Abstract：Objective: To compare the postoperative complications between laparoscopic surgery (LG) and open surgery (OG) for gastric cancer and to evaluate the safety of laparoscopic surgery in patients with gastric cancer. Methods: From January 2019 to December 2019, 60 patients with gastric cancer underwent radical gastrectomy in Lu'an Hospital Affiliated to Anhui Medical University. Among them, 28 patients underwent laparoscopic subtotal gastrectomy or total gastrectomy, and 32 patients underwent open or laparoscopic subtotal gastrectomy or total gastrectomy, The baseline data were collected and the postoperative complications were analyzed by propensity score matching method. Results: The baseline data of the two groups were not statistically significant (P>0.05) after matching; The Clavien-Dindo grades before and after matching were statistically significant (P<0.05) between the two groups. The incidence of pleural infection and effusion before and after matching was greater than that of LG group (P<0.05) in the OG group; postoperative bleeding before and after matching showed that the incidence of OG group was greater than that of LG group (P<0.05); Visceral dysfunction before and after matching showed OG greater than LG group (P<0.05); There was no significant difference between the two groups before anastomotic fistula matching, LG group had a higher incidence than OG group after matching (P<0.05). Conclusion: LG has the advantage of less complication rate for radical gastrectomy than OG. It has good application prospect.
