Phacoemulsification Combined with Trabeculectomy on Angle-Closure Glaucoma Combined with Cataract and Its Influences on Postoperative Visual Acuity and Angle Structure

Abstract:Objective: To explore the effects of phacoemulsification (PEA) combined with trabeculectomy in the treatment of angle-closure glaucoma (ACG) complicated with cataract on postoperative visual acuity and changes in angle structure. Methods: A total of 94 patients with ACG and cataract treated in the Affiliated Hospital of Panzhihua College, Sichuan Province between January 2019 and January 2022 were enrolled and divided into control group (n=46, trabeculectomy) and combination group (n=48, PE and trabeculectomy). The changes of visual acuity, intraocular pressure and chamber angle structure parameters before and after surgery were compared between the two groups. All were followed up to assess visual quality at 1 month after surgery. And perioperative complications were recorded. Results: Before surgery, there were no significant differences in visual acuity, intraocular pressure, central anterior chamber depth (ACD), trabecular iris angle (TIA), angle opening distance AOD500 (AOD500), and trabecular ciliary process distance (TCPD) between the two groups (P>0.05). At 1 month after surgery, the visual acuity, ACD, TIA, and AOD500 of the two groups were significantly increased compared with those before surgery, and the intraocular pressure was significantly decreased, and the differences of the above indexes before and after operation in the combined group were higher than those in the control group (P<0.05). At 1 month after surgery, scores of visual quality of watching TV in combined group was (3.01±0.39), scores of visual quality of reading was (2.86±0.43), scores of visual quality of night vision was (2.72±0.35), scores of visual quality of fine operation was (2.82±0.39), which were higher than those in control group (2.23±0.47), (2.20±0.45), (2.38±0.40), (2.34±0.41) (all P<0.05), and total incidence of complications was lower than that in control group (14.58% vs 34.78%) (P<0.05). Conclusion: For patients with ACG and cataract, PE combined with trabeculectomy can significantly improve visual acuity and visual quality, reduce intraocular pressure and improve structural parameters of chamber angle, with fewer complications. It can be applied as an ideal treatment method for such patients.
