序贯化疗联合<sup>125</sup>I放射性粒子植入对复发型宫颈癌患者的疗效及生活质量影响

摘要 目的:探究序贯化疗联合¹²⁵I放射性粒子植入对复发型宫颈癌患者的疗效及生活质量的影响。方法:选取2012年1月至2019年7月本院收治的复发型宫颈癌患者95例。按简单随机分层法分为研究组50例、对照组45例。对照组采用序贯化疗(4疗程),研究组采用序贯化疗联合¹²⁵I放射性粒子植入方案治疗。治疗后1个月,比较两组临床疗效、临床受益反应(CBR)、生活质量(QLQ-C30评分)。结果:研究组治疗总有效率(78.00%)、局部控制率(90.00%)均高于对照组(55.56%、71.11%),差异有统计学意义(P<0.05)。CBR分析显示,研究组体力明显恢复、体质量明显增加、疼痛程度明显减轻的患者占比分别74.00%、78.00%、82.00%,高于对照组的46.67%、51.11%、40.00%,差异有统计学意义(P<0.05)。治疗前两组QLQ-C30各项评分比较差异无统计学意义(P>0.05);治疗后研究组单项问题、症状领域评分低于对照组,功能领域、总体健康评分高于对照组,差异有统计学意义(P<0.05)。结论:序贯化疗联合¹²⁵I放射性粒子植入可提高复发型宫颈癌患者的治疗效果及临床受益率,并促进患者生活质量改善。

Abstract：Objective: To investigate the effect of sequential chemotherapy combined with ¹²⁵I radioactive seed implantation on the efficacy and quality of life of patients with recurrent cervical cancer. Methods: 95 patients with recurrent cervical cancer admitted to our hospital from January 2012 to July 2019 were enrolled. According to the simple random stratification method, 50 cases were divided into study group and 45 cases in control group. Sequential chemotherapy (4 courses) was used in the control group, and the study group was treated with sequential chemotherapy combined with ¹²⁵I radioactive seed implantation. One month after treatment, the clinical efficacy, clinical benefit response (CBR), and quality of life (QLQ-C30 score) were compared between the two groups. Results: The total effective rate (78.00%) and local control rate (90.00%) in the study group were higher than those in the control group (55.56%, 71.11%), and the difference was statistically significant (P<0.05). CBR analysis showed that the patients in the study group recovered significantly, the body mass increased significantly, and the pain degree was significantly reduced, accounting for 74.00%, 78.00%, and 82.00%, respectively, which was higher than the control group’s 46.67%, 51.11%, and 40.00%. The difference was statistically significant (P<0.05).There was no significant difference in the scores of QLQ-C30 between the two groups before treatment (P>0.05). After treatment, the single item and symptom area scores of the study group were lower than the control group, and the functional areas and overall health scores were higher than the control group. The difference was statistically significant (P<0.05). Conclusion: Sequential chemotherapy combined with ¹²⁵I radioactive seed implantation can improve the therapeutic effect and clinical benefit rate of patients with recurrent cervical cancer, and promote the improvement of patients' quality of life.
