 d and 3 d postoperative levels of coagulation parameters [activated partial thromboplastin time (APTT),fibrinogen (FIB),D-dimer (D-D)],inflammatory parameters [interleukin-6 (IL-6),supersensitive-C-reactive protein (hs-CRP),white blood cell count (WBC) WBC)] levels, 6 month and 12 months cardiac function parameters preoperative and postoperative [left ventricular ejection fraction (LVEF),left ventricular end-systolic diameter (LVESD),left ventricular end-diastolic diameter (LVEDD)],and 6-month and 12-month postoperative recurrence rates. Results: The difference between the two groups was not statistically significant when comparing the success rate of PVI (P>0.05); the operative time and ablation time in the cryo group were shorter than those in the RF group (P<0.05); compared with the preoperative period,the APTT level decreased in both groups at 1 d and 3 d postoperatively,but was higher in the cryo group than in the RF group; the plasma FIB,D-D,WBC and serum IL-6 and hs-CRP levels increased,but were lower in the cryo group than in the RF group (P<0.05).The incidence of complications was 6.38% in the cryo group compared with 13.04% in the RF group,and the difference was not statistically significant (P>0.05); the LVEF levels increased and the LVESD and LVEDD levels decreased at 6 and 12 months postoperatively compared with the preoperative levels in both groups,and the cryo group was better than the RF group (P<0.05); the recurrence rate at 12 months postoperatively was 19.15% in the cryo group compared with 39.5% in the RF group.The recurrence rate of cryo group at 12 months after surgery was 19.15% lower than that of the RF group,which was 39.13% (P<0.05). Conclusion: The cryo-balloon catheter ablation for the treatment of atrial fibrillation has the advantages of convenient operation,good postoperative cardiac function improvement,low recurrence rate,etc.,and has little impact on the body's coagulation system and mild inflammatory response.
