Abstract:Objective: To analyze the expression levels and clinical significance of neutrophil surface differentiation antigens CD67 and CD24 in paroxysmal nocturnal hemoglobinuria (PNH). Methods: The clinical data of 32 patients with PNH (PNH group) in our hospital were retrospectively analyzed,and the clinical data of 28 patients with aplastic anemia (AA) (AA group) and 32 healthy subjects with physical examination (control group) admitted to the hospital during the same period were collected. The differences in the levels of neutrophil surface differentiation antigens CD67,CD24,CD55 and CD59 were compared among the three groups. The correlation among the expression levels of the above indicators was evaluated in PNH group,and receiver operating characteristic curves (ROC curves) were used to evaluate the diagnostic value of neutrophil surface differentiation antigens on PNH. Results: There were no statistically significant differences in gender,age,body mass index,smoking history and drinking history among the three groups (P>0.05). Comparison of expression levels of neutrophil surface CD67,CD24,CD55 and CD59 in the three groups showed that PNH grouP<AA grouP<control group (P<0.05). Pearson correlation analysis showed that expression levels of neutrophil surface CD67,CD24,CD55 and CD59 were significantly positively correlated with each other in PNH group (P<0.05). After ROC curve analysis,it is found that the expression levels of neutrophil surface CD67,CD24,CD55 and CD59 had high diagnostic value on PNH (AUC=0.961,0.967,0.972,0.995,P<0.05),and their cut-off values were 81.88%,83.64%,84.42% and 81.37% respectively,and the combination of the four had the highest diagnostic value (AUC=1.000,P<0.05). Conclusion: The expression levels of neutrophil surface differentiation antigens CD67 and CD24 are severely reduced in PNH,and they are consistent with the changes of expression levels of CD55 and CD59,and the combined detection of the four indicators is of high value in diagnosing PNH,and can guide clinical diagnosis and treatment.
