Abstract:Objective: To analyze the difference of clinical effect of anterior median approach and combined anterior posterior approach in the treatment of complex tibial plateau fractures. Methods: 114 cases of anterolateral patients with posterolateral tibial plateau fracture admitted in Hanchuan People's Hospital from 2014 to 2017 were selected and randomly divided into the control group (anterolateral medial approach) and the experimental group (anterolateral joint approach), with 57 cases each. The morphological indexes (varus angle, femoral tibial angle, caster angle), range of motion (knee flexion, knee extension), scale scores (Rasmussen score, HSS score) and statistical differences of knee function were compared between the two groups before and after treatment (before operation, after operation, at discharge, 3 months after discharge). The incidence of complications was compared between the two groups. Results: There were significant differences between the two groups in postoperative (caster angle, knee flexion, Rasmussen score), discharge (varus angle, femoral tibial angle, caster angle, knee flexion, knee extension, Rasmussen score, HSS score, knee function) and 3 months after discharge (varus angle, femoral tibial angle, caster angle, knee flexion, knee extension, Rasmussen score, HSS score, knee function) There were significant differences in varus angle, femoral tibial angle and caster angle (P<0.05), knee flexion and extension (P<0.05), Rasmussen score and HSS score (P<0.05), and knee function index (P<0.05). The incidence of complications in the experimental group (28.07%) was lower than that in the control group (57.89%), the difference was statistically significant (P<0.05). Conclusion: The treatment of complex tibial plateau fractures by combined anterior and posterior approach can achieve relatively better clinical efficacy, better recovery of knee joint function and fewer complications.
