Changes and Clinical Significance of Serum Troponin and Myocardial Enzymes in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease and Hypoxemia

Abstract:Objective: To investigate the changes and clinical significance of serum troponin and myocardial enzymes in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and hypoxemia. Methods: A total of 123 patients with AECOPD admitted to the hospital from January 2020 to January 2022 were selected. Patients with arterial partial pressure of oxygen (PaO2) ≥ 60 mmHg were included in the control group (n=62), and patients with PaO2 <60 mmHg were included in the observation group (n=61). The levels of serum troponin I (cTnI), creatine kinase (CK) and creatine kinase isoenzyme (CK-MB) were compared between the groups, and between patients with different severity of hypoxemia. Spearman correlation analysis was performed to analyze the relationship between the severity of hypoxemia and serum cTnI, CK and CK-MB. Results: The levels of serum cTnI, CK and CK-MB in the observation group were significantly higher than those in the control group (P<0. 05). The levels of serum cTnI, CK and CK-MB increased with the aggravation of hypoxemia (P<0. 05). Spearman correlation analysis showed that the severity of hypoxemia was significantly positively correlated with serum CTnI, CK and CK-MB (r=0. 916, 0. 665, 0. 805, P<0. 05). Conclusion: The levels of serum cTnI, CK and CK-MB in patients with AECOPD and hypoxemia are higher than those in patients with simple AECOPD. The levels of serum cTnI, CK and CK-MB are positively correlated with the severity of hypoxemia, which deserves clinical attention.
