Effects of Different Intracavitary Radiotherapy on Short-Term Curative Effect Dosimetry Comparison Toxicity and Immune Function in Locally Sdvanced Cervical Cancer

Abstract:Objective: To explore the effects of different intracavitary radiotherapy on short-term curative effect, dosimetry comparison, toxicity and immune function in locally advanced cervical cancer. Methods: A total of 50 patients with locally advanced cervical cancer undergoing three-dimensional implant after-loading radiotherapy between January 2019 and March 2021 was enrolled as 3D implant after-loading group, another 50 patients with balanced and comparable baseline data undergoing three-dimensional intracavitary after-loading radiotherapy during the same period were enrolled as three-dimensional after-loading group, and 50 more patients with balanced and comparable baseline data undergoing two-dimensional intracavitary after-loading radiotherapy during the same period were enrolled as two-dimensional after-loading group. The short-term curative effect, dosimetry comparison, toxicity and immune function before and after treatment were compared among three groups. Results: The total response rate in three-dimensional implant after-loading group was 96.00%, higher than that in three-dimensional after-loading group and two-dimensional after-loading group (84.00%, 72.00%) (P<0.017). The D90 and D100 of middle-risk clinical target and high-risk clinical target in three-dimensional implant after-loading group were higher than those in three-dimensional after-loading group and two-dimensional after-loading group (P<0.05), D2cm3 values of small intestine, sigmoid colon, rectum and bladder were lower than those in three-dimensional after-loading group and two-dimensional after-loading group (P<0.05). The incidence rates of radiation enteritis and radiation cystitis in three-dimensional implant after-loading group were 20.00% and 10.00%, lower than those in two-dimensional after-loading group (46.00%, 32.00%) (P<0.017). The difference values of CD3+, CD4+ andCD4+/CD8+ before and after treatment in three-dimensional implant after-loading group were lower than those in three-dimensional and two-dimensional after-loading groups (P<0.05). Conclusion: Three-dimensional implant after-loading radiotherapy can effectively improve short-term curative effect on patients with locally advanced cervical cancer, increase radiation dose of tumor tissues, reduce radiation dose of riskorgans and reduce toxicity, with few effects on immune function. And the incidence of toxicity is lower than that of two-dimensional after-loading radiotherapy.
