Abstract:Objective: To study the relationship between leukoaraiosis and cognitive function/functional outcome in the patients with acute ischemic stroke.Methods: A total of 100 patients with acute ischemic stroke were selected from the neurology ward of the First Affiliated Hospital of Harbin Medical University.The cognitive function was evaluated by MMSE,MOCA and HAMD,and the National Institute of health stroke was used.The functional outcomes were evaluated by scale (NIHSS),modified Rankin Scale (MRS) and ability of daily living (BI).Based on Fazekas method,leukoaraiosis was divided into periventricular hyperintensities (PVH) and deep white matter matter hyperintensities (dwmh),and divided into 4 grades (0-3 grades).The correlation between cognitive function score and functional outcome score and the degree of leukoaraiosis was analyzed.Results: The age of patients in the PVH positive groups was higher than that in the control group,and the more severe the degree of leukoaraiosis was,the older they were,and the difference was statistically significant (P<0.05).The score of MMSE and MoCA in the PVH and DWMH positive groups were lower than those in the control group,and the more severe the degree of leukoaraiosis was,the lower the score of MMSE and MoCA were,and the difference was statistically significant (P<0.05).The score of NIHSS and mRs in the PVH and DWMH positive groups were higher than those in the control group,and the more severe the degree of leukoaraiosis was,the higher the score of MMSE and MoCA were,and the difference was statistically significant (P<0.05).The score of BI in the PVH and DWMH positive groups were lower than those in the control group,and the more severe the degree of leukoaraiosis was,the lower the score of BI was,and the difference was statistically significant (P<0.05).PVH and DWMH were negatively correlated with MMSE and MoCA of cognitive function score,while they were positively correlated with NIHSS and mRs of functional outcome score.Furthermore,compared with PVH,DWMH was significantly correlated with cognitive function and functional outcome.Conclusion: With the aggravation of the degree of leukoaraiosis,the cognitive function score decreased and the functional prognosis score increased,suggesting that the aggravation of the degree of leukoaraiosis could be used as a predictor of cognitive dysfunction and adverse functional prognosis in patients with acute ischemic stroke.Furthermore,DWMH was associated greater with cognitive function and functional outcome than PVH.
